Die Erfindung betrifft neue Kontrastmittel für die.Ultraschall-Diagnostik
von mit Flüssigkeit gefüllten Gefässem
und Hohlräumen im menschlichen «and tierischen Körper ge— mäss der Ansprüche.
Es ist allgemein bekaraxit, . dass dar !Contrast in der Ultraschall-Diagnostik dureh die Anwesenheit von Gas&läschert
in der Flüssigkeit (Blut), die das Untersuehusrageobjekt
durchströmt, erhöht wird. ' Zu dias©ra Zweck karara man die-=

Obwohl die durch diese Massnahmesn ©raielt© Kontrastvorbesserung
beträchtlich sein kamm, körmen die so ersenagten
Gasbläschen gefährliche Zwischenfälle hervorrufen wie beispielsweise Gas-Embolien. Da weder die Grosse noch die
Konzentration der Bläschen durch die angeführten Verfahren garantiert werden können, ist die Reproduzierbarkeit
des Echokontrastes nicht gewährleistet« Eine Verbesserung
bietet die Möglichkeit, vorgefertigte gasgefüllte Mikrokapseln zu verwenden, die aus einer Ge la/tine-Membran "bestehen
(US.P.h,276,885). Die Suspension dieser Mikrokap-.-sein
in einer Trägerflüssigkeit wird in den Blutstrom injiziert,
wo sie die Kontrasterhöhung bewirken. Der Nachteil
dieser Methode liegt in den hohen Herstellungskosten begründet und auch darin, dass vor der Untersuchung- die Suspension
aus Mikrokapseln und Trägerflüssigkeit durch
Mischen erst hergestellt werden muss, und so die geforderte Sterilität nicht gewährleistet ist» Durch die Bereitstellung
der erfindungsgemässen neuen Ultraschall-Kontrast"=·
mittel werden die geschilderten Nachteile beseitigt. Es
ist überraschend, dass durch die Injektion des neuen Mittels in den Blutstrom Gasbläschen entstehen, die hervorragend
geeignet sind, den Ultraschall-Kontrast zu erhöhen ohne Komplikationen oder Zwischenfälle (Gasembolien bzw.
durch Partikel verursachte Embolien) hervorzurufen.
Darüber hinaus ist die gewünschte Kontrastdichte durch
Vergrösserung bzw. Verminderung des injizierten Volumens des Kontrastmittels gut steuerbar. Während die Kontrastmittel nach dem Stand der Technik als nichtsterile Suspensionen vorliegen, steht das erfindungsgemässe Kontrastmittel als klare sterile Lösung spritzfertig zur Verfügung und muss nicht erst durch Mischen vor der Untersuchung hergestellt werden.
Darüber hinaus ist die gewünschte Kontrastdichte durch
Vergrösserung bzw. Verminderung des injizierten Volumens des Kontrastmittels gut steuerbar. Während die Kontrastmittel nach dem Stand der Technik als nichtsterile Suspensionen vorliegen, steht das erfindungsgemässe Kontrastmittel als klare sterile Lösung spritzfertig zur Verfügung und muss nicht erst durch Mischen vor der Untersuchung hergestellt werden.
Geeignet als Ultraschall-Kontrastmittel ist die Lösung eines
oder mehrerer Tenside insbesondere eines oder mehrerer nichtionogener Tenside in einer wässrigen oder mit Wasser
mischbaren Trägerflüssigkeit.
Als Tenside sind sowohl nichtionogene als auch ionogene
Tenside geeignet. Als nichtionogene Tenside seien ■
genannt: Lecithine, Lecithinfraktionen und deren Abwandlungsprodukte, Polyoxyethylenfettsäureester wie Polyoxyethylenstearate, Polyoxyethylenfettalkohole oder
Polyoxyethylenfettalkoholather, polyoxyethylierte Sorbitanfettsaurester, Glycerin-polyethylenglykoloxystearat,
Glycerinpolyethylenglykolrhizinoleat, ethoxylierte Sojaster ine, äthoxylierte Rizinusöle und deren hydrierte Derivate, Cholesterol, Polyoxyethylenpolyoxypropylen-Polymere, wobei Polyoxyäthylenfettsäurestearate und Polyoxyethylenpolyoxypropylen-Polymeres mit dem Molgewicht 68OO bevorzugt sind.

Als ionogene Tenside kommen infrage: Quarternäre Atnmoniumbasenf
Natriumlaury!sulfat, Natriumdioctylsulfosuccinat.

Die wässrige Kontrastmittel-Lösung kann dabei 0,01 bis 10% eines Tensides oder des Gemisches mehrerer Tenside
enthalten, wobei der bevorzugte Gehalt 0,5 bis 1,5% beträgt.
Das Tensid oder das Tensidgemiseh wird für die Verwendung als Ultraschall-Kontrastmittel in einer Trägerflüssigkeit
gelöst. Die wässrig© oder mit Wasser mischbare
Trägerflüssigkeit muss physiologisch verträglich
sein. Es kommen dafür in Betracht Wasser ©der die wässrigen Lösungen von ein- oder mehrwertigem Alkoholen wie
Äthanol, Propanol und längerkettige Alkohole, Polyethylenglykol,
Glycerin, Propylenglykol,
weiterhin physiologische Elektrolytlösuingan wie
physiologische Kochsalzlösung, Ringerlösung oder die wässrigen Lösungen von Natriumchlorid, Kalsiuraehlorid, Watriumhydrogencarbonat,
Natriumeitrat, Natriwisasetat oder Watriumtartrat
oder Salzlösungen, wie sie üblicherweise als Infusionslösungen verwendet werden»
Geeignet sind auch wässrige Lösungen von Mono- oder Polysacchariden
wie Glucose, Lävulose, Galactose, Sorbit, Mannit, Xylit, Saccharose.
Die dafür geeigneten Polysaccharide sind beispielsweise Dextrane, Cyclodextrine oder Hydroxyäthylstärke„
Als dafür infrage kommende Polyole seien insbesondere
1,2-Propandiol, Polyglykole, Glycerin und Inolin genannt»
AIa -wässrige Lösungen kommen weiterhin die Lösungen von
Proteinen, proteinähnlichen Stoffen, Aminosäuren oder von Blutersatzstoffen infrage, wie beispielsweise Plasraaproteine,
Gelatine, Oxypolygelafine und Gelatinederivate,
oder Polyvinylpyrrolidon sowie deren Gemische untereinander .
Die Konzentration dieser genannten Stoffe in Wasser kann
von 0,5 bis 50% betragen, wobei die Höchstkonzentration
auch vom gelösten Stoff abhängt. So kann beispielsweise Glucose-Lösung mit einer Konzentration von 0,5 bis 50%
verwendet werden, wogegen eine Gelatinelösung eine bevorzugte Konzentration von 0,5 bis 2% hat. Die Oxypolygelatine-LÖsung
dagegen wird bevorzugt mit einer Konzentration von 0,5 - 10% eingesetzt.
Es kann für die Kontrasterhöhung des neuen Ultraschallkontrastmittels
von Vorteil sein, wenn das Kontrastmittel ein physiologisch verträgliches Gas enthält. Als
Gase kommen für diese Zwecke sterile Gase wie Luft, Sauerstoff, Stickstoff, Kohlendioxid oder Edelgase infrage,
wobei Luft oder Kohlendioxid bevorzugt sind. Der Gehalt an Gas in der Lösung ist durch den bei der Begasung angewendeten
Druck, der bis zu 1 atm., vorzugsweise 0.75 atm. beträgt, abhängig.
Im Falle der Verwendung von Kohlendioxid als physiologisch verträgliches Gas ist es auch möglich, das Gas vor der
Verwendung des Kontrastmittels in der Lösung zu erzeugen, in dem man zu der Lösung des Tensides in der Trägerflüssigkeit,
die in diesem Fall 1 bis 3% Alkali- oder Aramonium-bikarbonat
oder -karbonat enthält, die 1 bis 3%ige
wässrige Lösung einer Säure, die durch Reaktion mit dem Alkali- oder Ammonium-bikarbonat oder-Karbonat physiologisch
verträgliche Salze bildet, wie beispielsweise Zitronensäure, Salzsäure, zu*»gibt.
Zur Anwendung kommen je nach dem gewünschten Kontrast 0,1 bis 10 ml der Kontrastmittel-Lösung, die während der
Ultraschall-Untersuchung in eine Vene injiziert wird. So werden beispielsweise für die Ultraschall-Kontrastaufnahme
der rechten Herzkammer beim Hund (Beagle Hunde von 1712 kg Körpergewicht, 2,5 Jahre alt, männlich, geschlos-

und mit dieser Kontrastrnittel-Lösung die besten Ergebnisse

der. Ultraschall-Echos sowie deren diagnostische Auswertung erfolgt in an sich bekannter Weise und ist sum. Beispiel

Kochsalzlösung oder Ringer-Lösung enthält.

gekennzeichnet, dass es als Trägerflüssigkeit die wässrige Lösung von 0,5 - 10% Oxypolygelatine enthält«

16. Ultraschallkontrastmittel gemäss Anspruch 10, dadurch gekennzeichnet, dass es ein physiologisch verträgliches
steriles Gas wie Luft, Kohlendioxid, Sauerstoff,
Stickstoff oder deren Gemische unter einem Druck bis zu 1 atm. enthält.
