Vor dem Einführen wird der Stent um das Ballonteilstück am distalen Ende eines Katheters plaziert und zusammengedrückt, um die Außenmaße zu reduzieren.
- Sobald der Stent in den Körper in der richtigen Axialposition eingebracht ist, kann er expandiert und somit plastisch verformt werden, indem der Ballon aufgepumpt wird. In dieser Endposition hat der Stent seinen größten Durchmesser und sollte so funktionieren, daß er das umgebende Gewebe abstützt, was eine unerwünschte Formänderung auf einen viel kleineren Durchmesser verhindert, zumindest lokal.
- Daher muß der Stent ausreichende Steifigkeit in Radialrichtung haben, aber auch gewisse Flexibilität in Axialrichtung, wenn er sich in der Endposition befindet. Ferner sollte die Materialmenge möglichst klein sein, und an der Innenfläche des Stents nicht den Durchfluß durch den Kanal (z. B. für Blut) behindern oder zu starke Turbulenz verursachen.
- Probleme, die mit diesen Stents allgemein auftreten, sind wie folgt: Nach dem Zusammendrücken des Stents auf seinen kleinsten Durchmesser um den Ballon hat der Stent stets eine gewisse elastische Rückfederung auf einen etwas größeren Durchmesser, was Probleme verursachen kann, wenn der Katheter in den Patientenkörper eingebracht wird. Außerdem kann die Axialreibung zwischen Ballon und Stent so klein werden, daß der Stent vom Katheter rutscht. Ferner ist ein großer Stent normalerweise nachteilig.
- Ein weiteres Problem ist das sogenannte Zurückschnappen dieser Stents. Dies bedeutet, daß nach Expansion durch den Ballondruck der Außendurchmesser stets etwas kleiner wird, sobald man den Ballon abläßt. Dieser Grad an Zurückschnappen kann bis 10 % betragen, was Migration des Stents verursachen kann.
- Eine andere Art von Stent ist aus einer mehr oder weniger elastisch expandierenden Struktur hergestellt, die auf dem Katheter durch eine gewisse externe Einrichtung gehalten werden muß. Ein Beispiel für diese Art ist ein Stent, der in seinem eingespannten Zustand durch eine Abgabehülle gehalten wird, die in dem Moment entfernt wird, in dem sich der Stent auf seine natürliche Form entfalten sollte.
- Einige dieser Stents sind aus Formgedächtnismaterial mit superelastischem Verhalten oder temperaturempfindlicher Auslösung der Expansionsfunktion hergestellt.
- Ein Nachteil dieser selbstexpandierenden Stents ist, daß die Abgabehülle notwendig ist, was einen größeren Einführungsdurchmesser bewirkt. Zudem erfordert das Entfernen der Hülle einen Hüllenrückzugmechanismus, der am proximalen Ende aktiviert werden muß.
- Die meisten Stents beider Arten haben ferner den Nachteil einer relativ großen Längenänderung während der Expansion und eines schlechten hydrodynamischen Verhaltens aufgrund der Form der Metalldrähte oder -streben.
- Ein weiterer Nachteil einiger Stents ist die positive Federrate, was bedeutet, daß weitere Expansion nur durch höheren Ballondruck erreicht werden kann.
- Der Aufbau bekannter Stents erfolgt normalerweise so, daß die Außenkräfte, die auf den Stent in Radialrichtung wirken, lediglich Biegekräfte auf die Streben oder Drähte der Struktur verursachen.
- Beispielsweise hat eine Zelleinheit eines Palmaz-Schatz-Stents, hergestellt von Johnson & Johnson Interventional Systems, oder des "ACT One Coronary"-Stents, hergestellt von Progressive Angioplasty Systems, Inc., in ihrem zusammengefalteten Zustand eine flache, rechtwinklige Form und in ihrem expandierten Zustand eine mehr oder weniger rautenförmige Gestalt mit nahezu geraden Streben (Palmaz-Schatz) oder stärker gekrümmten Streben (ACT-One).
- Normalerweise ist die Form der Zelleinheit solcher Stents mit vier Streben symmetrisch, die jeweils den gleichen Querschnitt haben. Außerdem bewirkt die Belastung der Zelle in Axialrichtung normalerweise eine elastische oder plastische Verformung aller Streben, was zu einer Verlängerung der Zelleinheit in Axialrichtung führt. Diese Zelleinheiten haben eine positive Federrate. In Stents auf der Grundlage dieser Zelleinheiten hängt die Stabilität gegen Radialdruck lediglich von der Biegefestigkeit der Streben und ihrer Verbindungen ab.
- In den folgenden Verweisen des Stands der Technik sind Beispiele für bekannte Stents offenbart.
- Die FR-A-2728156 offenbart einen Stent nach dem Oberbegriff von Anspruch 1.
- Die WO-A-96/18359 offenbart einen Stent zum Expandieren von Gefäßen mit einem dünnwandigen röhrenförmigen Metallelement, das eine Folge von Perforationen aufweist, wobei der Stent unter Innendruck radial expandiert werden kann.
- Die WO-A-95/09584 offenbart einen röhrenförmigen expandierbaren Stent für eine intraluminale Endoprothese mit Längsabschnitten, die durch Paare von Querabschnitten gekoppelt sind.
- Die WO-A-96/09013 offenbart einen expandierbaren intraluminalen Stent, der ein intaktes Gefäßimplantat stützen kann und ein Röhrenteil mit mehreren steifen Stützlaschen aufweist.

- Die WO-A-93/22986 offenbart einen radial selbstexpandierenden Stent zur Behandlung von Ösophagusstrikturen mit einer proximalen und einer distalen Manschette mit Durchmessern, die größer als ein Mittelbereichdurchmesser sind.

- Die US-A-4994071 offenbart einen sich gabelnden Stent zum Einführen in ein sich gabelndes Gefäß, wobei der Stent aus einer Einführungskonfiguration in eine implantierte Konfiguration mit Hilfe eines Ballonkatheters expandiert werden kann.
- Die GB-A-2081173 offenbart ein Verfahren zum Formen eines Musters innerhalb einer Glasfassungsöffnung eines Brillengestells.
- Die EP-A-0734698 offenbart einen Stent mit einem röhrenförmigen Körper, der zur Expansion aus einem zusammengedrückten Zustand in einen expandierten Zustand mit einem vergrößerten Durchmesser fähig ist.
- Die WO-A-96/29028 offenbart einen expandierbaren Stent für ein Gefäß mit einem Röhrenteil, das Schlitze hat, die mehrere miteinander verbundene längliche Teile bilden.
- Die EP-A-0744164 offenbart einen Stent zum Unterstützen eines Gefäßes mit einem röhrenförmigen Drahtrahmen mit Reihen länglicher Zellen, die durch zwei U-förmige Drahtteilstücke gebildet sind.
- Die GB-A-2169515 offenbart eine Katheterhalterung mit einem flexiblen Endotrachealtubus zum Einführen in die Trachea eines Patienten.
- Die EP-A-0421729 offenbart einen Gelenkstent mit einer Anzahl von Stentsegmenten, die durch Gelenke miteinander verbunden sind, die so verschweißt sind, daß sie für Gelenkbewegungen zwischen Stentsegmenten sorgen.
- Zusammenfassung der Erfindung
- In dieser Patentanmeldung wird eine neue Art von Stent mit einer Zelleinheit beschrieben, die eine negative Federrate und eine bistabile Funktion hat. Dies bedeutet, daß sie zwei Konfigurationen hat, in denen sie stabil ist, ohne daß eine Außenkraft nötig ist, um sie in dieser Form zu halten. Die Zelleinheit ist mit Hilfe mindestens zweier unterschiedlicher Abschnitte bzw. Teilstücke gebildet. Ein Teilstück ist weniger nachgiebig als das andere und wirkt als relativ steife Stütze, die die Formänderung des nachgiebigeren Teilstücks in einer Richtung verhindert. In der anderen Richtung kann das nachgiebige Teilstück verformt werden, aber aufgrund der Gegenkraft vom steifen Teilstück ist die Stabilität des nachgiebigen oder flexiblen Teilstücks stark erhöht.
- Außenkräfte in senkrechter Richtung zum nachgiebigsten Teilstück werden zum steifen Teilstück verteilt, und der Querschnitt des nachgiebigen Teilstücks wird lediglich im Kompressionsmodus belastet. Dies macht den Aufbau viel fester als bekannte Stents. In bekannten Stents haben alle Streben allgemein den gleichen Querschnitt und die gleichen mechanischen Eigenschaften und werden lediglich im Biegemodus verwendet.
- Der Aufbau eines auf dieser Zelleinheit beruhenden Stents führt zu einer Vorrichtung, die durch Fingerdruck um den Ballon leicht elastisch zusammengedrückt werden kann.
- Unterhalb eines bestimmten kritischen Durchmessers schnappt der Stent weiter auf einen stabilen, kleinsten Durchmesser zusammen, wodurch er den abgelassenen Ballon fest auf der Oberfläche des Katheters mit einem Einführungsdurchmesser hält, der möglichst klein ist. Eine zusätzliche Hülle ist nicht erforderlich, kann aber zwecks zusätzlicher Sicherheit verwendet werden.
- Nach Einbringen des Stents in den Patientenkörper an der richtigen Axialposition kann der Ballon aufgeblasen werden, bis der Stent seinen kritischen elastischen Gleichgewichtsdurchmesser erreicht. Etwas oberhalb dieses Durchmessers expandiert der Stent automatisch weiter auf seinen größten Enddurchmesser, wo er seine maximale Stabilität gegen Radialdruck erreicht. Die Gestaltung ermöglicht ein großes Expansionsverhältnis mit konstanter Länge, eine zuverlässige Expandierbarkeit und/oder ein kleines Oberflächenverhältnis.
- Eine weitere Ausführungsform der Erfindung ist die Möglichkeit einer Art von schrittweise expandierendem Stent mit einem Bereich von stabilen Durchmessern.
- Bei einem weiteren Teil der Erfindung handelt es sich um einen Stent mit mehreren Außendurchmessern über seine Länge, um sich möglichst gut an die Form einer Körperhöhle anzupassen, in der er plaziert ist.
- Ein weiterer Teil der Erfindung ist die Möglichkeit, das Spannungs- und Dehnungsmuster in der Zelleinheit mittels einer Wärmebehandlung so zu modifizieren, daß die Kraft-Weg-Kennlinie dieser Zelleinheit asymmetrisch wird oder sogar eine monostabile statt einer bistabilen Funktion zeigt, wobei der expandierte Durchmesser oder der zusammengefaltete Durchmesser der stabilste Zustand ist.
- Eine weitere Ausführungsform der Erfindung ist die Abwandlung der Geometrie des Querschnitts einiger Streben, um die symmetrischen oder asymmetrischen bistabilen oder monostabilen Kraft-Weg-Kennwerte einer Zelleinheit zu erreichen.
- Beschreibung des Aufbaus
- Der Aufbau des Stents weist eine Folge von Elementen mit einer Anordnung von Zelleinheiten auf, die die Stabilität auf besondere Weise ermöglichen. Jede Zelleinheit besteht aus mindestens zwei verschiedenartigen, mechanisch verbundenen Teilstücken mit unterschiedlichem mechanischem Verhalten. Ein Teilstück wirkt als relativ steife Stütze für das flexiblere entgegenwirkende Teilstück. Das flexiblere Teilstück ist für die meiste, wenn nicht sogar die gesamte, Expansion des Stents zuständig. Es gibt mehrere Möglichkeiten, einen Stent auf der Grundlage dieses Prinzips herzustellen, und er kann aus mehreren Materialien wie Polymeren, Verbundstoffen, herkömmlichen Metallen oder Formgedächtnislegierungen mit super elastischem Verhalten oder mit temperaturempfindlichem Verhalten hergestellt sein.
- Er kann aus einer Draht- oder Bandanordnung hergestellt sein, die an spezifischen Stellen verschweißt ist. Eine weitere Möglichkeit ist Metallabscheidung im gewünschten Muster auf einen Untergrund oder der Einsatz von Sintern von vorlegiertem Pulver.
- Ein weiteres Verfahren besteht darin, den Stent aus einem röhrenförmigen Ausgangsmaterial mit einem Muster aus Schlitzen oder Einschnitten herzustellen, die in der Wand mit Hilfe von Ätzen, Schleifen, Schneiden (z. B. mit einem Laser, Wasser usw.), Funkenerosion oder jedem anderen geeigneten Verfahren hergestellt sind. Das Muster kann auch in einer flachen Platte gebildet und dann zu einer mehr oder weniger zylindrischen Form oder einem zylindrischen Mittelteilstück mit zwei konischen Enden mit größeren Durchmessern geschweißt, hartgelötet oder gecrimpt werden.

1 zeigt das Prinzip, auf dem der Stent beruht,1a zeigt einen Stab1 mit einer Länge L, der in seine Axialrichtung zusammengedrückt wird; er erreicht seine Knickspannung. Der Mittelteil des Stabs biegt sich in einer Seitwärtsrichtung nach außen zur Position2 oder3 (gestrichelte Linien in1b ). Wird die Axialverschiebung1 der Enden des Stabs durch äußere Einspannungen4 stabil gehalten, ist es möglich, das Mittelteilstück des Stabs zwischen den beiden stabilen Positionen2 und3 zu bewegen. Diese Bewegung erfolgt in eine Richtung X senkrecht zur ursprünglichen Längsachse A-A des Stabs. Alle Positionen zwischen den stabilen Positionen2 und3 sind instabil. In1b muß der Mittelteil des Stabs über einen Winkel β drehen, bevor der Stab in Richtung X bewegt werden kann.1c zeigt eine Krümmung zweiter Ordnung im Stab1 , die auftritt, wenn der Drehung über den Winkel β entgegengewirkt wird, indem der Mittelteil des Stabs1 eingespannt und dieser Teil parallel zur Achse A-A gehalten wird. -
2 zeigt die Kraft F als Funktion der Verschiebung (des Wegs) X, wobei X in waagerechter Richtung dargestellt ist. Der Stab wird aus der oberen stabilen Position2 in die untere3 von1 bewegt. Die Kraft nimmt von null auf Fmax schnell zu. In diesem Moment wird das Einsetzen der Krümmung erster oder zweiter Ordnung von1b und1c erreicht. Eine weitere Verschiebung in Richtung X bedarf weniger Kraft, da dieses Federsystem eine negative Federrate hat. Die Kraft wird in der Mittelposition sogar null, und eine weitere Bewe gung tritt automatisch auf. Aus2 geht hervor, daß das System vollkommen symmetrisch ist und die Kraft, die zum Zurückbewegen aus der unteren in die obere Position nötig ist, den gleichen Kennwert hat. -
3 zeigt einen Stab5 , der eine asymmetrische Kraft-Weg-Kennlinie hat, da er bereits eine voreingestellte Krümmung auch in der unbelasteten Position hat, wobei die Länge bereits L – ΔL beträgt. Erreichen läßt sich dies durch plastische Vorverformung, Wärmebehandlung oder die Verwendung einer asymmetrischen Geometrie des Querschnitts des Stabs (nicht gezeigt). Der Stab5 in3 kann zwischen zwei Einspannungen auf einer Länge L – ΔL angeordnet werden, und bei seiner elastischen Verformung auf die gleiche Weise wie der Stab in1b und1c hat er eine unterschiedliche Spannungsverteilung im Querschnitt in den Endpositionen2 und3 verglichen mit dem Stab von1 . Dies bedeutet, daß der Stab eine bevorzugte unbelastete stabile Position gemäß3 eingenommen hat. -
4 zeigt die asymmetrische Kraft-Weg-Kennlinie des vorgekrümmten Stabs von3 . Die Anfangsverschiebung aus der stabilen oberen Position benötigt eine Anfangskraft F1, und befindet sich der Stab in seiner stabilen unteren Position, beträgt die Anfangskraft in Gegenrichtung nur F2, was kleiner als F1 ist. Die Kraft F2 kann so klein wie gewünscht gemacht werden, sogar null oder negativ, muß aber einen positiven Wert haben, wenn Stabilität der unteren Position erforderlich ist. -
5a und5b zeigen das allgemeine Aussehen eines erfindungsgemäßen röhrenförmigen Stents im voll kontrahierten bzw. voll expandierten Zustand. In seinem allgemein bei50 gezeigten voll kontrahierten Zustand und in seinem allgemein bei60 gezeigten voll expandierten Zustand weist der Stent mehrere miteinander verbundene bistabile Zelleinheiten auf (im expandierten Zustand bei64 in5b gezeigt). Die bistabilen Zelleinheiten sind aus einem ersten relativ steifen Segment52 (66 in5b ) und einem zweiten relativ flexiblen Segment54 (68 in5b ) gebildet, die an Enden70 und72 miteinander gekoppelt sind. Die zweiten relativ flexiblen Segmente68 sind mit benachbarten relativ steifen Teilen66 gegenseitig verbunden. Benachbarte Zellen in Längsrichtung (die Längsachse ist mit der Bezugszahl75 bezeichnet) sind an den Enden70 und72 gekoppelt. Durch Ausüben einer gleichmäßigen Radialauswärts- oder -einwärtskraft kann der Stent aus einer voll kontrahierten in eine voll expandierte Konfiguration oder umgekehrt direkt überführt werden. -
6 (entspricht dem Ausschnitt6 in5b ) veranschaulicht einen kleinen Teil eines Stents wie dem in5 gezeigten, der die bistabile Funktion einer Zelleinheit erfindungsgemäß verwendet. Die Zeichnung zeigt eine waagerechte Linie A-A, die parallel zur Mittelachse des Stents liegt. Vorhanden sind zwei Folgen sinusförmiger Segmente mit verschiedenartiger Größe (siehe auch9 für einen Überblick). Die Segmente7 und9 haben einen relativ großen Querschnitt. Nur das Segment9 ist insgesamt gezeigt. Die Segmente9 und10 haben einen relativ kleineren Querschnitt, und hier ist nur das Segment8 im ganzen gezeigt. Die Segmente sind an Verbindungsstellen11 und12 miteinander verbunden, z. B. verschweißt. - Aufgrund der Querschnittdifferenz der Segmente
8 und9 ist die Verformungskraft des Segments8 viel geringer als für das Segment9 . Daher kann das Segment9 als relativ steife Einspannung wie die Einspannungen4 in1b betrachtet werden, die einer Relativverschiebung zwischen den Verbindungsstellen12 in Axialrichtung parallel zur Achse A-A entgegenwirkt. Dagegen wirkt das Segment8 als flexibler Stab wie der in1 beschriebene Stab1 oder der in3 beschriebene Stab5 . Diese Kombination aus Segmenten7 und8 oder9 und10 bildet eine Zelleinheit, die als bistabiles Federsystem mit einer Kraft-Weg-Kurve F-X wie die beschriebenen Kurven von2 und4 in Abhängigkeit vom unbelasteten Zustand und von der Geometrie der Segmente wirkt. Statt Segmente oder Streben mit unterschiedlichem Durchmesser zu verwenden, können alternativ die Segmente den gleichen Durchmesser (d. h. die gleiche Querschnittfläche) haben und unterschiedliche Festigkeiten oder Steifigkeiten aufweisen und immer noch den gleichen Effekt erreichen. Eine Möglichkeit, solche Festigkeits- oder Steifigkeitsdifferenzen zu erhalten, würde darin bestehen, unterschiedliche Materialien für die Segmente zu verwenden. Eine weitere Möglichkeit wäre, das gleiche Material, z. B. ein Metall, für alle Segmente zu verwenden, aber jene Segmente selektiv zu verstärken (z. B. durch Wärmebehandlung), die steif sein müssen. Zu beachten ist, daß Wärmebehandlung nicht alle Materialien verstärkt. Beispielsweise wird Nitinol als Ergebnis von Wärmebehandlung nachgiebiger. Gleichwohl läßt sich diese Eigenschaft von Nitinol ausnutzen, um ein Teilstück aus Nitinol nachgiebiger gegenüber einem zweiten, nicht wärmebehandelten Teilstück aus Nitinol zu machen. -
7 zeigt denselben Teil des Stents (wie in6 gezeigt) nahe der elastischen Gleichgewichtsposition. Das Segment8 wurde in Richtung X aufgrund der Kraft F verformt, wogegen das Segment9 wegen seiner größeren Steifigkeit nahezu seine Ausgangsform hat. -
8 zeigt dieselbe Zelleinheit des Stents von6 und7 , nachdem er über die elastische Gleichgewichtsposition hinaus zusammengedrückt wurde. Automatisch schnappt er in seine stabile Position von8 zusammen. Diese Einschnappkraft kann stark genug sein, einen abgelassenen Ballon dicht auf dem Katheterschaft (nicht gezeigt) zu halten, was von den mechanischen Kennwerten (z. B. der Festigkeit) des (der) zur Herstellung der Segmente verwendeten Materials (Materialien) abhängt. Mit der Geometrie gemäß diesen Darstellungen fügen sich die Segmente8 und9 eng aneinander, wobei sie einen minimalen Platzbedarf haben, wenn der Stent seinen kleinsten stabilen Durchmesser hat. -
9 zeigt ein zur Veranschaulichung abgeflachtes Teilstück des Stents von5 , wobei sie mehrere flexible Segmente in der zusammengefalteten stabilen Form (Segmente14 ,18 und20 ) und ein Segment16 in der expandierten stabilen Form darstellt. Segmente13 ,15 ,17 und19 sind relativ steife Segmente und behalten im wesentlichen ihre Ausgangsform bei. Der Abstand zwischen zwei relativ steifen Segmenten ist als (h) im zusammengefalteten stabilen Zustand und als (H) im expandierten stabilen Zustand gezeigt. Der Wert der Verschiebung (H – h) in Richtung X hängt von der Höhe einer expandierten Zelleinheit oder von der Amplitude der Segmente und der Größe der Verbindungsstellen ab. Der beschriebene Teil des Stents ist als flache Oberfläche dargestellt, wobei aber klar sein sollte, daß ein zylindrischer Stent wie der von5 geformt wird, wenn die Segmente13 und20 mit Verbindungsstellen21 direkt miteinander verbunden werden. Anders gesagt ist der Stent entlang den Verbindungsstellen21 getrennt und in einem abgeflachten Zustand gezeigt. - Der Bereich stabiler Durchmesser des Stents ändert sich mit dem Wert (H – h)/π jedesmal dann, wenn ein flexibles Segment aus der zusammengefalteten stabilen Position in die expandierte stabile Position schnappt. Das Ergebnis ist ein Stent mit einer extrem steifen Oberfläche an allen Durchmessern, der den Außenkräften besser als bei herkömmlichen Stents widerstehen kann. In Längsrichtung kann die Flexibilität des Stents erhöht sein, indem mehrere Zelleinheiten von ihren Nachbarzelleinheiten gelöst sind, z. B. durch Zerschneiden der Mitte einer oder mehrerer Verbindungsstellen, während mehrere verbundene Stücke als Verbindungsstellen beibehalten werden.
- Ein weiteres Verfahren zur Erhöhung der Flexibilität ist, die Geometrie mehrerer Teilstücke der Zelleinheiten in Längsrichtung von der relativ flexiblen zur relativ steifen Form mehrmals über die Gesamtlänge des Stents zu ändern. Anders gesagt könnten gemäß
9 ein oder mehrere oder jedes der Segmente13 bis20 mit Teilstücken mit größerem und kleinerem Durchmesser (oder ansonsten flexiblen und steifen Teilstücken) aufgebaut sein, die sich nach jeder Verbindungsstelle21 abwechseln. - Eine weitere Möglichkeit gemäß
10 ist der Gebrauch einer Folge kurzer multistabiler Stents100 , die endweise längs ausgerichtet und mit Flexibilität aufweisenden Verbindungsstellen104 verbunden sind, die die gleiche oder eine unterschiedliche Geometrie oder Konfiguration verglichen mit den Verbindungsstellen haben, die die einzelnen Zelleinheiten bilden. - Zum Schutzumfang der Erfindung sollten alle Arten von Material gehören. Eines der interessantesten Materialien ist superelastisches Nitinol aufgrund seiner großen elastischen Dehnung, seinen wohldefinierten Spannungswerten, hervorgerufen durch ihre Plateauspannungen und die Möglichkeit, die gewünschte Krümmung im Material mit Hilfe von Wärmebehandlung festzulegen. Ein Stent aus Nitinol kann durch Bilden von Schlitzen oder Einschnitten in einer Röhre hergestellt werden, während er seinen zusammengefalteten oder kleineren stabilen Durchmesser hat. Danach wird die geschlitzte Röhre durch ein separates Formgebungswerkzeug auf gedehnt und auf diesem Werkzeug wärmebehandelt, um den expandierten stabilen Durchmesser als ungespannte Form festzulegen.
- In einem allgemeineren Sinn betrifft die Erfindung eine Vorrichtung mit mehreren stabilen Konfigurationen. Die Vorrichtung weist mehrere miteinander verbundene multistabile Zellen auf. Die Zellen verfügen über ein oder mehrere relativ steife Teilstücke und ein oder mehrere relativ flexible Teilstücke, die miteinander verbunden sind, um eine Zellstruktur in Form eines multistabilen Federsystems mit mehreren stabilen Konfigurationen zu bilden. In einer bevorzugten Ausführungsform weisen die Zellen ein erstes gebogenes Teil mit einem ersten und einem zweiten Ende sowie ein zweites gebogenes Teil mit einem ersten und einem zweiten Ende auf, wobei das erste Ende des ersten Teils mit dem ersten Ende des zweiten Teils in Verbindung steht und das zweite Ende des ersten Teils mit dem zweiten Ende des zweiten Teils in Verbindung steht. Allerdings ist zu beachten, daß Teile nicht streng bogenförmig sein müssen. Anders geformte Teile, u. a. relativ gerade Teile, sind ebenfalls erwogen.
- Insbesondere erwägt die Erfindung bistabile Zellen, d. h. Zellen mit zwei stabilen Konfigurationen. In einer solchen Zelle ist der Abstand zwischen entsprechenden Punkten auf dem ersten und zweiten Teilstück größer im ersten stabilen Zustand der Zelle als im zweiten stabilen Zustand der Zelle. Die Zellen selbst sind so aufgebaut und angeordnet, daß die Vorrichtung selbst mindestens bistabil und möglicherweise multistabil ist. Eine solche Vorrichtung, ein zylindri scher Stent mit zwei oder mehr Konfigurationen mit einer Anfangsdurchmessergröße und einer größeren Enddurchmessergröße, wurde zuvor beschrieben. Erwogen sind aber auch multistabile Stents. So kann z. B. ein Stent aufgebaut sein, in dem die Zellen so gestaltet und angeordnet sind, daß für einen Bereich von Durchmessern schrittweise gesorgt ist. Eine solche Möglichkeit, dies zu realisieren, bestünde darin, mehrere unterschiedliche Arten von Zellen im Stent zu verwenden, wobei jede Zellart eine unterschiedliche Federkonstante hat, so daß je nach aufgewendetem Kraftbetrag der Stent einen unterschiedlichen Durchmesser annehmen würde. In
11 ist ein solcher Stent in einem teilweise expandierten Zustand schematisch gezeigt. Ein teilweise expandierter Stent ist bei120 allgemein dargestellt. Der Stent verfügt über relativ steife Segmente123 ,127 ,131 und135 , die ihre Ausgangsform im wesentlichen beibehalten, und relativ flexible Segmente125 ,129 und133 . Die Segmente sind mit Verbindungsstellen122 untereinander verbunden. Darstellungsgemäß haben die ersten flexiblen Elemente125 und133 eine expandierte Konfiguration, während das zweite flexible Element129 eine kontrahierte Konfiguration hat. Durch Ausüben einer Radialauswärts- oder Tangentialkraft kann das flexible Element129 in seine voll expandierte Konfiguration schnellen, was zu einem Stent (nicht gezeigt) mit einem größeren Durchmesser führt. Gemäß11 sind Zellen138 auch im kontrahierten Zustand größer als Zellen136 . Die ersten flexiblen Elemente125 und133 sind durch einen anderen Flexibilitätsgrad als das zweite flexible Element129 gekennzeichnet. - Eine weitere Form von Stent, die allgemein bei
150 in der schematischen Darstellung von12 gezeigt ist, hat einen ersten Durchmesser an einem ersten Ende152 , einen zweiten Durchmesser an einem zweiten Ende154 und einen (oder mehrere) Zwischendurchmesser in einem Bereich156 zwischen dem ersten Ende152 und zweiten Ende154 , wobei sich der Zwischendurchmesser vom ersten und zweiten Durchmesser unterscheidet. Die miteinander verbundenen Zellen in einem solchen Stent, der allgemein bei150 in12 gezeigt ist, können alle die gleiche Kraftkonstante haben und somit bei Ausübung der notwendigen Kraft alle zugleich öffnungsfähig sein, oder es können mehrere unterschiedliche Arten von Zellen jeweils mit ihrer eigenen Kraftkonstante vorhanden sein. Um die mehreren Durchmesser zu realisieren, können Zellen mit unterschiedlichen Größen verwendet werden. In einer Ausführungsform dieser Art von Stent sind der erste und zweite Durchmesser gleich, während sich in einer weiteren Ausführungsform der erste und zweite Durchmesser unterscheiden. - Ein Verfahren zum Implantieren eines expandierbaren Stents mit mehreren stabilen Konfigurationen weist die folgenden Schritte auf: Aufbringen des Stents auf eine Expandiereinrichtung auf einem Katheter, Abgeben des Stents zu einer gewünschten Körperstelle, Expandieren der Expandiereinrichtung, um den Stent aus einer ersten stabilen Konfiguration in eine gewünschte zweite stabile Konfiguration zu expandieren, wobei die zweite stabile Konfiguration einen größeren Durchmesser als die erste stabile Konfiguration hat, und Setzen des expandierten Stents an der gewünschten Körperstelle. Die Expandiereinrichtung kann ein Ballon, eine mechanische Vorrichtung auf oder im Katheter, eine Wärmequelle, an der die Zellen zur Änderung von Zuständen durch Erwärmung gebracht werden können, oder jede andere geeignete Expandiereinrichtung sein. Der Stent kann auf den Ballon in der ersten stabilen Konfiguration aufgebracht werden oder kann in der zweiten stabilen (expandierten) Konfiguration im Aufbringungsschritt aufgebracht werden. Im letztgenannten Fall kann Radialeinwärtsdruck auf den Stent ausgeübt werden, um den Stent in die erste stabile Konfiguration zu drücken, damit er auf den Katheter aufschnappt. Hat der Stent zusätzliche stabile Zustände, kann der Stent auf den Ballon in einem stabilen Zwischenzustand aufgebracht werden, in dem der Durchmesser des Stents zwischen dem Durchmesser im ersten Zustand und dem Durchmesser im zweiten Zustand liegt. Wiederum kann der Stent auf der Expandiereinrichtung durch weiteres Ausüben eines Radialeinwärtsdrucks arretiert werden.
Claims (9)
- Stent mit einer Oberfläche, die eine Vielzahl von Zellen (
64 ) umfasst, wobei jede Zelle (64 ) eine stabile zusammengefaltete Konfiguration (50 ) und eine stabile expandierte Konfiguration (60 ) aufweist, wobei die stabile expandierte Konfiguration (60 ) einen größeren Bereich als die stabile zusammengefaltete Konfiguration (50 ) umfasst, wobei jede Zelle (64 ) ersten und zweiten miteinander verbundenen Abschnitt (66 ,68 ) aufweist, dadurch gekennzeichnet, dass: der zweite Abschnitt (68 ) flexibler als der erste Abschnitt (66 ) ist und der erste Abschnitt (66 ) das Durchbiegen des zweiten Abschnitts (68 ) in einer Hauptrichtung verhindert oder sperrt, was zu einer Struktur führt, die in der anderen Hauptrichtung um die rechtwinklige Hauptrichtung deformiert werden kann, wenn die Zelle zwischen der stabilen zusammengefalteten Konfiguration (50 ) und der stabilen expandierten Konfiguration (60 ) wechselt. - Stent nach Anspruch 1, wobei jede Zelle (
64 ) entweder nur die stabile zusammengefaltete Konfiguration (50 ) oder die stabile expandierte Konfiguration (60 ) annehmen kann. - Stent nach Anspruch 1, wobei jede Zelle (
64 ) isothermisch entweder nur die stabile zusammengefaltete Konfiguration (50 ) oder die stabile expandierte Konfiguration (60 ) annehmen kann. - Stent nach Anspruch 1, wobei die Vielzahl von Zellen (
64 ) konstruiert und angeordnet sind, dass die Zellen (64 ) zwischen den beiden stabilen Konfigurationen (50 ,60 ) durch Aufbringen einer gleichmäßigen, in radialer Richtung gerichteten Kraft auf eine Innenfläche des Stents (50 ,60 ) wechseln können. - Stent nach Anspruch 1, wobei jede Zelle (
64 ) zu einer Vielzahl von diskreten stabilen Zwischenkonfigurationen zwischen der stabilen zusammengefalteten Konfiguration (50 ) und der stabilen expandierten Konfiguration (60 ) deformiert werden kann. - Stent nach Anspruch 1, wobei jede Zelle (
64 ) eine zentrale Gleichgewichtsstellung und eine asymmetrische Kraftverdrängungscharakteristik um die zentrale Gleichgewichtsstellung herum aufweist, wobei die expandierte Konfiguration (60 ) die stabilste Konfiguration jeder Zelle (64 ) ist. - Stent nach Anspruch 1, wobei jede Zelle (
64 ) eine zentrale Gleichgewichtsstellung und eine asymmetrische Kraftverdrängungscharakteristik um die zentrale Gleichgewichtsstellung herum aufweist, wobei die zusammengefaltete Konfiguration (50 ) die stabilste Konfiguration jeder Zelle (64 ) ist. - Stent nach Anspruch 1, wobei eine erste Untermenge der Vielzahl von Zellen zweite Abschnitte umfasst, die eine erste Steifigkeit aufweisen, und eine zweite Untermenge der Vielzahl von Zellen zweite Abschnitte umfasst, die eine zweite Steifigkeit aufweisen, wobei sich die erste Steifigkeit von der zweiten Steifigkeit unterscheidet.
- Stent nach Anspruch 1, wobei jede Zelle zwischen der stabilen zusammengefalteten Konfiguration (
50 ) und der stabilen expandierten Konfiguration (60 ) durch Anwendung einer Kraft innerhalb des Stents wechselt, die radial nach außen gerichtet ist, wobei jede Zelle (64 ) einen Übergangspunkt passiert, der es gestattet, dass die Kraft während des Übergangs zwischen der stabilen zusammengefalteten Konfiguration (50 ) und der stabilen expandierten Konfiguration (60 ) abnimmt.
