Description
Die Erfindung betrifft eine Gefäßprothese, die hauptsächlich aus einem porösen Schlauch oder Rohr aus Polytetrafluoräthylen
(nachfolgend abgekürzt mit 11PTFE") besteht, und mit der
eine Verbesserung der Anticoagulationseigenschaften ihrer inneren Oberfläche, die Verhinderung des Blutens, eine Verbesserung
der Festigkeit des Schlauches bzw. Rohres und eine Verbesserung der Fähigkeit ihrer äußeren Oberfläche, sich
mit den Geweben eines Patienten zu verbinden, angestrebt wird.
Aus vielen Artikeln ist bekannt, daß ein poröser Schlauch oder ein poröses Rohr aus PTFE, hergestellt unter Anwendung
eines Streckverfahrens, klinisch verwendet werden kann als Gefäßprothese. Eine solche Prothese wird als besser angesehen
als konventionelle Gefäßprothesen, die aus gewirkten oder gewebten Geweben bestehen. Ein PTFE-Schlauch, der einer Verstreckungsbehandlung
unterworfen worden ist, hat eine Mikrostruktur, die aus sehr feinen Fasern und durch diese Fasern
miteinander verbundenen Knoten besteht. Die Durchmesser der Fasern variieren in Abhängigkeit von den Verstreckungsbedingungen,
sie können jedoch viel kleiner gemacht werden als diejenigen der Fasern der oben erwähnten gewirkten oder gewebten
Gewebe. Da der Porendurchmesser und die Porosität des Schlauches bzw. Rohres bei seiner Verwendung als künstliches Gefäß
frei variiert werden können, ist er (es) biegsam und erlaubt kaum die Bildung von Thromben. Der Schlauch bzw. das Rohr
weist auch eine gute Bildung einer Neointima auf seiner (ihrer) inneren Oberfläche auf, ohne daß irgendein merklicher nachteiliger
Effekt auf die umgebenden Gewebe auftritt. Der ge-

streckte Schlauch bzw. das gestreckte Rohr wird daher als eine der besten Gefäßprothesen angesehen.
Der durch Verstrecken hergestellte poröse PTFE-Schlauch ist jedoch
nicht völlig frei von einer Thrombenbildung und es ist noch Raum für Verbesserungen. Der verstreckte PTFE-Schlauch hat
ferner den Nachteil, daß dann, wenn er als Gefäßprothese verwendet und mit dem lebenden Körper verbunden wird, die Nadel
oder das Nahtmaterial dazu neigt, den Schlauch bzw. das Rohr zu zerreißen. Darüber hinaus ist mit der Elastizität des
porösen PTFE-Schlauches allein ein natürlicher Verschluß der Nahtlöcher schwierig und nach dem Verbinden ist manchmal ein
Bluten aus den Nahtlöchern festzustellen. Ein anderes Problem ist die geringe Fähigkeit der äußeren Oberfläche des porösen
PTFE-Schlauches, sich mit den umgebenden Geweben eines Patienten zu verbinden. Diese Probleme werden mit der vorliegenden
Erfindung gelöst.
Ziel der vorliegenden Erfindung ist es, eine Gefäßprothese zu entwickeln, die frei von der Bildung von Thromben ist und
nicht zerreißt bzw. einreißt und den natürlichen Verschluß der Nahtlöcher erlaubt.
Gegenstand der Erfindung ist eine Gefäßprothese, die enthält oder besteht aus einem porösen Schlauch bzw. Rohr aus
Polytetrafluoräthylen (PTFE), das eine Anticoagulationssubstanz enthält, und einem an seine äußere Oberfläche gebundenen
porösen Elastomerüberzug, der eine Substanz (Gegensubstanz) enthält,dia darAnticoagulationssubstanz entgegenwirkt.

Da die erfindungsgemäße Gefäßprothese eine Anticoagulationssubstanz
enthält, bilden sich keine Thromben auf ihrer inneren Oberfläche. Durch das Aufbringen eines porösen Elastomerüberzuges
wird das Problem des Zerreißens bzw. Einreißens des Schlauches bzw. Rohres vermieden und die Nahtlöcher werden
auf natürliche Weise durch die Elastizität dieses Elastomerüberzugs verschlossen. Sollte an den Nahtstellen ein Bluten
auftreten, so wird es durch die in dem beschichteten Abschnitt enthaltene Gegensubstanz gestoppt. Diese Gegensubstanz
kann außerdem das Bluten verhindern, das auf das Austreten der Anticoagulationssubstanz zurückzuführen ist. Ferner
wird durch den porösen Elastomerüberzug die Fähigkeit des Schlauches bzw. Rohres, sich mit den umgebenden Geweben eines
Patienten zu verbinden, verbessert.
Der erfindungsgemäße poröse PTFE-Cchlauch wird nach dem in
der japanischen Patentpublikation 13 560/67 und beispielsweise in den US-Patentschriften 3 962 153 und 3 953 566 beschriebenen
Verfahren hergestellt.
Dabei wird ein flüssiges Gleitmittel (Schmiermittel) mit einem ungesinterten Pulver aus Polytetrafluoräthylen gemischt
und die Mischung wird mittels eines Kolbenextruders zu einem Schlauch bzw. Rohr extrudiert. Das erfindungsgemäß verwendete
PTFE hat vorzugsweise ein Molekulargewicht von 10 bis 10 . Der Schlauch bzw. das Rohr (nachfolgend der Einfachheit halber stets
als "Schlauch" bezeichnet) wird uniaxial verstreckt, nachdem
das flüssige Gleitmittel (Schmiermittel) gegebenenfalls davon entfernt worden ist. Der Schlauch wird dann auf eine
Temperatur oberhalb 327 C erhitzt, bei der es sich um die Sintertemperatur handelt, während er an Ort und Stelle fixiert

wird, um ein Schrumpfen zu vermeiden. Auf diese Weise wird die gestreckte und expandierte Struktur fixiert unter Bildung
eines Schlauches mit einer erhöhten Festigkeit. Der dabei erhaltene poröse PTFS-Schlauch hat eine MikroStruktur
aus sehr feinen Fasern und durch diese Fasern miteinander verbundenen Knoten. Da die Durchmesser und Längen dieser
Fasern und die Größen und die Anzahl der Knoten variieren können in Abhängigkeit von den Verstreckungs- und Sinterungsbedingungen, können der Porendurchmesser und die Porosität
des dabei erhaltenen porösen Schlauches frei bestimmt werden. Es wurde klinisch bestätigt, daß dann, wenn dieser
Schlauch als Gefäßprothese verwendet wird, er zweckmäßig einen durchschnittlichen Porendurchmesscr von etwa 2 /um bis
etwa 100 um, eine Porosität von mindestens etwa 70 "L und eine
Uanddicke von etwa 0,3 bis etwa 1,0 mm hat.
Beieinarbevorzugten Form der MikroStruktur des erfindungsgemäß
verwendeten porösen PTFE-Schlauches sind die Fasern nicht in einer Richtung, sondern radial verteilt. Diese Faserstruktur
wird erhalten durch biaxiales Verstrecken cfes PTFE-Schlauches, nämlich durch Verstrecken desselben in axialer Richtung
und Ausdehnen (Expandieren) seines Durchmessers. Die Ausdehnung seines Durchmessers kann dadurch erzielt werden,
daß man den Druck auf der äußeren Oberfläche des Schlauches vermindert oder seine innere Oberfläche unter Druck setzt
oder beides gleichzeitig durchführt. Alternativ kann der Durchmesser des Schlauches mechanisch vergrößert werden, indem
man einen Formkörper einer geeigneten Konfiguartion durch das Innere des Schlauches hindurchführt. Das Verstrecken des

seines Durchmessers werden gleichzeitig oder nacheinander durchgeführt oder sie können gleichzeitig mit der Endsinterungsstufe
durchgeführt werden. Der nach dem biaxialen Verstreckungsverfahren
erhaltene poröse PTFE-Schlauch ist biegsamer und neigt weniger zum Zerreißen bzw. Einreißen in
Längsrichtung als ein poröser PTFE-Schlauch, der nur in axialer Richtung verstreckt worden ist, weil die Fasern nicht
nur in axialer Richtung, sondern radial in allen Richtungen verteilt sind. Für die Verwendung als Gefäßprothese bleibt
jedoch immer noch Raum für Verbesserungen.
Der erfindungsgemäß vorgesehene poröse Elastomerüberzug hat
den Zweck, das Zerreißen bzw. Einreißen des Schlauches zu verhindern, das Bluten durch den natürlichen Verschluß der
Nahtlöcher zu stoppen und die Fähigkeit des Schlauches, sich mit den umgebenden Geweben eines Patienten zu verbinden, zu
verbessern.
Erfindungsgemäß kann jedes beliebige Elastomere verwendet
werden, das für den Körper unschädlich ist.
Geeignete Beispiele sind Fluorkautschuk, Silikonkautschuk,
Urethankautschuk, Acry1kautschuk und Maturkautschuk.
In der Regel werden die Elastomeren im vernetzten Zustand verwendet und auch erfindungsgemäß werden die Elastomeren
vorzugsweise im vernetzten Zustand verwendet, um ihren Abbau (Verschlechterung) im lebenden Körper zu verhindern.
Für die Verwendung als Gefäßprothese liegt ein geeigneter
durchschnittlicher Porendurchmesser des porösen ELastomer-

Überzugs innerhalb des Bereiches von etwa 10yum bis zu etwa
500/um und eine ausreichende Dicke des Überzugs ist gleich der
Wanddicke des porösen PTFE-Schlauches oder kleiner, d.h. sie beträgt etwa 20 bis etwa 500/um und das Elastomere hat eine
Porosität von etwa 50 bis etwa 90 %.
Ein poröser Überzug aus dem Elastomeren kann auf der äußeren Oberfläche des porösen PTFE-Schlauches unter Anwendung verschiedener
Verfahren gebildet werden, beispielsweise unter Anwendung eines Verfahrens, das darin besteht, daß man eine
getrennt hergestellte poröse Folie aus dem Elastomeren um die äußere Oberfläche des Schlauches herumwickelt und damit verbindet,
eines Verfahrens, das darin besteht, daß man eine Lösung einer Elastomer mischung, die ein Treibmittel enthält,
in Form einer Schicht auf die äußere Oberfläche des Schlauches aufbringt und dann das Treibmittel zersetzt, eines Verfahrens,
das darin besteht, daß man eine Lösung einer Elastomermischung mit einer darin dispergierten löslichen Substanz
in Form einer Schicht auf die äußere Oberfläche des Schlauches aufbringt .und die lösliche Substanz herauslöst unter Bildung
einer porösen Struktur, eines Verfahrens, das darin besteht, daß man eine Elastomermischung in einem Gemisch aus einem
Lösungsmittel und einem Nicht-Lösungsmittel löst, die Lösung in Form einer Schicht auf die äußere Oberfläche des Schlauches
aufbringt und den Überzug trocknet, um ihn porös zu machen, oder eines Verfahrens, das darin besteht, daß man eine Lösung
einer Elastomer mischung in Form einer Schicht auf die äußere Oberfläche des Schlauches aufbringt und das restliche
Lösungsmittel entfernt durch Eintauchen des beschichteten
SchLauches in ein Nicht- Losungsmitte 1 b:ul oder durch Erhitzen
desselben auf eine Temperatur oberhalb des Siedepunktes des

Lösungsmittels, um dadurch den Überzug porös zu machen. Ein
Verfahren zur Herstellung eines porösen Überzugs aus einem Elastomeren, das darin besteht, daß man die äußere Oberfläche
eines porösen Schlauches aus PTFE mit einer Lösung einer Elastomermischung oder einer flüssigen Elastomermischung beschichtet
und vor dem Trocknen des Elastomerüberzugs die Innenwand des porösen Schlauches mit einem Gas oder einer Flüssigkeit
unter einen negativen Druck setzt, um das Elastomere ai verschäumen und so den Überzug porös zu machen, hat sich als
für die Zwecke der vorliegenden Erfindung am besten geeignet erwiesen. Der hier verwendete Ausdruck "Elatomermischung" bezieht
sich auf ein Gemisch aus dem Elastomeren und einem Vernetzungsmittel und dgl. Der hier verwendete Ausdruck "negativer
Druck" steht für einen Druck, der auf der Innenwand grosser
ist als auf der Außenwand des Schlauches bzw. Rohres. In der Regel läßt man einen Druck von etwa 0,05 bis etwa 1 bar
auf die Innenwand des Schlauches bzw. Rohres einwirken. Eine bevorzugte Viskosität des Elastomerüberzugs vor der Verschäumung
beträgt etwa 100 bis etwa 500 cP bei 25°C.
Bei der dabei erhaltenen Struktur aus dem porösen PTFE-Schlauch und dem mit seiner äußeren Oberfläche verbundenen
porösen Elastomerüberzug enthält der poröse PTFE-Schlauchanteil eine Anticoagulationssubstanz und der poröse Elastomerüberzug
enthält eine Substanz (Gegensubstanz), die der Anticoagulationssubstanz entgegenwirkt.
Die Anticoagulationssubstanz dient dazu, die Anticoagulationseigenschaften
der inneren Oberfläche des porösen PTFE-Schlauches zu verbessern und ein künstliches Gefäß zu ergeben, das
eine hohe Durchgängigkeit aufweist, ohne daß Thromben gebildet werden. Sie liegt innerhalb der Poren des porösen PTFE-Schlauches
vor. Die Anticoagulationssubstanz kann gleichmäs-

sig über die gesamten Poren des porösen PTFE-Schlauches verteilt
sein, vorzugsweise liegt sie jedoch nur auf der inneren Oberfläche des Schlauches vor.
Zu Beispielen für Anticoagulationssubstanzen gehören PoIysaccharidsulfate,
wie Heparin, seine Derivate, Chondroitinsulfat, Charoninsulfat, und organische Säuren, wie Citronensäure.
Heparinnatrium ist am wirksamsten und am leichtesten
zugänglich. Um die Anticoagulationssubstanz in die Poren des porösen PTFE-Schlauches einzubringen, wird der Schlauch in eine
Lösung der Anticoagulationssubstanz eingetaucht und getrocknet oder es wird ein polymeres Gel oder dgl. in die Poren eingebracht
und die Anticoagulationssubstanz wird daran festgehalten. Das zuletztgenannte Verfahren ergibt im allgemeinen
bessere Ergebnisse in bezug auf die Beständigkeit der Anticoagulationseigenschaften
der Substanz. Zu Beispielen für Materialien für das polymere Gel gehören synthetische wasserlösliche
Polymere, wie Polyvinylalkohol, Polyäthylenoxid, Polyäthylenglykol, Polyvinylpyrrolidon und Polyacrylsäure,
sowie natürliche hydrophile Polymere, wie Cellulosederivate, Pectin und Alginsäure.
Die Anticoagulationssubstanz kann in dem PTFE-Schlauch festgehalten
werden, indem man sie mit einer Lösung eines solchen Polymeren mischt, die Poren des porösen PTFE mit der Lösung
imprägniert und die Lösung nach einem für das jeweilige Polymere geeigneten Verfahren geliert(unter "Gelieren" ist hier
"Vernetzen" zu verstehen). Ein anderes wirksames Verfahren besteht darin, daß man die Poren des PTFE-Schlauches mit einer
Lösung eines polymeren Amins, wie Polyäthylenimin oder PoIyvinylamin,
imprägniert, das Polymere vernetzt, es quaternisiert
und das Produkt mit einer Lösung von Heparinnatrium oder dgl. in Kontakt bringt, um Heparinnatrium ionisch zu binden,

wie in der japanischen Patentanmeldung (OPl) 13 694/1979
(der Ausdruck 11OPI" steht für eine "publizierte, ungeprüfte
japanische Patentanmeldung"), die der US-PatentanmeldungNr.
921 680 entspricht, beschrieben. Es wurde bereits gefunden, daß ionisch gebundenes Heparin oder dgl. eine bessere Anticoagulationswirkung
hat als ein solches, das nach anderen Verfahren gebunden ist, und sein Effekt hält über einen längeren
Zeitraum hinweg an. Erfindungsgemäß hat sich ebenfalls
dieses Bindeverfahren als am wirksamsten erwiesen. Wenn ein solches hydrophiles Polymeres verwendet wird, um die Anticoagulationssubstanz
festzuhalten (zurückzuhalten), führt dies zur Bildung eines hydrophilen Anteils auf der Oberfläche
von PTFE, das stark wasserabstoßend ist und deshalb weist das Produkt auch einen Anticoagulationseffekt auf, der auf
eine ausgewogene Kombination von Wasserabweisung und Hydrophilizität zurückzuführen ist. Gekoppelt mit der Aktivität
der Anticoagulationssubstanz erhält man dadurch eine Gefäßprothese mit überlegenen Eigenschaften.
Eine geeignete Konzentration der polymeren Lösung, mit der der poröse PTFE-Schlauch imprägniert werden soll, beträgt
in der Regel nicht mehr als 10 % und eine geeignete Konzentration der Anticoagulationssubstanz liegt innerhalb des Bereiches
von 0,2 bis 5 % (im Falle von Heparinnatrium). Zweckmäßig wird die Imprägnierung nur an der inneren Oberfläche
des porösen PTFE-Schlauches durchgeführt, weil nur die Anticoagulationseigenschaften
der inneren Oberfläche verbessert werden können.
Die Substanz gegen die Anticoagulationssubstanz ist in dem

porösen Elastomerüberzugsanteil enthalten und dient dazu, das Bluten zu stoppen. Di "·. ."'.gensubstar:?, umfaßt eine Substanz,
die der AnticoaguL; tionssubs tanz auf der inneren Oberfläche
des Schlauches entgegenwirkt, und eine Substanz, welche
die Funktion des Anticoagulans herabsetzt oder ausschaltet.
Beispiele für geeignete Gegensubstanzen sind anorganische oder organische Coagulationssubstanzen, wie Siliciumdioxid,
Aluminiumoxid, Ruß oder Aktivkohle und antagonistisch
wirkende Agentien gegen Anticoagulationssubstanzen. Typische Beispiele für antagonistische Agentien gegenüber Heparin sind
Protamin und seine Derivate, wie Protaminsulfat und Protaminzink. Eine solche Gegensubstanz kann als Füllstoff in das
Elastomere eingearbeitet werden oder in den Poren des porösen Elastomerüberzugs vorliegen. Wenn sie mit dem Elastomeren
gemischt wird, so beträgt ihre Menge zweckmäßig bis zu 10 Gev.-%, bezogen auf das Gewicht des Elastomeren. Wenn sie
in denPoren des Elastomerüberzugs vorliegt, kann ihre Menge gering sein. Zweckmäßig wird sie in einer Menge verwendet,
die größer ist als diejenige, die erforderlich ist, um die Gesamtmenge der Anticoagulationssubstanz zunichtezumachen.
Die Coagulationssubstanz wird in die Poren des Elastomerüberzugs zweckmäßig dadurch eingebracht, daß man das poröse Elastomere
mit einer Dispersion der Coagulationssubstanz imprägniert und das imprägniert Elastomere trocknet. Es hat
sich gezeigt, daß die in den Poren des Elastomeren enthaltene Coagulationssubstanz riebt austritt. Um die Coagulationssubstanz

Ein bevorzugter Gedanke der Erfindung liegt in einer Gefäßprothese,
die enthält oder besteht aus einem porösen Schlauch oder Rohr aus Polytetrafluoräthylen,, das eine Anticoagulationssubstanz
enthält, und einem an seine äußere Oberfläche gebundenen porösen Elastomerüberzug, der eine Substanz enthält,
die der Anticoagulationssubstanz entgegenwirkt (hier auch als "Gegensubstanz" bezeichnet).
Wie vorstehend näher beschrieben, stellt die erfindungsgemäße
Gefäßprothese eine Verbesserung gegenüber konventionellen Gefäßprothesen dar, die hauptsächlich aus einem porösen PTFE-Schlauch
bestehen, und sie weist auch eine bessere Funktion sowie bessere Eigenschaften auf, wie sie bisher nicht erzielbar
waren.
Die Erfindung wurde zwar vorstehend unter Bezugnahme auf bevorzugte
Ausführungsformen näher erläutert, es ist jedoch für
den Fachmann selbstverständlich, daß sie darauf keineswegs beschränkt ist, sondern daß diese in vielfacher Hinsicht abgeändert
und modifiziert werden können, ohne daß dadurch der Rahmen der vorliegenden Erfindung verlassen wird.

Gefäßprothese nach mindestens einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß es sich bei der Gegensubstanz um einen Heparinantagonisten handelt.5. Gefäßprothese nach Anspruch 4, dadurch gekennzeichnet, daß es sich bei dem Heparinantagonisten um Protamin oder ein Derivat davon handelt.6. Gefäßprothese nach mindestens einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß der Schlauch oder das Rohr aus Polytetrafluoräthylen einen Porendurchmesser von etwa 2 bis etwa 100 um, eine Porosität von mindestens etwa 70 % und eine Wanddicke von etwa 0,3 bis etwa 1,0 mm aufweist.7. Gefäßprothese nach mindestens einem der Ansprüche 1 bis6, dadurch gekennzeichnet, daß das Elastomere ausgewählt wird aus der Gruppe Fluorkautschuk, Silikonkautschuk, Urethankautschuk, Acrylkautschuk und Naturkautschuk.8. Gefäßprothese nach Anspruch 7, dadurch gekennzeichnet, daß das Elastomere eine Porosität von etwa 50 bis etwa 90 % aufweist.9. Gefäßprothese nach einem der Ansprüche 1 bis 8, dadurch gekennzeichnet, daß der Elastomerüberzug etwa 20 bis etwa 500 um dick ist.10. Gefäßprothese nach mindestens einem der Ansprüche 1 bis030017/08379, dadurch gekennzeichnet, daß die Anticoagulationssubstanz ausgewählt wird aus der Gruppe der Polysaccharidsulfate und organischen Säuren.11. Gefäßprothese nach mindestens einem der Ansprüche 1 bis10, dadurch gekennzeichnet, daß die Anticoagulationssubstanz in dem Schlauch oder Rohr aus Polytetrafluoräthylen in einer in einem polymeren Gel dispergierten Form enthalten ist.12. Gefäßprothese nach mindestens einem der Ansprüche 2 bis11, dadurch gekennzeichnet, daß das Heparin oder die Derivate davon in einer an ein vernetztes polymeres Amin ionisch gebundenen Form in dem Schlauch oder Rohr aus Polytetrafluoräthylen enthalten ist.13. Gefäßprothese nach Anspruch 12, dadurch gekennzeichnet, daß die Anticoagulationssubstanz in einer Menge von etwa 0,2 bis etwa 5 Gew.-% in dem polymeren Gel enthalten ist.14. Gefäßprothese nach mindestens einem der Ansprüche 1 bis13, dadurch gekennzeichnet, daß die Substanz (Gegensubstanz) ausgewählt wird aus der Gruppe Siliciumdioxid, Aluminiumoxid, Ruß, Aktivkohle und der antagonistischen Mittel für Anticoagulantien.15. Gefäßprothese nach mindestens einem der Ansprüche 1 bis14, dadurch gekennzeichnet, daß die Substanz (Gegensubstanz) in dem Elastomeren in einer Menge von etwa 0,5 bis etwa 10 Gew,-%, bezogen auf das Gewicht des Elastomeren, vorliegt.030017/083716.
