Description
- Hintergrund der Erfindung
- Diese Erfindung betrifft vaskuläre Prothesen mit verbesserter Biokompatibilität und insbesondere ein Verfahren, ein Kollagenmaterial auf einen expandierbaren Stent aufzutragen. Eine derartiges Verfahren stellt einen endovaskulären Stent zur Verfügung, der die Gefäßwand schützt und für den Stent in dem Gefäßlumen ein nicht-thrombogenes Polster bildet.
- Sie betrifft ebenso Stents in Kombination mit einem Kollageneinsatzmaterial. Eine derartige Kombination stellt einen endoluminalen Stent zur Verfügung, der sich in der luminalen Wand festsetzt und im Fall von Gefäßanwendungen eine nicht-thrombogene Oberfläche ausbildet, wie auch das Wachstum der endothelialen Zellen gewährleistet, und der ferner ein Reservoir oder einen Befestigungspunkt für therapeutische Wirkstoffe bei jedweder Verwendung zur Verfügung stellt.
- Sie betrifft ebenfalls Kombinationen aus beiden der vorgenannten Arrangements. Ganz allgemein bezieht sie sich auf Stents, die mit einer äußeren Umhüllung aus Kollagenmaterial und/oder einem luminalen Einsatz daraus verknüpft sind.
- Stents sind im allgemeinen rohrförmig im Aufbau, mit offenen Enden, und sind zwischen einem im allgemeinen nicht-expandierten Insertionsdurchmesser und einem expandierten Implantierungsdurchmesser, der größer ist als der nicht-expandierte Insertionsdurchmesser, radial expandierbar. Derartige intravaskuläre Implantate werden zur Aufrechterhaltung der Gefäßdurchgängigkeit bei Menschen und Tieren verwendet.
- Stents werden üblicherweise mittels eines mechanischen transluminalen Verfahrens platziert oder implantiert. Eine übliche Vorgehensweise bei der Implantierung eines Stents ist es, zunächst die Gefäßregion mit einem Ballonkatheter zu öffnen und anschließend den Stent in einer Position zu platzieren, welche den behandelten Gefäßteil mittels eines Platzierungskatheters überbrückt.
- Die Patente des Standes der Technik beziehen sich auf die Konstruktion und den Aufbau von Stents, wie auch auf Geräte zur Positionierung von Stents in einem Gefäß. Im allgemeinen offenbaren diese Patente beispielsweise eine Technik zur Positionierung eines länglichen, zylindrischen Stents im Bereich eines Aneurysmas, einer Stenose oder dergleichen. Der Stent expandiert wie erforderlich nach dem Einsetzen mit Hilfe eines Katheters auf eine implantierte Konfiguration.
- Insbesondere offenbart das US-Patent 4,733,665 von Palmaz, veröffentlicht am 29. März 1988, eine Reihe von Stentkonfigurationen zur Implantierung mit Hilfe eines Katheters. Der Katheter umfasst Mittel zur Befestigung und zum Zusammenhalten des Stents, vorzugsweise auf einem aufblasbaren Teil des Katheters. Der Stent wird durch Positionieren innerhalb des Blutgefäßes und Überwachen seiner Position auf einem Sichtmonitor implantiert. Sobald der Stent richtig positioniert ist, wird der Katheter expandiert und der Stent von dem Katheterkörper entfernt. Der Katheter kann anschließend aus dem Subjekt zurückgezogen werden, wobei der Stent vor Ort innerhalb des Blutgefäßes zurückbleibt. Das am 21. August 1990 veröffentlichte US-Patent 4,950,227 von Savin et al. beschreibt Ähnliches.
- Ein weiteres ähnliches US-Patent, 5,019,090, offenbart einen im allgemeinen zylindrischen Stent und eine Technik zu dessen Implantierung unter Verwendung eines entleerten Ballonkatheters, um den Stent innerhalb eines Gefäßes zu positionieren. Sobald der Stent richtig positioniert ist, wird der Ballon aufgeblasen, um den Stent gegen die inneren Wandabschnitte des Gefäßes zu drücken. Der Ballon wird anschließend entleert und aus dem Gefäß zurückgezogen, wobei der Stent vor Ort zurückbleibt.
- Ein Patent von Dotter, US-Patent 4,503,569, veröffentlicht am 12. März 1985, offenbart einen Federstent, der bei Temperaturveränderung auf eine implantierte Konfiguration expandiert. Der Federstent wird in einer zusammengerollten Orientierung implantiert und erwärmt, um die Feder zur Expansion zu bringen, aufgrund der Eigenschaften der Form-Gedächtnis-Legierung, aus der der Stent gemacht ist. In ähnlicher Weise offenbart das US-Patent 4,512,338 von Balko et al., veröffentlicht am 23. April 1985, einen Form-Gedächtnis-Legierungs-Stent und ein Verfahren zu dessen Aussetzung und Verwendung. Andere Arten selbst-expandierender Stents sind im Stand der Technik bekannt.
- Die Einführung und Expansion des erfindungsgemäßen Stents ist die gleiche wie bereits im Stand der Technik bekannt, und wird mit dem Stent der
1 und6 ausgeübt. Das US-Patent 5,195,984 von Schatz, veröffentlicht am 23. März 1993, beschreibt eine typische Ballon-Expansionsprozedur bei einem expandierbaren Stent. Dieses Patent beschreibt einen Katheter mit einem damit verbundenen expandierbaren, aufblasbaren Teil. Auf herkömmliche Weise werden Katheter und Stent an die gewünschte Stelle in einem Körperdurchgang gebracht, wo es erwünscht ist, den Stent für die Implantierung zu expandieren. Fluoroskopie und/oder andere herkömmliche Techniken können verwendet werden, um sicherzustellen, dass Katheter und das Transplantat (graft) an die gewünschte Stelle gebracht werden. Der Stent wird anschließend in kontrollierter Weise durch kontrolliertes Expandieren des expandierbaren, aufblasbaren Teils des Katheters, üblicherweise ein Ballon, expandiert und deformiert. Im Ergebnis wird der Stent radial auswärts bis zum Kontakt mit den Wänden des Körperdurchgangs verformt. In dieser Hinsicht kann der expandierbare, aufblasbare Teil des Katheters ein herkömmlicher Angioplastie-Ballon, wie bereits im Stand der Technik bekannt, sein. Nachdem die erwünschte Expansion und Verformung des Stents bewerkstelligt wurde, kann der Angioplastie-Ballon entleert werden und der Katheter auf herkömmliche Weise aus dem Durchgang entfernt werden. - Die vorliegende Erfindung ist ebenfalls nützlich bei selbstexpandierbaren Stents wie diejenigen, die in den US-Patenten 4,732,152 und 4,848,343 offenbart sind.
- Die vorliegende Erfindung stellt ein Verfahren, wie im Anspruch 1 angeben, zur Verfügung. Bevorzugte Ausführungsformen werden in den abhängigen Ansprüchen dargelegt.
- Gemäß einer bevorzugten Ausführungsform wird ein metallischer oder anderer Stent zur vaskulären Implantierung mit einer deckenden Hülle aus Kollagenmaterial zur Verfügung gestellt. Falls der Stent von der Art mit variablem Durchmesser ist, kann die Hülle zurechtgezogen werden oder auf andere Weise zwischen dem Stent und der Gefäßwand positioniert werden, wenn der Stent gesetzt oder entfaltet wird. Ein Medikament oder ein anderer Wirkstoff, wie Heparin oder dergleichen, kann in dem Kollagen enthalten sein, um nach der Entfaltung des Stents freigesetzt zu werden.
- Gemäß einer anderen bevorzugten Ausführungsform wird ein metallischer oder anderer Stent für die vaskuläre Implantierung mit einem luminalen Einsatz (liner) aus Kollagenmaterial zur Verfügung gestellt. Ein Medikament oder ein anderer Wirkstoff, wie Heparin oder dergleichen, kann in dem Kollagen enthalten sein, als Oberflächenbehandlung oder für die Freisetzung nach dem Einsetzen des Stents.
- In einer weiteren bevorzugten Ausführungsform wird ein Stent mit sowohl einem inneren Kollagenliner und einer äußeren Kollagenbeschichtung zur Verfügung gestellt.

10 ist üblicherweise metallisch, wie z.B. aus rostfreiem Stahl, Nitinol, superelastischen Legierungen und anderen Metallen, oder aus einem geeigneten polymeren Kunststoff, und kann einen Fixdurchmesser oder einen variablen Durchmesser aufweisen, wobei Letzteres bevorzugt ist und im Stand der Technik wohl bekannt ist. Die Typen mit variablem Durchmesser sind üblicherweise entweder Ballon-expandierbar oder selbstexpandierend, wobei beide ebenfalls im Stand der Technik bekannt sind. Beispiele des ersteren Typs sind im US-Patent 4,733,665, im US-Patent 4,739,762 sowie im US-Patent 4,776,337 gezeigt. Der letztere Typ wird für die Zwecke der vorliegenden Erfindung gegenwärtig bevorzugt, d.h. selbstexpandierende, insbesondere solche aus Nitinol, wovon ein Beispiel in den US-Patenten 4,503,569 und 4,512,338 diskutiert wird. Ebenso verwendbare Stents sind beispielsweise in WO-A-95/31945, mit dem Titel "Verbesserte gewebestützende Vorrichtungen" gezeigt. - In jedem Fall stellt ein Stent im allgemeinen eine Stützgerüststruktur zur Verfügung, die vielerlei Formen annehmen kann. Typischerweise sind Stents offen oder perforiert und können aus einem Netzwerk von Streben oder aus einer drahtähnlichen Struktur bestehen. Der Stent
10 besteht aus Streben. - Die in
1 gezeigte Kollagenhülle12 kann aus Kollagen per se bestehen oder sie,12a , kann auf einem Träger12b wie in2 gezeigt, getragen werden, wobei der Träger12b aus DACRON® Stoff oder dergleichen besteht, wie bekannt und z.B. in den US-Patenten 5,256,418, 5,201,764 sowie 5,197,977 offenbart ist, vor allem diese Teile, die sich auf die Bildung von Kollagenrohren beziehen. Der Träger12b kann ein Stoff sein, gewebt oder geflochten, und kann ebenso aus Polyester, Polyethylen, Polyurethan oder PTFE sein. Der Begriff "Kollagenmaterial" wird hierin so verwendet, dass er sich sowohl auf geträgertes und ungeträgertes Kollagen für das Hüllenelement dieser Erfindung bezieht. - Das gegenwärtig bevorzugte Kollagen für die Zwecke dieser Erfindung scheint das aus Rinder- oder Schweine-Typ I oder Typ IV-Kollagen und Kombinationen davon in einer doppelschichtigen blattartigen Form zu sein. Das Kollagen kann auch aus Typ III oder Kombinationen aus beliebigen der verschiedenen Typen sein. Die US-Patente 4,837,379; 4,902,508; 4,950,483; 4,956,178; 5,106,949; 5,110,064; 5,256,418; 5,275,826; 5,281,422 sowie 5,024,841 betreffen Kollagenzusammensetzungen und deren Herstellung, die in der vorliegenden Erfindung verwendbar sind. Kollagen kann aus verschiedenen strukturellen Geweben wie im Stand der Technik bekannt extrahiert werden, und in Blätter oder Röhren umgewandelt werden und auf einem Stent unter Trocknung aufgebracht werden. Im allgemeinen wird die Dicke dieser Blätter oder Röhren von etwa 5 bis 200 Mikron betragen. Ein gegenwärtig bevorzugtes Kollagen ist das im US-Patent 4,902,508 offenbarte, aufgetragen wie im US-Patent 5,275,826 beschrieben, um zweischichtiges SIS wie nachfolgend weiter beschrieben zur Verfügung zu stellen. Ein weiteres bevorzugtes Kollagen ist das als ein "Kollagenkonstrukt" in dem US-Patent 5,256,418 beschriebene, insbesondere das, bei dem das permeable Substrat ebenfalls Kollagen ist.
- Die Zellen der Blutgefäßwand synthetisieren und scheiden verschiedene Arten von Makromolekülen ab, welche die extrazelluläre Matrix bilden. Die Bestandteile dieser Matrix umfassen diverse große Proteine, die synthetisch umgebaut werden können, um Filme, Röhren oder vielschichtige Blätter und andere Konstrukte zu bilden. Unter diesen biologischen Bestandteilen sind Kollagene verschiedener Arten, Elastin, Glycosaminoglycane (GAGs), Fibronektin und Laminin. Kollagene sind dreikettige Glycoproteine mit Molekulargewichten von etwa 300.000. Elastin ist ein unlösliches nichtpolares, aminosäurereiches, vernetztes Protein. Die GAGs sind lineare kettenförmige Polysaccharide mit diversen negativen Ladungen und diversen Molekulargewichten, die von einigen Tausend bis zu Millionen reichen. In den GAGs eingeschlossen sind Heparin und Heparinsulfat, Dermatinsulfat und Chondroitinsulfat. Fibronektin ist ein 2-kettiges, klebriges Glycoprotein mit einem Molgewicht (MW) von 440.000, das als Substrat für viele Zellarten und in Zelle-Zelle-Wechselwirkungen agiert. Laminin ist ein 2-kettiges Glycoprotein mit einem MW von etwa 850.000 und wirkt als eine Grundmembranstruktur für zellular-molekare Wechselwirkungen. Jedes dieser Makromoleküle kann in einer Vielzahl von Kombinationen zur Bildung von Kompositen kombiniert werden. Dies alles sind natürliche Materialien, die spezifischen Funktionen dienen und unter normalen Reparaturbedingungen dem Blut ausgesetzt sind. Es wird daher erwartet, dass, wenn eine abdeckende Hülle für einen Stent aus diesen Makromolekülen hergestellt und im Verlauf der Intervention verwendet wird, die Reparatur eines Blutgefäßes auf natürlichere Weise voranschreitet, als wenn eine ähnliche Vorrichtung aus synthetischen Polymeren wie z.B. Polyethylen, Polyterephthalat oder Polyurethanen aufgebaut ist. Derartige Materialien werden vorliegend ebenfalls allgemein als "Kollagen" bezeichnet. Der Begriff "Kollagen", wie hier verwendet, bezieht sich daher nicht nur auf die spezifische Klasse von Makromolekülen, die als Kollagen bekannt ist, sondern auch auf solche natürliche Materialien, die normalerweise oder natürlicherweise Membranen mit Kollagen bilden, wie z.B. Laminin, Keratin, Glycosaminoglycane, Proteoglycane, reine Kohlenhydrate, Fibrin, Fibronektin, Hyaluronsäure und dergleichen, sowie andere natürliche Materialien, die mit Kollagen in Kontakt kommen und zu Filmen verarbeitet werden können, einschließlich Albumin, Globuline und andere aus Blut stammende Proteine. Rohrförmige Filme, die aus jeder beliebigen Kombination der obigen Materialien hergestellt werden, dienen im wesentlichen dem gleichen Zweck wie die aus reinem Kollagen.
- Die Wechselwirkung von Blut mit den verschiedenen oben beschriebenen Membranbestandteilen bestimmt die nachfolgenden Reaktionen bei der Reparatur des Blutgefäßes. Die anfängliche Thrombusbildung, Adhäsion und Aktivierung von Plättchen und die anfänglichen Vorgänge im Zusammenhang mit intimaler Hyperplasie, wie z.B. Schädigung der internen elastischen Schichten, gehören zu diesen Vorkommnissen. Diese Vorgänge sind natürliche Bestandteile des Reparaturprozesses. Normalerweise behindern diese Vorgänge nicht die Durchflussbedingungen des Blutes, mit Ausnahme von Fällen schweren Traumas. Auf Blutgefäßoberflächen bilden sich ständig Microthrombi und lösen sich wieder auf, so dass es vorteilhaft wäre, Stent- oder Transplantat-Beschichtungen aus Materialien zu bilden, die daran gewöhnt sind, eine Thrombusform aufzuweisen, so dass nachfolgende Lysisreaktionen dieser Thrombi auf natürliche und ungehinderte Weise voranschreiten können. Eine Hülle oder ein Liner aus diesen makromolekularen Bestandteilen, der eine schützende Schicht bildet, wird sich als vorteilhaft erweisen, wenn er zusammen mit Stents verwendet wird. Metallische oder polymere Stents, die der arteriellen Wand mechanische Stabilität verleihen, um zerstörtes Gewebe aufrechtzuhalten, können auch verwendet werden, um eine Hülle bestehend aus Kollagen zu halten.
- Trotzdem ist es vernünftig, sich gegen solche Vorgänge zu schützen, da etwas, das nicht als ein natürlicher Bestandteil im Körper gebildet wird, extreme und unerwartete Reaktionen zeigen kann, wie z.B. Blutgefäßverschluss aufgrund von Thrombusbildung oder Spasmen, und weil eine Beschädigung von Blutgefäßen durch den Akt des Einsetzens einer Vorrichtung selbst extrem und übermäßig verletzend auf die Blutgefäßoberfläche einwirken kann. Die oben beschriebenen Materialien sind in der Lage, so verändert zu werden, dass sie hydrophil oder hydrophob werden, mit Dicken, die von etwa 5 bis zu mehreren Hundert Mikron betragen. Sie können wasserlöslich gemacht werden, unlöslich und mit verschiedenen Porositäten.
- Sie können auch so konstruiert werden, dass sie Bereiche verschiedener Hydrophilie und Porosität aufweisen. Die Porositätssteuerung ist gut bekannt.
- Als solche können die Stent-Hüllen oder Liner, die aus diesen Materialien konstruiert wurden, als Reservoire für pharmazeutische Wirkstoffe und dergleichen verwendet werden. Hydrophile Wirkstoffe wie Heparin oder Hirudin zum Schutz gegen Coagulation, oder hydrophobe Wirkstoffe, wie Prostaglandine oder Aspirin und Vitamin E, können zum Schutz gegen Plättchenaktivierung verwendet werden. Vitamin E und andere Antioxidantien wie Natriumascorbat, Phendies, Carbazole und Tocotrienole können als Schutz gegen Oxidation verwendet werden. Am bevorzugtesten wird das Kollagenmaterial eine Menge an Wirkstoffmaterial wie Heparin enthalten, das auf bekannte Weise zur Freisetzung nach der Platzierung des Stents in dem Kollagen eingebaut werden kann. Im allgemeinen können die Wirkstoffmaterialien die bekannten antithrombotischen Wirkstoffe, antibakteriellen und/oder antimikrobiellen Wirkstoffe, Antipilzmittel und dergleichen enthalten.
- Während des Herstellungsprozesses der Hülle oder des Blattes können verschiedene Bestandteile vor dem Trocknen zu der Lösung zugesetzt werden, oder sie können nach der Bildung der Vorrichtung separat zugesetzt werden. Heparin kann direkt zu der Herstellungslösung zugesetzt werden, wie auch Aspirin. Benzalkonium-Heparin, eine modifizierte Form von Heparin, die stärker hydrophob ist, kann verwendet werden, um die gebildete Vorrichtung oder den Film aus einer alkoholischen Lösung zu beschichten. Prostaglandine PGI2 oder PGE2 können aus einer Lösung von Propanol oder Propanol/Methylenchlorid auf eine aus einer wässrigen Basis gebildete Kollagenhülle aufgebracht werden. Vitamin E kann sogar aus noch weniger polaren Lösungen wie Chloroform zugesetzt werden. RGD-Peptid, Thrombomodulin, TPA ("Tissue Plasminogen Activator", Gewebe-Plasminogen-Aktivator) und Urokinase sind Beispiele von bioaktiven Proteinen, die zugesetzt werden können. Gentherapeutische Wirkstoffe wie z.B. Antiplättchen- und Antikörperfragmente, z.B. GB2B3A, können eingebaut werden. Andere Wirkstoffe können auf ähnliche Weise zugesetzt werden. Der Begriff "Wirkstoff' wird hier so verwendet, dass er all diese Additive umfasst.
- Vitamin E ist ein bekanntes Antioxidans. Es wird in Polymeren und als medizinischer Wirkstoff verwendet. Es kann auch in biologisch abbaubaren Stents für vielfältige Zwecke verwendet werden. In derartigen Stents des Polymertyps, die eine bestimmte Form von Energie wie Wärme oder Licht benötigen, um zugeführt werden zu können, kann es dazu dienen, die darin enthaltenen Polymere gegen unerwünschte Oxidation durch die Energiequelle zu schützen. Auch weil Gewebeschädigung durch Oxidation verursacht wird, die von zellulären Bestandteilen wie Macrophagen und Neutrophilen stammt, kann Vitamin E zum Schutz des Gewebes dienen, wenn es aus implantierten Vorrichtungen austritt. Es kann auch dazu dienen, das Polymer während der Extrusions- oder Wärmebehandlung als Pressfilme zu schützen. Es kann auch dazu dienen, das Material anstelle von anderen nicht FDA zugelassenen Materialien zu plastifizieren. Es ist daher beabsichtigt, dass Vitamin E auch in Kombination mit dem Stent oder Kollagenmaterial oder dergleichen in dieser Erfindung für verschiedene Zwecke verwendet werden kann.
- Ein primäres Ergebnis der Verwendung einer Kollagenhülle aus natürlichen Bestandteilen ist, dass das zelluläre Wachstum des Endotheliums auf einem natürlichen Substrat stattfinden wird, das im wesentlichen unbeschädigt und gleichmäßig ist und gegen Gewebelappen und Aussetzung von nekrotischem und arteriosklerotischem Gewebe gegen Blut schützt. In dieser Hinsicht gewährleistet die Hülle einen biologischen Schutz.
- Von metallischen Stents weiß man, dass sie manchmal bei der Expansion das Gewebe physisch schädigen. Eine aus einem biologischen Material hergestellte Hülle ist natürlicherweise weich im Vergleich zu den zur Herstellung von Stents verwendeten Metallen oder Polymeren. Eine aus Kollagen bestehende Hülle kann ausreichend dick und fest hergestellt werden, so dass sie jede durch die Streben oder andere Elemente eines der metallischen Stents verursachte Beschädigung des übrigen gesunden Endotheliums und der internen elastischen Lamina verhindert oder diese auf ein Minimum reduziert. Die Porosität der Hülle kann die Diffusion von essentiellen Fluidkomponenten aus dem Blut zum darunter liegenden überlebenden Gewebe ermöglichen. In dieser Hinsicht verhalten sich der Vorzug des biologischen Gewebeschutzes durch die Hülle und die gewährleistete physische Protektion additiv.
- Sowohl die biologischen als auch die physischen Vorteile, die hier beschrieben werden, können nicht durch synthetische Hüllen wie Dacron oder PTFE gewährleistet werden.
- Im Falle von Stents mit variablem Durchmesser kann die Hülle ein wenig elastisch sein und auf den zusammengezogenen Stent passen und sich zusammen mit diesem bei der Aussetzung ausdehnen, oder sie kann relativ locker angepasst sein, um sich dem expandierten Stent bei der Aussetzung ohne jede zusätzliche Spannung anzupassen. Alternativ und am bevorzugtesten kann der Stent zeitweise expandiert sein und das Kollagen darauf platziert werden. Das Kollagen kann anschließend hydriert werden und der Stent auf seine nicht expandierte Form kontrahiert werden. Anschließend wird das Kollagen dehydriert und passt fest auf den Stent.
- Eine Hülle oder ein Liner können so hergestellt sein, dass sie durch Veränderung der Vernetzungsdichte des Kollagens stärker elastisch sind. Dies kann auf eine Vielzahl von Wegen bewerkstelligt werden. Kollagenhüllen können mit einer sehr geringen Vernetzungsdichte hergestellt werden. Die Vernetzungsdichte kann auf eine Vielzahl von Wegen erhöht werden, Dehydrierung, Aussetzung an Strahlung oder Wärme sind einige Beispiele hierfür. Chemische Wirkstoffe, die mit dem Kollagen reagieren, z.B. kurzkettige Dialdehyde oder Formaldehyd, können angewendet werden, um das Kollagen zu vernetzen. Die Vermeidung der vorgenannten Verfahren kann eine nicht vernetzte Struktur sicherstellen und zu einem gewissermaßen elastischen Material führen. Die Vernetzung mit den entsprechenden Reagenzien kann auch die Elastizität der Kollagenhülle erhöhen. Derartige Reagenzien sind langkettige, difunktionale Moleküle von C12 und mehr, wie z.B. Polyether oder aliphatische Dialdehyde, aktivierte Diester, wie N-Hydroxy-Succinimidester und Disäurechloride. Diese aktiven Ester reagieren mit den vorliegenden Aminen auf den Kollagenketten, wobei diese durch eine flexible Verbindung verbrückt werden, was eine Expansion ohne Defekte und Risse ermöglicht. Auch wird mit einer als Amid geschützten Aminfunktionalität die irreversible Zwischenkettenamidbildung, die aus der Dehydrierung resultiert, verhindert.
- Gemäß der Erfindung kann eine Vielzahl von Stentarten verwendet werden. Einige Beispiele sind in den
3 –6 gezeigt. In3 ist ein geflochtener selbst-expandierender Stent gezeigt, allgemein als40 bezeichnet. Aus der Figur ergibt sich, dass der Stent40 eine zylindrische Konfiguration aufweist. Der Stent kann in einer Flechtmaschine hergestellt werden, wobei die Monofilamente aus rostfreiem Stahl aus einer Vielzahl von Drähten bestehen, die jeweils eine Dicke von beispielsweise 0,08 mm haben.4 zeigt eine andere Stent-Konfiguration50 , die in dieser Erfindung verwendet werden kann. Andere Beispiele dieser Art von Stent sind im US-Patent 4,655,771, im US-Patent 4,732,152, im US-Patent 4,954,126 sowie im US-Patent 5,061,275 offenbart. - Bezugnehmend auf die
5 und6 wird ein Gelenkstent60 mit drei Stentsegmenten62 und zwei verbindenden Gelenkelementen64 gezeigt. Die Stentsegmente62 sind jeweils aus einzelnen Drahtelementen, die miteinander verschweißt sind, hergestellt. Die Gelenke64 können aus einem biokompatiblen Federmaterial gemacht sein und können aus einem kleineren Durchmesser als die, die zur Bildung der Stentsegmente62 verwendet werden, sein. Die Gelenke64 sind an jedem Ende der Stentsegmente62 unter Verwendung entweder von Laser- oder Widerstandsschweißtechniken angeschweißt. Die Gelenke64 sind vorzugsweise beide an der gleichen Seite der Stentsegmente62 befestigt. Der Stent60 , gezeigt in5 , kann in einer Arterie66 mit einer Hülle12 , wie in6 gezeigt, installiert werden und wie gezeigt gebogen werden. - Andere Stentkonfiguration und Materialien werden denen, die mit dieser Technik vertraut sind, offensichtlich erscheinen.
- Kollagenhüllen können so hergestellt werden, um beide Seiten des Stents, innen und außen zu bedecken, so dass dessen Oberflächen vollständig mit Kollagen umfasst sind.
- Ein Beispiel einer derartigen Kombination ist in
7 gezeigt, die einen rohrförmigen Stent umfasst, allgemein gezeigt bei10 , der mit einer inneren Hülle13 aus Kollagenmaterial und einer äußeren Hülle12 aus Kollagenmaterial kombiniert ist, was zum Zwecke der Gefäß-Implantierung eine Kombination aus Stent/Hülle zur Verfügung stellt, allgemein gezeigt bei15 . In einigen Fällen ist es bevorzugt, dass die Kollagenhülle13 an der inneren Oberfläche des Stents mittels geeigneter Mittel, wie z.B. eines Kollagengels, befestigt ist, welches als Klebstoff wirkt, insbesondere wenn der Stent vom variablen Durchmessertyp ist. Derartige Gele sind im Stand der Technik bekannt. - BEISPIEL
- Verfahren zur Herstellung des Hüllen-Stents der

- 1. Ein SIS-Blatt wird etwa 50% gedehnt, während man ihm erlaubt, an der Luft zu trocknen.
- 2. Das trockene SIS-Blatt wird auf einen aufgeblasenen Standard-Angioplastie-Ballon gewickelt und entlang der Naht angefeuchtet, um gute Haftung sicherzustellen.
- 3. Ein rohrförmiger Stent wird anschließend über dem SIS platziert.
- 4. Ein zweites Blatt von SIS wird über die Außenseite des Stents gewickelt. Dieses Blatt kann angefeuchtet werden, um die Handhabung zu erleichtern. Das SIS innerhalb des Stents kann ebenso unmittelbar vor dieser Wickelprozedur mit einer kleinen Menge destilliertem Wasser angefeuchtet werden.
- 5. Ein Offenzellschaumblatt wird anschließend auf die äußere zweite Schicht Kollagen aufgewickelt, gefolgt von einer Wicklung mit Dialyseschlauch. Dieser radiale Druck gewährleistet einen kontinuierlichen Kontakt und Haftung zwischen den Kollagenschichten.
- 6. Die gesamte Struktur wird anschließend kurzzeitig in Wasser eingetaucht, um das Kollagen zu befeuchten.
- 7. Die gesamte Kombination wird anschließend auf etwa 40°–70°C für etwa 0,5–3 Stunden erwärmt und anschließend auf Raumtemperatur abgekühlt. Der Zweck dieser Wärmebehandlung ist es, die Kollagenschichten miteinander zu verbinden. Gegebenenfalls kann dies auch durch Verwendung eines chemischen Vernetzungsmittels bewerkstelligt werden.
- 8. Die resultierende Vorrichtung wird von dem Ballon befreit, nachdem der Dialyseschlauch und der Schaum entfernt wurden. Jedes überschüssige Kollagenmaterial wird anschließend von den Enden des bedeckten Stents abgeschnitten.
- Eine gegossene oder geformte Version wird bei der Herstellung in
8 gezeigt, welche eine zylindrische Form80 umfasst, in die ein zylindrischer Stent82 am Ende platziert wird. Vorzugsweise ist die Form80 porös, z.B. eine poröse Keramik, um zu ermöglichen, dass Wasser durch die Form gezogen werden kann, um die Absetzung des eingegossenen Kollagens zu erleichtern. Eine Kollagengellösung83 wird anschließend in die Form80 um den Stent82 und in den Stent82 eingegossen und es wird ihr erlaubt, sich abzusetzen. Bei der Absetzung wird der in Kollagen eingebettete Stent aus der Form entfernt, und ein längliches Loch kann durch das Kollagen auf der Innenseite des Stents ausgebildet werden, um eine längliche Öffnung dadurch zu gewährleisten. Andererseits kann auch ein Dorn oder Formeinsatz für diesen Zweck ebenso gut verwendet werden. - Das Kollagenmaterial kann auf die Stentoberflächen wie erwünscht durch Aufsprühen oder Tauchbeschichtung oder eine elektrophoretische Technik oder dergleichen beschichtet werden. Die elektrophoretische Technik ist eine bevorzugte Beschichtungstechnik und kann beispielsweise in einer Lösung aus Essigsäure, Aceton, Wasser und Kollagen mit einem Metallstent als Kathode bei einer Spannung (potential) von etwa drei Volt durchgeführt werden. Dieses Verfahren weist eine gewisse Ähnlichkeit zu moderner Elektroplattierung auf, wo positiv geladene Metall-Ionen an der negativ geladenen Kathode zum entsprechenden Metall reduziert werden. Im Fall des Kollagen wird das Biomolekül in einer sauren Lösung gelöst oder suspendiert. Die Säure verleiht dem Protein oder Kollagen eine positive Ladung und ermöglicht es ihm, sich in einem elektrischen Feld zu bewegen. Durch Befestigen eines metallischen Objekts an der negativen Elektrode einer Stromquelle und anschließendes Eintauchen sowohl der positiven und negativen Elektroden in der sauren Kollagenlösung bildet sich eine Kollagenschicht auf der negativ geladenen Oberfläche. Das Ergebnis ist ein beschichteter Stent wie der in den
19 und20 , der vorzugsweise Öffnungen in der Beschichtung aufweist, die mit den Öffnungen im Stent zusammenfallen. - BEISPIEL
- Kollagenbeschichteter Stent (Typ IV) via Elektroabscheidung

- A. Eine Lösung von Sigma Typ IV Humankollagen (50 mg) wurde in einem Polypropylenrohr mit 3 ml Wasser, 1 ml Essigsäure und 2 ml Aceton platziert. Diese Mischung wurde zu einer viskosen Lösung mittels ca. 3 minütiger Mischung mit hoher Scherkraft homogenisiert. Die Lösung wurde mit Wasser verdünnt, anschließend durch einen Baumwoll-pfropfen gefiltert. Um Luftbasen zu eliminieren, wurde die Lösung eine Stunde stehen gelassen.
- B. Ein zylindrisches Gefäß wurde aus Polypropylen hergestellt und mit 1 ml der oben hergestellten Lösung A beladen. In diesen Behälter wurde ein Nitinolsubstrat, das an der negativen Leitung einer Stromquelle mit variabler Spannung, die auf 3 Volt eingestellt war, angeschlossen war, hineingegeben. Die positive Elektrode wurde mit einem Draht mit 0,010 Inch Durchmesser ausgestattet, der ca. 4 mm von dem Substrat entfernt platziert wurde. Die Stromquelle wurde eingeschaltet und sofort stellte sich eine Gasentwicklung auf den Oberflächen jeder Elektrode ein. Dieser Zustand wurde mehrere Minuten aufrecht erhalten, anschließend wurden die Elektroden aus der Kollagenlösung entfernt. Eine gleichförmige, gelatineartige Masse lag auf dem Substrat vor, welche etliche Bläschen enthielt. Nach ein- bis zweiminütigem Stehen waren die Blasen verschwunden und die Elektroden wurden wiederum in dem Bad platziert. Nach drei weiteren Minuten der Behandlung wurde das Substrat aus dem Bad entfernt und getrocknet. Bei visueller Prüfung erwies sich die Beschichtung als kontinuierlich.
- Eine weitere Beschichtungstechnik wird im US-Patent 5,275,826 gezeigt.
- Bezugnehmend auf
9 , trägt ein rohrförmiger metallischer Stent, allgemein mit10 bezeichnet, in sich einen zylindrischen Einsatz oder eine innere Hülle aus Kollagenmaterial, allgemein bezeichnet mit12 , um eine Kombination von Stent/Liner zu gewährleisten, die allgemein mit14 bezeichnet ist, zum Zwecke der vaskulären Implantierung. - Der Stent
10 kann, wie bereits hier oben beschrieben, von jeder beliebigen Art sein, und ist typischerweise aus einem Metall, z.B. rostfreier Stahl, Nitinol, superelastische Legierungen und andere Metalle, oder aus einem geeigneten Polymer oder aus jedem anderen geeigneten Material, und kann einen Fixdurchmesser oder einen variablen Durchmesser aufweisen, wobei letzteres mehr bevorzugt ist und im Stand der Technik gut bekannt ist. - Der Kollagenliner
12 , wie vorab beschrieben, kann aus Kollagen per se sein oder direkt auf dem Stent aufgetragen sein oder kann als12c auf einem Träger12d wie in10 gezeigt zur Aufbringung auf einem Stent getragen sein. - Wenn der Kollagenliner aus zwei verschiedenen Materialien aufgebaut ist, die wie in
10 gezeigt miteinander verbunden sind, wird dies als eine Doppelschichtstruktur bezeichnet. Bei der Platzierung im Stent wird die Schicht12c luminal platziert, mit der Schicht12d in Kontakt mit der inneren Oberfläche des Stent. Die Schicht12d , die durch die Öffnungen in dem Stent in einem derartigen Arrangement mit der Gefäßwand in Kontakt sein kann, ist vorzugsweise fest und ermöglicht der inneren luminalen Schicht12c selbst die strukturelle Integrität aufzuweisen, die notwendig ist, um eine einfach Bestückung, Einführung und Aussetzung sicherzustellen. Die Schicht12c kann beispielsweise aus einem kollagenartigen Material mit einer Dicke im Bereich von 5 bis 200 Mikron sein. Ein derartiges biologisch abgeleitetes Material kann aus einer Spender-Quelle gewonnen werden, von unerwünschten Geweben gereinigt werden und dadurch zu einem Röhrchen geformt werden, dass man es um einen Dorn herumwickelt und das Material mit sich selbst verklebt. Synthetische Materialien können zur Trägerung der Schicht12d des Liners verwendet werden, jedoch können auch vaskuläre Transplantatmaterialien wie PTFE, gewebtes Dacron, Polyurethan und dergleichen verwendet werden. Resorbierbare Polymere (PLLA, PGA, PDLLA, PHB, Polyanhydride) sind eine andere Wahlmöglichkeit für die Trägerschicht12d des Liners12 . Diese Materialien können durch Extrusion, Lösungsmittelguss, Spritzguss, etc. oder durch Spinnen zu Fasern und Weben in eine rohrförmige Struktur zu einem Rohr geformt werden. Ein Rohr aus einem der vorgenannten Polymere kann auch mittels einer Fliesfasertechnik konstruiert werden. Die innerste oder luminale Seite, d.h. die Schicht12c des Liners, dient einer anderen Funktion als die Trägerschicht12d . Die luminale Oberfläche oder Schicht12c muss ein Substrat für das Wachstum der Endothelialzellen sein, wie auch ein Reservoir für therapeutische Wirkstoffe. Das bevorzugte Material ist fibulares Typ I Kollagen und/oder Schweine-Typ IV Kollagen mit einer Dicke im Bereich von 5 bis 200 Mikron, wobei jedoch auch Fibrin für diesen Zweck verwendet werden kann. Hoch hydrierte Materialien wie vernetzte Hydrogele erfüllen das Wirkstoffverankerungserfordernis des luminalen Teils des Liners, und Beispiele hierfür sind Polyether, Polyalkohole, Polypyrrolidone, Polypeptide, Polysäuren und dergleichen. Die Schicht12 kann auch eine Mischung der obigen Materialien mit einem ionisch oder kovalent Wirkstoff-bindenden Molekül sein. Ein derartiges Molekül kann Protamin sein, das Heparin in wirksamer Weise ionisch bindet. Diese Polymere können auch mit Wachstumsfaktoren behandelt sein, wie z.B. RGD-Peptide, um die Endothelialisierung zu fördern. Das bevorzugte Verfahren zur Wirkstoffinkorporierung umfasst die Herstellung einer Lösung des therapeutischen Wirkstoffes und das Aufquellen der dehydrierten luminalen Seite der Hülle in der Lösung. Nach Verdampfen des Trägerlösungsmittels sitzt der Wirkstoff in der Matrix fest, welche die innere Schicht des Liners, d.h. die Schicht12c umfasst. Die Vorrichtung kann als Schwamm fungieren, um einen Wirkstoff in Lösung aufzusaugen und ihn bei Implantierung aus dem Stent zu eluieren. - Der Begriff "Kollagen" oder "Kollagenmaterial" sollte auch so verstanden werden, dass er Material einschließt, welches als „Dünndarm-Schleimhautgewebe" („Small Intestine Submucosa", SIS) bezeichnet wird, das in der vorliegenden Erfindung eine besondere Verwendung allein und in Kombination mit anderem Kollagenmaterial wie Typ I hat. SIS besteht aus einer Doppelschichtstruktur, in der eine Schicht vorherrschend (stratum compactum) Typ IV ist, und die zweite Schicht eine Mischung aus Typ I (muscularis mucosa) und Typ III Material ist. Dies ist detailliert beschrieben in den US-Patenten 4,902,508; 4,956,178 und 5,281,422. Die luminale Seite des SIS besteht hauptsächlich aus einem Typ IV Kollagenmaterial.
- Wie auch anderes Kollagenmaterial, kann das SIS hier mit oder ohne einer Trägerschicht wie die Schicht
12d aus10 verwendet werden. Es kann auch als Trägerschicht12c in Kombination mit einer Schicht12d aus Typ I Kollagen verwendet werden, wie in10 gezeigt. SIS funktioniert auch ohne Trägerschicht gut, da es selbst eine Multischichtstruktur aufweist. - Eine SIS-Schicht kann mit einer Typ I Schicht kombiniert werden, um eine Einbahndurchflusstruktur mit einer Reservoiranordnung, wie schematisch in
11 gezeigt, zur Verfügung zu stellen. In diesem Arrangement trägt ein zylindrischer Drahtnetz-Stent10 einen rohrförmigen Doppelschichtliner, allgemein gezeigt bei12 , bestehend aus zwei Schichten12c und12d . Die Schicht12c berührt die innere Oberfläche des Stent10 und ist ein Typ I Kollagenmaterial, und kann als Reservoir wirkend einen Wirkstoff oder dergleichen tragen. Die luminale Schicht12d ist SIS Material und wirkt inhärent so, dass es einen Durchfluss des Wirkstoffes aus der Schicht12c in das luminale Innere des Stent durch die luminale Schicht12d des Stent10 ermöglicht, dabei jedoch keinen nennenswerten Fluidfluss in die Schicht12c aus dem Inneren des Stent ermöglicht. - Variationen dieser Anordnung aus
11 sind in den12 und13 gezeigt. In der12 trägt der Stent10 einen inneren oder luminalen Liner aus Schichten12c und12d wie bei11 beschrieben, sowie eine äußere Schicht aus der selben Kombination, um hauptsächlich einen Einwegdurchfluss von Wirkstoff in das luminale Innere des Stent und in Richtung der Oberfläche, gegen die sich der Stent abstützt, wenn er implantiert ist, zu ermöglichen. - In
13 trägt der Stent10 eine äußere Schicht12 aus ungeträgertem SIS Material und einen inneren Liner bestehend aus Schichten12c und12d wie in den11 und12 . Die Schicht12c ist ein Typ I Material, das als Wirkstoffreservoir fungiert, wie vorher bei den11 und12 gezeigt. Die Schicht12d ist aus SIS und wirkt als Einwegdurchgang für Wirkstoffe und dergleichen aus der Schicht12c , wie vorher. - Wie bereits vorab herausgestellt, synthetisieren die Zellen der Blutgefäßwand verschiedene Arten von Makromolekülen, die eine extrazelluläre Matrix bilden, und sondern diese ab. Jedes dieser Makromoleküle kann in einer Vielzahl von Kombinationen zur Bildung von Kompositen kombiniert werden. Derartige Materialien werden hier ganz allgemein, wie bereits ausgeführt, als "Kollagen" bezeichnet.
- Ein primäres Ergebnis der Verwendung eines Kollagenliners aus natürlichen Bestandteilen ist, dass neues zellulares Wachstum des Endothelium auf einer natürlichen Oberfläche stattfindet, die im wesentlichen unbeschädigt und gleichförmig ist. In dieser Hinsicht gewährleistet der Liner einen biologischen Schutz.
- Der Liner kann fest auf dem Stent angepasst sein, auf die selbe Weise wie oben beschrieben, mit Bezug auf das Arrangement mit äußerer Hülle.
- Der Liner kann an dem Stent auf eine Vielzahl von Weisen befestigt sein. Das grundlegende Ziel der Befestigung in der bevorzugten Form ist es, eine Stenteinheit bereitzustellen, bei der das tragende Stentgerüst im wesentlichen, sofern nicht sogar vollständig, durch den Liner vom Blutfluss isoliert wird. Dies kann dadurch erreicht werden, dass der Liner
12 auf der inneren Ausdehnung des Stent10 platziert wird und die Enden des Liners über die Enden des Stent, wie bei16 in14 gezeigt, umgestülpt werden. Dies ist eine besonders bevorzugte Anordnung. Die Stulpen16 können entweder unter oder über der äußeren Hülle liegen. Die Stulpen16 können auf den Stent genäht werden, von einer Stulpe zur anderen genäht werden, oder auf andere Weise mit dem Stent oder dem Liner selbst verbunden werden. Das Kollagenmaterial kann durch Anwendung lokalisierter Hitze und Druck geschweißt werden, oder durch die Anwendung einer konzentrierten Lösung aus Kollagenmaterial, die wie ein Klebstoff wirkt. - Der Liner kann auch durch die Verwendung von Pledgets (nicht gezeigt) auf dem Stent befestigt werden. Das Pledget (oder kleines Fetzchen von Material) kann auf der Außenseite des Stent platziert werden. Der Liner, der auf der luminalen oder inneren Oberfläche des Stent sitzt, kann auf eine Vielzahl von Weisen mit dem Pledget verbunden werden. Darunter sind Nähen, Kleben und Hitzeschweißen. Im Fall einer Kombination aus Liner mit äußerer Hülle können diese Befestigungsmittel genauso gut verwendet werden.
- Der Liner kann in dem Stent nach verschiedenen Verfahren platziert werden. Der Stent kann porös oder perforiert gemacht werden, wodurch dem Linermaterial ermöglicht wird, als Formdorn für eine Kollagenhülle zu wirken. Das Kollagen kann auch auf dem Stent abgeschieden werden. Dieses Verfahren erfordert, dass der Stent in einer Lösung aus Kollagen erhitzt wird. Das Kollagen bildet eine Matrix auf der Oberfläche des Stent, und sobald gut befestigt, bildet das Kollagen eine faserartige, gut organisierte Struktur aus, die der Befestigung und dem Wachstum von Zellen förderlich ist.
- Eine Technik ist in
15 gezeigt, bei der das Kollagen12 auf eine Weise, die ähnlich zu der in Zusammenhang mit8 beschriebenen ist, in den Stent10 gegossen wird. Nach dem Absetzen wird der Stent mit dem Liner aus gegossenem Kollagen aus der Form entfernt, und es kann ein Loch durch das Kollagen ausgebildet werden, um eine längliche Öffnung dort hindurch bereitzustellen. Alternativ kann ein Dorn oder ein Formeinsatz (nicht gezeigt) ebenso für diesen Zweck verwendet werden. Löcher können auch durch die Stentwände in dem Kollagen gebildet werden, wenn der Stent perforiert wird oder dergleichen. - Der Liner kann an dem Stent mittels jedem von vielen Designmerkmalen befestigt sein, die bei dem Stent eingebaut sind. Durch Bereitstellung eines Stent mit Haken oder einer ähnlichen anderen Topographie (nicht gezeigt) kann die Hülle sofort an dem Stent befestigt werden. Das Hüllenmaterial kann an solchen Spitzen aufgespießt sein, wodurch die Hülle befestigt wird. Mit Haken der entsprechenden Größe wird das Kollagenmaterial nicht perforiert, sondern eher in der haltenden Topographie eingebettet.
- Wie aus dem Obengenannten entnommen werden kann, stellt die Erfindung in einer Ausführungsform einen Stent zu Verfügung, bei dem Kollagen-Linermaterial innerhalb und/oder auf der Außenseite des Stent platziert wird, wodurch die Thrombusbildung verringert wird und das Gefäß therapeutisch behandelt wird, wenn ein geeigneter Wirkstoff in dem Kollagen enthalten ist.
- Es sollte klar sein, dass das hierin als in Schichtform genannte Kollagenmaterial in der Form von Blättern vorliegen kann, die mit dem Stent verknüpft sind oder darauf abgeschieden sind, d.h. als Beschichtung.
- Daher kann das Kollagenmaterial auf die Stent-Oberflächen wie erwünscht durch Aufsprühen oder Tauchbeschichtung oder Elektroabscheidung oder dergleichen geschichtet werden, oder auf andere Weise befestigt, wie oben beschrieben. Eine derartige Beschichtung kann etwa 1–50 Mikron dick sein. Ein beschichteter Stent ist in
19 gezeigt, der vorzugsweise wie gezeigt perforiert ist. - Ein Kollagen-beschichteter Stent, der nach dem erfindungsgemäßen Verfahren hergestellt wurde, weist auch eine Kollagenhülle über der Kollagenbeschichtung oder unter der Kollagenbeschichtung auf. Beispielsweise kann man einen Stent in einer Kollagenhülle, wie in
1 gezeigt, mit einer Überlagerungspassung anordnen. Die Innenseite des Stent kann anschließend mit Kollagen beschichtet werden, so dass der Stent und die Innenseite der Hülle bedeckt und miteinander verbunden werden. Vorzugsweise ist bei einer derartigen Anordnung die Hülle aus SIS und die Beschichtung aus Typ I oder Typ IV. Es ist auch möglich, im Fall eines offen gearbeiteten Stent, wie dem in den1 oder3 gezeigten, die Stentstreben mit Kollagen zu beschichten, eine Kollagenhülle entweder über oder auf die Innenseite des Stent anzuordnen, oder beides, und anschließend die Hülle und/oder den Liner mit der Beschichtung wärmezuverkleben. Dies wird vorzugsweise mit Typ IV Kollagen gemacht, insbesondere mit SIS oder mit Fibrin. - In einigen Anwendungen kann es wünschenswert sein, Perforierungen im Kollagen vorzusehen, damit Fluidbewegung durch die Stent/Kollagenwand erfolgen kann. Eine derartige Anordnung ist leicht zu erhalten, da Stents im allgemeinen bezüglich ihrer Struktur offen oder perforiert sind, und Perforierungen in einem Kollagenliner leicht gebildet werden können, wobei sich die Perforierungen durch die Stentöffnungen erstrecken. Die Perforierung in Kollagenlinern mit etwa 10 bis 60 Mikron im Durchmesser hat sich als zufriedenstellend erwiesen. Die Verteilung der Perforierungen kann so gestaltet sein, dass sie gleichmäßig beabstandet sind, z.B. mit 30 bis 60 Mikron Abständen und zur Besetzung von etwa der Hälfte der Lineroberflächen. Dies kann jedoch selbstverständlich variieren.
- Schließlich gibt es eine bevorzugte Orientierung für die Anordnung von Kollagen auf dem Stent, wenn das Kollagen in Form eines Blattes verwendet wird, das um den Stent herumgewickelt wird oder als Rohr in den Stent eingesetzt wird. Es wurde herausgefunden, dass blatt förmiges Kollagen die Fähigkeit zur Ausdehnung besitzt, dass diese Dehnbarkeit jedoch hauptsächlich unidirektional ist. Das heißt, der Hauptanteil der Ausdehnung in einem Blatt verläuft in einer primären Richtung. Dies ist schematisch durch die parallelen Pfeile
100 in16 bei einem Kollagenblatt110 gezeigt, welches üblicherweise wenig oder keine Ausdehnung in einer Richtung zeigt, die rechtwinkelig zu den Pfeilen liegt. - Es wurde gefunden, dass das Kollagenblatt, wenn es als Hülle oder als Liner auf einem Stent verwendet wird, der einer Expansion und/oder Kontraktion unterliegt, sich besser daran anpasst, wenn das Kollagenblatt mit dem Stent "schräg" verknüpft ist. Dies wird vollständiger erklärt mit Bezug auf die
17 und18 . Wenn ein Stück Kollagenblatt110a aus dem Blatt110 mit den Orientierungen wie in17 gezeigt entnommen wird, kann gesehen werden, dass die Dehnungsrichtung, die durch die Pfeile100 angedeutet ist, schräg bezüglich dem Blatt110a liegt. In diesem Fall beträgt die Schräge 45° relativ zu den Kanten des Blattes110a . Wenn das Blatt110a senkrecht bezüglich eines Stent120 , wie in18 gezeigt, orientiert ist, das heißt die Kanten des Blattes110a sind senkrecht zu der Längsausdehnung des Stent120 , und wenn dann das Blatt110a um den Stent120 zur Ausbildung einer Hülle oder zu einem Röhrchen zum Einsetzen in den Stent120 als Liner herumgewickelt oder aufgerollt wird (in der bei112 in18 gezeigten Richtung), dann liegen die Dehnungseigenschaften des Blattes110a schräg bezüglich der Längsausdehnung des Stent120 . In diesem Fall beträgt die Schräge 45°, was eine bevorzugte Schrägrichtung ist. Andere Schrägegrade sind ebenso akzeptabel, aber 45° ist bevorzugt. - Aus dem Vorgenannten kann entnommen werden, dass aufgrund der variablen Dimensionen sowohl im Durchmesser als auch der Länge, die bei Stents auftreten, eine derartige Anordnung die Kollagenhüllen und Liner besser an dimensionale Veränderungen in beiden Richtungen anpasst, ohne dass Nähte aufreißen oder Risse in dem Material auftreten.
- Eine bevorzugte Stentart zur Verwendung in der vorliegenden Erfindung ist einer des Typs, der in der verwandten Anmeldung WO-A-95/31945 offenbart ist.
- Die
19 und20 zeigen einen beschichteten Stent, allgemein angedeutet bei140 , wobei die Kollagenbeschichtung142 am besten in20 zu sehen ist. Die Beschichtung142 ist sowohl auf der Innenseite, als auch auf den äußeren Oberflächen des Stent gezeigt, obwohl sie genau so gut auf nur einer dieser Oberflächen vorliegen kann. - Es kann aus dem Obigen gesehen werden, dass die Erfindung nützlich für ein Behandlungsverfahren zur Implantierung eines Stents ist, bei dem Kollagenmaterial zwischen dem Stent und der Gefäßwand platziert wird, wodurch die Thrombusbildung reduziert wird und das Gefäß therapeutisch behandelt wird, wenn in dem Kollagen ein geeigneter Wirkstoff eingeschlossen ist.
Claims (17)
- Ein Verfahren, eine Kollagenbeschichtung auf einen expandierbaren Stent aufzutragen, wobei der expandierbare Stent eine Metalloberfläche aufweist, umfassend die Schritte: Beschichten der Metalloberfläche mit Kollagen durch elektrolytische Abscheidung und Platzieren einer Hülle von Kollagenmaterial um den Stent oder innerhalb des Stents.
- Das Verfahren aus Anspruch 1, wobei die Metalloberfläche als eine Kathode in einem Anoden-Kathoden-Paar funktioniert, und der Stent in einer wässrigen Elektrolytlösung enthaltend Kollagen eingetaucht ist, und ein elektrisches Potential zwischen der Anode und der Kathode aufgebaut wird, das ausreichend ist, um elektrische Abscheidung des Kollagens aus der Lösung auf die Metalloberfläche zu erhalten.
- Das Verfahren aus Anspruch 2, wobei das Potential ungefähr 3 Volt beträgt.
- Das Verfahren aus Ansprüchen 2 oder 3, wobei die Lösung aus Essigsäure und Wasser besteht.
- Das Verfahren aus einem der vorhergehenden Ansprüche, wobei die Hülle um den Stent platziert wird.
- Das Verfahren aus einem der Ansprüche 1 bis 4, wobei die Hülle innerhalb des Stents platziert wird.
- Das Verfahren aus einem der Ansprüche 1 bis 6, wobei der Stent ein selbst-expandierbarer Stent ist.
- Das Verfahren aus einem der Ansprüche 1 bis 6, wobei der Stent ein Ballon-expandierbarer Stent ist.
- Das Verfahren aus einem der vorhergehenden Ansprüche, wobei die Hülle aus Kollagenmaterial einen therapeutischen Wirkstoff umfasst.
- Das Verfahren aus Anspruch 9, wobei der therapeutische Wirkstoff Heparin umfasst.
- Das Verfahren aus einem der vorhergehenden Ansprüche, wobei die Hülle perforiert ist.
- Das Verfahren aus Anspruch 11, wobei die Perforationen etwa 10–60 Mikron im Durchmesser aufweisen.
- Das Verfahren aus einem der vorhergehenden Ansprüche, wobei die Hülle wenigstens zwei Typen von Kollagen umfasst.
- Das Verfahren aus Anspruch 13, wobei die Hülle in der Form einer Doppelschicht ist.
- Der Stent aus Anspruch 13 oder 14, wobei die Hülle Typ I- und Typ IV-Schichten umfasst.
- Das Verfahren aus einem der vorhergehenden Ansprüche, wobei das Metall Nitinol ist.
- Das Verfahren aus einem der vorhergehenden Ansprüche, wobei die Beschichtung ungefähr 1–50 Mikron dick ist.
