本発明は - フ エ ジ ピ ン の新規る製剤に係 ]? 、 詳 し く は易吸収性であ かつ持続性のニ フ ェ ジ ピ ン製剤に闋 する
二 フ エ ジ ピ ン は優れた冠血管拡張作用 よび降圧怍 用を有するが、 その反面水に難溶で体内への吸収性が 不良であ !) 、 しかも代謝 · 排泄が速いとい う 欠点があ る
しか して、 二 フ エ ジ ピ ンは現在狭心症発作治療剤と して賞用されているが、 狭心症発 ^は予告な しに来襲 し、 それに対 しては緊急の投薬と速効性が要求され、 そのために二 フ エ ジ ピ ンを易吸収性 ( 速効性 ) と した 製剤が種 々 提案されている。 例えば、 二 フ エ ジ ピ ンを 液状ポ リ エ チ レ ング リ コ ー ノレ に溶解 し、 こ の溶液を軟 カ プセ ル に充填する方法 (: 特開昭 4 8 - 2 8 6 2 1 号 公報 ) 、 二 フ エ ジ ピ ンを固溶体と し製剤化する方法 ( 基礎 と臨床、 第 1 3 卷、 1 6 4 3 頁、 1 9 7 9 年 ) な どがそれである
かかる易吸収性の製剤は、 - フ エ ジ ピ ンが容易に体 内へ吸収され、 その血中濃度が速やかに高値に達する
の で確かに狭心症発^の治療には適している。 しかし —方、 かかる易吸収性製剤においては、 二フ エジ ピ ン の代謝、 排泄も ま た速やかであ ]? 、 その血中濃度は急 速に低下して しま う 。
しか して、 二 フ エ ジピ ン の治療上の適用につ ては、 単に狭心症発^の治療に止ま らず、 狭心症の予防る ら びに高血圧症の治療が期待でき る。 かかる 目的に対し ては、 二 フ エ ジ ピ ン が容易に吸収される ことのほかに、 その血中濃度が出来るだけ長時間にわたって一定の有 効値に保持される こ とが要求される。
本発明の 目的は、 二 フ エ ジ ピ ン が容易に吸収される と と も に、 1 回の服用に よって長時間にわたって血中
- フ ェ ジ ピ ン濃度が一定の治療上の有効値範囲に維持 される - フ エ ジ ピ ン製剤、 即ち易吸収性と持続性とを 兼ね備えた二 フ エ ジ ピ ン製剤を提供する こ と にある。
末発明の他の目 的は、 狭心症および高血圧症の予防 • 治療に用いて好適 - フエジ ピ ン製剤を提供する こ とにある。
即ち、 本発明は下記の組成物 および (B) をニフ ェ ジ ピ ン の重量比で 1 5 : 8 5 〜 5 0 : 5 ひ の割合で 配合してるる持続性ニフ ェ ジピ ン製剤である。
組成物 (A ) : 平均粒子径 5 以下の 二 フ エ ジ ピ ン微 粉末を有効成分 と する速放出性製剤組成 物。
組成物 (B ) : 平均粒子径 5 以下の 二 フ エ ジ ピ ン微 粉末を有効成分 と し、 かつ表面部に無毒 性の水難溶性物質 と腸溶性高分子 と か ら る被膜を 施与された遅放出性製剤組成 物。
こ こ で、 速放出性製剤組成物 と は、 上記二 フ エ ジ ピ ン微粉末それ 自 体、 あ る いは該微粉末を通常の医薬添 加物 と共に常法に従って穎粒、 細粒等 と した も の を云 う 。 ま た遅放出性製剤組成物と は 、 上記の速放出性製 剤組成物 と 同様の製剤組成物に さ らに特定の徐放出化 手段を 沲 した も のであ る。 かかる組成物 と組成物 ( B と か ら成る本発明の製剤の有効性ない し特徴は、 本発明の製剤 a ( 実施例 1 ) と 従来の固溶体製剤 ( 比 較製剤 ィ ) および - フ エ ジ ピ ン微粉末を常法に従って、 単に穎粒 と した も の ( 比較製剤 α ) をそれぞれ投与 し た場合における血中 ニ フ ェ ジ ピ ン濃度の対比 (: 試験例
1 、 第 1 図 ) か ら明 らかであ る
すな わち 、' 第 1 図はそれぞれ 二 フ エ ジ ピ ン 2 0 を
含有する上記 3 種の製剤をビ ー グル犬に経口投与 し、 投与後の血中 二 フ エ ジ ピ ン濃度の推移を調べた も の で あるが、 この結果か ら明 らか よ う に、 固溶体製剤 ( 比較褽剤 ィ ) よ び微粉末顆粒 ( 比較製剤口 ) におけ る血中濃度は投与後速やかに上昇し、 かつ極めて高い 値に達するが、 その後急速に低下 して しま う 。 かかる 投与初期の高濃度は副^用の面か ら好ま し ぐ る く 、 ま た急速る血中濃度の低下は持続性の点で失格で あ る。
これに対 して、 本発明製剤 a では、 血中濃度曲線は 極めて平坦 ¾ パタ ー ンを示 し、 血中 二 フ エ ジ ピ ン濃度 は投与 1 時間後か ら 9 時間後に至る合計約 8 時間に わ たって 4 0 〜 6 O ng wの範囲内に維持されてお ]) 、 かつ投与初期の血中濃度上昇 も 十分満足すべき も の で あって、 持続性製剤と して好適に用い得る こ と がわか る
以下に末発明製剤の搆成 ¾ らびに製造法について詳 述する。
まず、 本発明の製剤においては、 有効成分 と して平 均粒子径が 5 #以下、 好ま し く は 1 〜 4 # の微粉末伏 - フ エ ジ ピ ン を用いる こ と を第 1 の特徵とする 。
かかる ニ フ ェ ジ ピ ン微粉末は、 曰
SB伏 - フ エ ジ ピ ン
( 通常その平均粒子径は 3 0 # 以上であ る ) を、 ボ ー ノレ ミ ノレ 、 ジェッ ト ミ ノレ 、 雷塊機 どを用いる常法に よ つて粉砕 し、 さ らに必要に応 じて これを分級する こ と に よって容易に得る こ とができ る。
しか して、 上記の平均粒子径 5 ^以下 との要件は、 後に述べる特定の徐放出化手段と共に、. 持続性製剤に 求め られる薬物の易吸収性と血中薬物濃度の持続性と の 2 つの要請を満足せ しめる上で重要な も のである。
即ち第 2 図は、 平均粒子径が 2. 1 · 、 5. 0 よび
9. 6 # の二 フ エ ジ ピ ン徵粉末をそれぞれ用いて得 られ た製剤 〔 順に本発明邀剤 a 、 本発明製剤 b 、 およ び比 較製剤ハ ( 試験例 2 参照 ) 〕 を ビ ー グル犬に投与 した 時の血中 - フ ヱ ジ ピ ン濃度の推移を示 したも の で あ る が、 この結果か ら明 らかる よ う に、 本発明製剤 a ( 二 フ エ ジ ピ ンの平均粒子径 2. 1 :) および b ( 同 5. 0 u
:) では、 血中二 フ エ ジ ピ ン濃度は投与 1 時間後には 4 0 ngノ; ^ を越え、 その後 8 〜 9 時間 目 に至るま で約 4 0 〜 6 O ngノ の一定範囲内に保持される の に対 し、 比 較製剤ハ ( 同 9. 6 ) では、 二 フ エ ジ ピ ン の血中濃度 が 4 O n gノ に達するの に投与後約 3 時間 も要 し、 し かも約 7 時間後には 4 0 ng ^以下 と 吸収性、 持
続性と も に不良である。 - ブ ェ ジ ピ ン の持続性製剤を調製するに際し、 有効 成分と して易吸収性の二 フ エジ ピ ン固溶体を用 、 そ の一部を徐放性とする こ と も考え られるが、 本発明者 らの研究結果によれば、 固溶体の場合には、 これを徐 放性とすると投与後消化管内に いて吸湿によ ]? ニ フ ェ ジピ ン が結晶伏に析出 してその吸収性が低下する傾 向がみられ、 長時間一定範囲の二 フ エ ジ ピ ン血中濃度 を維持でき難いこ とが明 らか とるっている。 従って実 用上望ま しい持続性二 フ エ ジ ピ ン ¾剤は、 -平均粒子径
5 ^"以下の二 フ エ ジ ピ ン微粉末を甩いる本発明に よつ ては じめて得られるのである。
本発明の製剤の他の特澂の 1 つは、 上記 - フ エジ ピ ン微粉末を用いて速 ¾出性製剤組成物 ( 組成物 A と 後記特定の徐放出化手段を施した遅放出性製剤組成物
( 組成物 B ) と を調製し、 それ らを所定の配合比で配 合する こ とにある。
組成物 (A) は前に定義した如 く 、 二 フ エジ ピ ン微粉 末それ自体、 あるいは該微粉末を賦形剤、 増量剤、 結 合剤 どの医薬添加物と共に常法に従って穎粒伏ある

与時速やかに易吸収性の - フ ェ ジ ピ ン 微粉末を放出 し, 主と して血中 二 フ エ ジ ピ ン濃度の早期上昇を与える。
一方、 組成物 (B) は 、 二 フ エ ジ ピ ン微粉末を上記の 組成物 (A) の場合と 同様に顆粒伏あ る いは細粒伏等 と した後、 これに薬学的に許容される無毒性の水難溶性 物質 ( 以下単に難溶性物質 と う ) と腸溶性高分子と から る る被膜を コ ー テ ィ ン グする こ と に よって得 られ る 。 こ の場合、 コ ー テ ィ ン グ被膜 と して難溶性物質 と 腸溶性高分子 とか ら ¾ る混合被膜を用い る こ と は、 本 発明の ま 一つの重要な特徴を す も の で あ ]) 、 これ に よ つ て 二 フ ェ ジ ピ ン の緩慢かつ持続的る放出が保証 され、 血中 二フ エ ジ ピ ン濃度は長時間一定値範囲 に保 持される れに代えて難溶性物質 ある は腸溶性高 分子を単独で用いて コ ー テ ィ ン グ を行い、 これ を組成 物 (A) と配合 して も 前者 ( 比較製剤 二 ) では - フ エ ジ ピ ン の放出が過度に遅延する ためその利用率が低下 し, 一方、 後者 ( 比較製剤ホ ) では逆に放出が速す ぎる た め、 血中 - フ エ ジ ピ ン濃度の急速 低下を来 し、 いず れ も 本発明製剤 ( 末発明製剤 a :) 同様の効果は期待で き ¾ ( 試験例 3 , 第 3 図参照 ) 。 組成物 (B) で用い る難溶性物質 と しては油脂類 と ェ チノレ セ ル ロ ー ス が代

表的 ¾ も のである。 こ こで油脂類とは具体的には、 第 十改正日 本薬局方に収載されている硬化油、 あ るいは 第四版食品添加物公定書に収載されている グ リ セ リ ン 脂肪酸エ ス テ ル等を云い、 硬化油と しては例えば魚油, 鯨油、 大豆油な どに水素添加 したも のがある。 また、 グ リ セ !; ン脂肪酸エ ス テ ノレは、 モ ノ , ジ , および ト リ エ ス テル のいずれであって も よ い。 これら難溶性物質 は 2 種以上を併用する こ と も でき る。
腸溶性高分子と しては、 通常の腸溶性襄剤に適用さ れるセノレ ロ ー ス ァ セ テ 一 ト フ タ レ ー ト 、 ヒ ド ロ キ シ フ。 口 ピ ノレ メ チ ノレ " έ ノレ 口 一 ス フ タ レ ー ト 、 メ タ ク リ ノレ酸 ー メ タ ク リ ノレ酸 メ チ ノレ共重合体な どが使用でき 、 これら も また必要に応 じて 2 種以上を併用 しても よ 。
以上の如き難溶性物質と腸溶性高分子 とを混合する 場合、 その組合せには種々 の ものがあるが、 こ こで好 ま しい組合せの一例を挙げれば、 例えば硬化油と ヒ ド ロ キ シ プ ロ ピ ノレ メ チ ノレ セ ノレ ロ ー ス フ タ レ ー ト 、 硬 fヒ油 と メ タ ク リ ノレ酸 - メ タ ク リ ノレ酸 メ チ ノレ共重合体、 グリ セ リ■ ン モ ノ ス テ ア レ ー ト と ヒ ド ロ キ シ プ ロ ピ ノレ メ チ ノレ セ ノレ ロ ー ス フ タ レ ー ト等がある。 また、 それ らの混合 比は、 用 る難溶性物質ある は腸溶性高分子-の種類

等に よって も 若干異 るが、 一般には難溶性物質 と腸 溶性高分子 と を 重量比で 1 : 5 〜 5 : 1 、 特に 1 : 2
〜 2 : 1 の範囲で混合使用する のが好ま しい。
コ ー テ ィ ン グ方法 と しては、 上記の難溶性物質 と 腸 溶性高分子 と を 、 それ ら の共通溶媒、 例えば エ タ ノ ー ノレ 、 ジ ク ロ ノレ メ タ ン等に混合溶解 し、 こ の溶液をパ ー ン コ ー テ ィ ン グ法、 流動層 コ ー テ ィ ン グ法る ど常法に 従って前記の顆粒あ る いは細粒等に コ ー ティ ン グする 方法が使用で き る
コ ー テ ィ ン グ は コ ー テ ィ ン グ被膜の組成、 被 コ 一テ ィ ン グ物の形伏、 粒度等に よ って異 ]) 一概には 云え いが、 一般には固形分増加銮で 7 〜 1 0 0 重量 の範囲 と する のが よ い。

本発明製剤の剤型と しては非圧縮型が好ま し く 、 特 に両組成物 (A) , (B) を ずれ も 穎粒伏あるいは細粒 伏と しておき 、 これを単に混和するか、 も しく はこれ をさ らにカ プ セ ノレ に充填してカ プセ ノレ剤とする のが よ い。 な 、 場合に.よっては、 両組成物 (A) , (JB) は、 これを予め配合する こ とな く 同時服用 して も よ く 、 か かる 態様も 本発明に包含される。

健康る成.人男子 4 名に、 二 ブ ·ェ ジ ピ ン換算で本発 明製剤 a では 2 0 W . ま た比較製剤 ィ では 1 相 当の製剤を投与し、 投与後経時的に採血 して血中の 二 フ エ ジ ピ ン濃度を : E C D ガ ス ク ロ マ ト グ ラ フ ィ に よ ]? 求めた。 結果は被験者 4 名の平均値で示 した。

結果を第 5 図 に示 した。 第 5 図の結果に よれば、 本発明製剤 a 投与時の血中 二 フ エ ジ ピ ン濃度は、 投 与 1 時間後か ら 1 0 時間後に至る釣 9 時間にわたつ て、 ヒ ト における最低有効血中濃度と いわれている
