Description
- Diese Erfindung betrifft im allgemeinen eine Vorrichtung für die Behandlung und die Ablation von Körpermassen, wie Tumoren, und genauer gesagt eine Vorrichtung mit vielen einziehbaren Nadelelektroden, die ein Äußeres eines Tumors mit einer Mehrzahl von Nadelelektroden umgibt und ein Ablationsvolumen definiert.
- Derzeitige äußere Verfahren zur Behandlung von Tumoren sind äußerst disruptiv und verursachen einen großen Schaden an gesundem Gewebe. Während des chirurgischen Verfahrens muß der Art Sorge tragen, daß der Tumor nicht in einer Weise geschnitten wird, die ein Streuen bzw. Ausstreuen des Tumors erzeugt, welches zu Metastasenbildung führt. In den letzten Jahren war die Entwicklung von Produkten betont auf die Minimierung der traumatischen Eigenschaft der traditionellen chirurgischen Verfahren gerichtet.
- Es gab eine relativ große Menge an Aktivität auf dem Gebiet der Hyperthermie als ein Hilfsmittel zur Behandlung von Tumoren. Es ist bekannt, daß Erhöhung der Temperatur von Tumoren für die Behandlung und Steuerung von karzinomatösem Gewebe hilfreich ist. Die Mechanismen der selektiven Ausmerzung der Krebszellen durch Hyperthermie sind nicht völlig verstanden. Es wurden jedoch vier Zelleffekte der Hyperthermie auf karzinomatöses Gewebe vorgeschlagen, (i) Veränderungen in der Zell- oder Nuklearmembran-Permeabilität oder -Fluidität, (ii) zytoplasmische lysomale Zersetzung, welche die Freisetzung von verdauungsfördernden Enzymen verursacht, (iii) thermische Beschädigung der Proteine, die die Zellatmung und die Synthese von DNA oder RNA beeinflußt und (iv) potentielle Anregung des Immunsystems. Behandlungsverfahren zum Zuführen von Wärme zu Tumoren beinhalten die Verwendung von Direktkontakthochfrequenz-(RF)Applikatoren, Mikrowellenstrahlung, induktiv gekoppelte Hochfrequenzfelder, Ultraschall und eine Vielzahl von einfachen thermischen Leitungstechniken.
- Unter den Problemen, die mit all diesen Verfahren verbunden sind, ist die Anforderung, daß stark lokalisierte Wärme in Tiefen von einigen Zentimetern unterhalb der Körperoberfläche erzeugt werden muß. Bestimmte Techniken wurden mit Mikrowellenstrahlung und Ultraschall entwickelt, um Energie in verschiedenen gewünschten Tiefen zu bündeln. Hochfrequenzapplikationen können während einer Operation in der Tiefe angewandt werden. Der Umfang der Lokalisation ist jedoch im allgemeinen mangelhaft, mit der Folge, daß gesundes Gewebe geschädigt werden kann.
- Induktionswärme erreicht ebenso eine schlechte Lokalisation der einfallenden Energie. Obwohl induktives Heizen durch Plazieren einer Antenne auf der Oberfläche des Körpers erreicht werden kann, werden oberflächliche Wirbelströme in der unmittelbaren Umgebung der Antenne erzeugt.
- Wenn diese durch Hochfrequenzstrom angetrieben wird, tritt unerwünschtes Oberflächenheizen auf, welches das Heizen des darunterliegenden Gewebes schmälert.
- Nicht eindringende Verfahren für das Zuführen von Wärme zu internen Tumoren hatten daher Schwierigkeiten, eine substantiierte spezifische und selektive Behandlung zu erreichen.
- Hyperthermie, die von einer Hochfrequenz- oder Mikrowellenquelle erzeugt werden kann, appliziert Wärme auf Gewebe, aber übersteigt nicht 45°C, so daß gesunde Zellen überleben. In der Thermotherapie werden Wärmeenergien von > 45°C aufgebracht, was zu einem histologischen Schaden, Austrocknung und Denaturierung der Proteine führt. Hyperthermie wurde kürzlich zur Therapie von bösartigen Tumoren angewandt. In der Hyperthermie ist es wünschenswert, einen Zustand von Hyperthermie zu induzieren, der durch interstitielles Stromheizen auf einen bestimmten Bereich lokalisiert ist, während gleichzeitig ein minimaler thermischer Schaden an dem umgebenden gesunden Gewebe gewährleistet wird. Oftmals ist der Tumor subkutan lokalisiert, und das Angehen des Tumors erfordert entweder Operation, endoskopische Verfahren oder externe Strahlung. Es ist schwierig, Hyperthermie extern in tiefe Körpergewebe zu induzieren, da die Stromdichte aufgrund ihrer Absorption durch gesundes Gewebe abgeschwächt wird. Zusätzlich wird ein Teil der Hochfrequenzenergie an den Muskeln/Fett und Knochengrenzflächen reflektiert, was zu dem Problem des Aufbringens einer bekannten Energiemenge direkt auf einen kleinen Tumor hinzukommt.
- Versuche, interstitielle lokale Hyperthermie zu verwenden, haben sich als nicht sehr erfolgreich erwiesen. Im Ergebnis sind oftmals eine ungleichmäßige Temperaturverteilung innerhalb des Tumors erzeugt worden. Man nimmt an, daß eine Reduktion der Tumormasse durch Hyperthermie mit der thermischen Dosis in Zusammenhang steht. Die thermische Dosis ist die minimale effektive Temperatur, die innerhalb der Tumormasse für eine bestimmte Zeitdauer zugeführt wird. Da Blutfluß der Hauptmechanismus des Wärmeabbaus für aufgeheizte Tumore ist, und der Blutfluß innerhalb des Tumors variiert, wird ein gleichmäßigeres Aufheizen des Tumorgewebes benötigt, um eine effektivere Behandlung zu gewährleisten.
- Dasselbe trifft für die Ablation des Tumors an sich durch die Verwendung von Hochfrequenzenergie zu. Verschiedene Methoden wurden für die Hochfrequenzablation von Massen wie Tumoren angewandt. Anstelle des Heizens des Tumors wird er durch die Zuführung von Energie abgetragen. Dieser Prozeß konnte schwer erreicht werden aufgrund einer Vielzahl von Faktoren, die (i) das Positionieren der Hochfrequenzablationselektrode zum effektiven Entfernen der gesamten Masse, (ii) die Einführung der Hochfrequenzablationselektroden zu dem Ort des Tumors und (iii) die kontrollierte Zuführung und Überwachung der Hochfrequenzenergie, um eine erfolgreiche Abtragung ohne Beschädigung des nicht-tumorartigen Gewebes zu erreichen, beinhaltet.
- Es gibt eine Anzahl von unterschiedlichen Behandlungsmethoden und Vorrichtungen für die Behandlung von Tumoren mit minimaler Eindringung. Ein solches Beispiel ist ein Endoskop, wie es in dem US-Patent Nr. 4,920,978 offenbart ist, das Hochfrequenzhyperthermie in Tumoren erzeugt. Eine Endoskopvorrichtung für Mikrowellen wird in US-Patent Nr. 4,409,993 beschrieben. In dem US-Patent Nr. 4,920,978 ist ein Endoskop für Hochfrequenzhyperthermie offenbart.
- In dem US-Patent Nr. 4,763,671 verwendet ein minimal invasives Verfahren zwei Katheter, die interstitiell in den Tumor eingeführt werden. Die Katheter werden innerhalb des Tumorvolumens plaziert, und jeder ist mit einer Hochfrequenzleistungsquelle verbunden.
- In dem US-Patent Nr. 4,565,200 wird ein Elektrodensystem beschrieben, in dem eine einzelne Kanüle im Eingangsbereich verwendet wird, um eine Elektrode in einen ausgewählten Ort des Körpers einzuführen.
- Die Vorrichtung mit einer medizinischen Sonde, die in der WO-A-94-04220 beschrieben ist, weist einen Katheter für die Einführung in eine Öffnung in einem Körper auf und hat ein Stilettführungsgehäuse mit einem oder mehreren Stilettanschlüssen in einer Seitenwand hiervon und eine Stilettführung, um eine flexibles Stilett durch einen Stilettanschluß und durch dazwischen liegendes Gewebe mit einem vorbestimmten einstellbaren Winkel nach außen zu einem Zielgewebe zu lenken. Der Katheteraufbau beinhaltet ein Stilettführungslumen, das mit dem Stilettanschluß und einem Stilett, das in dem Stilettführungslumen für eine longitudinale Bewegung von dem Anschluß durch ein dazwischen liegendes Gewebe zu einem Zielgewebe positioniert ist, verbunden ist. Das Stilett kann ein elektrischer Leiter, der mit einer nicht leitenden Schicht umgeben ist, sein, wobei der elektrische Leiter eine Radiofrequenzelektrode ist.
- Das deutsche Gebrauchsmuster DE-U-8909492 beschreibt eine interokulare Phakofragmentationsvorrichtung, die ein Paar von Greifarmen besitzt, die über eine Cornea eingeführt werden kann. Durch die Wirkung des Hochfrequenzstromes über die Greifarme, wird Denaturierung der Proteine in dem Linsenkern erreicht. Durch die Greifarme ist es möglich, daß der Kern der Linse, der zu hart für die Ultraschallfragmentierung ist, während der nach hinten gerichteten Bewegung dieser Arme, zerdrückt wird. Die Greifarme erstrecken sich von einer Führungshülle bzw. Muffe aus, die benachbart zu der äußeren Fläche der Cornea gehalten wird.
- Laut Seite 1 der deutschen Offenlegungsschrift DE-A-2,124,684 beschreibt diese Schrift eine Einführelektrode für biomedizinische Verwendungen, die größeren Objekten wie Elektroden oder Massageköpfen ermöglicht, in Körpergewebe oder Körperhöhlen eingeführt zu werden. Laut Seite 3 können die Elektroden beispielsweise aus Drähten, leitfähigen Gasen und leitfähigen Flüssigkeiten gebildet werden. Gemäß Seite 4 können die Drähte als Röhren ausgebildet sein, so daß die Flüssigkeiten und Gase hindurchtreten können. Die auf Seite 3 erwähnten Elektroden werden nur ver wendet als Antenne, zum Heizen für Kurzwellentherapie und zur Kauterisation, sowie als Operationsinstrumente für die Iontophorese oder die Kaskadeniontophorese, für die Zerstörung von Tumoren und schließlich für mechanische Vibratoren zum Lösen des Gewebes und die Zerstörung von Tumoren.
- Um eine effektive Behandlungsvorrichtung für einen Tumor zu sein, müssen die Elektroden relativ zu dem Tumor richtig positioniert sein. Nachdem die Elektroden positioniert sind, ist es dann wünschenswert, eine kontrollierte Zuführung und Ablagerung von Hochfrequenzenergie zum Abtragen des Tumors zu haben. Dies reduziert die Zerstörung von gesundem Gewebe.
- Es ist folglich ein Ziel der Erfindung, eine Vorrichtung zur Gewebeablation mit Hochfrequenz zur Verfügung zu stellen, die einen gewünschten Gewebeort, wie z. B. einen Tumor, in einer minimal invasiven Art und Weise abträgt bzw. entfernt.
- Gemäß der Erfindung wird eine Vorrichtung zur Gewebeablation mit Hochfrequenz bzw. Radiofrequenz zur Verfügung gestellt, die aufweist: einen Zuführungskatheter, der in das Körpergewebe eingefügt werden kann, mit einem distalen Ende und einem proximalen Ende; Einrichtungen zum Halten des Katheters am proximalen Ende; und Elektrodeneinsatzeinrichtungen mit einem Elektrodenvorrück- und Zurückziehglied, die zumindest teilweise in dem Katheter positioniert sind, und eine Mehrzahl von einziehbaren elektromagnetisch leitenden Elektroden, die sich jeweils von dem distalen Ende des Zuführungskatheters vorrücken lassen, die sich jeweils in das Gewebe hineinstecken lassen, die ein durchbohrendes distales Ende haben, so daß sie das Gewebe durchdringen und einen vorgespannten nicht eingesetzten Zustand haben, wenn sie innerhalb des Katheters positioniert sind, und einen entfalteten eingesetzten Zustand haben, wenn sie von dem distalen Ende des Katheters vorgerückt sind, wobei die einziehbaren Elektroden so betreibbar sind, daß sie sich entfalten, wenn sie von dem distalen Ende des Katheters hervortreten, wenn sie hiervon vorgerückt werden, um sich in einer seitlichen Richtung weg von der Peripherie des distalen Endes des Zuführungskatheters einzusetzen, so daß sie ein dreidimensionales Ablationsvolumen in einer eine ausgewählte Masse in dem Gewebe umgebenden Beziehung zwischen den eingesetzten Elektroden definieren, die Elektroden an eine steuerbare Hochfrequenzenergiequelle zur kontrollierten Applikation der Hochfrequenzenergie in das dreidimensionale Ablationsvolumen angeschlossen werden können, jede der Mehrzahl von Elektroden, die vorgespannt sind, so daß in einem eingesetzten Zustand: die Elektrode entlang ihrer Länge die Richtung ändert, so daß sie außerhalb des Katheters mindestens zwei Krümmungsradien besitzt, wobei diese mindestens zwei Krümmungsradien vorgespannt sind, und wobei einer der Krümmungsradien veranlaßt, daß sich die Elektrode seitlich von dem Katheter weg einsetzt, wenn sie von dem Katheter gelöst wird, wobei eine Mehrzahl der eingesetzten Elektroden sich dadurch seitlich in Bezug auf den Katheter kollektiv aufweiten, um das dreidimensionale Ablationsvolumen zu definieren.
- Eine Ausführungsform der Erfindung kann folglich eine Vorrichtung zur Gewebeablation mit Hochfrequenz zur Verfügung stellen, die eine auswählbare Mehrzahl von einziehbaren Elektroden beinhaltet, die von einem Zuführungskatheter vorgerückt werden, um ein Ablationsvolumen zu definieren. Der Zuführungskatheter liefert eine langgestreckte Anordnung, die in das Körpergewebe eingeführt werden kann, um das Einführen der Elektroden an eine gewünschte Position innerhalb des Körpers zu erlauben.
- Eine Ausführungsform der Erfindung kann eine Vorrichtung zur Gewebeablation mit Hochfrequenz zur Verfügung stellen, die eine Mehrzahl von Elektroden beinhaltet, die zu und von einem Zuführungskatheter zurückgezogen werden können. Die Elektroden sind zumindest teilweise in einem nicht eingesetzten Zustand in dem Zuführungskatheter angeordnet und werden in einem eingesetzten Zustand erweitert, wenn sie aus einem distalen Ende des Zuführungskatheters vorgerückt werden und das Ablationsvolumen definieren.
- Eine Ausführungsform der Erfindung stellt eine Vorrichtung zur Gewebeablation mit Hochfrequenz zur Verfügung mit eingesetzten Elektroden, die einen ersten Abschnitt mit einem ersten Krümmungsradius haben und einen zweiten Abschnitt, der sich über den ersten Abschnitt hinaus erstreckt, der einen zweiten Krümmungsradius hat.
- Eine Ausführungsform der Erfindung kann auch eine Vorrichtung zur Gewebeablation mit Hochfrequenz zur Verfügung stellen mit eingesetzten Elektroden mit zwei oder mehr Krümmungsradien.
- In einer speziellen Ausführungsform beinhaltet eine Vorrichtung zur Gewebeablation ein Zuführungskatheter mit einem distalen und einem proximalen Ende. Ein Griff ist an dem proximalen Ende des Zuführungskatheters angebracht. Er beinhaltet eine Mehrzahl von Elektroden, die in das distale Ende des Katheters hinein und aus dem distalen Ende des Katheters heraus gezogen werden können. Die Elektroden sind in einem nicht eingesetzten Zustand, wenn sie innerhalb des Zuführungskatheters positioniert sind. Wenn sie aus dem distalen Ende des Katheters hervorgerückt werden, werden sie eingesetzt und definieren ein Ablationsvolumen. Jede Elektrode hat einen ersten Abschnitt mit einem ersten Krümmungsradius und einen zweiten Abschnitt jenseits des ersten Abschnitts mit einem zweiten Krümmungsradius.
- Alternativ dazu kann jede eingesetzte Elektrode mindestens zwei Krümmungsradien haben, die ausgebildet werden, wenn die Nadel durch das distale Ende des Zuführungskatheters vorgerückt wird und an einem ausgewählten Gewebeort positioniert wird.
- Eine Elektrodenschablone kann an dem distalen Ende des Zuführungskatheters positioniert sein. Sie unterstützt das Führen des Einsetzens der Elektroden, so daß sie eine ausgewählte Masse in einem Gewebe von außen umgeben. Die Elektroden können hohl sein. Ein einstellbarer bzw. ju stierbarer Elektrodenisolator kann an jeder oder manchen Elektroden diese direkt umfassend angeordnet sein. Der Elektrodenisolator ist justierbar und kann so gehandhabt werden, daß er entlang der Elektroden vorgerückt und zurückgezogen werden kann, um die leitfähige Oberfläche einer Elektrode zu definieren.
- Die Einrichtung zum Einsetzen der Elektroden kann einen Nocken aufweisen, der die Elektroden in das distale Ende des Zuführungskatheters zurückführt und aus dem distalen Ende des Zuführungskatheters hervorrückt. In dem Zuführungskatheter sind optional ein oder mehrere Führungsschläuche bzw. -röhrchen enthalten, die mit einer oder mehreren Elektroden verbunden sind. Die Führungsschläuche sind an dem distalen Ende des Zuführungskatheters angeordnet.
- Ausgangsmaterial von Infusionsmedien, das elektrolytische und chemotherapeutische Lösungen beinhaltet, aber nicht darauf beschränkt ist, können mit den hohlen Elektroden verbunden sein. Elektroden können spitze, verjüngt zulaufende Enden haben, um ihre Einführung durch Gewebe und das Hervorrücken zu dem ausgewählten Gewebeort zu erleichtern.
- Die Einrichtung zum Einsetzen der Elektroden ist von dem Zuführungskatheter abnehmbar. Eine Schließvorrichtung ist ursprünglich innerhalb des Zuführungskatheters positioniert. Sie kann ein gespitztes distales Ende haben. Der Zuführungskatheter kann mit der in dem Zuführungskatheter positionierten Schließvorrichtung durch die Haut hindurch zu einem inneren Körperorgan oder -ort vorgerückt werden. Einmal positioniert, wird die Schließvorrichtung entfernt und die Einrichtung zum Einsetzen der Elektroden wird in den Zuführungskatheter eingeführt. Die Elektroden sind in nicht eingesetzten Zuständen und vorzugsweise zusammengepreßt oder federnd vorgespannt, während sie innerhalb des Zuführungskatheters angeordnet sind. Sie sind aus einem Material mit ausreichender Stärke gefertigt, so daß, wenn die Elektroden von dem distalen Ende des Zuführungskatheters hervortreten, sie dreidimensional in seitlicher Richtung weg von der Peripherie des distalen Endes des Zuführungskatheters hinweg eingesetzt werden. Die Elektroden behalten ihre laterale Bewegung bei, bis die Kraft, die durch das Gewebe aufgebracht wird, die Nadeln veranlassen, ihre Wegrichtung zu ändern.
- Jede Elektrode hat jetzt zumindest zwei Krümmungsradien. Zusätzlich kann die Elektrodeneinsetzvorrichtung eine oder mehrere eingesetzte Geometrien für die unterschiedlichen Elektroden von der Vielzahl beinhalten. Es ist nicht notwendig, daß jede Elektrode dieselbe eingesetzte Geometrie besitzt.
- Nachdem die Elektroden um eine Masse wie einen Tumor herum positioniert sind, kann eine Vielzahl von Lösungen einschließlich, aber nicht beschränkt auf, elektrolytische Flüssigkeiten, über die Elektroden zu der Masse in einem Vorablationsschritt eingeführt werden. Hochfrequenzenergie wird zugeführt und die Masse wird ausgetrocknet. In einem Postablationsverfahren kann ein Chemothe rapeutikum dann zu dem Ort eingeführt werden, und die Elektroden werden dann in den einführenden Katheter zurückgezogen. 10 gemäß der Erfindung ist in1 dargestellt. Die Ablationsvorrichtung10 beinhaltet ein Zuführungskatheter12 , der den Fachleuten gut bekannt ist, mit einem proximalen Ende14 und einem distalen Ende16 . Der Zuführungskatheter12 kann von der Größe von etwa 5 bis 16 F sein. Ein Griff18 ist abnehmbar an das proximate Ende14 angebracht. Eine Einrichtung zum Einsetzen der Elektroden ist mindestens teilweise innerhalb des Zuführungskatheters12 positioniert und beinhaltet eine Mehrzahl von Elektroden20 , die in das distale Ende16 hinein und aus diesem heraus bewegt werden können. Die Elektroden20 können von verschiedenen Größen, Formen und Konfigurationen sein. - In einer Ausführungsform sind sie Nadelelektroden mit Größen in dem Bereich des Standardmaßes von 27 bis 14. Elektroden
20 sind, während sie in dem Zuführungskatheter zurückgehalten werden, in nicht eingesetzten Positionen. In den nicht eingesetzten bzw. nicht entfalteten Positionen können die Elektroden20 in einem zusammengedrückten, federnd gespannten, im allgemeinen begrenzten oder im wesentlichen gestreckten Zustand sein, wenn sie aus einem geeigneten Metall mit Gedächtniseffekt wie Nitinol hergestellt sind. Wenn die Elektroden20 aus dem distalen Ende16 vorgerückt sind, werden sie in einen eingesetzten bzw. entfalteten Zustand erweitert, der ein Ablationsvolumen definiert, in dem Gewebe abgetragen wird, wie ausführlicher in2 gezeigt ist. Die Elektroden20 arbeiten entweder in dem bipolaren oder monopolaren Modus. Wenn die Elektroden in dem bipolaren Modus verwendet werden, wird das Ablationsvolumen im wesentlichen durch die Peripherie der Vielzahl von Elektroden20 definiert. In einer Ausführungsform ist die Querschnittsbreite des Ablationsvolumens etwa 4 cm. Es versteht sich jedoch, daß verschiedene Ablationsvolumina mit der Gewebeablationsvorrichtung10 erreicht werden können. - Das Ablationsvolumen wird zunächst bestimmt, um eine Masse wie einen Tumor zu definieren, die abgetragen werden soll. Die Elektroden
20 werden für die volumetrische Ablation eine Masse oder Tumor umfassend plaziert. Ein abbildendes System bzw. Bildsystem wird zur ersten Festlegung des Volumens des Tumors oder der ausgewählten Masse verwendet. Geeignete Abbildungssysteme schließen Ultraschall, Computertomographie (CT)-Abtastung, Röntgenfilm, Röntgenfluoroskopie, Magnetresonanzabbildung, elektromagnetische Abbildung und ähnliches ein, sind aber nicht darauf beschränkt. Die Verwendung solcher Einrichtungen, um ein Volumen einer Gewebemasse oder eines Tumors zu definieren, ist den Fachleuten gut bekannt. - In Bezug auf die Verwendung von Ultraschall überträgt ein Ultraschalltransducer Ultraschallenergie in eine interessierende Region in einem Patientenkörper. Die Ultraschallenergie wird durch verschiedene Organe und verschiedene Gewebetypen reflektiert. Die reflektierte Energie wird durch den Transducer erfaßt und das resultierende elektrische Signal wird verarbeitet, um ein Bild der interessierenden Region zu liefern. Das Ablationsvolumen wird dann über diesen Weg ermittelt, und die geeignete Elektrodeneinsetzvorrichtung wird in den Zuführungskatheter
12 eingeführt. - Das Ablationsvolumen wird, im wesentlichen bevor die Ablationseinrichtung
10 in eine ablative Behandlungsposition eingeführt wird, definiert. Dies unterstützt die geeignete Positionierung der Ablationsvorrichtung10 . Auf diese Weise wird das Volumen des abgetragenen Gewebes reduziert und im wesentlichen auf eine definierte Masse oder Tumor begrenzt inklusive eines gewissen Gebietes, das solch einen Tumor umgibt und welches gut kontrolliert und definiert ist. Ein kleiner Bereich um den Tumor wird abgetragen, um zu gewährleisten, daß der ganze Tumor abgetragen wird. - Die Elektrodenabschnitte
20(a) sind gemäß2 in eingesetzten Zuständen, wenn sie aus dem distalen Ende16 herausgeführt werden. Obwohl die Elektroden20 im allgemeinen in einer nicht erweiterten Konfiguration in dem nicht eingesetzten Zustand sind, während sie in dem Zuführungskatheter12 positioniert sind, können sie auch erweitert sein. Im allgemeinen sind die Elektrodenabschnitte20(b) in zurückgehaltenen Positionen, während sie nicht eingesetzt sind. Die Elektroden sind vorgespannt, begrenzt in dem Zuführungskatheter12 und werden nur entfaltet (ausgedehnt), wenn sie von dem Zuführungskatheter12 freigegeben werden. Wie in2 gezeigt, sind die Elektroden20 federnd vorgespannt, während sie in dem Zuführungskatheter12 gehalten werden. Dies ist die nicht eingesetzte Position. Wenn sie aus dem Zuführungskatheter12 hervor und in das Gewebe hineinrücken, werden die Elektroden20 entfaltet bzw. eingesetzt und beginnen von dem distalen Ende16 "aufzufächern", wobei sie sich in seitlicher Richtung relativ zu einer longitudinalen Achse des Zuführungskatheters12 bewegen. Wenn die eingesetzten Elektroden20 ihre Vorrückung fortsetzen, wächst die aufgefächerte Fläche an und erstreckt sich über den Durchmesser des distalen Endes16 hinaus. - Jede Elektrode
20 ist in einer eingesetzten Position signifikant erweitert, und die eingesetzten Elektroden20 definieren zusammen ein Gewebevolumen, das abgetragen werden wird. Wie vorher erwähnt ist es, wenn gewünscht wird, einen entweder gutartigen oder bösartigen Tumor abzutragen, vorzuziehen, einen Bereich abzutragen, der ein wenig über den hinausgeht, der durch die äußere Oberfläche des Tumors definiert wird. Dies erhöht die Wahrscheinlichkeit, daß der gesamte Tumor gründlich beseitigt wird. - Die eingesetzten Elektroden
20 können eine Vielzahl von verschiedenen eingesetzten Geometrien haben, die beinhalten (ii) mindestens zwei Krümmungsradien, (iii) einen gekrümmten Abschnitt mit einer Biegung, die in der Nähe des distalen Endes16 des Zuführungskatheters positioniert ist, und einen nicht gekrümmten Abschnitt, der jenseits des gekrümmten Abschnitts liegt, oder (iv) ein gekrümmter Abschnitt in der Nähe des distalen Endes16 , ein erster linearer Abschnitt und darauf folgend ein anderer gekrümmter Abschnitt oder ein zweiter linearer Abschnitt, der gegenüber dem ersten linearen Abschnitt abgewinkelt ist. Die eingesetzten Elektroden20 müssen nicht parallel zueinander sein. Die Vielzahl von eingesetzten Elektroden20 , die einen Teil der Nadelelektrodeneinsetzvorrichtung definieren, können alle die gleiche entfaltete bzw. eingesetzte Geometrie haben, das heißt alle mit mindestens zwei Krümmungsradien. - Ein Nocken
22 oder eine andere Betätigungseinrichtung kann innerhalb des Zuführungskatheters angeordnet sein und verwendet werden, die Elektroden20 aus dem Zuführungskatheter12 vorzu rücken und in den Zuführungskatheter12 zurückzuziehen. Die konkrete Bewegung des Nockens kann an dem Griff18 gesteuert werden. Geeignete Nocken haben einen üblichen Aufbau, der den Fachleuten gut bekannt ist. - Verschiedene geometrische Anordnungen der Elektroden
20 sind in den3 ,5 und6 dargestellt. Die Elektrode20 hat in3 einen ersten Krümmungsradius20(c) und einen zweiten Krümmungsradius20(d) . Sie kann mehr als zwei Krümmungsradien aufweisen.4 zeigt eine Elektrode20 mit mindestens einem Krümmungsradius, der sich auf drei Ebenen erstreckt, was keine Ausführungsform der vorliegenden Erfindung ist. In5 hat jede Elektrode einen ersten gekrümmten Abschnitt20(e) , der in der Nähe des distalen Endes16 des Zuführungskatheters12 ist. Ein erster im allgemeinen linearer Abschnitt20(f) erstreckt sich jenseits des gekrümmten Abschnitts20(e) , und die beiden treffen sich an einer Biegung20(g) . Die Elektroden20 können als Anoden und Kathoden dienen. Die Vielzahl der Elektroden20 können lineare Abschnitte20(f) haben, die im allgemeinen parallel zueinander sind, oder sie können nicht parallel sein.6 stellt eine Elektrode20 dar, die einen ersten gekrümmten Abschnitt20(e) , der in der Nähe des distalen Endes16 des Zuführungskatheters12 positioniert ist, einen ersten linearen Abschnitt20(f) , und einen zweiten linearen Abschnitt20(h) , der sich jenseits des ersten linearen Abschnitts20(f) erstreckt, beinhaltet. Abschnitt20(h) kann linear, gekrümmt oder eine Kombination aus beidem sein. Die Vielzahl der Elektroden20 , die in6 dargestellt sind, können parallele oder nicht parallele erste Abschnitte20(f) haben. - In einer Ausführungsform der Erfindung sind die Elektroden
20 in ihren nicht eingesetzten Positionen vorgespannt und zusammengedrückt. Wenn die Elektroden20 aus dem distalen Ende16 des Zuführungskatheters12 vorgerückt werden, werden sie eingesetzt und fächern auf. Die Elektroden20 behalten diese Auffächerrichtung bei, bis der Widerstand des Gewebes die Stärke des die Elektrode20 bildenden Materials übersteigt. Dies veranlaßt die Elektrode20 , sich in einer inneren Richtung relativ zu ihrer ursprünglichen äußeren Fächerrichtung zu biegen und zu bewegen. Das Biegen erzeugt die gekrümmten Abschnitte20(c) und20(d) von3 und kann ebenso zu der Ausbildung der anderen Geometrien der Elektrode20 von4 ,5 und6 führen. Das Ausmaß der fächerartigen Bewegung der Elektrode20 hängt von der Stärke des Materials, aus dem sie gefertigt ist, ab. Geeignete Elektrodenmaterialien schließen Edelstahl, Platin, Gold, Silber, Kupfer und andere elektromagnetisch leitenden Materialien inklusive leitfähigen Polymeren ein. Die Elektrode20 ist vorzugsweise aus Edelstahl oder Nickeltitan und hat eine Größe in den Standardmaßen von etwa 27 bis 14. - In einer Ausführungsform ist die Elektrode
20 aus einem Metall mit Gedächtniseffekt hergestellt, wie z. B. Nickeltitan, das kommerziell von Raychem Corporation, Menlo Park, Kalifornien erhältlich ist. Zusätzlich kann ein Widerstandsheizelement in einem inneren Hohlraum der Elektrode20 positioniert werden. Das Widerstandsheizelement kann aus einem geeigneten Metall, das Wärme zu der Elektrode20 leitet, gefertigt sein, so daß die eingesetzte Elektrode20 abgelenkt wird, wenn die Temperatur der Elektrode20 ein Niveau erreicht, das das Elektrodenmaterial wie z. B. ein Metall mit Gedächtniseffekt, veranlaßt abzubiegen, wie es in der Technik bekannt ist. Nicht die ganze Elektrode20 muß aus einem Metall mit Gedächtniseffekt gefertigt sein. Es ist möglich, daß nur der distale Endabschnitt der Elektrode20 , der in das Gewebe eingeführt wird, aus dem Metall mit Gedächtniseffekt gefertigt ist, um die gewünschte eingesetzte geometrische Konfiguration zu erzielen. Mechanische Einrichtungen, die Steuerungsdrähte einschließen, aber nicht auf diese beschränkt sind, können zusätzlich an dem distalen Ende der Elektrode20 angebracht sein, damit sie gelenkt, gebogen und entlang einer gewünschten Richtung um das Gewebe herum bewegt werden kann, bis sie ihre ruhende Endposition erreicht, um eine Gewebemasse abzutragen. - In dem Zuführungskatheter sind optional ein oder mehrere Führungsröhrchen
24 beinhaltet,7 , die dazu dienen, die Ausdehnung der Elektroden20 in das Fächermuster zu lenken, wenn sie aus dem distalen Ende16 des Zuführungskatheters12 herausgeführt werden. Die Führungsröhrchen24 können aus Edelstahl, Springstahl und Thermoplasten, die Nylon und Polyester einschließen, aber nicht auf sie begrenzt sind, gefertigt sein und haben eine ausreichende Größe und Länge, um die Elektroden zu einem spezifischen Ort in dem Körper zu bringen. -
8 stellt eine Ausführungsform der Elektrode20 mit einem spitzen distalen Ende24 dar. Durch das Einschließen eines sich verjüngenden oder stechenden bzw. schneidenden Endes24 , ist das Vorrücken der Elektrode20 durch das Gewebe leichter. Die Elektrode20 kann untergliedert sein und eine Vielzahl von Fluidverteilungsanschlüssen26 beinhalten, die gleichmäßig um alle oder nur einen Abschnitt der Elektrode20 angeordnet sind. Flüssigkeitsverteilungsöffnungen26 sind in der Elektrode20 gebildet, wenn sie hohl ist und erlaubt die Einführung und den Fluß einer Vielfalt von Fluiden bzw. flüssigen Medien durch die Elektrode20 zu einem gewünschten Gewebeort. Solche fließfähigen Medien umfassen, sind aber nicht beschränkt auf, elektrolytische Lösungen, Salben oder Gele sowie chemotherapeutische Mittel. Beispiele von geeigneten leitfähigen Gelen sind Karboxymethylzellulosegele, hergestellt aus wäßrigen Elektrolytlösungen wie z. B. physiologische Kochsalzlösungen und dergleichen. - Die Größe der Flüssigkeitsverteilungsanschlüsse
26 kann variieren abhängig von der Größe und der Form der Elektrode20 . Mit der Elektrode20 ist ebenso eine einstellbare Isolationshülle28 verbunden, die entlang einer äußeren Oberfläche der Elektrode20 verschoben werden kann. Die Isolationshülle28 wird vorgerückt und zurückgezogen entlang der Elektrode20 , um die Größe einer leitfähigen Fläche der Elektrode20 zu definieren. Die Isolationshülle28 wird an dem Griff18 durch den Arzt betätigt, und ihre Position entlang der Elektrode20 wird gesteuert. Wenn die Elektrode20 sich aus dem Zuführungskatheter12 und in das Gewebe bewegt, kann die Isolationshülle28 entlang der Elektrode positioniert werden, während sie sich ihren Weg durch das Gewebe bahnt. Die Isolationshülle28 kann alternativ entlang einer gewünschten Länge der Elektrode20 nachdem die Elektrode20 um eine Zielmasse, die abgetragen wird, herum positioniert wurde, hervorgerückt werden. Die Isolationshülle ist somit in der Lage, durch das Gewebe zusammen mit der Elektrode20 vorzurücken, oder sie kann sich durch das Gewebe bewegen, ohne daß die Elektrode20 die Ursache der Bewegung ist. Das gewünschte Ablationsvolumen wird somit durch die eingesetzten Elektroden20 als auch durch die Positionierung der Isolationshülle28 auf jeder Elektrode definiert. Auf diese Art und Weise wird ein sehr präzises Ablationsvolumen erzeugt. Geeignete Materialien, die Isolatorhüllen bilden, umfassen, sind aber nicht beschränkt auf, Nylon, Polyamide, andere Thermoplaste und dergleichen. -
9 stellt einen die Haut durchdringenden Einsatz der Vorrichtung zur Gewebeablation10 dar. Die Gewebeablationsvorrichtung10 kann durch die Haut hindurch verwendet werden, um die Elektroden20 zu der ausgewählten Gewebemasse oder dem Tumor einzuführen. Die Elektroden20 können in ihren nicht entfalteten bzw. nicht eingesetzten Positionen verbleiben, während sie durch die Haut hindurch in den Körper eingeführt werden und zu einem ausgewählten Organ geführt werden, das die ausgewählte Masse, die abgetragen werden soll, enthält. Der Zuführungskatheter12 ist von dem Griff18 entfernbar. Wenn er abgenommen ist, kann die Elektrodeneinsatzeinrichtung (die Vielzahl der Elektroden20 ) eingeführt werden und von dem Zuführungskatheter12 entfernt werden. Eine Schließvorrichtung30 ist zu Beginn in dem Zuführungskatheter12 eingesetzt, wenn ein perkutanes Verfahren durchgeführt wird. Die Schließeinrichtung30 kann, wie in10 gezeigt, ein spitzes distales Ende32 haben, das Gewebe durchbohrt und die Einführung des Zuführungskatheters12 zu einem ausgewählten Gewebeort erleichtert. Der ausgewählte Gewebeort kann ein Körperorgan mit einem Tumor oder einer anderen Masse sein oder der konkrete Tumor selbst. - Die Schließeinrichtung
30 wird dann von dem Zuführungskatheter12 (11 ) entfernt. Die Elektrodeneinsatzeinrichtung wird dann in den Zuführungskatheter12 eingesetzt, und der Katheter wird dann wieder mit dem Griff18 (12 ) verbunden. Die Elektrodeneinsatzvorrichtung kann, wie in12 dargestellt ist, optional eine Elektrodenschablone34 beinhalten, um den Einsatz der Elektroden20 zu einer eine ausgewählte Masse in dem Gewebe umgebenden Position zu führen. - Die Elektroden
20 werden dann aus dem distalen Ende16 des Zuführungskatheters12 hervorgerückt und werden eingesetzt, um ein gewünschtes Ablationsvolumen zu bilden, das die Masse umgibt. Der Zuführungskatheter12 in13 ist in der Nähe der Leber positioniert. Die Elektrodeneinsatzvorrichtung wird in den Zuführungskatheter12 mit der Elektrodenschablone34 eingeführt. Die Elektrodeneinsatzeinrichtung durchdringt nun die Leber und der Nocken22 rückt die Elektroden20 aus dem Zuführungskatheter12 in die eingesetzten Positionen vor. Jede einzelne Elektrode20 durchdringt die Leber und läuft durch sie, bis sie in unmittelbarer Umgebung von des Tumors positioniert ist. Das Ablationsvolumen ist wählbar und wird als erstes durch Abbilden des abzutragenden Bereichs bestimmt. Das Ablationsvolumen wird durch die Peripherie von allen der eingesetzten Elektroden20 bestimmt, die das Äußere des Tumors umgeben. Sobald das Ablationsvolumen bestimmt ist, wird dann ein Elektrodensatz ausgewählt, der eingesetzt werden wird, um das Ablations volumen zu definieren. Eine Vielzahl von verschiedenen Faktoren sind wichtig bei der Erzeugung eines Ablationsvolumens. In erster Linie werden verschiedene Elektroden20 , basierend auf der Art des Elektrodenmaterials, dem Grad der Vorspannung der Elektroden und der geometrischen Konfiguration der Elektroden in ihren eingesetzten Zuständen, verschiedene Einsatzgrade haben. Die Gewebeablationsvorrichtung10 erlaubt es, verschiedene Elektrodensätze20 in den Zuführungskatheter12 einzusetzen, um eine Vielzahl von Ablationsvolumina zu definieren. - Vor der Ablation des Tumors kann ein Vorablationsschritt durchgeführt werden. Eine Vielzahl von verschiedenen Lösungen, inklusive elektrolytischer Lösungen wie Kochsalzlösung, können an den Ort des Tumors eingeführt werden, wie in
14 gezeigt.15 zeigt die Zuführung der Hochfrequenzenergie zu dem Tumor. Der Elektrodenisolator28 wird auf Abschnitten der Elektroden20 positioniert, wo keine Ablation stattfindet. Dies definiert ebenfalls das Ablationsvolumen. Die konkrete Elektroaustrocknung des Tumors oder anderer Zielmassen oder -geweben ist in16 gezeigt. Wieder definieren die eingesetzten Elektroden20 , mit ihren Elektrodenisolatoren28 , die auf Abschnitten der Elektroden positioniert sind, das Ablationsvolumen und die resultierende Menge der Masse, die ausgetrocknet bzw. entwässert wird. - Auf die Austrocknung bzw. Entwässerung folgend können die Elektroden
20 optional eine Vielfalt von Lösungen in einem Postablationsprozeß einführen. Dieser Schritt ist in17 illustriert. Geeignete Lösungen umfassen, sind aber nicht limitiert auf, chemotherapeutische Mittel. -
18 stellt die Gewebeablationsvorrichtung10 dar, die in einem bipolaren Modus betrieben wird. Ihr monopolarer Betrieb ist in19 gezeigt. Jede der Vielzahl von Elektroden20 kann unterschiedliche Rollen in dem Ablationsverfahren spielen. Es kann eine Polaritätsverschiebung zwischen den unterschiedlichen Elektroden geben. - Ein Gewebeablationssystem
36 , das modular sein kann, ist in20 gezeigt und kann eine optische Anzeigeeinrichtung38 beinhalten. Das Gewebeablationssystem36 beinhaltet die Hochfrequenzenergiequelle und kann ebenso eine Mikrowellenquelle, eine Ultraschallquelle, Visualisierungseinrichtungen wie Kameras und VCRs, elektrolytische und chemotherapeutische Lösungsquellen und eine Steuereinrichtung, der für die Überwachung der Temperatur oder der Impedanz verwendet werden kann, beinhalten. Eine der eingesetzten Elektroden20 kann eine Mikrowellenantenne sein, die mit einer Mikrowellenquelle verbunden ist. Diese Elektrode kann ursprünglich mit der Hochfrequenzleistungsquelle42 verbunden sein und wird dann an die Mikrowellenquelle geschaltet. - Eine Stromversorgung
40 liefert gemäß21 die Energie in den Hochfrequenzleistungsgenerator (Quelle)42 und dann zu den Elektroden20 der Gewebeablationsvorrichtung10 . Ein Multiplexer46 mißt Strom, Spannung und Temperatur (an zahlreichen Temperatursensoren, die auf den Elektroden20 positioniert sein können). Der Multiplexer46 wird durch einen Controller bzw. eine Steuerein richtung48 angetrieben, der ein digitaler oder analoger Controller oder ein Computer mit Software sein kann. Wenn der Controller48 ein Computer ist, kann er eine CPU, die über einen Systembus verbunden ist, beinhalten. Dieses System kann eine Tastatur, ein Diskettenlaufwerk oder andere nicht flüchtige Speichersysteme, eine Anzeige und andere Peripherie wie in der Technik bekannt ist, beinhalten. Mit dem Bus ist auch ein Programmspeicher und ein Datenspeicher verbunden. - Ein Benutzer Interface
50 beinhaltet Benutzerkontrollen52 und eine optische Anzeige38 . Der Controller48 ist mit den abbildenden Systemen verbunden, die Ultraschalltransducer, Temperatursensoren und Sichtoptiken und optischen Fasern beinhalten, falls vorhanden. - Strom und Spannung werden benutzt, um den Widerstand zu berechnen. Untersuchungen werden durch Ultraschall, CT-Abtastung oder andere in der Technik bekannte Methoden durchgeführt. Bildaufnahme kann vor, während und nach der Behandlung durchgeführt werden.
- Die Temperatursensoren messen die Spannung und den Strom, der geliefert wird. Der Ausgangswert dieser Sensoren wird von dem Controller
48 verwendet, um die Zuführung der Hochfrequenzleistung zu steuern. Der Controller48 kann ebenfalls die Temperatur und die Leistung steuern. Die Menge der gelieferten Hochfrequenzenergie steuert die Leistungsmenge. Ein Profil der gelieferten Leistung kann in dem Controller38 aufgenommen werden, genauso wie eine voreingestellte zu liefernde Energiemenge als Profil aufgenommen werden kann. - Zur Rückkopplung kann die Messung der Impedanz oder der Temperatur verwendet werden und tritt entweder am Controller
48 oder an der elektromagnetischen Energiequelle42 , z. B. Hochfrequenz oder Mikrowelle, wenn sie einen Regler bzw. Controller beinhaltet, auf. Für eine Widerstandsmessung kann dies durch Zuführen einer kleinen Menge von Nicht-Ablations-Hochfrequenzenergie erreicht werden. Spannung und Strom werden dann gemessen. - Verschaltungstechnik, Software und Rückkopplung zu dem Controller
48 führen zu einer Verfahrenssteuerung und werden verwendet, (i) die Leistung inklusive Hochfrequenz, Ultraschall und dergleichen, (ii) den Arbeitszyklus (an/aus und Wattzahl), (iii) die monopolare oder bipolare Energielieferung, (iv) die Zuführung chemotherapeutischer und elektrolytischer Lösungen, die Flußrate und den Druck zu verändern und (v) durch Zeit, Temperatur und/oder Widerstand zu bestimmen, wann die Ablation vollständig ist. Diese Prozeßvariablen können gesteuert und variiert werden, basierend auf der an vielen Orten überwachten Temperatur und der Impedanz gegen den Stromfluß, der überwacht wird und der Veränderungen in der Stromtragfähigkeit des Gewebes während des Ablationsverfahrens anzeigt. - Es wurden Ausführungsformen von Vorrichtungen zur Tumorbehandlung mit Hochfrequenz beschrieben, die für minimal invasive Verfahren nützlich sind. Ausführungsformen wurden dargestellt, wo das Äußere des Tumors mit Behandlungselektroden umgeben wird, die ein kontrolliertes Ablationsvolumen definieren, und die Elektroden anschließend eine kontrollierte Menge von Hochfrequenzenergie liefem. Es wurden Ausführungsformen mit Infusionsfähigkeit während eines Vorablationsschrittes beschrieben, wobei nach der Ablation das umgebende Gewebe mit elektromagnetischer ("EM") Energie bei Hyperthermietemperaturen von weniger als 45°C vorbehandelt werden kann. Dies könnte für die synergetischen Effekte der Chemotherapie, Einträufelung einer Vielzahl von Flüssigkeiten an dem Ort des Tumors nach der lokalen Ablation und Hyperthermie sorgen.
- Die vorhergehende Beschreibung von bevorzugten Ausführungsformen der vorliegenden Erfindung wurde für den Zweck der Illustrierung und Beschreibung geliefert. Sie beabsichtigt nicht, umfassend zu sein oder die Erfindung auf die präzisen offenbarten Ausführungen zu begrenzen. Es ist klar, daß viele Veränderungen und Variationen für fachmännische Praktiker offensichtlich sind. Die Ausführungsformen wurden ausgewählt und beschrieben, um die Prinzipien der Erfindung und ihre praktische Anwendung am besten zu erklären, wodurch andere Fachleute in die Lage versetzt werden, die Erfindung für verschiedene Ausführungsformen und mit verschiedenen Veränderungen, wie sie für die spezielle in Betracht gezogene Verwendung geeignet sind, zu verstehen.
Claims (27)
- Hochfrequenz-Gewebeablationsvorrichtung bestehend aus: einem Zuführungskatheter (
12 ), der in das Körpergewebe eingefügt werden kann mit einem distalen Ende (16 ) und einem proximalen Ende (14 ), Einrichtungen (18 ) zum Halten des Katheters am proximalen Ende, und Elektroden-Einsatzeinrichtungen einschließlich eines Elektrodenvorrück- und Zurückziehgliedes (22 ), die zumindest teilweise in dem Katheter positioniert sind, und einer Mehrzahl von einziehbaren elektromagnetisch leitenden Elektroden (20 ), wobei jede von dem distalen Ende des Zufühnungskatheters vorrückbar ist, sich in das Gewebe hineinstecken läßt, ein durchbohrendes distales Ende hat, so daß sie das Gewebe durchdringt und einen vorgespannten nicht eingesetzten Zustand hat, wenn sie innerhalb des Katheters positioniert ist, und einen eingesetzten entfalteten Zustand hat, wenn sie von dem distalen Ende des Katheters vorgerückt ist, wobei die einziehbaren Elektroden derart betreibbar sind, daß sie von dem distalen Ende des Katheters hervortreten und sich entfalten, wenn sie hiervon vorgerückt werden, um in einer seitlichen Richtung weg von der Peripherie des distalen Endes des Zuführungskatheters einzusetzen, so daß sie zwischen den eingesetzten Elektroden ein dreidimensionales Ablationsvolumen in einer eine ausgewählte Masse in dem Gewebe umgebenden Beziehung definieren, wobei die Elektroden an eine steuerbare Hochfrequenzenergiequelle zur kontrollierten Aufbringung der Hochfrequenzenergie in das dreidimensionale Ablationsvolumen angeschlossen werden können, wobei jede der Mehrzahl von Elektroden vorgespannt bzw. vorgefedert ist, so daß in dem eingesetzten Zustand: die Elektrode entlang ihrer Länge die Richtung ändert, so daß die Elektrode zumindest zwei Krümmungsradien außerhalb des Katheters zeigt, wobei die zumindest zwei Krümmungsradien vorgebogen bzw. unter Vorspannung stehen, wobei einer der Krümmungsradien veranlaßt, daß sich die Elektrode seitlich von dem Katheter weg einsetzt, wenn sie von dem Katheter gelöst wird, wobei sich eine Mehrzahl der eingesetzten Elektroden dadurch gemeinsam lateral in Bezug auf den Katheter aufweiten, um das dreidimensionale Ablationsvolumen zu erzeugen. - Vorrichtung nach Anspruch 1, in der das Ablationsvolumen im wesentlichen durch die Peripherie der Mehrzahl von Elektroden im eingesetzten Zustand definiert ist.
- Vorrichtung nach einem der vorherigen Ansprüche, wobei die Elektroden, wenn sie von dem distalen Endabschnitt des Katheters vorgerückt werden, eingesetzt werden und sich von dem distalen Endabschnitt des Katheters fächerartig aufspannen.
- Vorrichtung nach einem der vorherigen Ansprüche, in der die Elektroden beim Vorrücken vom distalen Endabschnitts des Katheters eingesetzt werden und vom distalen Endabschnitt des Katheters durch das Gewebe aufgefächert werden.
- Vorrichtung nach einem der vorherigen Ansprüche, in der mindestens eine Elektrode im eingesetzten Zustand mindestens einen gekrümmten Abschnitt, der in der Nähe des distalen Endabschnitts des Katheters lokalisiert ist, und einen nicht gekrümmten Abschnitt jenseits des gekrümmten Abschnitts aufweist.
- Vorrichtung nach einem der vorherigen Ansprüche, in der eine Elektrode in einem nicht im Gewebe eingesetzten Zustand mindestens zwei Krümmungsradien aufweist.
- Vorrichtung nach einem der vorherigen Ansprüche, in der die Elektroden Metall mit Memory-Effekt oder Federstahl enthalten.
- Vorrichtung nach einem der vorherigen Ansprüche, die zusätzlich eine am distalen Endabschnitt des Zuführungskatheters positionierte Elektrodenschablone (
34 ) zum Führen des Einsetzens der Elektroden, so daß sie eine ausgewählte Masse im Gewebe außen umgeben, enthält. - Vorrichtung nach einem der vorherigen Ansprüche, die zusätzlich einen oder mehrere Steuerungsdrähte, die an die Elektroden angebracht sind, besitzt.
- Vorrichtung nach einem der vorherigen Ansprüche, in der die Elektroden hohl sind und Flüssigkeitsverteilungsöffnungen (
26 ) besitzen. - Vorrichtung nach Anspruch 10, die zusätzlich eine Infusionsmediumquelle, die mit jeder Elektrode verbunden ist, besitzt.
- Vorrichtung nach Anspruch 10, die zusätzlich eine Quelle von chemotherapeutischen Arzneimitteln, die mit jeder Elektrode verbunden ist, besitzt.
- Vorrichtung nach einem der vorherigen Ansprüche, in der eine der Elektroden ein gespitztes distales Ende (
24 ) aufweist. - Vorrichtung nach einem der vorherigen Ansprüche, die weiterhin einen justierbaren Elektrodenisolator (
28 ) aufweist, der an jeder Elektrode und diese umfassend angeordnet ist, so daß jeder justierbare Elektrodenisolator so gehandhabt werden kann, daß er entlang einer Elektrode vorgerückt und zurückgezogen werden kann, um die leitfähige Oberfläche der Elektrode zu definieren. - Vorrichtung nach einem der vorherigen Ansprüche, in der die Elektroden-Einsatzeinrichtung vom Zuführungskatheter abnehmbar ist.
- Vorrichtung nach einem der vorherigen Ansprüche, die zusätzlich eine abnehmbare Schließvorrichtung (
30 ), die innerhalb des Zuführungskatheter positioniert wird, besitzt. - Vorrichtung nach Anspruch 16, in der die Schließvorrichtung ein angespitztes distales Ende aufweist.
- Vorrichtung nach einem der Ansprüche 16 oder 17, in der die Schließeinrichtung im Zuführungskatheter positioniert ist, um den Zuführungskatheter zu einer Stelle innerhalb des Körpers zuführen.
- Vorrichtung nach einem der Ansprüche 16 bis 18, in der die Schließeinrichtung nach dem der Zuführungskatheter an einer Stelle innerhalb des Körpers positioniert worden ist, vom Zuführungskatheter abgenommen werden kann, so daß dann die Elektrodeneinsatzvorrichtung im Zuführungskatheter positioniert werden kann.
- Vorrichtung nach einem der vorherigen Ansprüche, in der die Halteeinrichtung einen Griff (
18 ), der von dem Zuführungskatheter abgenommen werden kann, aufweist. - Vorrichtung nach einem der vorherigen Ansprüche, in der eine Elektrode drahtartig ist.
- Vorrichtung nach einem der vorherigen Ansprüche, in der eine Elektrode einen hohlen Schlauch aufweist.
- Vorrichtung nach Anspruch 1, die zusätzlich eine elektrische Hochfrequenzenergiequelle aufweist, die mit mindestens einer Elektrode verbunden ist.
- Vorrichtung nach Anspruch 23, die eine Impedanzmessungseinrichtung zur Kontrolle der Applikation der Hochfrequenzenergie aufweist.
- Vorrichtung nach Anspruch 23, die eine Temperaturmessungseinrichtung zur Kontrolle der Applikation der Hochfrequenzenergie aufweist.
- Vorrichtung nach einem der vorherigen Ansprüche für monopolare Energiezulieferung.
- Vorrichtung nach einem der Ansprüche 1 bis 26, für bipolare Energiezulieferung.
