Abstract
Description
The invention relates to a surgical sewing instrument Perform a surgical suture. The sewing instrument should through an instrument channel to the sewing site and the sewing process can be controlled from the outside.
With such instruments, the surgical intervention remains on Body of a patient limited to the bare minimum, since the Instrument channel through natural orifices or inserted through a limited incision in the body becomes.
A sewing and stitching machine is known from EP 01 74 843 A2, through an instrument channel into a patient's body is insertable and can be operated from the outside. The sewing head of the machine is open to the side. A suction pipe ends there, can be sucked over the tissue, so that a tissue fold comes about. A needle with a threaded thread, moved then parallel to the machine axis and pierces the tissue. The thread can be carried over a safety gear are kept loop-forming. A knot is made separately.
Auf jeden Fall sind zusätzliche Instrumente dazu notwendig.US 49 35 027 discloses a surgical instrument as also a procedure by remote or endoscopic can be sewn. The pliers of the instrument are hollow and, when closed, form an eyelet through which a sewing thread can be pushed or transported. The pliers ends are designed in such a way that they have substances to be sewn despite the hollow get through. After each sewing step or the sewing process in general, the nodes are z. B. from the outside and is then pushed to the knot. Definitely additional ones Instruments necessary for this.
The invention has for its object a through an instrument channel insertable and manageable sewing instrument Provide with the surgical sutures on tissues or vessels and knots largely without additional instruments are feasible.
This object is characterized by those in claim 1 Features solved.

Fig. 6 Kopf des chirurgischen Nähinstruments mit weit aufklappbarem Maulteil Fig. 6 head of the surgical sewing instrument with a jaw part that can be opened wide
In Fig. 1 the head of the sewing instrument 1 is shown in side view. It consists of two mouth pockets 2, 3 . One rigid jaw part 2 is parallel, ie next to the instrument axis 4 . The other, movable jaw part 3 can be opened and closed and is mounted on the hinge axis 5 . It can be rotated by a design-limited angle α. For this purpose, a coaxial tube 6 has to be moved back and forth in the instrument tube 7 , which at the end is coupled to the jaw part 3 via a bolt 8 which runs in an elongated hole 9 of the movable jaw part 3 . If a needle 12 is clamped in one of the clamping devices 10, 11 , the movable jaw part 3 can be folded onto the rigid jaw part 2 to such an extent that the two ribbed surfaces 13, 14 fit into one another in a coherent manner. The head 1 of the sewing instrument can thus be used as a gripping device. The dashed outline of the jaw part 3 is drawn in this position.
At the distal end of the two jaw parts 2, 3 , immediately behind the ribbed surfaces 13, 14 , a clamping device 10, 11 , which can be detached separately at any time , is installed. Both clamping devices 10, 11 have a tip-shaped depression, which have a round or triangular or polygonal cross section, depending on the needle 12 used .
The clamping device 10 in the rigid jaw part 2 consists of two halves 15, 16 . One half 15 is firmly connected to the jaw part 2 . The other half 16 is mounted on the head of the sliding device 17 and can be moved back and forth with it, so that this clamping device 10 can be opened and closed. In the closed state, the clamping device 10 has a bead 18 in front of the tip. As a result, the sewing needle 12 can be clamped there in a form-fitting manner.
The sliding device 17 itself is coupled to a further, coaxial tube 19 which runs in the instrument tube 7 . The sliding device 17 can thus be operated remotely. Furthermore, it is covered from the half 16 of the clamping device 11 mounted on it to far beyond the overlap area of both jaw parts 2, 3 by a sheet 20 against the environment. The jaw 2 and this sheet 20 form a channel in which the sliding device 17 can be moved back and forth over the tube 19 .
The clamping device 11 in the movable jaw part 3 also has a tip-shaped depression, but consists of a piece and an elastic, resilient component 30 which lies around the needle and presses it in the axial direction and at the same time keeps it centered or defined. As a result, the needle can only be clamped or pulled out with a certain amount of axial force, ie it can be held by this clamping device 11 alone, since the resilient component 20 , which can be a spring ring, bears behind the tip of the needle in the tapered area.
The length of the head of the sewing instrument is approximately 17.0 mm in this exemplary embodiment. From the needle 12 to the joint axis 5 , it is approximately 12.0 mm. The jaws 2, 3 have a thickness of approximately 1.5 mm in the area of the clamping device. The device is suitable for an instrument channel of 5 mm or larger. Depending on the application, the design dimensions of the head of the sewing instrument can be modified.
Fig. 2 shows a plan view of the head 1 of the sewing instrument. The contour of the movable jaw part 3 has broken off in front of the clamping device. The top view shows the ribbed surface 13 on the rigid jaw part 2 and the associated clamping device with its pointed depression. The sheet 20 , which covers the sliding device 17 in the free area of the jaw part 2 , is guided in two grooves 21, 22 .
The sewing needle itself is boat-shaped and symmetrical to its longitudinal axis. It is shown in Fig. 3. It has uniform tips 23 at both ends, which connect to the needle body in a bead-like manner. The transition from the tip 23 to the sewing needle body 24 initially tapers like a cone and then merges into the needle body 24 . In the needle body 24 itself there is a longitudinal cavity 25 with the transverse bore on the bottom, the eye of the needle. There in particular the edges are all rounded. The sewing thread ends at this point and is fastened there without protruding. He also lies in the longitudinal cavity 25 during the sewing process, so that undesirable tears in the tissue when piercing are avoided.
Fig. 4 shows the longitudinal section through the needle 27 and Öhrachse 28th
Fig. 5 stresses once again, the design of the both sides of the same needle tip. The spring ring 29 , which tensions around the taper, advantageously exerts a clamping force on the contacting circumference in such a way that the needle is pressed both in the axial direction into the clamping device 11 and is held in the center. The tapered conical shape at this point is advantageously suitable for maintaining this defined position.
Das Arbeiten mit dem Nähinstrument läuft beispielsweise folgendermaßen ab:For example, working with the sewing instrument is ongoing as follows:
First, the needled at one end of the suture and attached needle 12 is longitudinally placed between the two folded or nearly folded jaw parts 2, 3 and may be clamped at one of its tip between the two corrugated surfaces 13,14. The sewing instrument is then guided through the instrument channel to the sewing location. The sewing thread is also pulled through the instrument tube 7 . At the sewing location, the movable jaw 3 is opened and the sewing needle is inserted into one of the clamping devices, preferably in the rigid jaw part 2 , with the aid of a holding instrument introduced via another instrument channel, since this can be opened and closed from the outside. A first stitch can now be made through the tissue to be sewn. Then the rotatable jaw part 3 is folded in the direction of the jaw part 2 until finally the still free needle tip snaps into the clamping device with the resilient component. The needle is then released from the clamping device 10 by the sliding device 17 and completely pulled through the stitch together with the sewing thread. The sewing process can be continued accordingly by stitch and needle transfer. If necessary, however, a knot can also be carried out after each stitch and thread pull, the tying of the knot possibly being carried out with the aid of an additional instrument.
Fig. 6 shows a further embodiment of the suturing instrument. Here, the movable jaw part 3 can be opened over 90 ° relative to the rigid jaw part. This can no longer be achieved using a simple lever drive, as indicated in FIG. 1.
A possible facility for this is e.g. B. that the movable jaw part 3 is opened and closed via a gear.
If the longitudinal axis 31 of the needle 32 used for the sewing process has the shape of the circular path 34 , which the tip of the clamping device 11 on the movable jaw part 3 describes when opening and closing, it is given that the needle 32 only pierces the possibly thicker tissue to be sewn and does not gather or even tear in the direction of the jaws during the sole folding movement. The needle 32 clamped on the movable jaw part 3 in FIG. 6 can be tightly surrounded by an elastic component 33 made of material suitable for surgical purposes in front of the clamping device II and flush with the jaw part 3 . For the sake of clarity, this is only shown on the movable jaw part 3 . This can also be so on the fixed jaw part 2 and also on the sewing instrument of FIG. 1.
