- Die Erfindung betrifft Stents und Verfahren zur Herstellung der Stents.
- HINTERGRUND
- Der Körper besteht aus verschiedenen Kanälen, wie zum Beispiel Arterien, andere Blutgefäße und andere Körperlumen. Manchmal werden diese Kanäle verengt oder geschwächt. Beispielsweise können diese Kanäle durch einen Tumor verengt, durch Plaque eingeengt oder durch ein Aneurysma geschwächt werden. Wenn dies geschieht, kann der Kanal mithilfe einer medizinischen Endoprothese wieder geöffnet, verstärkt oder sogar ersetzt werden. Eine Endoprothese ist typischerweise ein röhrenförmiges Element, das in ein Lumen im Körper eingesetzt wird. Beispiele von Endoprothesen umfassen Stents und beschichtete Stents, die manchmal „Stent-Grafts" genannt werden.
- Endoprothesen können mithilfe von Kathetern ins Innere des Körpers gebracht werden, wobei der Katheter die Endoprothese in verdichteter oder größenreduzierter Form stützt, wenn die Endoprothese an eine gewünschte Stelle transportiert wird. Wenn die Endoprothese die Stelle erreicht, wird sie ausgeweitet, dass sie zum Beispiel die Wandungen des Lumens berühren kann.
- Der Ausweitungsmechanismus kann beinhalten, dass die Endoprothese gezwungen wird, sich radial auszuweiten. Beispielsweise kann der Ausweitungsmechanismus beinhalten, dass der Katheter einen Ballon aufnimmt, der eine Ballon expandierbare Endoprothese aufnimmt. Der Ballon kann aufgeblasen werden, um die expandierte Endoprothese an einer vorbestimmten Position in Berührung mit der Lumenwandung zu verformen und zu befestigen. Der Ballon kann anschließend kollabiert werden und der Katheter kann entfernt werden.
- Bei einer anderen Einbringtechnik ist die Endoprothese aus einem elastischen Material gebildet, das reversibel komprimiert und ausgedehnt werden kann, zum Beispiel elastisch oder durch einen Phasenübergang des Materials. Während des Einbringens in den Körper ist die Endoprothese in verdichtetem Zustand eingeschränkt. Sobald die gewünschte Implantationsstelle erreicht wird, wird die Einschränkung entfernt, zum Beispiel indem eine Einspannvorrichtung, wie zum Beispiel eine Außenhülle, zurückgezogen wird, wodurch sich die Endoprothese durch die eigene innere elastische Rückstellkraft selbst entfalten kann.
- Endoprothesen sind zur Stützung eines geöffneten Kanals manchmal aus relativ festem Material, wie zum Beispiel Edelstahl oder Nitinol (einer Nickel-Titan-Legierung), das zu Verstrebungen oder Drähten geformt wird. Diese Materialien können jedoch relativ strahlendurchlässig sein. Das heißt, dass die Materialien bei Röntgendurchleuchtung nur schwer sichtbar sein können, was eine Technik ist, die angewandt wird, um die Endoprothesen während und nach dem Einbringen zu lokalisieren und zu kontrollieren. Zur Verbesserung ihrer Sichtbarkeit (zum Beispiel durch Erhöhung ihres Röntgenkontrastes) können die Endoprothesen mit einem relativ strahlendichten Material, wie zum Beispiel Gold, beschichtet werden, wie dies zum Beispiel aus der
WO2/39927 A EP-A-0824900 bekannt ist. Da die Endoprothesen üblicherweise eine relativ lange Zeit im Körper gelassen werden, ist es wünschenswert, dass sie eine gute Biokompatibilität aufweisen. - Die

- Die Erfindung wird definiert durch einen Stent gemäß Anspruch 1 und einem Verfahren zur Herstellung eines Stents gemäß Anspruch 18. Vorteilhafte Ausführungsformen sind als abhängige Ansprüche definiert.

1 zeigt eine Halterung12 , die einen Stent10 trägt, der die Form eines rohrförmigen Elements aufweist, das aus Verstrebungen11 und Öffnungen13 besteht. Je nach Art des Stents12 (zum Beispiel Ballon expandierbar oder selbst entfaltend), kann die Halterung12 ein Ballonkatheter oder ein Katheterschaft sein. Bezug nehmend auf2 umfasst der Stent10 eine Vielzahl von Querschnittsabschnitten. Insbesondere bestehen die Verstrebungen11 des Stents10 aus einem relativ strahlendurchlässigen Kern14 , der von einem relativ strahlendichten Teilbereich16 umgeben ist. Der strahlendichte Teilbereich16 umfasst eine strahlendichte Schicht18 , zum Beispiel aus Gold, und einer Schicht20 , zum Beispiel aus einer Gold-Titan-Legierung, die die Biokompatibilität des Stents10 erhöhen kann. Die Schicht20 kann beispielsweise passiviert werden, um eine relativ reaktionsträge Außenoberfläche am Stent10 zu bilden. - Der Stent
10 kann im Allgemeinen gebildet werden, indem man einen relativ strahlendurchlässigen Stent mit einem strahlendichten Material, wie zum Beispiel Gold oder Platin, beschichtet, um Schicht18 zu bilden. Schicht20 wird dann auf dem strahlendichten Material gebildet. Schicht20 kann auf der vorher gebildeten strahlendichten Schicht18 und/oder aus einem Teilbereich der strahlendichten Schicht gebildet werden. Schicht20 wird dann passiviert, zum Beispiel durch das Bilden einer Schicht aus einem Oxid oder Nitrid auf Schicht20 oder durch Umwandlung der Schicht20 in ein Oxid oder ein Nitrid. - Kern
14 ist im Allgemeinen aus einem oder mehreren Kernmaterialien gebildet, die so ausgewählt werden, dass sie den Stent10 mit bestimmten physikalischen und mechanischen Eigenschaften versehen. Beispielsweise wird das Kernmaterial so gewählt, dass es den Stent10 mit ausreichend „hoop strength" [Fähigkeit eines Stents, Innendrücken sowie Verbiegungskräften und Stauchkräften standzuhalten] und Radialfestigkeit versieht, sodass der Stent ein Körpergefäß geöffnet halten kann. Geeignete Kernmaterialien sind unter anderem Edelstahl (zum Beispiel Edelstahl Typ 316L), Nitinol (zum Beispiel für selbst entfaltende Stents), andere Titan-Legierungen, Tantal-Legierungen, Zirkonium-Legierungen und/oder Niob-Legierungen. Gleichzeitig ist es auch wünschenswert, Unterschiede oder Unausgeglichenheiten der mechanischen Eigenschaften (zum Beispiel Steifigkeit) zwischen Stent und Körpergefäß zu verringern (zum Beispiel minimieren). Die mechanische Unausgeglichenheit kann beispielsweise eine Entzündung und/oder einen Wiederverschluss des Gefäßes verursachen. Ein Verfahren zur Verringerung der mechanischen Unausgeglichenheit ist, einen Stent mit weniger Material zu bilden (zum Beispiel indem man kleinere Verstrebungen11 bildet), wodurch man sich der Flexibilität oder Elastizität des Gefäßes nähert. Die Verringerung der Menge des Kernmaterials des Stents10 kann jedoch auch die Röntgensichtbarkeit des Stents verringern. - Um die Röntgensichtbarkeit des Stents
10 zu erhöhen, umfasst der Stent einen strahlendichten Teilbereich16 , der über dem Kernbereich14 angeordnet ist. Der Teilbereich16 umfasst eine strahlendichte Schicht18 , die aus einem strahlendichten Material gebildet ist. Das strahlendichte Material kann jedes Material mit einer Dichte und/oder einem linearen Absorptionskoeffizienten sein, die bzw. der ausreicht, um die Röntgensichtbarkeit des Stents10 zu erhöhen. In Ausführungsformen hat das strahlendichte Material eine Dichte und/oder einen linearen Absorptionskoeffizienten, die bzw. der einen einfallenden Röntgenstrahl abschwächt. In einigen Fällen hat das strahlendichte Material eine Dichte von größer oder gleich ca. 10 g/cc. Beispiele strahlendichten Materials schließen beispielsweise Gold, Platin, Palladium, Tantal, Iridium, Kobalt, Titan, Wolfram, Edelstahl, Nitinol und Metalllegierungen ein, die einen ausreichenden Prozentanteil schwerer Elemente enthalten. Die strahlendichte Schicht18 kann beispielsweise bis zu 8 Mikrometer dick sein, zum Beispiel um die 6–8 Mikrometer dick. Verfahren zum Bilden einer strahlendichten Schicht18 umfassen beispielsweise elektrochemische Abscheidung, Vakuumaufdampfverfahren (z. B. Sputtern), chemische Aufdampfung, Galvanisieren und/oder Eintauchen (z. B. in eine Schmelze). - In manchen Fällen hat das strahlendichte Material jedoch nicht den gewünschten Biokompatibilitätsgrad und/oder die Biokompatibilität des Materials ist unbekannt (z. B. auf lange Sicht). Man geht zum Beispiel davon aus, dass Gold den Elektronenübergang bei bestimmten unerwünschten Reaktionen im Körper beeinträchtigen (z. B. katalysieren) kann. Dementsprechend umfasst der strahlendichte Teilbereich
16 eine relativ reaktionsträge Schicht20 , die über der strahlendichten Schicht18 angeordnet ist. - Die Schicht
20 erhöht die Biokompatibilität des Stents10 , indem der Stent mit einer Schicht (wie gezeigt, mit einer Außenschicht) versehen wird, die beispielsweise viel leichter als die strahlendichte Schicht18 passiviert werden kann. Zum Beispiel kann die Schicht20 reagieren (z. B. oxidieren) und Erzeugnisse bilden, wie zum Beispiel Oxide, Nitride und/oder Karbide, die reaktionsträger sind und deshalb eine höhere Biokompatibilität aufweisen als das Material oder die Materialien in der strahlendichten Schicht18 . Im Verhältnis zur Schicht18 hat die Schicht20 ein geringeres Oxidationspotential, das heißt sie kann leichter oxidiert werden um ein biokompatibles Erzeugnis zu bilden. - In einigen Ausführungsformen umfasst die Schicht
20 eine Mischung (hier eine Legierung) aus dem strahlendichten Material oder den strahlendichten Materialien in der strahlendichten Schicht18 und einem oder mehreren Legierungsmaterialien. Das Legierungsmaterial kann jedes Material sein, das mit dem strahlendichten Material oder den strahlendichten Materialien ein Gemisch bilden und ein Erzeugnis bilden kann, das leichter passiviert werden kann, als das/die strahlen dichten Material(ien). Das Legierungsmaterial kann beispielsweise Tantal, Titan, Niob, Zirkonium, Chrom, Silizium, Rhodium, Iridium, Platin und/oder Palladium sein. Alle Legierungsmaterialien können mit allen oben beschriebenen strahlendichten Materialien verwendet werden. - Beispielsweise kann Titan das Legierungsmaterial für eine strahlendichte Goldschicht
18 sein. Bei diesem Beispiel umfasst die Schicht20 eine Gold-Titan-Legierung, wie zum Beispiel Au0,30Ti0,70, die leichter passiviert werden kann, als Gold. Das heißt, dass die Gold-Titan-Legierung im Verhältnis zu Gold leichter ein Erzeugnis bilden kann oder in ein Erzeugnis umgewandelt werden kann, zum Beispiel in ein Oxid, das relativ reaktionsträge und biokompatibel ist. In Ausführungsformen kann bei der Gold-Titan-Legierung (AuxTiy) x im Bereich von ca. 0–30% und y im Bereich von ca. 70–100% liegen. Beispielsweise kann x größer oder gleich ca. 0%, 5%, 10%, 15%, 20% oder 25% und/oder kleiner oder gleich ca. 30%, 25%, 20%, 15%, 10% oder 5% sein. In Ausführungsformen kann bei der Konzentration von Titan y größer oder gleich ca. 70%, 75%, 80%, 85%, 90% oder 95% und/oder kleiner oder gleich 100%, 95%, 90%, 85%, 80% oder 75% sein. Die Schicht20 kann bis zu 20 Mikrometer dick sein, zum Beispiel ca. 0,1–10 Mikrometer dick. Ternäre (z. B. Au-Ti-Cr) oder höhere Gemische oder Legierungssysteme können gebildet werden. - In einigen Ausführungsformen kann die Schicht
20 auf einer vorgebildeten strahlendichten Schicht18 gebildet werden. Beispielsweise kann nach dem Bilden der strahlendichten Schicht18 die veränderte Schicht20 auf der strahlendichten Schicht durch Vakuumaufdampfverfahren inklusive Sputtern und ionenstrahlgestützter Deposition, chemischer Aufdampfung oder Elektrodeposition aufgebracht werden. Die Schicht20 kann auch durch das Bilden von Schichten, zum Beispiel alternierenden Schichten, aus dem strahlendichten Material und dem Legierungsmaterial auf der Schicht18 in einem vorbestimmten Verhältnis gebildet werden, und durch Erwärmen der Schichten (z. B. bei erhöhten Glühtemperaturen), um die Legierung durch Diffusion zu bilden. - Alternativ oder zusätzlich kann die Schicht
20 aus einem Teilbereich der gebildeten strahlendichten Schicht18 gebildet werden. Das heißt, dass ein Teilbereich der strahlendichten Schicht18 zur Schicht20 umgewandelt werden kann. Beispielsweise kann eine Gold-Titan-Schicht20 gebildet werden, indem Titanionen in eine gebildete strahlendichte Goldschicht18 implantiert werden und die strahlendichte Schicht geglüht wird. Dadurch wird eine bestimmte Dicke der strahlendichten Schicht (z. B. im Submikrometerbereich) in eine legierte veränderte Schicht umgewandelt, die passiviert werden kann. In einem weiteren Beispiel kann eine Schicht eines Legierungsmaterials, z. B. Ti, auf der strahlendichten Schicht18 , z. B. Au abgeschieden werden, und die Schichten können erwärmt, zum Beispiel geglüht, werden, um eine Legierung, zum Beispiel Au-Ti zu bilden. - Es sollte beachtet werden, dass während
2 eine strahlendichte Schicht18 und Schicht20 als zwei separate, klar abgegrenzte Schichten zeigt, es bei manchen Ausführungen der Fall ist, dass die Übergangsstelle zwischen den Schichten nicht eindeutig bestimmt ist. Deshalb kann die Endoprothese mit guten Hafteigenschaften und hoher Haltbarkeit (z. B. mit verringertem Risiko eines Ablösens) gebildet werden. Die Korrosion auf Grund von Berührung mit ungleichem Material kann auch verringert werden. Die Übergangsstelle kann nicht eindeutig bestimmt werden, wenn beispielsweise die veränderte Schicht20 aus einer gebildeten strahlendichten Schicht18 gebildet wird. - In manchen Ausführungsformen umfasst der strahlendichte Teilbereich
16 keine Übergansstelle zwischen zwei Schichten. Bezug nehmend auf3 ist eine Verstrebung22 eines Stents aus einem relativ strahlendurchlässigen Kern24 gebildet, der von einer relativ strahlendichten Schicht26 umgeben ist. Der Kern24 ist in der Regel der Gleiche, wie der oben beschriebene Kern14 . Wie oben beschrieben umfasst die strahlendichte Schicht26 ein oder mehrere strahlendichte Materialien und ein oder mehrere Legierungsmaterialien. Des Weiteren ist die strahlendichte Schicht26 so gebildet, dass sie einen Zusammensetzungsgradien ten aufweist, in dem die Konzentration(en) des Legierungsmaterials oder der Legierungsmaterialien und/oder des strahlendurchlässigen Materials oder der strahlendurchlässigen Materialien der Dicke der Schicht26 entlang variiert (Pfeile A und B). Beispielsweise kann bei einer strahlendichten Schicht26 aus einer Gold-Titan-Legierung die Schicht26 an der Oberfläche28 , die an den Kern24 angrenzt, relativ goldhaltig (oder titanarm) sein und relativ goldarm (oder titanhaltig) an der Außenoberfläche30 . An der Oberfläche28 kann die Konzentration des strahlendichten Materials bei ca. 100% liegen; und an der Außenoberfläche30 kann die Konzentration des Legierungsmaterials bei ca. 100% liegen. Die Konzentration(en) des strahlendichten Materials bzw. der strahlendichten Materialien und/oder des Legierungsmaterials bzw. der Legierungsmaterialien können linear oder nichtlinear (z. B. exponentiell) zwischen den Oberflächen28 und30 variieren. Die Konzentration(en) z. B. des Legierungsmaterials kann/können von Oberfläche28 zu Oberfläche30 zu- oder abnehmen. In bestimmten Ausführungsformen kann die Schicht26 , die den Zusammensetzungsgradienten aufweist, auf einer strahlendichten Schicht, wie zum Beispiel der strahlendichten Schicht18 , gebildet werden. - Verfahren zum Bilden der zusammensetzungsgradierten Schicht
26 umfassen den Einsatz von Vakuumaufdampfverfahren während der Überprüfung der Quelle der Materialien, die für die Deposition verwendet werden. Bei einem anderen Verfahren kann die Schicht26 gebildet werden, indem alternierende Schichten eines strahlendichten Materials und eines Legierungsmaterials in einem vorbestimmten Verhältnis gebildet werden und indem die Schichten geglüht werden. Bezug nehmend auf4 können beispielsweise Schichten aus Titan27a ,27b und27c alternierend mit Schichten aus Gold29a ,29b ,29c gebildet werden, um einen Konzentrationsgradienten aus Titan entlang der Schicht26 zu bilden. Die Titanschicht27a ist dicker als die Schicht27b , die wiederum dicker ist, als die Schicht27c . Die Goldschichten29a –29c haben die gleiche Dicke. Wenn die Schichten anschließend geglüht werden, können sie zusammen diffundieren und eine Gold-Titan-Legierung bilden, bei der die Titankonzentration entlang der Di cke der Schicht26 variiert (die hier mit zunehmender Entfernung vom Kern24 zunimmt). - Nachdem die Schicht
20 bzw.26 gebildet ist, kann Stent10 passiviert werden, indem der Stent einer geeigneten Umgebung ausgesetzt wird. Beispielsweise kann Stent10 oxidiert werden, indem man den Stent in einer Oxidationsatmosphäre erwärmt, wie zum Beispiel einer, die Sauerstoff und/oder Wasser enthält, um eine Oxidschicht auf der Schicht20 bzw.26 zu bilden. Nitride können gebildet werden, indem man den Stent10 in einer Umgebung erwärmt, die Stickstoff, Stickstoff-Wasserstoff und/oder Ammoniak enthält. Aufkohlung, z. B. die Erhöhung der Kohlenstoffkonzentration an der Oberfläche, kann durchgeführt werden, indem man den Stent10 bei erhöhter Temperatur einer Umgebung aussetzt, die einen hohen Gehalt eines Kohlenwasserstoffgases, zum Beispiel Methan, aufweist. Alternativ oder zusätzlich kann eine Passivierung durch Elektropolieren durchgeführt werden, um eine oxidhaltige Oberflächenschicht zu erzeugen. In einigen Fällen kann die Passivierung relativ spontan auftreten, zum Beispiel durch Lufteinwirkung, wenn das Oxidationspotential relativ gering ist. - Stent
10 kann dann mit herkömmlichen Verfahren nachbearbeitet werden, zum Beispiel durch Elektropolitur für eine glatte Oberfläche. Stent10 kann vor Passivierung nachbearbeitet werden. Alternativ kann Stent10 mit Oberflächentexturierung gebildet werden. - Stent
10 kann dann verwendet werden, zum Beispiel entsprechend herkömmlicher Verfahren eingebracht und entfaltet werden. - Üblicherweise kann Stent
10 selbst entfaltend, Ballon expandierbar oder eine Kombination beider Varianten sein. Beispiele des Stents10 und der Halterung12 sind in denUS Patenten Nr. 5,725,570 (Heath) und5,234,457 (Andersen) beschrieben. - In anderen Ausführungsformen ist Stent
10 ein Teil eines Stent-Grafts. Das Stent-Graft kann ein Stent sein, der an einer biokompatiblen nichtporösen oder halbporösen Polymermatrix aus Polytetrafluorethylen (PTFE), expandiertem PTFE, Polyethylen, Urethan oder Polypropylen angebracht ist. Stent10 kann einen lösbaren therapeutischen Wirkstoff oder eine pharmazeutische Aktivverbindung umfassen, wie in derUS 5,674,242 beschrieben wird. Die therapeutischen Wirkstoffe oder pharmazeutischen Aktivverbindungen können beispielsweise antithrombotische Mittel, Antioxidanten, entzündungshemmende Mittel, Antikoagulantien und Antibiotika einschließen. - Die folgenden Beispiele dienen der Veranschaulichung und sind nicht einschränkend gedacht.
- Beispiel
- Das folgende Beispiel beschreibt ionenstrahlgestützte Deposition (IBAD) als ein Verfahren zur Deposition dünner Schichten auf einem Trägermaterial, zum Beispiel einem Stent.
- Bezug nehmend auf
5 umfasst eine IBAD-Anlage50 üblicherweise eine Haltevorrichtung52 , die so gestaltet ist, dass sie einen Stent54 trägt und eine Depositionsvorrichtung56 . Die Anlage50 wird in einer Vakuumkammer51 bei Drücken von etwa 1 × 10–4 – 3 × 10–4 Torr verwendet, die teilweise von einer Diffusionspumpe58 geliefert werden. - Die Depositionsvorrichtung
56 umfasst zwei Tiegel60 und62 , deren entsprechende Verschlüsse64 und66 , zwei Elektronenstrahlverdampfer68 und70 und einen Ionenstrahlkanone72 . Die Tiegel60 und62 , die zum Beispiel aus Grafit sind, beinhalten Materialien zur Abscheidung, wie zum Beispiel Gold und Titan. Elektronenstrahlverdampfer68 und70 sind so ausgestaltet, dass sie einen Elektronenfluss generieren, der (z. B. mit Hilfe von Magnetfeldern) auf das Mate rial in den Tiegeln60 bzw.62 forciert werden kann, um die Materialien zu schmelzen und zu verdampfen, um thermisch verdampfte Materialien76 zu bilden. Die Verdampfer68 und70 können wassergekühlte Ummantelungen haben, die die Tiegel60 bzw.62 kühlen. Die Ionenstrahlkanone72 ist so ausgestaltet, dass sie einen Argonfluss (z. B. 2–4 sccm) aufnimmt und das Argon ionisiert, um ein Plasma74 zu bilden. Das Plasma74 wird mit (nicht gezeigten) Magneten aus der Ionenstrahlkanone72 zum Stent54 hin beschleunigt. Die Verschlüsse64 und66 können bewegt, zum Beispiel geschwenkt, werden, um den Fluss des verdampften Materials76 von den Tiegeln60 bzw.62 zu erlauben oder zu blockieren. - Die Haltevorrichtung
52 ist üblicherweise so ausgestaltet, dass der Stent54 gleichmäßig mit verdampftem Material76 beschichtet werden kann. Normalerweise kann beim thermischen Verdampfverfahren eine Schicht des Materials76 auf einem Trägermaterial aufgebracht werden, das auf einer Sichtlinie mit dem Tiegel60 oder62 liegt. Für eine einheitliche Beschichtung des Stents54 wird der Stent während der Deposition rotiert. In Ausführungsformen ist der Stent54 auf einer rotierbaren Spindel platziert. Die Reibung zwischen dem Stent und der Spindel kann den Stent während der Rotation in Position halten, um einen beschichteten Stent zu erzeugen, der keine Kontaktpunkte aufweist. Alternativ kann Stent54 an einer rotierbaren Welle befestigt werden. - Ein Quarzkristall
78 wird verwendet, um die Dicke des abgeschiedenen Materials zu bestimmen. Der Kristall78 ist mit einem (nicht dargestellten) Regler verbunden und wird oszilliert. Der Regler ist kalibriert, so dass die Dicke des auf den Kristall78 (und somit auch auf den Stent54 ) abgeschiedenen Materials berechnet werden kann, indem die Änderung der Oszillationsfrequenz des Kristalls gemessen wird. - Nun wird ein IBAD-Beschichtungsverfahren beschrieben.
- Der Stent
54 , zum Beispiel ein Stent aus Nitinol oder Edelstahl, wird gründlich chemisch gereinigt. Beispielsweise kann Stent54 in einem Lösemittel (wie zum Beispiel Isopropylalkohol oder Aceton) und einem Entfettungsmittel gereinigt und mit deionisiertem Wasser gespült werden. Wärme und/oder Erschütterung, zum Beispiel durch Verwendung von Ultraschall-Energie kann/können eingesetzt werden, um den Stent54 zu reinigen. Der Stent54 wird dann auf die Haltevorrichtung52 gesetzt, die dann in die Vakuumkammer51 eingebracht wird, so dass der Stent ca. 2 Fuß [ca. 61 cm] von den Tiegeln60 und62 entfernt ist. - Der Stent
54 durchlauft dann eine Sputter-Reinigung. Die Kammer51 wird auf einen Druck von ca. 1 × 10–5 Torr evakuiert und die Ionenstrahlkanone72 wird aktiviert. Die Ionenstrahlkanone72 ionisiert Argongas, um ein Plasma74 zu erzeugen und das Plasma wird zum Stent54 beschleunigt, um die Oberfläche des Stents zu sputterreinigen/sputterätzen. Der Einfallwinkel des Plasmas74 kann bei ca. 45–90° liegen, beispielsweise bei ca. 70°. In Ausführungsformen wird der Stent54 ca. 20–30 Minuten sputtergereinigt. Es können ungefähr 100–300 Angström Material entfernt werden. - Dann wird ein erstes Material, zum Beispiel Gold in Tiegel
60 , abgeschieden. Während den letzten zehn Minuten der Sputter-Reinigung werden die Elektronenstrahlverdampfer68 und70 langsam hochgefahren. Die Verschlüsse64 und66 befinden sich über ihren entsprechenden Tiegeln60 und62 , so dass sich kein Material auf dem Stent54 absetzen kann. Wenn die Sputter-Reinigung beendet ist und das abzuscheidende Material geschmolzen ist, bewegt sich der Verschluss64 , z. B. durch Schwenken, um zu ermöglichen, dass das verdampfte Material den Stent54 beschichtet. Die Oberfläche des Stents54 wird gleichzeitig mit dem Plasma74 beschossen. Man geht davon aus, dass, wenn sich die Ionen des ersten Materials auf dem Stent54 absetzen, das Plasma74 Energie auf die Ionen überträgt, wodurch sich einige Ionen von der Oberfläche des Stents lösen und es einigen Ionen ermöglicht wird, an die Oberfläche des Stents zu wandern. Demzufolge wird davon ausgegangen, dass ein Verbundwerkstoff, der das erste Material beinhaltet, mit erhöhter Dichte gebildet wird. - Ein zweites Material, zum Beispiel Titan, Tantal oder Platin wird dann abgeschieden. Nachdem die Dicke des zuerst auf Stent
54 aufgetragenen Materials zum Beispiel etwa 200–500 Angström erreicht, wird der Verschluss66 bewegt, sodass es dem zweiten Material (in Tiegel62 ) ermöglicht wird, zusammen mit dem ersten Material abgeschieden zu werden. Die Konzentrationen beider Materialien können kontrolliert werden, indem man den Stromfluss zu den Verdampfern68 und70 reguliert. Beispielsweise ist in Bezug auf6 die Konzentration des ersten Materials anfangs relativ hoch und dann wird das zweite Material langsam eingebracht. In Ausführungsformen wird der Verschluss64 zum Zeitpunkt t bewegt, sodass das erste Material nicht auf dem Stent54 abgeschieden werden kann und eine reine Schicht des zweiten Materials wird auf die Legierungsschicht (das heißt die Schicht aus dem ersten und dem zweiten Material) abgeschieden. Dann kann der Stent54 abkühlen, in der Kammer51 wird wieder ein atmosphärischer Druck hergestellt und der Stent wird aus der Kammer entnommen. - In Ausführungsformen wird Stent
54 dann geglüht. Glühen kann die Diffusion zwischen den Materialschichten und/oder den Schichten und dem Trägermaterial des Stents fordern und kann die Verbindung oder die Haftung zwischen den Schichten verstärken. In einigen Fällen kann ein Nitinol-Stent bei etwa 300–400°C geglüht werden und ein Edelstahl-Stent kann bei etwa 500–1000°C geglüht werden. Die Glühzeiten können variieren, zum Beispiel von einigen Minuten bis zu Tagen, zum Beispiel in Abhängigkeit der Diffusion der Materialien in Stent54 , welche temperaturabhängig sein kann. -
7 zeigt Bereiche für einige Prozessparameter. - Ein Stent wurde unter Anwendung der oben beschriebenen Verfahren mit Titan beschichtet. Die Prozessparameter werden in
8 gezeigt. - Ein Stent wurde unter Anwendung der oben beschriebenen Verfahren mit einem Platin-Gold beschichtet. Die Prozessparameter werden in
9 gezeigt. Der Platin-Gold-Gradient war ähnlich dem in6 gezeigten. - Andere Ausführungsformen
- In anderen Ausführungsformen können eine oder mehrere Zwischenschichten zwischen dem Kern
14 oder24 und der strahlendichten Schicht18 oder26 gebildet werden, das heißt, zumindest ein Teilbereich des Kerns und der strahlendichten Schicht berühren sich nicht. Beispielsweise können in Ausführungsformen, in denen eine Gitterfehlanpassung zwischen dem Kern und der strahlendichten Schicht besteht, (eine) Zwischenschicht(en) ausgewählt werden, sodass Zwischengitterparameter als Pufferschicht(en) dienen können, wodurch Spannungen zwischen dem Kern und der strahlendichten Schicht verringert (z. B. minimiert) werden. Die Zwischenschicht(en) kann/können beispielsweise eine Mischung aus dem Kernmaterial und der strahlendichten Schicht sein. - Es kann sein, dass Schicht
20 das strahlendichte Material oder die strahlendichten Materialien aus der strahlendichten Schicht18 nicht enthält/enthalten. Beispielsweise kann eine strahlendichte Schicht Gold enthalten, während die Schicht20 ein Material beinhaltet, das passiviert werden kann, wie zum Beispiel eine Platin-Titan-Legierung. - Die strahlendichte Schicht
18 , die Schicht20 und/oder die Schicht26 können alle oder nur einen oder mehrere ausgewählte Teilbereiche eines Stents bedecken. Zum Beispiel können die strahlendichte Schicht18 , die Schicht20 und/oder die Schicht26 nur auf einem oder mehreren Endbereichen des Stents gebildet werden. - In einigen Ausführungsformen können andere Arten von Schichten auf der Schicht
20 oder26 gebildet werden. Beispielsweise kann/können ein oder mehrere ausgewählte Teilbereich(e) eines Stents magnetisch undurchlässiges (d. h. durch Magnetresonanztomographie (MRT) sichtbares) Material auf Schicht20 oder26 beinhalten. Geeignete magnetisch undurchlässige Materialien beinhalten beispielsweise Nichteisenmetall-Legierungen, die paramagnetische Elemente enthalten (z. B. Dysprosium oder Gadolinium), wie zum Beispiel Terbium-Dysprosium, Dysprosium und Gadolinium; Nichteisenmetall-Lagen, die mit einer Oxid- oder Karbidschicht aus Dysprosium oder Gadolinium (z. B. Dy2O3 oder Gd2O3) beschichtet sind; Nichteisenmetalle (z. B. Kupfer, Silber, Platin oder Gold), die mit einer Schicht aus superparamagnetischem Material, wie zum Beispiel dem nanokristallinen Fe3O4, CoFe2O4, MnFe2O4 oder MgFe2O4; und nanokristallinen Partikeln der Übergangsmetalloxide (z. B. Oxide aus Fe, Co, Ni) beschichtet sind. - In anderen Ausführungsformen können die strahlendichte Schicht
18 , die Schicht20 und/oder die Schicht26 auf anderen medizinischen Vorrichtungen als Stents und Stent-Grafts gebildet werden, zum Beispiel auf Vorrichtungen, bei denen Röntgensichtbarkeit gewünscht ist, wie beispielsweise orthopädischen Implantaten. - Weitere Ausführungsformen ergeben sich aus den Ansprüchen.
Claims (27)
- Stent (
10 ), welcher umfasst: ein Element (11 ), welches beinhaltet einen ersten Bereich (16 ) mit einer ersten Schicht (18 ), welche ein strahlenundurchlässiges Material beinhaltet, und einer zweiten Schicht (20 ), dadurch gekennzeichnet, dass die zweite Schicht (20 ) eine äußere Oberfläche des Elementes (11 ) definiert und das strahlenundurchlässige Material der ersten Schicht (18 ) und ein zweites Material beinhaltet, welches ausgewählt ist aus der Gruppe bestehend aus Titan, Niob, Palladium, Chrom und Silizium. - Stent nach Anspruch 1, wobei das Element (
11 ) weiterhin beinhaltet einen zweiten Bereich (14 ), welcher im Inneren des ersten Bereiches (16 ) angeordnet ist, wobei der zweite Bereich (14 ) weniger strahlenundurchlässig ist als die erste Schicht (18 ), wobei die zweite Schicht (20 ) ein geringeres Oxidationspotenzial hat als ein Oxidationspotenzial der ersten Schicht (18 ). - Stent nach Anspruch 1 oder 2, wobei das strahlenundurchlässige Material ausgewählt ist aus der Gruppe bestehend aus Gold, Platin, Palladium und Tantal.
- Stent nach einem der Ansprüche 1 bis 3, wobei die zweite Schicht (
20 ) eine Legierung aus dem strahlenundurchlässigen Material und einem zweiten Material umfasst. - Stent nach einem der Ansprüche 1 bis 4, wobei der zweite Bereich (
14 ) ein Material umfasst, welches ausgewählt ist aus der Gruppe bestehend aus Edelstahl und einer Nickel-Titan Legierung. - Stent nach einem der Ansprüche 1 bis 5, wobei der zweite Bereich (
14 ) der innerste Bereich des Elementes (11 ) ist. - Stent nach einem der Ansprüche 1 bis 6, wobei der erste Bereich (
16 ) den zweiten Bereich (14 ) berührt. - Stent nach einem der Ansprüche 1 bis 7, wobei der erste und der zweite Bereich (
16 ,14 ) verschiedene Zusammensetzungen haben. - Stent nach einem der Ansprüche 1 bis 8, weiterhin eine polymere Schicht auf dem Element (
11 ) umfassend. - Stent nach einem der Ansprüche 1 bis 9, weiterhin eine Arzneimittel freisetzende Schicht auf dem Element (
11 ) umfassend. - Stent nach einem der Ansprüche 1 bis 10, wobei die zweite Schicht (
20 ) ein geringeres Oxidationspotenzial aufweist als ein Oxidationspotenzial der ersten Schicht (18 ). - Stent nach einem der Ansprüche 3 bis 11, wobei das Element (
11 ) weiterhin beinhaltet einen zweiten Bereich (14 ), welcher im Inneren des ersten Bereiches (16 ) angeordnet ist, wobei der zweite Bereich (14 ) strahlenundurchlässiger ist als der erste Bereich (16 ). - Stent nach Anspruch 12, weiterhin einen dritten Bereich zwischen dem ersten Bereich und dem zweiten Bereich umfassend.
- Stent nach einem der Ansprüche 1 bis 13, wobei der erste Bereich (
26 ) einen Konzentrationsgradienten eines strahlenundurchlässigen Materials aufweist. - Stent nach Anspruch 14, wobei die Konzentration des strahlenundurchlässigen Materials zunimmt als eine Funktion der Entfernung von der äußeren Oberfläche (
30 ) aus. - Stent nach Anspruch 14 oder 15, wobei der Konzentrationsgradient im Wesentlichen linear variiert entlang einer Dicke des ersten Bereiches (
26 ). - Verfahren zum Herstellen eines Stents (
10 ), welcher ein Element (11 ,22 ) beinhaltet, wobei das Verfahren umfasst: Bilden einer äußeren Schicht (20 ,26 ) auf dem Element (11 ,22 ), wobei die äußere Schicht (20 ,26 ) ein strahlenundurchlässiges Material und ein zweites Material umfasst, welches ausgewählt ist aus der Gruppe bestehend aus Titan, Niob, Palladium, Chrom und Silizium; und Oxidieren eines Bereiches der äußeren Schicht (20 ,26 ). - Verfahren nach Anspruch 17, wobei das Oxidieren des Bereiches Bilden eines Oxids aus der äußeren Schicht (
20 ,26 ) umfasst. - Verfahren nach Anspruch 17 oder 18, wobei das Oxidieren des Bereiches Bilden eines Nitrids aus der äußeren Schicht (
20 ,26 ) umfasst. - Verfahren nach einem der Ansprüche 17 bis 19, weiterhin Bilden einer strahlenundurchlässigen Schicht (
18 ), welche ein strahlenundurchlässiges Material umfasst, umfassend. - Verfahren nach einem der Ansprüche 17 bis 20, wobei die äußere Schicht (
26 ) mit einem zusammengesetzten Gradienten gebildet wird. - Verfahren nach einem der Ansprüche 17 bis 21, wobei die äußere Schicht (
20 ,26 ) durch einen Prozess gebildet wird, welcher ausgewählt ist aus der Gruppe bestehend aus physikalischer Gasphasenabscheidung, chemischer Gasphasenabscheidung und elektrolytischer Abscheidung. - Verfahren nach einem der Ansprüche 17 bis 22, wobei das Oxidieren des Bereiches auf der äußeren Schicht (
20 ,26 ) durch Elektropolieren ausgeführt wird. - Verfahren nach einem der Ansprüche 17 bis 22, wobei das Oxidieren des Bereiches auf der äußeren Schicht (
20 ,26 ) durch Aufheizen der äußeren Schicht (20 ,26 ) in einer oxidierenden Umgebung ausgeführt wird. - Verfahren nach einem der Ansprüche 17 bis 22, wobei das Oxidieren des Bereiches auf der äußeren Schicht (
20 ,26 ) durch Ionenimplantieren von Sauerstoff in eine äußere Schicht (20 ,26 ) und Aufheizen der äußeren Schicht (20 ,26 ) ausgeführt wird. - Verfahren nach einem der Ansprüche 17 bis 25, weiterhin Bilden einer polymeren Schicht auf der äußeren Schicht (
20 ,26 ) umfassend. - Verfahren nach einem der Ansprüche 17 bis 26, weiterhin Bilden einer Arzneimittel freisetzenden Schicht auf der äußeren Schicht (
20 ,26 ) umfassend.
