Description
Die Erfindung betrifft ein implantierbares Hörgerät zur Anregung des Innenohres, mit
einem elektromechanischen Wandler und einem Koppelelement, das mit seiner vom Innenohr
abgewandten Seite mit dem schwingenden Teil des Wandlers verbunden ist und
dessen anderes Ende durch eine artifizielle Bohrung unter Umgehung der Gehörknöchelchenkette
in die flüssigkeitsgefüllten Innenohrräume ragt.
Bei einem bekannten Hörgerät dieser Art (DE 39 18 086 C1) ist zum Übertragen der
von dem elektromechanischen Wandler erzeugten mechanischen Schwingungen ein hydromechanisches Koppelelement vorgesehen, bei dem es sich vorzugsweise um einen
flüssigkeitsgefüllten Schlauch handelt. An dem dem elektromechanischen Wandler zugewendeten
Schlauchende steht der Innenraum dieses Schlauches mit einer flüssigkeitsgefüllten
Kammer des Wandlers in Verbindung. Das von dem Wandler abliegende Ende
des flüssigkeitsgefüllten Schlauches ist mit einer dünnen Membran abgeschlossen. Dabei
werden die mechanischen Schwingungen der Wandlermembran in Druckschwankungen
in der Flüssigkeit des hydromechanischen Koppelelements umgesetzt. Diese Flüssigkeitsdruckschwankungen
werden ihrerseits in mechanische Bewegungen der Schlauchabschlußmembran
umgewandelt, die dann letztlich Druckschwankungen in den flüssigkeitsgefüllten
Räumen des Innenrohres bewirken. Die hydromechanische Kopplung
zeichnet sich dadurch aus, daß auf der Basis des hydrausischen Prinzips am Ende des
Koppelelements größere Auslenkungen im Vergleich zur antreibenden Wandlermembran
und damit hohe äquivalente Ausgangsschalldruckpegel erreichbar sind. Problematisch
sind hier im Hinblick auf die technische Realisation hygienische Fragen der keimfreien Flüssigkeitsfüllung und der langzeitstabilen Ankopplung an das Innenohr.
Der Erfindung liegt die Aufgabe zugrunde, ein Hörgerät mit einem technisch vereinfachten
Wandlersystem zur Anregung des Innenohres mit hoher Klangqualität zu schaffen,
das eine problemlose langzeitstabile Ankopplung an das Innenohr erlaubt.
Ausgehend von einem implantierbaren Hörgerät der eingangs genannten Art wird diese
Aufgabe erfindungsgemäß dadurch gelöst, daß das Koppelelement als steifes mechanisches
Koppelelement zur direkten Übertragung der mechanischen Schwingungen
des Wandlers an das Innenrohr ausgebildet ist.
Mittels des Hörgerätes nach der Erfindung können die Schwingungen der Membran des
elektromechanischen Wandlers über das mechanische Koppelelement ohne die Not
wendigkeit einer Abschlußmembran wie beim hydromechanischen Koppelelement der
DE 39 18 086 C1 direkt auf die Innenohrräume übertragen werden. Der elektromecha
nische Wandler kann dabei so ausgebildet sein, daß er auf dem Promontorium fixierbar
ist und das mit ihm verbundene mechanische Koppelelement durch eine Bohrung in der
Promontorialwand in die Skala tympani führbar ist. In einer weiteren Ausgestaltung der
Erfindung kann der elektromechanische Wandler auch so geformt sein, daß er nach
Entfernung des Steigbügels auf der Steigbügelfußplatte fixierbar ist und das mit ihm
verbundene mechanische Koppelelement durch eine Bohrung in der Fußplatte in die
Skala tympani führbar ist. Beim Zugang zum Innenohr durch die Steigbügelfußplatte
kann unmittelbar auf eine weit verbreitete Operationsmethode (Stapedektomie) zu
rückgegriffen werden, bei der eine Steigbügelprothese als dünner Stift durch eine Boh
rung in der Fußplatte geringfügig in das Innenohr geschoben wird. Dieses Verfahren ist
klinisch sehr sicher und liefert gute, reproduzierbare Ergebnisse. Der Stift ist dabei er
findungsgemäß mit dem schwingenden Teil des elektromechanischen Wandlers verbun
den.
Da die Länge des mechanischen Koppelelementes ohne größeren Einfluß auf die aku
stischen Übertragungseigenschaften des Systems ist, kann der elektromechanische
Wandler, der das Koppelelement antreibt, auch weiter entfernt von der Zugangsstelle
zum Innenohr positioniert sein. Beispielsweise kann der Wandler so gestaltet sein, daß
er an der Stelle des Trommelfells nach dessen Entfernung im Gehörgang fixierbar ist
und das mit ihm verbundene mechanische Koppelelement durch eine Bohrung in der
Promontorialwand oder in der Steigbügelfußplatte in die Skala tympani führbar ist.
Diese Lösung hat gegenüber der direkten Positionierung des Wandlergehäuses z. B. auf
dem Promontorium den Vorteil, daß eine größere Membranfläche des antreibenden
Wandlers möglich ist und damit höhere Auslenkungen des mechanischen Koppelele
ments bei gegebener Wandlerspannung erreicht werden.
Das mechanische Koppelelement kann vorteilhafterweise als entlang seiner Längsachse
verformbarer und in Längsrichtung steifer Stößel ausgebildet sein, wodurch eine Ver
formung des Koppelelementes dergestalt sichergestellt ist, daß das Stößelende
senkrecht durch die Bohrung in der Promontorialwand oder der Steigbügelfußplatte
führbar ist. Zweckmäßigerweise hat der Stößel einen kreisförmigen Querschnitt mit
einem Durchmesser von 0,4 mm bis 1,0 mm, vorzugsweise von 0,6 mm, und er besteht
aus einem biokompatiblen Material wie Keramik und/oder Polytetrafluorethylen. Zur
präzisen Einstellung der Eindringtiefe des mechanischen Koppelelements in das
Innenohr können der elektromechanische Wandler und/oder das mechanische
Koppelelement eine entsprechende Vorrichtung aufweisen.
Der elektromechanische Wandler kann auf dem elektrodynamischen, elektromagneti
schen oder vorzugsweise piezoelektrischen Prinzip beruhen und im letzteren Fall eine
vorzugsweise kreisförmige unimorphe oder bimorphe Piezokeramikscheibe aufweisen.
Eingangsseitig kann der Wandler über eine Zuleitung mit einer
Signalverarbeitungselektronik und einer Energiequelle verbunden sein.
Bevorzugte Ausführungsbeispiele der Erfindung sind nachstehend unter Bezugnahme
auf die Zeichnungen näher erläutert. Es zeigen:
Fig. 1 einen schematischen Schnitt durch die Innenbereiche eines menschli
chen Ohres mit auf dem Promontorium implantiertem elektromecha
nischem Wandler eines Hörgerätes,
Fig. 2 eine der Fig. 1 entsprechende Darstellung mit einem auf der Steigbü
gelfußplatte implantierten elektromechanischen Wandler,
Fig. 3 eine den Fig. 1 und 2 entsprechende Darstellung mit einem im Ge
hörgang implantierten elektromechanischen Wandler,
Fig. 4 einen Querschnitt durch einen piezoelektrischen Wandler an der Im
plantationsstelle nach Fig. 1.
Der in Fig. 1 dargestellte, in ein menschliches Ohr 10 implantierte elektromechanische
Wandler 11 ist in der durch das Trommelfell 12 vom äußeren Gehörgang 13 abgetrenn
ten Paukenhöhle 14 auf dem Promontorium 15 implantiert. Ein mit dem elektromecha
nischen Wandler 11 fest verbundenes, aus Fig. 4 ersichtliches mechanisches Koppelele
ment 16 ragt in den Innenohrraum 17 (Skala tympani), der mit Perilymphe angefüllt ist.
Im Wandler 11 werden elektrische Signale, die ihm über eine Zuleitung 18 zugeführt
werden, in mechanische Schwingungen umgewandelt, die durch das als Stößel ausgebil
dete mechanische Koppelelement 16 gemäß Fig. 4 durch eine Bohrung 19 in der Pro
montorialwand 20, die durch menschliches Bindegewebe 21 gegen den Stößel 16 abge
dichtet ist, auf die Perilymphe in der Skala typani übertragen. Die Lymphflüssigkeit
wiederum überträgt die Schwingungen auf die aus Fig. 1 ersichtliche Schnecke 22, wo
ein den Schwingungen entsprechender Höreindruck entsteht.
Im in Fig. 2 dargestellten menschlichen Ohr 10′ ist gegenüber dem Ohr 10 aus Fig. 1 der
Steigbügel 23 entfernt, und der elektromechanische Wandler 11′ ist auf der aus Fig. 1
ersichtlichen und in Fig. 2 durch den Wandler 11′ verdeckten Steigbügelfußplatte 24 im
plantiert. Das mechanische Koppelelement ist durch eine Bohrung in der Steigbügel
fußplatte 24 in den Innenohrraum 17′ geführt (in Fig. 2 nicht dargestellt).
Fig. 3 zeigt eine weitere Ausgestaltung 11′′ des elektromechanischen Wandlers. Gegen
über den Ausgestaltungen 11 und 11′ der Fig. 1 und 2 ist der Wandler 11′′ in Fig. 3 we
sentlich vergrößert. Er ist an Stelle des Trommelfells 12,12′ im äußeren Gehörgang 13′′
implantiert. Mit ihm verbunden ist ein längerer, als mechanisches Koppelelement zum
Innenohrraum 17′′ wirkender Stößel 16′, der durch eine Bohrung 25 in der Steigbügel
fußplatte 24′ in den Innenohrraum 17′′ geführt ist. Durch die vergrößerte Fläche des
Wandlers 11′′ im Vergleich mit den Wandlern 11 und 11′ der Fig. 1 und 2 ist die Über
tragung größerer Schwingungsamplituden auf das Innenohr 17′′ und damit verbunden
auch eine Verstärkung des Höreindrucks möglich.
Fig. 4 zeigt eine Realisierung des auf dem Promontorium 15 implantierten Wandlers 11
aus Fig. 1 nach dem piezoelektrischen Prinzip. Eine einlagige, kreisförmige Scheibe 26
aus Piezokeramik ist in ein ebenfalls kreisförmiges, einseitig offenes Gehäuse 27 fest
eingespannt. Die elektrische Kontaktierung erfolgt im Gehäuseinneren über eine her
metisch dichte Gehäusedurchführung 28, wobei ein dünner und flexibler Draht 29 von
der Durchführung 28 auf eine in der Darstellung oben liegende Elektrodenfläche 30 der
Piezokeramikscheibe 26 geführt ist. Die Kontaktierung der anderen, hier unten liegen
den Elektrodenfläche 31 erfolgt unmittelbar über den elektrischen Kontakt dieser
Elektrodenfläche mit dem Gehäuse 27, das beispielsweise aus Titan oder Niob gefertigt
sein kann. Sowohl zu der Durchführung 28 als auch zum Gehäuse 27 sind elektrische
Anschlüsse 32 und 33 geführt. Wird die piezokeramische Scheibe 26 über diese An
schlüsse 32, 33 elektrisch angeregt, ergibt sich eine mechanische Verformung der
Scheibe 26, die im wesentlichen in einer zur Scheibenebene senkrechten Bewegung des
Scheibenmittelpunktes und damit des mittig fest angekoppelten Stößels 16 resultiert.
Dadurch entsteht eine Bewegungsform des Stößels innerhalb der Skala tympani 17, die
zu Druckschwankungen in der Perilymphe und damit zu einem Höreindruck führt.
Der Stößel 16 ist vorzugsweise aus Polytetrafluorethylen oder biokompatibier Keramik
gefertigt. Die dem Promontorium zugewandte Elektrode 31 ist aus Gründen der Bio
kompatibilität mit einer geeigneten, dünnen Schutzschicht überzogen. Die Einstellung
der Eindringtiefe des Stößels 16 kann auf einfache Weise durch einen Ring 34 erfolgen,
der über ein Feingewinde 35 mit der offenen Seite des Wandlergehäuses 27 verbunden
ist. Durch Drehen dieses Ringes kann so ein vorher über eine Meßlehre ermitteltes Tie
fenmaß sicher eingestellt werden, das für die risikolose Applikation des Wandlers und
auch für die richtige physiologische Funktion des aktiven Wandlers wesentlich ist. Das
Gehäuse 27 wird nach der Positionierung auf dem Promontorium 15 vorteilhaft mit
Knochenzement 36 dauerhaft fixiert.
Die akustische und elektronische Verarbeitung des Audiosignals, die Energieversor
gung, die Steuerung des Systems und die hermetische Kapselung zur Implantation des
gesamten ansteuernden Systems erfolgen vorzugsweise in der aus DE 39 18 086 C1 be
kannten Weise.
Claims (10)
1. Implantierbares Hörgerät zur Anregung des Innenohres, mit einem
elektromechanischen Wandler und einem Koppelelement, das mit seiner vom Innenohr
abgewandten Seite mit dem schwingenden Teil des Wandlers verbunden ist und dessen
anderes Ende durch eine artifizielle Bohrung unter Umgehung der
Gehörknöchelchenkette in die flüssigkeitsgefüllten Innenohrräume ragt, dadurch
gekennzeichnet, daß das Koppelelement als steifes mechanisches Koppelelement (16,
16′) zur direkten Übertragung der mechanischen Schwingungen des Wandlers (11, 11′,
11′′) an das Innenohr ausgebildet ist.
2. Hörgerät nach Anspruch 1, dadurch gekennzeichnet, daß der elektromechanische
Wandler (11) so ausgebildet ist, daß er auf dem Promontorium (15) fixierbar ist und
das mit ihm verbundene mechanische Koppelelement (16) durch eine Bohrung (19)
in der Promontorialwand (20) in die Skala tympani (17) führbar ist.
3. Hörgerät nach Anspruch 1, dadurch gekennzeichnet, daß der elektromechanische
Wandler (11′) so ausgebildet ist, daß er nach Entfernung des Steigbügels (23) auf
der Steigbügelfußplatte (24) fixierbar ist und das mit ihm verbundene mechanische
Koppelelement durch eine Bohrung (25) in der Fußplatte in die Skala tympani (17′)
führbar ist.
4. Hörgerät nach Anspruch 1, dadurch gekennzeichnet, daß der elektromechanische
Wandler (11′′) so ausgebildet ist, daß er an der Stelle des Trommelfells (12, 12′)
nach dessen Entfernung im Gehörgang (13′′) fixierbar ist und das mit ihm verbun
dene mechanische Koppelelement (16′) durch eine Bohrung in der Promontorial
wand (20) oder in der Steigbügelfußplatte (24′) in die Skala tympani (17′′) führbar
ist.
5. Hörgerät nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß
das mechanische Koppelelement (16, 16′) als entlang seiner Längsachse verformba
rer und in Längsrichtung steifer Stößel ausgebildet ist.
6. Hörgerät nach Anspruch 5, dadurch gekennzeichnet, daß der Stößel einen kreis
förmigen Querschnitt mit einem Durchmesser von 0,4 mm bis 1,0 mm, vorzugsweise
von 0,6 mm, aufweist.
7. Hörgerät nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß
der elektromechanische Wandler (11, 11′, 11′′) und/oder das mechanische Koppel
element (16, 16′) eine Vorrichtung (34) zur präzisen Einstellung der Eindringtiefe
des mechanischen Koppelelements (16, 16′) in das Innenohr aufweisen.
8. Hörgerät nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß
der elektromechanische Wandler (11, 11′, 11′′) auf dem elektrodynamischen,
elektromagnetischen oder, vorzugsweise, dem piezoelektrischen Prinzip beruht.
9. Hörgerät nach der Anspruch 8, dadurch gekennzeichnet, daß der auf dem piezo
elektrischen Prinzip beruhende elektromechanische Wandler (11, 11′, 11′′) eine
vorzugsweise kreisförmige, unimorphe oder bimorphe Piezokeramikscheibe (26)
aufweist.
10. Hörgerät nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß
der elektromechanische Wandler (11, 11′, 11′′) über eine Zuleitung (18, 18′) mit ei
ner Signalverarbeitungselektronik und einer Energiequelle verbunden ist.
