Abstract

This invention is a medical instrument including a Veress needle (18), a dilating obturator (28), and a cannula (26). The needle has a tubular safety obturator (22) with an opening in its front section at a needle tip (20). A guide (10) with a guide slot can be positioned in a patient's vagina against the posterior fornix to guide the needle through the vaginal and peritoneal walls. A guide wire (24) may be passed through the needle, and through the hole (30) in the peritoneum wall formed by the needle to guide the obturator through the hole.

Claims

45AMENDED CLAIMS[received by the International Bureau on 12 February 1999 (12.02.99); original claims 1-21 replaced by new claims 1-21; remaining claims unchanged (8 pages)]
1. A medical instrument system comprising:
a first cannula;
a first assembly comprising a first dilating obturator and a first Veress needle assembly, the Veress needle assembly extending through the dilating obturator, and the first assembly being positionable through the cannula;
a second assembly comprising a first working sheath and a telescope, the telescope extending through the working sheath, the working sheath having an accessory device inlet, and the second assembly being positionable through the cannula,
wherein the first and second assemblies are alternatively positionable through the cannula, and wherein the first assembly is first used with the cannula to position the cannula through tissue of a patient, the first assembly is then removed from the cannula, and the second assembly is then positioned into the cannula to enable viewing and interaction with the patient beyond the tissue.
2. A system as m Claim 1 further comprising a second cannula, a second dilating obturator, and a guidewire, wherein the second dilating obturator is used to position the second cannula through the tissue, and the guidewire can be inserted through the second cannula to allow the second cannula to be removed from the tissue with the first dilating obturator and the first cannula then being inserted into the tissue over the guidewire.
3. A system as in Claim 2 further comprising a second sheath, wherein the second sheath does not include an accessory device inlet.

A system as m Claim 1 wherein the first Veress needle assembly has a first tubular needle shaft and a first needle tip obturator slidmgly located m the first needle shaft with a closed blunt front end; and the system further comprises a second Veress needle assembly having a second tubular needle shaft and a second needle tip obturator slidably located in the second needle shaft with a tubular shape having an open front end,
wherein the first and second Veress needle assemblies are alternatively slidably located in the dilating obturator.

A system as in Claim 4 wherein the second Veress needle assembly further comprises a handle with a rear end of the second tubular needle shaft fixed thereto, a tubular spring stop connected to the handle and coaxially telescopingly connected to a rear end of the second obturator, and a spring connected between the second obturator and the spring stop to bias the second obturator m an extended position.
6. A system as m Claim 5 further comprising a gas pressure sensor connected to the handle.
7. A system as m Claim 1 wherein the dilating obturator has retractable laterally extending cutters at a front end thereof.
8. A system as in Claim 1 wherein the first assembly comprises a combined Veress needle and dilator assembly having a tube shaped frame with a needle tip 47
section and an integral dilating outwardly expanding section behind the needle tip section, and a spring loaded safety obturator located at the needle tip section;
wherein the frame of the combined Veress needle and dilator assembly is adapted to both pierce a small hole through tissue and expand the hole m the tissue to about a same size as the cannula.
9. An instrument as m Claim 8 wherein the spring loaded safety obturator has a tube shaped member with an open front end located at the needle tip section.
10. An instrument as in Claim 8 wherein the tube shaped frame has a first section with a first diameter m front of the dilating outwardly expanding section and a second section with a second larger diameter behind the dilating outwardly expanding section.
11. An instrument as in Claim 8 wherein the tube is a one-piece member having the needle tip section and dilating section.
12. A system as in Claim 1 wherein the first Veress needle assembly comprises:
a frame having a general tubular shape with a center channel, a needle shaped open front end, and an open rear end into the center channel; and
a pressure signaler connected to the open rear end,
wherein the pressure signaler is adapted to signal a user of a difference in gas pressure 48
at the open front end relative to atmospheric pressure .
13. An assembly as m Claim 12 further comprising a spring loaded safety tip obturator mounted to the frame, the safety tip obturator having an opening therethrough .
14. A medical instrument guide comprising:
a leading end having a general rounded shape and a guide channel therethrough, the leading end being sized and shaped to seat in a posterior fornix of a vagina of a patient;
a shaft extending from the leading end; and
a rear end connected to the shaft which is sized and shaped to be held and manipulated by the user;
wherein the guide can be inserted into the patient' s vagina and positioned against the posterior fornix such that a medical instrument can be inserted through the guide channel to a position against the vaginal wall below the patient's cervix.
15. A guide as in Claim 14 wherein the guide channel is a slot extending into the leading end from a top surface of the leading end.
16. A guide as m Claim 14 wherein the guide has a section for removably mounting the guide to a cannula . 49
17. A guide as in Claim 14 wherein the leading end has a top surface with a recess to accommodate the cervix .
18. A guide as in Claim 14 further comprising a stop for preventing the medical instrument from advancing past a front of the leading end a predetermined distance.
19. A guide as in Claim 18 wherein the medical instrument is a Veress needle and the predetermined distance is about 15 mm.
20. A medical instrument for piercing through tissue comprising:
a Veress needle;
a positioner having a locator section sized and shaped to be inserted in a vagina of a patient and located against a posterior fornix of the patient, the positioner having the Veress needle movably mounted thereto, and means for moving the Veress needle forward on the positioner a predetermined limited longitudinal distance from a home position on the positioner.
21. An instrument as in Claim 20 wherein the means for moving comprises a spring for pushing the Veress needle forward relative to the positioner and a stop ledge on the positioner for stopping forward movement of the Veress needle at a predetermined position relative to the positioner.
22. A method of inserting a cannula through a peritoneum wall of a patient comprising steps of: 50
inserting a Veress needle assembly into a vagina of the patient and through the peritoneum wall to form a hole in the peritoneum wall;
expanding the size of the hole; and
sliding the cannula into the expanded size hole .
23. A method as in Claim 22 wherein the step of expanding the size of the hole comprises a tubular frame piece of the Veress needle assembly having a first section witn a first diameter, a second section with a larger second diameter, and a dilating expanding section between the first and second sections, wherein the dilating section expands the size of the hole as the tubular frame piece is passed through the hole.
24. A method as n Claim 22 wherein the step of expanding the size of the hole comprises sliding a dilating obturator over the Veress needle assembly through the hole and thereby 'wedging the hole wider.
25. A method as m Claim 24 wherein the step of sliding the cannula slides the cannula over the dilating obturator into the hole.
26. A methoα as m Claim 22 further comprising inserting a guide wire through the Veress needle assembly and out a front end of the Veress needle assembly.
27. A method as in Claim 26 further comprising removing the Veress needle assembly from the peritoneum wall and the vagina while the guide wire stays in the hole . 51
28. A method as m Claim 2""1 wnerem the step of expanding comprises inserting a dilating obturator over the guide wire and into the hole and wedging the hole wider.
29. A method as m Claim 2« wherein tne step of expanding the size of the hole comprises lateral cutters on the dilating obturator cutting slits on the sides of the hole.
30. A method as m Claim 29 wherein the cutters comprise blades that are moved between extended and retracted positions on the dilating obturator.
31. A method as m Claim 22 further comprising inserting a guide into the vagina, the guide having a guide slot therein, wherein the step of inserting the Veress needle assembly comprises placing the Veress needle assembly into the guide slot of the guide.
32. A method as in Claim 22 wherein the step of inserting the Veress needle assembly comprises actuating a drive mechanism on a guide to insert the Veress needle assembly through the peritoneum wall at high velocity.
33. A method as in Claim 32 wherein the step of actuating comprises releasing a compressed spring and the step of inserting further comprises limiting forward motion of the Veress needle assembly relative to the guide to a predetermined position.
34. A method as in Claim 22 further comprising determining if a needle tip of the Veress needle assembly has properly penetrated the peritoneum wall by sensing a change m gas pressure at the needle t p and signaling a user of the change in gas pressure. 52
35. A method of penetrating through a peritoneum wall of a patient comprising steps of:
positioning a guide against tissue of the patient proximate the peritoneum wall, the guide having a guide slot and an ultrasound probe tip;
generating an ultrasound picture at an ultrasound base unit connected to the ultrasound probe tip of a portion of the patient's body in front of the ultrasound probe tip; and
advancing a Veress needle through the tissue and the peritoneum wall and into a field of view of the ultrasound probe tip.
