Description
- Diese Anmeldung beansprucht die Priorität zu der provisorischen US-Anmeldung Seriennummer 60/033443, die am 20. Dezember 1996 eingereicht wurde.
- Gebiet der Erfindung
- Diese Erfindung betrifft im Allgemeinen Arzneimittel bzw. Arzneistoff freisetzende Beschichtungen für medizinische Vorrichtungen, die in den Körper eingeschoben oder implantiert werden. Insbesondere ist die Erfindung auf ein Verfahren zum Anwenden von Arzneimittel freisetzenden Beschichtungen an medizinische Vorrichtungen gerichtet, in denen ein Polymer in einem ersten Lösungsmittel (Lösungsmittel A) gelöst wird, unter Bildung eines Polymersystems und ein Arzneimittel in dem zweiten Lösungsmittel (Lösungsmittel B) gelöst wird, unter Bildung eines Arzneimittelsystems.
- Hintergrund der Erfindung
- Bei Verfahren des Standes der Technik wird eine Zusammensetzung, umfassend ein Gemisch aus einem Arzneimittel und einem Polymer in einem Lösungsmittel, auf die Vorrichtung aufgetragen, unter Bildung einer im Wesentlichen gleichförmigen Schicht aus Arzneimittel und Polymer; das heißt einer Schicht, in der das Arzneimittel in dem Polymer in im Wesentlichen gleichförmiger Weise dispergiert ist. In Abhängigkeit von den Arten des Polymers und den angewendeten Arzneimitteln, kann das Auffinden eines für diese zwei Bestandteile gemeinsamen Lösungsmittels problematisch sein. Wenn ein für das Arzneimittel und Polymer gemeinsames Lösungsmittel nicht verfügbar war, wurden Versuche unternommen, das Arzneimittel zu kleinen Teilchen so zu mikronisieren, dass es in der Polymerlösung suspendiert werden kann. Jedoch kann Mikronisierung zeitaufwendig sein und kann zu einem Verlust an gewünschten therapeutischen Eigenschaften des Arzneimittels führen. Deshalb wird ein Verfahren zum leichteren Hineingeben von sowohl Arzneimittel als auch Polymer in die Lösung, zur Gewinnung einer Arzneimittel freisetzenden Beschichtung für mechanische Vorrichtungen benötigt. Benötigt wird auch ein Verfahren zum Auftragen einer Arzneimittel/Polymerbeschichtung ohne das Arzneimittel und das Polymer in einer gemeinsamen Lösung oder Suspension zu vereinigen.
- Kurzdarstellung der Erfindung
- Die hierin offenbarten Verfahren wenden im Allgemeinen implantierbare Prothesen, insbesondere ausdehnbare Prothesen, wie als Ballon ausdehnbare Stents und selbstausdehnbare Stents, an. Stents, die gemäß der vorliegenden Erfindung beschichtet sind, können beispielsweise plastisch verformbar oder selbst ausdehnend sein. In bestimmten Ausführungsformen vermeiden Verfahren die Notwendigkeit des Mikronisierens eines Arzneimittels, bevor es auf einen Stent aufgetragen wird. In anderen Ausführungsformen können teilchenförmige Arzneimittel angewendet werden. Die vorliegende Erfindung kann auch verwendet werden, wenn es kein für sowohl das Polymer als auch das Arzneimittel gemeinsames kompatibles Lösungsmittel gibt.
- Im weitesten Sinne betrifft die Erfindung ein Verfahren zum Auftragen von Arzneimittel freisetzenden Beschichtungen, die ein Arzneimittel und ein Polymer enthalten, wobei das Arzneimittel in dem Polymer in einer im Wesentlichen gleichförmigen Weise verteilt wird. Das Polymer kann in einem ersten Lösungsmittel (Lösungsmittel A) gelöst sein, unter Bildung eines thermodynamisch stabilen Polymersystems, das im Wesentlichen frei von biologischen Wirkstoffen ist (bei spielsweise weniger als etwa 1 Gewichtsprozent des Polymers) und ein Arzneimittel kann in einem zweiten Lösungsmittel (Lösungsmittel B) gelöst sein, unter Bildung eines Arzneimittelsystems, das im Wesentlichen frei von Polymeren ist, (beispielsweise weniger als etwa 1 Gewichtsprozent des Arzneimittels). Das Lösungsmittel B kann das gleiche wie Lösungsmittel A oder davon verschieden sein. Im Allgemeinen werden verschiedene Lösungsmittel verwendet, wenn das Lösungsmittel in der Polymerlösung eine sehr niedrige Löslichkeit aufweist und kein für sowohl das Arzneimittel als auch Polymer gemeinsames Lösungsmittel gefunden werden konnte. Durch getrenntes und gleichzeitiges Auftragen des Polymersystems und Arzneimittelsystems auf die Oberfläche der medizinischen Vorrichtung bildet das Verfahren einen Verbundwerkstoff des Polymers und Arzneimittels, das heißt, eine Verteilung des Arzneimittels durch ein Polymernetzwerk. Ein solcher Verbundwerkstoff kann im Wesentlichen homogen sein. Das erfindungsgemäße Verfahren vermeidet die Notwendigkeit der Mikronisierung des Arzneimittels vor dem Vermischen desselben mit dem Polymer, unter Bildung des Verbundwerkstoffs aus Arzneimittel und Polymer. Obwohl das Verfahren eine Beschichtung oder eine Schicht einer Beschichtung, die einen Verbundwerkstoff von dem Arzneimittel und Polymer umfasst, bereitstellt, vermeidet es die Notwendigkeit des Vermischens des Arzneimittels und des Polymers in einer Lösung vor dem Auftragen der Beschichtung auf die Vorrichtung.
- Eine Beschichtung kann auf eine Vorrichtung, insbesondere einen Stent, durch getrenntes Sprühen des Polymersystems und des Arzneimittelsystems aufgetragen werden. Verfahren zum Beschichten von Stents werden in US-Patent Nummern 5 837 313 und 6 120 536 offenbart. Die erfindungsgemäßen Beschichtungen können durch gleichzeitiges Sprühen von Polymer und Arzneimittelsystemen, wie nachstehend weiter beschrieben, ausgeführt werden.
- In Abhängigkeit von der erwünschten Beschichtungsdicke können Mehrschrittbeschichtungen aufgetragen werden. In Abhängigkeit von der Lösungsmittelflüchtigkeit kann ein Wartezeitraum während des Sprüh- oder Tauchverfahrens erforderlich werden oder Wärme kann durch eine Heizpistole, eine Heizlampe oder eine Heizkammer zum Erleichtern der Verdampfung von dem/den Lösungsmittel(n) angewendet werden.
- Bei bestimmten Ausführungsformen ist es nicht notwendig, dass ein Arzneimittel in Lösungsmittel B gelöst wird. Feine Teilchen können in dem Lösungsmittel vermischt, vorzugsweise suspendiert, werden und die Beschichtung kann, wie hierin beschrieben, aufgetragen werden. In einigen Fällen können drei oder mehrere Systeme angewendet werden. Beispielsweise kann ein drittes Sprüh- oder Tauchsystem, das reines Lösungsmittel A oder B enthält, die Oberfläche glätten, wenn das Lösungsmittel des dritten Systems mit der Polymermatrix kompatibel ist oder ein drittes Sprüh- oder Tauchsystem, das ein zweites auslaugbares Mittel, wie ein Arzneimittel, enthält. Polyethylenglycol, Zucker, Salz in Lösungsmittel (die gleich wie oder verschieden von Lösungsmittel A und B sein können), können für den Stent angewendet werden. Das erhaltene Arzneimittelfreisetzungsprofil, wenn zwei oder mehr Arzneimittel aufgetragen werden, kann verschiedene Arzneimittel, die gleichzeitig oder nacheinander freigesetzt werden, ergeben.
- Eine geeignete erfindungsgemäße Folge könnte eine Auftragung einer Polymerlösung und einer Arzneimittellösung oder Suspension, gefolgt von Auftragung von reinem Lösungsmittel sein. Alternativ könnte die Folge drei parallele Sprühsysteme sein. Die Anzahl an Zyklen in jedem Vorgang könnte ein halb, eins, zwei oder mehr sein. Der Fachmann wird sicherlich zahlreiche Kombinationen von Beschichtungsreihenfolgen und Folgen für duale oder Mehrfachbeschichtungssysteme ohne Abweichen vom erfindungsgemäßen Konzept finden. Das Be schichtungssystem kann handbetrieben oder automatisiert sein, um verschiedene Kombinationen zu erzielen.

- Für die vorliegende Erfindung geeignete Polymere werden vorzugsweise hydrophobe, biostabile, elastomere Materia lien sein, die nicht abbauen und die die Gewebsabstoßung minimieren. Weiterhin sind die Materialien vorzugsweise jene, die Einkapselung unterliegen durch Gewebe, das zu der Stentimplantationsstelle benachbart ist. Polymere, die für solche Beschichtungen geeignet sind, schließen Silicone (beispielsweise Polysiloxane und substituierte Polysiloxane), Polyurethane (einschließlich Polycarbonaturethane), thermoplastische Elastomere im Allgemeinen, Ethylenvinylacetat-Copolymere, Polyolefinelastomere, EPDM-Kautschuke und Polyamidelastomere ein. Die vorstehend angeführten Materialien werden bezüglich der betrachteten Umgebung der Erfindung als hydrophob betrachtet.
- Nicht begrenzende Beispiele für andere Polymere, die zur Verwendung in der vorliegenden Erfindung geeignet sind, schließen bioabsorbierbare Polymere, wie Polymilchsäure, Polyglycolsäure, Polycaprolacton, Polymilchsäure-Polyethylenoxid-Copolymere, Cellulose und dergleichen, ein. Andere nicht begrenzende Beispiele für Polymere, die zur Verwendung in der vorliegenden Erfindung geeignet sind, schließen auch biostabile Kunststoffmaterialien, wie Acrylpolymere und dessen Derivate, Nylon und Polyester, ein.
- Nicht begrenzende Beispiele für Arzneimittel, die zur Verwendung in der vorliegenden Erfindung geeignet sind, schließen Antitrombotika, Anticoagulantien bzw. Gerinnungshemmer, Antitrombozytenmittel, Trombolytica, antiproliferative Mittel, entzündungshemmende Mittel, Mittel, die Hyperplasie und insbesondere Restenose inhibieren, Glattmuskelzellinhibitoren, Antibiotika, Wachstumsfaktoren, Wachstumsfaktorinhibitoren, Zellanhaftungsinhibitoren, Zellanhaftungspromotoren und Arzneimittel, die die Bildung von gesundem neointimalem Gewebe verstärken, einschließlich endotheliale Zellregeneration, ein. Die positive Wirkung kann von dem Inhibieren von besonderen Zellen (beispielsweise Glattmuskelzellen) oder Gewebsbildung (beispielsweise fibromuskuläres Gewebe) unter Anwendung unterschiedlicher Zellmigration (beispielsweise Endothelium) und Gewebsbildung (neointimales Gewebe) stammen.
- Das in der Erfindung verwendete Arzneimittel kann oder kann nicht mikronisiert sein. Wenn das Arzneimittel nicht mikronisiert ist, kann die Teilchengröße des Arzneimittels größer als etwa 10 μm sein. Wenn das Arzneimittel mikronisiert ist, ist die Arzneimittel-Teilchengröße vorzugsweise 5–12 μm und bevorzugter etwa 5 μm.
- Die in der vorliegenden Erfindung geeigneten Vorrichtungen schließen ohne Begrenzung eine Vielzahl von medizinischen Vorrichtungen, wie Katheter, By-Pässe, Stents (beispielsweise selbst ausdehnbare oder Ballon ausdehnbare vaskuläre oder nicht vaskuläre Stents), Herzklappen, Implantate und künstliche Organe oder Prothesen ein. Die Vorrichtungen können polymere, metallische oder Keramikoberflächen aufweisen.

10 gezeigt werden, schließen Metalle, wie Edelstahl, Tantal, Titan, Legierungen, einschließlich Nitinol (ein Nickeltitan, Thermogedächnislegierungsmaterial), DFT-Materialien, wie inUS 5 630 840 gezeigt und bestimmte Kobaltlegierungen, einschließlich Kobalt-Chrom-Nickel-Legierungen, wie Elgiloy® und Phynox®, ein. Bioabsorbierbare Materialien, wie Milchsäure, können auch verwendet werden, um die Stentstruktur zu bilden. Weitere Einzelheiten, die die Herstellung und Einzelheiten anderer Aspekte der Stents selbst betreffen, findet man in US-Patenten 4 655 771 und 4 954 126 Wallsten und 5 061 275 Wallsten et al. - Lösungsmittel, die in der vorliegenden Erfindung verwendet werden können, werden vorzugsweise mit dem Polymer oder dem Arzneimittel, mit dem sie vermischt werden, kompatibel sein. Beispielsweise wird das Polymer vorzugsweise vollständig in dem Lösungsmittel, mit dem es vermischt wird, löslich sein und das Arzneimittel wird vorzugsweise vollständig in dem Lösungsmittel, mit dem es vermischt wird, löslich sein. Es ist jedoch denkbar, dass bestimmte Ausführungsformen der vorliegenden Erfindung teilchenförmige Arzneimittel, wie Heparin, gemischt in Lösungsmittel einschließen. Wenn Arzneimittel in teilchenförmiger Form vorliegen, wird das Arzneimittel vorzugsweise in dem Lösungsmittel suspendiert. Die nicht begrenzenden Beispiele von Lösungsmitteln, die gemäß dieser Erfindung verwendet werden können, schließen Tetrahydrofuran, Methanol, Methylethylketon, Chloroform, Methylenchlorid, Acetonitril und Wasser ein.
- Die nachstehenden Beispiele beschreiben die erfindungsgemäßen Ausführungsformen, in denen die Beschichtungen auf medizinische Vorrichtungen durch Sprühbeschichtung (Vergleichsbeispiel und Beispiele 1 bis 3) oder durch Tauchbeschichtung (Beispiele 4 bis 6) aufgetragen wurden.
- Vergleichsbeispiel
- Ein Silicon-Xylol-Gemisch (~34% Feststoffgewicht von Applied Silicone Corporation) wurde eingewogen und der Feststoffgehalt an Silicon wurde berechnet. Ein Verhältnis von Wdex/WSiliconfeststoff = 0,1 wurde bei der Herstellung angewendet. Hier beziehen sich „dex" oder „DEX" auf Dexamethason (Upjohn, Dexamethasone USP, CAS # 50-02-2). Die Volumenmenge an Tetrahydrofuran (THF) (HPLC Qualität Aldrich oder EM Science), die verwendet wurde, wurde durch Multiplikation von 20 mit dem Gewicht des festen Silicons bestimmt. DEX wurde zuerst in THF gelöst und dann zu einem Silicon-Xylol-Gemisch überführt. Ein Vernetzungsmittel wurde zugesetzt und die Lösung wurde gut vermischt. Der geflochtene Stent wurde durch Sprühen von mehrfachen Zyklen der so auf dem Stent in einem expandierten Zustand hergestellten homogenen Lösung beschichtet. Der beschichtete Stent wurde dann zu einem Umluftofen bewegt und bei 150°C für 45 Minuten gehärtet. Er wurde dann mit Argongasplasma, gemäß dem in US-Patent Nummer 5 879 697 beschriebenen Verfahren behandelt. Die Ergebnisse werden in
1 ,4 und5 gezeigt.1 und4 sind Fotografien der Beschichtung in einem unbelasteten Zustand, das heißt bevor der Stent expandiert oder zusammengezogen wurde.5 ist eine Fotografie der Beschichtung, nachdem der beschichtete Stent mechanisch belastet wurde, das heißt nach Zusammenziehen und Ausdehnung des Stents. - Beispiel 1
- Eine 5%ige (Gewicht/Gewicht) Lösung von Siliconfeststoff in THF wurde durch Zugeben des erforderlichen THF und Vernetzungsmittels in das Silicongemisch herstellt. Eine getrennte 0,5%ige (Gewicht/Gewicht) Lösung von DEX in THF wurde durch Zugeben von THF in ein DEX enthaltendes Becherglas hergestellt. Das Verhältnis von Wdex/WSiliconfeststoff war 0,1. Ein Badger 150-4 Sprühsystem wurde mit zwei Reservoirs, gefüllt mit zwei verschiedenen Lösungen, verwendet. Die Beschichtung des Stents in einem ausgedehnten Zustand wurde durch Sprühen eines Zyklus von Siliconlösung, Warten für einen kurzen Zeitraum (etwa 30 Sekunden) und Sprühen von einem Zyklus DEX-Lösung, Warten für einen kurzen Zeitraum (etwa 30 Sekunden) und dann Wiederholen der Sprühfolge ausgeführt. Der letzte Sprühzyklus war Siliconlösung. Für eine Beschichtungsdicke von 30 μm wurden etwa 30 Zyklen jeweils angewendet. Die Anzahl an Sprühzyklen, die verwendet wurde, hing von der Lösungsviskosität, der Tröpfchengröße und der Fließgeschwindigkeit ab. Der beschichtete Stent wurde dann zu einem Umluftofen bewegt und bei 150°C 45 Minuten gehärtet. Er wurde dann mit Argongasplasma behandelt. Die Ergebnisse werden in
2 ,6 und7 gezeigt.2 und6 sind Fotografien der Beschichtung in einem unbelasteten Zustand.7 ist eine Fotografie der Beschichtung, nachdem der beschichtete Stent mechanisch belastet wurde. - Beispiel 2
- Das zum Herstellen dieses Beispiels verwendete Verfahren war ähnlich zu dem in Beispiel 1 angewendeten Verfahren, mit der Ausnahme, dass DEX in Methanol gelöst wurde, was ein schlechtes Lösungsmittel für Silicon darstellt. Das Verhältnis von Wdex/WSiliconfeststoff wurde bei der Herstellung bei 0,1 gehalten. Die Ergebnisse werden in
8 und9 gezeigt.3 und8 sind Fotografien der Beschichtung in einem unbelasteten Zustand.9 ist eine Fotografie der Beschichtung, nachdem der beschichtete Stent mechanisch belastet worden war, das heißt ausgedehnt und zusammen gezogen. - Beispiel 3
- Die in dem Vergleichsbeispiel, Beispiel 1 und Beispiel 2 hergestellten beschichteten Stents wurden in 2 cm-Segmente geschnitten und in 100 ml Polyethylenglycol (MW = 400, Aldrich) und Wassergemisch (40/60 Volumen/Volumen) gelegt. Die Lösung wurde häufig verändert, wenn Proben der Lösung genommen wurden. Die DEX-Konzentration wurde unter Verwendung eines Shimadzu Hochleistungsflüssigchromatografen, der mit einem UV-Detektor bei einer Wellenlänge von 254 nm arbeitete, analysiert. Eine C-8-Säule wurde bei der Analyse angewendet. Tabelle 1 und
15 zeigen die DEX-Freisetzungsdaten über einen Monat die beschichteten Stents. Alle drei Zubereitungen in dem Vergleichsbeispiel, Beispiel 1 und Beispiel 2 verlängerten die Arzneimittelfreisetzung und hatten ähnliche Freisetzungsprofile. - Eine Ausführungsform der vorliegenden Erfindung unter Beinhalten von Sprühtrocknen wird aufeinanderfolgend in
11 und12 erläutert. Die Arzneimittelfreisetzungsbeschichtungsapparatur2 hat ein rotierendes Stentelement4 und ein Sprühelement6 . - Ein Stent
8 wird an dem rotierenden Stentelement4 durch das Klammern desselben an beiden Enden zum Tragen von Stabelementen24' und24'' , die typischerweise Krokodilklammern darstellen, befestigt. Vorzugsweise wird Stent8 in einer Position festgeklammert, die mindestens teilweise ausgedehnt oder vollständig ausgedehnt ist, sodass sein Inneres leer bleibt, wodurch die Sprühbeschichtung sowohl an der äußeren als auch der inneren Oberfläche von Stent8 anhaftet. Stabelemente24' ,24'' passieren das Innere von Stent8 nicht. Motor10 dreht die Stabelemente24' ,24'' , wodurch Stent8 rotiert. Trägerstab12 trägt das rotierende Stentelement4 . - Sprühelement
6 hat einen Strukturträger14 , auf dem der erste Sprühkanister16 und ein zweiter Sprühkanister18 befestigt werden. Der erste Sprühkanister16 ist fluid mit dem ersten Sprühdosierer20 verbunden und der zweite Sprühkanister18 ist fluid mit dem zweitem Sprühdosierer22 verbunden. Gaszuführungsvorrichtungen26 ,28 sind mit den Sprühvorrichtungen verbunden, um gasförmige Mittel bereitzustellen, die das Polymer und Arzneimittellösungen vorantreiben. - Die ersten und zweiten Sprühkanister
16 ,18 können an Strukturträger14 an einem Zug (nicht gezeigt) befestigt werden, um Längsbewegung zurück und vorwärts zu ermöglichen. In einem vereinfachten Fall kann eine Hand als ein Träger14 dienen. In dieser Ausführungsform werden Kanister16 ,18 in einem vorbestimmten Abstand von einander befestigt, sodass sie sich in gleichmäßigem Abstand voneinander aufeinander zu und fort bewegen. Der ungefüllte Pfeil in11 zeigt Kanister16 ,18 vor der Linksbewegung. Das Sprühen wird beginnen, wenn Sprühdosierer20 ,22 dem Stent8 gegenüberstehen. Der gefüllte Pfeil in12 zeigt Kanister16 ,18 , die ihren Verlauf zurück nach rechts beginnen, erneut zu sprühen beginnen, wenn Dosierer20 ,22 dem Stent gegenüber stehen. Diese Folge kann, falls erwünscht, viele Male wiederholt werden. - In dieser Ausführungsform kann der erste Sprühkanister
16 das Polymer in einem Lösungsmittel aufbewahren und frei von Arzneimittel sein, während der zweite Sprühkanister18 ein Arzneimittel in einem Lösungsmittel aufbewahren kann und frei von Polymer ist oder umgekehrt. -
13 und14 erläutern anschließend eine andere Ausführungsform der vorliegenden Erfindung. Die Bezugsziffern in dieser Ausführungsform haben die gleiche Bedeutung wie jene in der vorangehenden Ausführungsform, die in11 und12 gezeigt werden. - Die ersten und zweiten Sprühkanister
16 ,18 können an Strukturträger14 auf den zwei Schlitten (nicht gezeigt) befestigt werden, um die Längsbewegung zurück und vorwärts zu ermöglichen. In dieser Ausführungsform können Kanister16 ,18 unabhängig voneinander zu und fort bewegt werden. In13 zeigt der gefüllte Pfeil den ersten Kanister16 , der sich nach links bewegt und den Stent besprüht, während der ungefüllte Pfeil den zweiten Kanister18 in einer stationären Position mit dem Potenzial zum Bewegen nach links ausweist. Erster Kanister16 bewegt sich nach links und stoppt dann nach dem Stent und nimmt eine stationäre Position ein (nicht gezeigt). Dann wie in14 gezeigt, bewegt sich der zweite Kanister18 nach links, um den Stent zu besprühen und dann links des Stents zu verbleiben. Anschließend kann der erste Kanister16 sich nach rechts bewegen, später gefolgt von dem zweiten Kanister18 und das Verfahren kann, falls erwünscht, wiederholt werden. - Andere erfindungsgemäße Verfahren können den ersten Kanister
16 einschließen, der zu und fort bewegt wird, während der zweite Kanister18 über den Stent hinaus verweilt, gefolgt von dem zweiten Kanister, der zu und fort bewegt wird, während der erste Kanister16 über den Stent hinaus verweilt. Andere nicht begrenzende Variationen, die Kanister16 und18 aufweisen, bewegen sich gleichzeitig, jedoch beabstandet von einander oder mit Kanistern16 und18 zickzack unter Sprühen. - Zusätzlich zur Sprühbeschichtung können die erfindungsgemäßen Beschichtungen durch Tauchen der medizinischen Vorrichtung in 1) ein Polymer, gelöst in einem ersten Lösungsmittel und 2) ein Arzneimittel, gelöst in einem zweiten Lösungsmittel, aufgetragen werden. Die Vorrichtung kann wiederholt in die Polymerlösung und Arzneimittellösung getaucht werden, bis eine Beschichtung der gewünschten Dicke erreicht wird. Im Allgemeinen wird das Polymer schrumpfen und quellen und das Arzneimittel wird in den anschließenden Tauchschritten, in Abhängigkeit von der relativen Löslichkeit der Lösungsmittel zu dem Polymer und Arzneimittel rekristallisieren oder sich erneut verteilen. Um Gleichförmigkeit der Beschichtung auf der Vorrichtung zu sichern, können bestimmte Schrit te unternommen werden. Diese Schritte schließen ohne Begrenzung ein: Auswählen eines Lösungsmittels mit einem niedrigen Viskositätsindex, Auswählen eines Polymers oberhalb seines Verknäulungsmolekulargewichts, sodass die innere/n Polymerschicht(en) quellen wird/werden, jedoch nicht leicht gelöst wird/werden, Blastrocknen der Beschichtung nach jedem Tauchen, Horizontalbeschichten der Vorrichtung, Rotieren der Vorrichtung während des Trocknens und unter Anwenden kurzer Beschichtungszeiten.
- Die nachstehenden Beispiele erläutern Ausführungsformen der Erfindung, die durch Tauchen hergestellt wurden.
- Beispiel 4
- 15 Gramm einer Silicondispersion von Applied Silicone Corp. (# 40000), die 35% Feststoffe enthält, wurden eingewogen. 0,52 Gramm Dexamethason (DEX) (Upjohn, Lot # 719 kT) wurden in 15 ml Tetrahydrofuran (THF) gelöst. Die Arzneimittellösung wurde zu der Silicondispersion gegeben und verrührt, bis sich eine homogene Lösung gebildet hatte.
- Ein Stentimplantat (Beispiel 4a) wurde in die Lösung getaucht und in ihrer horizontalen Position luftgetrocknet. Das Beschichtungsgewicht für diese Probe war 10,8 mg/cm2. Aufgrund der Schwerkraft wurde die Beschichtung ungleichmäßig. Um eine gleichförmigere Beschichtung zu erhalten, wurde ein zweites Stentimplantat (Beispiel 4b) nachdem es in die Lösung getaucht wurde, horizontal rotiert, während es luftgetrocknet wurde. Das Beschichtungsgewicht für diese Probe war 13,5 mg/cm2. Nach Stehen bei Raumtemperatur für etwa 1,5 Stunden wurden die beschichteten Proben bei 150°C für eine Stunde im Ofen getrocknet.
- Die Implantate wurde in 2 cm Stücke geschnitten und ihre Freisetzungsgeschwindigkeiten wurden durch Eintauchen der Stücke in eine 25/75 Volumen/Volumen wässrige Lösung von Polyethylenglycol (PEG) (MW = 400 von J. T. Baker) untersucht. Den Lösungen wurden periodisch Proben entnommen und durch frische Lösung ersetzt. Die Menge an freigesetztem DEX wurde durch eine Shimadzu-Hochleistungflüssigkeitschromatografie (HPLC), unter Verwendung einer C8-Säule (Phase Separation Ltd.) mit einer mobilen Phase von Acetonitril/H2O (1 : 1 Volumen/Volumen), einer Fließgeschwindigkeit von 1 ml/min und einem UV-Detektor bei einer Wellenlänge von 254 nm quantifiziert. Die Freisetzungsgeschwindigkeiten für diese zwei Proben werden in Tabelle 2 angegeben. Die Daten zeigen, dass die beschichteten Proben längere Arzneimittelfreisetzung bereitstellen können (siehe
16 ). - Beispiel 5
- Eine 4% Silicon in THF enthaltende Silicondispersion wurde hergestellt. Auch wurde eine 10%ige Lösung von DEX in THF hergestellt. Ein Stentimplantat wurde in die Siliconlösung zuerst getaucht und getrocknet, während es für mindestens eine Minute rotiert wurde. Der Stent wurde dann in die DEX/THF-Lösung getaucht und für mindestens eine Minute zum Trocknen des Stents rotiert. Diese Folge von Tauchen und Trocknen wurde etwa 20 mal wiederholt. Die letzte Tauchung erfolgte in der Siliconlösung. Nach Absetzen bei Raumtemperatur für etwa 1,5 Stunden wurden die beschichteten Proben bei 150°C für eine Stunde Ofen gehärtet.
- Wie die Stents in Beispiel 4a und 4b wurde auch diese Probe in 2 cm-Stücke geschnitten und die Freisetzungsgeschwindigkeit wurde gemäß dem in Beispielen 4a und 4b verwendeten Verfahren untersucht. Die Freisetzungsgeschwindigkeit für Beispiel 5 wird in Tabelle 2 und
6 wiedergegeben. Die Daten zeigen, dass die beschichteten Proben längere Arzneimittelfreisetzung bereitstellen können. - Beispiel 6
- Die Silicondispersion von Beispiel 5 wurde hergestellt. Auch wurde eine 10%ige Lösung von DEX in Methanol (MeOH) hergestellt, welches ein schlechtes Lösungsmittel für Silicon und folglich mit dem Silicon unkompatibel war. Ein Stentimplantat wurde gemäß dem verwendeten Verfahren beschichtet, um Beispiel 5 herzustellen. Diese Probe wurde in 2 cm-Stücke geschnitten und die Freisetzungsgeschwindigkeit wurde gemäß dem in Beispielen 4a und 4b verwendeten Verfahren untersucht. Die Freisetzungsgeschwindigkeit für Beispiel 6 wird in Tabelle 2 wiedergegeben. Die Daten zeigen, dass die beschichteten Proben längere Arzneimittelfreisetzung bereitstellen können, obwohl diese Probe eine schnellere anfängliche Arzneimittelauslaugungsgeschwindigkeit als Beispiele 4a, 4b und 5 zeigte (
16 ). - Die hierin angeführte Beschreibung wird zum Zweck der Erläuterung und nicht als Begrenzung angegeben. Änderungen und Modifizierungen können gemäß den Ausführungsformen der Beschreibungen erfolgen und liegen dennoch innerhalb des Umfangs der Erfindung. Weiterhin wird der Fachmann augenscheinliche Änderungen, Modifizierungen oder Variationen erkennen.
Claims (20)
- Verfahren zum Bilden einer Beschichtung auf mindestens einem Teil einer medizinischen Vorrichtung zum Einschub oder zur Implantation in den Körper eines Patienten, wobei die Beschichtung mindestens eine Beschichtungsschicht zum Freisetzen von mindestens einem biologischen Wirkstoff daraus umfasst, wobei das Verfahren umfasst: a) Auftragen eines Polymersystems, im Wesentlichen frei von einem biologischen Wirkstoff, umfassend ein Polymer und ein erstes Lösungsmittel, auf die Oberfläche und b) Auftragen eines Arzneimittelsystems, im Wesentlichen frei von jeglichem Polymer, umfassend einen biologischen Wirkstoff und ein zweites Lösungsmittel auf die Oberfläche, wobei Schritte a) und b) gleichzeitig durchgeführt werden und das Verfahren die Beschichtungsschicht bildet, die einen im Wesentlichen homogenen Verbundwerkstoff aus dem Polymer und den biologischen Wirkstoff umfasst.
- Verfahren nach Anspruch 1, wobei Schritte a) und b) wiederholt werden.
- Verfahren nach Anspruch 1, wobei die medizinische Vorrichtung einen Stent darstellt.
- Verfahren nach Anspruch 1, wobei die medizinische Vorrichtung einen sich selbst ausdehnenden Stent darstellt.
- Verfahren nach Anspruch 1, wobei das erste Lösungsmittel und das zweite Lösungsmittel die gleichen sind.
- Verfahren nach Anspruch 1, wobei das erste Lösungsmittel und das zweite Lösungsmittel verschieden sind.
- Verfahren nach Anspruch 1, wobei das Polymer ein hydrophobes biostabiles Elastomer darstellt.
- Verfahren nach Anspruch 7, wobei das Elastomer ausgewählt ist aus der Gruppe, bestehend aus Siliconen, Polyurethanen, thermoplastischen Elastomeren, Ethylenvinylacetat-Copolymeren, Polyolefinelastomeren, EPDM-Kautschuken und Polyamidelastomeren.
- Verfahren nach Anspruch 1, wobei der biologische Wirkstoff ausgewählt ist aus der Gruppe, bestehend aus antitrombotischen Mitteln, Gerinnungshemmern, Antitrombozytenmitteln, Trombolytika, antiproliferativen Mitteln, entzündungshemmenden Mitteln, Mitteln, die Hyperplasie inhibieren, Glattmuskelzellinhibitoren, Antibiotika, Wachstumsfaktoren, Wachstumsfaktorinhibitoren, Zellanhaftungsinhibitoren, Zellanhaftungspromotoren und Arzneimitteln, die die Bildung von gesundem neointimalem Gewebe verstärken.
- Verfahren nach Anspruch 1, wobei das Polymersystem oder Arzneimittelsystem durch Sprühbeschichtung aufgetragen wird.
- Verfahren nach Anspruch 1, wobei der biologische Wirkstoff in dem zweiten Lösungsmittel gelöst wird.
- Verfahren nach Anspruch 1, wobei der biologische Wirkstoff in dem zweiten Lösungsmittel suspendiert wird.
- Verfahren nach Anspruch 1, das weiterhin Auftragen eines Lösungsmittelsystems auf die Beschichtungsschicht umfasst.
- Verfahren nach Anspruch 1, das weiterhin Auftragen eines zweiten Arzneimittelsystems, im Wesentlichen frei von jeglichem Polymer und umfassend einen zweiten biologischen Wirkstoff, auf die Oberfläche umfasst.
- Apparatur (
2 –11 –14 ) zum Bedecken von mindestens einem Teil einer Oberfläche einer medizinischen Vorrichtung (8 –11 –14 ) mit einer Beschichtung, wobei die Apparatur (2 ) umfasst: a) ein Befestigungselement (4 –11 –14 ), umfassend mindestens ein Trägerelement (24 –11 –14 ) zum Befestigen der Vorrichtung (8 ) an der Apparatur (2 ) und b) ein Sprühelement (6 –11 –14 ), ausgerichtet mit dem Befestigungselement (4 ) in einer Weise, die erlaubt, dass die Oberfläche der Vorrichtung (8 ) besprüht wird, wobei das Sprühelement (6 ) umfasst i) einen Strukturträger (14 –11 –14 ) und ii) erste (16 11 –14 ) und zweite (18 11 –14 ) Sprühkanister, die beweglich an dem Strukturträger (14 ) befestigt sind, wobei der erste Kanister (16 ) fluid mit dem ersten Sprühdosierer (20 –11 –14 ) verbunden ist und der zweite Kanister (18 ) fluid mit einem zweiten Sprühdosierer (22 –11 –14 ) verbunden ist. - Apparatur nach Anspruch 15, die weiterhin Gasquellen umfasst, die mit den ersten (
20 ) und zweiten (22 ) Sprühdosierern verbunden sind. - Apparatur nach Anspruch 15, wobei die ersten (
16 ) und zweiten (18 ) Kanister bei einem vorbestimmten Abstand von einander befestigt sind. - Apparatur nach Anspruch 15, wobei die ersten (
16 ) und zweiten (18 ) Kanister unabhängig voneinander bewegt werden können. - Apparatur nach Anspruch 15, wobei die ersten (
16 ) und zweiten (18 ) Kanister beweglich auf dem Strukturträger (14 ) in einer Weise befestigt sind, dass die Kanister (16 ,18 ) nacheinander bewegt werden können. - Apparatur nach Anspruch 15, die weiterhin einen Motor (
10 ) zum Rotieren des Trägerelements (24 ) umfasst.
