- HINTERGRUND DER ERFINDUNG
- Gebiet der Erfindung
- Die vorliegende Erfindung betrifft Vorrichtungen zur Behandlung von Rücken- und Beinschmerz und insbesondere Nukleusersatz innerhalb des Annulus fibrosis. Die Vorrichtung ersetzt einen Teil einer zerstörten spinalen Bandscheibe, um die Funktion wieder herzustellen.
- Beschreibung des Standes der Technik
- In der Wirbelsäule ist die prinzipielle Funktion der scheibenvertebralen Verbindung, Druckbelastungen zu übertragen und trotzdem Flexibilität zu ermöglichen. Benachbarte Wirbel sind über einen dreifachen Verbindungskomplex miteinander verbunden. Der äußere Komplex oder Säule wird durch die Wirbelkörper gebildet, die wie abgeflachte Zylinder geformt sind, mit scheibenförmigen oder oval geformten Bandscheiben, die zwischen jedem Wirbelkörper sandwichartig eingelegt sind. Anschlussverbindungen hinten an jedem Wirbel haben eine glatte Knorpeloberfläche, schmierende Verbindungsflüssigkeit und eine abdeckende Kapsel. Die Anschlussverbindungen schränken die Scheibe auf kleine Grade an Biegung und Dehnung ein, begrenzen die Rotation und schützen gegen Translationsscherspannung. Die Scheibe selbst weist zwei prinzipielle Teile auf, den Nukleus pulposus im Kern und den Annulus fibrosis, der eine mehrlagige diagonale Umhüllung ist, welche den Nukleus umgibt. Der Nukleus beginnt im frühen Leben mit achtzig Prozent Wasser und trocknet mit dem Alter langsam aus.
- Eine zerstörte Scheibe kann Nervendysfunktion verursachen und entkräftenden Schmerz in Rücken, Armen und Beinen. Typische Behandlungen, welche Erleichterung zur Verfügung stellen und den Patienten erlauben, wieder zu funktionieren, beinhalten Rückenbandagen, medizinische Behandlung, physikalische Therapie und Chirurgie, um die Scheibe zu entfernen. Eine herkömmliche chirurgische Lösung entfernt die schlechte Scheibe und treibt neues Knochenwachstum in dem Raum voran, um die benachbarten Wirbel miteinander zu verbinden.
- Verschiedene unterschiedliche prothetische Bandscheiben-Vorrichtungen werden zum Beispiel von Casey K. Lee, et al., in „Prosthetic Intervertebral Disc," Kapitel 96, The Adult Spine: Principles and Practice, Raven Press, Ltd., New York, © 1991, beschrieben. Der Schluss von Lee, et al. ist, dass „Eine geeignet ausgelegte und hergestellte prothetische Bandscheibe eine verbesserte Alternative zu derzeit möglichen chirurgischen Ansätzen für untere Rückenerkrankungen zur Verfügung stellen kann." Lee, et al. beschreiben, dass ihre Arbeit in den orthopädischen Untersuchungslaboratorien an der New Jersey Medical School ist, „ein prothetisches Bandscheibendesign herzustellen, das biomechanische Charakteristiken hat, ähnlich zur natürlichen Scheibe." Ein Ergebnis war die Herstellung einer Einheit mit einem Kern, Annulus und Endplatten, die unter Wärme geformt und zu einer einzelnen prothetischen Scheibe zusammengefügt werden. Der Erfolg von einer solchen Vorrichtung hängt jedoch von der festen Knochenbefestigung ab. Die meisten bisherigen Konzepte waren übermäßig komplex und wurden nie verwendet.
- Eine Bandscheibenprothese aus zwei Protheseplatten und einem Kern, der sich zwischen und in Zusammenarbeit mit diesen Platten befindet, ist in
US 5 401 269 A offenbart. - Ein prothetischer Nukleusersatz kann chirurgisch in den Annulus fibrosis implantiert werden. Die natürlichen Befestigungen des Annulus können die erforderliche Zugfestigkeit der reparierten Stelle wieder herstellen. Der prothetische Nukleusersatz wäre primär Druckkräften ausgesetzt.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Ein Gegenstand der vorliegenden Erfindung ist, einen prothetischen Nukleusersatz zur Verfügung zu stellen, der sicherer als ein chirurgisches Implantat ist.
- Ein weiterer Gegenstand der vorliegenden Erfindung ist, einen prothetischen Nukleusersatz zur Verfügung zu stellen, der es ermöglicht, um über einen Zeitraum Röntgenbilder des Rückgrats zu verfolgen und diagnostische Beurteilung nicht zu beeinträchtigen.
- Kurz gesagt weist eine erfindungsgemäße prothetische Nukleusersatzausführungsform ein festes Material auf, das zu einer ovalen Scheibe abgeflacht ist. Die obere Oberfläche der Scheibe ist gewölbt und hat eine Krone, die etwa dreimal höher ist als die Krone der gewölbten Bodenoberfläche. Sowohl obere als auch untere Oberfläche sind daher konvex. Ein Stift erstreckt sich von der Mitte der unteren gewölbten Oberfläche hinab und wird dazu verwendet, um die Scheibe an dem unteren der beiden Wirbel, zwischen welchen sie angebracht ist, zu befestigen. Metallmarker werden in den Stift und eine Außenseitenkante der Scheibe eingesetzt, so dass Röntgenbilder verwendet werden können, um die Position der Scheibe in situ zu bestimmen. Der prothetische Nukleusersatz wird chirurgisch über einen Klapperschnitt in den ausgehöhlten Intervertebralraum in den natürlichen Annulus fibrosis implantiert. Der untere Wirbel wird so vorbereitet, dass er den Stift aufnehmen kann, indem das Material, welches die Knochenmatrix oben bedeckt, entfernt wird. Knochenzement wird um den Stift herum verwendet, um sicheren Sitz und unverrückbare Befestigung der Scheibe an dem unteren Wirbel sicherzustellen.
- Ein Vorteil der vorliegenden Erfindung ist, dass ein prothetischer Nukleusersatz zur Verfügung gestellt wird, der die normalen Druckbelastungen, die von natürlichen Wirbeln erfahren werden, flexibel trägt.
- Ein weiterer Vorteil der vorliegenden Erfindung ist, dass ein prothetischer Nukleusersatz zur Verfügung gestellt wird, der einen Satz von metallischen Markern hat, die einem Mediziner bei der Bestimmung der Orientierung des genannten prothetischen Nukleusersatzes in situ helfen kann, ohne normale Anatomie zu verdecken oder zukünftige Abbildung der Wirbelsäule zu schwächen.

1 veranschaulicht eine typische Platzierung einer erfindungsgemäßen prothetischen Rückgratnukleusersatzausführungsform, auf die hier durch die Referenznummer10 Bezug genommen wird. Eine menschliche Wirbelsäule12 weist üblicherweise eine Serie von Wirbeln14 bis18 auf, in die eine korrespondierende Serie von Scheiben20 bis23 ineinander greift. Jede natürliche Scheibe weist einen Nukleus pulposus auf, umgeben und enthalten in einem korrespondierenden Annulus fibrosis. Natürliche Nukleus pulposus haben gelartige Strukturen, die Kompressionsstoßbelastungen absorbieren und dämpfen können. Natürliche Annulus fibrosis-Strukturen weisen mehrere Schichten von diagonalen Filamentsätzen in 40°-Winkeln auf, die der Konstruktion einer Automobildiagonalreifenkarkasse ähneln. - Scheibe
22 , zwischen Wirbel16 und17 , wird in1 als degeneriert angenommen. Die spinale Nukleusersatzprothese10 wird chirurgisch in den Zwischenwirbelraum zwischen Wirbel16 und17 und im Inneren eines Annulus fibrosis24 eingebettet. - Erfindungsgemäße prothetische Nukleuseratzausführungsformen können ein festes Polymer aufweisen, das zu einer ovalen Scheibe abgeflacht ist. Im Allgemeinen kann jedes feste, biokompatible Material verwendet werden, einschließlich verschiedener Polymere und Kunststoffe, Titan, rostfreiem Stahl, Tantal, Chrom-Kobalt-Legierungen, etc. Ultrahochmolekulargewichtiges Polyethylen ist derzeit bevorzugt, sodass Metallradiographiemarker strategisch in der Nukleusersatzprothese
10 platziert werden können. - Wie in
2A gezeigt, hat eine Nukleusersatzprothese30 eine obere Hälfte32 , die gewölbt ist, und hat eine Krone, die etwa dreimal höher („3h") als die Krone („1h") auf der gewölbten Bodenhälfte34 ist. Das Ganze ähnelt einem Ellipsoid, das teilweise kollabiert ist. Sowohl obere als auch untere Oberfläche sind daher konvex. Der Außendurchmesser der Nukleusersatzprothese30 kann variieren, z.B. in dem Bereich von zwanzig bis sechsunddreißig Millimetern. Die Gesamthöhe kann ebenfalls variieren, z.B. in dem Bereich von acht bis sechzehn Millimetern. Die tatsächlich erforderlichen Abmessungen hängen von der Größe des Patienten und der exakten Stelle ab, welche das Implantat aufnehmen soll. Solche erforderlichen Größen werden auf Patientenröntgenbildern, CT-Scans und MRI-Scans erkennbar. - Ein Stift
36 erstreckt sich von der Mitte der unteren gewölbten Oberfläche34 nach unten. Der Stift36 ist typischerweise zwei bis vier Millimeter lang und wird dazu verwendet, um die Nukleusersatzprothese30 mit dem unteren Wirbel zu verstiften, z.B. Wirbel17 in1 . Ein Paar Metallradiographiemarker38 , z.B., einer in dem Stift36 und einer an einer Außenkante, werden so angebracht, das Röntgenbilder verwendet werden können, um die Position der Scheibe in situ zu bestimmen. Der prothetische Nukleusersatz30 wird chirurgisch über einen Klappenschnitt in den ausgehöhlten Intervertebralraum in dem natürlichen Annulus fibrosis implantiert. Ein solches „ Aushöhlen" wird Allgemein als Diskektomie bezeichnet. Der untere Wirbel wird vorbereitet, um den Stift36 aufzunehmen, indem das Material, welches die Knochenmatrix oben bedeckt, entfernt wird. Knochenzement wird um den Stift36 herum verwendet, um sicheren Halt und unbewegliche Befestigung der Scheibe an den unteren Wirbel sicherzustellen. -
3A und3B zeigen eine Variante der Ausführungsform der2A und2B . Eine Nukleusersatzprothese40 hat ein Kantensegment42 , das von einer sonst ovalen oder runden lateralen Form entfernt wurde. Dieser gestutzt Teil42 wird in situ so ausgerichtet, dass er dem hinteren Rückgrat gegenüber liegt. Eine obere Hälfte44 des Hauptkörpers ist gewölbt und hat eine Krone, die etwa dreimal höher („3h") als die Krone („1h") einer gewölbten Bodenhälfte46 ist. Sowohl obere als auch untere Oberfläche sind konvex und zumindest die obere Oberflächenform imitiert einen natürlichen Nukleus. Der äußere Hauptdurchmesser der Nukleusersatzprothese40 kann variieren, z.B. in dem Bereich von zwanzig bis sechsunddreißig Millimetern. Die gesamte Höhe kann ebenfalls variieren, z.B. in dem Bereich von acht bis sechzehn Millimetern. Die tatsächlichen Abmessungen, die erforderlich sind, hängen von der Größe des Patienten und der exakten Stelle ab, welche das Implantat aufnehmen soll. Solche erforderlichen Größen sind aus Patientenröntgenbildern, CT-Scans und MRI-Scans erkennbar. - Eine Vorrichtung, ähnlich zu den Nukleusersatzprothesen
10 ,30 und40 , wurde durch den vorliegenden Erfinder, Kevin Lawson, M. D., kürzlich in einer chirurgischen Prozedur verwendet. Der Patient war eine 64 Jahre alte Dame mit einer Anzahl von Jahren an Rückenschmerzen, schrittweise sich verschlechternd, zusammen mit Beinschmerzen und der Unfähigkeit, eine Entfernung zu gehen. Ebenes Röntgen, aufrechtstehend gegenüber Rückenlage, zeigte eine mobile Spondylolisthese von L4-5. Diskographie zeigte schmerzvolle L2-3- und L3-4-Scheiben mit einem annularen Leck von Kontrast und Reproduzierbarkeit von übereinstimmendem Schmerz. Die LS-S1-Scheibe war ohne Schmerz. Die L4-5-Ebene war leicht schmerzhaft und stenotisch. - Die Risiken und Vorteile eines chirurgischen Eingriffs wurden mit der Patientin besprochen und sie wünschte, dass der Eingriff stattfindet. Nicht-operative Behandlung war unwirksam gewesen und ihre Symptome verschlechterten sich. Der Patientin wurde aufgrund der Fettleibigkeit der Patientin, allgemeinen Gesundheitsbedingung und Rauchervergangenheit, ein Wiederaufbau der vorhergehenden Scheiben der L2-3- und L3-4-Ebenen empfohlen. Die Alternative war eine feste Verbindung mit einhergehender Steifheit. Hintere L4-5-Verbindung und Dekompression wurde empfohlen. Die Patientin wurde bezüglich vorderer Zwischenkörperverbindung bei L3-4 und L2-3 mit gleichzeitiger Verbindung posteriori von L2 bis L5 unterrichtet. Segmentale Instrumentierung wurde mit ihr ebenso besprochen, wie der FDA-regulatorische Status von verschiedenen Wirbelsäulenimplantaten. Die Patientin wurde informiert, dass dies ein neues und kein Standardverfahren ist, wobei orthopädische Standardimplantate, die zur Verwendung in Nicht-Rückenmarkanwendungen gedacht waren, für die Verwendung gestaltet werden.
- Die Patientin wird in den Operationssaal gebracht. Nach dem Einleiten zufriedenstellender allgemeiner Anästhesie und Anwendung von geeignetem intervenösem (IV) Zugang, einschließlich einer zentralvenösen Leitung und einer arteriellen Leitung, wird die Patientin sorgfältig auf ihrer rechten Seite gelagert, wobei die linke Seite oben auf einem Strahlendurchlässigen Vorsprung des Operationstisches ist. Ein Sitzsack wird in Position gebracht und komprimiert und die Patientin wird in dieser Lage gesichert. Anteriorposterior und laterale fluoroskopische Bilder werden bestätigt. Der vorhergehende Nephrektomieschnitt auf der linken Seite der Patientin ist geeignet für die vordere Diskektomie und Rekonstruktion. Die Flanke wird mit sterilen Betadin-Lösungen präpariert und mit einem sterilen Lobanverband und sterilen Abdeckungen abgedeckt. Der alte Schnitt wird markiert und längs eingeschnitten. Kleine Gefäße werden durch Elektroverbrennung verödet. Der Schnitt wird hinab durch das subkutane Gewebe hindurch vertieft und selbsthaltende Retraktoren werden angebracht. Der äußere schräge Muskel wird längs in Richtung mit seinen Fasern geteilt und mit mehreren schweren #1 VICRYL-Wundnähten versehen. Die inneren Quer- und Transversal-Muskelschichten werden zusammengeheftet und werden in Richtung mit dem Schnitt quer zur Faser des Muskels zerteilt.
- Der Retroperitonealraum wird dann identifiziert und Retroperitonealfett leicht identifiziert. Dieses wird dann freigelegt und die Wunde aufgetrennt und leicht von hinter der mittleren Axiliarlinie hinter dem Flankenbereich geöffnet. Der Retroperitonealinhalt wird geradeheraus mit einem Schwammstock, RAY-TEC-Schwämmen und einem dehnbaren Retraktor herausgenommen. Ein OMNI-Retraktor wird dann am Tisch befestigt und geeignete Klingen angebracht. Der Psoasmuskel wird identifiziert und die fernoralen Nervenzweige identifiziert und geschützt.
- Der Scheibenraum wird mit einem Erdnuss-Sezierer identifiziert. Fluoroskopie wird verwendet, um die richtige Ebene zu bestätigen, und die L3-L4-Scheibe wird mit einer Spinalnadel markiert. Geeignete anterior-posterior-Bilder und laterale Bilder werden erhalten. Das anterior-posterior-Bild erlaubt es dem chirurgischen Team, die Mitte des Vertebralkörpers zu lokalisieren. Der Muskel wird wiederaufgerichtet und mit einem PLIG-Nervenwurzelretraktor zurückgeholt. Die obere Kante des L4-Wirbelkörpers wird weggemeißelt, wobei die Flanke des Knochens mit dem Annulus, quer zur oberen lateralen Kante des L4-Wirbelkörpers, abbricht. Dies wird dann nach oben gefaltet und der Scheibenraum mit einer Hypophysen-Hohlmeißelzange und mit japanischen Greifhohlmeißelzangen ausgeräumt. Unter fluoroskopischer Beobachtung wird eine Spinalkürette mit großem Winkel verwendet, um die tiefere knorpelige Endplatte des Scheibenraums zu kürettieren und ein kleines Stück oder eine kleine Perforierung in der Mitte und einer Stelle leicht oberhalb der Endplatte in die Endplatte zu kürettieren. Dies wird dann in einem Versuch überprüft. Ein dreiundzwanzig mal acht mm Implantat wird verwendet und an der Stelle zusammengepresst und dann später ausgedehnt.
- Es wird auf den L2-3-Scheibenraum acht gegeben, wo die Einwirkung eingeengt ist. Eine einfache Annulotomie in rechtwinkliger Art und Weise wird mit einer distalen Falltür verwendet und dann nach außengefaltet. Die degenerierte Scheibe wird mit Hypophysen-Hohlmeißelzangen ausgeräumt und ernsthaft zerstörte Scheibenfragmente werden entfernt. Die distale Endplatte wird kürettiert und eine Perforation in dem zentralen Mittelstück vorgenommen.
- Die Scheibenräume werden mit glatten zehn mm und acht mm PLIG-Instrumenten auseinander gehalten. Jeder wird überprüft und ein Versuchsimplantat kann an der Stelle zusammengepresst werden. Ein sechsundzwanzig mal neun mm hohes PROFLIC-FIX-Patellar-Implantat wird für die L3-4-Scheibe ausgewählt, und ein dreiundzwanzig mm-PROFLIC-FIX-Patellar-Implantat aus einem RICHARDS-Satz wird für den L2-3-Scheibenraum ausgewählt. Beide Implantate werden präpariert. Die hintere Seite des sechsundzwanzig mm-Implantats wird mit einer kleinen Hochgeschwindigkeitsfräse und doppelt wirkender Hohlmeißelzange weg geschnitten, um die hintere Seite flach quer zu glätten und die hinten-vorne-Abmessungen auf etwa dreiundzwanzig mm zu verringern.
- PALACOS-Knochenzement und antibiotisches Pulver werden vermischt und zu einer teigigen Paste präpariert und in die vordere Seite des Implantats neben den Stift und eine Zement-zurückhaltende Rille gedrückt und dann mit dem Scheibenraum ausgerichtet. Die Annulusklappe wird nach oben gefaltet. Der Aufbau wird in den Scheibenraum bei L3-4 mit geeigneten Retraktoren unter Verwendung einer KOCHER-Klemme auf den nichtartikulären Seitenkanten des Implantats eingepresst. Eine Knochenklemme wird an der anderen Kante verwendet, um das Anklopfen von allem in sauberer Ausrichtung in dem Scheibenraum zu beenden. Anterior-posterior- und laterale Bilder werden überprüft und bestätigen, dass der Marker das Implantat ungefähr in dem Mittelteil des Scheibenraums von rechts nach links in einer leichten anterior-posterior-Richtung positioniert hat. Ein dreiundzwanzig mm-Implantat wird ebenso mit einer geringen Menge an PALACOS-Knochenzement präpariert und in einer Mittelposition innerhalb des Scheibenraums eingepresst.
- Der Annulus wird an der L2-3-Ebene mit #0 TYCRON-Wundnähten unter Verwendung einer Nadel vom Hernia-Typ durch den verbleibenden Annulus und Endplatte am L3-Körper repariert, wobei der an den L2-Körper angeheftete Annulus gegriffen und dies mit mehreren unterbrochenen Wundnähten repariert wird. Ebenso wird die Absplitterung des Knochens mit #0 ETHIBOND-artiger Wundnaht und zwei STAY-TAC-Wundnahtankern von L3 nach L4 repariert.
- Die Wunde wird während dieser Prozedur gespült und trocken gesaugt. Unterbrochene Spülung wird vorgenommen, um Gewebebefeuchtung aufrechtzuerhalten. Es gibt keine signifikante Blutung. Die segmentalen Gefäße werden sorgfältig während der Prozedur geschützt und keines wird verletzt oder durchtrennt. Eine geringe Menge an GELFOAM, getränkt mit THROMBIN, wird auf die Flächen des Annulus neben dem Psoasmuskel aufgebracht, der an beiden Stellen zurück in seine normale Position fallen lassen wird. Die Retroperitonealinhalte werden zurück an ihre normale Stelle fallen lassen. Der OMNI-Retraktor wird entfernt. Die tiefe Transversalisschicht und innere Querschicht wird mit mehreren unterbrochenen und dann einer durchlaufenden #1 VICRYL-Wundnaht verschlossen. Die äußere Querschicht wird dann mit einer getrennt laufenden #1-VICRYL-Wundnaht verschlossen. Das subkutane Gewebe wird mit unterbrochenen #2-#0 VICRYL-Wundnähten verschlossen und der letztendliche Hautverschluss des Flankenschnitts wird mit Klammern vorgenommen. XEROFORM, 4 × 8, und Schaumband werden für einen Verband aufgebracht. Die Abdeckungen werden entfernt und die erste Stufe der Prozedur wird beendet. Die zweite Stufe umfasst eine Standarddekompression und Fusion eines weiteren Teils der Rückgrats.
- Zusammenfassend werden die prothetischen Kerne der vorliegenden Erfindung allgemein unter Verwendung eines geraden vorderen oder vorderen lateralen Ansatzes mit Einschnitt der vorderen Längsligamente des Annulus implantiert. Eine Klappentechnik wird für den Einschnitt des Annulus verwendet und diese Gewebe werden mit herkömmlichen Wundnähten oder Wundnahtankern an dem Knochen repariert. Der Endplattenknorpel des oberen Wirbels wird für permanente Gelenkverbindung mit der implantierten Nukleusprothese erhalten. Der Endplattenknorpel des unteren Wirbels wird auf den Knochen hinabkürettiert. Der Knochen wird präpariert, um einen Stift aufzunehmen, der in der implantierten Nukleusprothese eingebettet ist. Ein solches Verstiften und auch Zement werden verwendet, um diese Grenzfläche permanent zu immobilisieren. Der gesamte Aufbau wird so weit wie möglich hinten zentriert, um die natürliche Kinematik des Beugens-Streckens und lateralen Biegens wieder herzustellen.
- Bei der Konstruktion und Ausführungsform der vorliegenden Erfindung wurde kein Versuch unternommen, die Kompressions-Dämpfungsqualitäten der natürlichen Scheibe zu reproduzieren. Daher können feste Materialien bei der Herstellung des Hauptkörperellipsoids verwendet werden.
- Obwohl besondere Ausführungsformen der vorliegenden Erfindung beschrieben und veranschaulicht wurden, ist damit nicht beabsichtigt, die Erfindung einzuschränken. Modifikationen und Änderungen werden ohne Zweifel dem Fachmann offensichtlich und es ist beabsichtigt, dass die Erfindung lediglich durch den Umfang der anhängenden Ansprüche eingeschränkt ist.
Claims (5)
- Nukleusersatzprothese (
10 ,30 ,40 ) zum Einsetzen in einen Annulus fibrosis in einem Teil einer humanen Wirbelsäule, umfassend einen elliptischen Körper mit einer konvexen Oberseite (32 ,44 ) zur Kontaktierung und Verbindung mit einem Endplattenknorpel eines oberen Wirbels und einer konvexen Unterseite (34 ,46 ) für einen unbeweglichen Kontakt mit einem unteren Wirbel, und einen Stift (36 ,48 ), der sich von dieser Unterseite des elliptischen Körpers erstreckt und eine Verstiftung hinsichtlich dieses unteren Wirbels zur Verfügung stellt, dadurch gekennzeichnet, dass diese konvexe Oberseite eine Kuppelkrone aufweist, die um etwa den Faktor drei größer ist als eine Kuppelkrone dieser konvexen Unterseite. - Nukleusersatzprothese (
10 ,30 ,40 ) von Anspruch 1, wobei der elliptische Körper ultrahochmolekulargewichtiges Polyethylen umfasst. - Nukleusersatzprothese (
10 ,30 ,40 ) von Anspruch 1, ferner umfassend einen Satz von metallischen Markern (38 ), die strategisch innerhalb des elliptischen Körpers und des Stifts platziert sind, und die einem Arzt dabei helfen können, die Orientierung dieser Nukleusersatzprothese in situ zu bestimmen. - Nukleusersatzprothese (
10 ,30 ,40 ) von Anspruch 1, ferner umfassend Zement, um die Fixierung und Immobilisierung dieser Nukleusersatzprothese an diesen unteren Wirbel zu unterstützen. - Nukleusersatzprothese (
10 ,30 ,40 ) von Anspruch 1, wobei der elliptische Körper einen vorderen Rand und einen hinteren Rand mit einem verkürzten Rand (42 ) aufweist, so dass ein solches stumpfes Ende in situ so orientiert ist, dass es dem Spina posterior gegenübersteht.
