Description
- Gebiet der Erfindung
- Die vorliegende Erfindung betrifft allgemein die Bereitstellung biostabiler elastomerer Beschichtungen auf den Oberflächen von Stents, welche biologisch aktive Spezies mit kontrollierten Freigabecharakteristiken in der Beschichtung umfassen, insbesondere die Bereitstellung einer nicht-thrombogenen Oberfläche während und nach der zeitgesteuerten Freisetzung (zeitlich gesteuerte Freisetzung, "timed release") von biologisch aktiven Spezies. Die Erfindung betrifft Beschichtungen von therapeutisch expandierbaren Stentprothesen zur vaskulären Implantation.
- II. Stand der Technik
- In chirurgischen oder anderen verwandten invasiven Prozeduren ist das Einführen und die Expansion von Stentvorrichtungen in Blutgefäßen, Harnwegen oder anderen Stellen, die andernfalls schwierig zugänglich sind zum Zweck der Verhinderung von Restenose, zur Bereitstellung von Gefäß- oder Lumenwandunterstützung oder Stärkung und für andere therapeutische oder restaurative Funktionen eine allgemeine Form der Langzeitbehandlung geworden. Typischerweise werden derartige Prothesen an einem Ort des Interesses angewandt, unter Benutzung eines vaskulären Katheters oder einer ähnlichen transluminalen Vorrichtung, um den Stent an den Ort von Interesse zu bringen, wo er danach freigegeben wird, um zu expandieren oder um in situ expandiert zu werden. Diese Vorrichtungen sind all-gemein als permanente Implantate entworfen, welche sich mit dem vaskulären oder anderem Gewebe vereinigen können, welches sie bei Implantation kontaktieren.
- Ein Typ von selbst-expandierendem Stent besitzt einen flexiblen röhrenförmigen Körper, der aus verschiedenen individuellen flexiblen Strangelementen geformt ist, von denen jedes sich in einer Helixkonfiguration ausdehnt, wobei die Mittellinie des Körpers als gemeinsame Achse dient. Die Elemente sind in der gleichen Richtung gewunden, jedoch axial relativ zueinander versetzt und treffen aufeinander, unter Überquerung einer gleichen Anzahl von Elementen, die ebenfalls so axial versetzt sind, jedoch die entgegengesetzte Windungsrichtung besitzen. Diese Konfiguration stellt eine nachgiebige geflochtene röhrenförmige Struktur bereit, die stabile Dimensionen bei Entspannung annimmt. Axiale Spannung produziert eine Verlängerung und eine entsprechende Kontraktion des Durchmessers, welche dem Stent erlaubt, auf einer Kathetervorrichtung montiert zu werden und durch das vaskuläre System als enge verlängerte Vorrichtung geführt zu werden. Sobald die Spannung in situ verringert wird, kehrt die Vorrichtung zumindest wesentlich zu ihrer ursprünglichen Form zurück. Prothesen der Klasse, die einen geflochtenen flexiblen röhrenförmigen Körper umfasst, sind in US-Patenten 4 655 771 und 4 954 126 von Wallsten und 5 061 275 von Wallsten et al. dargestellt und beschrieben.
- Implantierte Stents wurden benutzt, um medizinische Wirkstoffe zu tragen, wie etwa thrombolytische Wirkstoffe. US-Patent 5 163 952 von Froix offenbart eine ausdehnbare Kunststoffstentvorrichtung mit thermischem Gedächtnis, die ausgebildet ist, einen medizinischen Wirkstoff in dem Material des Stents selbst zu tragen. Pinchuk, in US-Patent 5 092 877, offenbart einen Stent aus einem polymeren Material, der eine Beschichtung besitzen kann, die mit der Abgabe von Medikamenten zu tun hat. Weitere Patente, die auf Vorrichtungen gerichtet sind, der Klasse, die bio-abbaubare oder bio-absorbierbare Polymere benutzt, umfassen Tang et al., US-Patent 4 916 193 und MacGregor, US-Patent 4 994 071.
- Ein Patent von Sahatjian, Patent Nr. 5 304 121, offenbart eine Beschichtung, die auf einen Stent aufgetragen wird, die aus einem Hydrogel-Polymer und einem vorausgewählten Arzneimittel besteht, wie z. B. Zellwachstumshemmern oder Heparin. Ein weiteres Verfahren zur Herstellung eines beschichteten intravaskulären Stents, welcher ein therapeutisches Material trägt, wird in Berg et al. beschrieben, US-Patent Nr. 5 464 650, erteilt am 7. November 1995 und entsprechend der europäischen Patentanmeldung Nr. 0 623 354 Al, veröffentlicht am 9. November 1994. In dieser Offenbarung wird ein Polymerbeschichtungsmaterial in einem Lösungsmittel gelöst und das therapeutische Material in dem Lösungsmittel verteilt; das Lösungsmittel verdampfte nach Anwendung.
- Ein Artikel von Michael N. Helmus (ein Miterfinder der vorliegenden Erfindung), betitelt "Medical Device Design – A Systems Approach: Central Venous Catheters" 22 International Society of the Advancement of Material and Process Engineering Technical Conference (1990) betrifft Polymer/Arzneimittel/Membransysteme zur Freisetzung von Heparin. Diese Polymer/Arzneimittel/Membransysteme erfordern zwei unterschiedliche Typen von Schichten, um zu funktionieren.
- Es wurde erkannt, dass das Kontaktieren von Blut mit der Oberfläche eines Fremdkörpers in vivo eine Tendenz besitzt, thrombogene Reaktionen herbeizuführen und dass, wenn sich die Oberfläche einer Fremdvorrichtung in Kontakt mit Empfängerblut vergrößert, die Tendenz zum Koagulieren und zur Klumpenbildung an diesen Oberflächen sich auch vergrößert. Dies hat zur Benutzung von festgelegten systemischen Gerinnungshemmern oder thrombolytischen Wirkstoffen geführt, wie etwa Heparin auf blutkontaktierenden Oberflächen, wie etwa Sauerstoff aufnehmende Vorrichtungen, um dieses Phänomen zu reduzieren. Eine derartige Herangehensweise ist durch Winters et al. in US-Patenten 5 182 317; 5 262 451 und 5 338 770 beschrieben, in welchen die funktionalen Amingruppen des aktiven Materials kovalent auf einer Siloxan-Oberfläche unter Benutzung von Polyethylenoxid (PEO) gebunden werden.
- Eine weitere Herangehensweise wird in US-Patent 4 613 665 von Larm beschrieben, in welchem Heparin chemisch kovalent an Kunststoffoberflächenmaterialien gebunden wird, die primäre Aminogruppen enthalten, um dem Material eine nicht-thrombogene Oberfläche zu verleihen. Weitere Herangehensweisen zum Anbinden von Heparin sind in Barbucci et al., "Coating of commercially available materials with a new heparinizable material" Journal of Biomedical Materials Re search, Bd. 25, 1259 bis 1274 (1991); Hubbell, J. A., "Pharmacologic Modification of Materials", Cardiovascular Pathology, Bd. 2, Nr. 3 (Supplement), 121S bis 1275 (1993); Gravlee, G. P., "Heparin-Coated Cardiopulmonary Bypass Circuits", Journal of Cardiothoracic and Vascular Anesthesia, Bd. 8, Nr. 2, Seiten 213 bis 222 (1994) beschrieben.
- Im Hinblick auf Stents können polymere Stents, obwohl effektiv, mechanische Eigenschaften haben, die denjenigen von Metallstents von gleicher Dicke und Flechtung unterlegen sein können. Metallische Vaskulärstents, die selbst aus relativ feinem Metall geflochten sind, können eine großes Maß an Stärke bieten, um nach innen gerichtetem Umfangsdruck zu wiederstehen. Ein Polymermaterial von vergleichbarer Stärke benötigt eine viel dickwandigere Struktur oder, ein schwereres, dichteres Filamentgewebe, welches seinerseits den Querschnittsbereich reduziert, der für den Durchfluss durch den Stent verfügbar ist und/oder die relative Menge von offenem Raum in dem Gewebe reduziert. Auch ist es üblicherweise schwieriger, polymere Stents unter Benutzung von Katheterzuführsystemen zu beladen und zuzuführen.
- Während bestimmte Typen von Stents, wie etwa geflochtene Metallstents, für bestimmte Anwendungen bevorzugt werden können, ist der Beschichtungs- und Beschichtungsmodifikationsprozess der vorliegenden Erfindung nicht so beschränkt und kann für eine breite Vielzahl von Stents benutzt werden. Daher ist im Fall von Stents die vorliegenden Erfindung auch anwendbar auf z. B. die Klasse der Stents, die nicht selbst-expandierend sind, einschließlich derer, die expandiert werden können, z. B. mit einem Ballon; und ist anwendbar auf polymere Stents von jeglicher Art. Ungeachtet detaillierter Ausführungsformen sollte die Anwendbarkeit der Erfindung nicht als beschränkt im Hinblick auf den Entwurf des Implantats, die Stelle des Implantats oder Konstruktionsmaterialien angesehen werden.
- Dementsprechend ist es eine Hauptaufgabe der vorliegenden Erfindung, eine Beschichtung und ein Verfahren zur Beschichtung eines Stents zur vaskulären Implantation bereitzustellen, der als eine eingesetzte Stentprothese verwendet werden soll, wobei die Beschichtung im Stande ist, zu einer effektiven kontrollierten Langzeitabgabe von biologisch aktiven Materialien.
- Eine weitere Aufgabe der Erfindung ist, eine Beschichtung und ein Verfahren zur Beschichtung einer Stentprothese zur vaskulären Implantation bereitzustellen, unter Benutzung eines biostabilen hydrophoben Elastomers, in welchem biologisch aktive Spezies in die Beschichtung aufgenommen sind.
- Noch eine weitere Aufgabe der vorliegenden Erfindung ist, eine Mehr-Schicht-Beschichtung und Verfahren zur Abgabe von biologisch aktiven Spezies bereitzustellen, in welchem der Prozentsatz von aktivem Material von Schicht zu Schicht variieren kann.
- Eine weitere Aufgabe der vorliegenden Erfindung ist, eine Mehr-Schicht-Beschichtung und Verfahren für die Abgabe von biologisch aktiven Spezies von einer Beschichtung mit einer nicht-thrombogenen Oberfläche bereitzustellen.
- Eine weitere Aufgabe der Erfindung ist, eine Mehr-Schicht-Beschichtung für die Abgabe von biologisch aktiven Spezies, wie Heparin, bereitzustellen, welches eine Deckschicht aus Fluorsilikon besitzt.
- Noch eine weitere Aufgabe der vorliegenden Erfindung ist, eine Mehr-Schicht-Beschichtung zur Abgabe von biologisch aktiven Spezies bereitzustellen, mit einer Oberfläche, die festgelegtes Polyethylenglykol (PEG) enthält.
- Weitere Aufgaben und Vorteile der vorliegenden Erfindung werden dem Fachmann bei dem sich vertraut Machen mit der Beschreibung und den anliegenden Ansprüchen ersichtlich.

- Die vorliegende Erfindung stellt eine relativ dünnschichtige Beschichtung von biostabilem elastomeren Material bereit, welches eine hierin verteilte Menge von biologisch aktivem Material in Kombination mit einer nicht-thrombogenen Oberfläche enthält, die nützlich zur Beschichtung der Oberflächen von Stents zur vaskulären Implantation ist.
- Der bevorzugte zu beschichtende Stent ist eine selbst-expandierende, erweiterbare, röhrenförmige Stentprothese mit offenen Enden. Obwohl andere Materialien, einschließlich Polymermaterialien, benutzt werden können, ist in der bevorzugten Ausführungsform der röhrenförmige Körper aus einem selbst-expandierenden offenen Geflecht von feinem Einzel- oder Polyfilamentmetalldraht gebildet, welcher sich, ohne zu Kollabieren, biegt, nicht axial zu einer längeren Form zur transluminalen Einführung über einen vaskulären Katheter deformiert und sich elastisch zu vorher bestimmten stabilen Dimensionen nach dem Zurückziehen in situ expandiert.
- In dem Verfahren wird die anfängliche Beschichtung vorzugsweise als eine Mischung, Lösung oder Suspension von polymerem Material und fein verteilten biologisch aktiven Spezies, die in einem organischen Vehikel oder einer Lösung oder partiellen Lösung solcher Spezies in einem Lösungsmittel oder Vehikel für das Polymer und/oder biologisch aktive Spezies verteilt sind. Zum Zweck dieser Anwendung bedeutet der Begriff "fein verteilt" jeden Typ oder Größe von aufgenommenem Material von aufgelösten Molekülen über Suspensionen, Kolloide und bestimmte Gemische. Das aktive Material ist in einem Trägermaterial verteilt, welches das Polymer sein kann, ein Lösungsmittel oder beides. Die Beschichtung ist vorzugsweise als eine Vielzahl von relativ dünnen Lagen aufgetragen, die sequentiell in einer relativ schnellen Sequenz aufgetragen werden und wird vorzugsweise mit dem Stent in einem radial ausgedehnten Zustand aufgetragen.
- Die mehrlagige Beschichtung wird bezeichnet oder gekennzeichnet als eine Unterschicht und eine Deckschicht enthaltend. Das Schichtdickenverhältnis der Deckschicht zu der Unterschicht kann mit dem gewünschten Effekt und/oder dem Eluierungssystem variieren. Diese sind von unterschiedlicher Formulierung, wobei typischerweise das meiste oder alles aktive Material in der Unterschicht enthalten ist und sich eine nicht-thrombogene Oberfläche in der Deckschicht findet.
- Die Beschichtung kann durch Eintauchen oder Besprühen unter Benutzung von verdampfenden Lösungsmaterialien oder relativ hohem Dampfdruck aufgetragen werden, um die gewünschte Viskosität zu produzieren und schnell Beschichtungsschichtdicken herzustellen. Das bevorzugte Verfahren basiert auf dem reziproken Sprühbeschichten eines rotierenden, radial expandierten Stents, unter Benutzung einer Spritzpistolenvorrichtung. Das Beschichtungsverfahren erlaubt dem Material, sich an die gesamte Oberfläche der Filamente der offenen Struktur des Stents anhaftend anzupassen und diese zu bedecken, aber in einer Weise, dass die offene Gitter-Natur der Struktur des Geflechts oder anderer Muster in der beschichteten Vorrichtung erhalten bleibt.
- Die Beschichtung wird der Belüftung bei Raumtemperatur für eine vorherbestimmte Zeit (möglicherweise eine Stunde oder mehr) ausgesetzt, zur Verdampfung von Lösungsmittel. Im Fall bestimmter Unterschichtmaterialien wird das Polymermaterial bei Raumtemperatur oder erhöhten Temperaturen gehärtet. Härtung ist als der Prozess des Umwandelns des elastomeren oder polymeren Materials in den vollendeten oder nützlichen Zustand durch Anwendung von Hitze und/oder chemischer Wirkstoffe definiert, welche physikalisch-chemische Änderungen herbeiführen. Wo z. B. polyurethanthermoplastische Elastomere als Unterschichtmaterial benutzt werden, kann die Verdampfung von Lösungsmitteln bei Raumtemperatur auftreten, wodurch die Unterschicht nützlich zur kontrollierten Freisetzung von Arzneimitteln ohne weiteres Härten gemacht wird.
- Die anwendbare Belüftungszeit und Temperatur zum Härten werden durch das bestimmte benutzte Polymer und die bestimmten benutzten Arzneimitteln bestimmt. Zum Beispiel wurden Silizium- oder Polysiloxanmaterialien (wie etwa Polydimethylsiloxan) erfolgreich benutzt. Urethan-Prepolymere können auch benutzt werden. Anders als die thermoplastischen Polyurethanelastomere werden einige dieser Materialien als Prepolymere in der Beschichtungszusammensetzung aufgetragen und müssen danach hitzegehärtet werden. Die bevorzugten Silikonspezies haben relativ niedrige Härtungstemperaturen und sind als, bei Raumtemperatur, vulkanisierbare (RTV) Materialien bekannt. Manche Polydimethylsiloxanmaterialien können z. B. durch der Luft Aussetzen bei ungefähr 90° für eine Zeitdauer, wie etwa 16 Stunden, gehärtet werden. Ein Härtungsschritt kann sowohl nach Auftragen der Unterschicht oder einer bestimmten Anzahl von unteren Lagen und der oberen Lagen oder als einzelner Härtungsschritt ausgeführt werden, der benutzt wird, nachdem die Beschichtung vollendet ist.
- Die beschichteten Stents können danach einem Nachhärtungsverfahren unterworfen werden, welches die Behandlung mit Edelgasplasma und Sterilisierung, welche Gammabestrahlung umfassen kann, ETO-Behandlung, Elektronenstrahl- oder Dampfbehandlung umfasst.
- In der Plasmabehandlung werden unbelastete beschichtete Stents in eine Reaktorkammer gelegt und das System wird mit Stickstoff gereinigt und ein Vakuum bei 20 bis 50 mTorr angewandt. Danach wird Edelgas (Argon, Helium oder Gemisch von beiden) in die Reaktionskammer zur Plasmabehandlung eingelassen. Ein Verfahren benutzt Argon- (Ar) Gas, das in einem Leistungsbereich von 200 bis 400 Watt arbeitet, einer Strömungsrate von 150 bis 650 Standard ml pro Minute, was äquivalent zu ungefähr 100 bis 450 mTorr ist und einer Aussetzungszeit von 30 Sekunden bis ungefähr 5 Minuten. Die Stents können unmittelbar nach der Plasmabehandlung entfernt werden oder bleiben in der Argonatmosphäre für eine zusätzliche Zeitperiode, typischerweise 5 Minuten.
- Übereinstimmend mit der Erfindung kann die Deckschicht oder Oberflächenbeschichtung in irgendeiner von verschiedenen Weisen aufgebracht werden, um weiter thrombolytische Effekte zu kontrollieren und optional, das Freisetzungsprofil zu kontrollieren, insbesondere die anfänglich sehr hohe Freisetzungsrate, die mit der Elution von Heparin verbunden ist.
- In einer Ausführungsform wird eine Außenschicht von Fluorsilikon (FSi) auf die Unterschicht als Deckschicht aufgetragen. Die äußere Schicht kann auch Heparin enthalten. In einer weiteren Ausführungsform wird Polyethylenglykol (PEG) auf der Oberfläche der Beschichtung festgelegt. In diesem Prozess wird die Unterschicht einer Edelgasplasmabehandlung unterworfen und unmittelbar danach mit Ammoniak (NH3) Plasma behandelt, um die Oberfläche zu aminieren. Aminierung, wie bei dieser Anwendung benutzt, bedeutet die Erzeugung von hauptsächlich Iminogruppen und anderen nitroenthaltenen Spezies auf der Oberfläche. Dies wird durch die unverzügliche Immersion in elektrophil aktivierte Polyethylenglykol (PEG) Lösung mit einem reduktiven Wirkstoff gefolgt, d. h., Natriumcyanoborohydrid.
- Die beschichteten und gehärteten Stents mit der modifizierten äußeren Schicht oder Oberfläche werden einer endgültigen Gammastrahlungssterilisierung bei nominal 2,5 bis 3,5 Mrad unterworfen. Argon (Ar) Plasma behandelte Stents, genießen volle Elastizität nach Bestrahlung, ob sie in einem belasteten oder unbelasteten Zustand ausgesetzt sind, während belastete Stents, die Gammasterilisierung unterworfen werden ohne Ar-Plasmavorbehandlung ihre Elastizität verlieren und sich nicht mit einer genügenden oder geeigneten Rate wiederherstellen.
- Die elastomeren Materialien, welche die Stentbeschichtungsunterlagen bilden, sollte bestimmte Eigenschaften besitzen. Vorzugsweise sollten die Lagen aus geeigneten hydrophobischen biostabilen elastomeren Materialien sein, die sich nicht abbauen. Oberflächenschichtmaterial sollte die Zurückweisung durch das Gewebe und die Entzündung von Gewebe minimieren, und den Einschluss durch die Sten timplantierungsstelle benachbartes Gewebe erlauben. Das ausgesetzte Material ist entworfen, um Verklumpungstendenzen bei kontaktiertem Blut zu reduzieren und die Oberfläche ist vorzugsweise entsprechend modifiziert. Daher werden Unterschichten der obigen Materialien vorzugsweise mit einer äußeren Fluorsilikonschicht bereitgestellt, welche eingebettetes bioaktives Material enthalten kann oder nicht, wie etwa Heparin. Alternativ kann die äußere Schicht im Wesentlichen aus Polyethlyenglykol (PEG), Polysaccarin, Phospholipiden oder Kombinationen der Vorhergehenden bestehen.
- Allgemein für die Unterschichten oder Unterbeschichtungen geeignete Polymere umfassen Silikone (z. B. Polysiloxane und substituiertes Polysiloxane), Polyurethane, thermoplastische Elastomere im Allgemeinen, Ethylenvinylacetatcopolymere, Polyolefinelastomere, Polyamidelastomere und EPDM-Kautchuk. Die oben bezeichneten Materialien werden als hydrophob im Hinblick auf die betrachtete Umgebung der Erfindung angesehen. Oberflächenschichtmaterialien umfassen Fluorsilikone und Polyethylenglykol (PEG), Polysaccharide, Phospholipide und Kombinationen der Vorhergehenden.
- Ein Antibiotikum ist als Wirkstoffmaterial enthalten.
- Geeignete Materialien zur Herstellung des geflochtenen Stents umfassen rostfreien Stahl, Tantalum, Titanlegierungen einschließlich Nitinol (ein Nickellegierungsmaterial mit Thermogedächtnis), und bestimmte Kobaltlegierungen einschließlich Kobalt-Chrom-Nickel-Legierungen wie etwa Elgiloy® und Phynox®. Weitere Details betreffend die Herstellung und Details weiterer Aspekte der Stents selbst können den oben in Bezug genommenen US-Patenten 4 655 771 und 4 954 126 von Wallsten und 5 061 275 von Wallsten et al. entnommen werden, welche hierin durch Bezugnahme aufgenommen werden.
- Verschiedene Kombinationen von Polymerbeschichtungsmaterialien können mit biologisch aktiven Spezies von Interesse zusammenwirken, um gewünschte Ef fekte zu produzieren, wenn zu implantierenden Stents damit in Übereinstimmung mit der Erfindung beschichtet werden. Die Beladungen von therapeutischen Materialien können variieren. Der Mechanismus, der bei Mischung von biologisch aktiven Spezies in die Oberflächenbeschichtung und der Austrittsmechanismus hängen beide von der Natur des Oberflächenbeschichtungspolymers und dem beizumischenden Material ab. Der Mechanismus der Freisetzung hängt auch von Modus der Beimischung ab. Das Material kann über Interpartikelwege eluieren oder via Transport oder Diffusion durch das umschließende Material selbst verabreicht werden.
- Für den Zweck dieser Beschreibung ist "Eluierung" definiert als jeder Prozess des Freisetzens, der die Extraktion oder Freisetzung durch direkten Kontakt des Materials mit Körperflüssigkeiten durch die Interpartikelwege beinhaltet, die mit dem Äußeren der Beschichtung verbunden sind. "Transport" oder "Diffusion" sind definiert, einen Mechanismus der Freisetzung zu umfassen, in welchem das freigesetzte Material durch ein weiteres Material hindurchgeht.
- Das gewünschte Freisetzungsratenprofil kann durch Variieren der Beschichtungsdicke maßgeschneidert werden, der radialen Verteilung (Schicht zu Schicht) von bioaktiven Materialien, der Mischmethode, der Menge an bioaktivem Material, der Kombination von verschiedenen Matrixpolymermaterialien bei verschiedenen Schichten und der Quervernetzungsdichte des polymeren Materials. Die Quervernetzungsdichte hängt von der Menge von Querverbindungen ab, welche auftritt und auch von der relativen Dichte der Matrix, die durch den bestimmten Quervernetzungswirkstoff erzeugt wird. Dies bestimmt während des Aushärtungsprozesses das Ausmaß an Querverbindung und so die Quervernetzungsdichte des Polymermaterials. Für bioaktive Materialien, die von der quervernetzten Matrix freigesetzt werden, wie etwa Heparin, wird eine dichtere Querverbindungsstruktur in einer längeren Freisetzungszeit und einem reduzierten Ausbruchseffekt resultieren.
- Es wird auch anerkannt werden, dass eine dünne, nicht mit Medikamenten versetzte Silikondeckschicht einigen Vorteil und zusätzliche Kontrolle über die Eluierung von Medikamenten bereitstellt; jedoch wurde im Fall von Heparin, z. B., gefunden, dass eine Deckschicht oder Oberflächenbeschichtung, die modifiziert wurde, um das anfängliche Heparinfreisetzungsprofil zur kontrollieren oder die Oberfläche mehr nicht-thrombogen zu machen, einen deutlichen Vorteil bietet.
- Die folgenden bevorzugten Ausführungsformen sind mit Heparin anstatt einem Antibiotikum als Wirkstoffmaterial offenbart, und diese Ausführungsformen dienen deshalb nur als Referenz und sind nicht Teil der Erfindung.

- Gemäß der vorliegenden Erfindung werden die Stentbeschichtungen, die biologisch aktive Materialien beinhalten, zur zeitgesteuerten Abgabe in situ in einem Körperlumen von Interesse, vorzugsweise in vielen dünnen Lagen aus vorbereiteten Beschichtungslösungen oder Suspensionen gesprüht. Die Schritte des Prozesses sind allgemein bei
1 dargestellt. Die Beschichtungslösungen oder Suspensionen werden bei 10 zubereitet, wie später beschrieben wird. Die gewünschte Menge an Quervernetzungswirkstoff (wenn vorhanden) wird der Suspensions-Lösung wie bei 12 hinzugefügt und das Material wird dann bewegt oder gerührt, um eine homogene Beschichtungszusammensetzung bei 14 zu produzieren, welche danach zu einem Aufbringungsbehälter oder -gerät übertragen wird, welches ein Behälter für Sprühfarbe sein kann, bei 16. Typische beispielhafte Zubereitungen von Beschichtungslösungen, die für Heparin und Dexamethason benutzt wurden, erscheinen als nächstes. - Allgemeine Zubereitung von Heparinunterschichtzusammensetzung
- Silikon als Polymervorgänger in Lösungsmittel-(Xylen) Gemisch wurde erhalten. Zum Beispiel wurde ein Gehalt von 35 Gew.-% festem Silikon-Xylen von Ap plied Silicone, Part #40.000 beschafft. Zunächst wurde das Silikon-Xylengemisch gewogen. Der Anteil an festem Silikon wurde gemäß einer Analyse des Verkäufers bestimmt. Vorberechnete Mengen von fein verteiltem Heparin (2 bis 6 Mikrons) wurden dem Silikon hinzugefügt, dann Tetrahydrofuron (THF) von HPCL-Qualität (Aldrich oder EM) hinzugefügt. Für eine 37,5%ige Heparinbeschichtung, z. B.: WSilikon = 5 g; Feststoffanteil = 35%; WHep = 5 × 0,35 × 0,375/(0,625) = 1,05 g. Die Menge von benötigtem THF (44 ml) in der Beschichtungslösung wurde berechnet durch Benutzung der Gleichung WSilikon solid/VTHF = 0,04 für eine 37,5%ige Heparinbeschichtungslösung. Schließlich wurde die Quervernetzerlösung des Herstellers unter Benutzung einer Pasteur P-Pipette hinzugefügt. Die Menge an hinzugefügtem Quervernetzer wurde gebildet, um das Freisetzungsratenprofil zu bewirken. Typischerweise wurden 5 Tropfen von Quervernetzerlösung pro a 5 g Silikon-Xylengemisch hinzugefügt. Die Lösung wurde unter Benutzung des Rührstabs gerührt, bis die Lösung homogen und milchähnlich war. Die Beschichtungslösung wurde dann in einen Farbtopf übertragen, der für die Auftragung mittels Sprühpistole verwendet wird.
- Allgemeine Zubereitung der Dexamethasonunterschichtzusammensetzung
- Silikon (35% Lösung wie oben) wurde auf einer Metler-Waage in einem Kochglas abgewogen. Das Gewicht von Dexamethason-freiem Alkohol oder Azetatform wurde berechnet durch Silikongewicht multipliziert mit 0,35 und dem gewünschten Prozentsatz von Dexamethason (1 bis 40%) und die benötigte Menge wurde dann abgewogen. Beispiel: WSilikon = 5 g; für eine 10% Dexamethasonbeschichtung, Wdex = 5 × 0,35 × 0,1/0,9 = 0,194 g und das in der Beschichtungslösung benötigte THF berechnet. WSilikon solid/VTHF = 0,06 für eine 10% Dexamethasonbeschichtungslösung. Beispiel: WSilikon = 5 g; VTHF = 5 × 0,35/0,06 = 29 ml. Das Dexamethason wurde in einem Kochglas auf einer analytischen Waage abgewogen und die Hälfte der Gesamtmenge des benötigten THF wurde hinzugefügt. Die Lösung wurde gut gerührt, um die vollständige Auflösung des Dexamethason sicherzustellen. Die gerührte DEX-THF-Lösung wurde dann zu dem Silikonbehälter übertragen. Das Kochglas wurde mit dem verbleibenden THF gewaschen und dies wurde zu dem Silikonbehälter übertragen. Der Quervernetzer wurde durch Benutzung einer Pasteur-Pipette hinzugefügt. Typischerweise wurden fünf Tropfen von Quervernetzer für 5 g Silikon benutzt.
- Das Auftragen des Beschichtungsmaterials auf den Stent war sehr ähnlich für alle Materialien und das gleiche für die Heparin- und Dexamethasonsuspensionen, die wie in den obigen Beispielen zubereitet wurden. Die aufzutragende Suspension wurde zu einer Auftragsvorrichtung übertragen, bei
16 in1 . Typischerweise wurde ein Farbtopf benutzt, der an einer Sprühpistole befestigt war, wie etwa einem Badger Modell 150, ausgestattet mit einer Druckluftquelle durch einen Regulator (Norgren, 0 bis 160 psi). Sobald der Pistolenschlauch an der Druckluftquelle stromabwärts des Regulators angeschlossen war, wurde die Luft aufgebracht. Der Druck wurde auf ungefähr 15 bis 25 psi angepasst und der Düsenzustand durch Herunterdrücken des Hebels geprüft. - Ein geeignetes Verfahren kann benutzt werden, um den Stent für das Sprühen zu sichern und drehende Befestigungen wurde im Labor erfolgreich benutzt. Beide Enden des entspannten Stents wurden an der Befestigung durch zwei elastische Zurückhalter befestigt, allgemein Krokodilklemmen, mit zwischen den Klipps angepasstem Abstand, so dass der Stent in einem entspannten, nicht-ausgestreckten Zustand verblieb. Der Rotor wurde dann angeschaltet und die Drehgeschwindigkeit auf die gewünschte Beschichtungsgeschwindigkeit angepasst, nominal ungefähr 40 U/min.
- Während der Stent sich in einer im Wesentlichen horizontalen Ebene dreht, wurde die Spraydüse angepasst, so dass der Abstand von der Düse zu dem Stent ungefähr 2 bis 4 Inch betrug und die Zusammensetzung wurde im Wesentlichen horizontal gesprüht, wobei der Pinsel entlang dem Stent von dem distalen Ende des Stents zu dem proximalen Ende gerichtet war und dann von dem proximalen Ende zu dem distalen Ende in einer schweifenden Bewegung bei einer Geschwindig keit, so dass ein Sprühzyklus in ungefähr drei Stentdrehungen aufgetreten ist. Typischerweise verging eine Pause von weniger als einer Minute, normalerweise ungefähr eine halbe Minute, zwischen Schichten. Selbstverständlich variierte die Anzahl der Beschichtungsschichten und wird mit der jeweiligen Anwendung variieren. Zum Beispiel, was typische Verbindungsschichten wie bei
18 in1 angeht, werden für ein Beschichtungsniveau von 3 bis 4 mg Heparin pro Quadratzentimeter des geplanten Bereichs, 20 Zyklen der Aufbringung von Beschichtung benötigt und ungefähr 30 ml Lösung werden für einen 3,5 mm Durchmesser durch den 14,5 cm langen Stent verbraucht. - Die Rotationsgeschwindigkeit des Motors kann selbstverständlich angepasst werden, wie auch die Viskosität der Zusammensetzung und die Flussrate der Spraydüse wie gewünscht, um die beschichtete Struktur zu modifizieren. Allgemein wurden mit den obigen Mischungen die besten Resultate bei Drehgeschwindigkeiten in dem Bereich 30 bis 50 U/min. und mit einer Sprühdüsenströmungsrate in dem Bereich von 4 bis 10 ml von Beschichtungszusammensetzung pro Minute erreicht, abhängig von der Stentgröße. Es wird anerkannt werden, dass ein ausgeklügelterer, computergesteuerter Beschichtungsapparat den im Labor als machbar gezeigten Prozess erfolgreich automatisieren wird.
- Verschiedene aufgebrachte Lagen bilden das, was bei
18 als Unterschicht bezeichnet wird. Zusätzliche obere Unterschichtlagen, welche von der gleichen oder unterschiedlichen Zusammensetzung sein können, im Hinblick auf das bioaktive Material und den Quervernetzungswirkstoff und die im Hinblick auf die polymeren Matrixmaterialien eine dazu unterschiedliche Zusammensetzung aufweisen, können als Deckschicht wie bei20 aufgebracht werden. Die Aufbringung der Deckschicht folgt derselben Beschichtungsprozedur wie die Unterschicht, wobei die Anzahl und Dicke der Schichten optional ist. Selbstverständlich kann die Dikke jeder Schicht durch Anpassen der Drehgeschwindigkeit des Stents und der Sprühbedingungen angepasst werden. Allgemein wird die Gesamtbeschichtungs dicke kontrolliert durch die Anzahl von Sprühzyklen oder Dünnschichten, welche die gesamte Beschichtung ausmachen. - Wie bei
22 in1 gezeigt, wird der beschichtete Stent dann einem Härtungsschritt unterworfen, in welchem das Prepolymer und die Quervernetzungswirkstoffe zusammenwirken, um eine gehärtete Polymermatrix zu produzieren, welche die biologisch aktiven Spezies enthält. Der Härtungsprozess umfasst Verdampfen des lösenden Xylens, THF, etc. und das Härten und Quervernetzen des Polymers. Bestimmte Silikonmaterialien können bei relativ niedrigen Temperaturen gehärtet werden (d. h. RT-50°C) in einem als Raumtemperatur-Vulkanisierungs-(RTV)-Prozess bekannten Verfahren. Typischer jedoch beinhaltet der Härtungsprozess Hochtemperaturhärtungsmaterialien und die beschichteten Stents werden in einen Ofen bei ungefähr 90° für ungefähr 16 Stunden gestellt. Die Temperatur kann auch bis zu 150°C für die Dexamethason enthaltenden beschichteten Stent erhöht werden. Selbstverständlich können die Zeit und die Temperatur mit bestimmten Silikonen, Quervernetzern und biologisch aktiven Spezies variieren. - In der beschriebenen Weise beschichtete und gehärtete Stents müssen vor dem Verpacken für die zukünftige Implantierung sterilisiert werden. Für die Sterilisierung ist Gammastrahlung eine bevorzugte Methode, insbesondere für heparinenthaltende Beschichtungen; jedoch wurde gefunden, dass Stents, die gemäß dem Prozess der Erfindung beschichtet und gehärtet wurden und einer Gammastrahlensterilisation unterworfen werden, zu langsam sein können, um ihre ursprüngliche Haltung wieder einzunehmen, wenn sie zu einer vaskulären oder einer anderen Lumenstelle unter Benutzung eines Katheters gebracht werden, es sei denn ein Vorbehandlungsschritt, wie bei
24 , wird erst auf den beschichteten, gehärteten Stent angewandt. - Der Vorbehandlungsschritt umfasst eine Behandlung der beschichteten, gehärteten Stents mit Argonplasma in der unbelasteten Konfiguration. In Übereinstimmung mit dieser Prozedur werden die Stents in eine Kammer eines Plasmaober flächenbehandlungssystems, wie etwa einer Plasmascience 350 (Himont/Plasmascience, Foster City, Kalifornien) platziert. Das System ist mit einer Reaktorkammer und einem RF-Festkörpergenerator ausgestattet, der bei 13,56 mHz und von 0 bis 500 Watt-Leistungsabgabe arbeitet und mit einem Mikroprozessor gesteuerten System und einer vollständigen Vakuumpumpeneinheit ausgestattet ist. Die Reaktionskammer enthält ein freies Arbeitsvolumen von 16,75 Inch (42,55 cm) mal 13,5 Inch (34,3 cm) mal 17,5 Inch (44,45 cm) in der Tiefe.
- In dem Plasmaprozess werden unbelastete beschichtete Stents in einer Reaktorkammer platziert und das System wird mit Stickstoff gereinigt und ein Vakuum bei 20 bis 50 mTorr angelegt. Danach wird Edelgas (Argon, Helium oder eine Mischung von diesen) in die Reaktionskammer für die Plasmabehandlung eingelassen. Ein stark bevorzugtes Verfahren des Betriebs besteht in der Benutzung von Argongas, Arbeiten in einem Leistungsbereich von 200 bis 400 Watt, einer Strömungsrate von 150 bis 650 Standard ml pro Minute, was äquivalent zu 100 bis 450 mTorr ist, und einer Aussetzungszeit von 30 Sekunden bis ungefähr 5 Minuten. Die Stents können unmittelbar nach der Plasmabehandlung entfernt werden oder verbleiben in der Argonatmosphäre für eine zusätzliche Zeitdauer, typischerweise 5 Minuten.
- Danach, wie bei
26 gezeigt, können die Stents der Gammasterilisierung bei 2,5 bis 3,5 Mrad ausgesetzt werden. Die Strahlung kann mit dem Stent in entweder dem radial nicht-belasteten Zustand oder in dem radial belasteten Zustand durchgeführt werden. - Vorzugsweise wird die Oberfläche jedoch vor der Plasmabehandlung oder gerade vor der Sterilisierung durch eine oder mehrere zusätzliche Verarbeitungsverfahren modifiziert, von denen einige im Zusammenhang mit den folgenden Referenzbeispielen beschrieben werden.
- Referenzbeispiel 1. Fluorsilikonoberflächenbehandlung von eluierender Heparinbeschichtung
- Die Unterschicht eines Stents wurde mit mehreren aufgebrachten Schichten wie oben beschrieben, beschichtet, und gehärtet wie bei
22 beschrieben. Der Heparingehalt der Unterschicht war 37,5% und die Schichtdicke war ungefähr 30 bis 40 μ. Fluorsilikon-(FSi)-Sprühlösung wurde bei30 aus einer Fluorsilikonsuspension (Applied Silicone, # 40032) durch Abwiegen einer Menge von Fluorsilikonsuspension und Hinzusetzen von Tetrahydrofuran (THF) gemäß der Beziehungsgleichung von VTHF = 1,2 × das Gewicht der Fluorsilikonsuspension zubereitet. Die Lösung wurde sehr gut gerührt und auf den Stent bei32 unter Benutzung der Technik der Aufbringung des Unterschichtverfahrens bei18 aufgesprüht und die beschichteten Stents wurden bei 90° für 16 Stunden gehärtet. Die beschichteten Stents werden mit Argonplasma vor der Gammasterilisierung gemäß den oben beschriebenen Prozeduren in Übereinstimmung mit Schritten22 bis26 behandelt. -
7 ist ein Diagramm von Heparinfreisetzungskinetiken mit Fluorsilikondeckschicht und ohne jede Deckschicht im Phosphatpuffersystem. Die Dicke der Deckschicht ist ungefähr 10 bis 15 μ. Während sie nicht auf dem Diagramm in7 erscheint, sollte bemerkt werden, dass die Freigaberate für die Beschichtung ohne FSi anfänglich ungefähr 25 mal höher als diejenige mit FSi ist, d. h. während der ersten zwei Stunden. Dies ist selbstverständlich klar außerhalb der Skala des Graphen. Es ist jedoch bemerkenswert, dass die Beschichtung mit der FSi-Deckschicht oder Diffusionssperre eine herabgesetzte anfängliche Freisetzungsrate kombiniert mit einer verstärkten Eluierungsrate nach dem ersten Tag und durch die erste Woche bis zu ungefähr dem 10 Tag zeigt. Zusätzlich behält die Fluorsilikon (FSi) Deckschicht, wegen der hohen Elektronegativität Fluoridierung nicht-thrombogene Oberflächenqualitäten während und nach der Eluierung der biologisch aktiven Heparinspezies. Zusätzlich, wegen der negativen Ladungen auf dem Heparin selbst, kann die Elektronegativität der Fluorsilikondeckschicht, zumindest teilweise, verantwortlich für das modifizierte kinetische Heparinfreisetzungsprofil sein. -
8 vergleicht eine Punktkurve eine Fluorsilikon-(FSi)-Deckschicht, die 16,7% eingebettetes Heparin mit einer, welche nur Fluorsilikon (FSi) enthält. Eine Unterschicht ist identisch mit derjenigen, die in7 benutzt wird, die ungefähr 37,5% Heparin bei einer Dicke von ungefähr 30 bis 40 Mikrometern enthält. Diese Eluierungskinetiken sind gut vergleichbar mit der heparinfreien FSi-Deckschicht, was den anfänglichen Ausbruch von Heparinfreisetzung sehr reduziert und andernfalls gewährt das Heparin in der FSi-Deckschicht eine leicht höhere Freisetzung über die Dauer des Tests. - Referenzbeispiel 2. Festlegung von Polyethylenglykol (PEG) auf Arzneimittel eluierender Unterschicht
- Eine Unterschicht wurde auf einem Stent bei
22 geschichtet und gehärtet, wie in Referenzbeispiel 1. Der Stent wurde dann mit Argongasplasma behandelt, wie bei24 , und Ammoniakgasplasma bei40 . Die Vorrichtung und das Verfahren zur Argongasplasmabehandlung war wie oben beschrieben. Die Ammoniakplasmabehandlung wurde unmittelbar nach der Argongasplasmabehandlung durchgeführt, um die Oberfläche der Beschichtung zu aminieren. Die Ammoniakströmungsrate war in dem Bereich von 100 bis 700 cm3 pro Minute (ccM), vorzugsweise in dem Bereich von 500 bis 600 ccM. Die Leistungsabgabe von Radiofrequenzplasma war im Bereich von 50 bis 500 Watt, vorzugsweise bei 200 Watt. Die Verfahrensdauer war im Bereich von 30 Sekunden bis 10 Minuten, vorzugsweise ~5 Minuten. - Unmittelbar nach der Aminierung wurden die Stents in elektrophil aktivierte Polyethylenglykol (PEG) Lösung bei
42 eingetaucht. PEG ist als Hemmer von Proteinabsorption bekannt. Beispiele von elektrophil aktiviertem PEG sind PEG-Nitrophenylkarbonate, PEG-Tresylate, PEG-Glycidylether, PEG-Isocyanate, etc., optional an einem Ende abgeschlossen mit einer Methoxylgruppe. Das Molekulargewicht von PEG war in dem Bereich von ungefähr 1000 bis 6000 und ist vorzugsweise ungefähr 3000. Es wurde beobachtet, dass einfache Amoniumaminierung nicht große Menge von primären und sekundären Aminen auf der elastomeren Polymeroberfläche (z. B. Silikon) erzeugen wird. Stattdessen werden Imine (> C = N – H) und andere, oxidativere nitroenthaltende Gruppen die Oberfläche dominieren. Es ist allgemein notwendig, einen reduktiven Wirkstoff in das Reaktionsmedium hinzuzufügen, wie etwa NaBH3CN, so dass die funktionale Gruppe auf PEG mit Imin reagieren kann und möglicherweise anderen nitroenthaltenden Spezies auf der Oberfläche, und daher PEG auf der Oberfläche festlegen kann. Die typische Konzentration von NaBH3CN ist ungefähr 2 mg/ml. Da PEG und seine Derivative sich in Wasser und vielen polaren und aromatischen Lösungsmitteln auflösen, muss das in der Beschichtung benutzte Lösungsmittel ein Lösungsmittel für PEG, aber nicht für das Medikament in der Unterschicht sein, um den möglichen Verlust des Medikaments durch Auslaugen zu vermeiden. In dem Fall von Heparin eluierender Beschichtung sind ein gemischtes Lösungsmittel aus Formamid und Methylethylketon (MEK) oder ein gemischtes Lösungsmittel aus Formamid und Azeton bevorzugte Lösungsmittel (vorzugsweise im Verhältnis von 30 Formamid zu 70 MEK oder Azeton im Volumen), da sie Heparin nicht auflösen. Die Konzentration von PEG, die Reaktionszeit, die Reaktionstemperatur und der PH-Wert hängen von der Art des verwendeten PEG ab. In Fall von Heparin eluierender Beschichtung wurden 5%iges PEG-Tresylat in (30 bis 70) Formamid/MEK erfolgreich benutzt. Die Reaktionszeit war 3 Stunden bei Raumtemperatur. PEG war dann kovalent an die Oberfläche gebunden. Gammastrahlung wurde dann zur Sterilisierung dieser Ausführungsform, wie zuvor beschrieben, benutzt. - Im Hinblick auf das gerinnungshemmende Material Heparin ist der Prozentsatz in der Unterschicht nominal ungefähr 30 bis 50% und derjenige der Deckschicht ungefähr 0 bis 30% aktives Material. Das Beschichtungsdickenverhältnis der Deckschicht zur Unterschicht variiert von ungefähr 1 : 10 zu 1 : 2 und ist vorzugsweise in dem Bereich von ungefähr 1 : 6 bis 1 : 3.
- Die Unterdrückung des Ausbrucheffekts ermöglicht auch eine Reduktion der Medikamentenladung oder in anderen Worten, erlaubt eine Reduktion in der Beschichtungsdicke, da der Arzt dem Patienten eine Bolus-Injektion von Antiblutblättchen-/Gerinnungshemmermedikamenten während des Stenteinsetzungsprozesses geben wird. Als Ergebnis kann das in den Stent eingebettete Medikament ohne Verschwendung voll benutzt werden. Zuschneiden der Freigabe des ersten Tages, jedoch Maximierung der Freigabe des zweiten und dritten Tages bei der dünnstmöglichen Beschichtungskonfiguration wird die akute oder subakute Thrombose reduzieren.

4 stellt den allgemeinen Effekt der Medikamentenbeladung für Beschichtungen von ähnlicher Dicke dar. Die anfängliche Eluierungsrate erhöht sich mit der Medikamentenbeladung, wie in5 gezeigt. Die Freigaberate erhöht sich mit der Dicke der Beschichtung bei gleicher Ladung, tendiert jedoch dazu, umgekehrt proportional zu der Dicke der Deckschicht zu sein, wie durch die gleiche Medikamentenbeladung und ähnliche Unterschichtdicke in6 gezeigt. - Was sich aus den bis jetzt erhobenen Daten ergibt, ist jedoch, dass der Prozess der vorliegenden Erfindung erlaubt, die Medikamenteneluierungskinetik in einer gewünschten Weise zu kontrollieren, um den Bedarf der bestimmten Stentanwendung zu befriedigen. In ähnlicher Weise können Stentbeschichtungen unter Benutzung einer Kombination von 2 oder mehr Medikamenten vorbereitet werden und die Medikamentenfreigabesequenz und Rate kontrolliert werden. Zum Beispiel können Antiproliferationsmedikamente in der Unterschicht mit Antiblutblättchenmedikamenten in der Oberschicht kombiniert werden. In dieser Weise werden die Antiblutblättchenmedikamente, z. B. Heparin, zuerst eluieren, gefolgt von Antiproliferationsmedikamenten, um eine sichere Einschließung des implantierten Stents besser zu ermöglichen.
- Die Messungen der Heparinkonzentration wurden unter Benutzung einer Standardkurve durchgeführt, die durch Komplexbildung eines Azur A Farbstoffs mit verdünnten Heparinlösungen vorbereitet wurde. 16 Standards wurden benutzt, um die Standardkurve in einer bekannten Weise zusammenzustellen.
- Für den Eluierungstest wurden die Stents in eine Phosphatpufferlösung bei pH 7,4 in einem Inkubator bei ungefähr 37°C eingetaucht. Periodische Proben der Lösung wurden verarbeitet, um die Menge des eluierten Heparins zu bestimmen. Nach jeder Probe wurde jeder Stent in heparinfreie Pufferlösung gelegt.
- Wie oben festgestellt, kann, während die erlaubte Ladung des elastomeren Materials mit Heparin variieren kann, im Fall von Silikonmaterialien, Heparin 60% des Gesamtgewichts der Schicht überschreiten. Jedoch ist die am vorteilhaftesten benutze Beladung allgemein im Bereich von ungefähr 10% bis 45% des Gesamtgewichts der Schicht. Im Fall von Dexamethason kann die Ladung 50% oder mehr des Gesamtgewichts der Schicht betragen, ist aber vorzugsweise im Bereich von ungefähr 0,4% bis 45%.
- Es wird anerkannt werden, dass der Mechanismus der Einschließung der biologisch aktiven Spezies in eine dünne Oberflächenbeschichtungsstruktur, die auf einen Metallstent auftragbar ist, ein wichtiger Aspekt der vorliegenden Erfindung ist. Der Bedarf an relativ dickwandigen Polymereluierungsstents oder jeglicher Membranüberzüge, die mit vielen vorhergehenden Arzneimitteleluierungsgeräten verbunden sind, wird überflüssig gemacht, wie auch der Bedarf zur Benutzung bioabbaubarer oder absorbierbarer Geräte zum Tragen der biologisch aktiven Spezies. Die Technik ermöglicht klar die Langzeitabgabe und minimiert die Beeinträchtigung der unabhängigen mechanischen oder therapeutischen Vorzüge des Stents selbst.
- Beschichtungsmaterialien sind im Hinblick auf eine bestimmte Beschichtungstechnik, Beschichtungs-/Arzneimittel-Kombination und einen bestimmten Arzneimittelinfusionsmechanismus entworfen. Die Berücksichtigung der besonderen Form und des Mechanismus der Freisetzung von biologisch aktiven Spezies in der Beschichtung erlaubt der Technik überlegene Resultate zu produzieren. In dieser Weise kann die Abgabe der biologisch aktiven Spezies von der Beschichtungsstruktur maßgeschneidert werden, um eine Vielzahl von Anwendungen zu ermöglichen.
- Während die obigen Beispiele Beschichtungen darstellen, die zwei verschiedene Medikamentenbeladungen oder Prozentsätze von biologisch aktivem Material, das freizusetzen ist, besitzen, ist dies in keiner Weise im Hinblick auf die Erfindung beschränkend und es wird anerkannt werden, dass jegliche Anzahl von Schichten und Kombinationen von Ladungen verwendet werden kann, um ein gewünschtes Freigabeprofil zu erreichen. Zum Beispiel kann eine gradweise Abstufung und Änderung der Beladung der Schichten benutzt werden, in welcher z. B. höhere Beladungen in den inneren Schichten benutzt werden. Auch können Schichten benutzt werden, die keine Arzneimittelbeladungen besitzen. Zum Beispiel kann ein pulsierendes Heparinfreigabesystem durch eine Beschichtung erreicht werden, in welcher abwechselnde Schichten mit Heparin zwischen unbeladene Schichten von Silikon oder anderen Materialien für einen Bereich der Beschichtung eingeschoben sind. Mit anderen Worten erlaubt die Erfindung ungenannte Anzahlen von Kombinationen, die in einem großen Ausmaß von Flexibilität im Hinblick auf das Kontrollieren der Freigabe von biologisch aktiven Materialien im Hinblick auf einen implantierten Stent erlauben. Jede aufgetragene Schicht ist typischerweise ungefähr 0,5 Mikrometers bis 15 Mikrometers dick. Die gesamte Anzahl von aufgesprühten Schichten, kann natürlich stark variieren, von weniger als 10 zu mehr als 50 Schichten; allgemein sind 20 bis 40 Schichten eingeschlossen. Die Gesamtdicke der Beschichtung kann auch stark variieren, kann aber allgemein von ungefähr 10 bis 200 Mikrometer sein.
- Während das Polymer der Beschichtung jedes kompatible biostabile elastomere Material sein kann, das fähig ist, als dünne Schicht auf dem Stentmaterial angehaftet zu werden, werden hydrophobische Materialien bevorzugt, da gefunden wurde, dass die Freisetzung von biologisch aktiven Spezies im Allgemeinen vorhersagbarer mit solchen Materialien kontrolliert werden kann. Bevorzugte Materialien umfassen insbesondere Silikonkautschukelastomere und biostabiles Polyurethan.
- Diese Erfindung wurde hier in erheblichem Detail beschrieben, um den Patentgesetzen zu entsprechen und um den Fachmann mit der Information zu versorgen, die notwendig ist, um die neuen Prinzipien anzuwenden und Ausführungsformen des Beispiels, wie benötigt, zu konstruieren und zu benutzen.
Claims (8)
- Implantierbare medizinische Vorrichtung mit einer äußeren Oberfläche, die zumindest teilweise mit einem konformen Überzug überzogen ist, umfassend eine untere Beschichtung aus einem hydrophoben biostabilen elastomeren Material, das nicht abbaut, enthaltend eine Menge eines Antibiotikums zur zeitgesteuerten Freisetzung daraus und einer oberen Beschichtung umfassend ein nicht-thrombogenes polymeres Material, das die untere Beschichtung zumindest teilweise bedeckt, wobei die obere Beschichtung und die untere Beschichtung hinsichtlich des polymeren Matrixmaterials unterschiedliche Formulierungen aufweisen, und die implantierbare medizinische Vorrichtung ein Stent für die vaskuläre Implantation ist.
- Vorrichtung nach Anspruch 1, wobei die obere Beschichtung aus dem nicht-thrombogenen polymeren Material besteht.
- Vorrichtung nach einem der Ansprüche 1 oder 2, wobei die Vorrichtung ein entfaltbarer Stent ist.
- Vorrichtung nach Anspruch 3, wobei der Stent einen Rohrköper mit offenen Enden und einer offenen Gitterseitenwandstruktur umfaßt, und wobei der Überzug so mit der Seitenwandstruktur konform ist, daß das offene Gitter beibehalten wird.
- Vorrichtung nach einem der Ansprüche 1 bis 4, wobei der meiste oder der gesamte Wirkstoff in der unteren Beschichtung enthalten ist.
- Vorrichtung nach einem der Ansprüche 3 bis 5, wobei der Stent aus rostfreiem Stahl ist.
- Verfahren zur Herstellung einer Vorrichtung nach einem der Ansprüche 1 bis 6, wobei das Verfahren die Schritte umfaßt: (a) Aufbringen einer unteren Beschichtung aus einer Formulierung enthaltend nichtvernetztes hydrophobes elastomeres Material im Gemisch mit Lösemittel und eine Menge des Antibiotikums, das fein verteilt ist; (b) Vernetzen des hydrophoben elastomeren Materials; und (c) Aufbringen einer oberen Beschichtung einer Formulierung umfassend das nicht-thrombogene polymere Material zur Bildung der oberen Beschichtung.
- Verfahren nach Anspruch 7, wobei die Vorrichtung ein entfaltbarer Stent aus rostfreiem Stahl ist.
