Description
- Gebiet der Erfindung
- Die vorliegende Erfindung betrifft allgemein medizinische Vorrichtungen und insbesondere eine medizinische Ultraschallvorrichtung, um auf kontrollierte Weise Gewebe in einem menschlichen Körper zu zerstören.
- Beschreibung des Stands der Technik
- Medizinische Geräte, die Ultraschallenergie nutzen, um Gewebe in einem menschlichen Körper zu zerstören, sind in der Technik bekannt. Ein Nachteil von bestehenden medizinischen Ultraschallgeräten, die Gewebe entfernen, ist, daß Ärzte sie im Vergleich mit Methoden wie etwa der chirurgischen Entfernung typischerweise als langsam betrachten. Diese wahrgenommene Langsamkeit wird zum Teil dadurch erklärt, daß die meisten bestehenden Ultraschallgeräte auf einer Längsschwingung der Sondenspitze basieren. Anders ausgedrückt, die Sondenspitze schwingt in einer Richtung in einer Linie mit der Längsachse der Sonde. Dies erzeugt eine Gewebe zerstörende Wirkung nur an der Sondenspitze.
- Eine vorgeschlagene Lösung ist, die Sondenspitze, zusätzlich zu dem Schwingen der Spitze in Längsrichtung, in Transversalrichtung – d. h. senkrecht zu der Längsachse der Sonde -schwingen zu lassen. Beispielsweise beschreibt das US-Patent Nr. 4 961 424 von Kubota et al. eine Ultraschallbehandlungsvorrichtung zum Zerstören und Emulgieren von Verwachsungen oder Gewebe in einem menschlichen Körper. Die Vorrichtung von Kubota et al. erzeugt sowohl eine Längs- als auch eine Transversalbewegung an der Sondenspitze. Das Patent von Kubota et al. basiert jedoch immer noch allein darauf, daß die Sondenspitze als eine Arbeitsfläche wirksam ist. Deshalb verbessert es zwar den Wirkungsgrad der Spitze, basiert jedoch immer noch darauf, daß die Sondenspitze sämtliche Schneidvorgänge ausführt.
- Kubota et al. beschreiben zwar das Vorsehen einer Transversalbewegung an der Sondenspitze, von einer Transversalbewegung entlang der Länge der Sonde wird jedoch allgemein abgeraten. Beispielsweise beschreibt das US-Patent Nr. 4 474 180 von Angulo ein Ultraschallgerät zur Zertrümmerung von Nierensteinen, wobei ein Dämpfungsmaterial an der Drahtsonde angebracht ist, um Seitenschwingungen des Drahts in dem Bereich der Verbindung mit dem Ultraschallwandler zu unterbinden.
- Ein anders vorgeschlagenes Verfahren zur Verbesserung der Geschwindigkeit der Gewebeentfernung durch Ultraschall sieht vor, zusätzlich zu dem Schwingen der Sondenspitze in Längsrichtung die Sondenspitze zu oszillieren. Beispielsweise beschreibt das US-Patent Nr. 4 504 264 von Kelman eine Ultraschallbehandlungsvorrichtung, welche die Geschwindigkeit der Gewebeentfernung durch Ultraschall verbessert. Bei der Vorrichtung von Kelman wird die Sondenspitze in Längsrichtung zum Schwingen gebracht und außerdem oszilliert, so daß der Schneidwirkungsgrad der Sondenspitze verbessert wird. Wiederum führt jedoch nur die Sondenspitze einen Schneidvorgang aus.

EP 0 891 744 beschreibt eine Ultraschallübertragungsvorrichtung zur Verwendung bei medizinischen Anwendungen. Eine Aufgabe dieses Dokuments war es, eine Angioplastievorrichtung bereitzustellen, die während der Übertragung von Ultraschallenergie von einer geeigneten Quelle zu der Spitze eine erwünschte Biegsamkeit, maximale Spitzenverlagerung und minimale Energieverluste zeigt. - Kurze Zusammenfassung der Erfindung
- Es ist die Aufgabe der vorliegenden Erfindung, eine medizinische Ultraschallvorrichtung bereitzustellen, die imstande ist, Gewebe durch Kavitation im menschlichen Körper mit einem höheren Wirkungsgrad mittels einer biegsamen Sonde, die in einem Transversalmodus arbeitet, zu zerstören und zu emulgieren. Im vorliegenden Zusammenhang dient der Begriff "Transversalbetriebsmodus" dazu, eine biegsame Sonde mit einer Vielzahl von Schwingungsknoten und Schwingungsbäuchen entlang der Länge der Sonde zu beschreiben.
- Gemäß dieser Aufgabe stellt die vorliegende Erfindung eine medizinische Ultraschallvorrichtung bereit, die folgendes aufweist: einen Ultraschallgenerator (
15 ) zum Erzeugen einer Ultraschallschwingung in einer Richtung entlang einer Längsachse des Ultraschallgenerators;
einen Ultraschallkopplungstrichter (16 );
mindestens einen Wandlerabschnitt (18 ,20 ), der durch den Ultraschallkopplungstrichter (16 ) in Ultraschallverbindung mit der Ultraschallquelle (15 ) ist, wobei der Wandlerabschnitt (18 ,20 ) die Amplitude der Ultraschallschwingung modifiziert; und ein biegsames Element (22 ), das von dem Wandlerabschnitt (18 ,20 ) angetrieben wird, dadurch gekennzeichnet, daß das biegsame Element die Ultraschallschwingung in einem Transversalbetriebsmodus unterstützt, um eine Vielzahl von Schwingungsknoten und Schwingungsbäuchen entlang mindestens einem Bereich der Länge des biegsamen Elements (22 ) zu erzeugen, wobei der Bereich der Länge des biegsamen Elements im Gebrauch imstande ist, mit einem biologischen Material in Kommunikation zu sein. - Die Ultraschallschwingung wird durch einen Ultraschallkoppler und eine Serie von Wandlerabschnitten übertragen, welche die Ultraschallschwingung verstärken. Ein biegsames Element ist mit dem distalen Ende der Wandlerabschnitte gekoppelt und wird somit durch die Wandlerabschnitte an seiner Basis mit einer Längsschwingung versorgt. Das biegsame Element ist so ausgebildet, daß es die Längsschwingung in eine stehende Welle umwandelt, die entlang der Länge des biegsamen Elements verläuft. Die stehende Welle erzeugt eine Serie von Schwingungsknoten und Schwingungsbäuchen entlang der Länge des biegsamen Elements. Jeder von den Schwingungsbäuchen erzeugt eine Kavitation in Fluiden, die mit der Sonde in Kontakt sind. Die Kavitation der Fluide bewirkt die Zerstörung von benachbartem Gewebe. Auf diese Weise wird also die gesamte Länge des biegsamen Elements zu einer Arbeitsfläche, die genutzt werden kann, um Gewebe zu zerstören.
- Im Gegensatz zu bekannten Ausbildungen, die nur eine Sondenspitze als eine Oberfläche nutzen, bildet deshalb bei der vorliegenden Erfindung die gesamte Länge des biegsamen Elements eine Schneidfläche.

- Wie die
1 und3 zeigen, hat die Ultraschallsonde einen Griffabschnitt10 und einen Sondenabschnitt12 . Der Griff ist von einem Ultraschalltreiber14 und einem Ultraschallkopplungstrichter16 gebildet. Der Ultraschalltreiber hat eine Längsachse. Der Treiber erzeugt eine Ultraschallschwingung im Bereich von 20 bis 80 kHz. Die Nennamplitude des Treibers ist 50 μm bei einer sinusförmigen Erregung von Spitze zu Spitze von 100 V. Die Schwingung tritt entlang der Richtung der Längsachse auf. Bei der gezeigten Ausführungsform ist der Wandler PZT-4. Der Treiber kann jedoch zahlreiche Methoden nutzen, um eine Ultraschallschwingung zu erzeugen, wie etwa piezoelektrische, magnetostriktive, pneumatische oder hydraulische Methoden, die dem Fachmann bekannt sind. Eine Steuereinheit (nicht gezeigt) steuert den Ultraschalltreiber. Die Steuereinheit ermöglicht es dem Bediener, die Frequenz und Amplitude der von dem Treiber erzeugten Schwingung einzustellen. Bei der hier gezeigten beispielhaften Ausführungsform ist die Sonde ausgebildet, um bei einer Frequenz von 20 kHz zu arbeiten. Die Sonde kann jedoch ausgebildet sein, um bei Frequenzen im Bereich von 20 kHz bis 80 kHz zu arbeiten, wie in dem Abschnitt über die Betriebstheorie im einzelnen erläutert wird. - Der Ultraschalltreiber ist mit einem Kopplungstrichter
16 gekoppelt, und die Ultraschallschwingung wird von dem Treiber zu dem Kopplungstrichter übertragen. Der Kopplungstrichter ist mit dem Sondenabschnitt12 verbunden. Der Sondenabschnitt hat eine Serie von Wandlerabschnitten18 ,20 . Die Wandlerabschnitte sind eine Serie von Achsen, die aus einem geeigneten Material wie etwa Ti-6Al-4v-Titan gebildet sind. Die Wandlerabschnitte übertragen Schwingungen von dem Kopplungstrichter zu einem biegsamen Element22 an dem distalen Ende des Sondenabschnitts. Bei dem Übertragungsvorgang wird die Amplitude der Schwingung durch die Wandlerabschnitte verstärkt. Der Durchmesser der Wandlerabschnitte ist so gewählt, daß ein geeigneter Grad an Längsschwingung an dem Ende des Wandlerabschnitts erzeugt wird. Die Verstärkung der Wandlerabschnitte wird durch das Verhältnis der Fläche der Abschnitte gesteuert. Bei der hier beschriebenen beispielhaften Ausführungsform sind die Wandlerabschnitte ausgebildet, um eine Verstärkung von ungefähr 4 bis 5 über den Wandler zu erzeugen. Dies wird durch Einstellen des Durchmessers der Wandlerabschnitte18 ,20 auf 0,150 bzw. 0,080 inches erreicht. Die Länge der Wandlerabschnitte18 ,20 ist 1,500 bzw. 7,554 inches. Der Wandlerabschnitt18 hat einen Gewindebereich24 , um mit dem Kopplungstrichter16 zusammenzupassen, und hat einen Bereich26 , der so ausgebildet ist, daß er mit einem Schraubenschlüsse oder einem anderen Werkzeug gegriffen werden kann, um die Verbindung festzuziehen. - Ein biegsames Element
22 ist an dem Ende des letzten Wandlerabschnitts angebracht und wird von dem letzten Wandlerabschnitt angetrieben. Das biegsame Element ist eine dünne, drahtartige Sonde, deren Durchmesser typischerweise kleiner als 1 mm ist. Bei der gezeigten Ausführungsform hat das biegsame Element einen Kreisquerschnitt mit einem Durchmesser von 0,51 mm (0,020 inches). Das biegsame Element kann andere Querschnitte wie etwa einen rechteckigen oder ovalen Querschnitt haben. Der biegsame Bereich der Sonde kann viele Wellenlängen lang sein. Bei der gezeigten Ausführungsform ist das biegsame Element 102,8 mm (4,046 inches) lang, was einer Vorrichtung entspricht, die mit einer Frequenz von ungefähr 20 kHz arbeitet. Das bevorzugte Material ist 6Al-4v-Titan; es kann jedoch jedes andere Material verwendet werden, solange die Betriebsparameter innerhalb der Betriebsgrenzen liegen, die durch die Festigkeit des Materials vorgegeben sind, wie noch im einzelnen erläutert wird. -
4 zeigt eine Querschnittsansicht der Griffanordnung einer Ultraschallsonde, die gemäß den Prinzipien der Erfindung ausgebildet ist.4 zeigt eine beispielhafte Trichteranordnung400 , die zur Verwendung bei der vorliegenden Erfindung geeignet ist. Ein Gehäuse402 hat eine Endkappe404 und einen hinteren Bereich406 . Die Endkappe404 hat einen inneren Gewindebereich408 , der mit einem äußeren Gewindebereich410 des hinteren Bereichs406 zusammenpaßt. Der hintere Bereich406 des Gehäuses402 hat eine Ausnehmung412 , die einen verlängerten Bereich414 des Trichters416 aufnimmt. Die Endkappe404 hat die Gestalt eines Rings mit einer Öffnung408 . Der Trichter416 paßt durch die Öffnung408 . Ein Flansch420 an dem Trichter416 ist größer als die Öffnung408 , so daß dann, wenn die Endkappe404 aufgeschraubt ist, der Trichter416 in dem Gehäuse402 fest gehalten wird. - Der Trichter
416 hat ein Innengewinde424 an einem Ende426 , das mit einer Sonde zusammenzupaßt. Nuten438 sind an dem Trichter416 vorgesehen. O-Dichtringe (nicht gezeigt) können in die Nuten eingesetzt werden, um eine im wesentlichen fluiddichte Abdichtung zu bilden. - Ein Stapel von piezokeramischen Treibern
428 ist um den Trichter416 herum angeordnet. Der Treiberstapel428 hat vier Treiberkeramikelemente430 und ein zusätzliches Rückkopplungskeramikelement432 . Das Rückkopplungskeramikelement432 dient dazu, die Treiberamplitude zu messen. Jeder Treiber ist mit einer Nickelelektrode zur Verbindung mit einer elektrischen Quelle (nicht gezeigt) versehen. Die elektrische Quelle liefert eine alternierende Wellenform mit der geeigneten Frequenz und Amplitude. Isolatoren434 sind vorgesehen, um die Piezokeramikelemente von dem Trichter und von dem Rückkopplungskeramikelement zu trennen. MACORTM-Isolatoren, erhältlich von Corning, sind ein geeigneter Isolatortyp. Der verlängerte Bereich414 des Trichters416 ist mit einem Gewinde versehen, so daß eine Mutter436 verwendet werden kann, um die piezokeramischen Treiber an dem Trichter zu befestigen. - Die
5A bis5D zeigen verschiedene Querschnittsprofile von Ultraschallsonden, die zur Verwendung mit der vorliegenden Erfindung geeignet sind. Wie noch im einzelnen erläutert wird, kann jedes Querschnittsprofil verwendet werden, solange bestimmte Konstruktionsbedingungen erfüllt werden. - Wie
6 zeigt, ist proximal zu der gewünschten aktiven Länge der Sonde608 die Sonde608 in einem Mantel600 angeordnet, der Irrigationskanäle und Aspirationskanäle602 ,604 bilden kann. Irrigation ist bevorzugt zwischen der Sonde608 und dem Mantel600 vorgesehen. Der Mantel600 besteht bevorzugt aus PTFE- oder Teflonschlauch, um die Ultraschallenergie zu absorbieren, die von in dem Mantel befindlichen Bereichen der Sonde ausgeht, so daß eine Kontrolle über die Gewebemenge, auf welche die Sonde wirkt, möglich ist. Die Ummantelung ist nicht auf die bevorzugten Materialien beschränkt, solange sie aus einem Material besteht, das durch die Ultraschallenergie nicht erwärmt wird, obwohl das Irrigationsfluid dazu verwendet werden kann, das Mantelmaterial zu kühlen. Die Sonde kann aus dem Mantel aus- oder in diesen eingefahren werden, um die freiliegende Menge zu modifizieren, wodurch die aktive Länge der Sonde modifiziert wird. Weitere Einzelheiten bezüglich eines geeigneten Mantels sind in der hier summarisch eingeführten ebenfalls anhängigen Anmeldung S/N 60/157 824 der Anmelderin beschrieben. - Betriebstheorie der Erfindung
- Es ist zwar keine Festlegung auf die nachstehende Betriebstheorie beabsichtigt; es wird jedoch davon ausgegangen, daß die nachstehende Theorie den Betrieb der Ultraschallsonde der vorliegenden Erfindung beschreibt. Im Betrieb bewirkt der Längsschub, der von den Wandlerabschnitten abgegeben wird, ein Biegen oder Knicken des dünnen Elements an dem Ende der Sonde. Das Knicken kann als eine Auslenkung oder stehende Transversalwelle entlang der Länge des Sondenabschnitts realisiert werden. Der Auslenkungsfall ist einfach der Transversalschwingungsmodus erster Ordnung, wie noch beschrieben wird.
- In einem Fluid oder einem Fluid enthaltenden Medium bewirkt jeder von den Schwingungsbäuchen (Positionen, die der maximalen Transversalverlagerung entsprechen) entlang der Länge der Sonde eine Kavitation des Fluids in einer Richtung senkrecht zu der Längsachse der Sonde.
- Kavitation ist ein Hohlraum oder eine Blase, die durch die Unfähigkeit des Fluids erzeugt wird, die durch die Bewegung der Sonde induzierten Beanspruchungen zu überwinden. Das Kollabieren der Kavitationsblasen in dem und um das zelluläre (oder biologische) Material herum erzeugt eine Stoßwelle, die das Material abträgt oder in Fragmente zerlegt, so daß es durch Aspiration und Ansaugen entfernt werden kann. Der Mechanismus der Kavitation und ihrer Auswirkungen auf Gewebe ist in der Technik wohl bekannt und in der Literatur wie etwa in dem US-Patent Nr. 3 526 219 von Balamuth beschrieben.
- Die Bewegungsgleichungen, die den Betrieb der Sonde bestimmen, werden durch Anwendung des zweiten Newtonschen Gesetzes auf die Kräfte und Beschleunigungen erhalten, die auf ein infinitesimales Segment einwirken. Die Bewegungsgleichung für die Transversaloszillationen eines dünnen Elements ist dann (unter Vernachlässigung von Verlusten im Material und in der Umgebung) gegeben durch: wobei x die Distanz entlang dem biegsamen Bereich ist, t die Zeit in Sekunden ist, die Transversalverlagerung ist, κ der Trägheitsradius ist und c die Geschwindigkeit des Schalls in dem Material ist.
- Wie für Grenzbedingungen gezeigt werden kann, bei denen von einem biegsamen Element einer Länge l ausgegangen wird, das an dem einen Ende festgelegt und an dem anderen Ende frei ist, hat die allgemeine Lösung dieser Gleichung die Form:
- Bei Anwendung der Grenzbedingungen kann gezeigt werden, daß wobei ω die Winkelfrequenz in Radian pro Sekunde ist, x die Distanz entlang dem biegsamen Element (wie vorher) ist und v die Phasengeschwindigkeit ist, die gegeben ist durch: wobei c die Fortpflanzungsgeschwindigkeit in Längsrichtung ist, die gegeben ist durch: wobei Y der Elastizitätsmodul ist und ρ die Dichte des Materials ist.
- Die Lösungen der Gleichungen 2.2 treten nur bei diskreten Frequenzen ein, wobei gezeigt werden kann, daß diese für die ersten vier Obertöne wie folgt sind:

- Die Positionen der Schwingungsknoten für ein Element der Länge l sind dann:
erster Oberton: x = 0 zweiter Oberton: x = 0, x = 0,7741 dritter Oberton: x = 0, x = 0,51, x = 0,8681 vierter Oberton: x = 0, x = 0,3561, x = 0,6441, x = 0,9051 usw. -
2 zeigt den biegsamen Bereich, der in Moden bis zu dem vierten Oberton oszilliert. - Für eine praktische Konstruktion müssen die auf das biegsame Element einwirkenden Kräfte innerhalb sicherer Grenzen für das gewählte Material gehalten werden. Das Biegemoment des biegsamen Elements ist gegeben durch die Gleichung: wobei A die Querschnittsfläche des biegsamen Elements ist. Die Gleichung 3.0 wird sofort als die Standard-Differentialgleichung für einen in Auslenkung befindlichen Balken bzw. ein in Biegung befindliches Element erkannt.
- Die Scherkraft, die entlang dem Element wirksam ist, ist gegeben durch die Gleichung:
- Die bevorzugte Ausführungsform ist eine Sonde mit Kreisquerschnitt, wie erläutert wurde; es können jedoch alternative Formen verwendet werden, solange bestimmte Konstruktionsbedingungen berücksichtigt werden. Der Schlüsselparameter ist der Yκ2-Term in den Gleichungen 3.0 und 3.1, der häufig als die Biegesteifigkeit bezeichnet wird. Für gehärtetes Ti-6AL-4V-Titan sind optimale Werte im Bereich von 2,5 × 107 bis 8,5 × 107 N/m. Es ist zu beachten, daß die Verwendung der Biegesteifigkeit als ein Konstruktionsparameter eine von der Form unabhängige Spezifizierung für das biegsame Element ermöglicht.
- Der Treiber und die Wandlerabschnitte sind ausgebildet, um eine ausreichende Längsamplitude zu erhalten, um die gewünschte Transversalmodusamplitude zu unterstützen (siehe den nachstehenden Abschnitt über Konstruktionsbedingungen). Typischerweise sind der Griff und die Sondenanordnung ausgebildet, um eine Längsamplitude zu unterstützen, die ausreicht, um ein Knicken in dem biegsamen Element zu induzieren. Die Länge der gesamten Sonden- und Griffanordnung ist so gewählt, daß sich ein Längsschwingungsbauch an dem Ende des biegsamen Elements befindet. Dies begrenzt die Länge der Griff- und Spitzenanordnung auf ganzzahlige Vielfache einer halben Längswellenlänge. In der Praxis hat man festgestellt, daß eine geringfügige Verstimmung von ungefähr 3 bis 5 % die Umwandlung in den Transversalmodus unterstützt. Es ist zu beachten, daß es keine Längsschwingung der Spitze gibt, da diese durch das Knicken des dünnen Elements an der Spitze vollständig in eine Transversalschwingung umgewandelt wird.
- Die Kraft oder der Längsschub, der dem biegsamen Element durch den Längsabschnitt erteilt wird, muß ausreichend sein, um ein Knicken zu induzieren. Die zu Beginn aufgebrachte maximale Längskraft muß den Eulerschen Bedingungen in bezug auf Knicken genügen, welche die Lösungen von Gleichung 3.0 sind, so daß die Formel für die Knickkraft erhalten wird:

- Bei einer optimalen Konstruktion wird versucht, so viele Schwingungsbäuche wie möglich entlang der Länge des biegsamen Elements vorzusehen. Bei der vorstehend beschriebenen und gezeigten beispielhaften Ausführungsform werden mit einem 3,748 inch langen biegsamen Element mit einem Durchmesser von 0,51 mm (0,020 inches) sechs Schwingungsknoten bei einer Frequenz von 20 kHz erzeugt.
- Die obigen Gleichungen zeigen, daß die Beanspruchungen des Materials mit der Frequenz zunehmen. In Verbindung mit der Notwendigkeit t, eine ausreichende Amplitude zu erzeugen, um Gewebe zu entfernen, können Frequenzobergrenzen festgelegt werden. Zur Erzeugung von Kavitation in Fluid sollte die Transversalamplitude mindestens 75 μm sein. Dies begrenzt die Frequenz auf ungefähr 80 kHz für 6Al-4V-Titan (wobei Materialverluste außer Acht gelassen sind, die experimentell bestimmt werden müssen). Die Frequenzuntergrenze wird gewöhnlich so gewählt, daß sie außerhalb des Bereichs des menschlichen Hörvermögens oder größer als 20 kHz ist.
- Die im Transversalmodus arbeitende Sonde ist erheblich effektiver zur Gewebeentfernung als die bekannten Längskonstruktionen. Ein Grund dafür ist, daß die Wirkung der Energie entlang dem größten Teil der Länge des freiliegenden biegsamen Elements zum Tragen kommt und nicht auf die Oberfläche der Spitze des Elements beschränkt ist. Die im Stand der Technik beschriebenen Sonden, die nur in Längsrichtung angetrieben werden, sind nur an der Spitze wirksam. Selbst bei einer massiven Spitze ist deren Wirkfläche, die mit dem Gewebe in Kontakt ist, viel kleiner als bei der im Transversalmodus arbeitenden Spitze. Ferner erstreckt sich die Zerstörung von Gewebe der im Transversalmodus arbeitenden Sonden umfangsmäßig bis zu 1 mm über die Sonde hinaus. Die nachstehenden Berechnungen zeigen die Wirksamkeit des Transversalmodus im Vergleich mit einer Standardlängssonde.
- Eine steife, massive 4 mm Sonde ist nur an der Spitze wirksam. Während sie sich vor- und zurückbewegt, erzeugt sie eine Kavitation in dem vor ihr befindlichen Fluid. Das Volumen von Gewebe, auf das eingewirkt wird, ist:
- Für eine Sonde mit einer Länge von 2 cm und einem Durchmesser von 0,5 mm, die im Transversalmodus arbeitet, gilt:
- Dies bedeutet, daß unter diesen Bedingungen die im Transversalmodus arbeitende Spitze Gewebe mit einer Rate entfernt, die um das 22,2fache höher als die der massiven Spitze ist, die im Längsmodus arbeitet. Ferner hat das im Transversalmodus arbeitende biegsame Element typischerweise eine Größe von 1/8 der Längssonde. Ein Vergleich von zwei 0,5 mm Sonden, von denen die eine im Längsmodus und die andere im Transversalmodus arbeitet, zeigt, daß die im Transversalmodus arbeitende Spitze Gewebe 1.428mal schneller als die Längssonde entfernt.
- Die im Transversalmodus arbeitende Sonde ist imstande, ihre Schwingung aufrechtzuerhalten, wenn sie gebogen wird,, wenn die Summe der durch die Transversalschwingung und die Biegebeanspruchungen aufgebrachten Beanspruchungen die Elastizitätsgrenze des Materials nicht überschreitet. Dies führt zu erheblichen Vorteilen gegenüber im Längsmodus arbeitenden Konstruktionen, die typischerweise über ihre gesamte Länge steif sind.
Claims (24)
- Medizinische Ultraschallvorrichtung, die folgendes aufweist: einen Ultraschallgenerator (
15 ) zum Erzeugen einer Ultraschallschwingung in einer Richtung entlang einer Längsachse des Ultraschallgenerators; einen Ultraschallkopplungstrichter (16 ); mindestens einen Wandlerabschnitt (18 ,20 ), der durch den Ultraschallkopplungstrichter (16 ) in Ultraschallverbindung mit der Ultraschallquelle (15 ) ist, wobei der Wandlerabschnitt (18 ,20 ) die Amplitude der Ultraschallschwingung modifiziert; und ein biegsames Element (22 ), das von dem Wandlerabschnitt (18 ,20 ) angetrieben wird, dadurch gekennzeichnet, daß das biegsame Element (22 ) die Ultraschallschwingung in einem Transversalbetriebsmodus unterstützt, um eine Vielzahl von Schwingungsknoten und Schwingungsbäuchen entlang mindestens einem Bereich der Länge des biegsamen Elements (22 ) zu erzeugen, wobei der Bereich der Länge des biegsamen Elements im Gebrauch imstande ist, mit einem biologischen Material in Kommunikation zu sein. - Vorrichtung nach Anspruch 1, wobei der Ultraschallgenerator eine Ultraschallschwingung im Bereich von 20 bis 80 kHz erzeugt.
- Vorrichtung nach Anspruch 1, wobei der Ultraschallgenerator eine Ultraschallschwingung von ungefähr 20 kHz erzeugt.
- Vorrichtung nach Anspruch 3, wobei eine Länge des biegsamen Elements (
22 ) so gewählt ist, daß acht Schwingungsnoten entlang der Länge des biegsamen Elements erzeugt werden. - Vorrichtung nach einem der Ansprüche 1 bis 4, wobei das flexible Element (
22 ) ein dünnes, biegsames Element (22 ) ist, das imstande ist, ausgelenkt und gelenkig bewegt zu werden, wenn die Vorrichtung in Betrieb ist. - Vorrichtung nach Anspruch 5, wobei mindestens ein Wandlerabschnitt (
18 ,20 ) aus einem von den folgenden Materialien gebildet ist: Titan, Aluminium oder Stahl. - Vorrichtung nach einem der Ansprüche 1 bis 4, wobei das biegsame Element (
22 ) aus einem von den folgenden Materialen gebildet ist: Titan, Aluminium oder Stahl. - Vorrichtung nach einem der Ansprüche 1 bis 4, wobei der Wandlerabschnitt (
18 ,20 ) so bemessen ist, daß er eine Verstärkung von ungefähr 4 bis 5 über den Wandler erzeugt. - Vorrichtung nach Anspruch 1, wobei das biegsame Element (
22 ) Kreisquerschnitt hat. - Vorrichtung nach Anspruch 9, wobei das biegsame Element (
22 ) einen Durchmesser von weniger als 1 mm hat. - Vorrichtung nach Anspruch 9, wobei das biegsame Element (
22 ) einen Durchmesser von 0,51 mm (0,020 inches) hat. - Vorrichtung nach Anspruch 9, wobei das biegsame Element (
22 ) einen Durchmesser von 0,76 mm (0,030 inches) hat. - Vorrichtung nach Anspruch 1, wobei das biegsame Element (
22 ) quadratischen Querschnitt hat. - Vorrichtung nach Anspruch 1, wobei das biegsame Element (
22 ) Rechteckquerschnitt hat. - Vorrichtung nach Anspruch 1, wobei das biegsame Element (
22 ) Ellipsenquerschnitt hat. - Vorrichtung nach Anspruch 1, wobei die Biegesteifigkeit des biegsamen Elements (
22 ) im Bereich von 2,5 × 107 bis 8,5 × 107 N/m ist. - Vorrichtung nach Anspruch 1, wobei die Ultraschallschwingung in eine stehende Transversalwelle umgewandelt wird.
- Vorrichtung nach Anspruch 1, die ferner eine Serie von Wandlerabschnitten (
18 ,20 ) aufweist, die zwischen dem Ultraschallgenerator (15 ) und dem biegsamen Element (22 ) angeordnet sind, wobei die Wandlerabschnitte (18 ,20 ) die Amplitude der Ultraschallschwingungen modifizieren. - Vorrichtung nach Anspruch 1, die ferner eine Steuereinrichtung aufweist, die mit dem Ultraschallgenerator verbunden ist, um die Frequenz und Amplitude der Ultraschallschwingungen zu steuern.
- Vorrichtung nach Anspruch 1, die ferner einen Mantel (
600 ) aufweist, der die Wandlerabschnitte (18 ,20 ) und einen Bereich des biegsamen Elements (22 ) umgibt. - Vorrichtung nach Anspruch 20, wobei der Mantel Irrigationskanäle (
602 ,604 ) aufweist. - Vorrichtung nach Anspruch 20, wobei der Mantel Aspirationskanäle (
602 ,604 ) aufweist. - Vorrichtung nach Anspruch 20, wobei der Mantel Irrigations- und Aspirationskanäle (
602 ,604 ) aufweist. - Vorrichtung nach Anspruch 20, wobei die Mantel (
600 ) und das biegsame Element (22 ) in bezug aufeinander axial verlagerbar sind, so daß eine variierende Anzahl von Schwingungsknoten freigelegt wird.
