Description
- HINTERGRUND DER ERFINDUNG
- Die vorliegende Erfindung bezieht sich auf implantierbare Wirbelvorrichtungen und Verfahren für ihre Herstellung. Insbesondere bezieht sich die vorliegende Erfindung auf Zwischenkörpervorrichtungen, die aus Knochen gebildet sind und die bei Wirbelverschmelzungen bzw. -fusionen verwendet werden können.
- Es steht eine Vielfalt von Zwischenkörperimplantaten für Wirbelverschmelzungseingriffe zur Verfügung. Diese Implantate wurden bzw. werden aus verschiedenen Materialien hergestellt, einschließlich Stahl, Titan, Verbundstoffen, allogenen Transplantaten, xenogenen Transplantaten oder anderen bioverträglichen Materialien, und besitzen die erforderliche Festigkeit, um zu verhindern, dass der Scheibenzwischenraum zusammenfällt, bevor die Verschmelzung stattgefunden hat. Andere Verfahren zur Wirbelverschmelzung umfassen die Anordnung von Knochentransplantatmaterial im Scheibenzwischenraum zusammen mit einem Platten- oder Stabgebilde, das den betroffenen Scheibenzwischenraum überspannt. Ein Nachteil an den obigen Vorrichtungen besteht darin, dass, sobald die Verschmelzung stattgefunden hat, die Implantate und Baueinheiten, die verwendet werden, um die Stabilität des Segments aufrechtzuerhalten, unnötig sind und als Fremdobjekt im Körper verbleiben.
- Andere Arten von Implantaten wurden aus bioverträglichen Metallen entwickelt, die Gewinde an der äußeren Oberfläche des Implantats beinhalten, die das Implantat im Scheibenzwischenraum festhalten, nachdem es in diesen geschraubt worden ist. Noch weitere Implantate wurden entwickelt, die aus Knochen bestehen. Beispiele solcher Abstandhalter, die aus Knochen bestehen und die bei Wirbeleingriffen Verwendung haben, sind im
US-Patent Nr. 5 989 289 offenbart. Die Abstandhalter im '289-Patent sind mit Wirbeleingriffsoberflächen an der oberen und der unteren Fläche des Implantats versehen, um einer Wanderung des Implantats im Scheibenzwischenraum und/oder einem Austreiben des Implantats aus dem Scheibenzwischenraum Widerstand zu leisten. Obwohl Abstandhalter, die aus Knochen bestehen, in Verschmelzungseingriffen eine stark verbesserte Integration bieten, begrenzt die innewohnende brüchige Art von Knochen, die sich aus einem hohen Mineralgehalt ergibt, insbesondere von einem lasttragenden Rindenknochen, sein Potential für die Verwendung in Anwendungen stark, die erfordern, dass das Implantat einer anderen Belastung als einer Belastung vom Auflage- oder Drucktyp standhält. Der Rindenknochen besteht beispielsweise typischerweise aus ungefähr 70% Mineralgehalt und 30% nicht mineralischem Stoff. Von diesem nicht mineralischen Stoff sind ungefähr 95% Collagen vom Typ I, wobei der Ausgleich zellulärer Stoff und Nicht-Kollagen-Proteine sind. -
WO-A-00/74607 - Knochentransplantate wurden üblicherweise in einer festen Form, pulverisiert oder als biegsamer entmineralisierter Knochen verwendet. Eine Form eines biegsamen Knochentransplantats ist ein entmineralisiertes Knochenmaterial typischerweise in Form eines Schwamms oder Kitts mit sehr geringer struktureller Integrität. Obwohl ein entmineralisiertes Knochensegment Eigenschaften beibehalten kann, die zum Unterstützen des Knocheneinwuchses geeignet sind, werden die strukturellen Eigenschaften des Knochens durch die Entfernung seines Mineralgehalts geändert. Folglich können solche Knochenschwämme und -kitte typischerweise nicht in Lasttrageanwendungen verwendet werden.
- Daher bleibt ein Bedarf bestehen an Knochenimplantaten mit den erforderlichen Lasttragefähigkeiten für Anwendungen, die sowohl Auflage- als auch Drucklasttragefähigkeiten erfordern, zusammen mit Fähigkeiten zum Standhalten einer anderen Belastung als einer Belastung vom Auflage- oder Drucktyp.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Die vorliegende Erfindung ist auf ein Knochenimplantat mit einem starren Abschnitt zum Einfügen zwischen benachbarte Knochenstrukturen und einem flexiblen Abschnitt zur Befestigung an den benachbarten Knochenstrukturen gerichtet.
- Gemäß einem Aspekt der Erfindung wird ein Implantat geschaffen, das umfasst:
einen Körperabschnitt, der in dem Scheibenzwischenraum zwischen einem oberen Wirbel und einem benachbarten unteren Wirbel positionierbar ist;
einem oberen Element, das sich von dem Körperabschnitt erstreckt und so konfiguriert ist, dass es sich längs des Körpers des oberen Wirbels erstreckt; und
einem unteren Element, das sich von dem Körperabschnitt erstreckt und so konfiguriert ist, dass es sich längs des Körpers des unteren Wirbels erstreckt, wobei der Körperabschnitt, das obere Element und das untere Element jeweils aus Knochenmaterial hergestellt sind. - Gemäß einem weiteren Aspekt der Erfindung wird ein Verfahren zum Präparieren bzw. zur Herstellung eines Knochenimplantats geschaffen, das umfasst:
Vorsehen eines starren Knochensegments, das einen Körperabschnitt mit einer oberen Lageroberfläche und einer gegenüberliegenden unteren Lageroberfläche besitzt, wobei das starre Knochensegment des Weiteren ein oberes Flanschelement und ein gegenüberliegendes unteres Flanschelement aufweist, die sich jeweils von dem Körperabschnitt erstrecken; und
wenigstens teilweises Entmineralisieren des oberen und des unteren Flanschelements, um ein flexibles oberes Flanschelement und ein flexibles untere Flanschelement zu schaffen, die sich von dem starren Körperabschnitt erstrecken, wobei das obere und das untere Flanschelement so konfiguriert sind, dass sie sich längs eines oberen Wirbelkörpers bzw. eines unteren Wirbelkörpers erstrecken, wenn der Körperabschnitt in einem Zwischenraum zwischen dem oberen Wirbelkörper und dem unteren Wirbelkörper positioniert ist bzw. wird. - Diese und weitere Aspekte, Vorteile, Merkmale, Ausführungsformen und Aufgaben der vorliegenden Erfindung sind für Fachleute auf der Basis der folgenden Beschreibung der erläuterten Ausführungsformen der vorliegenden Erfindung ersichtlich, welche nur beispielhaft gegeben werden.

4 ist eine perspektivische Ansicht eines Implantats einer nochmals weiteren Ausführungsform gemäß der vorliegenden Erfindung. - BESCHREIBUNG DER ERLÄUTERTEN AUSFÜHRUNGSFORMEN
- Mit Bezug nun auf
1 ist ein Implantat gemäß einer Ausführungsform der vorliegenden Erfindung gezeigt. Obwohl Implantate gemäß der vorliegenden Erfindung viele Verwendungen besitzen können, ist die in1 gezeigte Ausführungsform für das Fördern einer Zwischenknochenverschmelzung in der Wirbelsäule besonders angepasst. Insbesondere stellt1 ein Knochenimplantat10 mit einem ersten im Wesentlichen starrten Körperabschnitt12 dar, der sich zwischen einem Vorderende30 und einem Hinterende32 erstreckt. Das Implantat10 umfasst ferner am Hinterende32 ein erstes oder oberes Flanschelement14 , das sich vom Körperabschnitt12 nach oben erstreckt, und ein zweites oder unteres Flanschelement16 , das sich vom Körperabschnitt12 nach unten erstreckt. Vorzugsweise bestehen der Körperabschnitt12 und die Flanschelemente14 ,16 aus einem einzigen Stück aus Knochenmaterial und die Flanschelemente sind mit dem Körperabschnitt12 einteilig. Andere Ausführungsformen ziehen jedoch in Erwägung, dass die Flansche aus einem separaten Materialstück bestehen, wie z. B. Knochen oder Knorpel, und am Körperabschnitt12 über Befestigungseinrichtungen oder ein anderes bekanntes Verbindungsverfahren befestigt sind. - Die Flanschelemente
14 und16 wurden zumindest teilweise entmineralisiert, um flexible Flanschelemente zu erzeu gen, die sich vom starren Körperabschnitt12 erstrecken. Der entmineralisierte Abschnitt des Implantats10 kann sich durch den starren Körperabschnitt12 zwischen dem oberen Flanschelement14 und dem unteren Flanschelement16 erstrecken, wie dargestellt. Alternativ kann sich der entmineralisierte Abschnitt teilweise in den starren Körperabschnitt12 erstrecken oder an der Verbindungsstelle zwischen den Flanschelementen14 ,16 und dem starren Körperabschnitt12 enden. Vorzugsweise wurden zumindest die Flanschelemente14 und16 vollständig entmineralisiert, um eine maximale Flexibilität bereitzustellen. Die durch die Entmineralisierung erzeugte Flexibilität ermöglicht, dass die Flanschelemente14 und16 in Bezug auf den starren Körperabschnitt12 und in Bezug aufeinander beweglich sind, und folglich ähnlich einem Band funktionieren, das sich zwischen der benachbarten Knochenstruktur und dem Körperabschnitt12 erstreckt und an diesen befestigt ist. - Der Körperabschnitt
12 des Implantats10 besitzt einen Hohlraum18 , der vorzugsweise vom Zwischenmarkkanal des Knochens abgeleitet ist, aus dem das Implantat10 durch einen Querschnitt über die Diaphyse eines Wadenbeins, Oberschenkels oder ähnlichen langen Knochens erhalten wird. Der Hohlraum18 sieht einen Bereich zum Aufnehmen von Material vor, der die Knochenintegration und -verschmelzung fördert. Vor der Positionierung des Körperabschnitts12 im Scheibenzwischenraum kann ein Knochenwachstums-Förderungsmaterial28 im Hohlraum18 angeordnet werden, um das Knochenwachstum in den und durch den Körperabschnitt12 zu fördern. Das Knochenwachstumsmaterial kann eine beliebige Art von Material sein, das auf dem Fachgebiet bekannt ist. Wie weiter in2 gezeigt, umfasst das obere Flanschelement14 eine erste Befestigungseinrichtungsbohrung20 zum Aufnehmen einer ersten Befestigungseinrichtung24 und das untere Flansch element16 besitzt eine zweite Befestigungseinrichtungsbohrung22 zum Aufnehmen einer zweiten Befestigungseinrichtung26 . Die Befestigungseinrichtungen der vorliegenden Erfindung können in Form einer Gewindeschraube vorliegen und aus Metall, Knochen, Polymer, bioabsorbierbarem Material oder einem anderen auf dem Fachgebiet bekannten Material bestehen. - Wie in
2 gezeigt, zieht eine spezielle Anwendung des Implantats10 der vorliegenden Erfindung die Verwendung zur Verschmelzung der Wirbel der Halswirbelsäule in Erwägung. In dieser Ausführungsform wird das Implantat10 aus dem Wadenbein erhalten. Der Körperabschnitt12 kann eine beliebige Form aufweisen, einschließlich einer speziellen Form zur Verwendung im Halswirbelbereich, wie z. B. derjenigen Formen, die imUS-Patent Nr. 5 989 289 identifiziert sind. - Zu den Wirbeln V1 und V2 wird von einem vorderen Zugang unter Verwendung bekannter Operationsverfahren gelangt. Das Scheibenmaterial wird entfernt und die Scheibenzwischenraumhöhe wird unter Verwendung bekannter Operationsverfahren wiederhergestellt, falls erforderlich. Das Implantat
10 wird in den präparierten Scheibenzwischenraum eingesetzt. Der starre Körperabschnitt12 ist so ausgelegt, dass er eine strukturelle Abstützung zwischen der jeweiligen unteren Endplatte des oberen Wirbels V1 und der oberen Endplatte des Wirbels V2 vorsieht. In der dargestellten Ausführungsform besitzt der starre Körperabschnitt12 eine Höhe H, die ausreicht, um eine Abstützung für den gewünschten Abstand zwischen den benachbarten Wirbeln V1 und V2 vorzusehen und diesen aufrechtzuerhalten. Die Verschmelzung zwischen den Wirbeln V1 und V2 wird mit Knochenwachstum durch den Hohlraum18 erhalten, der mit Knochenwachstumsmaterial28 gefüllt wird. Die Verschmelzung zwischen den Wirbeln kann durch Reduzieren der Endplatten auf Blutung des Knochens vor dem Einsetzen des Implantats10 weiter gefördert werden. - Das Implantat
10 besitzt eine obere Lageroberfläche25 , die den oberen Wirbelkörper V1 berührt und abstützt, und eine untere Lageroberfläche27 , die das Implantat10 auf dem unteren Wirbelkörper V2 berührt und abstützt. Der Körperabschnitt12 besitzt eine Höhe H zwischen der oberen Lageroberfläche25 und der unteren Lageroberfläche27 , die im Wesentlichen gleich der Höhe des zwischen dem Wirbel V1 und dem Wirbel V2 gebildeten Zwischenscheibenraums ist. Für Fachleute ist verständlich, dass in der hierin dargestellten bevorzugten Ausführungsform die Höhe H im Wesentlichen konstant ist. Obwohl ein Implantat mit gleichmäßiger Höhe in2 gezeigt ist, ist ferner verständlich, dass die Implantate der vorliegenden Erfindung eine verjüngte Höhe aufweisen können, so dass das Implantat zum Herstellen oder Aufrechterhalten der korrekten Krümmung in der Wirbelsäule verwendet werden könnte. Der starre Körperabschnitt12 weist eine ausreichende Steifigkeit und strukturelle Integrität auf, um die Höhe H im Wesentlichen aufrechtzuerhalten und normalen Kräften, die auf die Wirbelsäule aufgebracht werden, standzuhalten. Die Flanschelemente14 und16 müssen keine solchen strukturellen Anforderungen aufweisen, obwohl vorzugsweise jedes die Implantatstabilität durch Halten des starren Körperabschnitts12 im Scheibenzwischenraum zwischen den zwei Wirbeln unterstützt. - Die Befestigungseinrichtungen
24 und26 werden durch die entsprechenden Befestigungseinrichtungsbohrungen20 und22 im oberen bzw. unteren Flanschelement14 und16 angeordnet, um das Implantat10 im Scheibenzwischenraum zu stabilisieren. Da die Flanschelemente14 und16 flexibel sind, können sie benachbart zu den Wirbelkörpern außerhalb des Scheibenzwischenraums ohne Erzeugung von großen Scher- und Biegespannungen im Implantat10 an der Verbindungsstelle zwischen den Flanschelementen14 ,16 und dem Körperabschnitt12 manipuliert und positioniert werden. - Obwohl in einer speziellen Ausführungsform in Erwägung gezogen wird, dass das Implantat
10 Anwendung für die Verschmelzung eines Halswirbelbereichs der Wirbelsäule hat, wird die Anwendung in anderen Bereichen der Wirbelsäule und an anderen Verbindungen, wo es erwünscht ist, ein Knochenimplantat mit einem starren Körperabschnitt zu haben, wobei sich ein Paar von flexiblen Elementen davon erstrecken, auch in Erwägung gezogen. Das Knochenimplantat10 schafft die erwünschten Merkmale, dass es aus einem sehr erfolgreichen Knochenverschmelzungsmaterial, d. h. natürlichem Knochen, gebildet ist, mit den Vorteilen, dass es flexible Elemente aufweist, die aus Knochen bestehen, um den starren Knochenkörperabschnitt des Implantats an der Implantationsstelle zu befestigen. - In einem weiteren Operationsverfahren kann eine Zugkraft auf das obere Flanschelement
14 vor dem Einfügen der Befestigungseinrichtung24 aufgebracht werden. Wenn die Befestigungseinrichtung24 am Wirbel V1 befestigt wird, wird die Zugkraft nachgelassen. Die Befestigungseinrichtung26 kann ebenso durch die Bohrung22 eines gespannten unteren Flanschelements16 eingesetzt werden. Das vorgespannte obere Flanschelement14 und das vorgespannte untere Flanschelement16 bringen folglich eine Druckbelastung auf den Körperabschnitt12 im Scheibenzwischenraum auf, was die Verschmelzung und Integration des Implantats10 weiter fördert und das Ausbreiten des Implantats10 aus dem Scheibenzwischenraum verhindert. - Mit Bezug nun auf
3 ist ein Implantat einer weiteren Ausführungsform gezeigt und mit50 bezeichnet. Das Implantat50 ist zum Implantat10 im Wesentlichen identisch. - Das Implantat
50 umfasst einen starren Körperabschnitt52 mit einem flexiblen oberen Flanschelement54 und einem flexiblen unteren Flanschelement56 , die sich davon erstrecken. Eine erste Befestigungseinrichtungsbohrung60 ist durch das obere Flanschelement54 ausgebildet und eine zweite Befestigungseinrichtungsbohrung62 ist durch das untere Flanschelement56 ausgebildet. Der Körperabschnitt52 umfasst einen Hohlraum58 , in dem Knochenwachstumsmaterial64 angeordnet wird. - Der Körperabschnitt
52 umfasst ferner eine Anzahl von oberen Knocheneingriffsleisten68 , die an der oberen Lageroberfläche66 ausgebildet sind und sich von dieser nach oben erstrecken, wobei ein identischer Satz von unteren Leisten72 an der unteren Lageroberfläche70 ausgebildet ist und sich von dieser nach unten erstreckt. Es ist selbstverständlich, dass, obwohl Leisten in der dargestellten Ausführungsform gezeigt wurden, es in Erwägung gezogen wird, dass eine Vielfalt von Strukturen vorhanden ist, die eine Oberfläche zum wirksamen Eingriff mit den Wirbelkörpern schaffen könnten, um das Austreiben aus dem Scheibenzwischenraum zu begrenzen. Beispiele von einigen solchen weiteren Strukturen sind imUS-Patent Nr. 5 989 289 erörtert. Ferner können die Endplatten oder Lageroberflächen der benachbarten Knochenstruktur aufgeraut oder anderweitig geformt werden, um den Körperabschnitt52 in seiner eingesetzten Position festzuhalten. - Mit Bezug nun auf
4 ist ein Implantat80 einer weiteren Ausführungsform zur Verwendung in Wirbelverschmelzungseingriffen gezeigt, das spezielle Anwendung in einem hinteren Zugang zum Scheibenzwischenraum hat, obwohl das Implantat80 in anderen Zugängen verwendet werden kann, einschließlich vorderer und seitlicher Zugänge. Das Implantat80 besitzt einen starren Körperabschnitt82 mit einem oberen Flanschelement84 und einem unteren Flanschelement86 , die sich jeweils vom starren Flanschelement82 an seinem Hinterende erstrecken. Das Implantat80 weist keinen Hohlraum auf und kann daher eine Breite besitzen, die geringer ist als die Breite der Implantate10 und50 . Der Zugang zum Scheibenzwischenraum zwischen benachbarten Wirbeln wird erreicht, wie auf dem Fachgebiet bekannt. Beispiele solcher Verfahren und von hinteren Knochenimplantaten sind in derPCT-Veröffentlichung Nr. WO 00/24327 - Sobald ein Zugang erreicht ist, wird der Scheibenzwischenraum gedehnt, falls erforderlich. Das Implantat
80 wird in den Scheibenzwischenraum bewegt, wobei der Körperabschnitt82 zwischen den benachbarten Wirbeln angeordnet wird und das obere Flanschelement84 und das untere Flanschelement86 benachbart zu den Wirbelkörpern außerhalb des Scheibenzwischenraums angeordnet wird. Sobald der Körperabschnitt82 im Scheibenzwischenraum D befestigt ist, können Befestigungseinrichtungen verwendet werden, um die Flanschelemente an dem jeweiligen benachbarten Wirbelkörper zu befestigen. Es ist selbstverständlich, dass ein zweites Implantat in dem zum ersten eingefügten Implantat benachbarten Scheibenzwischenraum angeordnet werden kann, um weitere Stabilität zu schaffen. - Obwohl nicht dargestellt, können die Implantate der vorliegenden Erfindung einen Schlitz oder eine Gewindebohrung für den Eingriff mit einem Antriebswerkzeug aufweisen, das dazu ausgelegt ist, das Implantat im Scheibenzwischenraum zu positionieren und in diesen zu schieben.
- Der Knochen für die Implantate der vorliegenden Erfindung wird vorzugsweise aus einem des Oberschenkels, des Schienbeins, der Wadenbeinspeiche oder der Elle oder einem anderen Knochensegment mit der erforderlichen Rin denknochenfestigkeit ausgewählt. Ferner wird in Erwägung gezogen, dass das Implantat
10 ein Knochen eines Eigentransplantats, eines allogenen Transplantats oder xenogenen Transplantats sein kann, wobei der Knochen wie auf dem Fachgebiet bekannt für die anschließende Implantation in den Empfänger behandelt wird. Insbesondere kann das Knochenimplantat aus einem Spenderknochen mit ausreichender Beständigkeit gegen Druck zwischen den oberen und unteren Oberflächen ausgewählt werden, so dass er in der vorgesehenen Umgebung Anwendung findet. - Die Erzeugung des entmineralisierten Abschnitts des Knochens wird nun beschrieben. Die Bearbeitung beinhaltet die Verwendung von Spenderknochen mit der Bearbeitung in einer Reinraumumgebung innerhalb einer Knochenbearbeitungseinrichtung. Ein solcher Spenderknochen kann ein allogenes Transplantat von menschlichen Quellen oder ein xenogenes Transplantat von tierischen Quellen umfassen. Ferner wird in Erwägung gezogen, dass, wenn die Technologie auf dem Gebiet der Knochenbearbeitung fortschreitet, der Spenderknochen im Herstellungsprozess entweder durch Knochenwachstum oder durch eine Bearbeitung von Bestandteilskomponenten von Knochen erzeugt werden kann, um künstliche Materialien mit Eigenschaften, die einem Knochen sehr ähnlich sind, zu erzeugen. Obwohl irgendein verfügbarer allogener oder xenogener Knochenrohstoff für die Prozedur verwendet werden kann, ist insbesondere Rindenknochen für die Wirbelverschmelzung wegen seiner strukturellen Eigenschaften herkömmlich bevorzugt, obwohl ein Rindenschwamm- oder Schwammknochen in Abhängigkeit von den speziellen Anforderungen für das Implantat verwendet werden kann.
- Bei der weiteren Bearbeitung werden die Verbindungsgewebe entfernt und der Knochen wird unter Verwendung eines Lösungsmittels wie z. B. Ethanol oder Wasserstoffperoxid gereinigt, gespült und entfettet. Der Knochen wird dann unter Verwendung von herkömmlichen Verfahren maschinell bearbeitet oder anderweitig geformt, um seine Endform zu erzeugen. Das obere und das untere Flanschelement und, falls erforderlich, der Körperabschnitt werden entmineralisiert, um die erforderliche flexible Fähigkeit zu erzeugen. Die Eindringung des Entmineralisierungsfluids in den Knochen benachbart zum gewünschten Flexibilitätsbereich kann durch einen hydrostatischen Druck gesteuert werden, wodurch der Entmineralisierungsbereich begrenzt wird. Die Menge an Mineral, das aus dem Knochen entfernt wird, kann eingestellt werden, um die gewünschte Menge an Flexibilität zu erzeugen. Diese Entmineralisierung verwendet herkömmlich eine organische Säure wie z. B. Chlorwasserstoff-, Salpeter- oder Zitronensäure. Vorzugsweise umfasst die Entmineralisierungslösung 0,1 bis 1,0 N HCl, am meisten bevorzugt 0,3 N HCl. Wenn ein xenogenes Transplantat verwendet wird, können bekannte Verfahren für die Verwendung von organischen Lösungsmitteln, um Knochenproteine zu inaktivieren und die Antigenität zu verringern, an diesem Punkt angewendet werden. Außerdem kann die Verwendung von Glutaraldehyd stattfinden, um die Collagenstruktur nach der Entfernung des Mineralteils weiter zu vernetzen. Sobald das Implantat maschinell bearbeitet und teilweise entmineralisiert wurde, kann es vor dem Einsetzen gelagert werden.
Claims (17)
- Implantat, mit: einem Körperabschnitt (
12 ), der in dem Scheibenzwischenraum zwischen einem oberen Wirbel und einem benachbarten unteren Wirbel positionierbar ist; einem oberen Element (14 ), das sich von dem Körperabschnitt erstreckt; und einem unteren Element (16 ), das sich von dem Körperabschnitt erstreckt, wobei der Körperabschnitt, das obere Element und das untere Element jeweils aus Knochenmaterial hergestellt sind, dadurch gekennzeichnet, dass das obere Element so konfiguriert ist, dass es sich längs des Körpers des oberen Wirbels erstreckt, und das untere Element so konfiguriert ist, dass es sich längs des Körpers des unteren Wirbels erstreckt. - Implantat nach Anspruch 1, wobei das obere Element und das untere Element jeweils flexibel sind, um eine Bewegung des oberen Elements und des unteren Elements in Bezug auf den Körperabschnitt zu ermöglichen.
- Implantat nach Anspruch 2, wobei das obere Element und das untere Element aus einem wenigstens teilweise entmineralisierten Knochen hergestellt sind.
- Implantat nach Anspruch 2, wobei das obere Element und das untere Element aus einem vollständig entmineralisierten Knochen hergestellt sind.
- Implantat nach Anspruch 1, wobei der Körperabschnitt eine obere Lageroberfläche und eine untere Lageroberfläche aufweist, die durch eine Höhe getrennt sind, wobei die Höhe so bemessen ist, dass sie den Zwischenraum zwischen den benachbarten Wirbeln aufrechterhält.
- Implantat nach Anspruch 5, wobei sowohl die obere als auch die untere Lageroberfläche eine Knocheneingriffoberfläche aufweist, um ein Austreiben bzw. einen Ausstoß des Implantats aus dem Scheibenzwischenraum zu verhindern.
- Implantat nach Anspruch 1, wobei der Körperabschnitt eine Wirbelverschmelzungsvorrichtung bzw. -fusionsvorrichtung ist und der Körperabschnitt so beschaffen ist, dass er einen gewünschten Zwischenraum zwischen den benachbarten Wirbeln aufrechterhält.
- Implantat nach Anspruch 7, wobei das obere Element und das untere Element jeweils eine durch sie verlaufende Öffnung (
20 ,22 ) besitzen, um eine Befestigungseinrichtung aufzunehmen, um das obere Element und das untere Element an den Körpern des oberen bzw. des unteren Wirbels zu befestigen. - Implantat nach Anspruch 1, wobei der Körperabschnitt, das obere Element und das untere Element aus einem einzigen Knochensegment gebildet sind.
- Implantat nach Anspruch 1, wobei der Körperabschnitt einen Hohlraum (
18 ) besitzt, der ein Knochenwachstum zwischen dem oberen und dem unteren Wirbel zulässt. - Verfahren zum Präparieren bzw. Herstellen eines Knochenimplantats, das umfasst: Vorsehen eines starren Knochensegments, das einen Körperabschnitt (
12 ) mit einer oberen Lageroberfläche und einer gegenüberliegenden unteren Lageroberfläche besitzt, wobei das starre Knochensegment des Weiteren ein oberes Flanschelement (14 ) und ein gegenüberliegendes unteres Flanschelement (16 ) aufweist, die sich jeweils von dem Körperabschnitt erstrecken; und wenigstens teilweises Entmineralisieren des oberen und des unteren Flanschelements, um ein flexibles oberes Flanschelement und ein flexibles unteres Flanschelement zu schaffen, die sich von dem starren Körperabschnitt erstrecken, wobei das obere und das untere Flanschelement so konfiguriert sind, dass sie sich längs eines oberen Wirbelkörpers bzw. eines unteren Wirbelkörpers erstrecken, wenn der Körperabschnitt in einem Zwischenraum zwischen dem oberen Wirbelkörper und dem unteren Wirbelkörper positioniert ist. - Verfahren nach Anspruch 11, bei dem das wenigstens teilweise Entmineralisieren das Beaufschlagen des starren oberen Flanschelements und des starren unteren Flanschelements mit einem Entmineralisierungsfluid umfasst.
- Verfahren nach Anspruch 11, das des Weiteren das Begrenzen des Kontakts des Körperabschnitts mit dem Entmineralisierungsfluid umfasst.
- Verfahren nach Anspruch 13, bei dem das Begrenzen den hydrostatischen Druck nutzt, um die Bewegung des Entmineralisierungsfluids in dem Körperabschnitt zu begrenzen.
- Verfahren nach Anspruch 11, das des Weiteren das Bilden einer Knocheneingriffoberfläche sowohl an der oberen als auch an der unteren Lageroberfläche des Implantats umfasst.
- Verfahren nach Anspruch 15, wobei die Knocheneingriffoberfläche so konfiguriert ist, dass sie eine Bewegung des Implantats verhindert.
- Verfahren nach Anspruch 11, wobei das Implantat aus einem einzigen Knochensegment gebildet ist.
