Abstract

The invention relates to a breathing mask for long-term positive pressure respiration of patients with pulmonary insufficiency. The breathing mask surrounds mouth and nose. In long-term respiration (about 8 hours a day) sealing of the breathing mask against the face presents problems: the mask must only rest against the face very lightly in order to avoid painful pressure sores; on the other hand, however, it may only have low losses due to leakage. This sealing problem is solved by an inflatable annular bead which is formed such that the particularly sensitive areas of the face, that is bridge of the nose and eyes, are left free. Sealing and tolerability of the breathing mask can be improved by controlling the internal pressure in the sealing bead, so that the air pressure in the bead is slightly higher on inspiration than the air pressure in the mask, while it is lower on expiration, by a special cross-sectional shape of the bead with sealing lips and by application of an intermediate layer of a skin-compatible material.

Description

The invention relates to a tight-fitting Facial mask used for long-term positive pressure ventilation People with lung insufficiency is used.

The respiratory mask ( 1 ) encloses the mouth and nose. The breathing air is supplied to the mask from a compressed air generator ( 2 ) ("respirator") through hose lines and a three-way switch valve ( 3 ) ("respirator valve"). As a result, an excess pressure is built up in the interior of the mask, which is sufficiently high to fill the lungs with breathing air without the user having to do anything.

When exhaling, the used air is blown out through the ventilation valve ( Fig. 1, Fig. 2).

With long-term ventilation, the mask increases daily worn as 8 hours.

In this operation, the ventilation is sealed problematic face mask:

  • - einerseits darf die Maske nur sehr leicht anliegen, um Druckstellen und Beschädigungen der Epidermis zu vermeiden.- on the one hand, the mask may only fit very lightly, around pressure points and damage to the epidermis avoid.
On the other hand, it may only have a small leakage have cracks.

To date, comparable ventilation masks are only for Short-term ventilation is known, e.g. B. for emergency ventilation of Accident victims.

Jan. 86).These ventilation masks are sealed off from the face e.g. T. by an inflatable, closed ring bead, the amount of air in the bead during ventilation remains constant (eg "pocket mask" from Lärdal Medical D 8031 Graefelfing, parts catalog, edition Jan. 86).

Lärdal Medical, w. o.).In some cases, sealing is also carried out using sealing lips, that press against the face under the internal pressure (For example "Lärdal Medical" silicone mask, see above).

Diese Beatmungsmasken sind zur Langzeit-Beatmung nicht geeignet:These ventilation masks are for long-term ventilation not suitable:

  • - entweder verläuft das Dichtungselement quer über den Nasenrücken und führt dort wegen der lokal erhöhten Druckbelastung, insbesondere bei Patienten mit einem "hohen" Nasenrücken, zu Hautreizungen mit schmerzhaften Druckstellen;- either the sealing element runs transversely over the bridge of the nose and leads there because of the locally increased pressure load, especially at Patients with a "high" bridge of the nose, too Skin irritation with painful pressure points;
  • - oder die Beatmungsmaske ist im Nasen-Augen- Bereich so bereit, daß der Augapfel belastet und/oder das Gesichtsfeld eingeschränkt wird.- or the respiratory mask is in the nose and eye - Area so ready that the eyeball is stressed and / or the visual field is restricted.

In addition, there are seals on this ventilation mask elements made of air and water impermeable material and lie directly on the skin of the face.

As a result, the skin moisture cannot in this area evaporate. Because wet skin is much more sensitive against permanent stresses, the risk of Damage to the epidermis increased.

The invention has set itself the task of ventilation to create a mask that has sufficient tightness and no physical activity even with continued continuous operation Leads to impairments of the user.

The sealing problem described is solved according to the invention by an inflatable annular bead ( 4 ) made of airtight, elastic material with a shape according to claim 1. The bridge of the nose and the eyeballs are kept free ( Fig. 3, Fig. 4, Fig. 5).

The seal on the nose is only in the upper half of the nose sidewalls, including on the cheek next to the Nostrils.

A concave curvature of the sealing bead, with a Radius of curvature less than 1 cm, as is otherwise the case when the sealing bead runs over the bridge of the nose and there leads to high local pressures avoided this kind.

In addition, a sufficient field of vision for the User guaranteed.

By requesting a sealing lip ( 8 ) according to claim 2, the additional sealing effect of a self-applying sealing lip under the internal pressure can serve to improve the seal ( FIG. 6).

The cross-sectional profile of the annular bead can be shaped according to claim 3 ( Fig. 7).

Die Kante der Dichtkippe bewegt sich dabei um den Be trag "H" (Fig. 8).If the ring bead is now filled with air, the ring bead will increasingly take on a circular cross section. The sealing lip is turned further and further away from the mask body in the direction of the face surface. The edge of the sealing tip moves around the loading "H" ( Fig. 8).

This effect favors a good adaptation of the Ventilation mask also on very different faces to form.

By controlling the internal pressure in the toroid speaking claim 4, the pressure load that the Ring bead on the face, on the necessary Minimum be reduced. In addition there is when exhaling a strong reduction of the contact pressure, what a recovery process in the affected skin can favor lots, e.g. B. by a better one Blood flow during the relief phase.

The control of the internal pressure in the ring bead can be done by an arrangement according to claim 5 ( Fig. 10). This arrangement has the advantage that the amount of pressure difference between the pressure in the bead and the pressure in the mask is adjustable to a certain extent. A disadvantage is that the additional throttle point ( 10 ) in the supply air line requires a higher performance from the breathing apparatus.

In an arrangement according to claim 6, the ventilator is not additionally claimed ( Fig. 11). This is particularly important for battery operation. There is no setting option for this.

By a skin-friendly liner (11) according to claim 7, the formation of a moisture film between facial skin and breathing mask is avoided (FIG. 5 and FIG. 12).

The job of the liner is to remove the one Absorb skin moisture and by capillary action to be transported to the outside or inside of the mask, where it can evaporate.

This will dry the skin in the affected area held, which increases their resilience. In addition, such an intermediate layer is inexpensive producible and easy to attach, e.g. B. by Buttons. So it can be replaced frequently, which is especially hygienic for continuous operation Reasons is recommended.

In a design of the sealing element accordingly claim 8, the ring bead and possibly its pressure control can be omitted, which simplifies the respiratory mask ( Fig. 9).

The membrane ( 13 ) can be fastened by means of a wire tension ring ( 17 ). This requires that a groove is provided on the outer edge of the mask, into which the wire tension ring is inserted, and that in addition the outer contour of the mask is convex in each area, so that the wire tension ring rests at every point.

Durch Anbringung des Beatmungsventils (3) direkt an der Beatmungsmaske entsprechend Anspruch 9 (Fig. 12) werden 2 Vorteile erzielt:By attaching the ventilation valve ( 3 ) directly to the ventilation mask according to claim 9 ( Fig. 12), two advantages are achieved:

1. By the elimination of a connecting piece between ventilation valve and respiratory mask, the is longer than 2 cm, the dead volume in the mask decreased. (Under "dead volume" is the volume to understand the amount of used air that remains in the mask after exhaling.) This improves the ventilation quality of the mask improved.
When executing the respiratory mask speaking claim 7, the flow resistance the ventilation valve for the printer be raised in the ring bead without that an additional hose line is required is.

By fixing the breathing mask on the user's teeth with the help of a fitting ( 15 ) according to claim 10 ( Fig. 13), slipping of the respiratory mask, z. B. prevented by head movements and ensures a constant fit of the mask. This improves the sealing of the ventilation mask.
