Description
- Dies ist eine medizinische Vorrichtung, die zur Behandlung embolischer Schlaganfälle verwendet wird. Insbesondere ist sie eine chirurgische Vorrichtung, die für gewöhnlich durch einen intravaskulären Katheter zugeführt wird. Sie kann auf verschiedene Weise verwendet werden. Sie kann zum Beispiel zum Öffnen eines freien Durchgangs neben einem Thrombus verwendet werden, damit sowohl Blut als auch Medikamente am Gerinnsel vorbei strömen können. Sie kann zum Durchstoßen und Beseitigen eines Thrombus verwendet werden. Diese Thromben finden sich häufig in gewundenen Blutgefäßen. Die Vorrichtung kann mehrere Abschnitte aufweisen: ein lang gestrecktes Kernelement, eine proximate Käfiganordnung, einen distalen Filter und ein Betätigungselement. Das Kernelement ist mindestens ein Kerndraht. Um das distale Ende des Kernelements ist eine in sich zusammenfallende, vorzugsweise aber selbst weitende oder selbst zusammenfallende proximale Käfiganordnung sowie ein distaler Filter, eine Käfiganordnung oder ein anderes Filterbauelement platziert, die weiter distal angeordnet und vorzugsweise selbst weitend oder selbst zusammenfallend ist. Diese Bauelemente sind vorzugsweise strahlenundurchlässig. Das proximale Ende des proximalen Käfigs ist typischerweise so an einem Betätigungselement angebracht, dass es eine Weitung des Käfigs nach der Platzierung erlaubt. Die proximale Käfiganordnung kann entweder mit oder ohne begleitende gerinnselauflösende medikamentöse Behandlung für das Aufnehmen von Emboli oder für deren Verlagern verwendet werden, um das Wiedereinsetzen eines Blutstroms zu ermöglichen. Der distale Abschnitt, sei es nun ein Käfig oder ein Fächer, wird distal des Thrombus platziert, um Teile des Thrombus aufzufangen, die sich während des Vorgangs der Behandlung oder Beseitigung lösen können.
- Diese chirurgische Vorrichtung ist so ausgelegt, dass sie im menschlichen Gefäßsystem befindliche Emboli verlagert oder durchdringt. Die Vorrichtung weist mehrere Hauptbestandteile auf, einschließlich einen proximalen (oder verdrängenden Käfig) und einen distalen Filter. Abhängig vom gewählten Vorgehen wird die Vorrichtung so eingeführt, dass der verdrängende oder proximale Käfig sich entweder hin zur venösen Seite des Gerinnsels oder neben dem Gerinnsel befindet. Der proximale Käfig wird geweitet, um entweder das Gerinnsel weg von der Arterienwand zu verlagern und ein Strömen von Fluid (z.B. Blut und Medikamente wie Antithrombolytika oder andere Lysemittel) hin zu der zuvor verschlossenen Stelle oder an dieser vorbei zu ermöglichen oder das Gerinnsel mit Hilfe des Käfigs als Kollektoranordnung zur Entfernung zu greifen, sich nach Passieren des Zielembolus zu weiten und die Embolie (oder einen Teil der Embolie) zur Entfernung aus den Blutgefäßen des Patienten aufzufangen oder einzunetzen. In manchen Situationen kann die Vorrichtung eingesetzt werden, um das Gerinnsel zu einer anderen Position im Gefäßsystem zu bewegen; etwa für die Beseitigung mit Hilfe einer anderen Vorrichtung oder für die Kanalisierung des Gerinnsels für eine verbesserte Blutströmung. Das distale Filterelement wird immer distal des Thrombus platziert, um alle Teile aufzufangen, die sich möglicherweise loslösen.
- Es sind andere Vorrichtungen zur Behandlung von Gefäßembolien bekannt. Die Verwendung von entfaltbaren Ballons zur Beseitigung von Embolien wird seit vielen Jahren praktiziert. Der „Fogarty-Katheter wird – üblicherweise im äußeren Körperbereich – verwendet, um Gerinnsel aus in Beinen und Armen befindlichen Arterien zu entfernen. Diese bekannten Vorrichtungen werden ziemlich ausführlich in dem U.S. Patent Nr. 3,435,826 für Fogarty und in den U.S. Patenten Nr. 4,403,612 und 3,367,101 beschrieben. Diese Patente beschreiben einen Ballonkatheter, bei dem ein Ballonmaterial bei Zusammenfallen in Längsrichtung gedehnt wird.
- Gelegentlich bilden sich Emboli um die Herzklappen aus und werden dann abgelöst und folgen dem Blutstrom in die distalen Bereiche des Körpers. Sie sind besonders gefährlich, wenn die Emboli das Gehirn erreichen und einen embolischen Schlaganfall auslösen. Wie nachstehend erläutert wird, setzen sich viele solche Verschlüsse in der mittleren zerebralen Arterie (MCA) fest, wenngleich dies nicht die einzige Stelle ist, an der sich Emboli festsetzen. Wenn der Blutstrom zu einem Teil des Gehirns behindert oder vollständig abgeschnitten wird, ist die Sauerstoffversorgung des Gehirns beschränkt, was schwere gesundheitliche Probleme verursacht.
- Bei Verfahren zur Entfernung von Emboli mit Hilfe des Fogarty-Katheters oder anderen ähnlichen Kathetern ist es üblich, zunächst das Gerinnsel mit Hilfe von Durchleuchtung zu orten. Der Embolektomie-Katheter wird dann eingeführt und zum Gerinnsel geleitet. Die distale Spitze des Ballonkatheters wird dann vorsichtig durch die Mitte des Gerinnsels bewegt. Sobald der Ballon durch die distale Seite des Gerinnsels getreten ist, wird der Ballon entfaltet. Der Ballonkatheter wird dann allmählich und sacht zurückgezogen. Auf diese Weise dient der Ballon zum Ziehen des Gerinnsels vor dem Ballon. Die meisten Vorgehen unter Verwendung eines Fogart-Katheters wiederholen diese Schritte, bis das betreffende Gefäß frei von Gerinnselmaterial ist.
- Solche Gefäßokklusionen treten an vielen verschiedenen Stellen im Körper auf. Das Festsetzen des Thrombus an verschiedenen Stellen wird häufig durch das Vorhandensein von Atherosklerose kompliziert. Diese Erkrankung lässt die Gefäße gewunden und verengt werden. Diese Anomalien werden oft als Ergebnis des Wachstums atherosklerotischer Plaques gesehen. In diesen erkrankten Gefäßen auftretende Gerinnsel sind unter Verwendung von eines Ballon- oder Fogarty-Katheters schwer zu entfernen.
- Das Entfernen von Embolie mit Hilfe von Ballonkathetern steckt voller möglicher Probleme. Ein solches Problem ergibt sich während der Entfernung eines Gerinnsels. Der Widerstand gegenüber einem solchen Entfernen bewirkt häufig, dass der Ballonteil des Katheters sich über die Spitze des Katheters stülpt. Wenn der Bediener dann den Ballon während eines solchen Zusammenfaltens teilweise zusammenfaltet, kann die distale Spitze des Ballons ausgedehnt und abgewinkelt werden. Eine weitere Schwierigkeit bei Ballonkathetern ist die Möglichkeit der Verletzung der Intima von Arterien. Aufblasdrücke können Kräfte erzeugen, die so groß sind, dass diese Gefäßschicht erreicht wird oder an einer solchen Wand festsitzendes Plaque abgelöst wird. Im schlimmsten Fall kann der Ballon reißen, wobei Ballonteile im Blutstrom zurückbleiben.
- Die Bewegung eines Ballons in der MCA bei alleiniger Verwendung eines Ballons kann das Gerinnsel durch proximalere Äste in andere große Gefäße wie die Arteria carotis interna (ICA) und dann in andere Gefäße verlagern und kann das Gerinnsel in sich verzweigende Gefäße drücken.
- Es gibt eine Vielzahl unterschiedlicher Vorrichtungen, die zur Verwendung an Stelle von Ballonkathetern und beim Einsatz einer anderen Vorrichtung als einem Ballonkatheter bei Entfernung der Emboli gedacht sind.
- Eine solche Vorrichtung wird in U.S. Patent Nr. 4,030,503 für Clark III gezeigt. Dieses Patent beschreibt eine an dem distalen Ende eines Katheters befestigte gewundene Spirale. Die gewundene Spirale ist insbesondere dafür ausgelegt, gedreht und durch das Gerinnsel nach vorne geschoben zu werden. Es heißt, dass sich die Spirale in das Gerinnsel schraubt, und dass, wenn sie fest in dem Gerinnsel eingebettet ist oder dieses passiert hat, der Katheter ohne Drehung aus dem Gefäß gezogen wird. Der Katheter funktioniert, wie es dort heißt, wie ein Korkenzieher.
- Ein ähnlicher Katheter wird in dem U.S. Patent Nr. 4,706,671 für Weinrib beschrieben. Auch dieser Katheter weist einen Spiralabschnitt an seinem distalen Ende auf. Der Spiralabschnitt wird, wie es heißt, zunächst in eine im Allgemeinen geradlinige Einführposition für das Bewegen in einem Gefäß gedehnt. Dann wird das Spiralelement zur Form eines hohlen konischen Schöpflöffels geweitet, um dann das Gerinnselmaterial aus dem Blutgefäß zu schöpfen. Das Spiralelement wird durch einen inneren Draht versteift, der dann entfernt wird. Der hohle Durchgang wird dann mit einer Flüssigkeit gefüllt, um die Spiralen zu versteifen. Die Spiralen sind, wie es heißt, aus einem elastomeren Material.
- U.S. Patent Nr. 4,762,130 für Fogarty et al. beschreibt einen spiralförmigen Ballon, der an dem distalen Ende eines Katheters angebracht ist. Der spiral- oder blasebalgförmige Ballon wird in einem im Allgemeinen geradlinigen Zustand gehalten und in ein Gerinnsel geleitet. Sobald der Katheterballon in dem Gerinnsel entfaltet ist, werden der Ballon and das angrenzende Gerinnsel zusammen entfernt.
- Eine andere ähnliche Vorrichtung, die eher zum Greifen und Scheren von atherosklerotischen Ablagerungen als zur Entfernung von Thromben verwendet wird, wird in U.S. Patent Nr. 4,890,611 für Montfort et al. beschrieben. Diese Vorrichtung enthält ein Paar spiralförmiger Drähte, die an dem distalen Ende eines Drahts platziert sind. Der biegsame Draht wird gegen einen biegsamen Katheter gezogen und die beiden spiralförmig ausgelegten Schlaufen weiten sich, um eine Schervorrichtung zu bilden. Die Gesamtheit der Vorrichtung wird dann mittels eines Griffs gedreht, so dass das Paar spiralförmig gewundener Schlaufen durchschneidet, und dann hält sie, wie es heißt, zur Entfernung aus dem Gefäß Abschnitte mit Plaque fest.
- Ein anderes Thrombusextraktionssystem wird in U.S. Patent Nr. 5,011,488 für Ginsberg beschrieben. Beim Einsatz dieser Vorrichtung wird ein entfaltbarer Ballon mit einer proximalen Kegelform zusammengefaltet und durch einen Thrombus geführt. Dann wird er geweitet und zurückgezogen, so dass der proximale Durchgang den Thrombus in Kontakt mit einer Saugvorrichtung zieht. Die Saugvorrichtung entfernt dann das Gerinnsel oder das thrombotische Material aus dem Gefäß.
- In dem gerade erwähnten Patent für Ginsberg wird auch eine alternative Konfiguration des weitbaren Elements beschrieben. Bei dieser Abwandlung wird eine Drahtspirale an einem Verlängerungsdraht angebracht, die zwischen einer ausgefahrenen und einer eingefahrenen Position bewegt werden kann. Die eingefahrene bzw. ausgefahrene Konfiguration wird eine Kegelform aufweisend gezeigt. Der Kegel wird als Kegel mit einem proximal kleineren Ende gezeigt.
- U.S. Patent Nr. 5,112,347 für Taheri zeigt einen entfaltbaren ballonartigen Embolektomie-Katheter. Der Ballon weist eine Reihe von Fingern auf, die in einer blattförmigen Anordnung im Inneren des Ballons angeordnet sind. Der Ballon wird hydraulisch entfaltet und bildet nach dem Entfalten einen Kegel. Die zusammengefaltete Vorrichtung wird in den
11 bis14 gezeigt, wie sie vor dem Entfalten distal an der Embolie vorbei bewegt wird. Nach dem Entfalten nimmt das große Ende des Ballons die Embolie auf, wenn sie an der geeigneten Stelle im Gefäß vorbei gezogen wird. - U.S. Patent Nr. 5,192,286 für Phan zeigt einen Entnahme-Katheter für das Entfernen von Materialien aus verschiedenen Körperlumen. Der Entnahme-Katheter wird ein schlaffes Netz aufweisend gezeigt, das für das Durchtreten in ein Lumen an dem aufzunehmenden Material vorbei zusammenfallen kann. Das Netz wird entfaltet und Materialien wie Harnleitersteine werden entfernt.
- U.S. Patent Nr. 5,411,509 für Hilal zeigt einen Embolektomie-Katheter mit einer distal angebrachten Spitze aus Elastomerschaum. Die Schaumspitze weist ein Betätigungsmittel auf, das als Reaktion auf das Betätigen des Betätigungsmittels für das Ausbilden des Schaumabschnitts sowohl in Längsrichtung als auch radial geeignet ist. In der Praxis wird die Katheterspitze an der Embolie vorbei gedrückt, entfaltet und zurückgezogen, wobei das Gerinnsel bei Erfolgen des Zurückziehens proximal geschoben wird.
- Das U.S. Patent Nr. 5,490,859 für Mische et al. zeigt eine Vorrichtung zur Entfernung von intravaskulärem Okklusionsmaterial mit einem weitbaren Element zur Materialentfernung, das aus einer Reihe von Drähten, die zwischen den beiden Enden dieses Elements verlaufen, einem Katheterschaft, einem Antriebsschaft für das Drehen des Materialbewegungselements in dem Blutgefäß und einem an dem Materialentfernungselement platzierten Aufnahmeteil für das Aufnehmen von Okklusionsmaterial, das durch das weitbare Materialentfernungselement entfernt wird, besteht. Die Antriebswelle kann von einem mit dem proximalen Ende der Antriebswelle verbundenen Motor betrieben werden.
- Keine dieser Patentschriften zeigt die nachstehend beschriebene Vorrichtung.
- GB-A-2 020 557 offenbart eine Vorrichtung für die Entfernung von Fremdkörpern aus physiologischen Kanälen, wie Harnleitern, Arterien, Bronchien und dergleichen, wobei die Vorrichtung dem Oberbegriff von Anspruch 1 entspricht.
- Gemäß der vorliegenden Erfindung wird eine Vorrichtung zur Emboliebehandlung, welche ein Kernelement, eine proximale Käfiganordnung, ein distales Kollektorelement und ein Betätigungselement umfasst, an die Hand gegeben:
- a. wobei das Kernelement mindestens einen lang gestreckten Kerndraht umfasst, der ein distales Ende sowie ein proximales Ende aufweist und durch das distale Kollektorelement und die proximale Käfiganordnung verläuft,
- b. wobei die proximale Käfiganordnung für das Sammeln oder Verlagern der Emboli dient und ein distales Ende sowie ein proximales Ende aufweist, die im wesentlichen koaxial zu dem Kernelement sind, wobei die proximale Käfiganordnung eine erste Platzierungsform und eine zweite geweitete Form aufweist, wobei sich die zweite geweitete Form von der ersten Platzierungsform unterscheidet,
- c. wobei das distale Element für das Aufnehmen von Emboli dient und ein distales Ende sowie ein proximales Ende aufweist und von der proximalen Käfiganordnung beabstandet und zu dieser distal ist und eine erste Platzierungsform und eine zweite geweitete Form aufweist, wobei sich die zweite geweitete Form von der ersten Platzierungsform unterscheidet, und
- d. dadurch gekennzeichnet, dass die proximale Käfiganordnung und das distale Kollektorelement jeweils in der jeweiligen ersten Platzierungsform sind, wenn sich das Betätigungselement in einer ersten Position befindet, und die proximale Käfiganordnung und das distale Kollektorelement jeweils in der jeweiligen zweiten geweiteten Form sind, wenn sich das Betätigungselement in einer zweiten Position befindet, und dass sich in der ersten und in der zweiten Position die proximale Käfiganordnung distal des Betätigungselement befindet.
- Die nachstehend beschriebenen und dargestellten bevorzugten Ausführungen der Vorrichtung werden für gewöhnlich durch einen intravaskulären Katheter zugeführt und sind unterschiedlich ausgelegt, um Emboli zu verlagern oder zu durchstoßen und zu entfernen, insbesondere wenn sie sich in gewundenen Gefäßen befinden.
- Die bevorzugten Ausführungen der Vorrichtung umfassen mehrere Abschnitte. Zunächst haben die Vorrichtungen ein Kernelement. Das Kernelement weist vorzugsweise mindestens mehrere Hauptfunktionen auf: erstens kann der distale Teil als Führungsdraht für die Navigation durch Gefäße zur Behandlungsstelle dienen; zweitens wirkt der distale Teil beim Durchstechen des Gerinnsels oder dessen Verlagern weg von der Arterienwand mit; und drittens arbeitet er während der Platzierung der Anordnung mit der Aufnahmeanordnung zusammen. Das Kernelement kann ein einfacher Kerndraht (eventuell mit einem anheftenden Geflecht verstärkt), der an dem distalen Ende der Käfiganordnung fest angebracht wird, oder ein rohrförmiges Element sein, das an der Aufnahmeanordnung angebracht wird, wobei es in seinem Innendurchgang einen beweglichen Kern oder Führungsdraht aufweist. Vorzugsweise kann das Kernelement bezüglich der Aufnahmeanordnung frei drehen, um dessen Eigenschaften als Führungsdraht zu wahren, während die Form und Unversehrtheit der Käfiganordnung gewahrt wird.
- An dem Kernelement sind distal ein Paar Käfig- oder Filterelemente platziert. Das proximale Element ist vorzugsweise eine weitbare und in sich zusammenfallbare Käfiganordnung. Das proximale Element kann selbst weitend oder selbst in sich zusammenfallend sein. Das distale Element kann auch ein Käfig und eine Konstruktion ähnlich dem proximalen Element sein oder von anderer Filterkonstruktion sein. Das distale Element soll einen wandernden Thrombus oder Gerinnselfragmente auffangen. Beide Elemente können entweder fest oder drehbar mit dem Kerndraht verbunden werden. Von den Konstruktionen für die Käfiganordnung umfassen die bevozugtesten ein superelastisches Legierungsband oder Drahtgeflecht, das mit einem Platindraht umwickelt oder anderweitig strahlenundurchlässig gemacht wird.
- Die Anordnung umfasst weiterhin ein Betätigungselement, das das Weiten oder Zusammenziehen der Kollektoranordnung nach der Platzierung zulässt oder bewirkt. Wenn das Betätigungselement mechanisch ist, wird es gewöhnlich an dem proximalen Ende der proximalen Käfiganordnung angebracht, um eine steuerbare Weitung der distalen und proximalen Käfiganordnungen zu ermöglichen bzw. zu bewirken.

6A ,6B ,6C und6D zeigen eine verallgemeinerte Folge von Schritten zur Verwendung der erfindungsgemäßen Vorrichtung für das Verlagern eines Thrombus für die Wiederherstellung von Strömen an der okkludierten Stelle vorbei. - Diese Vorrichtung ist ein chirurgisches Gerät. Sie wird unterschiedlich ausgelegt, um einen Thrombus zur Seite zu bewegen und einen Fluiddurchlass entlang dieses Thrombus vorzusehen oder um Emboli zu durchdringen und zumindest teilweise zu entfernen, die sich im menschlichen Gefäßsystem befinden. Sie soll schnell als Strömungswiederherstellungsvorrichtung verwendet werden. Selbst in den Fällen, da die Embolie nicht vollständig entfernt wird bzw. nicht vollständig entfernt werden kann, hält man diese erfindungsgemäße Vorrichtung für brauchbar beim Verlagern, Entnehmen oder Entfernen eines Teils des Gerinnsels und dadurch beim Ermöglichen einer Widerherstellung einer teilweisen Blutströmung. Sie kann zur Bewegung des Gerinnsels zu einer günstigeren Stelle im Körper verwendet werden.
- Sie ist von geeigneter Biegsamkeit, um in dem distalen gewundenen Gefäßsystem des Gehirns platziert zu werden, und ist somit brauchbar beim Behandeln von dort befindlichen verstopfenden Emboli. Diese Vorrichtung kann bei zusätzlicher Verwendung von Thrombolytika zum Beispiel als Teilbehandlung für embolischen Schlaganfall betrachtet werden.
- Im Rahmen praktischer Erfahrung ist ein Großteil der in das Gehirn eingeschwemmten Emboli selten länger als etwa 1 bis 2 Zentimeter und setzt sich aus einer Vielzahl von sowohl physiologischen als auch hämodynamischen Gründen in der mittleren zerebralen Arterie (MCA) ab. Wie in
1 gezeigt wird, verläuft die Strecke vom Ursprungsort eines wandernden Embolus – häufig das Herz – über die Arteria carotis communis an dem Abzweigungspunkt vorbei, der die Arteria carotis externa und Arteria carotis interna bildet, in die Arteria carotis interna (ICA) zur MCA. Die MCA gilt im Allgemeinen nach dem Siphon und nach den Verzweigungsstellen einer Vielzahl anderer Arterien, z.B. der Arteria ophthalmica, der Arteria communicans anterior, der Arteria communicans posterior und anderen, als die Fortsetzung und das Ende der ICA. Die Äthiologie einer solchen Okklusion ist verschiedenartig, uneinheitlich und kompliziert. Die Okklusion (100 ) wird in der MCA in1 an der genannten Stelle gezeigt. - Behandlungen für solche embolische Verschlüsse umfassen die Katheterisierung des Patienten und das Einbringen von Gewebe-Plasminogenaktivator (TPA) oder Urokinase oder anderen Gerinnsel lösenden Mitteln an der Stelle der Okklusion. Zusätzlich kann der embolische Verschluss durchdrungen werden – häufig mit einem Mikrokatheter – und der TPA oder die Urokinase kann distal der Okklusion eingebracht werden. Das Entfernen des Katheters erzeugt einen bescheidenen Durchlass für wiederaufgenommene oder vermehrte Blutströmung an der vorher teilweisen Okklusion vorbei.
- Diese erfindungsgemäße Vorrichtung dient für die schnelle Wiederherstellung der Fluid- oder Blutströmung an der Okklusion vorbei. Wir haben festgestellt, dass sie in verschiedenerlei Weise eingesetzt werden kann. Zum Beispiel kann die Vorrichtung bei Annäherung an das Gerinnsel von dem arteriellen Ende her entweder zum Umgehen des Gerinnsels entlang der Gefäßwand und bei Weiten der proximalen Käfiganordnung zum Ermöglichen einer Blutströmung quer über die Okklusion unter Nutzung des Inneren der proximalen Käfiganordnung als Fluiddurchlass verwendet werden.
- Alternativ kann die kann die erfindungsgemäße Vorrichtung zur Kanalisierung oder Entfernung einer Okklusion über die Schritte des Durchdringens der Okklusion, des Weitens einer Käfiganordnung distal der Embolie und vorzugsweise des Entfernens mindestens eines Teils der Embolie zusammen mit dem Käfig und dessen begleitenden Katheter verwendet werden.
- Das distale Kollektor- oder Filterelement dient zum Auffangen und Zurückhalten von Thrombusfragmenten und zur Verhinderung, dass diese sich distal bewegen.

2A ist eine schaubildliche Darstellung der Hauptteile einer erwünschten Abwandlung (200 ) der Vorrichtung und bietet ein allgemeines Übereinkommen für mehrere der hierin verwendeten Begriffe. - Die Anordnung umfasst gewöhnlich ein Kernelement, häufig einen Kerndraht (
202 ) und eine optionale Spiralspitze (204 ), um der distalen Spitze ein Maß an Strahlenundurchlässigkeit zu verleihen und vor Verletzung der Intima zu schützen, eine distale Kollektoranordnung, einen distalen Kollektorfilter oder ein distales Kollektorelement (206 ), das als ein Emboliekollektor verwendet werden kann, sowie eine proximate Käfiganordnung bzw. ein proximales Käfigelement (208 ). Aufgrund der „Feinheit" der Vorrichtung kann die Spiralspitze (204 ) auch die Funktion eines Führungsdraht erfüllen. Die Länge der Spiralspitze (204 ) kann in solchen Fällen bis zu 4 oder 5 Zentimeter betragen. Ferner und wie nachstehend erläutert kann das Kernelement eine Anordnung sein, die ein rohrförmiges Element umfasst, das weiterhin einen entfernbaren Kern oder Führungsdraht enthält. In einer höchst wünschenswerten Abwandlung der Erfindung wird ein Trennelement (210 ) zum Trennen des distalen Kollektors oder Elements (206 ) von dem proximalen Käfig (208 ) verwendet. Wie nachstehend eingehender erläutert wird, ist eine bevorzugte Abwandlung der Erfindung eine Abwandlung, bei der der distale Kollektor (206 ) und der proximale Käfig (208 ) aus einem einzelnen rohrförmigen Element bestehen, das unterschiedlich aus Draht oder Band geflochten ist. Der Kollektor und der Käfig werden durch ein Trennelement (210 ) getrennt, das wie in teilweisem Querschnitt in2A ersichtlich ein einfaches rohrförmiges Element ist, das z.B. aus einem geeigneten Metall oder Polymer besteht. - Die distale Kollektoranordnung (
206 ) und die proximale Käfiganordnung (208 ) sind für das Passieren entlang oder für das Durchdringen und Passieren durch die Embolie in sich zusammenfallend und sind im Allgemeinen selbst weitend, sobald sie ihre Position erreicht haben. Es können offensichtlich andere Komponenten und Vorrichtungen verwendet werden, um die distale Kollektoranordnung (206 ) und die proximale Käfiganordnung (208 ) zu weiten, und diese werden nachstehend ziemlich ausführlich erläutert. - Die distale Kollektoranordnung (
206 ) und die proximale Käfiganordnung können sich entweder selbst weiten, wenn eine Bewegung des Betätigungselements (212 ) von seiner Einführposition zu seiner Platzierungsposition zugelassen wird, oder sie können selbst in sich zusammenfallen. Ein Selbstzusammenfallen kann ein Maß an Sicherheit bieten, falls die Vorrichtung betriebsunfähig wird. Das Zurückziehen einer in sich zusammengefallenen Vorrichtung ist einfacher als das Entfernen einer geweiteten Vorrichtung. Das Betätigungselement (212 ) ist von einer Person bedienbar. -
2B zeigt die Form der in der2B gezeigten Abwandlung in der Form, in der sie eingebracht wird. Das (nicht dargestellte) Betätigungselement (212 ) wurde proximal gegen den (in2B nicht dargestellten) Kerndraht (202 ) gezogen. Wie nachstehend gezeigt wird, durchläuft diese schlanke oder erste Konfiguration einen geeignet bemessenen Zufuhrkatheter und kann vor Weitung zu der in2A gezeigten platzierten oder zweiten Konfiguration an einem Gerinnsel vorbei oder durch dieses hindurch geschoben werden. - Die distale Kollektoranordnung (
206 ) und die proximale Käfiganordnung (208 ) sind vorzugsweise aus einem Draht oder Band aus superelastischer Legierung gebildet. - Manche Edelstähle sind geeignet, doch die problemlose Verfügbarkeit von Nickel-Titan-Legierungen in einer Vielzahl an Formen und Größen macht diese Entscheidung einfach. Wir haben insbesondere festgestellt, dass Bänder mit einer Dicke von nur 0,019 mm (0,75 mil) und einer Breite von nur 0,051 mm (2 mil) für diese Vorrichtung geeignet sind. Dickere und breitere Bänder sind in einigen Fällen nach Erfordernis der Situation ebenfalls geeignet. Bevorzugte Bänder haben eine Dicke von 0,019 bis 0,038 mm (0,75 bis 1,5 mil) und eine Breite von 0,076 bis 0,178 mm (3 bis 7 mil). Am bevorzugtesten sind 0,02 bzw. 0,1 mm (0,8 mil bzw. 4 mil).
- „Drähte" sollen faserartige Formen mit Querschnitten einschließen, bei denen das Verhältnis Dicke/Breite größer als 0,5 ist und 1,0 einschließt. Die Querschnittform kann kreisförmig, quadratisch, oval, etc. sein.
- Zumindest eine Minderheit der Bänder kann aus einem geeigneten Edelstahl (z.B. 304SS, 306SS etc.) oder einer anderen solchen Legierung bestehen. Eine wünschenswerte Variante ist der Austausch eines oder mehrere Bänder des Geflechts (oder das Hinzufügen eines oder mehrerer Bänder) mit einem strahlenundurchlässigen Material wie Platin. Dies erlaubt offensichtlich dem Bediener während eines Eingriffs die Visualisierung der Position der distalen Kollektoranordnung (
206 ) und der proximalen Käfiganordnung (208 ). - Eine technische Grundlage für den Begriff „superelastisch" findet sich in der Klasse der Nickel-Titan-Legierungen, die als „Nitinol" bekannt sind – Legierungen, die vom United States Naval Ordnance Laboratory entdeckt wurden. Diese Materialien werden ausführlich in den U.S. Patenten Nr. 3,174,851 für Buehler et al., 3,351,463 für Roszner et al. und 3,753,700 für Harrison et al. beschrieben. Besonders für diese Erfindung geeignete Legierungen sind solche, die mindestens 1,5 Gew.-% und bis zu etwa 8 Gew.-% oder mehr eines oder mehrerer Legierungselemente gewählt aus der Gruppe bestehend aus Vanadium, Chrom, Mangan, Eisen und Cobalt enthalten. Der Begriff „Band" soll lang gestreckte Formen einschließen, deren Querschnitt nicht quadratisch oder rund ist und für gewöhnlich rechteckig, oval oder halboval sein kann. Sie sollten ein Seitenverhältnis von 0,5 (Breite/Dicke) oder weniger aufweisen.
- Der distale Kollektor (
206 ) und der proximale Käfig (208 ) können durch Wickeln eines Bands oder eines Drahts auf einen Dorn mit einer Außenform zur Erzeugung der in2A vorgefundenen entspannten Form hergestellt werden. Der Dorn und das gewickelte Band werden vorzugsweise eine geeignete Zeitdauer lang (z.B. bei 399°C bis 649°C (750°F bis 1.200°F) eine halbe bis zu zwei Stunden lang) erhitzt, um eine Anordnung zu erzeugen, die die dargestellte Form hat. Zur Verwendung in der MCA muss der Außendurchmesser der Vorrichtung nach Platzierung nicht größer als etwa 3 mm sein. Es ist wünschenswert, dass die in sich zusammengefallene oder nicht platzierte Vorrichtung einen Durchmesser von nicht mehr als etwa 1 mm hat. Das Weitungsverhältnis zwischen den hierin besprochenen platzierten und platzierbaren Vorrichtungen muss typischerweise nicht größer als etwa 3:1 sein, um in der MCA wirksam zu sein. Weitungsverhältnisse zwischen 2:1 und 10:1 sind in dieser Erfindung angebracht. Diese Verhältnisse sind für jede Abwandlung der Erfindung geeignet. Die Erfindung ist aber nicht hierauf beschränkt. - Bei manchen Konstruktionen der Vorrichtung ist es zulässig, statt einer reinen Hemmung der vorzeitigen Weitung der Käfiganordnung ein Betätigungselement als aktives Platzierungselement zu verwenden.
- Die
3A und3B zeigen einen Querschnitt einer anderen Abwandlung der erfindungsgemäßen Vorrichtung, die mit der in den2A und2B vorgefundenen Abwandlung verwandt ist, aber eine etwas kompliziertere distale Kollektor- oder Filteranordnung einsetzt. - Die
3A und3B zeigen wie oben jeweils einen teilweisen Querschnitt einer weiteren Abwandlung (220 ) der dargestellten Erfindung, wobei eine Vorrichtung einen mittleren Kerndraht (222 ) und eine proximale Käfiganordnung (224 ) aufweist. Die proximale Käfiganordnung (224 ) wird als gewebtes Geflecht aus Bändern gezeigt, vorzugsweise aus einer superelastischen Legierung, kann aber wie bei Abwandlungen dieser Anmeldung aus Drähten entsprechend der Festlegung dieses Begriffs hergestellt werden. In3A ist ersichtlich, dass die Form der proximalen Käfiganordnung (224 ) etwas säulenförmiger ist als die in2A gezeigte. Diese Form ist aufgrund ihrer einfachen Herstellung bevorzugt. Sie kann so erzeugt werden, dass sie für den Einsatz bei langen Emboli von signifikanter Länge ist. Ein rohrförmiges Betätigungselement (226 ) wird in dieser Abwandlung ebenfalls gezeigt, ebenso wie ein Trennelement (228 ). Ein Hauptunterschied zu der in den2A und2B gezeigten Abwandlung (200 ) ist der Aufbau der distalen Filteranordnung (230 ). Wenngleich der Aufbau der distalen Filteranordnung (230 ) für die Konzepte dieser Erfindung nicht ausschlaggebend ist, da sie von jeder gewünschten Form sein kann, die kleine Stücke der Emboli auffangen kann, die sich von dem Embolus lösen, gegen den eine proximale Käfiganordnung (224 ) drückt, ist die dargestellte distale Filteranordnung (230 ) beispielhaft für die Form der Anordnungen, die für die Anbringung an diesem Abschnitt der Gesamtanordnung geeignet sind. In dieser Abwandlung sind mehrere blütenblattförmige Drähte (232 ) ersichtlich, die sich bei Platzierung überlappen können oder auch nicht. Optionale Abstandsdrähte (234 ) sind als ein tragendes Anordnungsspitzenelement (236 ) ersichtlich, um dem distalen Ende des Kerndrahts (238 ) ein Maß an radialer Platzierung zu bieten. - Die
3A und3B zeigen die distale Anordnung in ihrer platzierten Konfiguration. Aufgrund des distalen Rechens der Filterelemente (232 ) können diese Filterelemente (232 ) mühelos in ein Katheterlumen eingezogen werden, wenn man diese Vorrichtung aus dem menschlichen Körper entfernen will. - Die
4A ,4B und4C zeigen verschiedene distale Behandlungen der Gesamtvorrichtung. Wie vorstehend erwähnt, kann der Kerndraht in den Kollektorkäfigelementen befestigt werden, er kann mit beschränkter Bewegung axial in den Kollektorkäfigelementen versehen werden oder er kann mit voller axialer Bewegung versehen werden, in welchem Fall der Kerndraht als Führungsdraht für die Vorrichtung wirkt und verwendet werden kann. -
4A zeigt eine sehr einfache Abwandlung der Erfindung, wobei ein Kerndraht (242 ) fest an einer distalen Spirale (244 ) angebracht ist. Die distale Spirale (244 ) dient auch als Verankerungsmittel für die distale Kollektoranordnung (246 ). Diese Abwandlung der Erfindung ist einfach und kostengünstig und ist insbesondere gut brauchbar, wenn die Vorrichtung nicht zu weit in das gewundene Gefäßsystem hinein reichen muss. -
4B zeigt eine andere Abwandlung der Erfindung, ebenfalls mit einem distalen Käfig (248 ) mit gewebtem Geflechtband. Im Gegensatz zu der in4A gezeigten Abwandlung gebraucht diese Abwandlung einen Kerndraht (250 ), der axial durch die Mitte des distalen Käfigs (248 ) frei beweglich ist. Der distale Käfig (248 ) weist eine rohrförmige Auskleidung (252 ) auf, die sich unabhängig vom Betätigungselement (in den2A und3A oben gezeigt) bewegt. Der Kerndraht (250 ) dient nicht als Führungsdraht und kann zum Vorsehen einer Bahn durch ein gewundenes Gefäßsystem verwendet werden, dem die Vorrichtung folgt. Die Auskleidung (252 ) besteht vorzugsweise aus einem Material wie Polytetrafluorethylen (z.B. Teflon) und erstreckt sich wünschenswerterweise die gesamte Strecke zum proximalen Ende des Katheters und dient als Betätigungselement für die Käfige. Eine wünschenswerte Abwandlung dieser Erfindung umfasst das Befestigen der distalen Käfiganordnung (248 ) durch Verwendung eines rohrförmigen Elements aus z.B. einem Thermoplast, das das distalste Ende der distalen Käfiganordnung (248 ) an der Innenauskleidung (252 ) befestigt. Ferner wird eine strahlenundurchlässige Marke (256 ) (die in dieser Abwandlung aus nachstehend erläuterten Gründen optional ist) dargestellt. -
4C zeigt eine weitere Abwandlung (258 ) dieser Erfindung, bei der der Kerndraht (260 ) bezüglich des distalen Käfigs (262 ) drehbar ist, aber darin eine beschränkte axiale Bewegung aufweist. Dies erlaubt dem Bediener offensichtlich, die gebogene Spiralspitze (244 ) in eine Position zu bringen, um eine Wahl oder Unterwahl bei Abbiegungen oder Gabelungen in dem gewundenen Gefäßsystem zu treffen, ohne die gesamte Vorrichtung (258 ) zu verdrehen. Die Beschränkung der Bewegung des Kerndrahts (260 ) in der Vorrichtung (258 ) wird über die Verwendung einer Hülse (264 ) verwirklicht, die distal in der distalen Käfiganordnung (262 ) angeordnet ist. Der Innendurchmesser der Hülse (264 ) lässt nicht zu, dass die Spirale (244 ) proximal passiert, und lässt auch nicht zu, dass die Spirale (266 ) distal passiert. Die Spirale (266 ) kann bei Bedarf als Markierungspirale verwendet werden. Die Hülse (264 ) bildet ferner wünschenswerterweise eine Verbindung mit dem äußeren rohrförmigen Element (268 ), um das distalste Ende der distalen Käfiganordnung (262 ) fest anzubringen. - Ein in
4C gezeigtes unabhängiges Merkmal ist das Vorhandensein eines Flechtelements (268 ), das an dem Kerndraht (260 ) anhaftet. Das Geflechtelement (268 ) dient dem speziellen Zweck, Knickfestigkeit und verbessertes Drehmomentverhalten zu bieten. Das als Geflecht an dem Kerndraht (260 ) verwendete Material kann ein beliebiges Material aus eine Reihe verschiedener Materialien sein. Höchst wünschenswert ist aber die Verwendung eines Geflechts aus einer superelastischen Legierung. Bestimmte Klassen dieser Materialien wurden vorstehend eingehend beschrieben. Das Geflechtmaterial (268 ) kann ein Edelstahldraht oder ein Polymerfilament sein. Bei manchen Abwandlungen können als flüssigkristalline Copolyester (liquid crystal polymers = LCP) bekannte Materialien benutzt werden, weil sie einfach eine extrem hohe Zugfestigkeit haben und sich nicht leicht dehnen. Dieses Geflecht ermöglicht es, dass der Gesamtdurchmesser der Kerndrahtanordnung – der Kerndraht (260 ) und das Geflecht (268 ) – kleiner als ein Kerndraht mit der erkennbar gleichen Leistung ist. Diese komplexeren Konstruktionen können in Situationen brauchbar sein, in denen ein extrem kleines Gefäß durchdrungen werden muss, um den wandernden Thrombus zu behandeln. - Wie an anderer Stelle erwähnt, sollte die Position der Käfiganordnung mit Hilfe der Fluoroskopie sichtbar sein. Vorstehend wurden verschiedene Verfahren vorgeschlagen. Ein höchst bevorzugtes Verfahren wird in den
5A und5B gezeigt. Im Wesentlichen sind mindestens einige der Elemente (Bänder oder Drähte), die die Käfiganordnung bilden, mittels der Anbringung einer strahlenundurchlässigen Abdeckung an diesen Elementen mit erheblicher Sichtbarkeit bei Durchleuchtung versehen. Eine Metallbeschichtung aus einem Metall mit einer bei Einsatz von Fluoroskopie verhältnismäßig besseren Sichtbarkeit als Edelstahl ist bevorzugt. Solche Metalle sind bekannt, umfassen aber Gold und Elemente der Platingruppe der Periodentafel, z.B. Platin, Palladium, Rhenium, Rhodium etc. Die Kollektorkäfiganordnungsdrähte oder -bänder können galvanisiert oder anderweitig mit einer durchgehenden Beschichtung versehen werden, eine dicke Beschichtung lässt sich aber erzielen, indem das Element mit einem strahlenundurchlässigen Draht oder Band umwickelt wird. - Ein anderes für einen solchen Dienst geeignetes Material ist ein Strukturmaterial, das mit einem strahlenundurchlässigen Material gefüllt ist. Zum Beispiel ist ein geeignet feiner Schlauch z.B. aus Nitinol oder Edelstahl, gefüllt mit Platin oder Gold, im Handel erhältlich. Fort Wayne Metals aus Fort Wayne, Indiana, vertreibt ein solches Produkt als „gezogenes gefülltes Rohr".

5A zeigt einen Teil einer Käfiganordnung (300 ), die aus einem gewebten Geflecht von Drähten (302 ) besteht, die wiederum eng mit strahlenundurchlässigen Drähten (304 ) umwickelt sind. Die rechte Seite von5A ist ein teilweiser Freischnitt, der die Drähte (302 ) und die strahlenundurchlässigen Drähte (304 ) im Querschnitt zeigt. - Als Beispiel für die in
5A gezeigte Vorrichtung haben wir festgestellt, dass ein Draht aus einer superelastischen Nickel-Titan-Legierung mit einem Durchmesser in dem Bereich von 0,0254 bis 0,1016 mm (0,001 Zoll bis 0,004 Zoll), der mit Platindraht mit einem Durchmesser von 0,0025 bis 0,051 mm (0,0001 Zoll bis 0,002 Zoll) geeignet und recht gut sichtbar ist. -
5B zeigt einen teilweisen Freischnitt einer Käfiganordnung (308 ), die wie vorstehend beschrieben aus Band (310 ) konstruiert und ferner mit strahlenundurchlässigen Drähten (312 ) umwickelt ist. Wie bei der Abwandlung von5A ist die rechte Seite der Zeichnung teils frei geschnitten, was das Innenband (310 ) zeigt. - Die Abwandlungen der in den
5A und5B gezeigten Käfiganordnungen können an Stelle der hier an anderer Stelle vorgefundenen Käfiganordnungen verwendet werden. - Die
6A bis6D zeigen in generischer Weise ein Verfahren zur Verwendung der hierin beschriebenen Vorrichtung. -
6A zeigt schematisch eine Arterie (400 ), die ein verstopfendes Gerinnsel (402 ) zeigt. Die Blutströmung durch die Arterie (400 ) ist stark beeinträchtigt. Eine Abwandlung der erfindungsgemäßen Vorrichtung (404 ), die der in den2A und2B gezeigten ähnelt, wird in6B in der Arterie (400 ) gezeigt. Diese Abwandlung (404 ) hat einen drehbaren Kerndraht (406 ), der als Führungsdraht geeignet ist. Sie weist distale gewebte Flechtkollektoranordnung (408 ), eine proximale Flechtkäfiganordnung (410 ) und ein rohrförmiges Betätigungselement auf. Weiterhin wird der Zufuhrkatheter (412 ) mit Öffnungen (414 ) für die Perfusion geeigneter Medikamente gezeigt. - Viele Gerinnsel wie das Gerinnsel (
402 ) schieben die Spitze des Kerndrahts (406 ) zur Seite, wenn der Führungsdraht das Gerinnsel berührt. In den meisten Fällen ist dies sogar bevorzugt. Der Kerndraht (406 ) schlüpft dann entlang der Wand der Arterie (400 ), wenn die Vorrichtung (404 ) vorbewegt wird. -
6C zeigt vor der Platzierung eine geeignete Positionierung der distalen Kollektoranordnung (408 ) an der Okklusion (402 ) vorbei und der proximalen Käfiganordnung (410 ) neben dem Gerinnsel. Die Längenanordnung der proximalen Käfiganordnung (410 ) wird wünschenswerterweise so gewählt, dass sie beide Enden des Gerinnsels (402 ) überlappt. - Bei Weiten der Käfigkollektoranordnungen (
408 ,410 ) zu ihrer geweiteten Form strömt Blut durch das Lumen der offenen proximalen Käfiganordnung (410 ) von dem arteriellen zu dem venösen Ende des Gerinnsels (402 ). Wünschenswerterweise hat die distale Käfiganordnung (408 ) einen geweiteten Durchmesser, der der Größe des Lumens der Arterie (400 ) nahe kommt. Die distale Filteranordnung (408 ) ist lediglich vorhanden, um die distale Bewegung des Gerinnsels (402 ) oder von Fragmenten des Gerinnsels zu verhindern. Durch die Öffnungen (414 ) können Thrombolytika zum Auflösen des Gerinnsels (402 ) eingebracht werden. Es ist höchst wünschenswert, dass die proximale Käfiganordnung (410 ) zum Verhindern einer Bewegung des Gerinnsels (402 ) in dem Gefäß (400 ) verwendet wird, damit die Thrombolytika das Gerinnsel (402 ) während dessen Festhaltens vollständig auflösen können. Dieses Vorgehen ermöglicht offensichtlich eine fortlaufende Zufuhr von Thrombolytika für das Gerinnsel (402 ) und sieht weiterhin eine große Gerinnselfläche vor, auf die die Thrombolytika einwirken können. - Alternativ oder zusätzlich zu den oben dargestellten Schritten kann das Gerinnsel (
402 ) durch die geweitete distale Käfiganordnung (408 ) festgehalten und aufgenommen und zusammen mit der distalen Käfiganordnung (408 ) und dem Katheter (412 ) zu einer anderen Stelle hin entfernt werden. - Es sollte offensichtlich sein, dass die Käfiganordnung dieser Erfindung wieder zusammengelegt und nach Bedarf neu platziert werden kann, um Emboli nach Bedarf oder Wunsch zu verlagern oder aufzunehmen.
- Die Konstruktion solcher Abwandlungen ist eine Routinearbeit und liegt im Können eines durchschnittlichen Konstrukteurs auf dem Gebiet. Analog fallen offensichtliche Varianten der oben beschriebenen Erfindung in den Schutzumfang der Erfindung. Diese und andere bekannte bzw. nicht ausschlaggebende Elemente wurden in den Zeichnungen der hier offenbarten Erfindung der Kürze und Klarheit halber weggelassen.
Claims (23)
- Vorrichtung zur Emboliebehandlung (
200 ,220 ,258 ,404 ), welche ein Kernelement, eine proximate Käfiganordnung (208 ,224 ,410 ), ein distales Kollektorelement (206 ,230 ,246 ,248 ,408 ) und ein Betätigungselement (212 ,226 ) umfasst: a. wobei das Kernelement mindestens einen lang gestreckten Kerndraht (202 ,222 ,242 ,250 ,260 ,406 ) umfasst, der ein distales Ende sowie ein proximales Ende aufweist und durch das distale Kollektorelement (206 ,230 ,246 ,248 ,408 ) und die proximale Käfiganordnung (208 ,224 ,410 ) verläuft, b. wobei die proximale Käfiganordnung für das Sammeln oder Verlagern der Emboli dient und ein distales Ende sowie ein proximales Ende aufweist, die im wesentlichen koaxial zu dem Kernelement sind, wobei die proximale Käfiganordnung eine erste Platzierungsform und eine zweite geweitete Form aufweist, wobei sich die zweite geweitete Form von der ersten Platzierungsform unterscheidet, c. wobei das distale Element für das Aufnehmen von Emboli dient und ein distales Ende sowie ein proximales Ende aufweist und von der proximalen Käfiganordnung beabstandet und zu dieser distal ist und eine erste Platzierungsform und eine zweite geweitete Form aufweist, wobei sich die zweite geweitete Form von der ersten Platzierungsform unterscheidet, und d. dadurch gekennzeichnet, dass die proximate Käfiganordnung und das distale Kollektorelement jeweils in der jeweiligen ersten Platzierungsform sind, wenn sich das Betätigungselement in einer ersten Position befindet, und die proximate Käfiganordnung und das distale Kollektorelement jeweils in der jeweiligen zweiten geweiteten Form sind, wenn sich das Betätigungselement in einer zweiten Position befindet, und dass sich in der ersten und in der zweiten Position die proximale Käfiganordnung distal des Betätigungselement befindet. - Vorrichtung zur Emboliebehandlung nach Anspruch 1, dadurch gekennzeichnet, dass die erste Platzierungsform jeweils der proximalen Käfiganordnung und des distalen Kollektorelements koaxial angrenzend zum Kerndraht (
202 ,222 ,242 ,250 ,260 ,406 ) sind. - Vorrichtung zur Emboliebehandlung nach Anspruch 1 oder 2, welche weiterhin ein Trennelement (
210 ,228 ) umfasst, welches das distale Kollektorelement (206 ,230 ) von der proximalen Käfiganordnung (208 ,224 ) axial trennt - Vorrichtung zur Emboliebehandlung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass der Kerndraht (
260 ) drehbar ist. - Vorrichtung zur Emboliebehandlung nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass der Kerndraht (
260 ) bezüglich der proximalen Käfiganordnung drehbar und nicht abnehmbar ist. - Vorrichtung zur Emboliebehandlung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass der Kerndraht (
202 ,222 ,242 ,250 ,260 ,406 ) in dem distalen Kollektorelement (206 ,230 ,246 ,248 ,408 ) und der proximalen Käfiganordnung (208 ,224 ,410 ) axial bewegbar ist. - Vorrichtung zur Emboliebehandlung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass das distale Kollektorelement (
206 ,230 ,246 ,248 ,408 ) und die proximale Käfiganordnung (208 ,224 ,410 ) selbstpositionierend sind, wenn sich das Betätigungselement (212 ,226 ) von der ersten Position in die zweite Position bewegt. - Vorrichtung zur Emboliebehandlung nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, dass das distale Kollektorelement (
206 ,230 ,246 ,248 ,408 ) und die proximale Käfiganordnung (208 ,224 ,410 ) selbst zusammenfallend sind, wenn sich das Betätigungselement (212 ,226 ) von der zweiten Position in die erste Position bewegt. - Vorrichtung zur Emboliebehandlung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass mindestens eines von distalem Kollektorelement (
206 ,230 ,246 ,248 ,408 ) und proximaler Käfiganordnung (208 ,224 ,410 ) aus lang gestreckten Elementen besteht, die eine superelastische Legierung umfassen. - Vorrichtung zur Emboliebehandlung nach Anspruch 9, dadurch gekennzeichnet, dass eine kleinere Menge der lang gestreckten Elemente des distalen Kollektorelements und der proximalen Käfiganordnung Edelstahl umfassen.
- Vorrichtung zur Emboliebehandlung nach Anspruch 9, dadurch gekennzeichnet, dass eine kleinere Menge der lang gestreckten Elemente des distalen Kollektorelements und der proximalen Käfiganordnung ein strahlenundurchlässiges Material umfassen.
- Vorrichtung zur Emboliebehandlung nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass der Kerndraht (
260 ) des Kernelements eine äußere und fest angebrachte Umflechtung (268 ) aufweist. - Vorrichtung zur Emboliebehandlung nach einem der Ansprüche 1 bis 9, dadurch gekennzeichnet, dass mindestens die proximale Käfiganordnung (
208 ,224 ,410 ) und das distale Kollektorelement (206 ,230 ,246 ,248 ,408 ) unter Röntgendurchleuchtung strahlenundurchlässig sind. - Vorrichtung zur Emboliebehandlung nach Anspruch 13, dadurch gekennzeichnet, dass mindestens die proximale Käfiganordnung (
208 ,224 ,410 ) und das distale Kollektorelement (206 ,230 ,246 ,248 ,408 ) aus Elementen bestehen, die aus der Gruppe bestehend aus Drähten (304 ) und Bändern (310 ) gewählt werden, die von einer strahlenundurchlässigen Abdeckung (304 ,312 ) bedeckt sind, wobei die Drähte Filamente mit Querschnitten sind, bei denen das Verhältnis von Dicke zu Breite größer als 0,5 und bis einschließlich 1 reicht, und die Bänder Filamente mit Querschnitten sind, die nicht quadratisch oder rund sind und bei denen das Verhältnis von Dicke zu Breite 0,5 oder weniger beträgt. - Vorrichtung zur Emboliebehandlung nach Anspruch 14, dadurch gekennzeichnet, dass die strahlenundurchlässige Abdeckung strahlenundurchlässigen Draht (
304 ,312 ) umfasst, der um die Elemente (302 ,310 ) gewickelt ist. - Vorrichtung zur Emboliebehandlung nach Anspruch 1, dadurch gekennzeichnet, dass das Kernelement ein fest an der proximalen Käfiganordnung angebrachtes rohrförmiges Element umfasst und der Kerndraht ein entfernbarer Kerndraht innerhalb des rohrförmigen Elements ist.
- Vorrichtung zur Emboliebehandlung nach Anspruch 1, dadurch gekennzeichnet, dass die proximale Käfiganordnung (
208 ,224 ,410 ) mehrere in eine Umflechtung eingewebte Bänder (310 ) umfasst, wobei die Bänder Filamente mit Querschnitten sind, die nicht quadratisch oder rund sind und bei denen das Verhältnis von Dicke zu Breite 0,5 oder weniger beträgt. - Vorrichtung zur Emboliebehandlung nach Anspruch 1, dadurch gekennzeichnet, dass die proximale Käfiganordnung (
208 ,224 ,410 ) mehrere in eine Umflechtung eingewebte Drähte (302 ) umfasst, wobei die Drähte Filamente mit Querschnitten sind, bei denen das Verhältnis von Dicke zu Breite größer als 0,5 ist und bis einschließlich 1,0 reicht. - Vorrichtung zur Emboliebehandlung nach einem der Ansprüche 1, 17 und 18, dadurch gekennzeichnet, dass das distale Kollektorelement (
206 ) mehrere in eine Umflechtung eingewebte Bänder (310 ) umfasst, wobei die Bänder Filamente mit Querschnitten sind, die nicht quadratisch oder rund sind und bei denen das Verhältnis von Dicke zu Breite 0,5 oder weniger beträgt. - Vorrichtung zur Emboliebehandlung nach einem der Ansprüche 1, 17 und 18, dadurch gekennzeichnet, dass das distale Kollektorelement (
206 ) mehrere in eine Umflechtung eingewebte Drähte (302 ) umfasst, wobei die Drähte Filamente mit Querschnitten sind, bei denen das Verhältnis von Dicke zu Breite größer als 0,5 ist und bis einschließlich 1,0 reicht. - Vorrichtung zur Emboliebehandlung nach Anspruch 1, dadurch gekennzeichnet, dass das distale Kollektorelement (
230 ) mehrere radial von dem Kerndraht (222 ) abstehende Kollektorelemente (232 ) umfasst. - Vorrichtung zur Emboliebehandlung nach einem der vorhergehenden Ansprüche, welche weiterhin einen Zufuhrkatheter (
412 ) umfasst, der dafür ausgelegt ist, die proximale Käfiganordnung (208 ,224 ,410 ) und das distale Kollektorelement (206 ,230 ,408 ) zu einer Embolie in einem Blutgefäß zu befördern. - Vorrichtung zur Emboliebehandlung nach Anspruch 22, dadurch gekennzeichnet, dass der Zufuhrkatheter (
412 ) ein distales Ende und Öffnungen (414 ) in dem distalen Ende des Zufuhrkatheters aufweist, welche dafür ausgelegt sind, Fluide durch die Öffnungen zu befördern.
