- HINTERGRUND DER ERFINDUNG
- Die vorliegende Erfindung betrifft implantierbare Versteifungsvorrichtungen. Insbesondere betrifft die vorliegende Erfindung Zwischenkörper-Versteifungsvorrichtungen, die aus Knochen geformt sind und sich zur Wirbelsäulenversteifung einsetzen lassen.
- Es steht eine Vielfalt von Zwischenkörper-Versteifungsimplantaten für Wirbelsäulenversteifungs-Verfahren zur Verfügung. Diese Implantate wurden bisher aus verschiedenen Materialien, u. a. Stahl, Titan, Verbundstoffen, allogenen Transplantaten, xenogenen Transplantaten oder anderen biokompatiblen Materialien, gefertigt. Die Einführung dieser Implantate kann entweder durch den Einsatz von fixierten Schutzrohren erfolgen, um die umschließenden Neuro- und Gefäßstrukturen zu schützen, oder mit Hilfe eines ungeschützten offenen Verfahrens. Eine Einschränkung hinsichtlich einer Vorrichtung, die in das Bandscheibenfach eingeführt werden soll, ist die Größe der Öffnung durch das umschließende Gewebe, die für den Zugang zum Bandscheibenfach zur Verfügung steht. Von einem hinteren Zugang zur Wirbelsäule aus, müssen die Dura und die Nervenwurzeln mobilisiert werden, um Zugang zum Bandscheibenfach zu erhalten. In ähnlicher Weise müssen von einem vorderen Zugang aus die Aorta und die Hohlvene mobilisiert werden, um Zugang zum Bandscheibenfach zu erhalten. Eine solche Mobilisierung wird häufig durch die anatomischen Strukturen eingeschränkt, wodurch sich folglich ein im Vergleich zur Größe des Bandscheibenfaches relativ kleiner Zugangsort ergibt. Die Vergrößerung des Eingangs zum Bandscheibenfach durch die Entfernung von zusätzlichen Bändern und Knochen kann das Gelenk zwischen zwei angrenzenden Wirbeln destabilisieren und schwächen. Darüber hinaus kann die übermäßige Retraktion von Gefäßen und Neuralstrukturen bei der Schaffung einer größeren Zugangsöffnung zu Schäden an diesen Geweben führen. Somit sind die Verfahren nach den bisherigen Stand der Technik darauf beschränkt gewesen, eine erste, durch die verfügbare Öffnung hindurchgehende Vorrichtung an einer Seite der Wirbelsäule zu platzieren und das Gewebe oder die Gefäße zu mobilisieren, um ein weiteres ähnliches Implantat an der gegenüberliegenden Seite der Wirbelsäule zu platzieren. Dabei war jedes Implantat hinsichtlich der Größe durch den verfügbaren Zugangsort begrenzt.
- Aufgrund dieses Sachverhalts sind dehnbare Implantate aus biokompatiblen Materialien, wie z. B. Titan und Verbundstoffen, entwickelt worden. Diese Vorrichtungen nutzen Scharniere oder die selektive Verformung des Implantatmaterials, um die Ausdehnung, nachdem sie im Bandscheibenfach positioniert wurden, zu ermöglichen. Während solche Vorrichtungen eine Einführungskonfiguration reduzierter Größe und eine ausgedehnte Abstandskonfiguration aufweisen, sind die für die Bildung der Implantate eingesetzten Materialien synthetisch und werden nicht in die angrenzenden Knochengeweben integriert. Während Knochen eine viel bessere Einlagerung bieten, schränkt die inhärente spröde Beschaffenheit von Knochen, die sich aus einem hohen Mineralgehalt ergibt, wie er besonders bei Rindenknochen für die Lastaufnahme vorliegt, die mögliche Knochenverformung sehr stark ein. Normalerweise besteht beispielsweise ein Rinderknochen aus ca. 70% Mineralgehalt und 30% nichtmineralischer Substanz. Von dieser nichtmineralischen Substanz sind etwa 95% Typ I Kollagen, der Rest Zellsubstanz und nicht kollagene Proteine.
- Knochentransplantate werden üblicherweise zusammen mit anderen Lastaufnahme-Implantaten in einer festen Form, pulverisiert oder in Form von biegsamen entmineralisierten Knochen eingesetzt. Eine Form eines biegsamen Knochentransplantats ist ein entmineralisiertes Knochenmaterial, das typischerweise in der Form eines Schwamms oder Kitts vorliegt und eine sehr geringe strukturelle Unversehrtheit aufweist. Während ein entmineralisiertes Knochensegment Eigenschaften beibehält, die das Einwachsen des Knochens unterstützen, werden die strukturellen Eigenschaften des Knochens durch die Entfernung seines Mineralgehalts geändert. Somit lassen sich solche Knochenschwämme und Knochenkitte typischerweise nicht bei Lastaufnahmeanwendungen einsetzen.

US-A-4,627,853 WO-A-98/34 568 - Deshalb gibt es weiterhin einen Bedarf für ein starkes Knochenimplantat mit einem biegsamen Bereich.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Hinsichtlich eines Aspektes stellt die vorliegende Erfindung ein biegsames Knochenimplantat bereit. Das Knochenimplantat der vorliegenden Erfindung umfasst einen ersten Knochenabschnitt, einen zweiten Knochenabschnitt und einen biegsamen Knochenabschnitt, der den ersten und den zweiten Knochenabschnitt miteinander verbindet. Der biegsame Zwischenknochenabschnitt lässt die Bewegung des ersten Knochenabschnits relativ zum zweiten Knochenabschnitt zu. Bei einer bevorzugten Ausführungsform würde die Bewegung des ersten und zweiten Knochenabschnitts zwischen einer Einführungskonfiguration reduzierter Größe und einer ausgedehnten Konfiguration erfolgen, die sich zur Aufrechterhaltung der Abstandsbeziehung zweier Knochenstrukturen eignet und, bei Wunsch, das Einwachsen des Knochens gestattet. Optional kann die Bewegung zwischen den ersten und zweiten Knochenabschnitt als elastischer Dämpfer eingesetzt werden, und zwar nachdem die Vorrichtung zwischen den angrenzenden Knochenstrukturen positioniert wurde.
- Gemäß eines weiteren Aspekts der Erfindung umfasst das Knochenimplantat ein Knochensegment, das mindestens einen teilweise entmineralisierten Bereich aufweist, der ein biegsames Segment des entmineralisierten Knochensegments bildet. Bei einer Ausführungsform ist ein gegenüberliegender Abschnitt des kortikalen Femoralringsegmentes so abgetrennt, dass sich das Ringsegment ausdehnen kann, sobald es in ein intervertebrales Bandscheibenfach eingeführt worden ist. Bei wieder einer anderen Ausführungsform umfasst die Vorrichtung mindestens zwei teilweise entmineralisierte Knochenabschnitte an im Wesentlichen gegenüberliegenden Abschnitten des Knochensegmentes. Bei dieser Konfiguration sind die im Wesentlichen starren Abschnitte so platziert, dass sich die Lastauflageflächen zwischen zwei angrenzenden Knochenstrukturen so berühren, dass der biegsame Abschnitt eine elastische Funktion übernimmt, wodurch eine normalere Bewegung oder eine bessere Lastaufnahme des an das Bandscheibenfach angrenzenden Knochens ermöglicht wird.
- Gemäß eines wieder anderen Aspekts der vorliegenden Erfindung wird ein Verfahren für die Präparation eines Knochenimplantats bereitgestellt. Das Verfahren umfasst die Bereitstellung eines starren Knochensegmentes und den Entwurf eines Zwischenabschnittes dieses Segmentes. Der mittige Abschnitt wird dann mindestens teilweise entmineralisiert, um ein biegsames Segment zwischen zwei angrenzenden Knochenabschnitten zu erzeugen. Das Verfahren für das mindestens teilweise Entmineralisieren eines Knochensegmentes zwischen zwei angrenzenden starren Knochensegmenten lässt sich so oft wiederholen, wie es erforderlich ist, um die gewünschte Struktur für die Implantation zu erzeugen.
- Diese und andere Aufgaben der vorliegenden Erfindung sind für den Fachmann auf der Grundlage der nachstehenden Beschreibungen der bevorzugten Ausführungsform der vorliegenden Erfindung offensichtlich.

10(b) ist abgewandelte Ausführungsform von10(a) . - BESCHREIBUNG DER BEVORZUGTEN AUSFÜHRUNGSFORMEN
- Um ein besseres Verständnis für die Prinzipien der Erfindung zu bekommen, wird jetzt auf Ausführungsformen, die in den Zeichnungen dargestellt sind, Bezug genommen und eine spezielle Sprache zur Beschreibung derselben verwendet. Es wird trotzdem deutlich gemacht, dass dadurch keine Einschränkung des Schutzbereiches der Erfindung beabsichtigt ist, sondern dass Abänderungen sowie weitere Abwandlungen an den dargestellten Vorrichtungen und derartige weitere Anwendungen der Prinzipien der Erfindung, wie sie hier erläutert werden, als solche betrachtet werden, auf die normalerweise ein Fachmann auf dem Gebiet, auf das sich die Erfindung bezieht, schließen kann.
- Jetzt wird auf
1 Bezug genommen, in der ein Implantat gemäß einer bevorzugten Ausführungsform der vorliegenden Erfindung dargestellt ist. Obwohl Implantate gemäß der vorliegenden Erfindung viele Einsatzzwecke haben können, ist die in1 dargestellte Ausführungsform speziell für die Förderung der Zwischenkörper-Versteifung in der Wirbelsäule ausgelegt. Speziell1 zeigt ein Knochenimplantat10 , das einen ersten, im Wesentlichen starren Abschnitt12 und einen zweiten, im Wesentlichen starren Abschnitt14 aufweist. Der erste Abschnitt12 ist mit dem zweiten Abschnitt14 mit Hilfe eines Zwischenabschnitts16 verbunden. Der Zwischenabschnitt16 wurde mindestens teilweise entmineralisiert, um einen biegsamen Bereich im Knochenimplantat zu erzeugen. Vorzugsweise wurde ein Bereich eines Zwischenabschnitts16 vollständig entmineralisiert, um eine maximale Biegsamkeit bereitzustellen. Mit Hilfe der durch die Entmineralisierung eines Zwischenabschnitts16 erzeugten Biegsamkeit lassen sich die starren Abschnitte12 und14 relativ zueinander bewegen. Die Vorteile dieses Merkmals werden nachstehend ausführlich beschrieben. - Der Knochenabschnitt
12 umfasst erhabene Knocheneingriffs-Druckkanten20 , die an der oberen Auflagefläche17 definiert sind, wobei ein identischer Satz von erhabenen Druckkanten21 an der unteren Auflagefläche (nicht dargestellt) definiert ist. In einer ähnlichen Weise umfasst der Knochenabschnitt14 erhabene Knocheneingriffs-Druckkanten18 , die an der oberen Auflagefläche15 definiert sind, und identische erhabene Druckkanten19 , die an der unteren Auflagefläche (nicht dargestellt) definiert sind. Es wird deutlich gemacht, dass, obwohl die erhabenen Druckkanten bei einer bevorzugten Ausführungsform dargestellt wurden, die Auffassung vertreten wird, dass es eine Vielfalt von Strukturen gibt, die eine Fläche für den wirksamen Eingriff an den Wirbelkörpern bereitstellen könnten, um die Austreibung aus dem Bandscheibenfach zu begrenzen. - Die starren Knochenabschnitte
12 und14 sind dafür ausgelegt, eine strukturelle Stützkonstruktion zwischen den entsprechenden oberen und unteren Auflageflächen bereitzustellen. Insbesondere kann das Knochenimplantat aus Spenderknochen ausgewählt werden, die eine ausreichende Kompressionsfestigkeit zwischen der oberen und unteren Begrenzungsfläche aufweisen, um so in der vorgesehenen Umgebung zum Einsatz zu kommen. Das Paar von starren Knochenabschnitten wirkt zusammen, um eine Stützkonstruktion für den Abstand zwischen den angrenzenden Wirbeln bereitzustellen. Obwohl die bevorzugten Ausführungsformen des Implantats gemäß der vorliegenden Erfindung mit zwei starren Knochenabschnitten dargestellt wurden, besteht die Auffassung, dass weitere starre Knochenabschnitte durch biegsame Knochenbereiche verbunden werden können, um weitere Implantatformen zur Verfügung zu stellen. - Jetzt wird auf die
2 -4 Bezug genommen, in denen ein Verfahren zur Einführung einer Vorrichtung für die Zwischenkörper-Versteifung zwischen den angrenzenden Wirbelkörpern dargestellt ist. Speziell in2 wird das Implantat10 in seiner Einführungskonfiguration reduzierter Größe gezeigt, wobei der erste Abschnitt12 im Wesentlichen angrenzend an den zweiten Abschnitt14 angeordnet ist. Wie in2 dargestellt, besteht die Auffassung, dass die starren Abschnitte in einer im Wesentlichen parallelen Ausrichtung positioniert werden können. Bei manchen Anwendungen ist jedoch dieser Grad der Flexibilität hinsichtlich des Zwischenabschnitts16 nicht erforderlich. Das Implantat kann in der Einführungskonfiguration in einem Einführungsrohr30 hinsichtlich seiner räumlichen Bewegungsfreiheit eingeschränkt werden. - Der Zugang zum Bandscheibenfach zwischen den angrenzenden Wirbeln wird so wie es im Fachgebiet bekannt ist realisiert. Obwohl sich der Zugang von jeder anderen Richtung aus realisieren lässt, veranschaulichen die
2 und3 , zur Erläuterung und ohne daraus eine Beschränkung hinsichtlich des Schutzbereiches der Erfindung abzuleiten, die Zugangsmöglichkeit mit Hilfe eines hinteren Zuganges. Sobald der Zugang realisiert ist, kann ein Schutzrohr angrenzend an das Bandscheibenfach positioniert und, falls erforderlich, das Bandscheibenfach gedehnt werden. Das Implantat10 wird so bewegt, dass es seine Einführungskonfiguration annimmt, in der die Knochenabschnitte12 und14 der Länge nach im Wesentlichen parallel zueinander ausgerichtet sind. Das Implantat wird, in seiner Konfiguration reduzierter Größe, im Schutzrohr30 positioniert und zum Bandscheibenfach D vorgeschoben. Es wird deutlich gemacht, dass, obwohl das Implantat10 eine viel größere Abmessung nach seiner Einführung aufweisen kann, die Dura34 nur innerhalb des Hohlraumes38 ausreichend zurückgezogen werden muss, um den Durchgang des Schutzrohres30 und der reduzierten Größe des Implantats zu ermöglichen. - Das Implantat
10 wird durch das Schutzrohr30 unter Einsatz einer Schiebevorrichtung (nicht dargestellt) so lange vorgeschoben, bis es das. Schutzrohr30 verlässt und in das Bandscheibenfach D eintritt (3 und4 ). Sobald es sich im Bandscheibenfach D befindet, dehnt sich die Vorrichtung entweder durch Freigabe einer elastischen Verformung aus, die im mittigen Abschnitt16 ausgebildet ist, oder es kann ein separates Instrument (nicht dargestellt) zwischen dem ersten Abschnitt12 und dem zweiten Abschnitt14 eingeführt werden, um zwangsweise eine Bewegung zwischen den entsprechenden Abschnitten herbeizuführen, um so die Vorrichtung in die ausgedehnte Abstandskonfiguration, die in3 dargestellt ist, zu bringen. Die Ausdehnung der Vorrichtung erzeugt ein Implantat, das eine größere Stabilität hinsichtlich des intervertebralen Faches aufweist, und zwar mit Hilfe eines breiteren Stützbereiches und einer geringeren Neigung zum Herüberfallen in das Bandscheibenfach. Außerdem stellt der Hohlraum33 zwischen den Abschnitten12 und14 einen Bereich zur Aufnahme von Material bereit, der die Knocheneinlagerung und Versteifung fördert. Sobald das Implantat10 ordnungsgemäß positioniert wurde, kann das den Knochenwuchs fördernde Material32 zwischen dem ersten Abschnitt12 und dem zweiten Abschnitt14 positioniert werden, um den Knochenwuchs in und durch das Implantat10 zu unterstützen. Obwohl es nicht dargestellt ist, soll deutlich gemacht werden, dass normalerweise ein zweites Implantat in das Bandscheibenfach D eingebracht wird, um die Stabilität weiter zu erhöhen. - Wie anhand der Darstellung in
4 besonders klar ersichtlich ist, hat das Implantat10 eine Höhe H, die im Wesentlichen gleich der Höhe des Bandscheibenfaches D ist, das zwischen dem Wirbel36 und dem Wirbel38 ausgebildet ist. Der Fachmann wird verstehen, dass bei der bevorzugten Ausführungsform, die hier erläutert wird, die Höhe H von der in3 dargestellten Einführungs- zu der in4 dargestellten ausgedehnten Konfiguration im Wesentlichen konstant ist. Während außerdem ein Implantat mit konstanter Höhe in2 dargestellt ist, ist einzusehen, dass das Implantat10 eine schräg zulaufende Höhe derart aufweisen kann, dass das Implantat eingesetzt werden könnte, um die ordnungsgemäße Wirbelsäulen-Lordosekrümmung herzustellen oder aufrechtzuerhalten. Unter Bezugnahme auf den starren Knochenabschnitt14 liegt die obere Auflagefläche25 am oberen Wirbelkörper38 an und stützt ihn, während die untere Auflagefläche27 am Implantat am unteren Wirbelkörper36 anliegt und ihn stützt. Die starren erhabenen Druckkanten18 und19 liegen jeweils an der Fläche der Wirbelkörper38 und36 an, um der Austreibung standzuhalten. Der starre Knochenabschnitt14 hat zusammen mit dem starren Knochenabschnitt12 einen ähnlichen Eingriff an den Wirbeln, weist ausreichend starre Abschnitte und eine strukturelle Unversehrtheit auf um im Wesentlichen eine Höhe H aufrechtzuerhalten und den auf die Wirbelsäule aufgebrachten Normalkräften standzuhalten. Der biegsame Bereich16 muss keine solche strukturellen Anforderungen erfüllen, obwohl er vorzugsweise die Implantatstabilität durch Aufrechterhaltung der Verbindung zwischen den zwei Stützwänden unterstützt. - Das biegsame Knochenimplantat
10 stellt die gewünschten Merkmale bereit, nämlich aus einem sehr erfolgreichen Knochenversteifungsmaterial, d. h. einem Naturknochen, geformt zu sein, und hat die Vorteile, sowohl über eine Einführungskonfiguration reduzierter Größe als auch einer ausgedehnten Abstandskonfiguration zu verfügen. Während somit das Implantat die gewünschte Höhe der Bandscheibenfach-Distraktion aufrechterhält, lässt sich die Breite des Implantats gegenüber dem mittigen Abschnitt16 leicht von der Einführungskonfiguration von2 auf die ausgedehnte Konfiguration von3 ausdehnen. Dieses Merkmal gestattet die Einführung durch einen kleineren Zugangsort und erhöht die Implantatstabilität im Bandscheibenfach. - Jetzt wird Bezug auf die
5 bis7 genommen, die eine weitere Ausführungsform des Implantats gemäß der vorliegenden Erfindung zeigen.5 zeigt einen mit Gewinde versehenen Rindenknochendübel50 , der gemäß der vorliegenden Erfindung abgeändert wurde. Der Knochendübel50 umfasst ein Gewinde58 für den Eingriff am angrenzendem Wirbel, um das Implantat auf eine kontrollierte Weise vorzuschieben und um der Austreibung standzuhalten. Das Implantat50 hat einen vertieften Schlitz64 für den Eingriff eines Eindrehwerkzeuges, das dafür ausgelegt ist, die Vorrichtung zu drehen. Gemäß der Erfindung ist der mit Gewinde versehene Knochendübel50 in eine erste Seitenwand52 und eine zweite Seitenwand54 unterteilt, die durch einen biegsamen Bereich56 und einen Schlitz62 getrennt sind. Wie hier weiter beschrieben wird, wird der biegsame Bereich56 dadurch erzeugt, dass der Knochen in diesem Bereich des Implantats mindestens teilweise entmineralisiert wird. Sowohl die erste als auch die zweite Seitenwand,52 bzw.54 , umfasst eine obere und untere Auflagefläche. Der Gewindedübel50 umfasst außerdem eine mittige Öffnung60 . Diese Öffnung kann durch den natürlichen Markkanal eines Diaphysenknochens oder durch die Entfernung eines schwammigen Abschnitts eines Spenderknochens erzeugt werden, obwohl dies von der Konfiguration des Spenderknochens abhängig ist. - Bei der Konfiguration von
5 lässt sich die Vorrichtung durch ein Einführungsrohr oder eine andere Vorrichtung in ein Bandscheibenfach einführen, so wie dies zuvor beschrieben wurde. Sobald die Positionierung so erfolgt ist, dass die Öffnung60 an den oberen und unteren Wirbelkörper angrenzt, werden die erste Seitenwand52 und die zweite Seitenwand54 voneinander weggedrückt, wobei sich das Implantat am biegsamen Abschnitt56 durchbiegt. Das Implantat50 ist in7 in seinem ausgedehnten Zustand dargestellt. Sobald die gewünschte Ausdehnung erzeugt worden ist, kann das den Knochenwuchs fördernde Material64 in den Innenbereich60 zwischen der ersten Seitenwand52 und der zweiten Seitenwand54 eingeführt werden. Die Seitenwände stellen eine strukturierte Stützkonstruktion bereit, um die Höhe des Bandscheibenfaches aufrechterhalten. Gemäß den Darstellungen in den5 bis7 weist das Knochenimplantat50 eine Einführungskonfiguration reduzierter Größe und eine ausgedehnte Abstandskonfiguration auf. - In
8 wurde bei wieder einer anderen Ausführungsform der vorliegenden Erfindung ein zylindrischer Gewindeknochendübel gemäß der vorliegenden Erfindung abgeändert. Es wurde speziell das Knochenimplantat80 abgeändert, damit dieses mindestens zwei Bereiche86 und88 mit einem verringerten Mineralgehalt aufweist, um einen Grad an Flexibilität im Implantat bereitzustellen. Die entmineralisierten Abschnitte86 und88 sind zwischen den starren Abschnitten82 und84 angeordnet. Somit werden plötzliche Änderungen hinsichtlich der Kräfte auf die starren Abschnitte82 oder84 durch die intervenierenden biegsamen Bereiche gedämpft. In8 ist eine solche Vorrichtung im Bandscheibenfach94 zwischen dem Wirbelkörper V1 und dem Wirbelkörper V2 implantiert, wobei die starren Abschnitten92 und94 jeweils angrenzend an die Wirbelkörper V1 und V2 positioniert wurden. Es ist einzusehen, dass wenn eine Kraft an den Wirbelkörpern V1 und V2 aufgebracht wird, das Implantat dazu neigen wird, sich an den entmineralisierten Bereichen86 und88 durchzubiegen, um ein Grad an Flexibilität im Implantat bereitzustellen und eine physiologische Belastungsumgebung bereitzustellen. Speziell können die Druckkräfte, die durch die Pfeile102 ,103 ,104 und105 repräsentiert werden, normaler durch Durchbiegen der biegsamen Abschnitte86 und88 jeweils auf die Positionen110 und112 übertragen werden. Solche Vorrichtungen können sowohl bei der Wirbelsäulenversteifung (Normalbelastung) als auch bei der Arthroplastik (Normalbewegung) zur Anwendung kommen. - In
9 ist wieder ein anderer Aspekt der vorliegenden Erfindung dargestellt. Der Spenderknochen120 ist ein im Wesentlichen ringförmiges Knochensegment, das einen Innenhohlraum130 , wie z. B. einen Femoralring, aufweist. Im Ring120 ist ein Schlitz128 ausgebildet. Gegenüber dem Schlitz128 wird ein Abschnitt126 so behandelt, dass mindestens ein Teil der Knochenmineralien entfernt wird. Dies erzeugt einen Flexibilitätsbereich am Abschnitt126 . So wird der Knochen in die Seitenwände122 und124 unterteilt, die durch den Schlitz128 und den biegsamen Abschnitt126 getrennt sind. Wie bereits zuvor beschrieben wurde, kann das Knochentransplantat dadurch ausgedehnt werden, dass nach der Einführung die Seitenwand124 von der Seitenwand122 weg bewegt wird. - Jetzt wird Bezug auf die
10(a) genommen, die eine alternative Ausführungsform des Implantats gemäß der vorliegenden Erfindung zeigt. Bei dem Distanzstück180 handelt es sich um ein Smith-Robinson-Knochentransplantat, das normalerweise in der Halsregion der Wirbelsäule eingesetzt wird. Das Distanzstück180 umfasst einen Innenhohlraum188 , der durch die Wände182 ,183 ,184 und191 definiert ist. Der Hohlraum188 ist für die Aufnahme des Knochentransplantat-Materials geeignet, das zur Förderung der Versteifung zwischen den angrenzenden Wirbeln dient. Um eine Ausdehnungsmöglichkeit bereitzustellen, wird eine Öffnung186 in der Wand183 geschaffen und ein gegenüber angeordneter biegsamer Scharnierbereich190 in der Wand191 dadurch geschaffen, dass mindestens eine Teilentmineralisierung erfolgt. Auf diese Weise können die Wände182 und184 jeweils in der Richtung der Pfeile192 und194 verschoben werden, um das Implantat nach dessen Einführung zwischen den angrenzenden Wirbeln auszudehnen. Es ist einzusehen, dass die Wand191 mindestens teilweise durch den Ausdehnungsprozess verformt wird. -
10(b) zeigt eine abgeänderte Ausführungsform des Implantats von10(a) . In10(b) hat das Distanzstück195 eine Innenkammer208 , die durch die Wände199 ,200 ,201 und202 definiert ist. Die Wand201 umfasst eine Öffnung196 , die durch dieselbe ausgebildet ist. Die biegsamen Knochenbereiche werden dadurch erzeugt, dass mindestens eine Teilentmineralisierung an den Scharnierbereichen197 und198 an den jeweiligen Wänden200 und202 erfolgt, die an die Verbindung mit der Wand199 angrenzen. Die Wände200 und202 können in der Richtung der Pfeile204 und206 verschoben werden, um die Ausdehnung des Distanzstückes195 zu ermöglichen. Durch den Einsatz von zwei Scharnierbereichen am Implantat lässt sich die Wand199 im Bandscheibenfach genau platzieren und die Ausdehnung kann seitlich stattfinden, ohne dass die Position eines Abschnitts der Wand199 während der Ausdehnung geändert wird. - Zusätzlich zu den oben beschriebenen Ausführungsformen kann es für die vorliegende Erfindung weitere Einsatzmöglichkeiten geben. Insbesondere, aber ohne Beschränkung, kann eine solche darin bestehen, dass Spenderknochensegmente umgebildet werden, damit sie besser an die Räume angepasst sind, für die die Implantate gebraucht werden. In einigen Fällen können die Spenderknochensegmente Formen aufweisen, die nicht mit der Form der Implantationsortes kompatibel sind. Diese Knochensegmente können flexible Bereiche aufweisen, um das Knochentransplantat so umzubilden, dass es passender für seinen vorgesehenen Einsatzzweck ist. Solche Segmente können einen biegsamen Bereich oder mehrere biegsame Bereiche derart aufweisen, dass sich die Gesamtform des Spenderknochensegments durch die Durchbiegung an den biegsamen Segmenten abändern lässt. Dadurch kann die Lastaufnahmefestigkeit des Implantats weitgehend erhalten bleiben. Dieser Einsatz der vorliegenden Erfindung kann die möglichen verwendbaren Abschnitte der Spenderknochen erhöhen, für die die Versorgung begrenzt ist. Die umfassende Verwendung von Spenderknochen und alternativen Transplantatformen werden in größerer Vollständigkeit in der am 29. Oktober 1998 eingereichten US-Patentanmeldung Nr.
09/181,353 - Jetzt wird die Erzeugung des entmineralisierten Knochenabschnitts beschrieben. Die Verarbeitung greift auf die Verwendung von Spenderknochen zurück, wobei die Verarbeitung in einer Reinraumumgebung innerhalb einer Knochenverarbeitungseinrichtung erfolgt. Ein solcher Spenderknochen kann ein allogenes Transplantat menschlichen Ursprungs oder ein xenogenes Transplantat tierischen Ursprungs sein. Außerdem besteht die Auffassung, dass wegen des zu erwartenden Fortschritts der Technik im Bereich der Knochenverarbeitung, der Spenderknochen im Fertigungsprozess erzeugt werden kann, entweder durch Knochenwuchs oder durch eine Verarbeitung der Knochenbestandteile, um künstliche Materialien zu erzeugen, die vergleichbare Eigenschaften wie Knochen haben. Während sich speziell jeder verfügbare allogene oder xenogene Knochenbestand für das Verfahren einsetzen lässt, wird der Rindenknochen gewöhnlich wegen seiner strukturellen Eigenschaften für die Wirbelsäulenversteifung bevorzugt, obwohl eine Rindenknochenschwamm- oder Knochenschwammsubstanz, je nach den für die implantierbare Vorrichtung gegebenen speziellen Anforderungen, eingesetzt werden kann.
- Bei der weiteren Verarbeitung werden die Bindegewebe entfernt und der Knochen wird gereinigt, gespült und entfettet, wobei ein Lösungsmittel wie z. B. Ethanol oder Wasserstoffperoxid eingesetzt wird. Danach wird der Knochen zur Herstellung seiner Form, die z. B. eine Keil-, Dübel- oder sonstige Form sein kann, spanend bearbeitet oder sonst wie unter Anwendung der konventionellen Techniken geformt. Ein zurischenabschnitt des Knochens wird so festgelegt, wie er für den erhöhten Grad an Flexibilität benötigt wird. Die Entmineralisierung erfolgt nur an dem Ort, an dem die Biegsamkeit benötigt wird. Die Eindringung der Entmineralisierungsflüssigkeit in den Knochen, der an den gewünschten Flexibilitätsbereich angrenzt, wird mit Hilfe des hydrostatischen Druckes überwacht, wodurch der Bereich der Entmineralisierung begrenzt wird. Die Menge des vom Knochen entfernten Minerals lässt sich für die Realisierung des gewünschten Grades an Flexibilität einstellen. Für die Entmineralisierung wird gewöhnlich eine organische Säure, wie z. B. Salz-, Salpeter- oder Zitronensäure, eingesetzt. Vorzugsweise umfasst die Entmineralisierungslösung 0,1 bis 1,0 N HCl, wobei der Wert 0,3 N HCl besonders bevorzugt wird. Wenn ein xenogenes Transplantat verwendet wird, können die bekannten Techniken hinsichtlich der Verwendung von organischen Lösungsmitteln in diesem Stadium zum Einsatz kommen, um die Knochenproteine zu inaktivieren und die Antigenität zu reduzieren. Außerdem kann der Einsatz von Glutaraldehyd erfolgen, um darüber hinaus im Anschluss an die Entfernung des Mineralienanteils die Kollagenstruktur quer zur Faserrichtung zu fixieren. Sobald die Vorrichtung spanend bearbeitet und teilweise entmineralisiert worden ist, lässt sie sich bis zum Zeitpunkt der Einführung lagern.
- Obwohl die oben beschriebene Verarbeitung hier als eine bevorzugte Ausführungsform offenbart wurde, besteht die Auffassung, dass auch andere geeignete Verfahren eingesetzt werden können.
Claims (16)
- Implantat (
10 ) zur Förderung der Versteifung zurischen angrenzenden oberen (38 ) und unteren (36 ) Wirbelkörpern, umfassend: einen ersten, aus Knochen hergestellten Knochenabschnitt (12 ); einen zweiten, aus Knochen hergestellten Knochenabschnitt (14 ); einen biegsamen Abschnitt (16 ), der den ersten Knochenabschnitt (12 ) und den zweiten Knochenabschnitt (14 ) verbindet, wobei der biegsame Abschnitt (16 ) die Bewegung des ersten Knochenabschnitts (12 ) in Bezug auf den zweiten Knochenabschnitt (14 ) zulässt und eine obere Auflagefläche für den Eingriff mit dem oberen Wirbel und eine untere Auflagefläche für den Eingriff mit dem unteren Wirbel, wobei diese Flächen durch eine erste Höhe getrennt sind und der zweite Knochenabschnitt (14 ) eine obere Auflagefläche (15 ) für den Eingriff mit dem oberen Wirbel (38 ) und eine untere Auflagefläche für den Eingriff mit dem unteren Wirbel (36 ) umfasst, wobei diese Flächen durch eine zureite Höhe getrennt sind und die erste und zureite Höhe für die Aufrechterhaltung des Abstandes zurischen angrenzenden oberen und unteren Wirbelkörpern (36 ,38 ) ausgelegt sind. - Implantat (
10 ) nach Anspruch 1, wobei der biegsame Abschnitt (16 ) mindestens teilweise ein entmineralisierter Knochen ist. - Implantat (
10 ) nach Anspruch 2, wobei der biegsame Abschnitt (16 ) einen Bereich aus einem vollständig entmineralisierten Knochen umfasst. - Implantat (
10 ) nach Anspruch 1, wobei die erste Höhe und die zureite Höhe im Wesentlichen gleich sind. - Implantat (
10 ) nach Anspruch 1, wobei jede der oberen und unteren Auflageflächen eine Knocheneingriffsfläche (18–21 ) umfasst, um der Austreibung aus einem Bandscheibenfach zwischen zwei angrenzenden Wirbeln (36 ,38 ) entgegenzuwirken. - Implantat (
10 ) nach Anspruch 1, wobei der erste und zweite Knochenabschnitt (12 ,14 ) korrespondierende schräg zulaufende Auflageflächen aufweisen, um ein Implantat bereitzustellen, das eine Distanzhöhe hat, die allmählich von einem ersten Ende aus zu einem gegenüberliegenden zweiten Ende hin zunimmt, wobei das Implantat (10 ) für den Einsatz zur Aufrechterhaltung der Lordose ausgelegt ist. - Implantat (
10 ) nach Anspruch 1, wobei der erste Knochenabschnitt (12 ), der zweite Knochenabschnitt (14 ) und der biegsame Abschnitt (16 ) aus einem einzigen Knochensegment geformt sind. - Implantat (
10 ) nach Anspruch 1, wobei das Implantat (10 ) im Wesentlichen ringförmig ist. - Wirbelsäulenversteifungs-Implantat (
50 ), das für die Einführung in das Fach zwischen angrenzenden ersten und zweiten Wirbelkörpern (V1, V2) ausgelegt ist, umfassend: einen ersten Knochenabschnitt (52 ), der eine erste Auflagefläche für den Eingriff mit einem ersten Wirbelkörper aufweist, wobei der erste Knochenabschnitt aus Knochen hergestellt ist; einen zweiten Knochenabschnitt (54 ), der eine zweite Auflagefläche für den Eingriff mit einem zweiten Wirbelkörper aufweist, wobei der zweite Knochenabschnitt aus Knochen hergestellt ist; ein proximales Ende und ein gegenüberliegendes distales Ende; einen ersten biegsamen Abschnitt (86 ), der angrenzend an das proximale Ende angeordnet ist, und einen zweiten biegsamen Abschnitt (88 ), der zum ersten biegsamen Abschnitt (86 ) einen Abstand aufweist und angrenzend an das distale Ende angeordnet ist und wobei die biegsamen Abschnitte (86 ,88 ) eine Bewegung zwischen dem ersten Knochenabschnitt und dem zweiten Knochenabschnitt (52 ,54 ) zulassen. - Implantat (
50 ) nach Anspruch 9, wobei die erste Auflagefläche und die zweite Auflagefläche in gleicher Richtung wirkende Gewindestrukturen umfassen, mit deren Gewinde das Implantat (50 ) in das Fach zwischen den ersten und zweiten Wirbelkörpern (V1, V2) eingeführt werden kann. - Implantat (
50 ) nach Anspruch 9, wobei das Implantat (50 ) aus einem einzigen Knochensegment geformt ist. - Verfahren für die Präparation eines biegsamen Knochenimplantats nach einem der Ansprüche 1 bis 11, umfassend: Bereitstellung eines starren Knochensegmentes; Entwurf eines Zwischenabschnittes des Knochensegmentes; Einwirkenlassen der Entmineralisierungsflüssigkeit auf den zwischenabschnitt und Begrenzung des Kontakts des an den zwischenabschnitt angrenzenden Knochens mit der Entmineralisierungsflüssigkeit durch Einsatz des hydrostatischen Druckes zur Bewegungsbegrenzung der Entmineralisierungsflüssigkeit in dem an den zwischenabschnitt angrenzenden Knochen zur Schaffung eines biegsamen Segmentes (
16 ,56 ) zwischen den angrenzenden Abschnitten des starren Knochens. - Verfahren nach Anspruch 12, wobei dieses außerdem die Ausbildung einer Knocheneingriffsfläche am Implantat umfasst.
- Verfahren nach Anspruch 13, wobei die Knocheneingriffsfläche erhabene Knochenstrukturen umfasst.
- Verfahren nach Anspruch 13, wobei die Knocheneingriffsfläche eine Struktur mit Gewinde ist.
- Verfahren nach Anspruch 13, wobei die Knocheneingriffsfläche so gestaltet ist, dass die Bewegung des Implantats verhindert wird.
