Description
- Die vorliegende Erfindung bezieht sich auf einen hochfesten chirurgischen Nahtfaden und insbesondere auf geflochtene Nahtfadenmischungen aus Polyethylen mit ultrahohem Molekulargewicht mit hoher Festigkeit und herausragenden Festzurreigenschaften.
- Die Nahtfadenfestigkeit ist ein wichtiger Punkt bei chirurgischem Nahtmaterial aller Art. Eines der festesten Materialien, das gegenwärtig in die Form von ausgezogenen Strängen gebracht werden kann, ist ein langkettiges Polyethylen mit ultrahohem Molekulargewicht, das typischerweise als Angelleine und ähnliches eingesetzt wird, und unter den Handelsbezeichnungen Dyneema oder Spectra erhältlich ist. Dieses Material ist viel fester als normaler chirurgischer Nahtfaden, besitzt aber keine annehmbaren Knotenfestzurreigenschaften, um bei Anwendungen in der Chirurgie eingesetzt werden zu können.
- In EP-A-0561108 ist ein Nahtfaden offenbart, der aus einem Kern bestehend aus einer Polyethylenfaser und einem Mantel aus geflochtenen Polyesterfasern gebildet wird.
- Die vorliegende Erfindung legt einen Nahtfadenstrang vor, der zur Verwendung als ein Nahtfaden oder Ligaturfaden geeignet ist und einen Kern aus verzwirnten Polyethylenfasern mit ultrahohem Molekulargewicht und einen Mantel, der von einer Vielzahl von geflochtenen Fasern gebildet wird, umfasst, wobei die Vielzahl von geflochtenen Fasern Polyethylenfasern mit ultrahohem Molekulargewicht umfasst, die aus mindestens einem von mindestens einer langkettigen, synthetischen Polymerfaser oder mindestens einer bioresorbierbaren Faser geflochten sind.
- In vorteilhafter Weise legt die vorliegende Erfindung einen hochfesten chirurgischen Nahtfaden mit verbesserten Festzurreigenschaften vor. Bei einer Ausführungsform besitzt der Nahtfaden einen geflochtenen Mantel aus einer langkettigen Polyethylenfaser mit ultrahohem Molekulargewicht, die mit einer Faser aus einem oder mehreren langkettigen, synthetischen Polymeren, vorzugsweise Polyester, geflochten ist. Die Festigkeit wird von dem Polyethylen geboten. Die verbesserten Festzurreigenschaften werden von dem Polyester geboten.
- Außerdem werden die Handhabungseigenschaften des hochfesten Nahtfadens durch die Verwendung von verschiedenen Materialien zur Beschichtung des Nahtfadens verbessert. Darüber hinaus können als Option Stränge mit einer Kontrastfarbe zu den geflochtenen Strängen hinzugefügt werden, um deren Erkennbarkeit zu verbessern. Bei den gefärbten Strängen handelt es sich vorzugsweise um gefärbte Filamente aus Polyester oder Nylon.
- Bei einer bevorzugten Ausführungsform beinhaltet der Nahtfaden einen Mantel aus mehreren Filamenten, der durch Flechten von Polyethylenfasern mit ultrahohem Molekulargewicht mit Polyester- und Nylonfasern gebildet wird. Der Mantel umgibt einen Faserkern, der zu einem beträchtlichen Teil oder ganz aus Polyethylen mit ultrahohem Molekulargewicht besteht. Vorzugsweise umfasst der Kern 3 Stränge Polyethylen mit ultrahohem Molekulargewicht, die mit ungefähr 3 bis 6 Zwirndrehungen pro Zoll verzwirnt sind.
- Der Mantel umfasst vorzugsweise 8 Stränge Polyethylen mit ultrahohem Molekulargewicht, die mit 6 Strängen Polyester und 2 Strängen Nylon geflochten sind, wobei die Nylonstränge in der Farbe Schwarz oder einer sonstigen Kontrastfarbe geboten wird, wie nachstehend ausführlicher erklärt wird.
- Geeignete Polyethylenfasern mit ultrahohem Molekulargewicht zur Verwendung in erfindungsgemäßen Ausführungsformen werden unter dem Warenzeichen Dyneema von der Fa. Toyo Boseki Kabushiki Kaisha vermarktet und in den USA von der Fa. Honeywell unter dem Warenzeichen Spectra hergestellt.
- Der Nahtfaden einer erfindungsgemäßen Ausführungsform besitzt in vorteilhafter Weise die selbe Festigkeit wie Ethibond Nr. 5-Nahtfaden, weist aber den Durchmesser und die Festzurreigenschaften von Ethibond Nr. 2 auf fühlt sich auch wie Ethibond Nr. 2 an. Folglich ist der Nahtfaden einer erfindungsgemäßen Ausführungsform ideal geeignet für die meisten Verfahren der Orthopädie, wie Reparatur der Drehmuskelmanschette, Reparatur der Achillessehne, Reparatur der Patellasehne, Rekonstruktion des/der vorderen und hinteren Kreuzbandes/Kreuzbänder, Rekonstruktionsverfahren für Schulter und Hüfte und als Ersatz von Nahtfäden, die in oder mit Nahtfadenverankerungen verwendet werden. Der Nahtfaden lässt sich gebrauchsfertig, an einen Nahtfadenverankerung befestigt verpacken.
- Der Nahtfaden kann unbeschichtet oder mit Wachs (Bienenwachs, Mineralölparaffin, Polyethylenwachs oder sonstige), Silikon (Dow Corning Silikonflüssigkeit 202A oder sonstige), Silikongummis (Nusil Med 2245, Nusil Med 2174 mit einem Haftkatalysator oder sonstige), PTFE (Teflon, Hostaflon, oder sonstige), PBA (Polybutylatsäure), Ethylzellulose (Filodel) oder sonstigen Überzügen beschichtet sein, um beispielsweise die Schlüpfrigkeit des Zwirns, die Knotensicherheit oder die Scheuerfestigkeit zu verbessern. Wie eingangs erwähnt, sind alle oder einige der Nylonfasern (oder der Polyesterfasern) im Mantel in vorteilhafter Weise zur Erkennbarkeit und Identifikation in einer Kontrastfarbe vorgelegt. Einige Beilauffäden, vorzugsweise eine leicht färbbare Faser wie Polyester oder Nylon, im Mantel in einer Kontrastfarbe vorzusehen, dient dem Operateur als Hilfe bei der Erkennung der Bewegungsrichtung des Nahtfadens bei der Operation, insbesondere bei arthroskopischen Operationen. Die Beilauffäden in einem sich regelmäßig wiederholenden Muster vorzusehen, ist besonders nützlich und ermöglicht dem Operateur die Unterscheidung von verschiedenen Enden von Nahtfadenstücken und die Ermittlung der Bewegungsrichtung eines sich bewegenden Nahtfadenstücks. Unter den leichter färbbaren Fäden ist Nylon bevorzugt, da es leicht Farbstoff aufnimmt. Polyesterfasern sind fester, nehmen aber Farbstoff weniger leicht auf als Nylon.
- Im folgenden werden erfindungsgemäße Ausführungsformen unter Bezugnahme auf die Begleitzeichnungen beschrieben, wobei in den Zeichnungen:

1 ist eine rasterelektronenmikroskopische Aufnahme eines Stücks Nahtfaden2 gemäß einer erfindungsgemäßen Ausführungsform veranschaulicht. Der Nahtfaden2 besteht aus einem Mantel4 und einem von dem Mantel umgebenen Kern6 . Siehe2 . Zur Bildung des Mantels4 sind Stränge aus Polyethylen mit ultrahohem Molekulargewicht (UHMW-PE)8 , das unter den Warenbezeichnungen Spectra oder Dyneema erhältlich ist, und Stränge aus Polyester10 und12 verflochten. Der Kern6 wird aus verzwirnten Strängen aus UHMW-PE gebildet. - UHMW-PE-Stränge
8 sind zu einem beträchtlichen Teil durchsichtig oder farblos. In der Mehrzahl sind die Polyesterstränge10 weiß (im ungefärbten Zustand), wobei ein oder mehrere zusätzliche Polyester- oder Nylonstränge12 eine Kontrastfarbe aufweisen, um als Beilauffaden im Nahtfaden zu dienen. Aufgrund der Durchsichtigkeit von UHMW-PE nimmt der Nahtfaden die Farbe der Stränge10 und12 an und erscheint daher weiß mit einem Beilauffaden mit der Kontrastfarbe. Gemäß einer erfindungsgemäßen Ausführungsform werden die Beilauffadenstränge12 vorzugsweise in schwarz vorgelegt. Der schwarze Beilauffaden hilft dem Operateur bei der Unterscheidung zwischen Nahtfadensträngen mit dem Beilauffaden und Nahtfadensträngen ohne den Beilauffaden. Darüber hinaus hilft der Beilauffaden dem Operateur zu erkennen, ob der Nahtfaden sich bewegt. - Einzelheiten erfindungsgemäßer Ausführungsformen sind nachfolgend in Verbindung mit den folgenden Beispielen beschrieben:
- Beispiel 1: USP-Größe 5 (EP-Größe 7) – ohne Nylon-Beilauffaden
- Bei den auf einer "16-Spulenträger-Flechtmaschine" von Hobourns hergestellten Garnen, die für den geflochtenen Mantel verwendet werden, handelt es sich um Honeywell Spectra 2000, Polyester Typ 712. Der Mantel wird unter Einsatz von acht Spulenträgern (Strängen) Polyester Typ 712 und Spectra gebildet. Der Mantel wird unter Einsatz von acht Spulenträgern (Strängen) mit einem Ende 190 Decitex-Polyester pro Spulenträger und acht Spulenträgern mit einem Ende 220 Decitex-Polyester pro Spulenträger gebildet. Der Kern wird aus Spectra unter Einsatz von einem Ende 440/1/3/gezwirnt 10 tpi "z" und 7 tpi "s" gebildet (der Kern ist nicht dampffixiert). 36 Schussfäden pro Zoll (ppi). Der Prozentanteil des Mantels bei der Bildung des Nahtfadens beträgt 71,31, während der Prozentanteil des Kerns 28,69 (Angaben in Gewichtsprozent) ausmacht. Die Lauflänge beträgt 1991 Meter pro Kilo.
- Von dem Gesamtnahtfaden macht das Polyester im Mantel (8 Spulenträger × 190 d'tex = 1520 d'tex) 33% des Nahtfadens aus und das Spectra im Mantel (8 Spulenträger × 220 d'tex = 1760 d'tex) macht 38,76% des Nahtfadens aus. Der Spectra-Kern (3 Spulenträger × 440 d'tex = 1320 d'tex) macht 28,69% des Nahtfadens aus.
- Beispiel 2: USP-Größe 5 (EP-Größe 7) – mit Nylon-Beilauffaden
- Bei den auf einer "16-Spulenträger-Flechtmaschine" von Hobourns hergestellten Garnen, die für den geflochtenen Mantel verwendet werden, handelt es sich um Honeywell Spectra 2000, Polyester Typ 712 und Nylon. Der Mantel wird unter Einsatz von acht Strängen 144 Decitex-Spectra pro Spulenträger, geflochten mit sechs Strängen 100 Decitex-Polyester und zwei Strängen gefärbtes Nylon gebildet. Der Kern wird aus drei Spulenträgern 144 Decitex-Spectra geflochten mit drei bis sechs Zwirndrehungen pro Zoll gebildet.
- Beispiel 3: USP-Größe 2
- Der Nahtfaden besteht aus 38,09% Polyester, 61,91% UHMW-PE oder ungefähr 40% Polyester und ungefähr 60% UHMW-PE (Angaben in Gewichtsprozent).
- Die vorstehend aufgeführten Beispiele beziehen sich auf Nahtfäden der Größen 2 und 5. Bei der Herstellung von verschiedenen Größen des erfindungsgemäßen Nahtfadens können unterschiedliche Decitex-Werte und unterschiedliche PPI-Einstellungen verwendet werden, um die erforderliche Größe und Festigkeit zu erreichen. Darüber hinaus kann bei kleineren Größen beispielsweise die Verwendung einer 12-Spulenträger-Flechtmaschine notwendig sein. Die allerkleinsten Größen werden vorzugsweise ohne Kern hergestellt. Insgesamt kann der Nahtfaden zwischen 5% und 90% (Gewichtsprozent) Polymer mit ultrahohem Molekulargewicht (vorzugsweise mindestens 40% Polymer mit ultrahohem Molekulargewicht) enthalten, wobei Polyester und/oder Nylon den Restanteil ausmachen. Der Kern macht vorzugsweise 18% oder mehr der Gesamtmenge (Gewicht) der Filamente aus.
- Der Nahtfaden ist vorzugsweise mit Wachs (Bienenwachs, Mineralölparaffin, Polyethylenwachs oder sonstige), Silikon (Dow Corning Silikonflüssigkeit 202A oder sonstige), Silikongummis (Nusil Med 2245, Nusil Med 2174 mit einem Haftkatalysator oder sonstige), PTFE (Teflon, Hostaflon, oder sonstige), PBA (Polybutylatsäure), Ethylzellulose (Filodel) oder sonstigen Überzügen beschichtet, um beispielsweise die Schlüpfrigkeit des Zwirns, die Knotensicherheit oder die Scheuerfestigkeit zu verbessern.
- Bei einer erfindungsgemäßen Ausführungsform bietet die Polymerkomponente mit ultrahohem Molekulargewicht (UHMW) die Festigkeit und die Polyesterkomponente bietet verbesserte Knotbarkeit und Festzurreigenschaften. Es wurde jedoch festgestellt, dass das UHMW-Polymer einen unerwarteten Vorteil bietet, indem es als Polster für die Polyesterfasern wirkt, die relativ hart sind und zur gegenseitigen Beschädigung neigen. Das UHMW-Polymer beugt Bruchbildungen vor, indem es die Beschädigung des Polyesters bei Beanspruchung des Nahtfadens reduziert.
- Gemäß einer alternativen, erfindungsgemäßen Ausführungsform wird ein zum Teil bioresorbierbarer Nahtfaden geboten, indem ein hochfestes Material, wie UHMW-PE-Fasern, mit einem bioresorbierbaren Material, beispielsweise PLLA oder eines der anderen Polylaktide, verflochten wird. Dementsprechend würde ein Nahtfaden mit einem Spectra- oder Dyneemaanteil von ungefähr 10%, der mit resorbierbaren Fasern geflochten ist, eine größere Festigkeit als bestehende bioresorbierbare Nahtfäden mit einem geringeren Dehnvermögen bieten. Im Lauf der Zeit würden 90% oder mehr des Nahtfadens resorbiert, so dass lediglich ein sehr kleiner Restanteil im Knoten verbleiben würde. Der bioresorbierbare Nahtfaden kann Überzüge und farbige Beilauffäden enthalten, wie vorstehend für nicht-resorbierbaren Nahtfaden erwähnt wurde.
- Bei einem Verfahren zur Anwendung des Nahtfadens bei einer erfindungsgemäßen Ausführungsform ist der Nahtfaden
2 an einem Nahtfadenanker14 befestigt, wie in3 dargestellt (steril abgepackt mit einer Einführhilfe16 ), oder an einem oder beiden Enden an einer halbrunden, konischen Nadel18 befestigt, wie in4A und4B dargestellt ist. - Wie vorstehend laut Beschreibung in Beispiel 1 ausgeführt wurde, können ein oder mehrere Stränge einer Faser im geflochtenen Mantel in vorgefärbten Farben, z.B. schwarz, vorgelegt werden, um als Beilauffaden zu dienen. Die Beilauffäden verbessern die visuelle Erkennbarkeit der Nahtfadenbewegung und die Fähigkeit zur Unterscheidung zwischen gefärbten und ungefärbten Nahtfadensträngen.
- Obwohl die vorliegende Erfindung im Hinblick auf bestimmte Ausführungsformen derselben beschrieben wurde, werden viele andere Variationen und Modifikationen und sonstige Anwendungen für den Fachmann offensichtlich sein. Daher wird die vorliegende Erfindung vorzugsweise nicht durch die speziellen Offenbarungen in diesem Schriftstück eingeschränkt, sondern lediglich durch die beigefügten Ansprüche.
Claims (10)
- Ein Nahtfadenstrang, der zur Verwendung als ein Nahtfaden oder Ligaturfaden geeignet ist und einen Kern (
6 ) aus verzwirnten Polyethylenfasern mit ultrahohem Molekulargewicht und einen Mantel (4 ), der von einer Vielzahl von geflochtenen Fasern gebildet wird, umfasst, wobei die Vielzahl von geflochtenen Fasern Polyethylenfasern mit ultrahohem Molekulargewicht umfasst, die aus mindestens einem von mindestens einer langkettigen, synthetischen Polymerfaser oder mindestens einer bioresorbierbaren Faser geflochten sind. - Der Nahtfadenstrang nach Anspruch 1, wobei mindestens eine der mindestens einen langkettigen, synthetischen Polymerfasern in einer Kontrastfarbe im Vergleich zu den anderen Fasern vorgelegt wird, um einen identifizierbaren Beilauffaden vorzulegen.
- Der Nahtfadenstrang nach Anspruch 1 oder Anspruch 2, wobei mindestens eine langkettige, synthetische Polymerfaser Polyester, Nylon oder beides ist.
- Der Nahtfadenstrang nach einem der vorhergehenden Ansprüche, wobei die Polyethylenfasern mit ultrahohem Molekulargewicht einen Anteil von mindestens 40% der geflochtenen Fasern ausmachen.
- Der Nahtfadenstrang nach einem der vorhergehenden Ansprüche, wobei der Kern (
6 ) mindestens 18% oder mehr der gewichtsmäßigen Gesamtmenge der Nahtfadenstränge (2 ) ausmacht. - Der Nahtfadenstrang nach einem der vorhergehenden Ansprüche, der darüber hinaus einen auf dem Mantel (
4 ) aufgebrachten Überzug umfasst. - Der Nahtfadenstrang nach Anspruch 6, wobei der Überzug aus der Gruppe bestehend aus Wachs, Silikon, Silikongummis, PTFE, PBA und Ethylzellulose ausgewählt ist.
- Der Nahtfadenstrang nach einem der vorhergehenden Ansprüche, der darüber hinaus einen an dem Nahtfadenstrang befestigten Nahtfadenanker (
14 ) umfasst. - Der Nahtfadenstrang nach einem der Ansprüche 1 bis 7, der darüber hinaus eine an einem oder beiden Enden des Nahtfadens befestigte halbrunde, konische Nadel (
18 ) umfasst. - Der Nahtfadenstrang nach einem der vorhergehenden Ansprüche, wobei die mindestens eine langkettige Polymerfasern mindestens eine nicht resorbierbare Polyesterfaser umfassen.
