Description
The invention relates to an endoprosthesis according to the preamble of claim 1.
It is known to rough the surface of an endoprosthesis stalten to connect with that in the roughness to improve growing bone tissue. Such roughness can be caused by the outward ridges of surveys of the Be formed prosthesis surface (DE 28 39 093 B2, US-PS 3 869 731). It is also known (GB 20 59 267 A) to which Denture surface a comparatively thin, perforation to apply gene containing material in a network form, which the Roughness forms and in which the ingrowing bone tissue can anchor itself.
The invention has for its object a prosthesis to create an element forming the surface roughness, which is easy to manufacture on the one hand and special on the other which offers favorable anchoring requirements.
This task is characterized by the characteristics of the Claim 1 solved.
Due to the roughened surfaces according to the invention structure caused by mechanical deformation of sheet metal (a other thin-walled structures) is generated, there is an intensive bone-prosthesis contact that the Waxing favors. In particular, a good rotation si security guaranteed. The strength of the connection leaves through bone screws, for those with the inventions areas provided with burrs according to the invention preferably have openings are intended to increase.
The burrs result from the local stamping deformation of the thin-walled prosthetic material to the crack, the ridges form the boundary in the crack area. That makes it easy Wise surface texture would be through corresponding processing with other measures only very much difficult to achieve. When the burrs are created using a punch punch there are burr areas with an annular cross-section, the against loads in the most varied Directions have favorable holding properties. Which is so resulting "craters" can also be more advantageous Fill in with body-absorbable material that an additional growth favors. If the craters are in increasing distance from the surface of the prosthesis in her Taper inside towards the opening, form mushroom-shaped anchors due to the ingrown bone tissue body's material, which is an exceptionally good connection Ensure extension between the bone and prosthesis surface.
With a corresponding design of the femoral part and / or the tibia part of a tibial prosthesis in the area of Contact area with the bone becomes an easy fixation achieved with excellent turning security.
According to an advantageous development of the invention burr-like roughening to be connected to the prosthesis Sheets or other plate-shaped connecting elements or on thin-walled areas of the prosthesis applied, which in particular to the femur or tibia back surfaces of the femur and tibia parts, which are perpendicular to the longitudinal axis form a prosthetic knee joint. These back surfaces act as contact surfaces between the femur or tibia part and the corresponding stump on the knee. To achieve full-surface contact and thus good anchoring si safety, only minimal bone resection is required. On the knee-side stumps are only flat, for Grind the longitudinal axis of the vertical contact surfaces.
For secure fixation of the femur part and the tibia part the resected bone stumps is in the invention "Inus nutmeg-like" interface design essentially only one hole for the passage of a knot chenschraub required, the outlet opening for the Lower the bone screw to the longitudinal axis of the femur or tibia right rear surface is provided.
Are several resected at an angle to the bone stump The artificial knee joint is ground on flat surfaces part shaped to match. It can be useful the knee joint part from a basic body and several plates assemble shaped connecting elements.
Tibia komponente verbunden. The plate-shaped connecting elements are preferred by welding to the back surface of the femur or tibia component connected.
Es zeigen:Advantageous developments of the invention result from the subclaims and are described below along with the description of a preferred embodiment of the Invention using the example of a knee joint prosthesis using the Figures shown in more detail. The knee joint endoprosthesis shown in FIG. 1 consists of the femoral part 1 and the tibia part 2 . Both parts are provided on their back surfaces facing the bone with plate-shaped connecting elements 3 and 4 . The connecting elements 3 have the task of connecting the femur part 1 of the artificial knee joint to the femur bone 5 and the connecting elements 4 have the task of connecting the tibia part 2 to the tibia bone 6 . The femur bone 5 is provided with a plurality of planar grindings angled relative to one another, as a result of which a plurality of connecting elements 3 or a correspondingly curved plate-shaped connecting element is also required. The connecting elements 3 and 4 are firmly connected to the femur or tibia component, preferably welded, and form the femoral part ( 1 ) to be made available to the surgeon and the tibia part ( 2 ) of the knee joint endoprosthesis. The back surfaces perpendicular to the femur and tibial longitudinal axes are provided according to the invention in the area of contact with the bone 5 and 6 with ridge-like roughening 7 in the manner of a nutmeg grater. These roughenings are preferably applied to plate-like connecting elements 3 and 4 provided for this purpose. These one-sided roughening 7 of the connecting elements 3 and 4 can advantageously be produced by punching holes 8 (FIGS . 2, 2a and 2b), the resulting sharp-edged ridges forming the roughening 7 . The two parts 1 and 2 of the knee joint endoprosthesis can be pressed into the bone substance with the roughened contact surfaces, thus ensuring extensive pre-fixation. Complicated-shaped, often barb-like anchoring pins, which make reoperation difficult and also have to be cemented into special bone cavities to be created for this purpose, are thus eliminated. The final fixation takes place by means of a few bone screws 9 which can be screwed through the femur or tibia part 1 or 2 including the connecting elements 3 or 4 into the femur or tibia bone 5 or 6 . The screw heads 10 are sunk vorzuse hen, since the bone screws 9 are screwed in from the sliding surfaces 11 and 12 of the joint. The pivot point 13 of the tibia part 2 and thus the tibia 6 is in the embodiment in the center of a ball 14 in the blunt femur 5 . This ball 14 is connected via a mandrel 15 to a shaft 16 of the tibia part 2 , so that a pivoting movement of the tibia 6 about the pivot point 13 is possible within a slot-like milling of the femur 5 and the femoral part 1 of the artificial knee joint.
Fig. 2 shows a section through the plate-shaped connec tion elements 4 of the tibia parts 2nd The bores 17 serve to pass the bone screws 9 through.
A second embodiment of a plate-shaped connecting element 4 is shown in Fig. 2a. This second variant differs from the first variant shown in FIG. 2, above all by the one-piece design, the shape and the arrangement of the bores 17 for the bone screws 9 .
An edge zone of a plate-shaped connecting element 4 according to FIG. 2 or 2a with a punched hole is shown in an enlarged representation in FIG. 2b. The sheet thickness of the plate-shaped connecting element 4 is approximately 0.2 mm to 0.5 mm, while the height of the burr-like roughenings 7 formed by the punched holes is approximately 0.7 mm to 1.6 mm and the hole diameter is approximately 0.6 mm. The dashed hatched area 8 is filled with absorbable material, so that the ingrowth of bone is favored. It can be seen that in the case of ingrowth of bone tissue in the area previously filled with resorbable material, mushroom-shaped anchors are formed from bones, which show an exceptionally good holding effect.
Tibiaknochen 5 bzw. 6 zu verbindende Femur- bzw. Tibiateil 1 bzw. 2 aufgeschweißt.In Fig. 3, the basic structure of a punching tool for generating the surface structure of the plate-shaped connecting elements 3 and 4 is shown in a perspective view. After punching out and / or sawing and / or drilling and / or milling the raw part, holes 8 , the ridges of which form the roughening 7, are punched out. A suitable punching tool essentially consists of a lattice-like support 19 for the blank 20 and a punch 21 which can be moved in the direction of the arrow and which has a plurality of pointed mandrels 22 , the diameter of the mandrels 22 corresponding to the hole diameter to be produced - therefore preferably 0.6 mm. corresponds. The mandrels 22 penetrate the blank 20 and the spaces between the support grid 19 in the direction of the arrow when force is applied. The grid-side edges of the punch holes 8 have sharp burrs after the punching process. The plate-shaped connecting element 3 or 4 produced in this way is welded onto the femur or tibia part 1 or 2 to be connected to the femur or tibia bone 5 or 6 .
