Aber auch weniger stark ausgeprägte Grenzflächen innerhalb weicher Gewebestrukturen erfahren eine geringe Kraftwirkung durch Stoßwellen. Diskutiert werden die mechanische Zerstörung von Zellen, Membranen und z. B. Knochentrabekel, aber auch die Zellstimulation durch reversible Deformation der Zellmembran. Sofern die zu behandelnden Gebiete nicht an der Oberfläche liegen, sorgt die Fokussierung zusätzlich dafür, die Wirkung im Therapiegebiet zu steigern und gleichzeitig Nebenwirkungen außerhalb des Gebietes zu reduzieren. Daraus ergeben sich ganz unterschiedliche Wirkungen im Gewebe, die einerseits zu primärer Zerstörung oder Reizung bzw. durch Stimulation zu Heilungsprozessen führen, wie sie speziell bei orthopädischen Anwendungen zu beobachten sind. Stoßwellen können bei geeigneter Wahl des Schallfeldes und Intensität auch als effektiver Massagestimulus verwendet werden.
