Microsporidia are an unusually difficult group of parasites to detect. A cDNA library is created from the mRNA of an Encephalitozoon cuniculi culture and immunogenic clones with polyclonal antibodies against E. cuniculi are identified and expressed as recombinant antigens after being cloned in an expression vector. Antigens of other types of microsporidia which cannot be cultivated are produced on a DNA-level by hybridising homologous gene sections and by consecutive isolation and expression. This produces a large number of pure antigens which can be used either directly for the serologic detection of antibodies or for immunisation and thus the production of mono and polyclonal antibodies. With these antibodies microsporidia can in turn be directly detected by detecting their antigens in the clinical samples of humans and animals as well as environmental samples and food struffs without previous cultivation of the agent, DNA-amplification techniques or microscopic methods being necessary. Es ist bekannt, daß Mikrosporidien eine ungewöhnlich schwer nachzuweisende Gruppe von Parasiten darstellen. Obwohl vor dem Auftreten von HIV-Infektionen klinische Manifestationen der Mikrosporidien selten waren, bzw. nicht als solche erkannt worden sind, deuten serologische Untersuchungen aus Schweden darauf hin, daß 12% immunkompetenter Erwachsener und 33% der AIDS-Patienten mit Mikrosporidien infiziert sind oder waren [Garcia & Shimizu (1993) Lab. Med. 24:13-18]. In Europa, Australien und den USA wurden sie bei 15 bis 30% von AIDS-Patienten, die an Durchfall litten, gefunden [Canning et al. in: Curry & Canning (1993) J. Inf. 27:229-236]. Neuere Untersuchungen belegen, daß auch für Deutschland davon ausgegangen werden muß, daß die überwiegende Zahl der Mikrosporidiosen nicht als solche diagnostiziert werden [Franzen et al. (1994) Infection 22:417-419]. Besondere Schwierigkeiten bereitet der Nachweis von Mikrosporidien der Gattung Enterozytozoon. E. bieneusi ist zusammen mit Encephalitozoon (Septata) intestinalis und Encephalitozoon hellem nicht nur am häufigsten für Mikrosporidieninfektionen bei AIDS- Patienten verantwortlich, sondern auch für die klinisch fulminantesten Verläufe [Curry & Canning (1993) J. Inf. 27:229-236; Garcia & Shimizo (1993) b. Med 24:13-18]. E. bieneusi befallt vor allem Εnterozyten des Dünndarms, aber auch den Dickdarm und führt zu unstillbaren, chronisch-sekretorischen Diarrhöen mit ausgeprägtem körperlichem Verfall. Über die Epidemiologie ist äußerst wenig bekannt, lediglich der Nachweis von E. bieneusi bei einem HIV-negativen Patienten mit schwerer Diarrhö [Sandfort et al. in: Curry & Canning (1993) J. Der Nachweis von Mikrosporidieninfektionen im Rahmen der konventionellen parasitologischen und histologischen Diagnostik ist unverändert problematisch. Einerseits wird der lichtoptische Nachweis durch die geringe Größe der Sporen erschwert (1 bis 1,5 μm bei E. bieneusi); andererseits stehen derzeit noch keine spezifischen Färbemethoden für Mikrosporidien zur Verfügung. Die Sporen fallen nicht nur in die Größenordnung von Bakterien, sondern müssen auch von Hefesporen und anderen Pilzen unterschieden werden. Alle bisher verwendeten Färbemethoden (PAS, Versilberung, modifizierte Trichromfärbung, Trichrom-Blau, modifizierte Ziehl-Neelsen-Färbungen, Chromotrop-Färbungen, Interferenz/Phasenkontrast) sowie die bisher untersuchten immunologischen Methoden haben nur den Stellenwert von Such- oder Screening-Methoden mit sehr begrenzter Sensitivität und Spezifität [Garcia & Shimizu (1993) Lab. Med. 24: 13-18; Weber et al. (1994) Clin. Microbiol. Rev. 7:426-461; Ombrouck et al. (1995) Am. J. Trop. Med. Hyg. 52:89-93]. Als immunologische Methoden wurden IFAT, Western-Blot und ELISA eingesetzt, wobei zur Erzeugung der mono- oder polyklonalen Antikörper stets Antigenpräparationen aus ganzen Sporen von E. cuniculi [Weiss et al. (1992) Am. J. Trop. Med. Hyg. 47:456-462], E. intestinalis [Ombrouck et al. (1996) C. R. Acad. Sei. Paris 319:39-43; Didier et al. (1995) J. Clin. Microbiol. 33:3138-3145; Beckers et al. (1996) J. Clin. Microbiol. 34:282-285], E. hellem [Visvesvara et al. (1994) J. Clin. Microbiol. 32:2760-2768; Schwartz et al. (1993) J. Ophthal. 115:285-292; Croppo et al. (1991) J. Protozool. 38:31S], Glugea atherinae [Ombrouck et al. (1995) Am. J. Trop. Med. Hyg. 52:89-93] oder mehreren, kult vierbaren Mikrosporidienspezies [Didier et al. (1995) Parasitol. 111:411-421; De Groote et al. (1995) J. Infect. Dis. 171:1375-1378; Visvesvara et al. (1995) J. Clin. Microbiol. 33:930-936; Zierdt et al. (1993) J. Clin. Microbiol. 31:3071-3074; Aldras et al. (1994) J. Clin. Microbiol. 32:608- 612; Visvesvara et al. (1991) J. Protozool. 38:105S-111S; Sobottka et al. (1995) J. Clin. Microbiol. 33:2948-2952] eingesetzt wurden. Zum Nachweis von Antikörpern gegen Mikrosporidien wurden ebenfalls entweder Sporen oder ihr Gesamtantigen verwendet [Visvesvara et al. (1994) J. Clin. Microbiol. 32:2760-2768; Schottelius et al. (1995) Folia Parasitol.42:169-172]. Eindeutige Identifikation und Artdifferenzierung waren bislang auf den elektronenmikroskopischen Direktnachweis angewiesen, der jedoch nur bei sehr hoher Parasitendichte erfolgversprechend ist und meist Biopsate und damit invasive Techniken voraussetzt. Der zweifelsfreie Nachweis im Sinne eines Goldstandards war somit nicht nur mit einem erheblichen methodischen Aufwand verbunden, sondern setzte außerdem noch eine ausreichende vorherige Anreicherung voraus, die direkt aus Stuhl oft nicht zu erreichen ist [Corcoran et al. (1995) J. Clin. Pathol. 48:725-727]. Eine hierzu wünschenswerte Kultivierung ist jedoch gerade für die klinisch wohl bedeutendste Art, E. bieneusi, bislang noch nicht gelungen. Der Direktnachweis von Mikrosporiden-DNA mittels PCR brachte deshalb einen wichtigen methodischen Fortschritt, ist bisher aber auf Forschungslabors beschränkt und derzeit noch nicht umfangreich validiert [u. a.: Franzen et al. (1996) J. Infect. Dis. 173:1038- 1040; Weiss et al. (1994) Folia Parasitol. 41:81-90; Katzwinkel-Wladarsch et al. (1997) Eur. J. Clin. Microbiol. Infect. Dis. 16:7-10]. Ein weiterer wichtiger Grund, der die Mikrosporidiendiagnostik bisher verhinderte, ist das Fehlen aussagekräftiger immundiagnostischer Tests. Die bisher untersuchten Antikörper zeigten noch kein ausreichendes diagnostisches Potential für einen sensitiven und spezifischen Nachweis von Mikrosporidien-Antigenen [Didier et al. (1995) J. Clin. Microbiol. 33:3138-3145]. Dies gilt vor allem für E. bieneusi, da bislang nur unzureichend gereinigte Antigenpräparationen als Ausgangsmaterial zur Verfügung stehen. B. einem Koproantigen-ELISA, zum Nachweis von Antigenen von Mikrosporidien aus klinischem Material und Umweltproben verwendet. Es ist mit Antikörpern unterschiedlicher Sensitivität und Spezifität zu rechnen: Für einen möglichst sensitiven Suchtest werden diejenigen Antikörper ausgewählt, die mit Antigenen einer möglichst großen Zahl verschiedener Mikrosporidienarten reagieren. E. cuniculi, kodiert, PCR-Primer synthetisiert und damit in einer PCR-Amplifikation der homologe Genabschnitt der anderen, u. U. nicht kultivierbaren Mikrosporidienart, z. B. E. bieneusi, isoliert. Dazu wird genomische DNA dieser Spezies verwendet, die aus klinischem Material, z. B. aus dem Stuhl eines infizierten Patienten, ohne vorherige Anreicherung oder Kultivierung gewonnen werden kann [z. B. gem. Katzwinkel-Wladarsch et al. (1996) Trop. Med. Int. Health 1:373-378]. Alternativ kann die homologe DNA-Sequenz der zweiten Mikrosporidienart auch durch Hybridisierung einer genomischen DNA oder einer cDNA mit dem Genabschnitt der ersten Mikrosporidienart identifiziert und isoliert werden, wozu aber eine größere Menge an genomischer DNA oder mRNA notwendig ist und deshalb von einer Anreicherung oder einer Kultivierung der Mikrosporidien abhängig ist.
