a citizen of the United States, residing at' Charlotte, in the county of lllecklenburg and State of North Carolina, have invented new and useful Improvements in Charts, of
which the following is a speciication.
This invention relates to indicating charts l f bers 10 and 11 will be crossed out, sovtliat and has for an lobj ect the provision of means 10 for keeping-'track of the time at which medi-A cine is to be administered to a person, so that mistakes and confusion is avoided when more than one person is acting in the capacity of nurse.
Another object is the provisionot an indicating chart of the above character which has associated therewith the usual physicians prescription blank, so that each prescription will have its own indicating chart which is severed therefrom.
Other objects and advantageous of the invention will appear as the *following description is read in connection with the accompanying drawings.
In the drawings Figure 1 is a View of a chart shown in connection with a physicians 'perscription blank.
Figure 2 shows the chart separated the blank.

Referring to the drawings in detail, the invention as illustrated in Figure 2 is formed of a sheet of paper which is divided into spaces 10 containing indicia for indicating the various days of the week, for example, S indicating Sunday, M. Monday, T., Tuesday, etc. Disposed at right angles to the spaces 10 and located beneath said spaces are columns 11, two of such columns being provided for each space 10. The columns contain vertical rows of numbers from 1 to 12 which are designed to represent the hours of a. day. One column beneath each space is provided at its top with the letters A. M. to indicate anti-meridian, while the top 'of the other column of each space contains the letters P. M. to indicate post-meridian. The columns of numbers located under A. M. is of a color to distinguish it from the numbers beneath P.' M., so that morning and evening may be readily distinguished.

,ln the use of the chart, should the physt` cian prescribe the medicine to beadmmis- "tered every two hours`and7the tirst dose given at 10 oelock A. M., Sunday, thenumany person consulting thechart will readily see that the next dose is to be administered at 12 oclock. rlhis method of crossing out the numbers will be followed throughout the use of the chart so that the next uncrossed number will be the time for administering the next dose of medicine. Should the time be 10:30 or v10:45, 8O or L15 may be written in the space 12 provided for minutes and the nurse or other person administering the dose may readily determine the correct time.
ln Figure 1 the chart is shown in connection with a physicians prescription blank which is indicated at 13. The chart in all other respects is similar to that described in Figure 2, while a lperforated line 14C separates the prescription blank and chart so that they may be separated. Then made into pad or tablet form, the chart forms a convenient rest for the hand orn the physician when writing the prescription. In Figure 3 there is illustrated an exceedingly simpleform ot the invention. ln'. this form they spaces for containing the indications for the days of the week are shown at 15 and contain the iirst letters of each day as explained in connection with the preceding form of the invention. Vertical columnsl are provided beneath each space 15 by means of spaced vertical lines 17. rFhe columns16 are divided by horizontally disposed lines 18 so as to define separate spaces which extend both vertically and horizontally beneath each of the spaces 15. To the eXtreme lett of the chart a relatively large space 19 is provided. This space is in horizontal alignment with the spaces provided by the lines 18 and contains the word Date. When using this form of the chart the time at which the dose i oit medicine is due, or should be given, is indithe left hand edge of the chart. The topmost space conta-ins the name Date and the spaces 2O in horizontal alignment with this Word are adapted to have Written therein the days of the month. Each of the spaces 21 beneath the Word Date contains the Word Due and the hour at Which the medicine is due may be Written in the spaces 2O beneath the dates.-
While the invention is shown and described for the use of keeping track of the time for prescribing medicine, it is useful for many other purposes and it is therefore not the purpose of the present application to restrict the invention to this particular use. y
The invention is susceptible of various changes in its form, proportions and minor details of construction and the right in herein reserved to make such changes as properly fall within the scope of the appended claim.
Having described the invention what is claimed is zyAn indicating chart comprising` a sheet scored to provide vertical spaces having indicia at the top indicating the days ott the Week, said spaces being further divided to provide vertical spaces having indicia at the top indicating anti-meridian and postmeridian and each of the last mentioned spaces having numbers from l to 12 indicating divisions of a day and a relatively wide blank space beneath thev numbers upon which fractional parts of an hour may be Written, in combination with a physicians prescription blank and a line of perforations separating the chart and blank.
