Index cards have also been known in dental practices for a long time. There, they serve to collect and provide various information about a patient in a concentrated and readily available form. For example, from DE 1 880 735 a dental index card is known in which a plurality of sheets are held together by a plastic cover. A disadvantage of such index cards and the method for fixing information on these index cards is that they do not meet modern requirements criteria, in particular the hygiene ability. In addition, the durability is very limited, the index cards are subject to heavy wear and contamination. A change of data is not possible or only with great effort. The index cards are not recyclable. In addition, the forms rarely fully comply with the requirements and wishes of users, as evidenced, for example, by the fact that the vast majority of the fields on the form are often not used. Filling by hand can lead to readability problems, for example if the practitioner changes within a group practice. The invention has set itself the task of creating such an index card and a method for fixing information on an index card without the disadvantages mentioned.

The index card for providing information according to the invention comprises a carrier card made of a plastic material and at least one transparent film which can be attached to the carrier card and printed by the printer of a computer. The index card according to the invention fulfills modern requirements criteria. A very significant advantage of the index card according to the invention is that it meets the hygiene requirements. The variable information is also perfectly legible on the card and the information is also stored in the computer system without noticeable additional effort. The index card can be used again when a patient changes, since only the foil carrying the variable information has to be exchanged. Also, a space saving can be achieved by thinner cards for storage (compared with conventional index cards), so that, for example, a larger annual index is possible. The index card according to the invention is economical because it has an extremely long life and the aforementioned reusability. A precondition is also created for the user to be able to individually design the form in such a way that on the one hand he supports the loss-free course of the user's workflows and, on the other hand, that he does not contain unnecessary fields for the user.

It is also possible that the card will continue to be labeled by hand, for example. This is a kind of emergency function or may be appropriate for smaller, less important entries.

In the preferred embodiment, the at least one film, which is releasably secured to the carrier card or the other film, within a few seconds and with at most simple tools (ie tools that are composed of a few parts) and most preferably completely without tools solvable.

In the further preferred embodiment, the carrier card has neither form nor variable information. In the preferred embodiment, one or more films are self-adhesive on the carrier card (or one another) to facilitate handling. The method is particularly easy to handle if the form and the variable information are printed on a single sheet. In this case, only a removable foil on the card is necessary. In addition, if necessary, the form can be adapted to the new variable information in one step.

A special cost-effectiveness is ensured, when form and variable information are printed on their own slides, ie printed on a slide a form and no variable information and on another slide a variable information and no form. In this case, it is possible to exchange only the film with the variable information. It therefore does not have to be reprinted with every change in the variable information of the form.

In order to prevent more than one film from being unintentionally peeled off a film with another film, the film containing the variable information is identified as last on the card and easier to detach than the at least one underlying film attached.

A special freedom in the design of the form is guaranteed if no printed form or variable information is printed on the carrier card. The form can then be completely designed by the user as desired, without being limited by already-pre-printed elements in the factory.

Again, the ease of use benefits if at least one film is attached so that it adheres itself.

Another possible application is when clipped on the tab - preferably on the carrier card - X-ray images or perforated sheets (or slides) attached to the index card through these holes or the sheets (or slides) are introduced into tabs.
