Description
The invention relates to a medical instrument with a retrieval bag for recovering during surgery, in particular an endoscopic operation, from a material to be removed to be removed material, which is held by means of a bracket to a shaft.
Medical instruments for recovering body tissue to be removed during endoscopic operations, such as the gallbladder, are known in various embodiments. In a known embodiment, a bag (= "retrieval bag") is held by means of a bracket made of a resilient material at the distal end of a shaft of the instrument. In the delivery state of the instrument, the bag is rolled up and is located with the compressed bracket in a sheath surrounding the shaft. In this condition, the shaft is inserted through the trocar. When the sheath tube is retracted on the shaft towards the proximal end of the sheath, the shackle and the bag attached to it will come out of the distal end of the sheath so that the bag may unroll and the strap unfolds to form a filling opening on the bag. After filling the material to be rescued (= "resectate") in the bag this can be closed, for example, by the cladding tube is again moved distally on the shaft and the distal end of the cladding tube collapses the bag on the bracket, the bracket after and after being compressed again. Another possible closure of the bag can be achieved by means of a so-called "purse-string suture". In this case, a thread loop which is guided in a channel of the bag, which surrounds this in the region of its filling opening, is contracted, wherein the bag is torn off by the elastic strap and the filling opening of the bag is closed.

A disadvantage of these previously known rescue instruments is that trocars with a diameter of 10 mm and larger are generally needed to introduce them into the affected body cavity. Using modern endoscopic surgical techniques, surgeons try to further minimize the accesses required to insert medical instruments into the affected body cavities - and thus the diameter of the trocars used.
These modern endoscopic surgical procedures, which are also referred to as laparoscopy, thoracoscopy, SILS (Single Incision Laparoscopic Surgery) or NOTES (Natural Orifice Transluminal Endoscopic Surgery), the task by a small trocar as possible with an inner diameter of 5 mm or smaller (meaning an external diameter of the instruments of a maximum of 5 mm), to insert a retrieval bag in the surgical area, which has sufficient filling volume to Resektate such. As the gallbladder, the cecum and the like and safely enclose liquid-tight, so that these Resektate can be removed safely and without risk of contamination for adjacent tissue from the surgical field.
The object of the invention is to provide a medical instrument by which a lockable retrieval bag in the affected body cavity, z. B. the abdomen, can be introduced without causing injury to the inner wall of the body cavity opposite the trocar or to injuries of organs located in the body cavity.
According to the invention, this is achieved by a medical instrument having the features of
The free insertion length of the instrument in the affected body cavity is limited by the size or by the depth of the same and can be very small, z. B. 60 mm and smaller. By cutting the digging bag to the distal direction, it is achieved that even with minimal body cavity depth, the retrieval bag can be safely introduced and filled into the surgical field because the tissue to be rescued does not align with the instrument shaft, as is known, at right angles to the longitudinal direction Buckets is filled down, but forward (in the distal direction), and thus much less space down is needed. In addition, the distal orientation corresponds to the working direction of the other equipment used in endoscopic operations.
The inventive, distally directed rolling or folding the Bergebeutels in the instrument causes the soft, formed by the flexible material of the Bergebeutels tip as a distal boundary of the instrument prevents it, especially at a shallow depth of the body cavity into which the instrument introduced will cause unwanted injury at the trocar point opposite side, since the first 4 cm to 8 cm or more rolled or folded Bergebeutel come out of the cladding tube before the headband comes to light.
The inventive, in the direction of the longitudinal axis of the instrument, distally directed cutting of Bergebeutels is designed so that Resektate can be safely brought to the bag bottom without overcoming a bottleneck and closing the bag by means of purse string, the trapped Resektat in the longitudinal direction of the bag, elongated and slender, thereby facilitating the removal of the resected matter from the body cavity.
The shank of the medical instrument should be designed to be sufficiently rigid so that buckling of the shaft is prevented when the bag is closed by pulling on the purse-string suture. This is achieved by an appropriate choice of material for the production of the instrument shaft, preferably made of stainless steel or suitable, bio-inert, fiber-reinforced plastics.
