Description
- Gebiet der Erfindung
- Die Erfindung betrifft allgemein Vorrichtungen und Verfahren, die beim Einfangen von embolischem Material in Blutgefäßen von Nutzen sind. Insbesondere stellen die Vorrichtungen und Verfahren ein Gefäßfiltersystem zum temporären Setzen in Arterien, z. B. der Halsschlagader und der Hauptschlagader, und in Venen, z. B. der Schlüsselbeinvene und der oberen Hohlvene, bereit. Das System weist auch einen Führungsdraht zum Leiten endovaskulärer Vorrichtungen, z. B. Atherektomie-, Stentsetzungs- oder Angioplastiekatheter, zu einem interessierenden Bereich sowie einen Führungskatheter mit Fluidspülfähigkeit zur Unterstützung beim Filtern auf.
- Hintergrund der Erfindung
- In letzter Zeit hat sich die Behandlung thrombotischer oder atherosklerotischer Läsionen in Blutgefäßen auf endovaskulärem Weg bei ausgewählten Patienten als effektive und zuverlässige Alternative zu chirurgischen Eingriffen erwiesen. Nützlich sind z. B. die direktionale Atherektomie und perkutane transluminale Koronarangioplastie (PTCA) mit oder ohne Stentsetzung bei der Behandlung von Patienten mit Koronarokklusionen. Die Atherektomie entfernt Plaque auf physische Weise durch Ausschneiden, Zermahlen oder Ausschälen in atherosklerotischen Arterien mit einer über einen Katheter zuführbaren Endarteriektomievorrichtung. Die Angioplastie vergrößert den Lumendurchmesser eines stenotischen Gefäßes durch mechanische Kraftausübung auf die Gefäßwände. Neben dem Gebrauch von Angioplastie, Stentsetzung und/oder Atherektomie an der Koronargefäßarchitektur erwiesen sich diese endovasku lären Techniken auch bei der Behandlung anderer Gefäßläsionen als nützlich, z. B. bei Halsschlagaderstenose, peripherer arterieller Verschlußkrankheit (besonders der Hauptschlagader, Hüftarterie und Oberschenkelarterie), durch Atherosklerose oder fibromuskuläre Erkrankung verursachter Stenose der Nierenarterie, Vena-cava-superior-Syndrom und thrombolysenresistenter okklusiver Thrombose der Hüftvene.
- Anerkanntermaßen ist eine der Komplikationen im Zusammenhang mit endovaskulären Techniken das Herauslösen embolischer Materialien, die beim Manipulieren des Gefäßes erzeugt werden, was Okklusion der stromabwärts gelegenen engeren Gefäße und Ischämie oder Infarkt des Organs verursacht, das vom Gefäß versorgt wird. 1995 offenbarten Waksman et al., daß distale Embolisierung nach direktionaler Atherektomie in Koronararterien und Transplantaten der Rosenvene verbreitet auftritt. Siehe dazu Waksman et al., American Heart Journal 129(3):430-5 (1995). In dieser Studie wurde festgestellt, daß es bei 28 % der Atherektomiepatienten (31 von 111) zu distaler Embolisierung kommt. Im Januar 1999 offenbarten Jordan, Jr. et al., daß die Behandlung von Karotisstenosen durch perkutane Angioplastie mit Stentsetzung mit einer mehr als achtfachen Rate von Mikroemboli als bei Endarteriektomie der Karotis einhergeht. Siehe dazu Jordan, Jr. et al. Cardiovascular surgery 7(1):33-8 (1999), hierin insgesamt durch Verweis aufgenommen. Mikroemboli, in dieser Studie durch transkranielles Doppler-Monitoring erfaßt, sind nachweislich eine potentielle Ursache für Schlaganfall. Zu den embolischen Materialien gehören Calcium, Intimatrümmer, atheromatöse Plaque, Thromben und/oder Luft.
- Zum Einfangen vaskulärer Emboli gibt es eine Anzahl von Vorrichtungen, die für Blutfilterung sorgen sollen. Der übergroße Teil dieser Vorrichtungen ist zur permanenten Plazierung in Venen vorgesehen, um Lungenembolie zu verhindern. Eine temporäre venöse Filtervorrichtung ist in der US-A-5053008 (Bajaj) offenbart. Die Vorrichtung nach Bajaj ist ein Intrakardialkatheter zur temporären Plazierung im Lungenarterienstamm eines Patienten mit Prädisposition für Lungenembolie, z. B. infolge von Hüftoperationen, großen Verletzungen, großen Bauch- oder Beckeneingriffen oder Immobilisierung. Die Vorrichtung nach Bajaj weist einen aus einem Maschenwerk hergestellten Schirm auf, der venöse Embolie einfängt, bevor sie die Lungen erreichen. Diese Vorrichtung ist zur venösen Filtration gestaltet und wegen der hämodynamischen Unterschiede zwischen Arterien und Venen nicht zum arteriellen Gebrauch geeignet.
- Es gibt sehr wenige intravaskuläre Vorrichtungen, die zum arteriellen Gebrauch gestaltet sind. Arterien sind viel flexibler und elastischer als Venen, und in den Arterien ist der Blutfluß pulsierend mit großen Druckabweichungen zwischen dem systolischen und diastolischen Durchfluß. Durch diese Druckvariationen werden die Arterienwände zum Expandieren und Kontrahieren veranlaßt. Blutflußgeschwindigkeiten in den Arterien variieren von etwa 1 bis etwa 5 l/min. Die US-A-4873978 (Ginsburg) offenbart ein arterielles Filtersystem, das einen Katheter mit einer Siebvorrichtung an seinem distalen Ende aufweist. Diese Vorrichtung wird in das Gefäß stromabwärts von der Behandlungsstelle eingeführt, und nach der Behandlung wird das Sieb um die eingefangenen Emboli zusammengelegt und aus dem Körper entfernt. Die Vorrichtung nach Ginsburg könnte keinen Durchflußgeschwindigkeiten von 5 l/min widerstehen. Sie ist nur für kleine Arterien gestaltet und könnte daher keine Emboli einfangen, die für alle Teile des Körpers bestimmt sind. Die DE-A-3417738 (Ing. Walter Hengst GmbH & Co.) offenbart ebenfalls ein weiteres arterielles Filter mit einem Faltgestängesystem, das das Filter aus dem zusammengefalteten in den expandierten Zustand überführt.
- Für die intravaskuläre Blutfiltration wurden Filter vorgeschlagen, die am distalen Ende von Führungsdrähten angeord net sind. Ein Großteil dieser Vorrichtungen verfügt über ein Filter, das an einem Führungsdraht befestigt ist und über Streben mechanisch betätigt wird, oder einen vorgeformten Korb, der sich im Gefäß entfaltet. Normalerweise sind diese Filter "Fallschirme" aus Maschenmaterial, die am Schaft des Drahts am distalen Ende und an Drahtstreben befestigt sind, die sich am proximalen Ende in Radialrichtung nach außen erstrecken. Die Radialstreben öffnen das proximale Ende des Filters zur Wand des Gefäßes. Durch das Gefäß fließendes Blut wird zwangsweise durch das Maschenmaterial geführt, wodurch embolisches Material im Filter eingefangen wird. Ein Hauptnachteil im Zusammenhang mit diesen Filtervorrichtungen ist, daß die Filter allgemein auf vaskulären Blutfluß angewiesen sind, um Trümmer in die Filter zu schieben. Bei Restriktion des Blutflusses im Gefäß ist das lose anhaftende embolische Material möglicherweise keinem normalen turbulenten Blutfluß ausgesetzt. Die embolischen Teilchen können im Gefäß proximal zum Filter verbleiben, bis der höhere normale Fluß wieder hergestellt ist (d. h. wenn das Filter entfernt ist), was die Wirksamkeit der Filtervorrichtungen verringert.
- Bei einer weiteren Möglichkeit zum Entfernen von embolischem Material werden temporäre Okklusionsvorrichtungen genutzt, z. B. Ballonokklusionskatheter und Gefäßklemmen, um ein Teilstück eines Gefäßes zu isolieren. Nachdem der Blutfluß im Gefäß isoliert ist, wird Fluid oder Blut im Gefäß aspiriert, um embolische Trümmer zu entfernen. Einer der Nachteile im Zusammenhang mit Okklusionsvorrichtungen ist, daß sie eine temporäre Einstellung oder Verringerung der distalen Perfusion erfordern, die die Sauerstoffversorgung distaler Organe beeinträchtigen kann. Shunts können distal zu den Vorrichtungen angelegt werden, um die Perfusion zu distalen Organen aufrecht zu erhalten. Allerdings führt das Einsetzen von Shunts zu zusätzlichen Gefäßverletzungen und kann zusätzliches embolisches Material erzeugen.
- Benötigt werden einfache und sichere Blutfiltervorrichtungen, die in den Arterien und Venen temporär plaziert und mit endovaskulären Instrumenten verwendet werden können, um distale Embolisierung wirksam zu verhindern. Dazu sind vorhandene Vorrichtungen unzureichend geeignet.
- Die US-A-5910154, die den mit dem Gegenstand der Ansprüche am engsten verwandten Stand der Technik repräsentiert, offenbart einen Filterführungsdraht, über dem ein Stentsetzungskatheter vorgeschoben werden kann.
- Die WO 97/42879 und WO 99/30766 beschreiben Aortenokklusionsvorrichtungen mit zugeordneten Filtern darin.
- Zusammenfassung der Erfindung
- In der Hauptschlagader vorhandene feste oder mobile Plaque kann sich herauslösen und Niereninfarkt oder Ischämie anderer Organe verursachen. Außerdem stellt der Plaqueaufbau in den Halsschlagadern ein Risiko für ischämische Attacken durch Embolisierung und eine zusätzliche Bedrohung eines sich verringernden Blutflusses durch Okkludieren des Gefäßlumens dar. In den Hüft- und Oberschenkelarterien vorhandene Plaque kann Ischämie der unteren Extremitäten verursachen, entweder durch distale Embolisierung von atheromatösem Material oder durch in-situ-Stenose des erkrankten Blutgefäßes, d. h. Verengung des Lumendurchmessers. Atherektomie oder Angioplastie mit oder ohne Stentsetzung in diesen Gefäßen verhindert das Auftreten dieser Krankheit, kann aber auch diese Zustände herbeiführen, sofern die Vorrichtung nicht speziell gestaltet ist, embolisches Material einzufangen, das im Procedere herausgelöst wird.
- Die Erfindung stellt Vorrichtungen zum temporären Plazieren von Blutfilterfähigkeiten in einer Arterie oder Vene im Verlauf von endovaskulären Verfahren bereit. Insbesondere stellt die Erfindung ein Gefäßfiltersystem nach Anspruch 1 bereit, das dem Einführen endovaskulärer Katheter zum Entfernen atherosklerotischer Plaque und/oder Thromben und Vergrö ßern des Lumendurchmessers eines stenotischen Gefäßes Rechnung trägt. Das Filtersystem stellt auch eine Einrichtung zum Aufrechterhalten der distalen Perfusion während der Isolierung des Blutflusses und zum Schieben embolischer Trümmer in das Filter in einem durchflußarmen Zustand bereit, was die Filterfähigkeiten erhöht.
- In einer Ausführungsform verfügt das Gefäßfiltersystem über ein expandierbares Filter, z. B. ein Schirm-, Korb- oder "Scroll"-Filter, das an einem distalen Ende eines Führungsdrahts angeordnet ist, und einen Katheter mit großem Durchmesser (z. B. Führungskatheter, Angiographiekatheter, Einführungsschleusenhülle, Gefäßdilatatoren) und mit einem Lumen, das mit einem proximalen Ende und einem Anschluß an seinem distalen Ende kommuniziert. Das distale Ende des Katheters ist zum Einführen in eine Arterie oder Vene geeignet. Das proximale Ende des Katheters kann ein hämostatisches Ventil aufweisen. Eine expandierbare Okklusionsvorrichtung, die einen Elastomerballon aufweisen kann, ist um das distale Ende des Führungskatheters angeordnet und kommuniziert mit einem Inflationslumen, um für Isolierung des Blutflusses im Gefäß zu sorgen. Das Lumen des Katheters ist geeignet, den Führungsdraht aufzunehmen, der den distalen Anschluß durchläuft. Ein proximales Ende des Führungsdrahts ist normalerweise von einer Stelle außerhalb des proximalen Endes des Katheters zum Manipulieren des Führungsdrahts unabhängig vom Katheter betätigbar. Außerdem ist das Lumen des Katheters geeignet, eine endovaskuläre Vorrichtung aufzunehmen, z. B. einen Angioplastie-, Stentsetzungs- oder Atherektomiekatheter.
- Normalerweise verfügen die endovaskulären Katheter über ein proximales Ende, ein distales Ende und ein Lumen, das den Führungsdraht aufnimmt. Ein Exzidierteil, z. B. eine Schneidklinge, ein abrasives Teil, ein Drahtschneider, Backen, Greifer, ein Brecher, eine Schlinge usw., ist am distalen Bereich eines Atherektomiekatheters vorgesehen. Ein expandierbarer Ballon ist an einem distalen Bereich eines Angioplastiekatheters vorgesehen. Ein expandierbarer Stent ist an einem distalen Bereich eines Stentsetzungskatheters angeordnet. Optional kann der Atherektomiekatheter ferner eine Einrichtung zur intravaskulären Abbildung aufweisen, z. B. einen Ultraschallwandler. In bestimmten Ausführungsformen können die Angioplastiekatheter einen Stent aufweisen, der um den Ballon an ihrem distalen Bereich angeordnet ist. Intravaskuläre Abbildungsvorrichtungen und Stents sind in der Technik vollständig beschrieben und werden hier nicht weiter diskutiert.
- In einer weiteren Ausführungsform weist das expandierbare Filter einen Expansionsrahmen und ein über dem Rahmen angeordnetes Maschenmaterial auf. In bestimmten Ausführungsformen weist der Rahmen mehrere Streben auf, die mit dem Führungsdraht an einem ersten Ende verbunden sind, und die Streben expandieren an einem zweiten Ende radial nach außen. Der Rahmen kann eine Inflationsdichtung zur Herstellung eines besseren Kontakts mit den Gefäßwänden aufweisen. Der Aufbau und Gebrauch von Expansionseinrichtungen und zugehörigem Filtermaschenmaterial sind in früheren Anmeldungen ausführlich diskutiert, u. a. in der US-Anmeldung Nr. 08/533137 (Barbut et al.), eingereicht am 7. November 1995, US-Anmeldung Nr. 08/580223 (Barbut et al.), eingereicht am 28. Dezember 1995, US-Anmeldung Nr. 08/584759 (Barbut et al.), eingereicht am 9. Januar 1996, US-Anmeldung Nr. 08/640015 (Barbut et al.), eingereicht am 30. April 1996, US-Anmeldung Nr. 08/645762 (Barbut et al.), eingereicht am 14. Mai 1996 und der US-A-5662671 (Barbut et al.).
- In noch einer weiteren Ausführungsform weist der Führungskatheter einen Infusionsanschluß proximal zum Okklusionsballon auf. Der Anschluß kommuniziert mit einem Infusionslumen im Katheter und ist zur Infusion von Fluid oder Pharmazeutika geeignet. Mit Hilfe des Infusionsanschlusses kann die Dosierung eines Pharmazeutikums, die zur Erreichung eines lo kalen Effekts erforderlich ist, gegenüber der Verabreichung auf systemischem Weg reduziert werden. Auch lassen sich Nebenwirkungen minimieren, z. B. Blutungen im Zusammenhang mit systemischer Verabreichung von t-PA. In bestimmten Ausführungsformen kann der Angioplastiekatheter einen Infusionsanschluß proximal zum Angioplastieballon und einen Perfusionsanschluß distal zum Ballon aufweisen. Der Infusions- und Perfusionsanschluß kommunizieren mit einem Infusions- bzw. Perfusionslumen im Angioplastiekatheter. Der Infusionsanschluß ist zur Aspiration von Fluid, Blut, Luft oder Gefäßtrümmern geeignet.
- Das Gefäßfiltersystem der Erfindung kann in Verfahren zum Schutz eines Patienten vor Embolisierung während eines endovaskulären Verfahrens verwendet werden, um Plaque und/oder Thromben aus folgendem zu entfernen: Koronararterie, Aorta, Arteria carotis communis, Arteria carotis externa und interna, Truncus brachiocephalicus, Arteria cerebri media, Arteria basilaris, Arteria subclavia, Arteria brachialis, Arteria axillaris, Arteria iliaca, Arteria renalis, Arteria femoralis, Arteria poplitea, Arteria coeliaca, Arteria mesenterica superior, Arteria mesenterica inferior, Arteria tibialis anterior, Arteria tibialis posterior und allen anderen, sauerstoffangereichertes Blut führenden Arterien. Die Verfahren weisen auch das Verhindern distaler Embolisierung während eines endovaskulären Verfahrens auf, um Thromben und/oder Fremdkörper im venösen Blutkreislauf zu entfernen, u. a. in der Vena cava superior, Vena cava inferior, Venae jugulares externi und interni, Vena brachiocephalica, Vena pulmonalis, Vena subclavia, Vena brachialis, Vena axillaris, Vena iliaca, Vena renalis, Vena femoralis, Vena profunda femoris, Vena saphena magna, Vena portae, Vena splenica, Vena hepatica und Vena azygos.
- In einem ersten Verfahren zur Verwendung des Gefäßfiltersystems wird das distale Ende des Führungsdrahts mit dem Filter in einem zusammengelegten Zustand durch eine Arterie oder Vene eingeführt. Das Filter und der distale Bereich des Führungsdrahts werden im Gefäß jenseits eines interessierenden Bereichs positioniert, woran sich die Expansion des Filters anschließt. Der Führungskatheter wird über dem Führungsdraht vorgeschoben, und der Okklusionsballon wird proximal zum interessierenden Bereich positioniert. Der distale Bereich eines Atherektomie-, Stentsetzungs- oder Angioplastiekatheters wird über dem Führungsdraht eingeführt, wobei der Führungsdraht im Lumen des Führungskatheters mitgeführt wird, und zum interessierenden Bereich vorgeschoben. Anschließend wird der Okklusionsballon expandiert, um den Blutfluß im Gefäß zu isolieren, während der endovaskuläre Katheter die stenotische Läsion im Gefäßlumen entfernt oder anderweitig behandelt. Danach kann der Katheter herausgezogen oder an Ort und Stelle belassen werden, und Fluid oder Blut wird durch das Lumen des Führungskatheters infundiert, um embolische Trümmer in das expandierte Filter zu spülen. In bestimmten Ausführungsformen wird das Fluid als Strahl zum Atheroma geleitet, um so das Atheroma von der Wand des Gefäßes und danach in das Filter abzustrahlen. Die Schritte des Einführens des endovaskulären Katheters und des Infundierens von Fluid, um embolische Trümmer auszuspülen, können wiederholt werden, bis ein ausreichender Lumendurchmesser hergestellt ist. Danach wird das Filter zusammengelegt und gemeinsam mit den eingefangenen embolischen Trümmern durch Herausziehen des Führungsdrahts aus dem Gefäß entfernt. Der Führungskatheter wird herausgezogen, nachdem der Okklusionsballon deflatiert ist.
- In einem weiteren Verfahren wird nach Positionierung des expandierten Filters und des Führungskatheters in einem Gefäß distal bzw. proximal zum interessierenden Bereich der durch den Angioplastiekatheter mitgeführte Angioplastieballon inflatiert, um das stenotische Gefäßlumen zu dilatieren. Blut, Fluid, Luft und/oder embolische Trümmer, die zwischen dem Okklusions- und Angioplastieballon vorhanden sind, können abgesaugt werden. Alternativ kann der Okklusionsballon während der Inflation des Angioplastieballons deflatiert werden, damit Blut aus dem proximalen Anschluß aspiriert und zum Perfusionsanschluß distal zum Angioplastieballon geleitet werden kann, wodurch die Perfusion zu den distalen Organen während der Angioplastie erhalten bleibt.
- Verständlich wird sein, daß es mehrere Vorteile beim Gebrauch der hierin offenbarten Vorrichtungen und Verfahren zum Einfangen und Entfernen embolischer Trümmer im Verlauf von endovaskulären Eingriffen gibt. Beispielsweise ist das Filterspülsystem (1) besonders gut zur temporärem Blutfilterung in jedem Gefäß geeignet, um embolische Trümmer einzufangen, was neurologische, kognitive und kardiale Komplikationen im Zusammenhang mit distaler Embolisierung minimiert, kann (2) hohem arteriellem Blutfluß längere Zeit widerstehen, weist (3) ein Maschenmaterial auf, das porös genug ist, um ausreichenden Blutfluß in einem Blutgefäß zu ermöglichen, während es mobile Emboli einfängt, ist (4) geeignet, einem endovaskulären Katheter mit oder ohne Abbildungsvorrichtung Rechnung zu tragen, kann (5) mobile Plaque in einem Gefäß durch Spülen durch den Führungskatheter entfernen, stellt (6) eine Einrichtung bereit, um Perfusion zu distalen Organen während endovaskulärer Verfahren zu wahren, stellt (7) eine Einrichtung bereit, um Pharmazeutika, z. B. Gewebe-Plasminogen-Aktivator oder Nitroglycerin, lokal zum interessierenden Bereich zu verabreichen, was Nebenwirkungen im Zusammenhang mit systemischer Verabreichung minimiert, und kann (8) bei Patienten im Erwachsenen- und Kindesalter zum Einsatz kommen.

- In einer ersten Ausführungsform wird ein Filtersystem zur temporären Plazierung in einem Gefäß, entweder einer Arterie oder einer Vene, gemäß
1A und1B bereitgestellt. Das Filtersystem verfügt über einen Führungsdraht10 mit einem proximalen Ende, einem distalen Ende11 und einem expandierbaren Filter20 , das am distalen Ende angeordnet ist. Das Filter weist einen Schirmrahmen22 und ein Maschenmaterial25 auf, das mit Streben26 des Schirmrahmens schallverschweißt oder verklebt ist. Antikoagulanzien, z. B. Heparin und Hepa rinoide, können auf das Maschenmaterial25 aufgetragen sein, um Thrombenbildung auf dem Maschenmaterial zu reduzieren. Das Filter kann gemäß1A zusammengefaltet sein, um das Einführen in ein Gefäß zu erleichtern, und anschließend gemäß1B entfaltet werden. Vielfältige geeignete Filterführungsdrähte zum Gebrauch hierin sind in der US-A-5910154 (Tsugita et al.) offenbart. - Ferner verfügt das Filtersystem über einen Führungskatheter
30 mit einem Lumen33 , das mit einem proximalen Ende und einem distalen Ende31 kommuniziert, und einen Okklusionsballon40 , der um das distale Ende angeordnet ist. Der Ballon40 kommuniziert mit einem Inflationslumen41 , das geeignet ist, Fluid oder Luft zur Expansion des Ballons aufzunehmen. Das Lumen33 des Katheters kommuniziert mit einem distalen Anschluß35 und ist geeignet, den Führungsdraht10 und andere endovaskuläre Vorrichtungen aufzunehmen, z. B. Atherektomiekatheter, endovaskuläre Abbildungsvorrichtungen, Stentsetzungskatheter, Angioplastiekatheter, Drucküberwachungsvorrichtungen, Elektrophysiologiekatheter und Aspiratoren. - Im Gebrauch werden gemäß
2A ,2B und2C das Filter20 (im zusammengelegten Zustand) und das distale Ende11 des Führungsdrahts durch eine periphere Arterie oder Vene perkutan eingeführt, normalerweise in Blutflußrichtung (erwogen ist aber auch, daß der Führungsdraht10 in Gegenrichtung zum Blutfluß eingeführt werden kann). Das Filter20 wird distal zu atheromatöser Plaque100 vorgeschoben und expandiert, um die Gefäßwände101 gemäß2A zu kontaktieren. Danach wird der Führungskatheter30 über dem Führungsdraht10 eingeführt, bis der distale Anschluß35 proximal zur Plaque100 positioniert ist. Ein Atherektomiekatheter mit einer an einem distalen Bereich angeordneten Atherektomievorrichtung42 wird im Lumen33 des Katheters und über dem Führungsdraht10 eingeführt. Die Atherektomievorrichtung42 wird distal zum An schluß35 des Katheters zu einer Position benachbart zur Plaque100 vorgeschoben. Der Okklusionsballon40 wird inflatiert, um den Blutfluß im Gefäß zu isolieren, während die Atherektomievorrichtung42 Plaque100 gemäß2B entfernt. - Nachdem eine ausreichende Lumengröße durch Atherektomie erreicht ist, wird der Okklusionsballon
40 deflatiert, wodurch der Blutfluß im interessierenden Bereich wieder hergestellt werden kann, um das im Procedere erzeugte embolische Material zum Filter20 zu schieben und distale Organe zu perfundieren. Der Okklusionsballon40 kann erneut inflatiert werden, um den Blutfluß für eine wiederholte Atherektomie oder ein anderes endovaskuläres Verfahren, z. B. Aspiration, zu isolieren. Nach Abschluß des Verfahrens wird der Atherektomiekatheter aus dem Führungskatheter30 gemäß2C herausgezogen. Fluid oder Blut wird durch das Lumen33 des Führungskatheters infundiert, um das embolische Material auszuspülen, das als Ergebnis des strömungsarmen Zustands zum Filter20 nicht beräumt wurde. Dies kann mit gesetztem oder nicht gesetztem Ballon40 geschehen. Die Fluidspülung kann auch auf die Gefäßwand gerichtet sein, um lose haftendes atheromatöses Material abzustrahlen, das nach der Atherektomie verbleibt. In bestimmten Fällen kann das Filter mit übermäßigen Mengen von Material beladen werden. In diesem Fall kann erwünscht sein, einen separaten Aspirationskatheter durch den Führungskatheter so einzusetzen, daß er sich distal zum Filter erstreckt und dadurch so arbeitet, daß er im Maschenmaterial eingefangenes embolisches Material absaugt. Der Führungskatheter30 wird aus dem Gefäß entfernt. Das Filter20 mit den eingefangenen embolischen Trümmern wird zusammengelegt und durch Herausziehen des Führungsdrahts10 aus dem Gefäß entfernt. Alternativ kann der Führungsdraht in den Führungskatheter30 eingezogen werden, wonach beide Vorrichtungen gleichzeitig herausgezogen werden. In einer weiteren Ausführungsform kann der Führungsdraht eine separate Aufnahme hülle haben, um bei seiner Plazierung und Entfernung durch Einziehen des Führungsdrahts in die Hülle zu helfen, bevor er plaziert oder entfernt wird. - Soll beispielsweise das hier offenbarte Filtersystem in der Aorta verwendet werden, berechnet sich die für die Vorrichtung erforderliche Fläche des Maschenmaterials nach der Bernoulli-Gleichung gemäß der Beschreibung in unseren früheren Anmeldungen, u. a. der US-Anmeldung Nr. 08/533137 (Barbut et al.), eingereicht am 7. November 1995, US-Anmeldung Nr. 08/580223 (Barbut et al.), eingereicht am 28. Dezember 1995, US-Anmeldung Nr. 08/584759 (Barbut et al.), eingereicht am 9. Januar 1996, US-Anmeldung 08/640015 (Barbut et al.), eingereicht am 30. April 1996 sowie der US-Anmeldung Nr. 08/645762 (Barbut et al.), eingereicht am 14. Mai 1996.
- In einer Ausführungsform des Gefäßfiltersystems, die in der Aorta zu verwenden ist, ist Maschenmaterial mit Maßen in den folgenden Bereichen erwünscht: die Maschenfläche beträgt 2,58 mm2–32,3 cm2 (0,004-5 Inch2), stärker bevorzugt 4,52 mm2–25,8 cm2 (0,007-4 Inch2), stärker bevorzugt 6,45 mm2–19,4 cm2 (0,010-3 Inch2), stärker bevorzugt 9,68 mm2–12,9 cm2 (0,015-2 Inch2), stärker bevorzugt 12,9 mm2–6,45 cm2 (0,020-1 Inch2), stärker bevorzugt 16,1 mm2–0,49 cm2 (0,025–0,076 Inch2); die Maschendicke beträgt 60–280 μm, stärker bevorzugt 70–270 μm, stärker bevorzugt 80–260 μm, stärker bevorzugt 90–250 μm, stärker bevorzugt 100–250 μm, stärker bevorzugt 120–230 μm, stärker bevorzugt 140–210 μm; der Fadendurchmesser beträgt 30–145 μm, stärker bevorzugt 40–135 μm, stärker bevorzugt 50–125 μm, stärker bevorzugt 60–115 μm, stärker bevorzugt 70–105 μm; und die Porengröße beträgt höchstens 500 μm, stärker bevorzugt höchstens 400 μm, stärker bevorzugt höchstens 300 μm, stärker bevorzugt höchstens 200 μm, stärker bevorzugt höchstens 100 μm, stärker bevorzugt höchstens 50 μm und ist gewöhnlich größer als mindestens ein rotes Blutkörperchen. In einer be vorzugten Ausführungsform der Erfindung beträgt die Maschenfläche 12,9–51,4 cm2 (2–8 Inch2), die Maschendicke beträgt 60–200 μm, der Fadendurchmesser beträgt 30–100 μm, und die Porengröße beträgt 50–300 μm. In einer weiteren bevorzugten Ausführungsform der Erfindung beträgt die Maschenfläche 19,4–32,3 cm2 (3–5 Inch2), die Maschendicke beträgt 60–150 μm, der Fadendurchmesser beträgt 50–80 μm, und die Porengröße beträgt 100–250 μm.
- In anderen Ausführungsformen weist das Filter eine dünne Folie mit lasergeschnittenen Löchern auf, um den Blutfluß zu ermöglichen. Zu typischen Maßen gehören eine Porengröße von 20–500 μm, eine Dicke von 0,013–0,076 mm (0,0005–0,003 Inch) und eine Fläche, die etwa die gleiche wie für die zuvor beschriebenen Maschenmaterialien ist.
- Sobald geeignete physikalische Kennwerte bestimmt sind, kann geeignetes Maschenmaterial aus technisch bekannten Standard-Maschenmaterialien ermittelt werden. Beispielsweise können Polyester-Maschenmaterialien verwendet werden, z. B. von Saati Corporations und Teko Inc. hergestellte Maschenmaterialien. Diese sind in Bahnenform erhältlich und lassen sich leicht schneiden und in eine gewünschte Form bringen. In einer bevorzugten Ausführungsform ist das Maschenmaterial zu einer Kegelform schallverschweißt. Geeignet sind auch andere technisch bekannte Maschenmaterialien mit den gewünschten physikalischen Kennwerten. Antikoagulanzien, z. B. Heparin und Heparinoide, können auf das Maschenmaterial aufgetragen sein, um die Möglichkeiten zu verringern, daß Blut am Maschenmaterial gerinnt. Zum Einsatz können auch andere Antikoagulanzien als Heparinoide kommen, z. B. monoklonale Antikörper wie ReoPro (Centocor). Das Antikoagulans kann auf das Maschenmaterial durch Anstrich oder Aufspritzen aufgetragen sein. Ein chemisches Tauchbad mit dem Antikoagulans kann ebenfalls verwendet werden. Andere in der Technik bekannte Verfahren zum Auftragen von Chemikalien auf Maschenmaterial können genutzt werden.

3A ,3B und3C zeigen eine weitere Ausführungsform des Filterspülsystems zur temporären Plazierung in einem Gefäß. Das Maschenmaterial25 des Filters20 ist mit der Inflationsdichtung27 an einer ersten Kante und einer Endplatte28 an einer zweiten Kante betrieblich verbunden. Die Inflationsdichtung ist zwischen einem kontrahierten Zustand und einem vergrößerten Zustand expandierbar. Im Gebrauch können in einem kontrahierten Zustand die Inflationsdichtung27 und das Maschenmaterial25 durch ein peripheres Gefäß in einen interessierenden Bereich, normalerweise distal zur atheromatösen Plaque100 , gemäß3A eingeführt werden. Die Inflationsdichtung wird durch Einspritzen von Flüssigkeit oder Gas expandiert, um einen Kontakt mit der Gefäßinnenwand101 zu erreichen. Der Führungskatheter30 wird über dem Führungsdraht10 eingeführt, und der distale Anschluß35 wird proximal zur Plaque100 positioniert. - Die perkutane transluminale Angioplastie war erfolgreich beim behandeln arterieller Stenose sowie thrombolysenresistenter okklusiver Venenthrombose. Siehe dazu American Heart Journal
125 (2 Pt 1):362-6 (1993). In3B ist ein Angioplastiekatheter50 mit einem an einem distalen Bereich angeordneten Angioplastieballon52 durch das Lumen33 des Führungskatheters über dem Führungsdraht10 eingeführt. In einem deflatierten Zustand wird der Angioplastieballon durch den Anschluß35 zu einer Position benachbart zur Plaque100 vorgeschoben. Durch Inflatieren des Ballons52 wird die atheromatöse Plaque komprimiert, was die Stenose im Gefäß dilatiert. - In bestimmten Ausführungsformen weist der Angioplastiekatheter einen Infusionsanschluß
54 proximal und einen Perfusionsanschluß55 proximal bzw. distal zum Ballon52 auf. Der Infusionsanschluß54 kann verwendet werden, Pharmazeutika, z. B. t-PA oder Nitroglycerin, zu verabreichen und Luft, Thromben, Plaque und/oder Gewebstrümmer zu aspirieren. Während der Angioplastie kann der Ballon40 inflatiert oder deflatiert sein. Sauerstoffangereichertes Medium oder Blut kann durch den Anschluß55 infundiert werden, um für Perfusion zu distalen Organen im Angioplastieverlauf zu sorgen und das Spülen von embolischem Material in das Filter zu erleichtern. - In bestimmten Ausführungsformen ist eine Prothese, z. B. ein Stent, dem Angioplastieballon eng zugeordnet. Normalerweise ist der Stent auf den Ballon gecrimpt und kann eine gesteuerte Radialexpansion in einem interessierenden Bereich bei Ausübung einer auswärts gerichteten Radialkraft aus dem Inneren des Stents vollführen. Der Aufbau eines Kathetersystems, das einen Stent mitführt, ist in der US-A-5749848 (Jang et al.) näher beschrieben.
- Danach kann der Angioplastiekatheter aus dem Gefäß durch das Lumen
33 des Führungskatheters nach Angioplastieabschluß gemäß3C herausgezogen werden. Der Okklusionsballon40 wird deflatiert, um den Blutfluß wieder herzustellen. Vor oder nach Deflation des Ballons40 kann Fluid oder Blut durch das Lumen33 und den Anschluß35 infundiert werden, um embolisches Material in das Filter20 zu spülen. Nachdem embolisches Material im Filter eingefangen ist und festgehalten wird, wird der Führungskatheter30 aus dem Gefäß entfernt. Danach wird das Filter durch Ablassen der Inflationsdichtung27 kontrahiert und mit dem eingefangenen embolischen Material aus dem Gefäß herausgezogen und aus dem Patientenkörper entfernt. - Eine weitere Ausführungsform des Filterspülsystems ist in
4A ,4B und4C gezeigt. Das Filter20 hat die Form eines vorgeformten Korbs mit dem Maschenmaterial25 in betrieblicher Verbindung mit der Inflationsdichtung27 . Durch Infundieren von Gas oder Flüssigkeit kann die Inflationsdichtung kontrahiert und expandiert werden. Im Gebrauch werden in ei nem kontrahierten Zustand die Inflationsdichtung27 und das Maschenmaterial25 durch ein peripheres Gefäß distal zur atheromatösen Plaque100 gemäß4A eingeführt. Die Inflationsdichtung wird expandiert, um die Gefäßinnenwand101 zu kontaktieren. Der Führungskatheter30 wird über dem Führungsdraht10 eingeführt, wobei der distale Anschluß35 proximal zur Plaque100 positioniert wird. - Primärstentsetzung für komplexe atherosklerotische Plaque war bei der Behandlung von Aorten- und Hüftarterienstenosen wirksam, die nicht für die Ballonangioplastie empfänglich sind. Siehe dazu Onal, et al., Cardiovascular Interventional Radiology 21(5):386-92 (1998). Katheterintervention im venösen System mittels expandierbarer Metallstents war bei der Behandlung des Vena-cava-superior-Syndroms erfolgreich, das hauptsächlich mit malignen Tumoren zusammenhängt und anderen Therapien gegenüber resistent ist, u. a. Ballonangioplastie und operativer Eingriff. Siehe dazu Nakanishi, et al., Rinsho Kyoby Geka 14(2):110-4 (1994). Ein endovaskulärer Katheter
65 , der mit einem selbstexpandierenden Stent60 ausgerüstet ist, der an einem distalen Bereich angeordnet ist, wird durch das Lumen33 des Führungskatheters über dem Führungsdraht10 gemäß4B eingeführt. Der Stent kann sich aus einem formstabilen Material zusammensetzen, z. B. Nitinol. Der Katheter65 weist eine Einrichtung zum Festhalten des selbstexpandierenden Stents auf. Die Festhalteeinrichtung kann die Form einer Hülle annehmen, die um den distalen Bereich des Katheters angeordnet ist, so daß der Stent der Hülle betrieblich zugeordnet und darin enthalten ist, und bei Entfernung der Hülle wird der Stent freigesetzt und automatisch auf einen expandierten Durchmesser vergrößert. In4B und4C ist die Plaque100 durch den selbstexpandierenden und thermisch aktivierbaren Stent60 reduziert dargestellt. Der Aufbau und das Setzen eines selbstexpandierenden Stents sind in der US-A-5224953 (Morgentaler) offenbart. - In
4C wird nach Setzen des Stents60 über der Plaque100 der endovaskuläre Katheter65 entfernt, wodurch das Lumen33 des Führungskatheters30 zur Infusion von Fluidmedium verfügbar bleibt. Eventuell stellt man fest, daß mobile Plaque, womit Gefäßablagerungen bezeichnet sind, die eine verfestigte Basis und einen an der Basis haftenden schlaffen Fortsatz haben und durch normalen pulsierenden Blutfluß herausgelöst werden können, durch das Maschenmaterial des Stents60 vorsteht. Durch Fluidspülung durch den Stent60 kann die mobile Plaque herausgelöst und mit dem Filter20 eingefangen werden. Nach Einfangen und Festhalten von embolischem Material im Filter wird der Führungskatheter30 aus dem Gefäß entfernt. Das Filter20 wird vor oder nach Entfernen des Führungskatheters durch Ablassen der Inflationsdichtung27 zusammengelegt und mit eingefangenem embolischem Material aus dem Gefäß herausgezogen und aus dem Patientenkörper entfernt. -
5 zeigt eine weitere Ausführungsform des Gefäßfiltersystems mit einem Infusionsanschluß70 , der proximal zum Okklusionsballon40 liegt und mit dem Lumen33 des Führungskatheters30 kommuniziert. Der Anschluß70 ermöglicht, daß Fluid aufgenommen wird und Blut von der proximalen Seite des Okklusionsballons fließt und aus dem distalen Anschluß35 des Katheters austritt. Um für Perfusion zu distalen Organen während eines endovaskulären Verfahrens zu sorgen. In bestimmten Ausführungsformen kann der Führungskatheter mehrere Infusionsanschlüsse proximal zum Okklusionsballon sowie optional ein Einwegventil am Infusionsanschluß aufweisen, um retrograden Blutfluß auszuschließen. In bestimmten Ausführungsformen kann der Führungskatheter einen (mehrere) Aspirationsanschluß (-anschlüsse) distal zum Okklusionsballon zum Aspirieren von Gefäßtrümmern aufweisen, die im endovaskulären Procedere erzeugt werden. - Die Länge des Führungskatheters und Führungsdrahts liegt allgemein zwischen 15 und 200 Zentimetern, vorzugsweise etwa zwischen 50 und 150 Zentimetern. Der Innendurchmesser des Katheterlumens liegt allgemein zwischen 1,0 und 7 Millimetern, vorzugsweise etwa zwischen 1,5 und 2,6 Millimetern. Der Durchmesser eines expandierten Okklusionsballons liegt allgemein zwischen 1,5 und 50,0 Millimetern, vorzugsweise etwa zwischen 3,0 und 8,0 Millimetern. Diese Bereiche sind lediglich zur Veranschaulichung typischer Vorrichtungsmaße aufgeführt. Die Ist-Maße einer gemäß den Grundsätzen aufgebauten Vorrichtung können natürlich außerhalb der aufgeführten Bereiche variieren, ohne von diesen Grundsätzen abzuweichen.
- Obwohl die vorstehende Erfindung zwecks Deutlichkeit und Verständnis anhand von Veranschaulichungen und Beispielen näher beschrieben wurde, wird klar sein, daß bestimmte Änderungen und Abwandlungen vorgenommen werden können, die dennoch in den Schutzumfang der beigefügten Ansprüche fallen. Zum Beispiel wird verständlich sein, daß jede Art von Expansionsrahmen gemäß der Offenbarung hierin oder in früheren Anmeldungen mit jedem der therapeutischen Interventionskatheter verwendet werden kann, wenngleich eine bestimmte Zeichnung möglicherweise nur eine spezielle Kombination zeigt. Außerdem können andere Okklusionsvorrichtungen als Ballons mit jeder der hierin offenbarten Ausführungsformen zum Einsatz kommen.
Claims (16)
- Gefäßfiltersystem mit einem Führungskatheter (
30 ) mit einem proximalen Endbereich, einem distalen Endbereich (31 ) und einem Innenlumen (33 ); einem therapeutischen Katheter, der im Innenlumen (33 ) des Führungskatheters (30 ) angeordnet ist, wobei der therapeutische Katheter ein Führungsdrahtlumen definiert; einem Führungsdraht (10 ), der im Führungsdrahtlumen des therapeutischen Katheters gleitfähig angeordnet ist; einem expandierbaren Filter (20 ), das mit dem Führungsdraht (10 ) gekoppelt ist; einem Okklusionsballon (40 ), der mit dem distalen Endbereich (31 ) des Führungskatheters (30 ) gekoppelt ist; und einem therapeutischen Element (42 ,52 ,60 ), das mit dem therapeutischen Katheter gekoppelt und distal zum Okklusionsballon (40 ) positioniert ist. - System nach Anspruch 1, wobei der therapeutische Katheter einen Atherektomiekatheter aufweist und wobei das therapeutische Element ein Exzidierteil (
42 ) aufweist. - System nach Anspruch 1, wobei der therapeutische Katheter einen Angioplastiekatheter aufweist und wobei das therapeutische Element einen Angioplastieballon (
52 ) aufweist. - System nach Anspruch 3, ferner mit einem Stent (
60 ), der um den Angioplastiekatheter angeordnet ist. - System nach Anspruch 1, wobei der therapeutische Katheter einen Stentsetzungskatheter aufweist und wobei das therapeutische Element einen selbstexpandierenden Stent (
60 ) aufweist. - System nach Anspruch 1, wobei das expandierbare Filter (
20 ) einen Expansionsrahmen (22 ) und ein über dem Rahmen (22 ) angeordnetes Maschenmaterial (25 ) aufweist. - System nach Anspruch 6, wobei das Maschenmaterial (
25 ) gewebt ist. - System nach Anspruch 6, wobei das Maschenmaterial (
25 ) eine dünne Folie mit Löchern aufweist, um Fluidfluß zu ermöglichen. - System nach Anspruch 6, wobei der Expansionsrahmen (
22 ) mehrere Streben (26 ) aufweist, die mit dem Führungsdraht (10 ) an einem ersten Ende verbunden sind, und der an einem zweiten Ende radial nach außen expandiert. - System nach Anspruch 6, wobei der Expansionsrahmen (
22 ) eine Inflationsdichtung (27 ) aufweist. - System nach Anspruch 1, wobei der Führungskatheter (
30 ) einen Aspirationsanschluß distal zum Okklusionsballon aufweist, wobei der Aspirationsanschluß mit einem Aspirationslumen kommuniziert. - System nach Anspruch 1, wobei der Okklusionsballon (
40 ) mit einem Inflationslumen kommuniziert. - System nach Anspruch 1, wobei der Führungskatheter (
30 ) einen Infusionsanschluß (70 ) proximal zum Okklusionsballon (40 ) aufweist. - System nach Anspruch 13, wobei der Infusionsanschluß (
70 ) mit dem Innenlumen (33 ) des Führungskatheters (30 ) kommuniziert. - System nach Anspruch 3, wobei der therapeutische Katheter einen Infusionsanschluß (
54 ) proximal zum therapeutischen Element (42 ,52 ,60 ) aufweist. - System nach Anspruch 15, wobei der Infusionsanschluß (
54 ) mit einem Perfusionsanschluß (55 ) distal zum therapeutischen Element (42 ,52 ,60 ) kommuniziert.
