Description

The invention relates to a medical endoscopy system for examining and / or treating a part of the body inside a body cavity by means of laser radiation.

Such medical endoscopy systems are used for endos copied interventions used in which with the help of La radiation in a body cavity an examination treatment. For example can with laser radiation with a wavelength of about 1 µm body tissues are coagulated while laser radiation with a wavelength of about 3 µm in particular good for removing and cutting body tissue, whereby the laser radiation has only a minimal thermal Damage to the surrounding areas of the treatment area causes.

For such an endoscopic procedure it is in the Usually required multiple radio in the patient's body opening channels. A first Ka nal the laser radiation with the help of a beam guidance unit, which is coupled with an endoscope, into the body cavity led. To the examination or treatment To be able to carry out is also an observation channel  required through which the examination or Treatment area with the help of observation optics can be observed during the laser application. There the body cavity to be illuminated for observation a third channel is also required, through intense visible illuminating light radiation can be passed through.

The object of the invention is a medical endosco Piesystem so that the endosko medical examination and / or treatment required handle can be reduced.

This task is solved by a medical endosko piesystem of the type described above with a Be fluorescent light source and a laser radiation source, the visible illuminating light radiation or La emit radiation, with a beam guidance unit, which both the illuminating light radiation and the La radiation to an insertable into the body cavity, endoscope coupled to the beam guide unit, and with the illuminating light radiation and the la radiation into the common beam guidance unit coupling coupling unit.

In the endoscopy system according to the invention, the for to observe the body cavity to be treated illumination light radiation together with the La radiation using a single, common beam guide unit, which is coupled to an endoscope, in guided the body cavity, so that for the laser applique tion and the lighting as a whole only one functional channel is required. The beam guidance is the Be fluorescent light radiation together with the laser radiation  no larger diameter of the beam guidance unit is required than for the beam guidance only one of the two Radiation types, so that with the help of the invention medical endoscopy system the scope of the endosko intervention can be significantly reduced.

While for laser application usually laser beam in the middle infrared region, that is with a Wavelength of about 1 to 3 µm, can be used as Illumination light source is a cold light source, for example wise a mercury vapor lamp are used, whose visible light radiation is particularly good for observers attention to the treatment and examination area is.

The illuminating light radiation and the laser radiation who in an advantageous embodiment of the invention in the coupling unit using a beam combination tion element directed into a common beam path and then using a focusing element on the focused proximal end of the beam guidance unit.

With a particularly simple and inexpensive manufacture The embodiment is in focus a coupling lens.

To radiation losses of the laser radiation on the way from their entry into the beam guidance unit up to the endos To keep cop low, the beam guidance unit is off a high transmission in the spectral range of ver used material formed with laser radiation, that also in the visible spectral range for the Transmission of illuminating light radiation only ver causes negligible transmission losses.

In a particularly advantageous embodiment of the Er Invention is provided that the beam guidance unit passed through the endoscope and in the area of the dis tale end of the endoscope from a body-compatible Material is formed so that radiation losses within of the endoscope are kept low and the beam guide unit in the area of the distal end of the endos kops can come into contact with body tissue without this causes the contacted body tissue to ignite. At for example, it can be provided that in a di stalen end of the endoscope adjacent area with the Beam guiding unit detachably connectable application fa This piece is made of a particularly body-friendly material is arranged.

To both the laser application and the Illumination of the treatment area for endoscopic To enable observation, it is advantageous if on distal end of the endoscope with the beam guide Unit coupled radiation optics is arranged.

The endoscope can be rigid, especially in front However, it is partial if the endoscope can be angled is trained.

In an advantageous embodiment of the medical Endoscopy system is provided that the beam guidance Unit comprises an optical fiber. The optical fiber can be formed, for example, as crystalline sapphire fibers det be or as zirconium fluoride (ZrF₄) or Chalcogenide (e.g. AsS, GeAsS, GeSeTe) fiber. In addition, the optical fiber can also be used as anhydrous Quartz fiber can be formed.

Because in particular for the treatment of body tissues Laser radiation with a wavelength of approximately 1 to 3 µm is suitable in an advantageous embodiment of the Invention provided that the laser radiation source Solid state laser includes. Such lasers stand out characterized by high stability and generate laser radiation in the desired spectral range with high energy.

It is particularly advantageous if the solid-state laser is tunable, so that depending on the desired effect Laser application Laser radiation with different Wavelength can be generated.

It is advantageous for the solid-state laser with a chrome to use doped laser active material. Especially for tunable solid-state lasers, for example, a GGG-, a GSGG-, a YAG-, a BeAl₂O₄-, a LiCAF (LiCaAlF₆) - or a LiSAF (LiScAlF₆) crystal, each because it is doped with chrome, used or with Cerium doped LiCaAlF₆ crystal.

In a further embodiment of the invention is before seen that the solid-state laser as a laser-active material comprises a YAG crystal. This can be used to generate Laser radiation with a wavelength of 2.94 µm with Er be doped with bium or with holmium or neodymium in particular for generating laser radiation with a Wavelength of 2.1 µm or 1.06 µm suitable are.

Further for the laser radiation source of the fiction suitable laser-active materials according to the endoscopy system are, for example, Alexandrite, Erbium-doped YSGG- Crystals or sapphire crystals doped with titanium.

In a particularly preferred embodiment of the inven It is intended that the medical endoscopy system several laser beams with different wavelengths emitting laser radiation sources. That’s it possible, at the same time laser radiation with difference Lichen generate wavelengths with the help of the one coupling unit and the beam guidance unit depending on ge desired medical effect simultaneously or sequentially other can be applied. To this end, the Coupling unit additional beam combining and Beam deflecting elements, so that by the individual laser radiation sources outgoing laser radiation into one single beam path can be directed to closing on the proxi using the focusing element male end of the beam guidance unit to be focused.

In a particularly preferred embodiment of the inven the medical endoscopy system comprises several La radiation sources in the form of differently doped Solid state laser. For example, an Er: YAG laser, a Ho: YAG laser and an Nd: YAG laser can be used, so that at the same time laser radiation with a wavelength of 2.94 µm, 2.1 µm and 1.06 µm are generated, each according to the desired radiation effect, for example to Ko agulation or used to remove body tissue can be.

Since in most applications as laser radiation for the human eye uses invisible radiation is in an advantageous embodiment of the invention provided that the medical endoscopy system a Light source to generate a visible target beam summarizes the optical means of the coupling unit in the beam guidance unit is coupled. With the help of  Aiming beam, along with the illuminating light radiation and the laser radiation using the beam guidance unit directed to the body part to be treated the direction of the laser radiation can be determined from the Direction of the visible target beam can be determined.

Abs light source to generate the visible target beam the medical endoscopy system, for example have a diode laser.

In a further preferred embodiment of the invention It is intended that the medical endoscopy system a beam splitting element comprises for separating from the Body cavity out by means of the beam guidance unit led reflection from the inserted into the body cavity led illuminating light and laser radiation. Will that Body tissue in the body cavity illuminated or with a laser radiation is irradiated, so part of the radiation from the Body tissue scattered back into the beam guidance unit and from this to the proximal end of the beam guide transfer unit. The backscattered radiation occurs on proximal end of the beam guiding unit from this and can with the help of the beam splitting element from the Beam path of the incoming radiation led out who the. The retroreflection contains information about the Condition of the irradiated body tissue and can with the help be analyzed by a measuring device.

The beam splitting element can be, for example, a dichroic be designed beam splitter.

The following description of a preferred Aus management form of the invention is used in connection with the Drawing of the detailed explanation.

The drawing shows a schematic representation of a medical endoscopy system with a jet generating unit 10 , which is supplied by a supply unit 12 with electrical energy and cooling water and which can be controlled with the aid of a control unit 14 . A treatment and illumination beam 22 generated by the beam generation unit 12 is guided with the aid of an optical fiber 16 to a distal end 20 of an endoscope 18 .

In the beam generating unit 10 , three laser radiation sources 24 , 26 , 28 and a coupling unit 21 are arranged.

The laser radiation sources 24 , 26 , 28 each comprise a pump light source 30 for exciting a laser-active material as well as a highly reflective end mirror 32 and a partially transparent coupling-out mirror 34 , which together form a resonator arrangement. While an erbium-doped YAG crystal 36 is positioned in the laser radiation source 24 between the two mirrors 32 and 34, YAG crystals 38 , 40 doped with holmium or neodymium are used in the laser radiation sources 26 and 28 .

From the laser radiation sources 26 and 28 each outgoing laser radiation 42 or 44 is deflected via deflection mirrors 46 or 48 positioned in the coupling unit 21 and also in the coupling unit 21 arranged coupling mirrors 50 or 52 into the beam path of a laser radiation 41 emanating from the laser radiation source 24 . In this beam path with a partially transparent mirror 54 from an illuminating light source in the form of a mercury vapor lamp 56 outgoing illuminating light radiation 58 is coupled in and thereby the treatment and lighting device beam 22 is formed.

The laser radiation 41 , 42 , 44 emitted by the laser radiation sources 24 , 26 , 28 , which, owing to the different doping of the laser-active materials, has three different wavelengths, namely 2.94 μm in the case of the Er: YAG crystal and in the case of the Ho: YAG Crystals 2.1 µm and in the case of the Nd: YAG crystal 1.06 µm, are coupled into the optical fiber 16 together with the illuminating light radiation 58 with the aid of a converging lens 60 .

The optical fiber 16 guides the treatment and lighting beam 22 to the distal end 20 of the endoscope 18th A radiation optics 19 is positioned at this end, via which the treatment and illumination beam 22 can be projected onto the part of the body to be treated.

A further deflecting mirror 62 and a coupling mirror 64 are additionally arranged within the beam generation unit 10 , with which a target beam 66 emitted by a diode laser 65 likewise positioned in the beam generation unit 10 can be directed into the beam path of the treatment and illumination beam 22 . The target beam is guided via the optical fiber 16 to the distal end 20 of the endoscope 18 and emerges from the endoscope in the direction of the laser radiation 41 , 42 , 44 which is invisible to the human eye. The direction of the infrared laser radiation can thus be determined on the basis of the visible target beam.

A dichroic beam splitter 68 is positioned in the beam path of the treatment and illumination beam 22 between the converging lens 60 and the optical fiber 16 . This allows retroreflection 70 , which is scattered back from the body part during treatment or examination of a body part in a body cavity and is guided out of the body cavity via the optical fiber 16 , separated from the treatment and illumination beam 22 and fed to a measuring device 72 . In the measuring device 72 , the retroreflection 70 can be analyzed so that information about the state of the irradiated part of the body can be obtained.

With the help of the optical fiber 16 , both the illuminating light radiation 58 and the laser beam reflections 41 , 42 and 44 are guided to the distal end 20 of the endoscope 18 , and in addition the reflection 70 is conducted in the opposite direction from the distal end 20 to the measuring device 72 . Thus, for endoscopic treatment, it is not necessary to open separate functional channels for the illuminating light radiation 58 on the one hand and the laser radiation 41 , 42 , 44 on the other hand and additionally for the back radiation 70 , but a single functional channel is sufficient, the diameter of which is extremely small can be kept, since it must be suitable for digesting through it an optical fiber 16 comprising an endoscope 18 who can.
