Abstract

The method involves applying inflation and deflation cycles to inflation cell series (S1, S2) during which a series is inflated and deflated till the transport and threshold pressures, respectively. A threshold pressure residual value is measured at the end of the deflation cycle. A set point value to be assigned to the transport pressure is fixed based on the residual value and is applied to the inflation pressure.

Description

The invention relates to a method for regulating an inflatable support of the mattress, mattress or cushioning type comprising at least two series of successively inflatable cells for preventing bedsores, generically called mattresses.

Some conditions can lead to prolonged periods of bed rest. A detrimental consequence of prolonged bed rest is the formation of bedsores, that is, necroses of the dermis and hypodermis, which are characterized by a crust that tends to separate from surrounding healthy tissues.

Les escarres sont particulièrement marquées au niveau des saillies osseuses notamment talon, sacrum, hanches, coude, épaule, etc...Pressure ulcers are caused by a lack of blood supply due to compression of the skin on a bearing surface on which a bedridden patient rests. The pressure sores are particularly marked at the level of the bony protrusions including heel, sacrum, hips, elbow, shoulder, etc ...

One means of preventing pressure ulcer formation is to place the patient on a mattress comprising at least two sets of pneumatically inflatable cells. These series of cells generally two in number extend over the width of the mattress and are inflated alternately. Thus, the tissues of the dermis and the hypodermis can be satisfactorily vascularized since, alternatively, they are facing a series of deflated cells, the patient then being carried by the series of inflated cells.

This type of mattress effectively fights the formation of bedsores in a patient having to undergo prolonged bed rest.

However, the implementation of a mattress of the aforementioned type can be problematic insofar as, on the one hand, each patient has a specific size and mass, which results in a distribution of the pressure exerted by the patient on the patient's own mattress and, on the other hand, the patient may have to change position, for example, to move from the seated position to the supine position, which results in a change in the distribution of the patient. the pressure exerted by the patient on the mattress.

The consequence for the medical staff in charge of a bedridden patient is that he must set the set value of the pressure to be applied in the inflated cells according to the weight of the patient.

To be fully effective, it is indeed necessary that the series of inflated cells actually wears the patient. In other words, it is important for the patient's body to rest mainly on the series of inflated cells so that the patient's body parts facing the deflated cells are revascularized satisfactorily.

In the same way, when a patient changes position, the medical staff should change the set point of the pressure to be applied in the inflated cells.

It can be seen that, in order to be totally effective, a mattress or any inflatable support of the aforementioned type must be used particularly attentively since it is necessary to adapt the setpoint value applied to a series of inflated cells as a function of the morphology of each patient, and according to the position of a patient during his bed rest.

The device according to document GB 2 290 958 therefore proposes to manage the pressure by an apparatus external to the mattress, in this case a mat placed under the mattress itself. This is not without disadvantage since the sheet of the sensor can cause a sliding of the mattress. In addition, the introduction of an additional element necessarily entails a risk of failure and a maintenance obligation.

This method provides for defining a preliminary support pressure which is determined by the equilibrium of the pressures prevailing in the cells A and B of the mattress. This process is difficult to practice since it requires controlling the pressure in both sets of cells to a pressure equilibrium. In addition, it slows the alternation of inflation and deflation of each cell.

A problem that the invention proposes to solve lies in the improvement of a method for regulating an inflatable support of the type mentioned above, so as to avoid the disadvantages indicated and, in particular, by proposing a control method that enables a automatic adaptation to the morphology and position of a patient on a mattress.

The invention also aims at providing a simple and reliable method.

The invention also aims to propose a method that can be implemented on pre-existing supports.

The subject of the invention is essentially a method of regulating an inflatable support capable of supporting a patient, comprising at least two series of successively inflatable cells, characterized in that it comprises the steps consisting of:
  • applying successively to each series of inflatable cells an inflation cycle in which a series of cells is gradually inflated to a carrying pressure followed by a deflation cycle in which the series of cells is progressively deflated to a maximum threshold pressure,
  • measure the residual value of the threshold pressure in the series of cells at the end of the deflation cycle,
  • set according to the residual value of the threshold pressure measured in a series of inflatable cells at the end of the deflation cycle, the set value to be assigned to the porting pressure for the series of cells which is in the inflation cycle,
  • apply to the inflation pressure the pressure at the set point previously determined to the series of inflatable cells which is in an inflation cycle.

The idea underlying the invention is to use the residual pressure measured in a series of deflated cells as an indicator of the weight exerted by the patient on the inflatable support.

Using this data, it is then possible to adjust each inflation cycle the set value of the porting pressure to be applied to the series of cells that is in the inflation cycle.

Thus, it is from data collected in the same inflatable support that the adjustment of the patient's carrying pressure is made.

It is noted that this collection of information, as to the weight and the posture of the patient, is done in masked time since it occurs while one series of cells ensures the carrying of the patient, while the other is in cycle deflation.

According to one embodiment, the method according to the invention consists of:
  • predefining at least two pressure ranges for the residual value of threshold pressure, each range corresponding to a set value of the porting pressure,
  • measure the residual value of the threshold pressure at the end of the deflation cycle in a series of inflatable cells,
  • comparing the measured residual value of the threshold pressure with a predefined pressure range,
  • apply the setpoint of the porting pressure corresponding to the predefined pressure range, to the series of inflatable cells in the inflation cycle.

Preferably, the pressure ranges for the residual value of the threshold pressure can be three in number and correspond respectively to three set values of the inflation pressure, which corresponds in practice to three types of patients: light, intermediate and heavy.

The number of tracks can be higher, for example four or more, to be adapted to different volumes of different media.

In addition, the residual value of the threshold pressure is averaged with at least a residual value of the threshold pressure measured during the previous deflation cycle.

For a good understanding, the invention is described with reference to the accompanying drawing showing by way of non-limiting example a mattress operating according to an embodiment of the method according to the invention.

Figures 1 and 2 show schematically an inflatable mattress comprising two sets of inflatable cells which are alternately inflated and deflated and regulated by the method according to the invention.

Figures 1 and 2 illustrate, schematically, a mattress which comprises two series S1, S2 of inflatable cells. These cells extend over the entire length of the mattress and a patient can come to rest on these cells.

Each series of cells consists of several inflatable transverse cells interconnected to be at the same pressure, nested with the transverse cells of the second series.

The figures show a mattress having two series of cells, which is a widespread provision, it is nevertheless possible to provide more than two sets of inflatable cells.

The pressurization of each series S1, S2 of inflatable cells is provided by a compressor 4 which is controlled by a microprocessor 5. A distribution circuit can be placed downstream of the compressor 4 and can direct the flow of compressed air to one or the other series of cells according to the instruction of the microprocessor 5.

The microprocessor 5 may also control vent valves that deflate the cells of the deflation members not shown in the drawing.

Conventionally, the microprocessor controls the compressor 4 and the deflation members, so that a series of cells S1 or S2 enters an inflation cycle during which a porting pressure is applied progressively to this series of cells, while the other series of cells enters a deflation cycle during which the pressure of this series of cells gradually decreases until a threshold pressure is reached which corresponds to the minimum pressure prevailing in a series of cells. Portage pressure means the pressure applied to a series of cells during an inflation cycle, which carries the patient.

It may be noted that a system of pressure sensors equips the inflatable cells of each series and informs the microprocessor on the pressure that resides in them; there is thus a conventional closed loop control.

Inflation cycles and deflation cycles are alternately followed alternately on each series, to allow a vascuiarisation éernée all of the dermal and hypodermic surface of the patient.

In one embodiment, when the pressure in the inflatable cell series reaches the set point of Pm, the microprocessor 5 switches the operation of the mattress to dynamic that is described previously. The set value is of the order of a few tens of hectopascals.

The control method according to the invention is then distinguished from known methods. The residual pressure at the end of the deflation cycle is then subject to a specific measurement. Indeed, the residual pressure at the end of the deflation cycle is representative of the weight exerted by the patient on the mattress.

In one embodiment of the control method, it is intended to define three residual pressure ranges at the end of the deflation phase.

According to this embodiment, there are three cases, namely:
  • A - residual pressure at the end of the deflation cycle less than P1. A low residual pressure at the end of the deflation cycle denotes a patient exerting a light weight on the mattress. It is then planned to set a low port pressure setpoint.
  • B - residual pressure at the end of the deflation cycle between P1 and P2. Residual pressure at the end of the average deflation cycle denotes a patient exerting a mean weight. It is then planned to set a mean port pressure setpoint. This setpoint makes it possible to effectively carry the patient.
  • C - residual pressure at the end of the deflation cycle greater than P2. A residual pressure at the end of the race denotes a patient exerting a significant weight on the mattress. It is then planned to set a high portage pressure setpoint. In practice, this setpoint can be obtained by the maximum flow rate of the compressor.

It should be noted that the value of the residual pressure at the end of the deflation cycle can be averaged with one or more values of this same pressure measured during the previous cycle; this has the effect of stabilizing the operation of the mattress. The measurement time of the residual pressure on the series of deflated cells may be about ten seconds.

It may be noted that the method according to the invention is also fully effective in case of change of position of the patient. Indeed, a change in position of the patient results in a change in the pressure exerted on the mattress. For example, the passage of the position lying in the sitting position implies an increase in the pressure exerted on the mattress located at the pelvis. The control method makes it possible to autonomously manage this change since, at each deflation cycle of each series of inflatable cells, the measurement of the residual pressure at the end of a deflation cycle on a series of cells makes it possible to determine:
  • if it is necessary to increase the set point of the porting pressure in the series of inflatable cells to ensure a good carriage of the patient, including in areas in which he exerts maximum pressure, (for example, case of change of position lying down at the sitting position),
  • if it is necessary to reduce the set value of the porting pressure in the series of cells which are in the inflation cycle (for example, the case of the change of position from the seated position to the supine position),
  • if there is no need to change the set value of the porting pressure in the series of cells that are in the inflation cycle.

For the comfort of the patient and for a good vascularization, an inflation / deflation cycle time of 4 to 6 minutes can be provided.

If the control method operates in dynamic mode, as just described, it is also possible to use the mattress in a so-called static mode in which the two series of cells are inflated. This mode of use can be useful for performing a care or during a transport of a patient.

Each mode of operation of the mattress may be accompanied by light and / or sound reminders that inform the medical staff of the operating mode on which the mattress operates.

Of course, the invention is not limited to the embodiment described above by way of example, but it embraces all embodiments. The numerical values of pressure and cycle time are in particular purely indicative. It is also conceivable to define residual pressure ranges at the end of deflation phase which are narrower or wider than those defined above, so as to increase the sensitivity of the mattress if necessary.

It is also conceivable to divide the mattress into several zones, for example head, trunk, foot, and to separately manage the pressure cycles in each zone with the method according to the invention.

It is also conceivable that the cells extend only over a portion of the mattress.
