- 1. Technisches Gebiet
- Die vorliegende Offenbarung betrifft im Allgemeinen Instrumente zum Einführen von Implantaten, und insbesondere solche Instrumente zur Einführung von spinalen Implantaten bzw. Wirbelsäulenimplantaten, um die Fusion benachbarter Wirbelkörper zu vereinfachen.
- 2. Beschreibung des Standes der Technik
- Eine große Anzahl orthopädischer Verfahren beinhaltet das Einführen von entweder natürlichen oder prothetischen Implantaten in Knochen oder zugehöriges Gewebe. Diese Verfahren umfassen, z. B. die Operation an Bändern, Gelenken oder der Ersatz von Gelenken, Frakturen, Oberflächenoperationen, die spinale Stabilisation und spinale Fusion. Bei einem typischen Verfahren wird ein Einsatz, ein Dübel oder eine Schraube in eine bereits im Knochen oder Gewebe gebildete Bohrung eingeführt, um die Operation sowie den Heilungsprozess zu vereinfachen. Siehe z. B. die US-Patente Nr.: 5,470,334 nach Ross et al.; 5,454,811 nach Huebner; 5,480,403 nach Lee et al.; 5,400,805 nach Warren; 5,358,511 nach Gatturna et al.; und 4,877,020 nach Vich.
- Einige Implantate sind insbesondere mit Hohlräumen und Bohrungen ausgebildet, um den Knocheneinwuchs zu vereinfachen und das Verankern des Implantats an der Einführstelle zu verbessern. Siehe z. B. die US-Patente Nr.: 4,328,593 nach Sutter et al.; 4,936,851 nach Fox et al.; und 4,878,915 nach Brantigan. Implantate in der Form von Fusionskäfigen mit internen Hohlräumen, um knochenwachstumsstimulierende Materialien, wie z. B. Knochensplitter und -fragmente aufzunehmen, sind, z. B., in den US-Patenten Nr.: 4,501,269 nach Bagby; 4,961,740 nach Ray et al.; 5,015,247 nach Michaelson; und 5,489,307 nach Kuslich et al. offenbart. Diese Implantate sind besonders für inter-vertebrale spinale Fusionsverfahren, die durch Verletzung, Krankheit oder einige degenerative Fehler der Bandscheibe notwendig werden, geeignet. Anschließend kann es zu einer fortschreitenden Degeneration kommen, die zu einer mechanischen Instabilität zwischen benachbarten Wirbeln führt, was wiederum eine direkte Fusion der Wirbel notwendig macht, bei der ein vorbestimmter Zwischenwirbelraum aufrecht erhalten wird. Diese Fusion kann dadurch erzielt werden, indem ein oder mehrere spezielle Implantate eingeführt werden, wie sie zuvor beschrieben wurden und ebenso in dem gemeinsam übertragenen US-Patent Nr. 5,026,373 beschreiben sind, und auf deren Inhalt der Fachmann für weitere Details verwiesen wird.
- Sowohl anteriore (trans-abdominale) und posteriore chirurgische Vorgehensweisen werden für zwischen den einzelnen Körpern stattfindenden Fusionen der Lendenwirbelsäule verwendet. Fusionen im zervikalen Bereich der Wirbelsäule werden vorwiegend durchgeführt unter Verwendung einer posterioren Vorangehensweise. Typischerweise wird ein Implantat, wie z. B. ein Einsatz ("plug"), ein Dübel, eine Prothese oder ein Käfig in einen vorgeformten Hohlraum innerhalb des interdiskalen Raumes eingeführt. Da es bei diesen Verfahren wünschenswert ist, eine "Knochen-Knochen"-Brücke zu fördern, wird verbindendes Gewebe und zumindest ein Abschnitt des distalen Gewebes entfernt. Vorzugsweise werden relativ tiefe Schnitte in die benachbarten Knochen vorgenommen, um in den weicheren, mehr vaskularisierten, spongiösen Bereich vorzudringen, um das Knochenwachstum quer über das Implantat zu erleichtern.
- Eine oder mehrere kritische Aufgaben, die bei der Einführung eines chirurgischen Fusionsimplantats, insbesondere bei der inter-vertebralen spinalen Fusion, durchgeführt werden, ist die Bildung des das Implantat aufnehmenden Hohlraumes oder der Bohrung zwischen/innerhalb benachbarter Wirbel. Insbesondere muss die gebohrte Bohrung gleichmäßig mittig innerhalb des inter-vertebralen Raumes sein, und vorzugsweise parallel zu den vertebralen Endplatten, um so das Entfernen von gleichen Knochenmengen aus den benachbarten Wirbeln über die gesamte Länge des Schnittes und das anschließende korrekte Einsetzen des Implantats relativ zu den Wirbelkörpern sicher zu stellen.
- Chirurgische Instrumente für die Einführung von spinalen Fusionsimplantaten sind bekannt. Zum Beispiel offenbart das US-Patent Nr. 5,484,437 nach Michaelson ein Verfahren und eine Vorrichtung, die eine äußere und eine innere Hülsenanordnung beinhaltet. Die Außenhülse befindet sich über dem spinalen Distraktor und besitzt an einem Ende Zähne, die direkt in die posteriore Oberfläche des benachbarten Wirbels getrieben werden. Die Innenhülse befindet sich innerhalb der Außenhülse und dient dazu, die Instrumente zu führen, wie z. B. einen Bohrer, der dazu verwendet wird, um die das Implantat aufnehmende Bohrung zu bilden. Die US-Patente Nr. 5,487,307 nach Kuslich et al.; 5,015,247 nach Michaelson; und 4,878,915 nach Brantigan beschreiben ebenso Außenhülsen mit Zähnen, die an den Wirbeln befestigt werden. Andere Anordnungen umfassen die Verwendung von Führungsstäben, die in Führungslöcher gesetzt werden, welche in den Wirbelkörpern gebildet sind. Die Führungsstäbe führen einen die Bohrung bildenden hohlförmigen Bohrer in den inter-vertebralen Raum.
- Obwohl einige gegenwärtige Instrumente und dazugehörige Verfahren zum Verbessern des Einsatzes von spinalen Fusionsimplantaten für ihre Zwecke im Allgemeinen sehr wirkungsvoll sind, so gibt es jedoch gewisse Beschränkungen hinsichtlich des Designs dieser Instrumente, die wiederum die Brauchbarkeit einschränken. Zum Beispiel sieht die Anordnung, die in dem Michaelson '437-Patent offenbart ist, als auch ähnliche Anordnungen keine automatische Ausrichtung der Außenhülse vor, um so sicher zu stellen, dass die Bohrung, die durch einen Bohrer gebildet wird, welcher in die Außenhülse eingeführt wird, sich in optimaler Ausrichtung für einen Gewindeschneidvorgang (falls erforderlich) und das Aufnehmen des spinalen Implantats befindet. Eine solche Orientierung hängt eher von den Fähigkeiten des Chirurgen ab. Überdies kann sich die Außenhülse, die über Zähne lediglich an ihrem äußersten distalen Ende an der posterioren Oberfläche des benachbarten Wirbels befestigt ist, ihre Orientierung ändern oder sich während der Einführung und/oder beim Entfernen des Bohrers und/oder des Gewindeschneidinstruments verschieben. Die Verwendung von Führungsstäben erhöht die Anzahl der Schritte, die erforderlich sind, um den Fusionskäfig zu implantieren.
- Die
DE 43 28 690 A1 offenbart eine Vorrichtung zum Fixieren von Wirbel relativ zueinander, wobei Abstandsstücke in den inter-vertebralen Raum eingeführt werden, nachdem der Raum erzeugt worden ist, indem benachbarte Wirbel mit Hilfe separater Mittel, z. B. einer Spreizzange, gespreizt werden. Entsprechend sind eine Mehrzahl struktureller Elemente und getrennter Schritte notwendig, um zwei gegenüberliegende Wirbel auszulenken bzw. zu spreizen. - Entsprechend betrifft die vorliegende Offenbarung ein Instrument, das die Einführung eines Fusionsimplantats erleichtert, und welches eine optimale Ausrichtung der Bohrung zur Aufnahme des Fusionsimplantats, und, falls erforderlich, der Gewindeschneidvorgänge sicherstellt. Das Instrument der vorliegenden Offenbarung verringert ebenso die Anzahl der Schritte, die erforderlich sind für die Implantation des Fusionskäfigs.
- ZUSAMMENFASSUNG
- Die vorliegende Offenbarung betrifft Instrumente, die beim Einsetzen eines spinalen Fusionsimplantats verwendet werden. Insbesondere ist ein chirurgischer Wundspreizer gemäß Anspruch 1 vorgesehen, der ein längliches Element mit einem proximalen und einem distalen Endabschnitt umfasst und eine längliche Durchführung zur Aufnahme chirurgischer Instrumente bestimmt. Der distale Endabschnitt des Elements umfasst einen ersten und einen zweiten Spreizarm, die sich im Allgemeinen in Längsrichtung erstrecken. Jeder Spreizarm besitzt eine erste und eine zweite Auflageoberfläche zum Ineingriffbringen von gegenüberliegenden benachbarten Gewebeabschnitten, z. B. gegenüberliegende Wirbelkörper. Jeder Spreizarm bestimmt eine Abmessung zwischen der ersten und der zweiten Auflageoberfläche, die ausreicht, die gegenüberliegenden Gewebeabschnitte, z. B. Wirbelkörper, bei dessen Eintritt auszulenken bzw. zu spreizen. Jeder Spreizarm besitzt einen konisch zulaufenden distalen Abschnitt, der das Eintreten in den inter-vertebralen Raum vereinfacht. Die erste und die zweite Auflageoberfläche eines jeden Spreizarmes verläuft vorzugsweise im Allgemeinen parallel zueinander und zur Längsachse des Hülsenelements. Die erste und die zweite Auflageoberfläche eines jeden Spreizarmes.
- KURZE BESCHREIBUNG DER ZEICHNUNGEN
- Bevorzugte Ausführungsformen der Offenbarung werden mit Bezug auf die Zeichnungen im Anschluss beschrieben, von denen:

13 eine vergrößerte Draufsicht im Teilschnitt von zwei Implantaten ist, die in dem inter-vertebralen Raum im Lendenwirbelsäulenbereich angeordnet sind. - DETAILLIERTE BESCHREIBUNG DER BEVORZUGTEN AUSFÜHRUNGSFORM
- Die bevorzugten Ausführungsformen des hierin beschriebenen Verfahrens und des Instruments werden hinsichtlich orthopädischer spinaler Fusionsprozeduren und Instrumente beschrieben. Es ist ebenso in Betracht gezogen worden, dass die Offenbarung auf eine große Anzahl von Prozeduren anwendbar ist, einschließlich, aber nicht beschränkt auf, Operationen an Bändern, Gelenken oder deren Austausch, Frakturen, Oberflächenrekonstruktion und die spinale Stabilisation. Zusätzlich wird davon ausgegangen, dass das vorliegende Verfahren und Instrument Anwendung sowohl bei offenen und minimal invasiven Prozeduren, einschließlich endoskopischen und athroskopischen Prozeduren, findet, wobei Zugang zur Operationsstelle über eine Kanüle oder eine kleine Inzision erzielt wird.
- Die folgende Diskussion umfasst eine Beschreibung eines jeden Instruments gemäß der vorliegenden Erfindung, die beim Ausführen einer spinalen Fusion verwendet wird, an die sich eine Beschreibung des bevorzugten Verfahrens für die spinale Fusion unter Verwendung des Instruments anschließt.
- In der im Anschluss folgenden Diskussion bezieht sich der Ausdruck "proximal", wie gewöhnlich, auf den Abschnitt des Aufbaus, der dem Bediener nächstgelegen ist, während der Ausdruck "distal" sich auf den Abschnitt bezieht, der von dem Bediener entfernt gelegen ist.
- Bezugnehmend nun auf die Zeichnungen, in denen gleiche Bezugszeichen ähnliche oder identische Elemente in sämtlichen Ansichten anzeigen, stellt
1 eine perspektivische Ansicht eines chirurgischen Wundspreizers der vorliegenden Offenbarung dar. Der Wundspreizer10 dient insbesondere zum Spreizen benachbarter Knochenstrukturen, z. B. benachbarter, gegenüberliegender Wirbelkörper, um das Einführen und Einsetzen eines Implantats zu vereinfachen, und um so eine Kanüle für die Einführung der Instrumente vorzusehen und eine korrekte Ausrichtung der Instrumente und das genaue Einführen des Implantats sicher zu stellen. Obwohl der Wundspreizer für spinale Prozeduren beschrieben ist, so kann er ebenso dazu verwendet werden, andere Strukturen auszulenken bzw. zu spreizen, z. B. Gelenke, Bänder etc. - Bezugnehmend nun auf die
1 und2 umfasst der Wundspreizer10 eine Hülse12 , die eine Längsachse "a" bestimmt und einen vergrößerten Kopf14 besitzt, der am proximalen Ende derselben angeordnet ist. Die Hülse12 bestimmt eine sich hierdurch erstreckende Längsöffnung, um die im Anschluss beschriebenen Instrumente aufzunehmen. Die Hülse12 und der vergrößerte Kopf14 sind vorzugsweise monolithisch aus einem geeigneten festen Material gebildet, einschließlich rostfreiem Stahl, einer Aluminiumlegierung oder dergleichen. Die Hülse12 kann ebenso aus einem geeigneten Polymer gebildet sein. Die Hülse12 kann unterschiedlich groß ausgebildet sein, z. B. mit einem Durchmesser von 12 mm, 14 mm, 16 mm oder 18 mm. Die verwendete Wundspreizergröße entspricht im Allgemeinen dem Durchmesser des zu verwendenden Instruments und/oder des Implantats. - Die Hülse
12 kann eine erste und einen zweite, sich in Längsrichtung erstreckende Öffnung16 , die an ihrer Außenwand gebildet ist, umfassen. Die Öffnungen16 sind diametral in Bezug zueinander angeordnet und sind an ihren distalen Enden durch einen Bund18 begrenzt. Jede Öffnung16 erstreckt sich in radialer Richtung um ungefähr 10 bis 50% des Umfangs oder der Umfangslänge der Hülse12 , und in Längsrichtung über mehr als 50% der Hülsenlänge12 . Die Öffnungen16 gestatten das seitliche Einführen chirurgischer Instrumente, die erforderlich sind, um die Fusionsprozedur auszuführen, quasi als Alternative zum Einführen der Instrumente durch das offene proximale Ende der Hülse12 . Diese Öffnungen16 verbessern ebenso die Beleuchtung der Operationsstelle. - Die Hülse
12 umfasst ferner einen ersten und einen zweiten, diametral gegenüberliegenden Spreizarm oder Finger20 . Die Spreizarme20 erstrecken sich in distaler Richtung von dem Bund18 im Allgemeinen in Längsrichtung parallel zueinander und bestimmen einen in Längsrichtung verlaufenden, geschlitzten Abschnitt22 . Jeder Arm20 besitzt eine bogenförmige Außenoberfläche (d. h. jeder Arm bestimmt einen Krümmungsradius, der im Wesentlichen gleich dem Krümmungsradius des verbleibenden Abschnitts der Hülse ist). - Jeder Spreizarm
20 besitzt eine erste und eine zweite Wirbelauflageoberfläche20a ,20b , die im Allgemeinen parallel zueinander sind und vorzugsweise parallel zur Längsachse der Hülse12 verlaufen. In der dargestellten Ausführungsform sind die Auflageoberflächen20a ,20b im Wesentlichen planar. Die Höhe "h" eines jeden Arms20 (d. h. der Abstand zwischen den Auflageoberflächen20a ,20b ) entspricht der Höhe des beabsichtigten Spreizabstandes zwischen benachbarten Gewebeabschnitten, d. h. benachbarter Wirbel. Zum Beispiel liegt bei einer spinalen Fusionsanwendung die Höhe "h" eines jeden Arms20 im Bereich von ungefähr 0,3 bis 0,4 inch, und bevorzugt im Bereich von ungefähr 0,28 bis ungefähr 0,35 inch. Dem Fachmann ist klar, dass diese Abmessung von der Prozedur abhängig ist und bei Bedarf variiert werden kann. Jeder Arm20 umfasst ferner verjüngt ausgebildete bzw. konisch zulaufende Endabschnitte23 , die eine im Allgemeinen V-förmige Konfiguration bestimmen. Die Endabschnitte23 erleichtern das Einführen der Spreizarme20 in die Operationsstelle, z. B. in den inter-vertebralen Raum. - Bezugnehmend auf die
1 und2 kann eine Stoßendkappe26 über dem vergrößerten Kopf14 positioniert werden, und besitzt vorzugsweise einen Innendurchmesser, der mehr oder weniger gleich dem Außendurchmesser des Kopfes14 ist, um so eine lösbare Reibungspassung zwischen den beiden Komponenten zu bilden. Die Stoßkappe26 dient dazu, den Stoß eines Treibinstruments abzufangen, das dazu verwendet wird, um den Spreizer10 in das Knochengewebe, wie dies im Anschluss beschrieben wird, einzuführen. Ein solcher Stoß treibt die Arme20 der Hülse12 in den Scheibenraum (wobei die Höhe h den Raum einnimmt) und spreizt die gegenüberliegenden Wirbelkörper, da die Oberflächen20a in Eingriff mit dem oberen (oder unteren) Wirbelkörper treten und die Oberflächen20b in Eingriff mit dem gegenüberliegenden Wirbelkörper treten, wodurch der Spreizer20 auf feste Weise befestigt wird, um seine Ausrichtung und Orientierung beizubehalten und sicher zu stellen, dass eine gleiche Materialmenge an beiden vertebralen Endplatten abgetrennt wird, falls ein Bohrer hierdurch eingeführt wird (wie unten beschrieben). - Anwendung des Instruments
- Die Verwendung des Instrumentenkitts in Verbindung mit dem Einführen eines Fusionskäfigs
20 in einen inter-vertebralen Raum, der zwischen benachbarten Lendenwirbeln bestimmt ist, wird nun beschrieben. Die im Anschluss folgende Beschreibung konzentriert sich insbesondere auf eine offene, posteriore spinale Fusionsprozedur, es ist jedoch klar, dass eine anteriore Herangehensweise ebenso in Betracht gezogen werden kann. - Der inter-vertebrale Raum wird unter Verwendung geeigneter Spreizer, z. B. laminarer Spreizer, duralen Extraktoren zugängig gemacht, um die posteriore Wirbeloberfläche freizulegen. Anschließend wird der Spreizer
10 der1 mit der daran befestigten Stoßkappe26 benachbart dem inter-vertebralen Raum positioniert. Bezugnehmend auf die3 werden die Spreizarme20 in den inter-vertebralen Raum eingeführt, und der Spreizer10 wird auf sanfte Weise in den Raum mit Hilfe eines Gummihammers getrieben. Die bevorzugte Orientierung der Spreizarme20 innerhalb des inter-vertebralen Raums ist in der3 gezeigt. Wie dargestellt, sind die Spreizarme20 derart angeordnet, dass die erste und die zweite Auflageoberfläche20a ,20b eines jeden Spreizarmes jeweils in Eingriff mit den gegenüberliegenden Wirbelkörpern V1, V2 treten. Nach Einführen der Spreizarme20 werden die Wirbelkörper V1, V2 auseinandergespreizt, wodurch die Spreizarme20 auf feste Weise in dem inter-vertebralen Raum fixiert werden. Die Anordnung der Spreizarme20 sieht einen Doppelpunkt-Kontakt mit jedem Wirbelkörper vor (gekrümmte Endplatte), d. h. die erste Auflageoberfläche20a des Spreizarms20 tritt in Eingriff mit dem Wirbelkörper V1 an zwei unterschiedlichen Stellen, die zueinander beabstandet sind. Die zweite Auflageoberfläche20b tritt in Eingriff mit dem Wirbelkörper V2 auf die gleiche Weise. Auf diese Weise wird die Last, die von den Wirbelkörpern V1, V2 ausgeübt wird, auf zwei verschiedene Stellen auf dem Spreizer und entlang der Gesamtlänge der Auflageoberflächen20a ,20b verteilt, wodurch der Spreizer10 in dem inter-vertebralen Raum fest und gleichmäßig eingebracht wird. Es wird ebenso darauf hingewiesen, wie zuvor beschrieben wurde, dass die besondere Anordnung der Spreizarme20 in dem inter-vertebralen Raum automatisch und korrekt den Spreizer10 in Bezug auf die Wirbelkörper V1, V2 ausrichtet, d. h. parallel zu den vertebralen Endplatten für den sich anschließenden Bohrprozess. Die verjüngt ausgebildeten Oberflächen24 der Spreizarme20 vereinfachen das Eintreten der Spreizarme20 in den inter-vertebralen Raum. Die Eindringtiefe der Spreizarme20 wird durch den Bund18 , wie zuvor beschrieben, beschränkt. - Bezugnehmend nun auf die
4 wird ein Bohrinstrument40 verwendet, um den Scheibenraum und die vertebralen Endplatten für die Einführung des Fusionsimplantats vorzubereiten. Die Schneidtiefe des Bohrinstruments40 wird, wie erwünscht, eingestellt (d. h. entsprechend der Länge des Fusionskäfigs), indem die Position des Bundes56 eingestellt und der Bund56 an der erwünschten Position mit dem Arretierbund58 , wie zuvor beschrieben, gesichert wird. Mit einem T-Griff, der an dem Bohrinstrument40 befestigt ist (nicht in den Figuren gezeigt), wird das Instrument in den Spreizer10 eingeführt und so weit vorwärts bewegt, bis er die posteriore Oberfläche der Wirbelkörper V1, V2 berührt. Der Bohrer48 steht nun in Verbindung mit den Wirbelkörpern V1, V2 über die geschlitzte Öffnung22 , die zwischen den Spreizarmen20 bestimmt ist (1 ). Das Bohrinstrument40 wird in den inter-vertebralen Raum vorwärts bewegt, indem der T-Griff in Richtung, die durch den Richtungspfeil der4 angezeigt ist, gedreht wird, bis der Positionierbund56 in Eingriff mit dem proximalen Ende des vergrößerten Kopfes18 des Spreizers10 tritt. Dies bewirkt das Abscheren des weichen Gewebes und schneidet den Knochen. Tiefenmarkierungen werden ebenso zur Unterstützung des Chirurgen beobachtet. Anschließend wird das Bohrinstrument40 entfernt, indem der T-Griff in entgegengesetzte Richtung gedreht wird und das Instrument40 von dem Spreizer10 entfernt wird. - Wenn nebeneinanderliegende Seiten benachbarter vertebraler Scheiben bzw. Bandscheiben auf geeignete Weise vorbereitet worden sind, indem die Löcher gebohrt und das verbleibende weiche Gewebe vollständig entfernt worden ist, wird ein Gewindeschneidinstrument
70 sodann ausgewählt und an dem T-Griff angebracht. Der Zweck des Gewindeschneidinstruments70 ist es, Gewinde in die gegenüberliegenden vertebralen Endplatten zu schneiden. Dies stellt sicher, dass das Implantat korrekt positioniert wird und den korrekten Ansatzpunkt an den Endplatten für den sofortigen Kontakt zwischen Knochengraftmaterial und Endplatte besitzt. Bezugnehmend auf die5 wird das Gewindeschneidinstrument70 in den Spreizer10 eingeführt und benachbart der Bohrung positioniert. Der T-Griff wird in Richtung des Richtungspfeils der5 gedreht, und zwar unter Verwendung des Spreizers10 als direkte Führung, während gleichzeitig ein ausreichender, nach unten gerichteter (distaler) Druck auf den T-Griff ausgeübt wird, um das Gewindeschneidinstrument70 zu bewegen und einen gleichmäßigen Ansatzpunkt bzw. Hebelwirkung an den Endplatten sicher zu stellen. Beim Vorwärtsbewegen des Gewindeschneidinstruments70 sammeln sich die entgrateten Knochenchips in einem Förderkanal des Gewindeschneidkopfes74 an und werden in proximaler Richtung bei der Drehbewegung des Gewindeschneidkopfes von der Gewindeschneidstelle weg bewegt. Das Gewindeschneidinstrument70 wird in den Knochen so weit bewegt, bis die erwünschte Tiefe erzielt worden ist, was dann erreicht ist, wenn das distale Ende des Gewindeschneidkopfes74 aus dem Boden des Knochens herauskommt. Um weiter sicher zu stellen, dass das Gewindeschneidinstrument70 die geeignete Tiefe erreicht, werden die Tiefenmarkierungen auf dem Gewindeschneidinstrument70 ebenso beobachtet. Der Gewindeschneidkopf74 steht in Verbindung mit den Wirbelkörpern V1, V2 durch die geschlitzten Öffnungen22 , die zwischen den Spreizarmen20 bestimmt sind. Erreicht das Gewindeschneidinstrument70 die geeignete Tiefe, wird das Gewindeschneidinstrument70 mit Hilfe des T-Griffes in eine entgegengesetzte Richtung gedreht, um das Instrument aus dem Knochen zu bewegen und das Instrument70 von dem Spreizer10 zu entfernen. - Bezugnehmend nun auf die
6 wird nun die Aufmerksamkeit auf das Einführen des ausgewählten Fusionsimplantats200 gelenkt. Der Käfigkorpus202 ist auf dem Einführinstrument befestigt. Mit dem auf die zuvor beschriebene Weise befestigten T-Griff wird das Einführinstrument100 mit dem daran befestigten Käfigkorpus202 in den Spreizer10 eingeführt, und der Käfigkorpus202 wird in die mit einem Gewinde versehene Bohrung positioniert, indem das Einführinstrument in die Richtung gedreht wird, die in6 dargestellt ist. Der Käfigkorpus202 wird so weit vorwärts bewegt, bis er vollständig in der Bohrung platzgenommen hat. Die Anzeigelinien auf dem Einführinstrument100 helfen dem Chirurgen bei der Bestimmung, ob der Käfig sich in der korrekten Position befindet. Richtstäbe zeigen dem Benutzer die Orientierung des Käfigs an, um behilflich zu sein, dass die Perforationen in Verbindung mit den vertebralen Endplatten stehen, wenn der Käfig seine endgültige Position eingenommen hat. Das Einführinstrument100 wird anschließend von dem Spreizer10 entfernt. - Bezugnehmend nun auf die
7 werden knochenwachstumsstimulierende Substanzen von, z. B. dem Becken-Darmbeinkamm, gewonnen und können in einen Spritzenkorpus oder Schlauch "s" (wie in7 gezeigt) oder eine andere Haltevorrichtung gepackt werden. Wie in8 dargestellt ist, werden die knochenwachstumsstimulierenden Substanzen aus der Spritze "s" entfernt, und zwar unter Verwendung einer Zange "f", und in den Käfigkorpus202 eingeführt, bis der Käfigkorpus202 vollständig mit knochenwachstumsstimulierenden Substanzen gefüllt ist. Die knochenwachstumsstimulierenden Substanzen können geringfügig komprimiert werden, um den Käfig zu füllen. - Bezugnehmend auf die
9 bis11 , und zwar nach dem Füllen des Käfigkorpus202 , wird die Endkappe212 an dem Käfigkorpus202 befestigt. Ein bevorzugtes Instrument300 zum Anbringen der Endkappe212 umfasst einen Griff302 und einen länglichen Abschnitt304 , der mit dem Griff verbunden ist und sich in distaler Richtung hiervon erstreckt. Am distalen Ende des länglichen Abschnitts304 befindet sich der Befestigungskopf oder Bereich306 . Der Befestigungskopf306 umfasst einen distalen ringförmigen Abschnitt308 mit einer ringförmigen Noppe310 , die hiervon hervorsteht, und vier gleichmäßig zueinander beabstandete Flansche312 . Die Flansche312 erstrecken sich in radialer Richtung und sind bevorzugt um ungefähr 90° zueinander beabstandet, wie dies in9 dargestellt ist. Die Flansche312 treten in Eingriff mit der Endkappe312 , um die proximale Biegung der Endkappe312 zu begrenzen, während sie an dem Käfigkorpus202 befestigt ist. Im befestigten Zustand der Endkappe212 auf dem Instrument300 wird die ringförmige Noppe310 des Instruments300 in der ringförmigen Öffnung216 der Endkappe212 aufgenommen. - Vorzugsweise sind die ringförmige Noppe
310 und die Öffnung206 einander entsprechend bemessen, so dass eine Reibungspassung zwischen den beiden Komponenten ermöglicht wird. - Bezugnehmend nun auf die
12 wird das Instrument300 , an dem die Endkappe212 befestigt ist, in die Operationsstelle eingeführt und zum Käfigkorpus202 bewegt. Anschließend wird die Endkappe212 an dem Käfigkorpus202 befestigt, indem die Endkappe212 in den Innenhohlraum eingeführt wird, wobei die Endkappe212 in eine entsprechend bemessene Nut, die in dem Käfigkorpus202 bestimmt ist, einschnappt. Beim Einführen wird dem Umfangsbereich der Endkappe212 gestattet, sich aufgrund teilweise der elastischen Eigenschaften seines Herstellungsmaterials und der Kerben216 zu deformieren, wodurch ermöglicht wird, dass die Endkappe212 in den Käfigkorpus202 gelangt. Es wird darauf hingewiesen, dass bei der Einführung die Flansche312 des Instruments300 (11 ) verhindern, dass die Endkappe212 sich relativ zu dem Instrument dreht. Mit der an dem Käfigkorpus212 befestigten Endkappe212 wird das Instrument300 entfernt. -
13 stellt zwei seitliche Fusionsimplantate200 dar, die in den inter-vertebralen Raum im Lendenbereich eingeführt sind. Der zweite Fusionskäfig200 wird entsprechend der zuvor beschriebenen Instrumente und des Verfahrens eingeführt. - Während die obige Beschreibung vielerlei Details enthält, so sollten diese Details nicht als Beschränkung des Bereichs der Erfindung ausgelegt werden, sondern lediglich als Verdeutlichung bevorzugter Ausführungsformen. Zum Beispiel kann die Zange und/oder das Einführwerkzeug für die Endkappe durch den Spreizer
10 vor dem Entfernen des Spreizers10 eingeführt werden. Der Fachmann zieht vielerlei andere mögliche Variationen in Betracht, die innerhalb des Bereichs der beigefügten Ansprüche liegen. Die Ansprüche identifizieren Ausführungsformen der Erfindung zusätzlich zu denjenigen, die zuvor im Detail beschrieben wurden.
Claims (7)
- Chirurgisches Wundspreizinstrument (
10 ), umfassend ein längliches Element (12 ) mit einem proximalen und einem distalen Endabschnitt (14 ,18 ), das eine in Längsrichtung verlaufende Durchführung (16 ) zur Aufnahme von chirurgischen Instrumenten bestimmt, wobei der distale Endabschnitt (18 ) einen ersten und einen zweiten Wundspreizarm (20 ) besitzt, die sich im Wesentlichen in Längsrichtung erstrecken, und wobei jeder Wundspreizarm eine erste und eine zweite Auflageoberfläche (20a ,20b ) zum Ineingriffbringen von gegenüberliegenden, benachbarten Gewebeabschnitten besitzt, und wobei die erste und die zweite Auflageoberfläche (20a ,20b ) eines jeden Wundspreizarms (20 ) im Wesentlichen planar ausgebildet sind, dadurch gekennzeichnet, dass jeder Wundspreizarm eine Abmessung zwischen der ersten und der zweiten Auflageoberfläche (20a ,20b ) bestimmt, die ausreicht, die gegenüberliegenden Gewebeabschnitte bei dessen Eintritt auszulenken, und wobei beide Oberflächen eines jeden Wundspreizarms jeweils die gegenüberliegenden, benachbarten Gewebeabschnitte in Eingriff bringen, und jeder Wundspreizarm (20 ) einen konisch zulaufenden Endabschnitt (23 ) besitzt, der das Eintreten in den Raum zwischen den gegenüberliegenden, benachbarten Gewebeabschnitten vereinfacht. - Chirurgischer Wundspreizer nach Anspruch 1 zum Auslenken benachbarter Wirbel, wobei der erste und der zweite Wundspreizarm (
20 ) sich in Längsrichtung von dem distalen Endabschnitt (18 ) des länglichen Elements (12 ) erstrecken, und die erste und die zweite Auflageoberfläche (20a ,20b ) eine erste Wirbelauflageoberfläche bzw. eine zweite Wirbelauflageoberfläche bestimmen, und wobei die erste und die zweite Wirbelauflageoberfläche (20a ,20b ) eines jeden Wundspreizarms auf jedem Wundspreizarm um einen vorbestimmten Auslenkabstand (h) zueinander beabstandet sind. - Chirurgischer Wundspreizer nach Anspruch 2, wobei jeder Wundspreizarm (
20 ) einen konisch ausgebildeten, distalen Abschnitt (23 ) besitzt, der das Einführen in den Zwischenwirbelraum vereinfacht. - Chirurgischer Wundspreizer nach Anspruch 2 oder 3, wobei die erste und die zweite Auflageoberfläche (
20a ,20b ) eines jeden Wundspreizarms (20 ) im Allgemeinen parallel zueinander verlaufen. - Chirurgischer Wundspreizer nach Anspruch 4, wobei die erste und die zweite Auflageoberfläche (
20a ,20b ) eines jeden Wundspreizarms (20 ) im Allgemeinen parallel zur Längsachse (a) des länglichen Körpers verlaufen. - Chirurgischer Wundspreizer nach einem der vorhergehenden Ansprüche, wobei das längliche Element (
12 ) zumindest eine in Längsrichtung verlaufende Öffnung (16 ) umfasst, die in einem Zwischenwandabschnitt bestimmt ist. - Chirurgischer Wundspreizer nach einem der vorhergehenden Ansprüche, wobei jeder Wundspreizarm (
20 ) konisch ausgebildete Endabschnitte (23 ) umfasst, die das Einführen der Wundspreizarme (20 ) vereinfachen.
