Abstract
Description
The invention relates to a method for the bone model-based production of implant drilling templates using three-dimensional radiographic image data.
Computer-assisted dental implantology is a rapidly developing field. It includes the surgeon's imaging, planning, and support during the procedure. Numerous methods and systems have already been established in clinical practice. Three-dimensional imaging techniques enable, among other things, computer-based, precise planning. The exact position of implants can be determined preoperatively. For the intraoperative implementation of the planning navigation systems and drilling templates are available, so-called direct and indirect navigation systems.
The automated generation of surgical proposals and the simulation of surgical procedures and the presentation of their effects in the context of virtual surgery can already be used. To simulate the operation impact by a virtual three-dimensional graphical operation planning are appropriate software programs that allow interactive viewing and manipulation from different directions within the 3D visualization.
There are instrument navigation systems available that allow the surgeon to represent the current instrument position in the surgical site on the three-dimensionally reconstructed image data set of the patient. Likewise, conversely, it is possible to specifically seek the location of a pathological or anatomical structure of the patient in the surgical site.
Decisive for successful preoperative planning and intraoperative interactive use of image data is the presence of two- and three-dimensional datasets. Often, multiple, partially complementary imaging techniques are used to obtain detailed, complementary information about the anatomical conditions. Advantages of computed tomography are the exact reproducible localization as well as the high-contrast representation of bones.
Conventional pre-implantology planning is usually performed on two-dimensional panoramic images. The actual bone supply in the bucco-lingual direction can not be estimated. It often turns out only during the operation that the bone is too small or not suitable for implantation because of concave interfaces and nearby nerves. For safety reasons, if there is no knowledge of the third dimension, large distances to neighboring structures must be maintained; the existing bone supply can not be optimally used, anatomical structures such as nerves and vessels or the jaw / nasal cavity are potentially at high risk.
In order to reduce this risk to a minimum, a precise planning for the position of the implant or the associated holes must take place.
A disadvantage is found in the previous method for the intraoperative implementation of the planning of considerable technical, financial and temporal effort, which can hardly be implemented in the field of dental practices average size. In particular, with the technical possibilities and the effort to prepare, care and control of such systems, which of course increases the risk of possible incorrect operation basically.
The invention has for its object to provide a way for a very simple to implement method, on the one hand, the possibilities of modern, safe planning on the Uses 3D image data and at the same time represents a tool that is easy and quick for the doctor to handle. Furthermore, the invention has the object to provide a drilling template produced by this method.
The first object is achieved by a method according to the features of
According to the invention, therefore, a method for producing a drilling template for introducing implantological bores is provided, based on physical 3D models of segmented, X-ray-based image data of a recorded data set of the tooth and jaw area of a patient. The virtual model of a relevant subarea of the acquired jaw area is created from the data in a segmentation process. As a result, a physical 3D model is produced from the virtual model. Subsequently, in each case one bore per planned implant is introduced into the physical model analogously to a planned operation. This positioning can take place under consideration or simulation of the planned dentures. The manual conversion then takes place in a form-fitting tooth- or jaw-supported drilling template with matching drill sleeves (cylindrical) for the market-standard implant systems.
In contrast to the prior art, which aims at generating a data record for a surgical navigation system or for a data-controlled surgical tool from the recorded data set of the jaw region of the patient, so as to initially plan and prepare the execution of the bore virtually from the data obtained, In order to then implement the most direct possible implementation of this virtual operation plan on the patient, the invention proceeds from a simpler consideration. According to the invention, the data set is not used for virtual operation planning, but rather a physical image of the respectively affected area of the jaw area is initially created. The surgical template is supported with its support area in a suitable tooth or jaw area of the patient and is chosen so that a clear positioning is ensured. The drilling template positioned in this way thus enables the reproduction of the previously introduced into the maxilla / mandibular bore as a bore in the jaw of the patient and forms a defined leadership of the drilling tool. The otherwise manual implementation of the operation on the patient thus leads to a precisely predetermined, previously verifiable result in that the drilling template defines the position and orientation of the dental operation aid with respect to the upper jaw / lower jaw.
Due to the fact that, according to a further advantageous embodiment of the method, the 3D model is produced using a rapid prototyping method, the desired 3D model can be generated in a very short time from the acquired data set of the virtual model, in this way for the patient inconvenient waiting time until insertion of the implant significantly shorten. For example, stereolithography or the 3D printing method are suitable for this purpose.
Furthermore, it proves to be particularly promising that the 3D model is made of a transparent material. As a result, the position and orientation of the hole much faster and easier to be checked already purely visually. In particular, the position of the nerve channels running in the jawbone can also be correspondingly taken into account in the planning of the operation. In this case, the 3D model can additionally be colored in at least one transparent partial area for visualizing essential features of the jaw area of the patient.
The patient can thus be given a nearly complete overview of the treatment steps and the associated interventions before the operation.
The object of providing a drilling template produced by this method on the basis of segmented X-ray-based image data is achieved in that the drilling template has a guide region for a dental drilling tool and a form-fitting contoured support region for the tooth or jaw region of the patient. In contrast to the methods known from the prior art, which serve to place the implant virtually on the basis of the created virtual model and to transfer the steps required for this purpose into a processing program, which the implementation in the operation by a fully automatic or semi-automated surgical tool on Patient serves, dispensed with the invention on such program-controlled aids. Rather, according to the invention an easy-to-use tool is provided to the surgeon, which consists of a drilling template, which is created individually for the patient on the basis of the created physical model and at the same time has a guide area, which was not virtually designed, but rather the Result of the analog processing of a 3D model is. In this case, a large number of individual parameters can be included directly in the processing of the 3D model, which are known to the attending physician, without them consuming in virtual parameters must be implemented. The result of the manual processing of a 3D model can thus be implemented without error in the region of the lower or upper jaw by means of the drilling template created hereafter.
