- GEBIET DER ERFINDUNG
- Die vorliegende Erfindung betrifft allgemein Vorrichtungen zum Stabilisieren der Wirbelsäule und Vorrichtungen zum Implantieren zwischen Wirbeln und insbesondere im Intradiskalraum. Im einzelnen betrifft die Erfindung hohle Zwischenwirbel-Abstandsstücke.
- HINTERGRUND DER ERFINDUNG
- Zwischenwirbelscheiben, die sich zwischen den Endplatten von benachbarten Wirbeln befinden, stabilisieren die Wirbelsäule, verteilen Kräfte zwischen Wirbeln und polstern Wirbelkörper. Eine normale Zwischenwirbelscheibe schließt einen halb-gallertartigen Bestandteil, den Nucleus pulposus, ein, der von einem äußeren faserigen Ring, genannt Anulus fibrosus, umgeben und eingeschlossen wird. Bei einer gesunden, unbeschädigten Wirbelsäule verhindert der Anulus fibrosus, daß der Nucleus pulposus außerhalb des Bandscheibenraums vorspringt.
- Bandscheiben können auf Grund von Verletzung, Krankheit oder Altern verschoben oder beschädigt werden. Eine Unterbrechung des Anulus fibrosus ermöglicht, daß der Nucleus pulposus in den Wirbelkanal vorspringt, ein Zustand, der allgemein als Diskushernie oder -prolaps bezeichnet wird. Der herausgedrückte Nucleus pulposus kann auf den Spinalnerv drücken, was zu Nervenschäden, Schmerzen, Taubheit, Muskelschwäche und Lähmung fülren kann. Zwischenwirbelscheiben können ebenfalls auf Grund des normalen Alterungsprozesses oder von Krankheit degenerieren. Wenn eine Bandscheibe Wasser verliert und sich verhärtet, wird die Bandscheibenraumhöhe verringert, was zu Instabilität der Wirbelsäule, verminderter Beweglichkeit und Schmerzen führt.
- Manchmal ist die einzige Linderung der Symptome dieser Leiden eine Nukleotomie oder operative Entfernung eines Abschnitts oder der Gesamtheit einer Zwischenwirbelscheibe, gefolgt von der Fusion der benachbarten Wirbel. Das Entfernen der beschädigten oder krankhaften Bandscheibe wird ermöglichen, daß der Bandscheibenraum zusammenfällt. Ein Zusammenfallen des Bandscheibenraums kann zusätzlich zu starken Schmerzen, Instabilität der Wirbelsäule, abnorme Gelenkmechanik, vorzeitiges Entwickeln von Arthritis oder Nervenschäden verursachen.
- Oft werden Knochentransplantate verwendet, um den Zwischenwirbelraum zu füllen und zu erhalten und eine Fusion zu fördern. Zum Beispiel präpariert der Chirurg bei der Smith-Robinson-Technik der Zervikalfusion die Endplatten der benachbarten Wirbelkörper, damit sie ein Transplantat annehmen, nachdem die Bandscheibe entfernt worden ist. Die Endplatten werden allgemein mit einem Hochgeschwindigkeitsfräser präparier, damit sie parallele Flächen sind. Der Chirurg modelliert das Transplantat, damit es fest zwischen die Knochenoberflächen paßt, so daß das Transplantat durch Kompression zwischen den Wirbellcörpern gehalten wird. Das Knochentransplantat soll eine strukturelle Stütze bereitstellen und das Einwachsen von Knochen fördern, um eine feste Fusion des betroffenen Gelenks zu eneichen.
- Unglücklicherweise bringt die Verwendung von Knochentransplantaten verschiedene Nachteile mit sich. Autogentransplantate, operativ vom Patienten entnommenes Knochenmaterial, können unerwünscht sein, weil sie nicht die ausreichende Menge an Transplantatmaterial ergeben mögen. Die zusätzliche Operation zum Entnehmen des Autogentransplantats erhöht außerdem die Gefahr von Infektion und Blutverlust. Darüber hinaus kann die strukturelle Integrität am Spenderort verringert werden. Darüber hinaus klagen manche Patienten, daß die Transplantatgewinnungsoperation schmerzhafter ist als die Fusionsoperation.
- Allogentransplantatmaterial, das von Spendern der gleichen Art gewonnen wird, ist einfacher zu gewinnen. Jedoch können Allogentransplantate auf Grund der Gefahr von Krankheitsübertragung, Immunreaktionen und religiösen Einwänden nachteilig sein. Darüber hinaus hat allogener Knochen nicht das osteoinduktive Potential von autogenem Knochen und kann daher nur eine zeitweilige Stütze gewährleisten.
- Sowohl ein Allogentransplantat als auch ein Autogentransplantat bringen zusätzliche Schwierigkeiten mit sich. Ein Transplantat allein kann die erforderliche Stabilität nicht gewährleisten, um Wirbelsäulenlasten zu widerstehen. Eine interne Fixierung kann ein Zusammenfallen des Transplantats verhindern, bringt aber eigene Nachteile mit sich, wie beispielsweise die Notwendigkeit einer komplizierteren Operation. Außerdem ist es oft erforderlich, daß der Chirurg das Transplantatmaterial wiederholt zurichtet, um die richtige Größe zu eneichen, um den Bandscheibenraum zu füllen und zu stabilisieren. Dieses Herangehen durch Pröbeln vergrößert die für die Operation erforderliche Zeitdauer. Darüber hinaus hat das Transplantatmaterial üblicherweise eine glatte Oberfläche, die keinen guten Reibschluß zwischen den benachbarten Wirbeln gewährleistet. Ein Rutschen des Transplantats kann sowohl Nerven- und Gefäßverletzungen als auch ein Zusammenfallen des Bandscheibenraums verursachen.
- Es können prothetische Implantate verwendet werden, um ein Zusammenfallen des Raums zu verhindern. Das Implantat muß eine zeitweilige Stütze bereitstellen und das Einwachsen von Knochen ermöglichen. Ein Erfolg des Nukleotomie- und Fusionsverfahrens erfordert die Entwicklung eines anschließenden Knochenwachstums, um eine feste Masse zu erzeugen, weil das Implantat den Kompressionslasten auf der Wirbelsäule nicht das gesamte Leben des Patienten widerstehen wird.
- Ein hohles Wirbelsäulenabstandsstück für einen Eingriff zwischen Wirbeln, das eine Vorderwand mit eine konvex gekrümmten vorderen Fläche und eine Rückwand mit einer flachen hinteren Fläche und zwei Seitenwände umfaßt, integral zwischen den entgegengesetzten Enden der Vorder- und der Rückwand verbunden, um eine Kammer zu definieren, bei dem die Wände außerdem eine obere Wirbeleingriffsfläche mit einer Öffnung in Verbindung mit der Kammer defmieren und ebenfalls eine gegenüberliegende untere Wirbeleingriffsfläche, ebenfalls mit einer Öffnung in Verbindung mit der Kammer, definieren, wird in WO-A-9508306 beschrieben. Dieses Dokument legt offen, daß das Abstandsstück aus einem Stück konstruiert wird, und die Breite der Vorder- und der Rückwand ausreichend ist, um die Wirbel zu überspannen.
- Es besteht weiterhin ein Bedarf an Fusionsvorrichtungen, die den Intradiskalraum erhalten und die Wirbelsäule stützen, bis die benachbarten Wirbel verschmolzen sind, und noch das Einwachsen von Knochen unterstützen, um eine feste Fusion zu eneichen. Es besteht ebenfalls weiterhin ein Bedarf an Vorrichtungen, um die Länge von chirurgischen Operationen und die Gefahr von Komplikationen zu verringern.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Nach der Erfindung wird ein hohles Wirbelsäulenabstandsstück für einen Eingriff zwischen Wirbeln bereitgestellt, das eine Vorderwand, eine Rückwand und zwei Seitenwände umfaßt, integral zwischen der Vorder- und der Rückwand verbunden, um eine Kammer zu definieren, wobei die Wände eine obere Wirbeleingriffsfläche mit einer ersten Öffnung in Verbindung mit der Kammer und ebenfalls eine untere Wirbeleingriffsfläche mit einer zweiten Öffnung in Verbindung mit der Kammer definieren, dadurch gekennzeichnet, daß das Abstandsstück außerdem eine Klinge umfaßt, die von einer der Eingriffsflächen vorsteht und eine Schneidkante hat, konfiguriert zum Durchstechen einer Wirbelendplatte.
- Folglich werden hohle Wirbelsäulenabstandsstücke mit anatomiefreundlichen Formen und Merkmalen für einen Eingriff zwischen Wirbeln bereitgestellt. Die Abstandsstücke werden vorzugsweise bemessen und konfiguriert, um einen Raum zwischen benachbarten Wirbeln zu füllen. Das Abstandsstück ist vorzugsweise D-förmig mit einer grundsätzlich flachen Geometrie und einer konvex gekrümmten vorderen Fläche an der Vorderwand. Das flache Abstandsstück kann auf Grund von aufgerauhten Wirbeleingriffsflächen einen Reibschluß gewährleisten. Das Abstandsstück kann smiley-förmig sein, mit einem Radius in der oberen und der unteren Fläche, der zur Form der Wirbelendplatten paßt. Das Abstandsstück kann zusätzlich zu einem Smiley-Umriß eine bikonvexe Form einschließen. Das hohle Abstandsstück kann Seitenflügel einschließen, die in den Bandscheibenraum ausgefahren werden können, um ein bedeutendes Absenken des Implantats in die Wirbelkörper zu verhindern.
- Folglich werden Implantate für einen Eingriff zwischen Wirbeln bereitgestellt, die den Zwischenwirbelscheibenraum wiederherstellen und die Wirbelsäule stützen, während sie das Einwachsen von Knochen fördern. Ebenfalls werden die Wirbelsäulenabstandsstücke die mit Allogentransplantaten und Autogentransplantaten verbundenen Probleme vermeiden, wie beispielsweise die Notwendigkeit eines pröbelnden Zurichtens des Transplantatmaterials, damit es in den Intradiskalraum paßt, die Morbidität des Spenderorts und die Gefahren einer Krankheitsübertragung.
- Ein Vorzug der Abstandsstücke der vorliegenden Erfindung ist, daß sie ohne die Notwendigkeit von invasiver Autogentransplantatgewinnung, Allogentransplantatkomplikationen oder interner Fixierung eine Struktur für den Raum bereitstellen, der sich aus der Entfernung einer Zwischenwirbelscheibe ergibt. Andere Vorzüge der vorliegenden Erfindung werden Personen mit normalen Kenntnissen auf dem Gebiet aus der folgenden schriftlichen Beschreibung und den beigefügten Abbildungen offensichtlich.

- Zum Zweck der Förderung eines Verständnisses der Prinzipien der Erfindung wird nun Bezug genommen auf die in den Zeichnungen illustrierten Ausführungsbeispiele, und es wird eine spezifische Sprache verwendet, um dieselben zu beschreiben. Es wird sich jedoch von selbst verstehen, daß dadurch keine Begrenzung des Rahmens der Erfindung, wie er in den Ansprüchen definiert wird, beabsichtigt wird.
- Die vorliegende Erfindung stelle hohle Wirbelsäulenabstandsstücke für einen Eingriff zwischen Wirbeln bereit, die bemessen und konfiguriert werden, um den nach einer Nukleotomie verbleibenden Raum zu füllen. Die erfindungsgemäßen Abstandsstücke stellen die Höhe des Zwischenwribelscheibenraums wieder her und gewährleisten ohne interne Fixierung eine unmittelbare Tragflähigkeit und eine Stütze für die Wirbelsäule. Diese Erfindung beseitigt die Notwendigkeit von invasiver Autogentransplantatgewinnung und pröbelndem Zurichten von Transplantatmaterial, damit es in den Intradiskalraum paßt. Die Implantate haben vorteilhafterweise anatomiefreundliche Formen und Merkmale, welche die Stabilität steigern und die Gefahr von Komplikationen vermindern.
- Ein Abstandsstück
10 für einen Eingriff zwischen den Wirbeln wird in1 bis3 abgebildet. Das Abstandsstück10 schließt eine Vorderwand11 mit entgegengesetzten Enden12 ,13 , eine Rückwand15 mit entgegengesetzten Enden16 ,17 und zwei Seitenwände20 ,21 ein. Jede der Seitenwände20 ,21 wird zwischen den entgegengesetzten Enden12 ,13 ,16 ,17 der Vorderwand11 und der Rückwand15 verbunden, um eine Kammer30 zu definieren. Die Wände definieren ebenfalls die obere Fläche35 , die eine erste Öffnung36 in Verbindung mit der Kammer30 definiert. Die obere Fläche35 schließt eine erste Wirbeleingriffsfläche37 ein. Wie in2 und3 gezeigt, definieren die Wände außerdem eine gegenüberliegende untere Fläche40 , die eine zweite Öffnung41 definiert, die in Verbindung mit der Kammer30 steht. Die untere Fläche40 schließt eine zweite Wirbeleingriffsfläche (nicht gezeigt) ein, die der ersten Wirbeleingriffsfläche ähnlich oder identisch mit derselben ist. - Die vorliegende Erfindung bietet eine Zahl von Verbesserungen gegenüber dem bekannten technischen Stand. Zum Beispiel legen im US-Patent Nr. 5 397 364 Kozak et al. eine verhältnismäßig komplexe Vorrichtung zur anterioren Wirbelkörperfusion offen, die Paare von Abstandsstücken umfaßt und aus mehreren Bestandteilen zusammengebaut wird. Im Gegensatz dazu profitiert die vorliegende Erfindung von einer vereinfachten Konstruktion aus einem einzelnen Materialstück, was Komplikationen verringert und die Einfachheit der chirurgischen Verwendung erleichtert. Darüber hinaus sind die Vorder- und die Rückwand ausreichend breit, so daß sie ermöglichen, daß ein einzelnes Abstandsstück die Breite der Wirbel überspannt. Dies ist ein Vorzug gegenüber den schmalen, durch Beckers und Schlapfer in PCT WO-A- 9508306 offengelegten, Abstandsstücken. Beckers und Schlapfer schlagen ein langes und schmales hohles Abstandsstück mit verhältnismäßig dünnen Seiten- und Rückwänden vor. Die vorliegende Erfindung erfordert nur ein Abstandsstück, um die Stütze von zwei oder mehr der Abstandsstücke nach dem bekannten technischen Stand bereitzustellen. Die Verwendung eines einzelnen Abstandsstücks bei einer Wirbelsäulenoperation verringert ebenso die Gefahr einer Infektion und eines Einbringens von Fremdverunreinigungen, wie es gegenüber der Konfiguration von zwei unabhängigen, um die Wirbelsäule angeordneten, Abstandsstücken eine gesteigerte Stabilität gewährleistet.
- Die vorliegende Erfindung stellt einige anatomisch kompatible und chirurgisch zweckmäßige Merkmale bereit, welche die Gefahr von Komplikationen während und nach der Implantation verringern. Als Zwischenwirbelscheiben-Ersatzimplantat kann ein hohles D-förmiges Wirbelsäulenabstandsstück verwendet werden. Die Vorderwand
11 wird, wie in1 bis3 gezeigt, konvex gekrümmt. Diese vordere Krümmung wird bevorzugt, um sich der Geometrie der härteren Substantia corticalis der benachbarten Wirbelkörper anzupassen. Die D-Form des Abstandsstücks10 verhindert jedes Vorstehen der Kanten der Vorderwand11 außerhalb des anterioren Aspekts des Bandscheibenraums, was für in der Halswirbelsäule implantierte Abstandsstücke besonders wichtig sein kann, um ein Beeinträchtigen von weichen Geweben zu vermeiden. - Das Abstandsstück kann smiley-förmig sein. Die obere und die untere Fläche werden jeweils mit einem Radius versehen, welcher der Form der benachbarten Wirbelendplatten entspricht. Unter Bezugnahme auf
4 und5 definiert die Vorderwand51 des Abstandsstücks50 eine obere Vorderfläche52 und eine untere Vorderfläche53 (auf Grund ihrer anatomischen Position, wenn die Abstandsstücke implantiert werden, so identifiziert). Die Rückwand56 des Abstandsstücks50 definiert eine obere Rückfläche57 und eine untere Rückfläche (nicht gezeigt). Die obere Vorderfläche52 hat eine konkave Form, die einen ersten Radius R1 definiert, der konfiguriert wird, um sich der vorderen Form einer unteren Wirbelendplatte anzupassen. Die untere Vorderfläche53 hat ebenfalls eine konkave Form, die einen zweiten Radius R2 definiert, der konfiguriert wird, um sich der vorderen Form einer oberen Wirbelendplatte anzupassen. Vorzugsweise werden die hinteren Abschnitte des Abstandsstücks ebenfalls ähnlich gekrümmt, wobei die obere Rückfläche57 einen dritten Radius R3 definiert, konfiguriert, um sich der hinteren Form einer unteren Wirbelendplatte anzupassen, und die untere Rückfläche einen vierten Radius (nicht gezeigt) definiert, konfiguriert, um sich der hinteren Form einer oberen Wirbelendplatte anzupassen. Jeder der Radien beträgt für eine Verwendung in der Halswvbelsäule zwischen etwa 1,2 cm (0,500 Zoll) und etwa 3,2 cm (1,250 Zoll). Am bevorzugtesten beträgt jeder der Radien etwa 0,750 Zoll. Diese Smiley-Form erhält die Wirbelendplatten besser als flache Implantate und verleiht dem Abstandsstück50 Drehstabilität. - Die Erfindung sieht ebenfalls bikonvexe Ausführungsbeispiele vor, die anatomiefreundlich sind und die Stabilität steigern. Ein Beispiel eines bikonvexen Abstandsstücks wird in
6 bis9 gezeigt. Die Seitenwände71 ,72 des Abstandsstücks70 definieren jeweils eine konvexe obere Seitenfläche80 ,81 und eine konvexe untere Seitenfläche84 ,85 . Jede der oberen Seitenflächen80 ,81 definiert einen oberen Radius Rs, konfiguriert, um sich der unteren Form einer Wirbelendplatte anzupassen. Gleichermaßen definiert jede der unteren Seitenflächen84 ,85 einen unteren Radius Ri, konfiguriert, um sich der oberen Form einer Wirbelendplatte anzupassen. Vorzugsweise betragen der untere Radius R1 und der obere Radius Rs für eine Verwendung in der Halswirbelsäule jeweils zwischen etwa 1,2 cm (0,500 Zoll) und etwa 3,2 cm (1,250 Zoll). Am bevorzugtesten betragen der untere Radius Ri und der obere Radius Rs jeweils etwa 1,9 cm (0,750 Zoll). Das am meisten bevorzugte anatomiefreundliche Abstandsstück wird, wie in6 und7 gezeigt, eine bikonvexe Form mit einer Smiley-Form verbinden. - Als Wirbeleingriffsmittel wird ein geeignetes Wirbeleingriffsmittel vorgesehen, das einen ausreichenden Reibschluß mit den Wirbeln gewährleistet, um ein Austreiben zu verhindern. Zum Beispiel definieren, wie in
10 gezeigt, die obere Fläche91 und die untere Fläche (nicht gezeigt) des Abstandsstücks90 eine aufgerauhte Oberfläche92 , dafür geeignet, einen Reibschluß mit den benachbarten Wirbeln bereitzustellen. Die aufgerauhten Wirbeleingriffsflächen92 können durch Grobsand-Sandstrahloder Kugelstrahl-Techniken erzeugt werden. Vorzugsweise werden die Vorderwand93 , die Rückwand94 und die Seitenwände86 ,97 während des Sandstrahlens abgedeckt, so daß sie glatt bleiben. - Andere Wirbeleingriffsmittel für Abstandsstücke dieser Erfindung sind Klingenelemente, die von einer der Eingriffsflächen oder beiden vorstehen. Das in
1 und12 gezeigte Abstandsstück101 schließt ein Paar von Klingen102 ein, die von der oberen Fläche105 und der unteren Fläche106 vorstehen. Die Klingen102 schließen jeweils eine Schneidkante108 ein, konfiguriert zum Durchstechen einer Wirbelendplatte. Die Klingen102 können in die Knochenoberfläche getrieben werden, um die Anfangsstabilität der Vorrichtung zu steigern. - Die Stabilität des Fusionsorts kann ebenfalls mit dem Flügelmerkmal gesteigert werden. Unter Bezugnahme auf
6 bis9 umfaßt das Abstandsstück70 Seitenflügel75 , die von einer Außenfläche jeder der Seitenwände71 ,72 vorstehen. Vorzugsweise werden die Flügel zwischen der oberen Fläche77 und der unteren Fläche78 angeordnet. Am bevorzugtesten verlaufen die Seitenflügel75 jeweils von der Vorderwand87 zur Rückwand88 . Dieses Flügelmerkmal stellt Zwischenwirbelabstandsstücken zusätzliche Stabilität bereit, was besonders wichtig sein kann, wenn die Endplatten schwach sind. Eine Komplikation, die auftreten kann, wenn die Endplatten schwach sind, ist, daß die Fusionsvorrichtung in den Wirbelkörper sinken oder sich absenken kann, was ermöglicht, daß der Bandscheibenraum zusammenfällt. Die Seitenflügel75 stehen in den Bandscheibenraum vor und berühren den Knochen, falls ein Absenken auftritt, um jedes weitere Absenken zu verhindern. Die Erfindung sieht vor, daß die Wände des Abstandsstücks eine ausreichende Dicke haben werden, um eine strukturelle Stütze bereitzustellen. Bei einem spezifischen Ausführungsbeispiel zur Verwendung in der Halswirbelsäule waren die Wände jeweils etwa 2 mm dick. - Die Abstandsstücke
10 dieser Erfindung werden außerdem vorzugsweise so geformt, daß sie auf zweckmäßige und wirksame Weise in aktuelle chirurgische Techniken einzubeziehen sind. Zum Beispiel ist ein Abstandsstück dieser Erfindung mit einer flachen Rückwand leicht in eine chirurgische Fusionstechnik nach Smith-Robinson einzubeziehen. Siehe zum Beispiel Smith, M .D., G. W. und Robinson, M. D., R. A., „The Treatment Of Certain Cervical-Spine Disorders By Anterior Removal Of The Intervertebral Disc And Interbody Fusion", J. Bone And Joint Surgery, 40-A: 607–624 (1958), und Cloward, M. D., R. B., "The Anterior Approach For Removal Of Ruptured Cervical Disks", beim Treffen der Harvey Cushing Society, Washington, D .C., 22. April 1958. Nach einer totalen oder partiellen Nukleotomie und Distraktion präpariert der Chirurg die Endplatten für das Abstandsstück10 , vorzugsweise, um flache Hinter- und Seitenkanten zu schaffen. Das Abstandsstück10 paßt mit seinen Flächen satt anliegend an die Hinter- und Seitenkanten, was eine mediale und seitliche Bewegung des Abstandsstücks in Wirbelarterien und -nerven verhindert. Dies verringert durch Vermeiden des pröbelnden Herangehens zum Erreichen einer guten Passung bei Knochentransplantaten außerdem vorteilhafterweise die für die Operation erforderliche Zeit. Normalerweise ist es erforderlich, daß der Chirurg wiederholt am Transplantat herumschneidet, um die richtige Größe zu eneichen, die in den Zwischenwirbelraum paßt. - Die Zwischenwirbelabstandsstücke der vorliegenden Erfindung erfordern keine interne Fixierung. Die Abstandsstücke werden durch die Kompressionskräfte der umgebenden Bänder und Muskeln gehalten. Eine zeitweilige externe Immobilisierung und Stütze wird allgemein empfohlen, bis eine angemessene Fusion erreicht wird. Zum Beispiel wird eine Schanz-Krawatte empfohlen, wenn das Abstandsstück in der Halswirbelsäule implantiert wird.
- Während der Fusionsoperation kann der Chirurg die Kammer
30 mit einem osteogenen Material73 füllen, wie in8 gezeigt. Das osteogene Material ist vorzugsweise ein osteoinduktives Material, wie beispielsweise ein Eigentransplantat (Autogentransplantat), bestimmte Biokeramiken oder Knochenosteogenese-Proteine, wie beispielsweise Knochenmorphogenese-Proteine (BMP – Bone morphogenic proteins), die alle auf dem Gebiet gut bekannt sind. Die Kammer30 kann ebenfalls mit einem osteokonduktiven Material, wie beispielsweise einem Fremdtransplantat (Allogentransplantat) oder bestimmten Biokeramiken, gefüllt werden. Alle geeigneten osteogenen Materialien werden vorgesehen. Die Biokeramiken können biphasische Kalziumphosphat-Keramiken, wie beispielsweise Hydroxyapatit/Trikalziumphosphat-Keramiken, einschließen, die im Handel erhältlich sind. Das osteogene Material kann ein Autogentransplantat oder ein Allogentransplantat sein, einschließlich von Osteozyten oder anderen durch den Chirurgen während des Präparierens der Endplatten für das Abstandsstück abgetragenen Knochen oder eines morcellierten Knochentransplantats aus einer Knochenbank oder eines Autogentransplantats, zum Beispiel vom Darmbeinkamm. - Wenn BMP in dem osteoinduktiven Material eingeschlossen werden, können die BMP natürlich gewonnene oder gentechnisch veränderte BMP sein. Am bevorzugtesten ist das Knochenmorphogenese-Protein ein BMP-2, wie beispielsweise rekombinantes Human-BMP-2. Jedoch wird jedes Knochemnorphogenese-Protein vorgesehen, einschließlich der als BMP-1 bis einschließlich BMP-13 bezeichneten Knochenmorphogenese-Proteine, aber nicht auf dieselben beschränkt. Solche BMP sind erhältlich von der Genetics Institute, Ine., 87 Cambridge Park Drive, Cambridge, MA 02140, und können ebenfalls durch einen Fachmann auf dem Gebiet präpariert werden, wie beschrieben in den US-Patenten Nr. 5 187 076 an Wozney et al., 5 318 898 an Israel, 5 166 058 an Wang et al., 5 366 875 an Wozney et al., 4 877 864 an Wang et al., 5 108 922 an Wang et al., 5 116 738 an Wang et al., 5 013 649 an Wang et al., 5 106 748 an Wozney et al. und den PCT-Patenten Nr. WO93/00432 an Wozney et al., WO94/25893 an Celeste et al. und WO94/26892 an Celeste et al., die hiermit als Referenz einbezogen werden.
- Die BMP werden vorzugsweise mit einem geeigneten Träger
74 in die Kammer30 eingebracht, wie in8 gezeigt. Der Träger kann jedes geeignete Medium sein, das in der Lage ist, die Proteine an das Implantat abzugeben. Solche Träger sind gut bekannt und im Handel erhältlich. Ein bevorzugter Träger ist ein absorbierbarer Kollagenschwamm, wie in8 gezeigt, der durch die Integra LifeSciences Corporation unter dem Handelsnamen Helistat® Absorbable Collagen Hemostatic Agent vermarktet wird. Ein anderer bevorzugter Träger ist ein offenzelliges Polymilchsäwepolymer (OPLA – open cell polylactic acid polymer). Andere potentielle Matrizen für die Zusammensetzungen können biologisch abbaubares und chemisch definiertes Kalziumsulfat, Trikalziumphosphat (TCP), Hydroxyapatit (HA), biphasische TCP/HA-Keramik, Polymilchsäwen und Polyanhydride sein. Andere potentielle Materialien sind biologisch abbaubar und biologisch gut definiert, wie beispielsweise Knochen- oder Hautkollagen. Weitere Matrizen bestehen aus reinen Proteinen oder extrazellulären Matrizenbestandteilen. Das osteoinduktive Material kann ebenfalls eine Beimischung des osteoinduktiven Zytokin und eines polymeren Acrylsäureester-Trägers sein. Das polymere Acrylsäureester kann ein Polymethylmethacrylsäureester sein. Die Träger werden vorzugsweise in Streifen oder Blättern bereitgestellt, die gefaltet werden können, um sich der Kammer30 anzupassen. - Die Trägerauswahl erfolgt auf der Grundlage der biologischen Verträglichkeit, der biologischen Abbaubarkeit, der mechanischen Eigenschaften und der Grenzflächeneigenschaften. Die spezielle Anwendung der Zusammensetzungen der Erfindung wird die passende Formulierung definieren. Der Träger kann jeder geeignete Träger. sein, der in der Lage ist, die Proteine an das Abstandsstück abzugeben.
- Wenn ein Transplantat als osteogenes Material ausgewählt wird, wird vorteilhafterweise nur eine sehr kleine Menge an Knochenmaterial benötigt. Es ist nicht erforderlich, daß das Transplantat selbst eine strukturelle Stütze bereitstellt, da diese durch das Abstandsstück
10 bereitgestellt wird. Statt dessen ist das Transplantat nur wegen seiner osteokonduktiven und/oder osteoinduktiven Eigenschaften erforderlich, um eine Fusion durch den Bandscheibenraum zu fördern. Die Spenderoperation für eine so kleine Knochenmenge ist weniger invasiv und wird durch den Patienten besser vertragen. Es gibt eine geringe Notwendigkeit einer Muskeldissektion beim Gewinnen so geringer Knochenmengen. Die vorliegende Erfindung beseitigt daher viele der Nachteile eines Autogentransplantats. - Die vorliegende Erfindung steigert die Effizienz des Chirurgen beim Implantieren des Abstandsstücks. Ein Durchgangsloch
50 wird in der Vorderwand11 definiert, das konfiguriert wird, um ein Implantierinstrument aufzunehmen. Jedes geeignete Instrument wird vorgesehen, wie beispielsweise der in13 abgebildete Implantateinsetzer120 . - Der Einsetzer
120 schließt einen Griffabschnitt121 ein, mit Rändelungen oder anderen geeigneten Mustern, um ein Greifen des Griffs mit der Hand zu verbessern. Ein Schaft122 steht vom Griff121 vor. Das distale Ende123 des Schafts122 schließt eine Spitze125 ein, die mit dem Durchgangsloch50 zusammenpaßt. Vorzugsweise haben die Spitze125 und das Durchgangsloch50 übereinstimmende zusammenpassende Gewindegänge126 ,51 . Wenn das Durchgangsloch50 , wie in1 gezeigt, in einer gekrümmten Wand definiert wird, schließt das distale Ende123 des Schafts122 vorzugsweise einen gekrümmten Abschnitt124 ein, der sich der gekrümmten Vorderfläche des Implantats anpaßt. Der Einsetzer120 schließt außerdem vorzugsweise einen T-Griff128 zum Steuern und Positionieren des Implantats ein. Vorzugsweise schließt der Einsetzer120 Mittel zum Drehen der Gewindespitze125 ein. In13 wird der Knopf130 über einen Innenschaft, der durch eine innere Bohrung (nicht gezeigt) in dem Griff121 und dem Schaft122 verläuft, an der Spitze125 in Eingriff gebracht. Die Spitze125 befindet sich vorzugsweise am Ende des Innenschafts, wobei der Innenschaft drehbar innerhalb des Griffs121 und des Schafts122 angebracht wird. - Bei der Verwendung des Einsetzers
120 wird ein Abstandsstück10 an der Gewindespitze125 in Eingriff gebracht, wobei der gekrümmte Abschnitt124 bündig mit der Vorderwand11 ist. Danach können der Einsetzer und das Abstandsstück perkutan in den Operationsort vorgeschoben werden, um das Abstandsstück im Intradiskalraum zu implantieren. Sobald das Abstandsstück10 richtig positioniert ist, kann der Knopf130 gedreht werden, um die Gewindespitze125 zu drehen und die Spitze vom Durchgangsloch50 des Abstandsstücks10 zu lösen. Danach kann der Einsetzer120 aus dem Operationsort herausgezogen werden und hinterläßt das Abstandsstück10 an seinem Platz. - Jedes geeignete biologisch verträgliche Material, das hohen Wirbelsäulenlasten widerstehen kann, wird vorgesehen. Die bevorzugtesten Materialien schließen Titan, rostfreien Stahl und bestimmte Bioverbundwerkstoffe, wie beispielsweise den im US-Patent Nr. 5 282 861 an Kaplan beschriebenen und unter dem Namen Hedrocel® vermarkteten, ein. Der Verbundwerkstoff von Kaplan schließt ein nichtmetallisches stanes Schaumsubstrat, das durchgehende, vermaschte Poren definiert, und eine Metallfolie ein, die das vermaschte Netzwerk wesentlich umhüllt. Das Material von Kaplan gewährleistet zwei wichtige Vorzüge: vollständige Porosität und Rauhigkeit. Wie im Patent von Kaplan erörtert, gewährleistet das offenzellige Tantahnaterial eine hochgradig vermaschte dreidimensionale Porosität, die das Einwachsen von Knochen unterstützt. Weil das Material des Abstandsstücks selbst porös ist und das Einwachsen von Knochen unterstützt, besteht keine Notwendigkeit zu zusätzlichem spanabhebendem Herstellen von offenen Seitenschlitzen. Dieses Material gewährleistet außerdem dadurch einen unerwarteten Nutzen, daß die Rauhigkeit der Oberfläche einen Reibschluß zwischen den Wirbelkörpern gewährleistet.
- Jedes geeignet bemessene Abstandsstück wird vorgesehen. Vorzugsweise hat das Abstandsstück
10 eine Höhe, die sich der Höhe eines bestimmten menschlichen Bandscheibenraums nähert. Bei einigen Anwendungen kann es vorzuziehen sein, daß die Höhe des Abstandsstücks10 geringfügig größer ist als die Höhe eines menschlichen Bandscheibenraums, um die Bandscheibenraumhöhe unter den Kompressionskräften der Wirbelsäule zu erhalten und um die Wirkungen von Absenkung und Knochenerosion zu vermeiden. Bei Halsanwendungen haben einige spezifische Ausführungsbeispiele eine Höhe von etwa 6 mm bis etwa 10 mm. Bei anderen spezifischen Ausführungsbeispielen zur Anwendung in der Halswirbelsäule haben die Abstandsstücke eine Breite von zwischen etwa 10 mm bis etwa 14 mm und eine Länge von zwischen etwa 10 bis etwa 14 mm. Die Erfindung sieht vor, daß die Wände des Abstandsstücks eine ausreichende Dicke haben, um eine strukturelle Stütze bereitzustellen. Bei einem spezifischen Ausführungsbeispiel zur Verwendung in einer Halswirbelsäule waren die Wände jeweils etwa 2 mm dick. Es wird ebenfalls vorgesehen, mit angemessenen chirurgischen Techniken Brust- und Lendenabstandsstücke mit einer angemessenen Größe zu verwenden. - Diese Erfindung stellt Implantate für einen Eingriff zwischen Wirbeln bereit, die den Zwischenwirbelscheibenraum wiederherstellen und die Wirbelsäule stützen, während das Einwachsen von Knochen gefördert wird. Die Abstandsstücke sind anatomiefreundlich und stellen einen Träger für osteogenes oder osteokonduktives Material bereit. Die Implantate dieser Erfindung vermeiden die mit einem Allogentransplantat und einem Autogentransplantat verbundenen Probleme, wie beispielsweise die Notwendigkeit eines pröbelnden Zurichtens von Transplantatmaterial, damit es in den Intradiskalraum paßt, die Morbidität des Spenderorts und die Gefahren einer Krankheitsübertragung.
- Während die Erfindung in den Zeichnungen und der vorstehenden Beschreibung illustriert und beschrieben worden ist, sind dieselben als von illustrativem und nicht-einschränkendem Charakter zu betrachten, wobei es sich von selbst versteht, daß nur die bevorzugten Ausführungsbeispiele gezeigt und beschrieben worden sind, und daß alle Veränderungen und Modifikationen, die in den Rahmen der angefügten Ansprüche fallen, geschützt werden sollen.
Claims (37)
- Hohles Wirbelsäulenabstandsstück (
10 ,50 ,70 ,90 ,101 ) für einen Eingriff zwischen Wirbeln, das eine Vorderwand (11 ,51 ,87 ), eine Rückwand (15 ,56 ,88 ) und zwei Seitenwände (21 ,22 ,71 ,72 ) hat, integral zwischen der Vorder- (11 ,51 ,87 ) und der Rückwand (15 ,56 ,88 ) verbunden, um eine Kammer (30 ) zu definieren, wobei die Wände eine obere Wirbeleingriffsfläche (35 ,77 ) mit einer ersten Öffnung in Verbindung mit der Kammer (30 ) und ebenfalls eine untere Wirbeleingriffsfläche (40 ,78 ) mit einer zweiten Öffnung in Verbindung mit der Kammer (30 ) definieren, dadurch gekennzeichnet, daß das Abstandsstück außerdem eine Klinge (102 ) umfaßt, die von einer der Eingriffsflächen (35 ,40 ,77 ,78 ) vorsteht und eine Schneidkante (108 ) hat, konfiguriert zum Durchstechen einer Wirbelendplatte. - Abstandsstück nach Anspruch 1, das außerdem ein osteoinduktives Material (
73 ) umfaßt, das innerhalb der Kammer (30 ) enthalten ist. - Abstandsstück nach Anspruch 2, bei dem das osteoinduktive Material (
73 ) ein Eigentransplantat ist. - Abstandsstück nach Anspruch 2, bei dem das osteoinduktive Material (
73 ) eine Biokeramik ist. - Abstandsstück nach Anspruch 4, bei dem die Biokeramik eine biphasische Kalziumphosphatkeramik ist.
- Abstandsstück nach Anspruch 2, bei dem das osteoinduktive Material (
73 ) ein Knochenmorphogeneseprotein in einem Träger (74 ) einschließt. - Abstandsstück nach Anspruch 6, bei dem das Knochenmorphogeneseprotein aus der Gruppe ausgewählt wird, die aus BMP-1, BMP-2, BMP-3, BMP-4, BMP-5, BMP-6, BMP-7, BMP-8, BMP-9, BMP-10, BMP-11, BMP-12 und BMP-13 besteht.
- Abstandsstück nach Anspruch 7, bei dem das Knochenmorphogeneseprotein BMP-2 ist.
- Abstandsstück nach Anspruch 8, das außerdem BMP-7 enthält.
- Abstandsstück nach Anspruch 6, bei dem der Träger (
74 ) aus der Gruppe ausgewählt wird, die aus Kalziumsulfat, Polymilchsäwen, Polyanhydriden, Kollagen, Kalziumphosphatkeramiken und polymeren Acrylsäureestern besteht. - Abstandsstück nach Anspruch 10, bei dem der Träger (
74 ) ein offenporiges Polymilchsäwepolymer ist. - Abstandsstück nach Anspruch 10, bei dem der Träger (
74 ) Kollagen enthält. - Abstandsstück nach Anspruch 12, bei dem der Träger (
74 ) fibrilläres Kollagen ist. - Abstandsstück nach Anspruch 12, bei dem der Träger (
74 ) ein Kollagenschwamm ist. - Abstandsstück nach Anspruch 10, bei dem der Träger (
74 ) in Streifen bereitgestellt wird, gefaltet, um sich der Kammer (30 ) anzupassen. - Abstandsstück nach Anspruch 10, bei dem der Träger (
74 ) in Blättern bereitgestellt wird, gefaltet, um sich der Kammer (30 ) anzupassen. - Abstandsstück nach Anspruch 1, das außerdem ein osteoinduktives Material (
73 ) umfaßt, das innerhalb der Kammer (30 ) enthalten ist. - Abstandsstück nach Anspruch 17, bei dem das osteoinduktive Material (
73 ) ein Fremdtransplantat (Allogentransplantat) ist. - Abstandsstück nach Anspruch 1, bei dem jede der Seitenwände (
7l ,72 ) eine Außenfläche hat und außerdem einen Seitenflügel (75 ) umfaßt, der von der Außenfläche jeder der Seitenwände (71 ,72 ) vorsteht, wobei jeder Flügel (75 ) zwischen der unteren (78 ) und der oberen Fläche (77 ) angeordnet wird. - Abstandsstück nach Anspruch 19, bei dem sich der Flügel (
75 ) von der Vorderwand (87 ) zur Rückwand (88 ) erstreckt. - Abstandsstück nach Anspruch 20, das außerdem ein erstes Paar von Klingen (
102 ), die von der oberen Fläche (77 ) vorstehen, und ein zweites Paar von Klingen (102 ), die von der unteren Fläche (78 ) vorstehen, umfaßt, wobei jede der Klingen (102 ) eine Schneidkante (108 ) hat, konfiguriert zum Durchstechen einer Wirbelendplatte. - Abstandsstück nach Anspruch 1, das außerdem ein erstes Paar von Klingen (
102 ), die von der oberen Fläche (35 ) vorstehen, und ein zweites Paar von Kingen (102 ), die von der unteren Fläche (40 ) vorstehen, umfaßt, wobei das erste und das zweite Paar von Kingen (102 ) jeweils eine Schneidkante (108 ) hat, konfiguriert zum Durchstechen einer Wirbelendplatte. - Abstandsstück nach Anspruch 1, bei dem die Vorderwand (
51 ) eine obere Vorderfläche (52 ) und eine untere Vorderfläche (53 ) definiert, wobei die obere Vorderfläche (52 ) eine konkave Form mit einem ersten Radius (R1) hat und die untere Vorderfläche (53 ) eine konvexe Form mit einem zweiten Radius (R2) hat. - Abstandsstück nach Anspruch 23, bei dem der erste Radius (R1) zwischen etwa 1,2 cm (0,500 Zoll) und etwa 3,2 cm (1,250 Zoll) beträgt und der zweite Radius (R2) zwischen etwa 1,2 cm (0,500 Zoll) und etwa 3,2 cm (1,250 Zoll) beträgt.
- Abstandsstück nach Anspruch 24, bei dem sowohl der erste als auch der zweite Radius (R1, R2) etwa 1,9 cm (0,750 Zoll) betragen.
- Abstandsstück nach Anspruch 23, bei dem die Rückwand (
56 ) eine obere Rückfläche (57 ) und eine untere Rückfläche definiert, wobei die obere Rückfläche eine konkave Form hat, die eine dritten Radius (R3) definiert, wobei der dritte Radius (R3) konfiguriert wird, um sich der hinteren Form einer unteren Wirbelendplatte anzupassen, und die untere Rückfläche eine konvexe Form hat, die einen vierten Radius definiert, wobei der vierte Radius konfiguriert wird, um sich der hinteren Form einer oberen Wirbelendplatte anzupassen. - Abstandsstück nach Anspruch 26, bei dem der dritte Radius (R3) zwischen etwa 1,2 cm (0,500 Zoll) und etwa 3,2 cm (1,250 Zoll) beträgt und der vierte Radius zwischen etwa 1,2 cm (0,500 Zoll) und etwa 3,2 cm (1,250 Zoll) beträgt.
- Abstandsstück nach Anspruch 26, bei dem der erste und der zweite Radius (R1, R2) etwa 1,9 cm (0,750 Zoll) betragen.
- Abstandsstück nach Anspruch 1, bei dem die obere Fläche ein Paar von oberen Seitenflächen (
80 ,81 ), definiert durch jede Seitenwand (71 ,72 ), einschließt, wobei die oberen Seitenflächen eine konvexe Form haben, und die untere Fläche ein Paar von unteren Seitenflächen (84 ,85 ), definiert durch jede Seitenwand (71 ,72 ), einschließt, wobei die unteren Seitenflächen eine konvexe Form haben. - Abstandsstück nach Anspruch 1, bei dem die Wände und die Flächen aus einem biokompatiblen Verbundstoff bestehen, der ein starres kohlenstoffhaltiges Schaummaterial und eine dünne Folie aus einem auf das kohlenstoffhaltige Material abgesetzten metallischen Material einschließt.
- Abstandsstück nach Anspruch 1, bei dem die erste Klinge (
102 ) von der oberen Fläche (35 ) vorsteht. - Abstandsstück nach Anspruch 31, das außerdem eine zweite Klinge (
102 ) umfaßt, die von der oberen Fläche (35 ) vorsteht, wobei die zweite Klinge (102 ) parallel zur ersten Klinge (102 ) verläuft und eine Schneidkante (108 ) hat, konfiguriert zum Durchstechen einer Wirbelendplatte. - Abstandsstück nach Anspruch 32, das außerdem ein Paar von unteren Klingen (
102 ) umfaßt, die von der unteren Fläche (40 ) vorstehen, wobei jede untere Klinge (102 ) eine Schneidkante (108 ) hat, konfiguriert zum Durchstechen einer Wirbelendplatte. - Abstandsstück nach Anspruch 1, bei dem die obere Fläche (
35 ) ein Paar von oberen Seitenflächen (77 ), definiert durch jede Seitenwand (21 ,22 ), einschließt, wobei die oberen Seitenflächen (77 ) eine konvexe Form haben, und die untere Fläche (40 ) ein Paar von unteren Seitenflächen (78 ), definiert durch jede Seitenwand (21 ,22 ), einschließt, wobei die unteren Seitenflächen (78 ) eine konvexe Form haben . - Abstandsstück nach Anspruch 34, bei dem die obere Fläche (
35 ) eine durch die Vorderwand (11 ) definierte obere Vorderfläche (52 ) einschließt, wobei die obere Vorderfläche (52 ) eine konvexe Form hat, die einen ersten Radius (R,) definiert, und bei dem die untere Fläche (40 ) eine durch die Vorderwand (11 ) definierte untere Vorderfläche (53 ) einschließt, wobei die untere Vorderfläche (53 ) eine konvexe Form hat, die einen zweiten Radius (R2) definiert. - Abstandsstück nach Anspruch 1, bei dem die Vorderwand (
11 ) eine konvex gekrümmte Vorderfläche hat und die Rückwand (15 ) eine flache Rückfläche hat. - Abstandsstück nach Anspruch 1, bei dem die Vorderwand (
11 ) ein Durchgangsloch (60 ) definiert, konfiguriert zum Aufnehmen eines Implantierinstruments (120 ).
