Die Erfindung betrifft ein Implantat zur Entlastung geschädigter Bereiche der Gelenkflächen von Hüft- oder Schultergelenken, das in den Spalt zwischen den im natürlichen Gelenk zusammenwirkenden Gelenkflächen des Gelenkkopfes (11) und der Gelenkpfanne (13) einbringbar ist, wobei das Implantat im implantierten Zustand schalen- oder kappenförmig ist, mit einer konvexen Außenseite in der Gelenkpfanne (13) gelegen ist und mit einer konkaven Innenseite auf dem Gelenkkopf (11) sitzt, und wobei das Implantat wenigstens zwei relativ zueinander bewegbare Artikulationslagen (15, 17) mit im implantierten Zustand einander zugewandten Artikulationsflächen umfasst.
Description
Implantat
Die Erfindung betrifft ein Implantat zur Entlastung geschädigter Bereiche der Gelenkflächen von Hüft- oder Schultergelenken.
Bei bekannten Gelenkprothesen werden üblicherweise beide Gelenkflächen durch aufeinander abgestimmte künstliche Gelenkteile ersetzt. Dies erfolgt bekanntermaßen im Rahmen von Operationen, bei denen in Abhängigkeit von der jeweils verwendeten Prothese ein mehr oder weniger großer Teil des gesunden Knochens entfernt wird. Bei Hüftoperationen beispielsweise wird der Femurkopf ganz oder teilweise entfernt, um einem Gelenkkopf Platz zu machen, der meistens über einen Schaft im Femur verankert wird. Entsprechend wird am Acetabulum (Hüftpfanne) des Hüftknochens durch entsprechende Bearbeitung während der Operation Knochenmaterial entfernt, um eine künstliche Gelenkpfanne oder Gelenkschale anbringen zu können, die zur Aufnahme des künstlichen Gelenkkopfes dient.
Derartige Operationen sind für den Patienten mit nicht unerheblichen Be- lastungen und Risiken verbunden. Problematisch gerade bei relativ jungen Patienten ist, dass eine einwandfreie Funktion eines künstlichen Gelenkes nicht über beliebig lange Zeiträume garantiert werden kann. Es sind in vielen Fällen Nachoperationen erforderlich. Derartige Revisionen sind allerdings häufig schwierig durchführbar, da bereits die Erstoperation dem Patienten einen häufig nicht unerheblichen Teil des gesunden Knochenmaterials gekostet hat.
Die erste Operation, die zu einem relevanten Verlust von Knochenmaterial führt, kann durch sogenannte Resurfacing-Techniken hinausgezögert wer- den. Hierbei werden nur die Oberflächen der betroffenen Gelenke durch
durch vergleichsweise dünne, kappen- oder napfförmige Implantate ersetzt. Auch diese bekannten Techniken erfordern jedoch belastende Operationen, die ebenfalls nicht minimal-invasiv möglich sind. Des Weiteren ist es auch beim Resurfacing erforderlich, im Bereich der beteiligten Ge- lenkflächen gesundes Knochenmaterial zu entfernen, wenn auch in einem geringeren Umfang als bei den zuvor erwähnten Operationen.
Von Bedeutung sind diese Überlegungen vor allem dann, wenn die Gelenkflächen von Arthrose betroffen sind und die beteiligten Knochen selbst an sich intakt sind, d.h. keine Frakturen behandelt werden müssen. Derartige Gelenkschäden kommen auch bei relativ jungen Patienten vor, weshalb ein Bedarf an Techniken zur Behandlung von geschädigten Gelenkflächen besteht, bei denen das gesunde Knochenmaterial möglichst erhalten bleibt. Um die Belastung für den Patienten so gering wie möglich zu halten, sollten derartige Techniken minimal-invasiv durchgeführt werden können.
Die Erfindung stellt ein Implantat zur Entlastung geschädigter Bereiche der Gelenkflächen bereit, das in den Spalt zwischen den im natürlichen Gelenk zusammenwirkenden Gelenkflächen des Gelenkkopfes und der
Gelenkpfanne eingebracht werden kann, wobei das Implantat im implantierten Zustand schalen- oder kappenförmig ist, mit einer konvexen Außenseite in der Gelenkpfanne gelegen ist und mit einer konkaven Innenseite auf dem Gelenkkopf sitzt, und wobei das Implantat wenigstens zwei relativ zueinander bewegbare Artikulationslagen mit im implantierten Zustand einander zugewandten Artikulationsflächen umfasst.
Das Implantat kann Druck- und/ oder Scherbelastungen von durch Arthrose betroffenen Bereichen der Gelenkflächen deutlich reduzieren. Da-
durch eignet sich das Implantat nicht ausschließlich, aber besonders gut für von Arthrose im Frühstadium betroffene Gelenke.
Der Zeitpunkt, an dem eine Erstoperation der eingangs geschilderten Art unumgänglich wird, kann durch das Implantat hinausgeschoben werden. Daher ist das Implantat nicht ausschließlich, aber besonders gut für relativ junge Patienten interessant.
Von Vorteil ist, dass das Implantat im Rahmen eines minimal-invasiven Eingriffs eingesetzt werden kann. Es ist keine Bearbeitung des Knochens erforderlich. Es sind zwar Techniken zum Einbringen des Implantats möglich, bei denen Knochen und Knorpel bearbeitet wird. Eine solche Knochenbearbeitung dient dann aber nicht primär zur Bearbeitung oder Vorbereitung der Gelenkflächen bzw. der Gelenkpfanne oder des Gelenkkop- fes, sondern lediglich zum Einbringen des Implantats in den Spalt zwischen den Gelenkflächen.
Es sind aber auch Implantationsmethoden denkbar und möglich, bei denen Knochen und/ oder Knorpel bearbeitet bzw. entfernt wird.
Das Implantat ermöglicht eine Konfiguration, bei der eine Artikulation zwischen Gelenkkopf und Gelenkpfanne ausschließlich über die Artikulationsflächen des Implantats erfolgt und zwischen Implantat und Gelenkflächen des Gelenkkopfes und der Gelenkpfanne keine Artikulation stattfin- det. Hierbei kann das Implantat so ausgebildet sein, dass die Artikulationslagen des Implantats durch geeignete Mittel an den Gelenkflächen des Gelenkkopfes und der Gelenkpfanne mechanisch fixiert werden können. Es ist auch möglich, zumindest die den Gelenkflächen zugewandten Außenseiten der Artikulationslagen des Implantats so auszubilden, dass sie mit den Gelenkflächen verwachsen.
Das Implantat kann so ausgebildet sein, dass es sich im Gelenkspalt selbst ausrichtet. Alternativ kann das Implantat an den natürlichen Knochen- oder Gewebestrukturen fixiert werden.
Ferner ist von Vorteil, wenn das Einsetzen des Implantats keine Subluxation des Gelenkkopfes erfordert.
Das Implantat kann aus einem flexiblen Material hergestellt sein, das zum Einbringen des Implantats in den zwischen den Gelenkflächen vorhandenen Spalt verformbar ist. Die Verformbarkeit des Implantats ermöglicht den Einsatz verschiedener minimal-invasiver Operationstechniken.
Des Weiteren kann das Implantat eine geschlossene Struktur umfassen, die den Zwischenraum zwischen den Artikulationslagen umschließt. Hierdurch kann der Zwischenraum von der Umgebung hermetisch abgeschlossen werden. Das Implantat kann beispielsweise als geschlossene Blase oder Ballonhülle ausgebildet sein.
Es ist dabei möglich, dass das Implantat eine einzige geschlossene Materiallage umfasst und die Artikulationslagen von im implantierten Zustand einander gegenüberliegende Teilabschnitten der einzigen Materiallage gebildet sind. Es ist alternativ möglich, dass das Implantat eine Mehrzahl von einzelnen Materiallagen umfasst, von denen wenigstens zwei benach- barte Materiallagen die Artikulationslagen bilden. Hierbei können die Artikulationslagen in ihren Randbereichen miteinander verbunden sein. Die Verbindung der Artikulationslagen kann als Dichtung ausgebildet sein, die den Zwischenraum zwischen den Artikulationslagen gegenüber der Umgebung abdichtet.
Ein geschlossenes Implantat hat den Vorteil, dass alle Artikulationsbewegungen innerhalb des Implantats stattfinden und so aufgrund des Zusammenwirkens der Artikulationsflächen eventuell entstehender Materialabrieb innerhalb des Implantats verbleibt.
Es ist jedoch nicht zwingend, das Implantat geschlossen auszubilden. Auch ein offenes Implantat kann eingesetzt werden. Das Implantat kann z.B. an den Rändern der Artikulationslagen offen sein.
Es hat sich gezeigt, dass durch ein geeignetes Material und/ oder durch eine geeignete Materialbeschichtung für das Implantat die natürliche Gelenkfunktion aufrechterhalten werden kann.
Die Artikulationsflächen können mit einer Verschleiß und/ oder Bildung von Abrieb hemmenden Beschichtung versehen sein. Geeignete Beschich- tungen können auch auf den Außenseiten der Artikulationslagen vorgesehen sein.
Das Implantat kann aus einem oder mehreren verschiedenen metallischen oder nicht-metallischen Materialien hergestellt werden. In Frage kommt beispielsweise ein Hydrogel, Polyurethan oder Polyethylen.
Das Implantat kann ein textiles Material enthalten oder aus einem textilen Material bestehen, wobei das textile Material ein textiles Substrat umfasst, und wobei zumindest auf einem Teil der Oberfläche des textilen Substrats eine Beschichtung vorgesehen ist, die eine aus der aus Hydrogelen, Polyurethanen, Polyvinylchlorid, Polytetrafluorethylen und beliebigen Kombinationen von zwei oder mehr der genannten Verbindungen bestehenden Gruppe ausgewählte Verbindung enthält.
Das textile Substrat kann aus Fasern bestehen, wobei wenigstens ein Teil der Fasern mit einer Beschichtung versehen ist, die eine aus der aus Hydrogelen, Polyurethanen, Polyvinylchlorid, Polytetrafluorethylen und beliebigen Kombinationen von zwei oder mehr der vorgenannten Verbin- düngen bestehenden Gruppe ausgewählte Verbindung enthält.
Hiermit lassen sich Gleiteigenschaften realisieren, die den natürlichen Verhältnissen im Gelenk zumindest weitgehend entsprechen.
Das Implantat oder zumindest eine Artikulationslage kann mit einer Tex- til-, Faser- oder Geflecht-, Gewirk- oder Gewebeverstärkung versehen sein.
Ein Verfahren zum Einbringen eines zur Entlastung geschädigter Bereiche der Gelenkflächen von Hüft- oder Schultergelenken dienenden Implantats in den Spalt zwischen den im natürlichen Gelenk zusammenwirkenden Gelenkflächen des Gelenkkopfes und der Gelenkpfanne kann gemäß einem Ausführungsbeispiel umfassen: Ausbilden eines in den Spalt zwischen den Gelenkflächen mündenden Kanals im Gelenkkopf oder in der Gelenkpfanne, Einbringen des Implantats in den Kanal, und Einbringen eines Treibmediums in den Kanal, um das Implantat durch den Kanal hindurch in den Spalt zu drücken.
Bei Hüftoperationen kann der Kanal beginnend im Bereich des großen Trochanter durch den Femurhals und den Femurkopf hindurch ausgebil- det werden.
Weitere mögliche Ausführungsbeispiele ergeben sich aus den abhängigen Ansprüchen, der Beschreibung sowie der Zeichnung.
Die Erfindung wird im Folgenden beispielhaft unter Bezugnahme auf die Zeichnung beschrieben. Es zeigen:
Fig. 1 und 2 ein Ausführungsbeispiel eines Implantats gemäß der Erfindung,
Fig. 3 ein weiteres Ausführungsbeispiel eines Implantats gemäß der Erfindung, und
Fig. 4 ein weiteres Ausführungsbeispiel eines Implantats gemäß der Erfindung.
Das Implantat kann sowohl für Hüftgelenke als auch für Schultergelenke eingesetzt werden. Nachstehend werden mehrere Ausführungsformen er- läutert, und zwar lediglich beispielsweise in Verbindung mit dem Hüftgelenk.
Hinsichtlich der für die Implantate verwendbaren Materialien und der möglichen Beschichtungen sowie weiteren möglichen Ausgestaltungen wird auch auf den Einleitungsteil verwiesen.
Das Implantat gemäß Fig. 1 und 2 umfasst eine einzige geschlossene Materiallage 19. Im implantierten Zustand gemäß Fig. 1 befindet sich das insofern als geschlossene Blase oder Hülle ausgebildete Implantat in dem Spalt zwischen den Gelenkflächen des Femurkopfes 11 und der Gelenkpfanne 13. In diesem Zustand liegen zwei Abschnitte der gewissermaßen eine "Ballonhülle" bildenden Materiallage 19 unmittelbar aneinander. Diese Abschnitte werden im Folgenden als Artikulationslagen 15, 17 bezeichnet, da sich die einander zugewandten Innenflächen der Artikulationsla-
gen 15, 17 berühren und somit bei Bewegungen des Gelenkes als Artikulationsflächen des Gelenkes wirksam sind.
Die Innenseite des Implantats und damit die Artikulationsflächen können mit einem verschleißarmen Material beschichtet sein. Alternativ oder zusätzlich kann ein Gleit- oder Schmiermittel verwendet werden, das vor der Herstellung der geschlossenen Struktur beispielsweise in Form eines Fluids auf die Innenseite des Implantats aufgetragen wird.
Ein Vorteil der geschlossenen Struktur des Implantats ist, dass aufgrund der Bewegungen der Artikulationsflächen gegeneinander eventuell entstehender Abrieb im Inneren des Implantats verbleibt und nicht in den Körper des Patienten gelangen kann.
Das Implantat kann des Weiteren derart beschaffen sein, dass zwischen dessen Außenseiten und den Gelenkflächen der Gelenkpfanne 13 und des Gelenkkopfes 11 keine oder allenfalls ein nicht relevantes Ausmaß aufweisende Relativbewegungen auftreten, so dass bei Gelenkbewegungen die Artikulation praktisch ausschließlich zwischen den einander zugewandten Artikulationsflächen der Artikulationslagen 15, 17 und damit ausschließlich innerhalb des geschlossenen Implantats stattfindet.
Hierzu kann das Implantat mit seinen Außenseiten durch geeignete Mittel am Hüftknochen 12 und am Femur 14 mechanisch befestigt werden. Al- ternativ oder zusätzlich kann das Implantatmaterial derart beschaffen sein, dass es an seiner Außenseite mit den Gelenkflächen der Gelenkpfanne 13 und des Gelenkkopfes 11 verwächst oder in sonstiger Weise eine feste Verbindung mit den Gelenkflächen eingeht.
Das Einsetzen des Implantats kann im Rahmen eines arthroskopischen Eingriffs erfolgen. Beispielsweise können die Gelenkflächen, z.B. unter Verwendung eines Extraktionstisches, separiert werden, und zwar ohne Subluxation des Femurkopfes 11.
Das Einsetzen des Implantats kann auch durch andere minimal-invasive Techniken erfolgen.
Eine Möglichkeit zum Einbringen des Implantats ist in Fig. 2 dargestellt. Ausgehend von einer Stelle im Bereich des großen Trochanter 29 wird in den Knochen ein Kanal 27 gebohrt, der durch den Femurhals und den Fe- murkopf 11 hindurch verläuft und in dem Spalt zwischen den Gelenkflächen mündet. Anschließend wird das Implantat in den Kanal 27 eingeführt. Durch Einbringen eines unter Druck stehenden Fluids in den Kanal 27 wird daraufhin das Implantat durch den Kanal 27 hindurch in den Spalt zwischen den Gelenkflächen gedrückt.
Ein weiteres Ausführungsbeispiel eines Implantats zeigt Fig. 3. Das Implantat umfasst zwei separate Materiallagen 21, 23, die Artikulationslagen 15, 17 mit einander zugewandten Artikulationsflächen bilden. Im Gegensatz zu der in Fig. 1 und 2 gezeigten geschlossenen Variante ist das Implantat gemäß Fig. 3 an den Rändern der im implantierten Zustand schalen- oder kappenförmigen Artikulationslagen 15, 17 offen.
Fig. 4 zeigt eine Ausführungsform, die auf dem Ausführungsbeispiel der Fig. 3 basiert. Im Gegensatz zu der offenen Struktur gemäß Fig. 3 sind die beiden separaten Materiallagen 21, 23 in ihren Randbereichen miteinander verbunden. Die Verbindung wird durch einen als Dichtung 25 wirksamen Materialabschnitt gebildet, der den Zwischenraum zwischen den
beiden Artikulationslagen 15, 17 gegenüber der Umgebung abdichtet und somit das Implantat verschließt.
Das Verschließen der Dichtung 25 erfolgt durch den im Inneren des Imp- lantats herrschenden Fluiddruck.
Die Vorteile der Anordnung gemäß Fig. 4 bestehen nicht nur darin, dass - wie beim Ausführungsbeispiel der Fig. 1 und 2 - eventuell entstehender Materialabrieb innerhalb des Implantats verbleibt, sondern dass darüber hinaus die Dichtung 25 einen größeren Bewegungsbereich ermöglicht, als dies beim Ausführungsbeispiel der Fig. 1 und 2 der Fall ist.

Claims
1. Implantat zur Entlastung geschädigter Bereiche der Gelenkflächen von Hüft- oder Schultergelenken, das in den Spalt zwischen den im natürlichen Gelenk zusammenwirkenden Gelenkflächen des Gelenkkopfes (11) und der Gelenkpfanne (13) einbringbar ist, wobei das Implantat im implantierten Zustand schalen- oder kap- penförmig ist, mit einer konvexen Außenseite in der Gelenkpfanne (13) gelegen ist und mit einer konkaven Innenseite auf dem Gelenkkopf (11) sitzt, und wobei das Implantat wenigstens zwei relativ zueinander bewegbare Artikulationslagen (15, 17) mit im implantierten Zustand einander zugewandten Artikulationsflächen umfasst.
2. Implantat nach Anspruch 1 , dadurch g e k e n n z e i c h n e t , dass das Implantat aus einem flexiblen Material hergestellt ist, das zum Einbringen des Implantats in den zwischen den Gelenkflächen vorhandenen Spalt verformbar ist.
3. Implantat nach Anspruch 1 oder 2, dadurch g e k e n n z e i c h n e t , dass das Implantat eine geschlossene Struktur umfasst, die den Zwischenraum zwischen den Artikulationslagen (15, 17) hermetisch umschließt.
4. Implantat nach einem der vorhergehenden Ansprüche, dadurch g e k e n n z e i c h n e t , dass das Implantat als geschlossene Blase oder Ballonhülle ausgebildet ist.
5. Implantat nach einem der vorhergehenden Ansprüche, dadurch g e k e n n z e i c h n e t , dass das Implantat eine einzige geschlossene Materiallage (19) um- fasst und die Artikulationslagen (15, 17) von im implantierten Zu- stand einander gegenüberliegenden Teilabschnitten der Materiallage
(19) gebildet sind.
6. Implantat nach einem der Ansprüche 1 bis 4, dadurch g e k e n n z e i c h n e t , dass das Implantat eine Mehrzahl von einzelnen Materiallagen (21 ,
23) umfasst, von denen wenigstens zwei benachbarte Materiallagen (21, 23) die Artikulationslagen (15, 17) bilden.
7. Implantat nach Anspruch 6, dadurch g e k e n n z e i c h n e t , dass die Artikulationslagen (15, 17) in ihren Randbereichen miteinander verbunden sind.
8. Implantat nach Anspruch 7, dadurch g e k e n n z e i c h n e t , dass die Verbindung der Artikulationslagen (15, 17) als Dichtung (25) ausgebildet ist, die den Zwischenraum zwischen den Artikulationslagen (15, 17) gegenüber der Umgebung abdichtet.
9. Implantat nach Anspruch 6 oder 7, dadurch g e k e n n z e i c h n e t , dass das Implantat an den Rändern der Artikulationslagen (15, 17) offen ist.
10. Implantat nach einem der vorhergehenden Ansprüche, dadurch g e k e n n z e i c h n e t , dass die Artikulationsflächen mit einer Verschleiß und/ oder Bildung von Abrieb hemmenden Beschichtung versehen sind.
11. Implantat nach einem der vorhergehenden Ansprüche, dadurch g e k e n n z e i c h n e t , dass das Implantat aus einem oder mehreren verschiedenen nichtmetallischen Materialien hergestellt ist, beispielsweise aus einem Hydrogel, Polyurethan oder Polyethylen.
12. Implantat nach einem der vorhergehenden Ansprüche, dadurch g e k e n n z e i c h n e t , dass das Implantat ein textiles Material enthält oder aus einem tex- tilen Material besteht, wobei das textile Material ein textiles Substrat umfasst, und wobei zumindest auf einem Teil der Oberfläche des textilen Substrats eine Beschichtung vorgesehen ist, die eine aus der aus Hydrogelen, Polyurethanen, Polyvinylchlorid, Polytetra- fluorethylen und beliebigen Kombinationen von zwei oder mehr der vorgenannten Verbindungen bestehenden Gruppe ausgewählte Verbindung enthält.
13. Implantat nach einem der vorhergehenden Ansprüche, dadurch g e k e n n z e i c h n e t , dass das Implantat ein textiles Material enthält oder aus einem tex- tilen Material besteht, wobei das textile Material ein textiles Sub- strat umfasst, und wobei das textile Substrat aus Fasern besteht und wenigstens ein Teil der Fasern mit einer Beschichtung versehen ist, welche eine aus der aus Hydrogelen, Polyurethanen, Polyvinylchlorid, Polytetrafluorethylen und beliebigen Kombinationen von zwei oder mehr der vorgenannten Verbindungen bestehenden Grup- pe ausgewählte Verbindung enthält.
14. Implantat nach einem der vorhergehenden Ansprüche, dadurch g e k e n n z e i c h n e t , dass das Implantat oder zumindest eine Artikulationslage mit einer Textil-, Faser-, Geflecht-, Gewirk- oder Gewebeverstärkung versehen ist.
15. Implantat nach einem der vorhergehenden Ansprüche, dadurch g e k e n n z e i c h n e t , dass die im implantierten Zustand den Gelenkflächen des Gelenkkopfes (11) und der Gelenkpfanne (13) zugewandten Außenseiten des Implantats zur Fixierung des Implantats an den Gelenkflächen ausgebildet sind.
16. Verfahren zum Einbringen eines zur Entlastung geschädigter Bereiche der Gelenkflächen von Hüft- oder Schultergelenken dienenden Implantats in den Spalt zwischen den im natürlichen Gelenk zusammenwirkenden Gelenkflächen des Gelenkkopfes (1 1) und der Gelenkpfanne (13), welches umfasst:
Ausbilden eines in den Spalt zwischen den Gelenkflächen mündenden Kanals (27) im Gelenkkopf (1 1) oder in der Gelenkpfanne (13),
Einbringen des Implantats in den Kanal (27), - Einbringen eines Treibmediums in den Kanal (27), um das
Implantat durch den Kanal (27) hindurch in den Spalt zu drücken.
17. Verfahren nach Anspruch 16, dadurch g e k e n n z e i c h n e t , dass das Implantat in den Spalt zwischen den Gelenkflächen eines natürlichen Hüftgelenks eingebracht und der Kanal (27) beginnend im Bereich des großen Trochanter (29) durch den Femurhals und den Femurkopf (1 1) hindurch ausgebildet wird.
