- the threads 47 of the ring 26 will fit into the threads 48 of the core 16 and will allow the ring 26 to bear on the surface 49 of the core 16 to control the axial play with the femoral head 36.
- External grooves 51 as well as cavities 52 are respectively provided at the top of the threaded ring 26 and at the base of the core 16 in order to serve as holding points during pre-assembly.
- the grooves 51 are designed so that the impactor-orienter comes to bear on the flared surface 43 of the ring 26 during the implantation of the sub-assembly of the core 16 in the acetabulum 46.
- the ring 26 has a flange 45 on which a tool can support to remove the core assembly from the cup 46 if necessary.
- a pivot 29 extends from the lower surface of the core 16 to engage slidingly in the central hole 40 in order to guide the core 16 during its insertion into the acetabulum 46.
- the sub-assembly 55 of the retentive core 60c, shown in FIG. 4, is an alternative model, the neck of which has external threads 67c for engaging internal threads 57 of the ring 56c.
- the ring 56c is targeted on the neck of the core 60c until the ring 56c rests on the surface 68c of the core 60c.
- Fins 58c are distributed along the circumference of the lower end of the ring 56c. The fins 58c are adjusted, with a slight pressure, on the neck of the core 60c to secure the femoral head 36 in position.
- the cavities 59c in the retentive ring 56c and the holes 61c in the bottom of the core, will be used for the pre-assembly of this retentive threaded acetabulum.
- the retentive ring 56c will preferably be installed in the factory to obtain an appropriate adjustment and thus reduce the operating time.
- the conical surface 62c of the core 60c will serve for the expansion of the surface 22 of the expandable acetabulum 46 shown in FIG. 3.
- the two circumferential edges 66c of the core 60c will be received in the angular grooves 20 of the expanding acetabulum 46.
- the alternative assembly 65 of the core allows the installation of each component by the surgeon.
- the cup 46 can be inserted freely, or with a light pressure, into the bone cavity using an orienter, graduated in degrees, according to the desired orientation shown in FIG. 10.
- the conical core 70b will be inserted, with a slight pressure, using a graduated impactor-orienter which will be retained and oriented by the groove 71b, in the acetabulum 46 slightly expanding until the desired positioning is obtained; then the femoral head 36 will be inserted into the housing 79b of the core 70b.
- the ring 75b will finally be inserted in position with a precise adjustment of the diameter 77b until it rests on the surface 73b.
- the edges 74b of the ring will be housed in the angular grooves 20 of the expanding cup 46 to secure the femoral head 36 in position.
- Figs. 6 and 7 represent a non-retentive 80 acetabular implant, which should be suitable for the majority of patients.
- the implant 80 shown in Figures 6 and 7 differs essentially from that illustrated in Figures 1 and 2 in that the core 85A is self-locking and that there is therefore no need to use a threaded ring to secure the core 85A in position in the acetabulum 46.
- the implant 80 comprises an acetabulum 46 similar to that illustrated in FIGS. 1, 2 and 3.
- the core 85A (FIG. 8) for its part comprises at least one self-locking fin 86a for engagement with one of the grooves 20 in the acetabulum 46.
- the core 85A can be locked at two insertion levels in the acetabulum 46 depending on whether the self-locking fin 86a is engaged in the first groove 20 or the second.
- the core 85A could also include two self-locking fins for engagement with each of the grooves 20 of the acetabulum 46, thus providing an additional degree of retention.
- the core 85A can be easily oriented by pushing the conical core gradually, towards the bottom 28 of the expandable acetabulum 46, until a position is rigid enough to check the orientation of the acetabulum with an impactor-orienter graduated in degrees. At this stage, it is possible to remove the core 85A from its stress position in order to reposition it if necessary.
- the grooves 89a of the core are designed so that three sections of the impactor-orienter come to bear on the surface 87a of the core during implantation and that the flange 88a can be used to remove the core if necessary.
- the bone cavity is milled beforehand.
- the orientation of the acetabulum should be between 35 and 45 degrees of inclination and between ten and twenty degrees of anteversion as shown in Fig. 10.
- the core 85a will be pushed to the maximum depth and will be guided by the pivot 81a in the central cavity 40 of the bottom 28 of the acetabulum. It is the angular surface 82A of the core 85A which initiates the radial expansion of the acetabulum 46 by the pressure exerted on the surface 22; at this stage, the repositioning can be carried out by removing the conical core 85A from its stress position.
- Fig. 9 shows an alternative assembly of a cup provided with a neck; for this model, identified by the letter "C", it is the cavities 93c which allow installation using an impactor-orienter according to the desired orientation shown in FIG. 10.
- the conical part 92c of the self-locking core 85C allows the radial expansion of the acetabulum 46.
- the fin 96c will be housed in the angular groove 20 of the expanding acetabulum and will secure the positioning of the 85C core in the expanding acetabulum 46.
