Abstract

Electrical current pulses are introduced into the patient by applied electrodes. Near the heart, at least three electrodes are distributed over the front and back of the patient. An electrical potential is applied to at least two of these electrodes so that at least one electrical field is generated and maintained for a short period. All the electrodes are connected to a central control. The latter can switch on various electrode pairs or groups. A vector loop corresp. to the natural physiological sequence can thus be formed by change over of switching. The heart areas stimulated by the electric field can thus be sequentially depolarised. ADVANTAGE - Reduced electrical loading of patient and hence lower risk.

Description

The invention relates to a method for cardioversion and defibrillation of ventricular or atrial fibrillation using electrical currents passing through to the patient's body applied electrodes are introduced.

The termination of arrhythmias, which is usually are tachycardic, by means of high-energy surges as Cardioversion, defibrillation or electrical version. Some of these terms are used synonymously, some are also differentiated conceptually between them, namely in the Way that with synchronized delivery of the electric shock, such as e.g. B. in the treatment of atrial fibrillation, the term cardio version and when the electric shock is not synchronized, such as B. ventricular fibrillation, the term defibrillation ver is applied. Defibrillation is with this choice of terms always an emergency measure, whereas a cardioversion usually represents a therapeutic treatment.

The cardioversion and defibril procedures used today lations are mostly based on the use of short transtho rakale DC pulses since it has been found that at  the administration of the longer lasting AC pulses can easily induce ventricular fibrillation. The usual today Cardioverters / defibrillators basically consist of one individual or a series of capacitors immediately before using a high voltage transformer with DC judge from the normal power grid within a time of two charged up to ten seconds.

The stored energy, up to four hundred watt seconds can be in a time of one to four milliseconds submitted. This energy is delivered via two plates electrodes that are eight to twelve centimeters in diameter in adults and from four to five centimeters in children to have. The electrodes are provided with plenty of contact gel and firmly placed on the patient's chest wall. At a Cardioversion the discharge is triggered by R-waves, syn chronicles twenty to forty milliseconds after the peak the R-wave.

The current impulse leads to a simultaneous depola All myocardium that is not refractory at this time areas, d. H. all areas of the heart muscle, their sensitivity to irritation not by contracting immediately for was lifted for a short time. This will re-excite the Reentry mechanism impossible. Then in the following Diastole (heart enlargement) receives the physiological step maker of the heart, the sinus node, opportunity, the excitement to take over the formation of the heart again.

The disadvantages of the previously known methods are that the very high burden on the patient's organism, the  with the application of the high amount of energy in a very short time goes hand in hand with voltages of many thousands of volts and currents of several amperes results in a high risk potential tial and risk of injury for the patient. There The inventor has set himself the task of creating a method develop that the patient's physical stress on a Minimum size reduced.

This task is based on the known state of the art nik, solved in an inventive way in that in the heart distance between the patient's chest or back distributed at least three electrodes are applied and to we at least two of each of these electrodes are electrical Voltage applied and thereby at least one electric field is generated and maintained for a short time, which by means of a central control device to which all electrodes are attached are closed by appropriate switching operations between the Electrode pairs or groups migrate, causing one in the natural physiological order trending vector loop forms and thereby the through the electrical Field affected cardiac areas successively depolarized will.

The stresses are in the process according to the invention so applied to the electrodes that the resulting elec trical field strength in the direction of the vector loop has its greatest value. A central tax serves this purpose unit to which all electrodes are connected and which voltages applied to the electrodes by appropriate Switching processes between the electrodes are generated and switched. This makes it possible for the electric field to be a vector loop generated in the natural physiological ranks  is followed and along the individual heart areas are successively depolarized.

The time in which one round of the vector loop is completed can be chosen freely. This time can be long less than the pulse duration of be a few milliseconds and the natural time of a heart excitation, that is between 0.1 and 1 second lie gene.

An embodiment of the method can consist in that in the chest and back areas of the Pa at least three electrodes are applied and the voltages on the elec tread in a synchronous manner for the chest and back areas be controlled. There are electrodes on the chest and back side of the patient, so is a finer control of the direction the flow of current in the patient, especially in the depth of the Body, possible. Is the procedure designed so that the Tension on the electrodes in the chest and back areas in controlled in such a way that the elec fields for the chest and back areas are different are strong, this control is particularly precise possible.

An advantageous embodiment of the method according to the invention can consist of several migratory fields at the same time generated or the field strength in its height by the Än change in the applied voltage is alternately changeable or the electric field strength over time can be changed by changing the applied voltage. If more than one electric field is created, or that  electric field or the electric fields become their height gradually and / or temporally by changing the adjacent span changes, so is the optimization of the vector loop run possible in an advantageous manner. For this creates a central control unit to which all electrodes are connected the voltages applied to the electrodes are independent from each other both in their amount and in their temporal course in the manner according to the invention that the maximum field strength between the electrodes in the direction of the straight to depolarisie end myocardial areas and depolarizes this area, whereby the vector loop in its natural physiological order is followed and all myocardial areas follow and after depolarize.

It is also in the application of the method according to the invention possible that the voltages applied to the electrodes by Signals are triggered or synchronized by the Her zen of the patient to be treated. Thereby can be a cardiac arrhythmia in a particularly gentle manner acts or be remedied by the natural arousal the heart is supported by the procedure. It is in particular, it is also possible to adjust the intensity of the patient gradually increasing currents acting on them limit required measure.

After going through the heart one or more times Vector loop is defibrillated, the method can now serve that there is serious damage to the heart excitation system of the patient the function of an external Pacemaker performs. In the event of serious damage of the cardiac excitation conduction system, which is an autonomous beating would prevent if this procedure were used  as an external pacemaker even a normal contraction of the Atria possible.

The advantages of this method compared to the previously known Procedures are in the significantly reduced and therefore scho lower and safer current that is on the body of the Patient and which results from the fact that the tension the heart areas be depolarized and the passage of the vector loop takes longer than with previous methods the voltage pulse was applied. In case of application as Pacemakers would also be a normal con traction of the atria possible.
