Description
Die Erfindung betrifft eine Gelenkendoprothese zum Ersetzen von Knochengelenkcn, die als einstückiger
Körper aus einem flexiblen, chemisch neutralen Material hergestellt ist, mit einem verbreiterten
Mittclabschnitt und zwei sich von dem Mittciabschnitt weg nach außen erstreckenden Schaftabschnitten, die
im wesentlichen den Abmessungen der Knochenmarkkanälc der dem Gelenk benachbarten Knochen
entsprechen, und daß sich der Miitelabschniti auf beiden
Seiten der Schaftabschnittc in seitlicher Richtung nach außen erstreckt und an seiner volaren Seite eine
Aussparung aufweist, welche sich läng.^ des Mittclabschnitts
erstreckt und zwischen den beiden Schaftabschnitten ein flexibles Gelenk bildet.
Aus der US-PS 34 62 765 ist eine Gelenkendoprothese dieser Art bekannt, dia sich in der medizinischen
Praxis bereits sehr gut bewährt und zu einer erheblichen Verbesserung der Prognosen für die betreffenden
Patienten geführt hat. Sowohl die Bruchgefahr als auch die nachteiligen Wirkungen des Gewebewachstums sind
hierdurch bereits weitgehend eliminiert worden. Ferner sind die negativen Auswirkungen auf den Patienten
mininal.
Aus der DE-PS 19 60 087 ist ein künstliches Gelenk aus Kunststoff für die human- und ticrmedizinischc
Verwendung bekannt, das aus einem Mittelteil mit zwei an denselben angeformten, nach einander entgegengesetzten
Seiten verlaufenden, sich nach außen verjüngenden Zapfen zum Einstecken in die durch das Gelenk zu
verbindenden Knochen besteht, wobei der Mittelteil eine Einschnürung aufweist, die einen die Gelenkstein
bildenden Steg enthält, der beiderseits anschließende dickere Abschnitte des Mittelteils verbindet, an denen
die Zapfen angebracht sind. Dieses Gelenk vermag deshalb nicht zu befriedigen, weil es keine ausreichende
Stabilisierung des Gelenks gegen seitliches Verdrehen ermöglicht.
Die DE-OS 22 52 840 offenbart eine Gelenkendoprothese mit einem flexiblen Bereich, der durch zwei
Markkanal-Stäbe verlängert ist und einen hohlen flexiblen Bereich aufweist Auch diese Prothese besitzt
nicht die erforderliche Stabilität gegen ein seitliches Abknicken des gebeugten oder nicht gebeugten
■s Gelenks.
Die GB-PS 13 20 956 offenbart ebenfalls eine Gelenkendoprothese mit einem verjüngten Mittelabschnitt
und einer elastischen, deformierbaren Hülle, die das Eindringen des umgebenden Gewebes in das
GeJenk verhindert. Auch diese Gelenkendo-Prothese vermag bezüglich ihrer Lebensdauer und der Führung
der beiden Schaftabschnitte nicht zu befriedigen.
Schließlich sind aus Ann. rheum. Dis. (1969), 28, Supplement, Seiten 44—45, Gelenkendoprothesen der
im Oberbegriff des Hauptanspruchs definierten Gattung bekannt. Diese Prothesen vermögen insofern nicht
vollständig zu befriedigen, als ihre Biegsamkeit erleichtert, ihre Lebensdauer verlängert, ihre Stabilität erhöht
und die Führung der Schaftabschnitte verbessert werden kann.
Die der Erfindung zugrunde liegende Aufgabe wird darin gesehen, eine Gelenkendoprothese der genannten
Bauart zu schaffen, die sich noch leichter durchbiegen bzw. abknicken läßt und deren Lebensdauer noch
größer ist, ohne daß dies durch eine Einbuße an Festigkeit oder Stabilität erkauft würde; darüber hinaus
soll die Führung der beiden Schaftabschnitte bei der Bewegung relativ zueinander verbessert werden.
Di »se Aufgabe wird erfindungsgemäß dadurch gelöst, daß die Gelenkendoprothese am distalen Ende ihres
Mitielabschnitis einen verdickten und verstärkten
Abschnitt aufweist, der so ausgebildet ist, daß er sich beim Abwinkein des einen Schaftabschnitts gegenüber
dem anderen Schaftabschnitt in eine Vertiefung hinein
>5 durchbiegt, mit der die proximale mittlere Zone der volaren Seile des Mittelabschnitts versehen ist.
Da der verdickte und verstärkte Abschnitt in die Vertiefung eintreten kann, läßt sich das Gelenk leichter
durchbiegen bzw. abknicken. Infolgedessen wird die Biegebeanspruchung verringert und gleichzeitig die
Lebensdauer des Gelenks verlängert, ohne daß die Festigkeit und Stabilität des Gelenks darunter leiden
würde. Der verdickte und verstärkte Abschnitt bildet darüber hinaus beim Eintritt in die Vertiefung eine
v, zusätzliche Führung des distalen .Schaftabschnitts am
proximalen Schaflabschnitl, was noch dadurch unter stützt wird, daß die Vertiefung notwendigerweise
verdickte Endbereiche des Mittelabschnitts impliziert, so daß eine bessere Geradführung als bisher erzielbar
w ist, d. h.. der distale Schaftabschnitt kann weniger leicht
seitlich ausknicken, da nun einmal an den Finden des
Mittelabschnitts die Kompressionskräfte größer sind als im Mittelbercich. in welchem die Wandstärker geringer
ist und geringere Biegekräfte auftreten.
Die Erfindung und vorteilhafte Einzelheiten der Erfindung werden im folgenden anhand schcmatischer
Zeichnungen an einem Ausführungsbeispiel näher erläutert. Es zeigt

Die Gelenkendoprothese 10 weist einen Verbreiterten Mittelabschnitt 12 auf, von dem aus sich zwei
Schaftabschnitte 14 und 16 in entgegengesetzten Richtungen erstrecken. Der Einfachheit halber wird der
kürzere Schaftabschnitt 14 im folgenden als der »distale Schaftabschnitt oder Endabschnitt« und der längere
Schaftabschnitt 16 als der »proximale Schaftabschnitt oder Endabschnitt« bezeichnet. In Anlehnung an F i g. 2
wird die Oberseite der Gelenkendoprothese 10 als die »dorsale Seite« und die Unterseite 20 als die »volare
Seite« bezeichnet.
Es ist möglich, die Abmessungen, die genaue Form und die Winkel, unter denen sich die Schaftabschnitte
von dein verbreiterten Mittelabschnitt aus erstrecken, zu variieren, um die Gelenkendoprothese dem Teil des
Körpers anzupassen, in den sie implantiert werden soll.
Gemäß Fig. 2 bis 7 ist der verbreiterte Mittelabschnitt 12 mit einer sich quer zur Längsachse der
Gelenkendoprothese erstreckenden, kanalförmigen Aussparung 22 derart versehen, daß im Bereich dieser
Aussparung ein flexibles Gelenk gebildet ist. Die jo Aussparung 22 öffnet sich zur volaren Fläche 20 hin und
erstreckt sich von ihr aus nach oben in die Gelenkendoprothese hinein. Auf der dorsalen Seite 18
ist über der Aussparung 22 eine abgerundete, sich in der
Querrichtung erstreckende Verdickung 24 vorhanden.
Die Seitenwand 26 der Aussparung 22 eistreckt sich auf der proximalen Seite so nach unten, daß sie mit dem
proximalen Schaftabschnitt 16 eine Schnittlinie bildet. Gemäß Fig.0 hat der proximale Schafuihsdinitt 16
einen im wesentlichen rechteckigen Querschnitt, und er w
verjüngt sich von seinem dicksten Teil dort, wo er sich an die Wand 26 anschließt, über seine ganze Länge zum
freien Ende hin. Die volare Fläche 20 des Schaftabschnitts 16 ist gemäß F i g. 4 so nach außen geschweift.
daß sie dort, wo sie in die Wand 26 übergeht, an der volaren Seite eine abgerundete volare Fläche 28 bildet.
Dtr distale Schaftabschnitt 14 hat gemäß F i g. 5 einen ähnlichen, im wesentlichen rechteckigen Querschnitt
wie der proxi.nale Schaftabschniti 18. und er verdickt
sich allmählich von seinem äußeren Ende aus bis zum oo
distalen Ende des Mmelabschnitts 12. An dieser Stelle
weist die Gelenkendoprothese einen verdickten und verstärkten Abschnitt 30 auf. Der Abschnitt 30 am
distalen Ende des Mittelabschnitts 14 ist auf der volaren Seite 20 so nach oben in die Aussparung 22 hinein auf
der der Wand 26 gegenüberliegenden Seite gekrümmt, daß sie die zweite Begrenzung der Aussparung 22 bildet.
Der distale Schaftabschnitt 14 ist gemäß F i g. 4 etwas versetzt angeordnet bzw. in Richtung auf die dorsale
Seite des proximalen Schaltabschnittes 16 verlagert, um eine bessere Beherrschung der durch die Handfläche
ausgeübten Verlagerungskräfte zu ermöglichen. Der verdickte und verstärkte Abschnitt 30 hilft dazu, die
Durchbiegung auf den Bereich der Aussparung 22 zu konzentrieren und so jedem Bestreben der Gelenkendoprothese
sich am Übergang zwischen dem distalen Schaftabschnitt und dem Mittelabschnitt durchzubiegen,
entgegenzuwirken.
In der proximalen mittleren Zone 26 der volaren Seite
(Wand 26) des Mittelabschnittes 12 ist eine Vertiefung 34 ausgebildet, die dazu dient, beim Abwinkein des
Gelenks den verdickten und verstärkten Abschnitt 30 des distalen Schaftabschnitts 14 aufzunehmen. Da sich
der verdickte Abschnitt 30 in die Vertiefung 34 hinein bewegen kann, läßt sich das Gelenk somit leichter
abwinkein. Außerdem ermöglicht die Vertiefung 34 sine Umverteilung der Druckkräfte während des Abwinkeins
derart, daß das Gelenk in der richtigen Lage gehalten wird. Durch das Vorhandensein des verdickten
Abschnitts 30 des distalen Schaftabschnitts 14. der in der
beschriebenen Weise mit der Vertiefung 34 im mittleren Teil der Wand 26 zusammenarbeitet, wird die
Biegebeanspruchung verringert, außerdem verlängert sich die Lebensdauer des ständig auf Durchbiegung
beanspruchten Gelenks und gleichzeitig bleiben die Festigkeit und die Stabilität des Gelenks erhalten, wobei
der Abschnitt 30 in der Vertiefung 34 darüber hinaus eine Führung findet. Die dorsale Fläche 18 und die
volare Fläche 20 des distalen Schaftabschnitts 14 und
des proximalen Schaftabschnitts 16 können gemäß Fig. 3 und 7 mit Unregelmäßigkeiten, z.B. Grübchen
40. versehen sein, in die das benachbarte Gewebe hineinwachsen kann, um das Implanat fester zu
verankern.
Die implantierbare Gelenkendoprothese nach der Erfindung wird vorzugsweise als Formteil aus einem
flexiblen, keine physiologischen Wirkungen hervorrufenden Elastomermaterial hergestellt, beispielsweise
aus Silikonkautschuk.

Claims (1)
- Patentanspruch:Gelenkendoprothese zum Ersetzen von Knochengelenken, die als einstückiger Körper aus einem flexiblen, chemisch neutralen Material hergestellt ist, mit einem verbreiterten Mittelabschnitt und zwei sich von dem Mittelabschnitt weg nach außen erstreckenden Schaftabschnitten, die im wesentlichen den Abmessungen der Knochenmarkkanäle der dem Gelenk benachbarten Knochen entsprechen, und daß sich der Mittelabschnitt auf beiden Seiten der Schaftabschnitte in seitlicher Richtung nach außen erstreckt und an seiner volaren Seite eine *\ussparung aufweist, welche sich längs des Mittelabschnitts erstreckt und zwischen den beiden Schaftabschnitten ein flexibles Gelenk bildet, d a durch gekennzeichnet, daß die Gelenkendoprothese am distalen Ende ihres MitteUbschnitts (12) einen verdickten und verstärkten Abschnitt aufweist, der so ausgebildet ist, daß er sich beim AbwinkeJn des einen Schaftabschnittes (14) gegenüber dem anderen Schaftabschnitt (16) in eine Vertiefung (34) hinein durchbiegt, mit der die proximale mittlere Zone der volaren Seite des Mittelabschnitts versehen ist.
