Microespuma inyectable para esclerosis. La esclerosis de varices se basa en la inyección de sustancias líquidas de conocida capacidad para suprimirlas. La presente invención se refiere a la preparación de sustancias esclerosantes en forma de microespuma. Al elaborar la microespuma con esclerosantes, y su posterior inyección en la vena a tratar, esta microespuma desplaza la sangre que contiene la vena y garantiza el contacto del esclerosante con el endotelio del vaso, a concentración conocida y durante un tiempo controlable.

Description

Microespuma Inyectable conteniendo un agente esclerosante.
Estado de la técnica. La esclerosis de varices se basa en la inyecón en ellas de sustancias líquidas, que provocando una reacción inflamato¬ ria localizada propicia la eliminación de estas venas anorma¬ les.
Al inyectar un liquido esclerosante, se produce una mezcla de este con la sangre contenida en la vena y se diluye en proporción no conocida. Los resultados son inseguros( por sobre o subdosificación) y limitados a cortos segmentos varicosos .
Confrome decrece el tamaño de las varices a inyectar, menor es esta dilución y más previsibels son los resultados que se obtienen. En nuestros días la esclerosis es una técnica de elección en casos de pequeñas y medianas varices; quedando el tratamiento de aquellas con diámetro igual o superior a 7mm. para la cirugía.
Esclerosis y cirugía se complementan actualmente, pero la escleroterapia continua sin poder ser aplicada a grandes troncos varicosos.
En estas varices de gran tamaño, al inyectar una sustancia esclerosante, se desconoce: La concentración de esta en la vena; su homogénea distribución en la sangre y el tiempo que va a estar en contacto con las paredes interiores del vaso tratado.
En 1946 Orbach inyecta en varices de pequeño calibre, unos pocos centímetros cúbicos de aire y comprueba un desplazamiento de la sangre en el interior del vaso, que es ocupada por el aire inyectado. La solución esclerosante introducida a conti¬ nuación es más eficaz que si hubiese sido inyectada en la sangre.
En gruesas varices, al inyectar aire, no se produce el fenómeno descrito del desplazamiento de la sangre por el aire inyectado, sino que este forma una burbuja en el interior de la vena que hace ineficaz el procedimiento en estos vasos.
Este mismo autor ideó, pocos años más tarde, la inyección de espuma obtenida por agitación de un envase conteniendo Tetradecyl Sulfato de Sodio, esclerosante detergente aniónico con alta capacidad espumante.
El procedimiento resulta poco útil por el gran tamaño de las burbujas formadas y peligroso por los efectos colaterales del Nitrógeno atmosférico, poco soluble en sangre.
Ambos métodos tuvieron escasa repercusión práctica siendo empleados sólo en pequeñas varices.
Descripción de la invención Esta invención se refiere a la preparación de una microes¬ puma esclerosante.
De acuerdo con la presente invención se ha descubierto que inyectando en posición horizontal una microespuma de suero fisiológico estéril inerte farmacológicaente; se comprueba que la microespuma provoca el desplazamiento de la sangre contenida en el vaso, incluso en las varices más desarrolladas, debido a que es poca la presión de la sangre contenida en ellas en horizontal.
La elevación del miembro inyectado disminuye aún más la presión venosa, facilitando el relleno exclusivo de la vena con microespuma; permaneciendo ésta en el vaso en tanto no se levante al paciente de la mesa de exploraciones.
Al sustituir la microespuma elaborada con el suero fisio- lógico, por microespuma elAborada con sustancia esclerosante, e inyectarla en la vena, esta desplaza la sangre que contiene la vena y garantiza el contacto del esclerosante con el endote- lio del vaso, a concentración conocida y durante un tiempo controlable, logranadose una esclerosis de todo el segmento ocupado.
Las ventajas de este procedimiento permiten : l9 Conocer la concentración del esclerosante en el vaso, pues la microespuma desplaza la sangre y no se diluye como un líquido en ella. 22 Garantizar la homogénea distribución del producto de eclerosis en su interior.
3S Controlar el tiempo que se va a mantener en contacto con las paredes internas de la vena. Factores, todos ellos, no conocidos con exactitud ni controlables utilizando esclerosantes líquidos.
La elaboración de la presente invención se lleva a efecto con la preparación de una microespuma con cualquier sustancia esclerosante, tales como: Polidocanol, Tetradecyl Sulfato de Sodio, Soluciones hipertónicas glucosadas o glucosalinas, Glycerina Cromada, Ethanolamina Oleato, Morrhuato Sódico, Soluciones Iodadas.
Una vez elaborada la microespuma esclerosante por cual- quier procedimiento de los existentes, de los que se describen dos a continuación, se introduce en cualquier recipiente estéril que pueda servir para ser luego inyectado en los vasos a tratar, y que permita la estabilidad de la misma, de forma que pueda ser extraída por una jeringa, o cualquier otro instrumento que posibilite su inyección en los vasos a tratar. Ejemplo 1:
La elaboración de la microespuma esclerosante se realiza mediante la mezcla en un recipiente estéril, hermético, y conectado si se desea a una botella a presión de Oxígeno, mezcla de Oxígeno y Carbónico u otros gases fisiológicos; se efectúa un batido mecánico por medio de un micromotor que hace girar un escobillón sumergido en la solución esclerosante a espumar.
Batiendo entre 8.000 y 15.000 rpm., un tiempo comprendido entre 60 y 120 segundos se consigue la microespuma.
Esta se introduce en cualquier contenedor que pueda servir para su posterior almacenamiento y su ulterior inyección en los vasos a esclerosar.
En el caso de que la sustancia esclerosante no posea capacidad espumante se le añade Polisorbato 20, polisorbato 80, Poligelina o cualquier otra sustancia con capacidad espumante admitida como inerte para uso intravenoso.
Ejemplo 2:
Se introduce la sustancia esclerosante en un recipiente hermético, presurizado y estéril y por agitación de la solución se consigue la microespuma, con una salida del recipiente para su posterior uso.

Claims

Reivindicaciones:
1 Microespuma inyectable para usos terapéuticos, preparadas o de preparación extemporánea, caracterizada porque la mi¬ croespuma es elaborada con cualquier sustancia esclerosante.
2 Microespuma inyectable para usos terapéuticos, según la reivindicación 1, caracterizada porque la sustancia esclerosan¬ te es Policadonol.
3 Microespuma inyectable para usos terapéuticos, según la reivindicación 1, caracterizada porque la sustancia esclero¬ sante es Tetradecyl Sulfato de Sodio.
4 Microespuma inyectable para usos terapéuticos, según la reivindicación 1, caracterizada porque la sustancia esclero¬ sante es una solución hipertónica glucosada o glucosalina. 5 Microespuma inyectable para usos terapéuticos, según la reivindicación 1, caracterizada porque la sustancia empleada es Glycerina Cromada.
6 Microespuma inyectable para usos terapéuticos, según la reivindicación 1, caracterizada porque la sustancia empleada es Ethanolamina Oleato.
7 Microespuma inyectable para usos terapéuticos, según la reivindicación 1, caracterizada porque la sustancia empleada es Morrhuato Sódico.
8 Microespuma inyectable para usos terapéuticos 1, caracte- rizada porque la sustancia empleada es cualquier solución iodada.
9 Microespuma inyectable para usos terapéuticos, según la reivindicaciones anteriores, caracterizada por su aplicación en flebología. 10 Microespuma inyectable para usos terapéuticos, según la reivindicaciones 1 a 8, caracterizada por su aplicación en el tratamiento de varices esofágicas.
11 Microespuma inyectable para usos terapéuticos , según la reivindicaciones 1 a 8, caracterizada por su aplicación en proctología.
12 Microespuma inyectable para usos terapéuticos, según la reivindicaciones 1 a 8, caracterizada por su aplicación en angiología.
