Abstract

The support surfaces (12, 13) of the implant are formed by the outsides of parallel running arms (3, 4) of a U-shaped support body (1). In a rib (5) of the support body connecting the arms in one piece with one another, a tapped hole (8) is arranged in which a screw (9) is screwed. The screw locates on the inner surfaces (6, 7) of the arms adjacent to one another and bends the arms more or less outwards. The screw in its point area (10) conically narrows and is free of threads. The screw in the front third of the arms locates on their inner surface. The thickness of the arms from the rib joining them to the free end increases.

Description

The invention relates to an intervertebral implant adjacent vertebral bodies and supporting surfaces an adjusting device for changing the relative position of the Support surfaces.

After the removal of intervertebral bodies, there is the Danger of neighboring vertebral bodies immediately touching one another lying there and sham by this unnatural misalignment damage to both the support apparatus and the surrounding area Soft tissues enter.

To avoid this, it is known the intervertebral vertebrae body after removal through an intervertebral implant to replace, for example in the form of a cuboid or egg a wedge. The insertion of such intervertebral bodies is difficult, therefore intervertebral bodies are common used that are not the required size. If the intervertebral bodies are chosen slightly smaller  The implantation is easier, but the support can function cannot be completely fulfilled.

It is already known to ver vertebral implants turn, the external structure after insertion by a suitable adjustment device can be changed. So be For example, WO 92/14423 writes an intermediate wire bel implant with a tie rod, in which between neighboring beard end faces of the implant pivotally arranged nete side wall sections by a tie rod more or can be pivoted less to the outside. It will this enables the size of the intervertebral implant after implantation change, building a sol Chen intervertebral implant is exceptional complicated and also leaves no area of the Support surface on the vertebral body too, since the support surfaces are small must be pivoted spatially.

Außerdem ergibt sich hier nur eine linienförmige Anla ge, da die Außenfläche dieses Implantats einen kreisförmi gen Querschnitt aufweist.EP 260044 B1 describes a two-part intervertebral Known implant, in which between two parts spreader are arranged by, also by tie rods and axia le shifting the spreading surfaces away from each other can. Here too, the structure of the implant is relative complicated because a larger number of parts is necessary is. In addition, there is only a linear layout here ge, since the outer surface of this implant has a circular has a cross-section.

A similar structure of an intervertebral implant writes WO 90/00037. Wedge-shaped expansion elements between Two support surfaces are increased by a tie rod or less approached each other and press the  Support surfaces apart. Again, there is a large number of individual parts necessary for the intervertebral Implant must be assembled so that the danger malfunctions cannot be excluded.

It is an object of the invention to provide a generic intermediate to improve vertebral implant so that a particularly simple Safe and robust construction and trouble-free function are guaranteed.

This task is performed with an intervertebral implant solved in accordance with the invention, that the support surfaces through the outside of the in essence Chen parallel legs of a U-för Migen support body that are formed in one of the Schen kel integrally connecting web of the support body pers a threaded hole is arranged and that in this a screw is screwed in to the free end the thighs approaching inner surfaces the leg rests and the leg accordingly its screwing depth more or less bends outwards.

So it becomes in contrast to all other known Zwi vertebrae implants with an adjustment option lumpy support body related, in the as adjusting device only one screw is screwed in. These The screw lies directly on the inner surfaces of the legs of the support body and presses them depending on their on screwing depth more or less apart. This two vertebra implant is extremely simple because it is consists of only two parts, and it allows despite the very sensitive adjustment of the support surfaces al  lein by bending the legs outwards. This ge ring-shaped pivoting of the support surfaces is also made of ana tomic view favorable because in this way a slight common lordosis of the vertebral bodies can be achieved this size can also be adjusted with the screw just adjust. There is still the option of Amount of spread to vary by differing screws are screwed into the support body the absolute amount of the Set the adjustment per screw depth of the screw be occurs.

A special advantage of bending the legs is also to be seen in the fact that this causes the implant to slide in dorsal direction is prevented with certainty since the Vertebral space in this direction narrows and that Implant due to its wedge shape on another ver movement in the dorsal direction is prevented.

It is favorable if the screw is in its tip position richly tapered, this way it can be larger Contact surface of the screw on the inner surface is reached that and therefore less friction between the screw and inner surface when screwing in. It is still there advantageous if the screw ge in its tip area is designed to be windless.

The screw is preferably in the front third of the Legs on their inner surface so that the legs then once through the web at the rear end and then through the Screw in the area of the front end are supported.

It is favorable if the thickness of the legs is different the web connecting the legs is enlarged towards the free end. By the degree of the change in thickness can otherwise also the bending curve of the leg to a small extent influence, so that the bending curve of the Leg can be adapted to the shape of the vertebral body.

The inner surfaces of the unbent legs can be made of be educated.

It is favorable if at least the support surfaces are structured are. This results in a fixation of the support body pers between the vertebral bodies and also this can Structuring help keep the bone material in ingrows the support body.

For example, the structuring can be carried out by the support protruding spikes or tips are formed or through recesses worked into the supporting surfaces. The For example, they can be bores or slots be educated.

It is particularly favorable if the structuring by egg ne porous coating of the support surfaces is formed, the at the same time a particularly bone-friendly surface can be so that the bone material in this surface ingrows.

The support body is preferably in the undeformed state cuboid.

In a special embodiment it can be provided that the support body is seen from above on its web side  one-sided by a bevel or by an arch is shortened. This makes it possible to stay longer overall trained support body in the space between two vertebrae accommodate without the support body over the cross the roughly circular vertebral body cut along protrudes from a corner. The enlargement of the support surface leads to lower surface pressures and thus to a low one ren danger that the support body into the bone material sinks.

Es zeigen:The following description of preferred embodiments the invention serves in connection with the drawing of the detailed explanation. Show it:

Außerdem nimmt die Dicke der Schenkel 3, 4 vom Steg 5 zu den freien Enden der Schenkel 3, 4 hin zu.The intervertebral implant of the present invention comprises a substantially U-shaped support body 1 with a cuboid cross-section. In this support body 1 , an interior 2 with a trapezoidal cross-section is excluded, so that the support body 1 has two legs 3 and 4 which run essentially parallel to one another and an essentially perpendicular to it, the two legs 3 and 4 connecting web 5 . Due to the trapezoidal cross section of the interior 2 , the inner surfaces 6 , 7 of the legs 3 , 4 are inclined towards one another in such a way that they approach each other towards the free ends of the legs 3 , 4 . In addition, the thickness of the legs 3 , 4 increases from the web 5 to the free ends of the legs 3 , 4 .

In the web 5 there is a continuous threaded bore 8 , into which a screw 9 is screwed, which has a thread-free contact surface 10 in the form of a truncated cone at its free end. This lies in the front third on the two inner surfaces 6 , 7 and widens the interior 2 more or less depending on the screwing depth, that is, the legs 3 , 4 are bent elastically outwards under the influence of the screw 9 . The screw 9 can be screwed in, for example, with a polygonal wrench 11 which plunges into a polygonal opening of the screw, not shown in the drawing.

The outer surfaces of the legs 3 , 4 form support surfaces 12 , which are essentially flat when the support body 1 is undeformed and which, when the screw is screwed in 9, bulge slightly outwards into the support body 1 . These support surfaces 12 , 13 can be provided with a structure in a manner not shown in the drawing, for example tips or mandrels can be placed there or holes can be drilled into the support surface 12 , 13 . This structuring improves the contact with bone tissue lying against the support surface 12 . In another embodiment, the support surfaces 12 , 13 and possibly the entire support body 1 are coated with a rough surface, for example with a microporous surface, so that on the one hand the roughness is increased and on the other hand the engagement with adjacent bone tissue can be improved.

The support body 1 and the screw 3 are made of body-compatible metals, metal alloys or plastics, for example titanium.

As is clear from the illustrations of FIGS. 1 and 2 shows the supporting body 1 is so inserted between two adjacent vertebral bodies 14, that the support surfaces 12 and 13 men kom surface of the adjacent vertebral bodies to the plant. The support bodies 1 are always inserted in pairs between two vertebral bodies, the webs 5 are oriented in the dorsal direction, the free ends of the legs 3 and 4 in the ventral direction. In this way, it is possible to screw the screw 9 from the back into the support body 1 more or less deeply, thereby bending the legs 3 , 4 to achieve and thus a change in the distance and possibly to a small extent mutual orientation of the neighboring vertebrae by 14 . With the simplest structure of the support body 1 , the necessary support functions can be taken over by this men. It is particularly advantageous that the intervertebral implant can also be removed in a simple manner. It is sufficient to unscrew the screw 9 so far that the legs return to the undeformed position, then the support body 1 can be removed in a simple manner and replaced if necessary.

While the previously described support body has cuboidal outer contours, it is possible to deviate slightly from it. For example, the parallelepiped-shaped support body 1 in the region of the web 5 can have a cross section which is chamfered at a corner or shortened by an arc, as shown by the dashed boundary line 15 in FIG. 1. A modified in this way support body 1 is adapted to the outer contour of the vertebral body 14 and can therefore be formed accordingly longer without protruding pers 14 on the contour of the vertebral body. Since the described intervertebral implants are constructed completely symmetrically to a horizontal plane, it suffices to use the side beveled in this way or provided with an arcuate contour 15 support body 1 in different orienta tion, so that the left corner on one side and on the other hand, the right corner of the support body is shorter, as shown in Fig. 1.
