Abstract
Description
Verfahren und Vorrichtung zur exkorporalen Method and device for extracorporeal
Purification of blood The invention relates to a method for extracorporeal Purification of blood, with waste products and blood water in a first procedural stage be filtered out of the blood and in a second process step so a suitable amount of blood water is supplied in a dosed manner.
The extracorporeal cleaning of blood has so far been carried out by a so-called artificial kidney performed. The artificial kidney serves to function a natural kidney in terms of elimination of waste products and excess To take fluid from the blood. Several methods are known for this purpose should approximately achieve the natural function of the kidney. The most common today applied procedure is. the so-called dialysis.
In this case, the slag substances are electrolytically osmotic into a liquid, called dialysate, through a filter-like membrane, dialyzer called, directed. This dialysate is in relative large amount needs and requires a complex preparation. Be through the dialyzer not all waste products removed, but only up to a certain molecular size.
Another type of artificial kidney ensures a more extensive one Removal of waste products, called hemofiltration. A special foil is used for this with greater permeability (for example polyamide or polysulfone) blood water and waste products with a wider range of molecular sizes from the blood filtered out by overpressure.
This creates a so-called hemofiltrate, which in its composition corresponds to what the natural kidney corresponds to in a first process step Functional process also generated.
This hemofiltrate must now be replaced by a substitution solution whose composition corresponds to the filtrate, but no waste products which are present in the first-mentioned hemofiltrate.
The substitution solution is achieved by means of an electronic balancing device fed back into the blood concentrate.
It is significantly more natural and healthier if the filtrate itself is freed from waste products and then fed back into the blood concentrate becomes, which corresponds perfectly to the way the natural kidney works. This does not apply the production of a substitute solution, which is only approximate in its composition can correspond to the body's own filtrate. The removal of waste products from the filtrate, also called urinary substances, is already through appropriate filter media possible, but this process is very expensive and dangerous, as after some time the filter material is saturated and no more waste products can be included.
Darüber hinaus ist dieses Verfahren sehr kostenintensiv. In addition, this process is very costly.
Starting from this, the invention is based on the object of a method and to provide a device for the extracorporeal purification of blood, with which in a relatively simple way the blood water including waste products from the Blood can be removed and, after cleaning, the blood concentrate can be returned. The procedural Solution to this problem is described in claims 1 to 5.
The method according to the invention is based on the knowledge that the natural kidney has a high need for oxygen for the biological-chemical The second stage of filtering and cleaning is called tubulus. There about 25% of the blood volume carrying the oxygen flow.
Even if these processes are very complex, it can be assumed that that when the filtrate comes into contact with oxygen (or an oxygen-carrying Gas) comes very close to the natural cleaning effect.
The invention provides a continuous process with the without great effort and cleaning of the filtrate without changing the external conditions can be achieved. The procedure is based on the fundamental knowledge that by an oxidation of the waste products with an oxidizing, gaseous one Medium, preferably oxygen, the slag materials can be foamed out of the filtrate are.
A device according to the invention for carrying out the method is referred to in claims 6 to 8.
Here, the gaseous medium, for example oxygen, is in very finely distributed form, which is achieved, for example, by an air stone is passed into a contact tube and in very close contact with the filtrate brought.
The oxidizing medium oxidizes the oxidizable slag constituents of the filtrate, forming an oxide foam. The individual ones that form the oxide foam Bubbles rise in the filtrate liquid so that they are above the liquid level forms an oxide foam layer. The oxide foam is in a downstream one above Deposited condensate chamber, where it collapses into a condensate and back in a collecting vessel is diverted. The collapse of the oxide foam can through Silicone or other suitable substances are accelerated.
While the filtrate is being pumped through the device, takes place the contact with the oxidizing medium several times in the circuit. which Types of target waste products in this oxide foam weeded out is not recorded in detail. The resulting foam forms a Column growing upwards, with the previously wet foam in the upper part getting drier, that is, getting firmer. By one provided according to claim 8 Adjusting the height of the foam chamber, the foam can emerge from this point in which the amount of liquid from the foam condensate after completion of the treatment corresponds to the amount of fluid that the blood due to the nonfunction the kidney must also be removed in addition to the waste products.
The purified filtrate is removed from the vessel in which it was previously circulated has been derived below, where it is already largely gas-free. It then arrives via a riser pipe that has a constant filtrate level through its upper drain determined in the circulation chamber, in a collecting vessel, a so-called buffer chamber. Any gas that may still be present can rise here during a dwell time into the condensate chamber.
The purified filtrate can be extracted from this chamber through a level-controlled Pump the blood concentrate again are fed.
That way is. continuous blood purification possible, ensuring that the blood before and after cleaning with the exception of Slag has practically the same composition.
The -Vorrichtuno according to the invention is described in more detail in the drawing.
It shows: FIG. 1 the diagram of a cleaning device; FIG. Fig. 2- a Detail in enlarged view.
Die Strömungsrichtung des zu reinigenden Blutes bzw.The direction of flow of the blood to be cleaned or
The sequence is shown schematically in FIG. 1. The blood flows in the process first via a line to a pump 1, from which the blood to a filter chamber 4 is further funded.
The filter chamber 4 represents a so-called hemofilter in which the blood water and waste products are largely separated from the blood concentrate will.
The blood concentrate flows through another line to an air trap trained collecting vessel 3.
The separated slag-rich blood water flows to a cleaning device 5, in which the slag materials are removed, and those in FIG. 2 in detail will be described. After purification, the blood water is carried through the further Pump 2 is promoted to the collecting vessel 3 and there by means of an electronic balancing device returned to the blood. The electronic balancing facility is in the status of Technology is known, so that a more detailed description can be dispensed with here.
With 6 collecting container is designated, in which the described later Condensate is collected.
The cleaning device is shown in more detail in FIG. The from the line coming from the pump 1 opens into an approximately cylindrical vessel, which has an inlet chamber 7 near its bottom. About the mouth of the appropriate Feed line is a chimney-like tube 10 turned over, which is open at the top and at the bottom is provided with passage openings near the bottom. In the chimney-like pipe is also an air stone 11 is arranged through which the oxidizing gaseous medium can flow in. For this can for example or also oxygen oxygen-containing air can be used. This arrangement forms in the Vessel at 8 a circulation chamber, since the filtrate flowing through is inevitably inside rises in the pipe 10 and on the outside of the same in the housing or
in der Umwälzkammer wieder nach unten fließen muß.must flow back down in the circulation chamber.
As a result, it is at least partially through the pipe 10 several times passed, so that an intensive mixing of incoming atmospheric oxygen and Filtrate takes place. Above the lower mouth or openings of the tube 10, however at a great distance from the mouth of the vessel, a riser 13 is branched off, which in turn opens into a downstream collecting vessel 14. The riser pipe 13 is used at the same time as a leveling tube for the filtrate level in the circulation chamber 8, there this forms a system of communicating tubes. Into the chimney-like pipe 10 receiving vessel, a telescope is inserted, which forms part of the vessel. Above the chimney-like pipe, where the foam collects, is a foam chamber 9 formed, which is tapered towards the overhead mouth. The mouth flows into a condensate collection chamber 12, which has a lockable Drain opens outwards. As can be seen from FIG. 1, the sequence includes a Collection container 6 placed.
The condensate chamber 12 is covered by a cover and a Line connected to the corresponding cover of the collecting vessel 14. It becomes so an overpressure in the chamber 14 is prevented. For the purpose of determining the telescopic displaceable screw chamber 9 is formed on the outside of this a bend which can be fixed by means of a locking screw 9a. It should also be noted that the supply of gas to the outflow device 11, for example by means of a Air pump can be done. But it can also be oxygen in status nascendi as a gaseous form Medium can be used. The operation of the device is as follows.
Oxidizing gaseous medium is released through the discharge device 11 introduced into the contact tube 10. There it becomes with the likewise flowing in Associated filtrate. The slag portion of the filtrate is foamed and the resulting oxide foam is continuously drained upwards. In the condensate chamber The foam collects and is deposited, collapsing and as condensate is directed to a collecting vessel 6.
While the filtrate is pumped through the apparatus shown, the contact is made in the contact tube 10 several times in the circuit. The emerging Foam forms a column growing out of the mouth of the chamber 9, whereby in the upper part of the foam the wet foam is always drier, that is to say firmer will. By adjusting the height of the chamber 9, the foam can now come out at the point this exit upward into the chamber 12, in which the amount of liquid from the Foam condensate after completion of the treatment corresponds to the amount of liquid that is to be removed from the filtrate. The purified filtrate is removed from the circulation chamber derived below, where it is already gas-free. It reaches the riser pipe 13, which through its upper drain -a constant filtrate level in the circulation chamber 8 determined, in the buffer chamber of the vessel 14. Here, during a dwell time Remove any residual gas upwards into the condensate chamber 12. Down will the purified filtrate then removed by a level-controlled pump 2 and the Blood concentrate supplied again at 3.
All new items disclosed in the description and / or drawing and combination features are considered essential to the invention.
