Description

The invention relates to a medical treatment device an ultrasound resection unit for resection of tissue in a living organism, the ultrasound resection one ultrasonic vibration generator and one with the probe connected to the ultrasonic vibration generator by the ultrasonic vibration generator has generated vibrations and the probe with a distal End section for transmitting the vibrations to the right dissecting tissue is provided.

Conventional procedures for resection of human tissue see a cauterization of the tissue with a high frequency current. In this treatment, it is cauterized Tissue, however, degenerates into whitish tissue, making it it is difficult to delimit the resection area. Therefore be there is a risk in the conventional method that normal tissue is resected. If the high frequency electrode accidentally in contact with a patient's muscle tissue is brought, the muscle tissue reacts and is pierced, causing heavy bleeding.

To the after-effects of this conventional treatment Removing parts has been a treatment for some time  used for resection, which with ultrasound Vibrations resected. Such a treatment device is by Japanese Patent Application Laid-Open 61-1 59 953 (Japanese Patent Application No. 60-1201). This treatment device has a probe Vibration transmission element, which as a tube or the like. out forms and is introduced into a living organism and an ultrasonic vibrator that works with the proximal end of the probe through a conical, the ultra sound vibration amplifying horn is connected. In the A through hole is provided with a Ab suction unit is connected.

To resect the affected tissue, the distal End of the probe into a body cavity through for example a channel of one already inserted into the body cavity Endoscope inserted and pressed against the tissue. The distal end of the probe is then vibrated and the tissue is resected. Through the through hole in the resected tissue is opened from an opening at the probe the end face of the probe to a suction deducted direction.

With this treatment, the tissue does not turn whitish Tissue degenerates so that the tissue to be resected can be easily recognized. If the operator accidentally injured blood vessels during treatment, bleeding may occur. Bleeding can also be caused by the suction force arises when the resected tissue is removed is pulled.

The invention has for its object a medical To design treatment device of the type mentioned in the introduction that one at resection of the tissue in a living Organism bleeding can be stopped quickly.  This object is achieved with a medical Treatment device of the aforementioned mentioned solved that a hemostasis device to stop the bleeding, if the tissue is resected, which has a hemostatic part in the area of the distal end of the probe is arranged.

According to the invention, the medical resection device assigns a hemostatic device immediately to be resected Bleeding area of the tissue of the living organism stop immediately. The hemostatic part of the hemostasis direction is near the distal end of the Ultra's probe sound resection tool arranged. Therefore, one can bleeding from the resection quickly through the blood breastfeeding device can be stopped.

The handle part 2 has a cylindrical outer housing 5 and a front conical jacket part 7 . An ultrasonic vibrator 4 is accommodated in the housing 5 . The conical man part surrounds a conical horn 6 , which is arranged at the front end of the vibrator 4 . At the front opening, the housing 5 has on its outer lateral surface an external threaded part 5 A , onto which an internal threaded part 7 A is screwed onto the inner surface in the area of the rear opening of the lateral part 7 . The housing 5 is thus integrally connected to the front jacket part 7 . The vibrator accommodated in the interior of the housing is a Langevin vibrator which has a piezoelectric element 10 , an electrode plate 11 , a front metal block 12 and a rear metal block 13 . A through hole (not shown) extends through all of these parts. A cylindrical bolt 14 protrudes through the through hole. A front end portion of the bolt 14 is screwed to the rear end portion of the horn 6 in front of the metal block 12 . At the rear end of the bolt 14 , a nut 15 is screwed on the rear end of the metal block 13 and so horn 6 , block 12 piezoelectric element 10 , electrode plate 11 and metal block 13 holds together. In the inner Oberflä surface in the central region of the housing 5 there is an annular groove 5 B. An O-ring 17 is inserted between this groove 5 B and the outer surface of the rear metal block 13 . Another O-ring 19 is inserted between a flange section 18 of the metal block 12 and an inner projection 7 B of the jacket part 7 . These O-rings 17, 19 fix the ultrasonic vibrator 4 in the housing 5 .

The ultrasonic vibrator 4 has a pair of electrodes 11 . A lead wire 20 is connected to one electrode and a lead wire 21 to the other electrode. This Lei line wires 20 , 21 are led out as a cable 22 from the housing 5 . The cable 22 is connected to a control voltage generator 24 .

The end portion 6 A with the small diameter of the horn 6 accommodated in the conical shell part 7 protrudes forwardly from the front end surface of the shell part 7 . On the protruding end section 6 A an external thread de 6 B is provided. A connecting hole 6 C is provided in the central axis of the horn 6 . A proximal portion 26 A of the probe or the metal tube 26 of the insertion part 8 is fixed in the connection hole 6 C. A flange section 26 B is held in firm contact with the end face of the end section 6 A by a union nut 28 which is screwed onto the threaded part 6 B of the horn. By loosening the union nut 28 , the probe can be removed from the horn 6 and replaced by another probe (tube).

In the inner wall of the distal end portion (smaller diameter) of the jacket part 7 , an annular groove 7 C is seen before. An O-ring 32 is inserted between the groove 7 C and the outer surface of the end section 6 A with a small diameter of the horn 6 , which thus ensures the waterproof construction of the jacket part 7 . In the tube 26 , a suction hole 29 is provided to withdraw the resected tissue. The suction hole 29 is connected to an inner hole 34 A of a connecting pipe 34 which protrudes outward from the rear end part 3 of the housing 5 . On the connecting tube 34 , a suction hose 35 is pushed. The suction hose 35 is connected to a suction device 30 , which has a (not shown) suction tank and a (not shown) suction pump. In a through hole 5 C of the housing through which the connecting tube 34 is passed out, an annular groove 5 D is provided. Between the groove 5 D and the outer surface of the connec tion tube 34 , an O-ring 36 is inserted.

Der Oszillator 40 liefert einen Hochfrequenzstrom an die Elektrode 38.As shown in FIGS. 1 and 2, a bipolar electrode 38 is provided on the distal end surface of the insertion part 8 as a hemostatic part 37 A of a blust device 37 . A lead wire 39 of the electrode 38 ver runs along the inner wall of the tube 26 of the jacket part 7 and the housing 5 and is connected to a high frequency oscillator. The oscillator 40 supplies a high frequency current to the electrode 38 .

If a specific human tissue (not shown) is to be resected with the aid of the ultrasound treatment device in the arrangement described above, the insertion part 8 is inserted into the body cavity through a channel of an endoscope. The distal end of a guide member 8 is pressed against the tissue in the region in question. From the ultrasonic vibrator 8 ultrasonic waves are guided onto the insertion part and at the same time the suction device is switched on. The affected tissue is resected by the ultrasound vibrations transmitted to the area to be resected.

The cut tissue is pulled through the suction hole 29 and transported out of the body through the suction hose 35 . The suction force of the suction device 30 causes ei ne bleeding in the vicinity of the resected tissue. A high-frequency current is conducted from the high-frequency oscillator 40 to the bipolar electrode 38 , which cauterizes the bleeding tissue and thereby stops the bleeding.

Accordingly, both resection and hemostasis can of the tissue in question using the simple, above be structure can be achieved.

Modifications of the hemostasis device at the distal end of the insertion part 8 will now be described with reference to FIGS. 3 to 5.

Fig. 3 shows a first modification in which a nichrome wire 42 is located in the distal end surface of tube 26. A current source 41 causes a current to flow through the wire 42 , which heats it up and can stop the bleeding of the tissue.

Fig. 4 zeigt eine zweite Modifikation. Fig. 4 shows a second modification. A thermistor with a positive characteristic, ie a PTC element 43, is arranged here at the distal end of the tube. If this PTC element 43 is heated, the bleeding can be stopped.

Fig. 5 shows a third modification in which a quartz fiber 44 is inserted into an existing in the wall of the tube 26 through hole. The distal end of the quartz fiber 44 protrudes from the distal end surface of the tube 26 . A laser unit 45 is connected to the proximal end portion of the fiber 44 . A laser beam emitted by the laser unit is emitted from the distal end section, ie the tip 44 A of the fiber 44 onto the bleeding part and thereby stops the bleeding.

A lead wire 39 connected to a heating element is embedded in the wall of the tube 26 of the insertion part 8 . However, the lead wire 39 can also run ver along the outer or inner wall surface of the tube 26 .

Below is another procedure for hemostasis resection with an ultrasound treatment device described tissue.

As shown in FIG. 6, a hemostasis probe 46 is provided independently of the ultrasound treatment part. A guide part of the ultrasound treatment device and an observation line 49 are inserted into a sleeve 48 of an endoscope 47 in order to resize the tissue of interest. When the tissue in question is completely removed, the insertion part of the ultrasound treatment device is removed from the endoscope 47 and instead the hemostasis probe 46 is inserted into the sleeve 48 in order to stop the bleeding. A PTC element 50 is provided as a heating element at the distal end of the probe 46 . The operator presses the distal end of the probe 46 against the bleeding tissue part while observing it with the observation line 49 and can thus stop the bleeding.

As shown in FIG. 7, a Peltier element 51 with a radiation surface 51 A for the heat can be provided at the distal end of the hemostasis probe 46 instead of a heating element 50 . This probe can also be used for hemostasis.

An ultrasonic treatment device according to a second embodiment of the present invention will now be described with reference to FIGS. 8 and 9.

The ultrasound treatment device according to this embodiment form has essentially the same structure as the first Embodiment. Same parts of the second version example are provided with the same reference numerals and on their detailed description is omitted.

At the rear end portion of the handle part 2 , this embodiment has a branch line 52 provided with a tap, which is at an angle to the axis of the insertion part 8 . The branch line 52 is connected to a suction channel 29 .

A single-pole antenna 53 is arranged as a heating device at the distal end of the probe 26 in order to emit microwaves ( FIG. 9). The antenna 53 is connected to a coaxial cable 54 which is introduced into the channel 29 and in the branch line 52 . The cable 54 protrudes from the branch line 52 and is connected to a microwave supply unit 55 via a plug (not shown). The microwave supply unit supplies a microwave current to the single-pole antenna 53 in order to emit microwaves to the affected tissue. The microwave supply unit 55 comprises an oscillator 56 with an oscillation tube (not shown) that can conduct a microwave current onto the cable 54 , a cooling device 57 for cooling the oscillation tube and a power supply unit 58 for electrical energy for the oscillator 56 and the cooling device 57 put. The microwave supply unit 55 and the single-pole antenna 53 form the microwave treatment device.

The single-pole antenna 53 is slidably mounted in the probe 26 of the ultrasound treatment device. The antenna 53 is pushed out of the distal end surface of the probe 26 as far as needed.

In order to resect the affected tissue in the living organism with the aid of the arrangements described above, the probe 26 is inserted into the living organism by, for example, guiding an endoscope channel into a body cavity. The single-pole antenna remains within the probe 26 . The distal end of the probe 26 is pressed against the affected tissue. Then the unipolar antenna 53 is moved out of the probe 26 and pressed against the tissue in question. Then the microwave supply unit is switched on. Then a micro wave current reaches the antenna 53 and destroys the living tissue to be resected with the microwave. There is no bleeding even if the destroyed tissue is resected. The ultrasonic vibrator 4 and the suction device 30 are switched on, so that ultrasonic waves are directed onto the distal end of the probe 8 . The destroyed tissue is now resected using the ultrasound waves. At the same time, the resected tissue is transported out through the suction channel 29 with the aid of the suction device.

In this procedure, the tissue is resected after it has been destroyed. Therefore, no resection occurs during resection Bleeding and the resected tissue can easily be removed be drawn. Therefore, the tissue in question can be safe and easily rebuild.

Modifications of this embodiment are shown in FIGS. 10 to 13.

In the modification shown in Fig. 10, bipolar microwave antennas 60 are slidably disposed in the end surfaces of the son de 26 . Cables (not shown) connected to the bipolar antennas are slidably housed in a pair of through holes in the wall of the probe 26 .

FIGS. 11 and 12 show a second modification. In this modification, bipolar antennas are slidably housed in the distal end surface of the cylindrical sleeve 48 of the endoscope. The probe 26 of the ultrasound resection unit is inserted together with the observation line 49 into the inner channel of the sleeve 48 . A pair of through holes are formed in the wall of the sleeve 48 in the longitudinal direction. The bipolar antennas 60 and cables 62 are each slidably inserted into a through hole. The antennas 60 are arranged in a position in which the operator can observe the antennas 60 with the observation line 49 .

When this treatment device is used, the sleeve 48 is inserted into the living organism and the operator finds the tissue in question by viewing the body cavity in the observation line 49 . The antennas 60 are pressed against the tissue to destroy it. Then the tissue is cut with ultrasonic vibrations and removed by the suction unit.

Fig. 13 shows a third modification in which a heating device is attached to the observation line 49 with an optical image guide 49 A. In the observation line 49 , a pair of through holes 49 B are provided in the longitudinal direction. The bipolar antennas 60 are slidably inserted together with their cables 62 into the through holes 49 B. The probe 26 of the ultrasound resection unit is inserted into a through hole 49 in the observation line.

In the treatment devices according to the second embodiment form the microwave antenna in the probe, the sleeve, the observation line, etc.

A third embodiment of the present invention will be described with reference to FIGS. 14 to 16.

The device also consists of the handle parts 2 and the insertion part 8 . The ultrasonic vibrator 4 is accommodated in the handle part 2 . The vibrations generated by the ultrasonic vibrator 4 are transmitted to the probe 26 of the guide part 8 . The suction hole of the probe 26 communicates with a metal piece 68 which is located on the handle part 2 . A tap 70 is attached to the metal piece 68 . With the tap 70 , a suction hose 35 is connected ver. In this treatment device, a freezing probe 72 , as shown in FIG. 16, can be inserted through a channel 69 formed in the handle part 2 , ie through a suction hole. The freezing probe 72 has a tube 74 with a smaller outside diameter than the inside diameter of the channel 69 and a hand-held part 76 connected to the tube 74 . A hose 78 is connected to part 76 to supply freezing liquid or gas. The tap 80 in the hand-held part 76 is used to open or close the guide line 75 in the tube 74th A connector is provided at the proximal end of the tube 78 and can be releasably connected to the chassis of a freezer unit 86 which has a container 184 for storing, for example, liquid nitrogen.

A procedure for using the freezer will follow described standing.

The probe 26 in the insertion part 8 is also inserted into the body cavity through an endoscope channel that has already been inserted into the body cavity. The operator brings the distal end of the probe 26 into contact with a tissue section to be resected while viewing this tissue with the endoscope. The freeze probe 72 is inserted into the channel 69 of the probe 26 and the distal end of the tube 74 comes close to the tissue to be resected. Liquid nitrogen is then sprayed onto the tissue, thereby freezing this section of tissue to be resected.

The distal end of the ultrasound probe 26 is pressed against the frozen portion and then the ultrasound vibrator is turned on. The frozen tissue is resected with the help of ultrasonic vibrations. The resected tissue is removed from the body through the suction hole of the probe 26 , ie through the channel 69 and the suction hose 35 .

The removal of the resected tissue and the supply of liquid nitrogen can be controlled using the cocks 70 and 80, respectively. In this embodiment of the treatment device, the tissue can be resected easily and with high efficiency with ultrasonic vibrations after it has been frozen beforehand. The blood of the tissue is also frozen, so the bleeding that occurs is minimal.

A modification of this third exemplary embodiment is described with reference to FIGS. 17 to 18.

An ultrasound probe 26 and a freeze probe 72 are inserted into the sleeve 48 of the endoscope 47 . In addition, an optical observation tube 49 is inserted into the sleeve 48 . The tissue can be frozen and resected while the operator observes the area to be resected with the observation tube 49 . The observation tube 49 is connected to an eyepiece 88 which is removably attached to the proximal end portion 48 A of the sleeve 48 .

In the illustrated embodiment, ultrasonic probe 26 and freezer probe 72 are arranged separately from each other. However, it is also possible to introduce the refrigerant gas or the refrigerant liquid directly into the inner hole of the ultrasound probe 26 , so that it is used as a common probe for resection and for freezing.

A fourth embodiment of the present invention will be described with reference to FIGS. 19 to 20B.

Fig. 19 shows a resectoscope for resection of affecting the tissue with ultrasonic waves. The resectoscope 90 has a sleeve 48 which has a cylindrical shape and is provided for insertion into a body cavity. The eyepiece 88 of the optical observation tube 49 is removably attached to the proximal end portion 48 A of the sleeve 48 . The distal end portion of the optical observation tube 49 is arranged near an opening 91 at the distal end of the sleeve 48 . An objective lens (not shown) is built into the distal end portion of the observation tube 49 . An image imaged by the objective lens is transmitted to the eyepiece 88 on the grip part via a relay lens and a prism, which are built into the observation tube 49 . The operator can observe the image through the eyepiece.

A tubular ultrasonic probe 26 is placed in parallel to the optical observation tube 49 in the sleeve 48 . The probe 26 extends through a connecting portion 93 . The housing 5 , which includes the ultrasonic vibrator, is attached to the proximal end of the probe 26 . A finger ring 94 is fastened as an actuating element 101 to a frame 92 of the housing 5 . A handle 96 is mounted on the connecting portion 93 of the optical observation tube 49 . On the finger piece 96 , a guide rod 98 is attached, on which the frame 92 is guided.

A spring 100 surrounding the guide rod 98 is clamped between the frame 92 and the finger piece. The ultrasound probe 26 is thereby always biased backwards. The operator holds the actuation arrangement so that the thumb of a hand that encloses the finger piece 96 is hooked into the finger ring 94 so that the frame 92 can be pushed forward with the thumb against the restoring force of the spring 100 . The distal end of the probe 26 can then protrude forward from the distal opening 91 of the sleeve 48 .

In this embodiment, a flushing connection 102 is provided on the proximal end 48 A of the sleeve 48 . What server supply 104 is connected to port 102 . The ultrasonic probe 26 has a suction path and this suction path is connected to the suction hose 35 . At the suction hose 35 , the suction device 30 is ruled out.

The lines 22 connect the ultrasonic vibrator 4 to the control voltage generator 24 . The suction device 30 and the control voltage generator 24 are installed in a control unit 112 .

As can be seen from FIGS. 20A and 20B, a hemostasis electrode 114 is seen at the distal end of the sleeve 48 . The electrode 114 coated with electrical insulation 118 is embedded in the lower section at the distal end of the sleeve. The electrode 114 is connected to a single-core conductor 116 which is encased by insulation material. Conductor 116 is also covered by insulation 118 and secured along the inner surface of sleeve 48 . The conductor 116 is connected to a cable connector 120 at the proximal end 48 A of the sleeve 48 . A high frequency power supply 122 is connected to the cable connector 120 via a supply line 124 .

A method of using the resect described above toskops is explained below:

The sleeve 48 of the resectoscope is inserted into the living organism and the operator looks at the tissue concerned with the optical observation tube 49 in the sleeve 48 . The distal end 91 of the sleeve 48 is fixed directly to the tissue to be resected. Then the operator turns on the ultrasonic vibrator and hooks his thumb of the hand holding the finger piece 96 into the finger ring 94 to move the probe 26 forward. As a result, the distal end of the probe 26 protrudes forwardly from the distal end surface of the sleeve 48 . The probe 26 bears against the tissue to be resected, which is disrupted and resected by the ultrasonic vibrations from the distal end of the probe. The tissue area to be resected is completely removed by repeated application of the resection cycle. The resected tissue is removed from the body through the suction path of the probe 26 and the suction hose 35 . At the same time, irrigation fluid is passed from the water supply 104 through the sleeve 48 into the body cavity. A device for degassing the bubbles in the rinsing liquid can be provided between the water supply and the rinsing connection 102 . This allows degassed irrigation fluid to be conducted into the body cavity.

When the tissue with the ultrasonic vibrations is resected by the ultrasonic probe 26 , bleeding sections from small blood vessels, in particular veins, are rubbed by the ultrasonic vibrations transmitted from the distal end of the probe 26 and the bleeding of these sections is caused stopped by the friction heat. However, if the bleeding is from a large blood vessel, the ultrasound probe 26 is withdrawn into the sleeve 48 . A high frequency current is applied to the hemostatic electrode 114 to stop bleeding by heating.

In this exemplary embodiment, the ultrasound probe is arranged in the lower region of the sleeve 48 and the blood-draining electrode 114 is located in part of the lower wall at the distal end of the sleeve 48 . If a blood vessel is cut open during the resection and bleeding starts from the resected section, the bleeding can be stopped quickly because the hemostatic electrode 114 is close to the bleeding site.

FIG. 21A and 21B show a modification of the fourth embodiment.

In this embodiment, the electrode 114 is attached to the outer surface at the distal end of the sleeve 48 , which is made of an electrically insulating material. Thus, no current also flows from the electrode 114 to a treatment tool when the distal end of the inserted into the sleeve 48 treatment tool comes into contact with the inner surface of the sleeve 48th

FIG. 22A and 22B show another example of execution. The conductor 116 runs in the wall of the sleeve 48 . For this purpose, an inner tube 126 and an outer tube 128 are used to form the sleeve 48 and the conductor 116 is inserted into the space between the inner and outer tubes 126 and 128, respectively. If the tubes 126 and 128 are made of an electrically insulating material, a conductive film or tube can be inserted into the space.

If the sleeve 48 is made of a conductor, an insulated conductor 116 can be inserted into the space between the two tubes 126 and 128 .

In the fourth embodiment and in its modifications is a high frequency electrode used as a heating element. This is not a requirement. For example, a bipolar Electrode arrangement consisting of two electrodes as a heating element be used. A heating probe and temperature self-regulating heating element, for example a PTC Element, can also be used. Alternatively can be a nichrome wire that electrically insulate with a the film is coated can also be used. Further a laser conductor can be formed in the sleeve and a Laser emitter as a heating element at the distal end of the sleeve be arranged.

A treatment tool inserted into the sleeve must not an ultrasound probe, but can also, for example as a high frequency resection tool.

Modifications of the distal end section of the probe 26 in the ultrasound treatment device according to the invention are described below.

Fig. 23 shows a first embodiment of the distal end portion. The distal end portion 130 of the probe 26 of the insertion part 8 has a smooth, rounded surface.

With this probe 26 , the contact area between the distal end 130 of the probe 26 and the part of the body cavity can be increased. In addition, no sharp parts are used, which prevents damage to the fabric. As a result, blood vessels of the living organism are not injured and the tissue can be effectively resected with ultrasound waves.

Fig. 24 zeigt ein zweites Ausführungsbeispiel. Fig. 24 shows a second embodiment. The outer diameter of the distal end portion 130 of the insertion part is enlarged so that a protrusion 131 is formed on the outer surface. From the end face from the distal end portion 130 is formed by a curved surface. In this embodiment, the cross-sectional area of the distal end portion 130 is larger than that of the distal end portion in the first embodiment. Therefore, the ultrasonic vibrations can be transferred to the tissue more effectively.

Fig. 25 zeigt ein drittes Ausführungsbeispiel. Fig. 25 shows a third embodiment. The inner diameter of the distal end of the probe 26 is reduced and forms a projection 132 on the inner surface. Since the distal end is narrowed, the suction hole 29 cannot clog with resected tissue.

In a fourth exemplary embodiment, which is shown in FIG. 26, the underside of the distal end of the probe 26 protrudes and forms an arcuate section 133 . The contact area between the distal end portion 130 and the tissue is increased.

In a fifth and sixth embodiment ( FIGS. 27 and 28), the distal end sections of the tubes 26 are flanged inwards and outwards.

In a seventh embodiment shown in FIGS. 29 and 30, the shape of the distal end portion of the tube 26 of the insertion part 8 differs from that of the distal end portions 130 of the previous embodiment examples. A substantially U-shaped resection element 134 is shown in the distal end portion of the tube 26 of the insertion part 8 in FIG. 29. An opening 136 , which communicates with the suction hole 29 in the tube 26 of the insertion part 8 , is arranged in the central region of the distal end surface 134 A of the resection element 134 .

Fig. 30 shows the state in which the optical observation tube 49 is arranged together with the resection element 134 in the sleeve. In order to press the U-shaped resection element 134 against the tissue in the body cavity, the sleeve 48 is inserted into the body cavity and then the optical observation tube 49 is inserted into the upper part of the interior of the sleeve 48 . The ultrasound probe 46 with the resection element 134 is inserted into the lower part of the interior of the sleeve 48 . The operator brings the distal end surface 134 A of the resection element 134 against the tissue, looking at the tissue with the observation tube 49 . The ultrasonic vibrations are applied to the tissue so that the tissue is resected. Then, the suction device is operated to remove the resected tissue from the suction hole 136 on the distal end face 134 A.

In this exemplary embodiment, the contact area between the affected tissue and the distal end surface 134 A of the resection element 134 is increased compared to the previous exemplary embodiments.

In an eighth embodiment, a flat reection element is arranged at the distal end of the ultrasound probe 26 .

In a ninth embodiment shown in FIG. 32, a cylindrical resection element 140 is provided at the distal end of the ultrasound probe 26 . In a distal end surface 140 A of the resection element 140 , a guide hole 142 is formed into which the optical observation tube 49 can be inserted. A suction hole 136 is provided in the lower region of the resection element 140 . In the resection member 140 , the viewing tube of the sleeve 48 can be reciprocated as the tube is inserted into the hole 142 .

In this embodiment, the contact zone between the tissue and the distal end of the ultrasound probe be increased so that the resection effect by the Ultra sound vibrations can still be improved.

If the suction hole 136 has a flat shape or a U-shaped shape, a maximum cross-sectional area can be obtained by utilizing the space for forming the suction hole.

In the seventh to ninth embodiments, the distal end surfaces in Figs. 29 to 32 are shown flat. However, they do not have to be flat, they can also be rounded. If the end surfaces have a smooth round surface, the normal tissue is not injured during the resection.
