Description
Hintergrund und Beschreibung der ErfindungBackground and description of the invention
The present invention relates generally to endoprostheses, which are also known as support and / or expansion devices, so-called "stents", as well as their preparation or production. In particular, the invention relates to endoprostheses having a plurality of welded connections interconnecting adjacent convolutions of undulating, flexible segments aligned in a helical pattern along the length of the endoprosthesis. The flexible segments impart a radial expandability to the endoprosthesis which can be designed to vary the hoop strength of the endoprosthesis while still retaining the ability of the endoprosthesis to conform to the contour of the vessel in which it is to be deployed. Endoprostheses of the present invention also exhibit exceptional uniformity of expansion and maintain the desired percentage of solid surface area substantially throughout the endoprosthesis, especially after insertion thereof.
Various endoprosthesis devices or or "stents", hereinafter referred to as support devices, have been developed or proposed for use in conjunction with angioplasty and other medical treatments or procedures that utilize expandable-element devices, such as balloon catheters to treat pathological change in a body vessel. The endoprosthesis or support device is of the type of a usually tubular or cylindrical device which is used by a balloon or the like and which remains on a site to be treated upon withdrawal of the balloon catheter or other insertion and / or treatment device in the vessel.

Accordingly, previous approaches in the art of endoprostheses or support devices have suggested or provided devices with good hoop strength, which is particularly important in the art Supporting applications may be, which might be subject to forces that tend to collapse the endoprosthesis, such as when relatively large vessels are supported or when the support device is used within a vessel that may be exposed to forces from outside, such as in the leg. Other known endoprostheses or supporters have lower hoop strength, but are more compliant in that they are more apt to conform to the contour of the vessel, rather than being so relentless as to deform the vessel after it has been inserted. A typical disadvantage of the more resilient support devices is that they tend to deform during or after their insertion and to have support surfaces which can have a desired uniformity over the entire working surface area of the support device. The development of non-uniformity in the work surface area of the support device may be particularly evident during expansion of the support device from its collapsed insertion diameter to its expanded, implanted diameter. Sometimes, this lack of uniformity in expansion is enhanced by convolutions or other non-uniformities in a balloon on which the support device is mounted for insertion.
It has been found that the endoprostheses according to the present invention have the ability to conform to the contour of a vessel being supported, while still having the hoop strength required for adequate support, such as that required by less compliant structures Of the Palmaz type, as discussed herein, while providing the added benefit of providing uniform expansion to provide an expanded support having the desired percentage of support surface area. Furthermore, with the present invention, these important properties can be adjusted or tailored so that they satisfy the particular requirements of the problem addressed by varying depending on the requirements of compliance and hoop strength.
In summary, the present invention achieves these advantages and propels the endoprosthesis technique forward through an endoprosthesis constructed of a strand having flexible segments arranged in a wave-like and substantially uniform manner, the undulating strand being generally helical Configuration is wound to form the Endoprothesenkörper, which consists of a plurality of complete, continuous along the helical course circular windings. In general, the corrugations of adjacent turns line up so that they either touch each other or are closely spaced from one another. Welds are applied at selected locations to thereby connect adjacent turns. At least one weld is arranged along each turn. In a particularly preferred embodiment, the welds are oriented relative to one another to form a helical pattern of welds along the endoprosthesis.
It is therefore a general object of the present invention to provide an improved endoprosthesis with a plurality of welds, as well as their preparation.
It is a further object of the invention to provide an improved endoprosthesis or support which has good strength while having the ability to conform to the contour of the vessel in which it is implanted.
It is another object of the invention to provide an improved endoprosthesis which minimizes the risk of developing intimal hyperplasia or irritation induced by its insertion within a living vessel.
It is a further object of the invention to provide an improved endoprosthesis and insertion procedure whereby the support device can be extended by at least 0.5 cm on either side of the dissection d. H. of the section under treatment, even in the case of a long section having no straight contour.
It is a further object of the invention to provide an improved endoprosthesis and method for providing a support device of fidelity comparable to that of a much less flexible support device, and still having a flexibility that can be achieved Many applications are required, including inside coronary vessels.
It is a further object of the invention to provide an endoprosthesis with multiple melt-welded joints which has a flexibility that is only 10 to 15% less than that of a similar device without welded joints, while at the same time preserving the dimensional accuracy of support structures Provides which have much lower flexibility or compliance properties.
It is a further object of the invention to provide an improved brace or endoprosthesis with exceptional uniformity in having a support surface area over the entire working surface of the brace.
It is a further object of the invention to provide an improved endoprosthesis or support which decreases in length during deployment during deployment while increasing the pitch of the helix which roughly defines the configuration of the endoprosthesis.
It is a further object of the invention to provide an improved endoprosthesis or support device which, when inserted, avoids overlapping structural elements of the support device so as to provide a minimum thickness support over the entire support device to reduce the likelihood of accelerated hyperproliferation or thicker cell growth as a protective reaction against a thickened wall surface.
It is a further object of the invention to provide endoprostheses and their preparation, which can be simultaneously tailored for the particular end uses desired, including so-called vascular, bronchial, tracheal, urological, rectal, trans-interhepactic shunting, the so-called "bilary-tree" treatment method in the field of bile duct plastic and the like.
It is a further object of the invention to provide an improved multi-weld endoprosthesis that substantially reduces the external expansion of a support device when inserted into curved-contoured vessels.
These and other objects, features and advantages of this invention will be more clearly understood by reference to the following detailed description.
Kurzbeschreibung der ZeichnungBrief description of the drawing
The present invention is further explained in the following description with reference to the accompanying drawings.
Es zeigt:It shows:
Beschreibung der einzelnen AusführungsbeispieleDescription of the individual embodiments
Expansion is initiated after the balloon and endoprosthesis are positioned within the vessel so as to be radial from a diseased or damaged area of the vessel are spaced. In radial expansion as described, the balloon inserts the endoprosthesis to abut and support the diseased or damaged portion. It has been found that the effectiveness of this support procedure is particularly improved when the endoprosthesis spans a greater length than the length of the diseased portion, so that there is an overlap of at least about 0.5 cm of the endoprosthesis over each end of diseased or damaged Sections results. Accordingly, the insertion procedure of the present invention includes providing an endoprosthesis having a length greater than the length of the diseased area when the endoprosthesis is disposed along the diseased area, taking into account changes in the contour of the vessel at the diseased portion.

While the illustrated preferred embodiments show the plurality of welds in a helical fashion along a generally continuous path or generally helical line of connection points, other weld alignments are also possible. For example, one of the lines of weld connection points having a multiple connection point line configuration may omit welds therealong, so that every other adjacent pair of connection sections may remain unwelded along this interrupted helical line of connection points. Also, in a multi-line configuration of junctions, adjacent pairs of alternate junctions may remain unwelded, preferably staggered, such that each adjacent coil is secured to the adjacent coil (s) by at least one weld at a pair of junctions. It is possible to align helical lines of connection points in a different orientation than the one depicted following the pitch angle of the wound helix, for example patterns of weld joint lines that are generally parallel to the axis of the endoprosthesis, and patterns of weld joint lines that are counterclockwise aligned helix follow, instead of the clockwise aligned helical line of connection points shown in the drawings. In addition, although the drawings illustrate endoprostheses with two or three patterns of weld joint lines, patterns with a single line of connection points or four or more lines of connection points are also possible.
More specifically, it is presently generally accepted that the support surface area (typically the "metal" outside or working surface of the support) should be between about 12% and about 15% of the cylindrical surface defined by the support. Otherwise, insufficient support would be provided. This means that under present assumptions it is desirable to have between about 85% and about 88% open space presented by the external cylindrical boundary of the support. The configuration of the support device according to the invention is designed to fall within these guidelines. More importantly, the present invention provides a structure in which the amount of support surface or "metal" provided to the vessel by the support device is a consistent percentage throughout the entire length and circumference of the support device. Thus, if 12 to 15% of the support surface is provided by the support, all portions of the cylindrical support surface, both prior to expansion and expansion upon insertion, provide a support surface area within this percentage range. This uniformity of support surface is important. This feature avoids, for example, the undesirable situation in which a support device may meet the 12-15% guideline, taking the average of the entire support surface, but at the particular location of the support device where support is most needed below the policy percentage. Similarly, if certain locations of the support provide too high a percentage of support surface or metal, accelerated hyperproliferation may occur, resulting in thicker than desired cell growth and constriction of the body passageway at those sites of excessive support area this position would result.
Endoprostheses made in accordance with the invention are also particularly well suited for insertion into vessels with curved contours. It will be understood that the combination of non-welded pairs of connecting sections and welded pairs of connecting sections of the endoprosthesis allow it to lie and conform to a bend in a vessel without having excessive distances between non-welded connecting section pairs, for example on an outside of a curve or a curvature with a larger radius. It has been found that the outer curvature of the endoprosthesis opens in similar support device structures without the welding pattern about two to three times as far as the spacing between adjacent turns without expansion and in the longitudinal direction. The present invention dampens this expansion by at least about 60% to 70%, while still allowing the endoprosthesis to conform to the natural contour of the vessel. This results in a significant reduction of excessive clearance that the outer curvature of the endoprosthesis has. In addition, the weld pattern helps to prevent excessive overlap of endoprosthesis strand material to the inner curvature of the vessel contour.
Stranded material from which the endoprostheses of the present invention are made must be capable of bonding under welding or heating conditions. In addition, the stranded material should have formability properties. Included are tantalum, titanium, silver, gold, heat-treated elastic metal materials, and alloys containing them. Polymers may also be used, such as polyethersulfone, polyimides, polycarbonates, polypropylenes, high molecular weight polyethylenes, carbon fibers, kevlar polymer, and the like. It is also possible to coat these materials after the completed formation of supporting devices having porous or structured surfaces for ingrowth of cells and the like or with non-thrombogenic agents such as pyrolytic carbon, heparin, hydrogels, Teflon materials, silicones, polyurethanes and the like. Treatments can also be made so that drugs or drugs can be eluted therefrom. It is also possible to make certain support devices from physiologically degradable materials. Of course, the stranded material must be physiologically compatible. Tantalum is particularly preferred as stranded material. For example, materials such as tantalum have the ability to plastically deform without significantly affecting the strength of the metal. Such a property is typically not offered by more elastic materials, such as stainless steel, which lose a significant percentage of their strength after flexing.
It will be understood that the described embodiments of the present invention are illustrative of the application of the principles of the present invention. Numerous applications can be made by those skilled in the art without departing from the spirit and scope of the invention.
The invention thus provides an endoprosthesis having an endoprosthetic body of a helically wound strand of generally mouldable material and exhibiting a repeating pattern of corrugations following a generally helically wrapped axis. Adjacent full circular convolutions each have at least one weld joining crest-like portions of adjacent complete circular convolutions. In a preferred embodiment, a plurality of these welds define a generally in-line helical pattern of welds along the endoprosthesis or support device. One, two, three or more of these substantially in-line helical patterns of welds may be provided. Also provided is a method of forming the endoprosthesis comprising fusion welding, typically in an inert gas atmosphere and such that the entirety of the support device lies along the same generally cylindrical three-dimensional spatial surface. In one application, the endoprosthesis or support device is implanted by insertion with a suitable expander, wherein upon insertion the pattern of corrugations is expanded in a uniform manner to provide a particularly consistent support surface throughout the endoprosthesis.
