Description
- Gebiet der Erfindung
- Die Erfindung betrifft allgemein Stents und insbesondere einen endovaskulären Stent zur Medikamentenabgabe, der ein spezifisches Immunsuppressivum zur Stentbehandlungsstelle abgibt.
- Hintergrund der Erfindung
- Stents finden breiten Einsatz bei Eingriffen in Koronararterien. Nach einer Angioplastie oder anderen Intervention kann ein Stent in die Behandlungsstelle eingeführt werden, um die Wand der Arterie abzustützen.
- Das Hauptproblem beim Stentgebrauch ist ein als Restenose bezeichneter Wiederverschlussvorgang. Das Restenoseproblem ist weithin anerkannt. Aus Forschungsarbeiten geht hervor, dass sich die Restenosemechanismen nach einem Ballonverfahren im Detail von den Heilungsprozessen im Zusammenhang mit der Stentplazierung unterscheiden.
- Die mit dem Stentgebrauch zusammenhangenden biologischen Reaktionen bewirken eine Kaskade von Zellwachstum und -proliferation. Die mechanische Wirkung des Stents an der Arterienwand (Rückfedern) und das Einführen des Fremdstoffs in den Körper fuhren zu einer Entzundungsreaktion, durch die Zytokine genannte Signalmolekule entstehen, die vielfaltige biologische Vorgange vermitteln. Obwohl Größe und Verlauf der Entzundungsreaktion zwischen Patienten stark variieren, isoliert der Korper das Fremdmaterial des Stents, indem er ihn mit Zellwachstum einkapselt. Dadurch wird eine Pseudo-Intima auf der Oberflache des Stents erzeugt. Im allgemeinen ist die Vermehrung eines glatten Muskelgewebes von der Pseudo-Intima erwünscht, allerdings resultiert bei einigen Patienten die Proliferation glatter Muskelzellen und ihre Umwandlung in sekretorische Muskelzellen in kurzer Zeit im Wiederverschluss des Gefäßes. Obwohl dies eine normale Reaktion auf das Einsetzen eines Fremdkörpers ist, führt es angesichts der Lage des Stents zu schweren klinischen Problemen. Zudem existieren auch andere kurzfristige Komplikationen, u. a. akute Thrombose. Vorgeschlagen wurde die Abgabe von antithrombozytischen Medikamenten und anderen thrombolytischen Praparaten, um diese kurzfristige Art von Wiederverschluss zu behandeln.
- Mehrere Bemuhungen wurden unternommen, den langerfristigen Restenosevorgang zu unterbinden oder zu verzögern. Ein Weg bestand darin, radioaktive Stents zu implantieren, wobei die lokale Emission von Betastrahlung Hyperplasie hemmt. Obwohl intrakoranare Strahlung Restenose effektiv verhindert, stört diese stark das zuvor beschriebene Heilungsverfahren und kann zu Sekundarkomplikationen führen, z. B. Kantenrestenose und Spätthrombose. Aus der
US-A-5871437 (Alt) ist ein Beispiel für diese Stenttechnologie bekannt. Diese Literatur lehrt den Gebrauch einer Mehrfachbeschichtung auf dem Stentsubstrat. Eine Beschichtung tragt einen Betastrahler, während andere Beschichtungen ein gerinnungshemmendes Medikament abgeben. - Ein weiterer Weg zur Therapie akuter Thrombose in Stentbehandlungen ist in der
US-A-5788979 (Alt et al.) diskutiert. Der Stent gemäß dieser Erfindung verwendet eine biologisch abbaubare Beschichtung, um eine gesteuerte Menge eines therapeutischen Mittels freizusetzen, z. B. eines gerinnungshemmenden, antithrombogenen oder antistenotischen Medikaments. Die biologisch abbaubare Beschichtung sorgt für eine lokale Medikamentenfreisetzung, was die Biokompatibilitat des Stents verbessert und Entzündung sowie die Hyperplasievorgänge reduziert. Die Aufgabe dieser anderen Verfahren ist, die Proliferation der glatten Muskelzellen zu stören und einzudämmen. - Obwohl die verschiedenen beschichteten Stents die Restenoseraten für einige Patienten verbessern, bleibt ein voll biokompatibler Stent ein schwer zu fassendes Problem, da die Faktoren der lokalen Thrombusbildung, Gefäßverletzung und Entzündung auf komplexe und individuell variable Weise interagieren. Aus diesen Gründen sind Reokklusions- und Restenoseprobleme klinisch schwer zu handhaben. Für Patienten bleibt Restenose ein erhebliches Risiko.
- Eine Aufgabe der Erfindung besteht darin, einen Stent bereitzustellen, der Restenose hemmt und eine normale Wundheilung an der Behandlungsstelle garantiert.
- Erfindungsgemäß wird diese Aufgabe mit den Merkmalen von Anspruch 1 gelöst.
- Im Gegensatz zum Stand der Technik gibt der Stent der Erfindung eine wirksame Dosis des Immunsuppressivums Tacrolimus zur Stentbehandlungsstelle ab. Tacrolimus wird mit einer Rate und in einer Konzentration abgegeben, die beide die Proliferation glatter Muskelzellen vom kontraktilen Typ fördern; und obwohl diese Zellen für den restenotischen Prozeß verantwortlich sind, sind glatte Muskelzellen vom kontraktilen Typ zur regulären und schnellen Wundheilung unabdingbar. Dies ist der Hauptunterschied im Vergleich zu anderen Zytostatika und anderen Immunsuppressiva, z. B. Sirolimus, da Tacrolimus die Proliferation von Zellen nur geringfügig hemmt, aber die Umwandlung solcher Zellen in Muskelzellen vom sekretorischen Typ begrenzt. Dieses Verfahren und Herangehen unterscheiden sich von bekannten zellwachstumshemmenden Techniken, in denen Taxol und verwandte Medikamente in einer Gesamtstrategie zum Einsatz kommen, um die Heilungsreaktion zu stören und zu verzögern.
- Erfindungsgemäß gibt ein Stent Tacrolimus zu den Zellen ab, die auf der Oberfläche des Stents proliferieren. Der Stent bildet eine Primärstruktur, und die Beschichtung hat sekundären Prozesscharakter. Allgemein ist bevorzugt, eine Polymerschicht zu verwenden, wenngleich eine Oberflächenmodifikation des Stants selbst, um eine Medikamentenabgabefläche zu erzeugen, möglich, aber nicht bevorzugt ist.
- Die bevorzugte Vorrichtung gibt das Medikament durch Elution aus einer Polymermatrix ab, die als Beschichtung auf den Stent aufgetragen ist. Die Polymermatrix kann permanent und nicht biologisch abbaubar oder kann biologisch abbaubar sein. Ein Beispiel für ein geeignetes biologisch abbaubares Material ist Polymilchsäure (PLA). Zu Beispielen für permanentere Matrixmaterialien zahlen Polyethylen, Vinylacetat und medizinische Polyurethane sowie Silikonkautschuk. Im Schutzumfang der Erfindung sind auch andere biologisch abbaubare und nicht biologisch abbaubare Materialien erwogen. Hauptanforderung ist die Bildung einer biokompatiblen Matrix, um die Elution von Tacrolimus zu ermöglichen.
- Die lokalisierte und selektive Abgabe von Tacrolimus und Verbindungen enthaltendem Tacrolimus fördert die Endothelisierung des Stents mit glatten Muskelzellen und anderen Endothelzellen und unterbindet die Proliferation und Umwandlung solcher Zellen in sekretorische glatte Muskelzellentypen.
- Der Nutzeffekt von Tacrolimus und seiner Analoga ist in diesem Kontext unbekannt, und das Präparat ist nicht zum Gebrauch bei kardiovaskulären Interentionen indiziert oder gekennzeichnet.
- Obwohl arterielle endovaskuläre und speziell Koronarinterventionen eine wichtige Anwendung für die Erfindung sind, sollte deutlich sein, dass andere biomedizinische Vorrichtungen und Vorrichtungslagen, Größen und Medikamentenkonzentrationen im Schutzumfang der Erfindung erwogen sind. Ferner sollte klar sein, dass zusätzliche Merkmale und Strukturen der Erfindung zugefügt werden können, ohne vom Schutzumfang der Erfindung abzuweichen.
- Kurze Beschreibung der Zeichnungen
- In den Zeichnungen bezeichnen gleiche Bezugszahlen durchweg identische Strukturen. Es zeigen:
-
1 einen Stent bei seinem Weg zu einer Behandlungsstelle; und -
2 einen Stent mit der aufgetragenen bevorzugten Beschichtung. -
1 zeigt ein en Stent10 , der auf einen Angioplastieballon12 gecrimpt ist. Die Anordnung14 wird gerade zu einer Behandlungsstelle16 in einem arteriellen Gefäß16 geführt. Bei einem typischen Eingriff ist der Stent10 aus einem Metallmaschenmaterial hergestellt, und diese Primärstruktur wird auf einen Ballon mechanisch verformt, nachdem sie mit dem Medikament beschichtet wurde. Nach Einführen in die Behandlungsstelle16 wird der Stent10 entfaltet, indem er in die Gewebe an der Behandlungsstelle expandiert wird. Die Sekundärbeschichtung auf der Oberfläche der Primärstruktur steht in Kontakt mit dem Gewebe an der Behandlungsstelle, und der Stent wird einem kontinuierlichen Blutfluß an dieser Stelle ausgesetzt. -
2 zeigt einen Teilquerschnitt durch ein Segment des Stents10 . Das Metallsubstrat20 sorgt für die mechanischen Kennwerte für den Stent und ist die Primärstruktur, was in der Technik bekannt ist. Darstellungsgemäß ist der Stent10 eine Hohlröhre mit offenen Enden, die durch ein offenes Ende24 dargestellt sind, und einem Seitenwandsubstrat20 mit zahlreichen Öffnungen, die durch eine Öffnung26 dargestellt sind. - Vorzugsweise ist die Sekundärbeschichtung
22 ein biologisch abbaubares Polymer, z. B. PLA, das eine Konzentration des Medikaments Tacrolimus enthält. Der bevorzugte Weg besteht darin, Tacrolimus im ausgewählten Polymer zu lösen und den Stent durch Tauchen zu beschichten. In diesem bevorzugten Verfahren wird Tacrolimus in der Beschichtung gleichmäßig verteilt. Wichtig ist zu beachten, dass auch andere Wege zur Anwendung kommen können. Beispielsweise kann die Oberfläche des Stents modifiziert werden, um Porosität zu zeigen. Diese Matrix kann als Sekundärbeschichtung betrachtet werden, und sie kann mit Tacrolimus oder einer Tacrolimus enthaltenden Verbindung in einem weiteren Vorgang beladen oder gefühlt werden. - Beispiel
- Im folgenden Protokoll wird ein zur Veranschaulichung dienender vorgeschlagener erfindungsgemäßer Teststent näher dargestellt.
- Etwa 40 mg R203, eine Polymilchsäure mit einem Molekulargewicht von 30 kDa, werden in 0,6 ml Chloroform zusammen mit 10 mg Tacrolimus gelöst. Die resultierende Lesung ergibt einen gewichtsbezogenen Gehalt von 20% Tacrolimus in der Beschichtung. Möglich sind Gehalte zwischen 5 und 30 Gew.-%.
- Die Stents werden bei reduzierter Temperatur in mehreren Schritten durch Tauchen beschichtet; um eine etwa 10 um dicke Beschichtung auf der Stentoberfläche aufzutragen. Die Dicke der Beschichtung (oder Rauhigkeit der Stentoberfläche) hängt von den Maßen des Stents ab und kann z. B. für Stents größer sein, die in dicken Blutgefäßen implantiert werden.
- Danach werden die Stents auf einen Ballon gecrimpt. Bei Verwendung in einem Patient werden die Stents auf sinen maximalen Durchmesser von 4,5 mm in einem Gefäß eines Patienten expandiert. Die Expansion des Stents bringt die Polymermatrix; die als Sekundärbeschichtung wirkt; in Kontakt mit den Geweben an der Stelle der Stentplazierung. Zum Gebrauch vorgeschlagene Stents haben eine herkömmliche Gestaltung und sind im Handel von InFlow Dynamics zu beziehen (InFlow Starflex Stent Design).
- Erwarteter Wirkungsmechanismus und Wechselwirkung mit dem Körper
- Tacrolimus wurde 1984 im Kulturmedium einer Bakterie entdeckt, die in Japan im Boden festgestellt wurde. Die Bakterie wurde Streptomyces tsukubaensis genannt und zeigte interessante Eigenschaften. Zunächst nahm man an, dass sie zur Gruppe der Makrolid-Antibiotika wie Erythromycin gehört.
- Tacrolimus hat ein Molekulargewicht von 822 Da, es ist ein weißes kristallines Pulver, und es zeigt sowohl ein lipophiles als auch ein stark hydrophobes Verhalten, was in der Erfindung genutzt wird. Es ist in Chloroform, Ethylacetat, Aceton und Ethanol löslich und in Wasser praktisch unlöslich. Zu beziehen ist dieses Medikament von Fujisawa Inc. Japan.
- Die zugelassenen Indikationen für das Medikament variieren von Land zu Land. In den USA ist Tacrolimus derzeit zum Einsatz in der Prophylaxe von Organabstoßung bei Patienten zugelassen, die Leber- oder Nierentransplantate erhalten. In Großbritannien und Kanada ist Tacrolimus zur Primärtherapie und Notfalltherapie bei Transplantatabstoßung mit Resistenz gegen herkömmliche Immunsuppressiva indiziert, und mehrere europäische Länder haben das Präparat zur Herztransplantation zugelassen.
- Vom Anmelder wurde bei der Restenose beobachtet, dass die Infiltration von Lymphozyten, Makrophagen und Plasmazellen in der Matrixproduktion glatter Muskelzellen endet.
- Angenommen wird, dass die Produktion von Helfer-T-Zellen und die Produktion von Zytokinen wichtige Mechanismen bei der Immunantwort auf einen Fremdkörper sind. Wird die Stentoberfläche als Fremdkörper durch die CD4-identifizierbaren Helferzellen (TH1, TH2 erkannt, induziert dies eine T-Zellproliferation. Die Helferzellen produzieren verschiedene Zytokine, z. B. Interleukin-2, Interferon-γ (IFNy), die eine Aktivierung von Killer- und zytotoxischen T-Zellen sowie polynuklearen Granulozyten und Mastzellen bewirken. Die Mastzellen erzeugen selbst Interleukin-1 und -2, die die Proliferation der T-Zellen ebenfalls verstärken: Diese Reaktion führt zu zellularer Zytotoxizität und Antikörperbildung.
- Interferon-γ verstärkt die Expression interzellulärer Adhäsionsmoleküle (ICAM 1), was die Anhaftung von T-Zellen an die Endothelzellen erhöht. Außerdem bewirkt dies eine lokale Thrombusbildung an den Endothelzellen und erhöht die Endothelpermeabilitat.
- Es zeigt sich, daß Interleukin-8 die Anhaftung und Transepithelmigration von T-Zellen in den Neointima-Aufbau besonders fördert. Bei Transplantatabstoßungen geht eine Erhöhung der Produktion von Interleukin-8 der Abstoßung des Organs mehrere Tage vorher. Zusammengefaßt wird die Abstoßung eines Fremdkörpers primär vorangetrieben durch T-Lymphozyten, Monozyten, Makrophagen und Killerzellen, die durch vielfaltige Zytokine hochgesteuert werden, z. B. Interleukin-2, -4, -5, -6, -8 und -10, Interferon-γ und TNFα.
- Erkennt eine T-Zelle die antigene Fremdoberfläche, so induzieren bei Aktivierung Phospholipasen (PLC) die Erzeugung von Inositoltrisphosphat (IP3), einer primär kalziumabhängigen Signalübertragung. Calcineurin diphosphorylisiert den Kernfaktor aktivierter Lymphozyten im Zytoplasma (NF-ATC) und induziert seine Translokation in den Kern (NF-AT).
- Im Kern induziert dieser Komplex die Transkription von Interleukin-2, -3, -4, -5 und -8-Genen sowie die Transkription von TGFβ und von Tumor-Nekrose-Faktor α. Die Transkription der spezifischen Zytokingene in mRNA führt zur Produktion der jeweiligen Zytokine durch die T-Zelle.
- Tacrolimus hat eine spezifische Bindungsstelle im Zytoplasma. Dieses Bindungsprotein heißt FKBP-12. Durch die Bindung von Tacrolimus an diesen Rezeptor wird Calcineurin gebunden und die kalziumabhängige Signalübertragung gehemmt. Auf diese Weise wird die Translokation des zytoplasmatischen NF-ATC aus dem Zytoplasma in den Kern gehemmt und dadurch die Expression der o. g. Zytokine.
- TGFβ wird nicht nur durch T-Lymphozyten, sondern auch durch aktivierte Endothelzellen freigesetzt. Endothelzellen haben einen breiten Zweck und Wirkungsbereich. Neben der Produktion von Stickstoffmonoxid NO, das die Proliferation glatter Gefäßmuskelzellen hemmt; können Endothelzellen bei Stirnulierung, auch Wachstumsfaktoren zu ereugen, z. B. Insulin-ähnlichen Wachstumsfaktor (IGF-1), basischen Fibroblasten-Wachstumsfaktor (bFGF), Interleukin-6 und besonders transformierenden Wachstumsfaktor TGF-β. Steigt zusätzlich bei Stimulierung von Interleukin-4, Tumor-Nekrose-Faktor α und Interferon-γ die Expression von ICAM-1, ist die Endothelschicht für Zytokine starker permeabel und ermöglicht ihnen, die Endothelschicht zu durchdringen.
- TGFβ hat die Fähigkeit, glatte Muskelzellen aus ihrem kontraktilen Zustand in ihre proliferative Form zu überführen. In dieser Form sind die Zellen sehr sekretorisch und produzieren eine große Vielfalt an interzellulärer Matrix, darunter verschiedene Kollagene und Proteoglykane.
- Der Anmelder nimmt an, daß die Primärwirkung von Tacrolimus darin besteht, sowohl als Suppressor der Entzündungsreaktion gegen den fremden Stentkörper als auch als kompetitiver Inhibitor am FKBP-12-Rezeptor von glatten Muskelzellen zu wirken und sie daran zu hindern, in den sekretorischen Zustand überzugehen. Der Anmelder nimmt an, dass der wichtige Faktor, dem man sich bei Stentbeschichtungen zuwenden muss, die Immunantwort auf den Fremdkörper des Stents ist.
- In einer jüngeren Studie wurden die unterschiedlichen hemmenden Effekte von Immunsuppressiva auf glatte Aortenmuskelzellen und die Endothelzellenproliferation bei Menschen und Ratten untersucht. Aus diesem Versuch ging hervor, dass Tacrolimus sehr mäßige antiproliferative Eigenschaften auf Gefäßzellen hatte. Dies bedeutet, dass die normale Wundheilungsreaktion nicht beeinträchtigt, aber der Übergang zum sekretorischen glatten Muskelzellentyp, der für den Restenoseaufbau verantwortlich ist, praktisch gänzlich unterbunden ist. Dies ist der Hauptunterschied zwischen diesem Medikament und anderen Immunsuppressiva, die alle normalen Wundheilungsreaktionen hemmen:
Außerdem zeigte Methylprednisolon eine graduelle Hemmung über ein breites Konzentrationsintervall in glatten Muskelzellen bei Menschen und Ratten, aber nicht in menschlichen Endothelzellen. - Dosierung
- Da nur eine begrenzte Medikamentenmenge auf die Oberfläche eines Stents aufgetragen werden kann, sollte das wirksamste Medikament verwendet werden. In der klinischen Praxis liegt die Dosierung für Tacrolimus normalerweise in einem Bereich von 0,04 bis 0,06 mg/kg/Tag bei intravenöser Verabreichung. Dies bedeutet, dass etwa 4 mg pro Tag für einen 75 kg schweren Patienten verabreicht werden, wobei ein Wert von 10 bis 20 ng/ml im gesamten Blut das primäre Ziel ist.
- Die Dosierungen von Cyclosporin und von Mycophenolsäure, die zur Immunsuppression verwendet werden, sind 10 bis 100 mal höher, um einen ähnlichen Effekt zu erreichen:
Ein zweiter Aspektmacht Tacrolimus gegenüber Cyclosporin hoch überlegen: seine Doppelwirkung auf die Zytokinhemmung während Cyclosporin auch die Zytokinfreisetzung aus T-Zellen hemmt, hat es nicht den kompetitiven hemmenden Effekt von TGFβ in glatten Muskelzellen; was nach der Hypothese dieser Studie die Hauptwirkung von Arteriosklerose und Restenose im Zusammenhang mit dem Fremdkörperimplantat eines Stents ist. - Tacrolimus ist ein lipophiler Stoff, der in Wasser praktisch unlöslich ist. Daher erfolgt seine Verteilung im Blut primär intrazellulär in roten Blutkörperchen, im Plasma ist es an saure α1-Glycoproteine und Albumine gebunden. Das heißt, dass bei Auftrag auf einen Stent die Konzentration in den Zellen hoch ist, während die Lösung in Plasma gering ist, was zu einer hohen lokalen Konzentration führt.
- Unter der Annahme einer Freisetzung über mehr als 10 Tagen beträgt die freigesetzte Gesamtdosis weniger als 1/1000 der intravenös verabreichten Dosierung. Nimmt man an, dass der Großteil nicht in das Blut freigesetzt wird und dass Tacrolimus eine Halbwertzeit von 12 Stunden hat, liegt die aus der Stentbeschichtung freigesetzte systemische Dosierung 105-6 unter dem, was für eine systemische Wirkung erforderlich ist.
- Unter Annahme einer Freisetzung ähnlich wie bei anderen Medikamenten, die in den PLA-Träger eingebaut sind, kann ein minimaler intrazellulärer Wert von Tacrolimus im therapeutischen Bereich von 5 bis 20 ng/ml Gewebe in der benachbarten Wand erreicht werden.
- Der Abbau der PLA-Matrix wurde in der Vergangenheit getestet und beschrieben. Im Prinzip testet bei 37°C auch ein Magnetrührer den Gewichtsverlust von Stents in einer physiologischen Kochsalzlösung. PLA wird durch Hydrolyse zu Milchsäure abgebaut. Frühere Berechnungen zeigten. dass bei sofortigem Abbau der gesamten Polymermenge dies zu etwa 5 pmol Milchsäure führen würde, die in einem mittleren Blutvolumen von 7:1 verdünnt wurde. Dies würde einer Milchsäurebelastung von etwa 10–6 μmol/ml Laktat entsprechen, weit unter dem Laktatwert, der im Blut nach anstrengender körperlicher Belastung vorhanden ist (2 bis 4 μmol/l).
- Nicht bioresorbierbare Beschichtung
- Nicht spaltbare Beschichtungen können auch verwendet werden. Zusätzlich zur Oberflächenmodifikation des Metalls des Stents sind mehrere Polymerschichten erwogen. In dieser Ausführungsform wirkt die Polymerbeschichtung als Alternative zur PLA, und die auf die Dosisverabreichung bezogene Pharmakokinetik muss so optimiert sein, dass eine therapeutische Dosis auf der Grundlage der Zusammensetzung des Polymers bereitgestellt wird. Erwartungsgemäß wird das Medikament durch einen Diffusionsvorgang über eine Anzahl von Tagen freigesetzt. Die Polymermatrix sollte so maßgeschneidert sein, dass sie eine therapeutische Freisetzung erreicht. Erwartet wird, dass Polyurethane und Polyethylen sowie ähnliche Kunststoffe in dieser Anwendung von Nutzen sind. Möglich ist auch das bloße Anhaften des Medikaments ohne Träger an der Oberfläche des Stents, besonders wenn die Oberfläche des Stents rauh ist.
- Die Erfindung stellt ein Verfahren zur Behandlung eines Blutgefäßes mit den folgenden Schritten bereit: Einsetzen eines Stents in das Gefäß; Expandieren des Stents in Kontakt mit Gefäßwänden; Eluieren von Tacrolimus aus dem Stent in die Stelle des Stents.
- Dieses Verfahren ist insoweit von Vorteil, da der Restenoseprozeß nach Implantation eines Stents in die Arterie des menschlichen Körpers ohne Beeinträchtigung der normalen Wundheilung gehemmt wird. Das Verfahren hemmt die Zellproliferation durch den Mechanismus des selektiven Hemmens der durch den Fremdkörper induzierten Entzündungsreaktion und des Übergangs glatter Muskelzellen aus ihrem kontraktilen Zustand in einen proliferativen Zustand und sekretorischen Zustand.
- Tacrolimus wird auf die Oberfläche des Stents entweder direkt aufgetragen oder wird vorzugsweise in oder auf einer Beschichtung des Stents gleichmäßig verteilt.
Claims (1)
- Verwendung einer Menge von Tacrolimus zur Herstellung eines endovaskulären Stents zum Verhindern von Restenose mit: einem hohlen Rohr mit offenen Enden und einer Seitenwand mit einer Vielzahl von Öffnungen, das eine Primärstruktur bildet; wobei das hohle Rohr deformiert werden kann, um das hohle Rohr in einem Gefäß, zu positionieren; und einer sekundären Beschichtung auf der Primärstruktur; wobei die Menge von Tacrolimus in der sekundären Beschichtung gelegen ist, wobei die Menge bemessen und abgestimmt ist zum Verhindern von Restenose durch Erreichen einer lokalen Konzentration innerhalb des Gefäßes an dem Implantationsort des Stents, um eine normale Wundheilung zu ermöglichen und gleichzeitig an diesem Ort eine Entzündungsreaktion, die in einer Restenose resultieren würde.
