Description
- Die vorliegende Erfindung bezieht sich auf eine implantierbare Vorrichtung zum Fördern der Fusion zweier benachbarter Knochenstrukturen. Insbesondere bezieht sich die Erfindung auf einen expandierbaren Funktionskäfig, der in einer reduzierten Größe in einen Raum zwischen zwei Wirbeln eingebracht und nach dem Einbringen auf eine gewünschte Größe expandiert wird. Obwohl die Vorrichtung gemäß der vorliegenden Erfindung auch in anderen Bereichen des Körpers angewendet werden kann, wird die vorliegende Erfindung vorzugsweise für die Zwischenwirbelfusion verwendet.
- Es gab eine hohe Anzahl von Versuchen, ein außergewöhnliches Bandscheibenimplantat zu entwickeln, welches dazu verwenden werden könnte, eine beschädigte Bandscheibe zu ersetzen und dennoch die Stabilität des Bandscheibenzwischenraumes zwischen benachbarten Wirbeln zu erhalten, zumindest bis eine vollständige Arthrodese erreicht ist. Diese „Vorrichtungen zur Zwischenwirbelversteifung" („interbody fusion devices") haben vielfältige Formen. Zum Beispiel hat eine der weit verbreiteten Konstruktionen die Form eines zylindrischen Implantates. Diese Typen von Implantaten werden durch die Patente von Bagby, US-Patent Nr. 4,501,269; Brantigan, US-Patent Nr. 4,878,915, Ray, US-Patente Nr. 4,961,740 und Nr. 5,055,104; und Michelson, US-Patent Nr. 5,015,247 beschrieben. Bei den zylindrischen Implantaten kann der Außenbereich des Zylinders mit einem Schraubgewinde versehen sein, um das Einsetzen der Zwischenkörperfusionsvorrichtung zu erleichtern, wie dieses durch die Patente von Ray, Brantigan und Michelson beschrieben ist. Als Alternative sind einige der Fusionsimplantate so konstruiert, dass sie in den Bandscheibenraum eingeschlagen werden. Diese Art von Vorrichtung wird durch das Patent von Brantigan beschrieben.
- Zwischenkörperfusionsvorrichtungen können generell in zwei Basiskategorien aufgeteilt werden, nämlich in feste Implantate und Implantate, die ausgelegt sind, um das Einwachsen von Knochen zu erlauben. Feste Implantate werden beschrieben in den US-Patenten Nr. 4,879,915; 4,743,256; 4,349,921 und 4,714,469. Die verbleibenden oben erwähnten Patente berücksichtigen den Aspekt, dass Knochengewebe durch das Implantat wachsen kann und es wurde gefunden, dass die Vorrichtungen, die ein natürliches Einwachsen von Knochengewebe fördern, eine schnellere und stabilere Arthrodese erreichen. Die in den Patent von Michelson beschriebene Vorrichtung ist repräsentativ für diese Art eines hohlen Implantates, das typischerweise mit einer das Knocheneinwachsen induzierenden Substanz gefüllt ist, um das Einwachsen von Knochengewebe in und durch die Vorrichtung zu fördern. Dieses Implantat weist mehrere kreisförmige Öffnungen auf, die mit dem hohlen Inneren des Implantates kommunizieren, wodurch ein Weg für das Gewebewachstum zwischen den vertebralen Endplatten und dem das Knochenwachstum fördernden Material in dem Implantat vorgesehen wird. Bei der Vorbereitung des Bandscheibenraumes werden die Endflächen vorzugsweise bis auf den reinen Knochen reduziert, um das Einwachsen von Gewebe zu erleichtern. Während der Fusion hilft die durch Michselson vorgeschlagene Metallstruktur, die Durchgängigkeit und die Stabilität des zu versteifenden Bewegungssegmentes aufrechtzuerhalten. Wenn zudem einmal Arthrodese erreicht wird, dient das Implantat selbst als eine Art Anker für die feste knöcherne Masse.
- Ein Problem, das bei den obigen bekannten Vorrichtungen nicht angesprochen ist, betrifft das Erhalten und Wiederherstellen der normalen Anatomie des versteiften Wirbelsäulensegmentes. Wenn die Bandscheibe einmal entfernt ist, ist die normale lordotische oder kyphotische Krümmung der Wirbelsäule natürlich eliminiert. Mit den bekannten Vorrichtungen wird die Notwendigkeit, diese Krümmung wieder herzustellen, nicht beachtet. Zum Beispiel können benachbarte Wirbelkörper mit einer zylindrischen Reibvorrichtung, die auf das spezielle Implantat abgestellt ist, abgerieben werden. In einigen Fällen wird die normale Krümmung vor dem Abreiben eingerichtet und erst danach das Implantat eingefügt. Jedoch wird das Abreiben des hinteren Bereiches wegen der Entfernung von lasttragendem Knochen der Wirbel generell nicht sehr akzeptiert, auch weil es typischerweise schwierig ist, durch den hinteren Bereich des unteren lumbalen Segmentes zu reiben, wo die Lordose am größten ist. In den meisten Fällen wird, wenn Implantate dieses Types verwendet werden, nichts unternommen, um die lordotische Krümmung wieder herzustellen, sodass das zylindrische Implantat unter Umständen eine kyphotische Deformation verursachen kann, wenn der Wirbel sich um das Implantat setzt. Dieses Phänomen kann oft zu einer erneuten chirurgischen Operation führen, da das Rückgrat nicht ausbalanciert sein kann.
- In jedem der oben aufgelisteten Patente ist der transversale Querschnitt des Implantates im Wesentlichen über die gesamte Länge konstant und typischerweise in Form eines geraden kreisförmigen Zylinders. Andere Implantate sind für Zwischenkörperfusionen entwickelt worden, die keinen konstanten Querschnitt haben. Zum Beispiel zeigt das Patent von McKenna, das US-Patent Nr. 4,714,469 ein hablbkugeliges Implantat mit länglichen Ausstülpungen, die in die vertebrale Endplatte hineinragen. Ferner zeigt das US-Patent Nr. 5,669,909 von Zdeblick et al. ein kegelstumpfförmiges Implantat, das mit Hilfe einer Schraubverbindung in dem Zwischenwirbelraum aufgenommen wird. Jedoch erfordern diese Vorrichtungen eine Öffnung, die zumindest so groß wie das größte Segment der Vorrichtung ist. Das Erfordernis für solch eine relativ große Öffnung kann die Verwendung von solchen Vorrichtungen beschränken, insbesondere, wo ein Zugang zu der Wirbelsäule aufgrund von behindernden Gefößen und neurologischen Strukturen begrenzt ist.
- Es wurden noch weitere Implantate entwickelt, bei denen die Größe des Implantates nach dem Einführen eingestellt werden kann. Die US-Patente 5,665,122 von Kambin, 5,554,191 von LaHille et al. und 6,563,763 von Errico et al. beschreiben Implantate, die zumindest einen gewissen Grad der Einstellbarkeit für die Höhe des Implantates zulassen, um die Lordose wieder herzustellen. Jedoch erlauben es diese Implantate nicht, dass die Vorrichtung leicht und sicher in einen Bandscheibenraum eingeführt werden kann, und der innere Expansionsmechanismus begrenzt die Fähigkeit, das Innere mit einer großen Menge von Knocheneinwachsmaterial zu füllen
- Die WO97/00054 offenbart ein Implantat aus zwei separaten Elementen, die durch eine komplizierte Einrichtung mit einem Endbereich miteinander verbunden sind, um einen Winkel zwischen diesen Elementen einzuhalten.
- US-A-5,683,394 beschreibt ein Implantat, das aus einem einstückigen Käfig mit einem Enddeckel gebildet wird.
- Die US-A-5,702,455 bezieht sich auf ein Implantat, das durch zwei rohrförmige Elemente mit Löchern gebildet wird, die durch ein mit Gewinde versehenes Rohr verbunden werden.
- Angesichts der Beschränkungen der bekannten Einrichtungen verbleibt daher ein Erfordernis für eine Zwischenkörper-Fusionskäfig, der fähig ist, die Wirbelsäule in einer Art zu stabilisieren, die vergleichbar mit dem gegenwärtig verwendeten Zwischenkörper-Implantatkonstruktionen ist, und gleichzeitig einen Mechanismus zum Wiederherstellen der normalen Lordose der Wirbelsäule liefert. Nach der Expansion sollte das Implantat einen inneren Hohlraum haben, in dem ein Knochenpolymer oder ein Knochensubstitut aufgenommen werden kann, durch das Knochenwachstum durch das expandierte Implantat angeregt wird.
- ZUSAMMENFASSUNG DER ERFINDUNG
- In Antwort auf die Erfordernisse, die durch bekannte Vorrichtungen noch ungelöst sind, schlägt die vorliegende Erfindung einen expandierbaren Fusionskäfig vor, der zwischen ein Paar von Rückenwirbelkörpern einsetzbar ist, um die normale Winkelbeziehung zwischen benachbarten Wirbeln wieder herzustellen. Insbesondere weist eine Vorrichtung gemäß der vorliegenden Erfindung einen Körper mit einer äußeren Knochenanlagefläche und einer inneren, eine interne Kammer begrenzenden Innenfläche sowie einen Haltemechanismus auf. Der Körper hat einen festen Bereich und einen expandierbaren Bereich. Der expandierbare Bereich ist in zumindest einem ersten Abschnitt und in einen zweiten Abschnitt unterteilt, die an dem festen Bereich miteinander verbunden sind. Der erste Bereich ist in bezug auf den zweiten Bereich in dem expandierbaren Bereich bewegbar, um die gesamte Größe des Fusionskäfigs zu expandieren. Ein Expansionselement ist so ausgebildet, dass es zumindest teilweise in der internen Kammer aufgenommen wird, und hat eine äußere Oberfläche, die so ausgestaltet ist, dass sie an dem Haltemechanismus anliegt. Eine Bewegung des Expansionselementes innerhalb der internen Kammer verschiebt den ersten Bereich in bezug zu dem zweiten Bereich, wodurch der expandierbare Bereich expandiert. Das Expansionselement wird durch Anlage an dem Haltemechanismus des Körpers in Position gehalten.
- Gemäß einer anderen Ausführung bezieht sich die Erfindung auf ein Einführwerkzeug, das mit einem expandierbaren Fusionsimplantat zusammenwirkt. Das Einführwerkzeug dient dazu, das Implantat einzufügen und das Expansionselement zu bewegen, um den Käfig zu expandieren. Ein Käfig gemäß der vorliegenden Erfindung wird mit einem Einführwerkzeug verwendet, der eine äußere Hülse aufweist, der mit dem Körper des Käfigs verbindbar ist, um eine Druckkraft auf den Körper zu übertragen. Das Einführwerkzeug weist ferner einen inneren Schaft auf, der beweglich in der äußeren Hülse gelegen ist. Der innere Schaft ist ausgebildet, um in zumindest einem Bereich der internen Kammer des Käfigs eingeführt zu werden. Der innere Schaft weist ein distales Ende auf, das entfernbar in das Expansionselement
50 eingreift, wobei eine Bewegung des inneren Schaftes in bezug zu der äußeren Hülse das Expansionselement in bezug zu dem Körper bewegt. - Ferner sind Verfahren zum Einführen einer expandierbaren Zwischenkörpervorrichtung beschrieben. Gemäß einem Aspekt wird eine Vorrichtung gemäß der vorliegenden Erfindung in den Raum einer Bandscheibe eingefügt. Der bewegliche Bereich der Vorrichtung wird so positioniert, dass er an einem Bereich des Wirbels anliegt, für den ein zusätzlicher Abstand erforderlich ist. Das Expansionselement wird dann in der internen Kammer gegen den beweglichen Bereich bewegt, um den Käfig auf die gewünschte Höhe zu expandieren. Das Expansionselement kann in den Haltemechanismus eingreifen, um die Bewegung des Expansionselementes zu beschränken.
- Eine Aufgabe de vorliegenden Erfindung ist es, ein Implantat anzugeben, das eine reduzierte Größe und Konfiguration für die Einführung aufweist und von der Einführungskonfiguration auf eine größere Konfiguration expandiert werden kann.
- Eine weitere Aufgabe der vorliegenden Erfindung ist es, ein expandierbares Implantat anzugeben, das eine im Wesentlichen ungehindert zugängliche interne Kammer aufweist, um dort ein das Knochenwachstum anregendes Material aufzunehmen.
- Eine noch weitere Aufgabe der vorliegenden Erfindung ist es, einen Fusionskäfig anzugeben, der leicht eingeführt werden kann und auf eine größere Gestalt expandierbar ist, um eine Lordose zu etablieren.
- Weitere Aufgaben und Vorteile der vorliegenden Erfindung werden aus der folgenden Beschreibung ersichtlich.

- Zum Zwecke der Verbesserung des Verständnisses der Prinzipien gemäß der Erfindung wird nun Bezug genommen auf die in den Figuren dargestellten Ausführungsbeispiele, wobei eine spezifische Sprache für deren Beschreibung verwendet wird. Es ist nichts desto trotz verständlich, dass hiermit keine Beschränkung des Gegenstandes der Erfindung beabsichtigt wird, wobei auch solche Veränderungen und Modifikationen in den dargestellten Vorrichtungen und weitere Anwendungen der Prinzipien der Erfindung, wie dieses dargestellt ist, betrachtet werden, sofern sie in den Gegenstand der anhängenden Ansprüche fallen.
- In den
1 bis3 ist ein erstes Ausführungsbeispiel eines expandierbaren Käfigs10 gemäß der vorliegenden Erfindung dargestellt. Bei diesem Ausführungsbeispiel der Erfindung hat der expandierbare Käfig10 eine zylindrische äußere Oberfläche11 , die ein externes Schraubenmuster12 (nicht gezeigt in3 ) aufweist, mit dem in zwei benachbarte Wirbel (siehe7a und7b ) eingegriffen werden kann und der Käfig in dem Raum einer Bandscheibe vorwärts bewegt wird, wenn der Käfig10 um die Längsachse13 verdreht wird. Wie klar in3 gezeigt, ist der expandierbare Käfig10 im Wesentlichen hohl mit einer inneren Oberfläche21 , die einen inneren Hohlraum17 begrenzt. Der expandierbare Käfig10 weist ein Fenster16 durch einen expandierbaren Abschnitt24 auf, durch das Knochengewebe einwachsen kann, sowie ferner ein identisches Knocheneinwachsfenster19 , das durch den expandierbaren Abschnitt26 hindurchgreift. Diese Einwachsfenster ermöglichen eine Kommunikation zwischen den Wirbelkörpern (7 ) und der inneren Kammer17 . In den meisten Anwendungen wird ein Knochenwachstum förderndes Material in der internen Kammer17 des expandierbaren Käfigs platziert, um das Knochengewebe anzuregen, in und durch die Fusionsvorrichtung10 zu wachsen. - Wie in den begleitenden Zeichnungen dargestellt, ist der expandierbare Käfig
10 vorzugsweise aus vier separaten Abschnitten zusammengesetzt, die jeweils an einem expandierbaren Ende18 durch einen Kanal voneinander getrennt sind, der sich längs von dem expandierbaren Ende (18 ) gegen ein festes Ende20 erstreckt. Gemäß der2a ist ein erster expandierbarer Abschnitt24 von einem ersten festen Abschnitt40 durch einen Kanal14 getrennt. In einer ähnlichen Weise ist ein zweiter expandierbarer Abschnitt26 von dem ersten festen Abschnitt40 durch einen Kanal22 getrennt. Jeder der Kanäle14 und22 erstreckt sich von dem expandierenden Ende18 gegen das feste Ende20 . Die Kanäle enden in einer Öffnung mit einem geringfügig größeren Durchmesser, die vorzugsweise als Gelenk während der Expansion der Vorrichtung dient, um den Druck und Deformation in der Nähe des festen Endes20 zu konzentrieren. In einer ähnlichen Weise, wie dieses in3 gezeigt ist, ist ein zweiter fester Abschnitt41 von einem ersten expandierbaren Abschnitt24 durch einen Kanal15 und von einem zweiten expandierbaren Abschnitt26 durch einen ähnlichen Kanal (nicht gezeigt) getrennt. Auf diese Weise wird der expandierbare Käfig10 durch vier Abschnitte gebildet, wobei jeder Abschnitt von einem anderen an dem expandierbaren Ende18 durch Kanäle18 getrennt ist, die sich von der äußeren Oberfläche11 in die interne Kammer17 erstrecken. Die Abschnitte sind an dem festen Ende20 durch einen Verbindungsbereich44 verbunden, sodass jeder der Abschnitte sich mehr oder weniger unabhängig von den anderen Abschnitten am expandierbaren Ende18 bewegen kann, während sie mit der Vorrichtung durch den Verbindungsbereich44 verbunden bleiben. Obwohl das vorliegende Ausführungsbeispiel einstückig geformte Abschnitte aufweist, kann es berücksichtigt werden, separate Komponente miteinander zu verbinden, um den expandierbaren Käfig zu bilden, ohne dass hierdurch von der Erfindung abgewichen wird. - Während in den bevorzugten Ausführungsbeispielen vier voneinander getrennte Abschnitte gezeigt werden, ist es natürlich möglich, mehr oder wenige Abschnitte zu verwenden, ohne dass von dem Gegenstand der Erfindung abgewichen wird. Obwohl darüber hinaus bei einer bevorzugten Ausführung der Erfindung die Kanäle von der äußeren Oberfläche sich in die interne Kammer
17 erstrecken, ist es natürlich möglich, dass die Kammern sich nicht bis in das Innere der Kammer erstrecken. Ein solcher Kanal könnte durch eine überlappende Zwischenfläche zwischen zwei benachbarten Abschnitten gebildet werden, ohne dass eine Öffnung für das Einwachsen von Knochengewebe in die interne Kammer17 geschaffen wird. Auch wenn ferner Kanäle in der vorgestellten Ausführung des expandierbaren Käfigs gezeigt werden, ist es möglich, dass diese Kanäle erst gebildet werden, wenn das Implantat expandiert wird. Im Rahmen eines Beispieles und ohne Beschränkung könnte dieses erfolgen, indem ein zerbrechlicher Bereich zwischen benachbarten Abschnitten aufgebrochen wird, oder indem Material zwischen benachbarten Abschnitten deformiert wird. - Bei einem Aspekt der Erfindung bildet die interne Kammer
17 das größte Volumen des gesamten Käfigs10 . Insbesondere in bezug zu2a wird durch die äußere Oberfläche ohne das Schraubenmuster12 ein Durchmesser63 definiert. Die interne Kammer17 hat einen Durchmesser64 , der dem Durchmesser63 abzüglich der Dicke62 der beiden Abschnitte entspricht. - Bei einem bevorzugten Ausführungsbeispiel ist die Dicke
62 der Abschnitte so gewählt, dass das Volumen der internen Kammer17 insbesondere unter Einschluß der Leerräume, die durch die Einwachsfenster16 und19 und die verschiedenen Kanäle offen sind, die Mehrheit des gesamten Volumens des Käfigs10 bildet. Daher liefert die vorliegende Erfindung Raum für ein großes Volumen von Knochenwachstum förderndem Material, das in die Vorrichtung eingefügt wird, um das Einwachsen von Knochengewebe zu fördern. - An dem Expansionsende
18 , das in2a benachbart zu den Abschnitten24 ,40 und26 gelegen ist, hat jeder der Abschnitte geneigte Oberflächen28 ,34 bzw.30 . Benachbart zu dem inneren Ende jedes der geneigten Oberflächen28 ,34 und30 ist eine innere Schulter36 ,42 bzw.38 ausgebildet. Gemäß3 weist der Abschnitt41 eine ähnliche geneigte Oberfläche35 und eine innere Schulter43 auf. Wie in2a dargestellt, sind die bogenförmig geneigten Oberflächen28 und30 mit dem Winkel32 in bezug zueinander geneigt. Bei einer bevorzugten Ausführung ist dieser Winkel ungefähr 96°, obwohl es selbstverständlich ist, dass andere Winkel verwendet werden können, und zwar abhängig von dem Grad der gewünschten Expansion und dem Verstellweg, die ein Expansionselement benötigt, um diese Expansion zu erzeugen. - In
2b ist der Käfig10 sowie ein Expansionskeil50 dargestellt, der an dem expandierbaren Ende18 angeordnet ist. Der Expansionskeil50 weist erste und zweite gegenüberliegende Expansions-Schrägflächen52 und54 auf, die eine sich verjüngende gebogene Oberfläche aufweisen, die an die geneigten Oberflächen28 und30 der Abschnitte24 bzw.26 angepasst sind und mit diesen korrespondieren. Der Keil50 weist Seitenwände58 und60 mit ebenen Oberflächen auf, die an den geneigten Oberflächen34 und35 der festen Abschnitte40 bzw.41 liegen. Darüber hinaus weist der Expansionskeil50 eine zentrale Öffnung56 auf, in der ein Antriebswerkzeug aufgenommen werden kann. Bei einer bevorzugten Ausführungsform weist die zentrale Öffnung56 ein Schraubengewinde auf, in das ein Antriebswerkzeug mit einem entsprechenden Schraubengewinde eingreift. - Es ist ersichtlich, dass dann, wenn der Keil
50 gegen das feste Ende20 voranbewegt wird, die Oberflächen52 und54 gegen die geneigten Oberflächen28 bzw.30 wirken, um die Abschnitte24 und26 voneinander an dem expandierbaren Ende18 zu entfernen. Wenn der Expansionskeil50 längs der geneigten Oberflächen28 und30 eingeführt wird, tendieren die Abschnitte24 und26 dazu, am Expansionsende18 zu expandieren, wobei eine Deformation an dem festen Ende20 erfolgt. Das zusammenhängende Verbindungsmaterial44 verbindet alle Abschnitte und erlaubt keine Expansion an dem festen Ende. Wenn der Keil50 weiter gegen das feste Ende20 voranbewegt wird, wird der Keil über die Schultern36 und38 gestoßen, sodass der Keil50 innerhalb des Käfigs10 eingefangen wird. Als Ergebnis der Anlage der Rückfläche55 des Keiles gegen die Schultern36 und38 wird der Expansionskeil50 daran gehindert, aus dem Käfig auszutreten, und der Käfig wird in einem expandierten Zustand gehalten, wobei der Keil sicher in Position gehalten wird. Es ist ersichtlich, dass die Anlage der ebenen Flächen58 und60 an den festen Abschnitten40 bzw.41 dazu führt, dass diese Abschnitte geringfügig, wenn überhaupt, deformierbar expandiert werden und daher sie in ihren originalen Positionen fixiert gehalten werden. Ferner weisen die festen Abschnitte40 und41 jeweils eine Schulter42 und43 auf, die an der Rückfläche55 des Keiles50 anliegt, sobald dieser über die Schultern hinaus geglitten ist. Aus diesem Grunde halten die festen Abschnitte40 und41 ebenfalls den Keil am Platz und verhindern sein Austreiben aus der internen Kammer17 . - In den
8 bis10 ist eine alternative Ausführungsform des expandierbaren Käfigs gemäß der vorliegenden Erfindung dargestellt. Bei dieser Ausführung hat der expandierbare Käfig310 eine äußere Oberfläche311 und ein externes Schraubenmuster312 (nicht in10 gezeigt). Wie bei dem Ausführungsbeispiel gemäß1 weist der expandierbare Käfig310 zwei gegenüberliegende expandierbare Abschnitte324 und326 und zwei gegenüberliegende feste Abschnitte340 und341 auf, die an dem festen Ende320 verbunden sind. Alle diese Abschnitte weisen abgeschrägte Oberflächen und innere Schultern nahe dem expandierbaren Ende318 auf, die an dem Expansionskeil350 anliegen und diesen aufnehmen. Im Gegensatz zu der Ausführung in1 weist der expandierbare Abschnitt324 zwei Fenster316 und317 auf, die durch eine Rippe330 getrennt sind und in die Knochengewebe einwachsen kann. In ähnlicher Weise weist der Abschnitt326 zwei Fenster322 und323 auf, die durch eine Rippe332 getrennt sind und in die Knochengewebe einwachsen kann. Die Verwendung von mehreren Knocheneinwachsfenstern in den expandierbaren Abschnitten erhöht die Gesamtsteifigkeit des Abschnittes, was notwendig ist für längere Käfige oder Käfige, die aus relativ weichen Materialien konstruiert sind. Obwohl diese Ausführung mit zwei Fenstern in jedem expandierbaren Abschnitt dargestellt worden ist, können natürlich auch mehr als zwei Knocheneinwachsfenstern verwendet werden, ohne von dem Gegenstand der Erfindung abzuweichen. - In
6 ist ein Einführungswerkzeug70 gezeigt, das für die Verwendung mit einem expandierenden Käfig gemäß der vorliegenden Erfindung geeignet ist. Das Einführungswerkzeug70 weist eine äußere Hülse72 mit einem Antriebsvorsprung73 auf, die in eine Antriebsnut46 des expandierbaren Käfigs10 eingreift. Während das Einführungswerkzeug70 nur mit einem Vorsprung73 dargestellt ist, ist es selbstverständlich, dass die Vorrichtung einen entgegengerichteten Vorsprung (nicht gezeigt) aufweist, die mit einer Antriebsnut (nicht gezeigt) zusammenwirkt, die gegenüber der Antriebsnut46 an dem Käfig10 angeordnet ist. Das Einführwerkzeug70 weist ferner einen Handgriff71 auf, mit dem eine Drehkraft auf die äußere Hülse72 ausgeübt werden kann, um den Käfig10 unter Drehung einzuführen. Die äußere Hülle72 weist eine interne Kammer auf, die durch den inneren Schaft75 des Einführwerkzeuges begrenzt ist. An dem distalen Ende77 weist der innere Schaft75 des Einführwerkzeuges einen externen Gewindebereich auf, der in das entsprechende Innengewinde der zentralen Öffnung56 des Expansionselementes50 eingreift. An dem proximalen Ende des inneren Schaftes25 ist ein Handgriff76 vorgesehen, um den inneren Schaft77 drehend anzutreiben. Eine Serie von äußeren Gewinden78 sind auf dem inneren Schaft75 nahe dem Handgriff76 angeordnet. Eine innere Gewindenut79 ist um den inneren Schaft75 vorgesehen und greift in Gewinde78 ein, um den inneren Schaft in bezug zu der äußeren Hülse72 zu bewegen. - Bei der Anwendung wird das Einführwerkzeug
70 mit dem expandierbaen Kä10 so in Eingriff gebracht, wie dieses in7a dargestellt ist. Der Antriebsvorsprung73 des äußeren Rohres72 greift in die Antriebsnut46 des expandierbaren Käfiges10 ein, und das mit einem Gewinde versehene distale Ende77 des inneren Schaftes75 greift in die mit einem Gewinde versehene Öffnung56 des Expansionskeiles50 ein. Auf diese Weise wird der Expansionskeil50 sicher in seiner Position nahe dem Expansionsende18 gehalten, während der mit dem Hauptgewinde versehene zylindrische Käfig in den Raum zwischen den Wirbeln eingeführt wird. Bei einer bevorzugten Ausführungsform wird diese Vorrichtung in posteriorer Anwendung zu der Wirbelsäule verwendet, wobei der Expansionskeil50 am vorderen Ende der Vorrichtung positioniert ist. - Gemäß
7a ist der expandierbare Käfig10 in den Zwischenwirbelraum92 eingeführt, wobei das Schraubenmuster12 in die Wirbel80 und82 eingreift, um den Käfig in den Bandscheibenraum vor zu bewegen und diesen dort sicher in einer Position zu halten, sobald er eine endgültige Position erreicht hat, wie dieses in7a gezeigt ist. Da der expandierbare Käfig vorzugsweise ein Zylinder mit einem gleichmäßigen Durchmesser ist, kann er über ein Einführungsrohr mit einem mit dem Schraubendurchmesser des Käfigs10 gleichen Durchmesser eingeführt werden. Wie in7a dargestellt, ist die Oberfläche88 des Wirbels80 in Kontakt mit der äußeren Oberfläche11 des Käfigs10 . Es ist selbstverständlich, dass bei vielen Anwendungen ein Teil der vertebralen Endplatte vor der Einführung des Käfigs entfernt wurde, sodass der Käfig in den teilweise abgetragenen Knochen des Wirbels eingreift. In ähnlicher Weise ist die Oberfläche90 des Wirbels82 in Kontakt mit der äußeren Oberfläche11 des Käfigs10 . In der initialen Einführposition ist die Ausrichtung84 des Wirbels80 und die Ausrichtung86 des Wirbels82 im Wesentlichen parallel mit der Längsachse13 und den expandierbaren Abschnitten24 und26 des Käfigs10 . - Gemäß
7b ist das mit einem Schraubengewinde versehene Ende77 der Einführvorrichtung in engem Eingriff in der mit einem Schraubengewinde versehenen Öffnung56 des Expansionskeils50 und der Antriebsvorsprung73 in Eingriff mit der Antriebsnut46 ; dann wird die mit einem Schraubengewinde versehene Mutter79 um das externe Schraubengewinde78 verdreht, um den Schaft75 in das äußere Rohr72 (6 ) einzuschrauben, wodurch der Expansionskeil50 gegen das feste Ende20 voran bewegt wird. Wenn der Expansionskeil50 gegen das feste Ende20 voran bewegt wird, schieben die schrägen Oberflächen des Expansionskeils50 die expandierbaren Abschnitte24 und26 an dem expandierbaren Ende18 voneinander weg. In einer ähnlichen Weise werden auch die Wirbel80 und82 in der Nähe des expandierbaren Endes18 voneinander weg gedrückt, sodass die Ausrichtungen84 und86 im Wesentlichen parallel zu den expandierbaren Abschnitten24 bzw.26 verbleiben, aber nicht parallel zu der Längsachse13 des Käfigs10 . Auf diese Weise kann die lordotische Krümmung der Wirbelsäule eingerichtet und während des Hei lungsprozesses aufrecht erhalten werden. Zudem verhindert der Eingriff des Expansionskeiles50 an den vorher beschriebenen Schultern der beiden Abschnitte den Expansionskeil daran, aus dem Käfig10 herausgedrückt zu werden. Das Einführwerkzeug kann entfernt und der zugängliche Bereich in der internen Kammer17 mit Knocheneinwachsmaterial gefüllt werden, um ein Wachsen von Knochengewebe durch die Vorrichtung anzuregen. Ein solches Knocheneinwachsmaterial kann zum Beispiel ein Autotransplantat, ein Fremdtransplantat, knochengewebeerzeugende Proteine in einem Träger oder ein anderes Knochengewebewachstum förderndes Material sein. Das Einführen und die Expansion des in den8 bis10 gezeigten alternativen Ausführungsbeispieles erfolgt in der gleichen Weise. - In den
11 bis13 ist ein weiteres Ausführungsbeispiel eines expandierbaren Käfigs gemäß der vorliegenden Erfindung gezeigt. Wie bei den oben beschriebenen Ausführungsbeispielen gemäß der Erfindung ist der Käfig110 eine zylindrische Vorrichtung mit einer äußeren Oberfläche112 , die eine äußere Gewindeanordnung112 aufweist. Der Käfig110 begrenzt eine im Wesentlichen zylindrische interne Kammer117 . Der Käfig110 weist zwei gegenüberliegende expandierbare Abschnitte152 und154 auf, die voneinander durch ein Paar von gegenüberliegenden festen Abschnitten148 und150 getrennt sind. Der feste Abschnitt148 ist von den expandierbaren Abschnitten152 und154 durch Kanäle144 bzw.146 getrennt. Der feste Abschnitt150 ist von den expandierbaren Abschnitten154 und152 durch einen Kanal147 bzw. einen ähnlichen Kanal (nicht gezeigt) getrennt. Der Käfig110 weist ferner Knocheneinwachsfenster129 und120 auf, die den expandierbaren Abschnitt152 durchdringen, und ferner ein identisches Paar von Knocheneinwachsfenstern119 und121 , die durch den expandierbaren Abschnitt154 hindurch greifen. Jedes der Knocheneinwachsfenster erstreckt sich von der äußeren Oberfläche111 bis in die interne Kammer117 . Wie bei den oben beschriebenen Ausführungsbeispielen weist der Käfig110 eine große zugängliche interne Kammer117 auf, die sich längs der Längsachse130 von dem expandierbaren Ende118 in Richtung auf das feste Ende114 erstreckt. Bei dem Ausführungsbeispiel gemäß den11 bis13 weisen die Fenster120 und121 jeweils eine Nase122 bzw.123 in der Nähe des expandierbaren Endes118 auf. - Ein Expander
130 ist dimensioniert, sodass er in der internen Kammer117 aufgenommen wird. Der Expander130 weist einen ersten Bereich125 mit einem Vorsprung126 auf, der sich in die Nase122 erstreckt, und ferner einen gegenüberliegenden zweiten Bereich127 mit einem Vorsprung128 , der sich in die Nase123 erstreckt. Die Vorsprünge126 und128 wirken mit einem externen Gewinde versehenen Kolben124 zusammen, sodass die Position des Expanders130 in dem Käfig110 gehalten wird. Der Expander130 weist ferner eine Biegung132 auf. Wenn in der bevorzugten Ausführungsform eine Biegung bzw. ein Kniestück verwendet wird, ist es selbstverständlich, dass der Expander130 eine Falte oder ein Gelenk zwischen den Bereichen125 und127 aufweisen kann, das eine Anpassung an eine in der Konfiguration reduzierte in12 gezeigte Größe ermöglicht. Der erste Bereich125 weist eine Längsachse72 auf, wohingegen ein zweiter Bereich127 eine Längsachse77 aufweist. In der Konfiguration für eine Einführung in reduzierter Größe, die in12 gezeigt ist, bildet die Längsachse70 einen Winkel162 zu der Längsachse72 . Bei einer bevorzugten Ausführung ist dieser Winkel162 annähernd 90°, obwohl andere Winkel auch betrachtet werden können. Bei der expandierten Konfiguration (nicht dargestellt) kann der Winkel zwischen den Längsachsen70 und72 sich an 180° annähern, wobei ein Winkel von 180° die maximale Expansion der Vorrichtung liefert. - Die interne Kammer
117 wird durch vier vorher beschriebene Abschnitte148 ,150 ,152 und154 begrenzt, die jeweils einen Teil eines Schraubgewindes160 (lediglich teilweise in12 gezeigt) aufweisen. Der Stopfen124 weist ein korrespondierendes externes Schraubgewinde auf, das in das Schraubgewinde160 eingreift. Bei einer bevorzugten Ausführung erstreckt sich ein Verbindungsbereich149 zwischen den festen Abschnitten148 und150 , um eine Aufweitung der festen Abschnitte zu verhindern, wenn der mit dem Schraubgewinde versehene Stopfen124 gegen das expandierbare Ende118 bewegt wird. Der mit dem Schraubgewinde versehene Stopfen124 weist ferner eine zentrale Öffnung136 auf, in die ein Fortsatz eines Einführwerkzeuges eingreifen kann. Bei einer bevorzugten Ausführung ist die zentrale Öffnung136 als hexagonales Muster ausgebildet, um ein ähnliches hexagonal geformtes Einführwerkzeug (nicht gezeigt) aufzunehmen. Der Käfig110 weist ferner eine Antriebsnut142 nahe dem festen Ende114 auf, in die ein Vorsprung eines Antriebswerkzeuges eingreifen kann, um die Einführung des Käfigs110 zwischen zwei benachbarten knöchernden Strukturen zu erlauben. Das Antriebswerkzeug gemäß6 kann mit dem Käfig110 verwendet werden, mit der Ausnahme, dass der innere Schaft75 des Antriebswerkzeuges dann einen hexagonal geformten Bereich an dem distalen Ende77 aufweisen müsste. Es ist ersichtlich, dass dann, wenn der mit einem Schraubgewinde versehene Stopfen124 gegen das expandierbare Ende118 geschraubt wird, der Expander130 in einen expandierten Zustand gezwungen wird, wobei die Abschnitte152 und124 voneinander fortbewegt werden. Wie in13 gezeigt, liefert der Espansionsmechanismus gemäß der vorliegenden Erfindung einen großen internen Hohlraum, in dem Material aufgenommen werden kann, das das Knochenwachstum fördert. - Ein weiteres Ausführungsbeispiel gemäß der vorliegenden Erfindung ist in
14 dargestellt. Der Käfig180 ist in einer ähnlichen Weise wie der Käfig110 ausgebildet, mit der Ausnahme, dass er mehrere kleinere Fenster für das Einwachsen von Knochenwachstum aufweist und nicht nur zwei große Fenster in den expandierbaren Abschnitten192 und194 . Variationen der Anzahl, Größe, Form und der Anordnung der Fenster zum Einwachsen von Knochengewebe, die von den gewünschten Eigenschaften des Einwachsens des Knochengewebes abhängig und den Materialeigenschaften des Käfigs abhängig sind, werden durch die vorliegende Erfindung berücksichtigt. Zudem differiert der Expansionsmechanismus darin, dass der Expander134 eine ebene Vorrichtung ist, d.h. keine Knicke oder Gelenke aufweist und eine Längsachse198 hat. Der Expander184 ist eben und hat ein erstes Ende188 , das in eine Ecke186 greift, die zwischen dem expandierbaren Abschnitt194 und der Endwand199 liegt. Das gegenüberliegende Ende190 liegt an der inneren Oberfläche123 des expandierbaren Abschnittes122 und bewegt sich längs dieser. Ein mit einem Gewinde versehener Kolben182 ist mit einem inneren Gewindemuster196 verschraubt, das längs der inneren Oberflächen der Abschnitte ausgebildet ist. Es ist ersichtlich, dass in dem nicht expandierenden Zustand die Achse198 in bezug zu der Längsachse197 des Käfigs180 geneigt ist. Wenn jedoch der mit dem Schraubgewinde versehene Kolben182 gegen das expandierbare Ende181 voran bewegt wird, bewegt sich der Expander184 in Richtung einer aufrechten Position zu der Achse198 und bewegt sich in eine senkrechte Richtung in bezug zu der Achse197 . Die Bewegung des Expanders184 in Richtung auf eine aufrechte Position expandiert den Käfig180 . In der expandierten Position ist eine große frei zugängliche interne Kammer189 vorhanden, die sich von dem Kolben182 bis zu der Öffnung187 in der Nähe des festen Endes195 erstreckt. Damit kann die große interne Kammer189 mit Knocheneinwachsmaterial gefüllt werden, um die Fusion zwischen benachbarten Wirbeln zu fördern. - In
15 ist ein weiteres Ausführungsbeispiel gemäß der vorliegenden Erfindung dargestellt. Der Käfig210 weist eine obere Oberfläche211 mit einem dort angeordneten Gewindemuster212 auf. Der Käfig210 weist eine Antriebsnut246 auf, in der ein Antriebswerkzeug, wie dieses vorher beschrie ben wurde, aufgenommen werden kann. Der Käfig210 weist ferner mehrere Fenster214 auf, die sich zwischen der äußeren Oberfläche211 bis in eine interne Kammer217 erstrecken. Die interne Kammer217 wird durch schräge Oberflächen216 und215 (gezeigt in gestrichelten Linien) begrenzt, die von dem expandierbaren Ende222 in Richtung auf das feste Ende224 geneigt sind. Die Neigung der geneigten Oberflächen216 und215 könnte ebenso in umgekehrter Richtung liegen, um eine Expansion durch eine Bewegung des Kolbens218 in entgegengesetzter Richtung zu erlauben. Das Kolbenelement218 weist ein externes Schraubengewinde auf, das mit einem Schraubengwinde220 der Oberflächen216 und215 in Eingriff steht. Es ist ersichtlich, dass dann, wenn das Kolbenelement218 nach vorne in Richtung auf das feste Ende224 geschraubt wird, die Abschnitte230 und231 voneinander abgespreizt werden. Wie in15 gezeigt, weist der Abschnitt230 einen Bereich mit reduzierter Weite225 auf, der deformiert wird, wenn das Kolbenelement218 nach vorne bewegt wird. Wie dargestellt, weist das Kolbenelement218 eine zentrale Öffnung237 auf, in dem ein Vorsprung eines Einführwerkzeuges aufgenommen werden kann, um eine Drehkraft zu liefern. Während hier ferner die Antriebsnut246 am expandierbaren Ende222 ausgebildet ist, ist es ersichtlich, dass für einige Einführtechniken es wünschenswert ist, eine Nut246 für ein Einführwerkzeug am festen Ende224 auszubilden. Ebenso kann eine zentrale Öffnung durch das feste Ende224 vorgesehen werden, um einen Fortsatz eines Einführwerkzeuges mit Eingriff in dem Kolben218 aufzunehmen. - Die Kolben
124 des Ausführungsbeispieles gemäß11 , der Kolben182 gemäß dem Ausführungsbeispiel in14 und der Kolben218 gemäß dem Ausführungsbeispiel in15 sind mit einer Serie von externen Schraubengewinden dargestellt und beschrieben worden, die mit entsprechenden Schraubengewinden auf den Abschnitten der Vorrichtung in Eingriff stehen; es ist jedoch offensichtlich, dass all diese Abschnitte oder lediglich die festen Ab schnitte jedes der Vorrichtungen so ausgebildet werden können, um eine Serie von Ratschen zu bilden. Mit einer Ratschenkonfiguration kann jeder der Kolben124 ,182 und218 mit einer äußeren Oberfläche versehen werden, die geeignet ist, über die Ratschen voran bewegt zu werden, um die Vorrichtung zu expandieren, wobei ein hinterer Bereich vorgesehen ist, um in die Ratschen einzugreifen und damit ein Herausdrücken zu verhindern. Auf diese Weise können die Kolben ohne Schraubbewegung voran bewegt werden. Wenn man diese Technik verwendet, kann das Einführwerkzeug ebenso sicher den äußeren Käfig halten und dessen weitere Vorwärtsbewegung als Ergebnis einer Druck- oder Zugkraft auf die Kolbenelemente verhindern. Zusätzlich zur Modifikation des Kolbens kann der Käfig selbst für eine Einführung mit Einstoßen konfiguriert sein und kann in seinen Formen anders als ein Zylinder ausgebildet sein. - Käfige gemäß der vorliegenden Erfindung werden vorzugsweise aus einem biokompatiblen Material mit einer ausreichenden Steifigkeit ausgebildet, um den Lasten zu widerstehen, die auf sie bei einer gegebenen Anwendung wirken. Zusätzlich sollte bei einer bevorzugten Ausführung dieses Material eine ausreichende Flexibilität haben, um zumindest eine geringe Deformation bei dem Expansionsprozeß zu verkraften. Alternativ kann es für einige Vorrichtungen wünschenswert sein, Schwenkpunkte vorzusehen, anstatt dass das Material gewisse Deformationen erlaubt. Besonders bevorzugt ist das Material für Käfige gemäß der vorliegenden Erfindung eine medizinische Titan-Legierung. Jedoch können die Vorrichtungen auch aus rostfreiem Stahl, verschiedenen Arten von Kunststoffen, verschiedenen Zusammensetzungen einschließlich Vorrichtungen aus Kohlenstofffasern und Knochen bzw. Knochensubstituten gebildet werden.
- Die Erfindung wurde im Detail in Verbindung mit den Zeichnungen und der vorherigen Beschreibung dargestellt und beschrieben, wobei dieses als lediglich beispielhaft und nicht in den Eigenschaften beschränkend angesehen werden soll. Es ist ersichtlich, dass lediglich ein bevorzugtes Ausführungsbeispiel dargestellt und beschrieben worden ist, und dass alle Verbindungen und Modifikationen im Rahmen der anhängenden Ansprüche ebenso zu schützen sind.
Claims (17)
- Expandierbarer Fusionskäfig (
10 ) der zwischen ein Paar von Rückenwirbelkörpern (80 ,82 ) einsetzbar ist, mit: einem Körper (20 ) mit einer äußeren Knochenanlagefläche und einer inneren, eine interne Kammer (17 ) begrenzenden Innenfläche sowie mit einem Haltemechanismus, wobei dieser Körper (20 ) zumindest einen ersten Abschnitt (24 ,26 ) aufweist und dieser Abschnitt (24 ,26 ) einen fixierten Bereich, der mit einem ersten Ende des Körpers (20 ) verbunden ist, und einen expandierbaren Bereich (18 ) aufweist, der radial im Hinblick auf eine longitudinale Achse des Körpers (20 ) bewegbar ist, wobei der Körper (20 ) zumindest einen zweiten Abschnitt (40 ) aufweist, der mit dem ersten Ende des Körpers (20 ) verbunden ist, und mit einem Expansionsmechanismus (50 ) mit einer Größe, dass er zumindest teilweise in der internen Kammer (17 ) aufgenommen und an dem expandierbaren Bereich (18 ) in Anlage gebracht werden kann, wobei der Expansionsmechanismus (50 ) eine äußere, an dem Haltemechanismus anliegende Fläche aufweist, und wobei eine Bewegung des Expansionsmechanismus innerhalb der internen Kammer (17 ) den expandierbaren Bereich (18 ) des ersten Abschnittes (24 ,26 ) radial in Bezug zu der longitudinalen Achse des Körpers (20 ) verschiebt, wodurch der Körper (20 ) expandiert wird; dadurch gekennzeichnet, dass der Expansionsmechanismus den expandierbaren Bereich (18 ) des ersten Abschnittes (24 ,26 ) mit einer größeren Rate als den zweiten Abschnitt (40 ) verschiebt, wobei der Expansionsmechanismus durch Anlage an dem Haltemechanismus des Körpers (20 ) in Position gehalten wird. - Vorrichtung nach Anspruch 1, wobei die Knochenanlagefläche ein externes Gewinde (
12 ) ist. - Vorrichtung nach Anspruch 1, wobei der Haltemechanismus ein Gewindemuster (
160 ) auf der Innenfläche ist und der Expansionsmechanismus einen mit einem externen Gewinde versehen Bolzen (124 ) aufweist, der zum Eingriff in das Gewindemuster (160 ) ausgebildet ist. - Vorrichtung nach Anspruch 1, wobei der Haltemechanismus eine interne Schulter (
28 ) ist und der Expansionsmechanismus ein Teil (50 ) ist, der an der Schulter (28 ) anliegt. - Vorrichtung nach Anspruch 1, wobei der Körper einen dritten Abschnitt (
26 ) aufweist, der dem ersten Abschnitt (24 ) gegenüber liegt, wobei der dritte Abschnitt (26 ) einen mit dem ersten Ende des Körpers (20 ) verbundenen fixierten Bereich und einen expandierbaren Bereich (19 ) aufweist, wobei eine Bewegung des Expansionsmechanismus (50 ) innerhalb der internen Kammer (17 ) den ersten (24 ) und den dritten Bereich (26 ) auseinander schiebt. - Vorrichtung nach Anspruch 1, wobei der Körper (
20 ) ein fixiertes Ende (20 ) und ein expandierbares Ende (18 ) und ferner vier longitudinale Kanäle (14 ,22 ,15 ) aufweist, die sich zwischen der äußeren Fläche und der innern Fläche und von dem expandierbaren Ende (18 ) in Richtung auf das fixierte Ende (19 ) erstrecken, wobei dieses Kanäle (14 ,22 ,15 ) ein dem ersten (24 ) und zweiten (26 ) Abschnitt gegenüber liegendes Paar bilden, die durch ein gegenüberliegendes Paar eines dritten (40 ) und vierten (41 ) Abschnittes getrennt sind; wobei der Expansionsmechanismus die gegenüber liegenden ersten (24 ) und zweiten (26 ) Abschnitte mit einer im Wesentlichen größeren Rate als die gegenüber liegenden dritten (40 ) und vierten (41 ) Abschnitte expandiert. - Vorrichtung nach Anspruch 1, wobei der Haltemechanismus zumindest ein Flansch (
28 ) in Nachbarschaft zu der inneren Fläche aufweist und der Expansi onsmechanismus (50 ) eine mit dem Flansch (28 ) kooperierende Schulter (52 ) aufweist, um die Position des Expansionsmechanismus (50 ) zu halten. - Vorrichtung nach Anspruch 1, wobei der Haltemechanismus ein Gewindemuster (
12 ) ist und der Expansionsmechanismus (50 ) ein korrespondierendes externes Gewindemuster aufweist. - Vorrichtung nach Anspruch 1, die ferner Anregungsmittel zum Knochenwachstum aufweist, die in der internen Kammer (
17 ) angeordnet sind. - Vorrichtung nach Anspruch 1, wobei die interne Kammer (
17 ) eine mit einem Gewinde versehende geneigte Fläche (215 ,216 ) aufweist und diese Fläche (215 ,216 ) benachbart zu dem expandierbaren Abschnitt gelegen ist; und der Expansionsmechanismus eine äußere mit einem Gewinde versehene Fläche aufweist, die ausgebildet ist, um in die mit einem Gewinde versehene geneigte Fläche (215 ,216 ) einzugreifen, wobei der Expansionsmechanismus beim Schrauben längs der mit einem Gewinde versehenen geneigten Fläche (215 ,216 ) vorwärts bewegt wird, um den bewegbaren Bereich zum Expandieren des Körpers (210 ) zu verschieben, wobei der Expansionsmechanismus in Position durch Eingriff in die mit einem Gewinde versehene geneigte Fläche (215 ,216 ) gehalten wird. - Vorrichtung nach Anspruch 1, wobei der Körper (
20 ) ferner Öffnungen (16 ) zwischen der inneren Fläche und der äußeren Fläche zum Einwachsen von Knochenmaterial aufweist. - Vorrichtung nach Anspruch 1, wobei die interne Kammer (
17 ) zumindest 50 % des Volumens der Vorrichtung aufweist, so dass ein wesentlicher Anteil von Knochenwachstum anregendem Material in die interne Kammer (17 ) eingeführt werden kann. - Vorrichtung nach Anspruch 1, wobei der Expansionsmechanismus eine zentrale Öffnung (
56 ) aufweist, der eine Kommunikation mit der internen Kammer (17 ) ermöglicht. - Zwischenkörper- Fusionsvorrichtung mit: einem expandierbaren Fusionskäfig nach Anspruch 1, dadurch gekennzeichnet, dass die Vorrichtung ein Einführwerkzeug (
70 ) mit einer äußeren Hülse (72 ), die mit dem Körper (20 ) verbindbar ist, um Druckkräfte zu übermitteln, und einen inneren Schaft (75 ) aufweist, der beweglich in der äußeren Hülse (72 ) gelegen und ausgebildet ist, um in zumindest einen Bereich der inneren Kammer (70 ) eingeführt zu werden, wobei der innere Schaft (75 ) ein proximales Ende und distales Ende (77 ) aufweist, wobei das distale Ende (77 ) ausgebildet ist, um entfernbar in das Expansionselement (50 ) einzugreifen, wobei eine Bewegung des inneren Schaftes (75 ) in Bezug zu der äußeren Hülse (72 ) das Expansionselement (50 ) in Bezug zu dem Körper (20 ) bewegt. - Vorrichtung nach Anspruch 14, wobei der Expansionsmechanismus (
50 ) eine zentrale Öffnung (56 ) mit einem Gewindemuster und das distale Ende (77 ) des inneren Schaftes (75 ) ein korrespondierendes externes Gewindemuster (78 ) aufweist. - Vorrichtung nach Anspruch 14, wobei der Expansionsmechanismus (
124 ) eine zentrale hexagonale Öffnung (136 ) und das distale Ende des inneren Schaftes (75 ) eine korrespondierende externe hexagonale Form aufweist. - Vorrichtung nach Anspruch 14, wobei der Körper (
20 ) ferner eine Antriebsnut (46 ) und die äußere Hülse (72 ) einen korrespondierenden passenden Antriebsvorsprung aufweist.
