Description
Die Erfindung betrifft ein Bandscheibenimplantat, insbesondere
für den Lendenwirbel- und Brustwirbelbereich, gemäß dem Oberbegriff des
Anspruchs 1.
Bekanntlich übernimmt die Bandscheibe in der Wirbelsäule meh
rere zentrale Funktionen. Sie dient sowohl als Dämpfer, als
Abstandhalter als auch als Gelenk zwischen den Wirbelkörpern.
Operationen bei Schäden an Bandscheiben der Lenden- und/oder
Brustwirbelsäule werden durch deren Lage im ventralen Bereich,
d. h. auf der Vorderseite der Wirbelsäule, erschwert. Der ven
trale Zugang wird nach Möglichkeit vermieden, da er operativ
sehr aufwendig und besonders schwierig ist. Er wird insbeson
dere durch die Lage der inneren Organe erschwert und macht
meist die zusätzliche Hinzuziehung eines Bauchchirurgen erfor
derlich.
Beim dorsalen, d. h. vom Rücken ausgehenden Zugang lassen die
dorsalen Strukturen der Wirbelsäule (Wirbelbögen, kleine Wir
belgelenke und Dornfortsätze) und das Nervensystem bei Ein
griffen nur einen geringen Spielraum für operative Maßnahmen
zu. Deshalb ist eine gängige Operationsmethode - auch bei Aus
räumung der gesamten Bandscheibe - die dorsale Fixierung der
Wirbel mit einem internen Befestigungsmittel, einem sog.
"fixateur intern", d. h. einem Stab- oder Plattensystem, wel
ches über Pedikelschrauben an den Übergängen zwischen Wirbel
bogen und Wirbelkörper, den Pedikeln, befestigt wird, wie es
beispielsweise aus der DE 42 43 951 A1 bekannt ist. Dabei wird
die Funktion der Abstandshaltung von dem "fixateur intern"
übernommen, während die Dämpfungs- und Gelenkfunktion der
Bandscheibe gänzlich entfällt.
Dieses Vorgehen ist deshalb problematisch, weil die Abstützung
weit entfernt vom Ausgangsort erfolgt. Dadurch treten be
trächtliche Kräfte an dem System "Pedikel - Pedikelschraube -
Fixateurstäbe" auf, die zu Schraubenausrissen und -brüchen
führen können. Wegen dieser Nachteile und Risiken für den Pa
tienten wird statt dessen das Prinzip der Behandlung von Schä
den unmittelbar am Entstehungsort nach Möglichkeit vorgezogen.
So ist es bekannt, sogenannte "Titan-Cages" zur unmittelbaren
Abstützung der Wirbelkörper einzusetzen. Die "Titan-Cages"
sind zylinder- oder quaderförmige Käfige, die dorsal durch ei
ne Öffnung von ca. 1 cm Durchmesser längs in den Zwischenwir
belraum eingebracht werden. Anschließend werden diese Käfige
mit autologen, d. h. körpereigenen Knochenspänen aufgefüllt,
wodurch ein Zusammenwachsen der Wirbelkörper erreicht werden
soll. Für die Gewinnung der Knochenspäne, die in der Regel vom
Beckenkamm entnommen werden, ist vorher ein Sekundäreingriff
erforderlich.
Aber auch mit diesem bekannten Verfahren sind Probleme verbun
den. Der Käfig bedeckt meist nur einen geringen, und zwar den
weichen, spongiösen Teil der Wirbelkörperdeckplatten. Dadurch
kann ein Einbrechen in einen der angrenzenden Wirbelkörper er
folgen, wodurch sich die gewünschte Abstützung nicht ein
stellt. Ein weiterer Nachteil besteht darin, daß der Sekundär
eingriff einen zusätzlichen operativen Aufwand mit den ent
sprechenden Beeinträchtigungen und Nebenwirkungen erfordert.
Aus der DE 43 15 757 C1 ist ein Wirbelkörperimplantat bekannt,
das aus einem zwischen benachbarten Wirbelkörpern einsetzbaren
Abstandskörper mit oberer und unterer Deckplatte und einem
zwischen den beiden Deckplatten angeordneten federelastischen
Zwischenelement besteht. An den Außenseiten der Deckplatten
sind Wirbelkörper-Verankerungselemente vorgesehen. Das Zwi
schenelement weist eine etwa U-, S- oder W-förmig gebogene
Platte aus biokompatiblem Material auf. Inwieweit sich dieses
Implantat in der Praxis bewährt hat, ist nicht bekannt. Da das
Implantat aber stets eine ganze Bandscheibe ersetzen soll und
in einem Stück in den Zwischenwirbelraum eingebracht werden
muß, muß eine relativ weite Zugangsöffnung über den aufwendi
gen ventralen Zugang freigelegt werden.
In der EP 0 453 393 A1 wird eine Zwischenwirbelprothese be
schrieben, die aus einem elastischen, flüssigkeitsdichten
Hohlkörper aus einer langgestreckten, flexiblen Kammer be
steht, die mit einem inkompressiblen, fließfähigen Medium
füllbar ist. An dem einen Ende hat die Kammer ein um eine Ach
se drehbares Ventil, auf das die Kammer aufwickelbar ist, wäh
rend sich das andere Ende der Kammer in einem kompakten Band
fortsetzt. Das Implantat wird zunächst in langgestreckter Form
von dorsal her durch ein Rohr hindurch in den Zwischenwirbel
raum eingeführt, dann mit Hilfe eines Antriebsriemens die Kam
mer auf das Ventil aufgewickelt, das freie Ende des Bandes
durch Schweißen oder Nähen auf sich selbst fixiert und der
Riemen entfernt. Schließlich wird die Kammer mit dem Füllmedi
um gefüllt. Um das Aufwickeln und Füllen des Implantats in dem
Zwischenwirbelraum zu bewerkstelligen, ist eine ziemlich kom
plizierte, ausgeklügelte Technik erforderlich, die sehr auf
wendig und infolgedessen teuer in der Herstellung und schwie
rig in der Handhabung ist.
Der Erfindung liegt die Aufgabe zugrunde, ein gegenüber dem
dargestellten Stand der Technik in der Konstruktion, Handha
bung und Wirkungsweise wesentlich verbessertes Bandscheibenim
plantat zu entwickeln, welches durch einen sehr klein gehalte
nen Zugang in den Zwischenwirbelraum eingebracht werden und
eine sichere Abstützung der angrenzenden Wirbelkörper bei
weitgehender Erhaltung des ursprünglichen Wirbelkörperabstan
des gewährleisten kann. Ferner soll das Bandscheibenimplantat
so konstruierbar sein, daß es zusätzlich mindestens zu einem
Teil die Dämpfungs- und Gelenkfunktionen einer Bandscheibe
übernehmen kann.
Diese Aufgabe wird erfindungsgemäß gelöst durch ein Bandschei
benimplantat gemäß den kennzeichnenden Merkmalen des Anspruchs
1. Vorteilhafte Ausgestaltungen des erfindungsgemäßen Band
scheibenimplantats sind in den Unteransprüchen 2 bis 38 cha
rakterisiert.
Dementsprechend zeichnet sich das insbesondere für den Lenden
wirbel- und/oder Brustwirbelbereich geeignete und den Zwi
schenwirbelraum zu einem wesentlichen Teil ausfüllenden und
abstützenden Bandscheibenimplantat dadurch aus, daß es einen
streckbaren, d. h. auf- und zuklappbaren bzw. ab- und aufroll
baren Formkörper bildet, der massiv und so ausgestaltet ist,
daß er im zugeklappten bzw. aufgerollten Zustand die Form ei
ner Bandscheibe oder zumindest eines Teils einer Bandscheibe
nachbildet.
Damit das erfindungsgemäße Bandscheibenimplantat durch eine
möglichst klein gehaltene Öffnung in den Zwischenwirbelraum
eingeführt werden kann, ist es erforderlich, daß das Implantat
vor dem Einführen in den Zwischenwirbelraum in einer gestreck
ten Form vorliegt und erst im Zwischenwirbelraum seine vorbe
stimmte, den Zwischenwirbelraum ganz oder zumindest zu einem
wesentlichen Teil ausfüllende Gestalt annimmt.
Das erfindungsgemäße Bandscheibenimplantat ist in einer bevor
zugten Ausführungsform aus mehreren zusammenhängenden, unter
einander beweglich verbundenen massiven Segmenten zusammenge
setzt. Diese sind so ausgestaltet, daß durch ihre Gesamtheit
die Form einer Bandscheibe oder zumindest eines Teils einer
Bandscheibe nachbildbar ist. Die Anzahl der Segmente, aus de
nen das Implantat besteht, kann sehr verschieden sein und bei
spielsweise zwischen 2 und 20 liegen. Da die Eigenschaften des
Bandscheibenimplantats auch von der Anzahl der Segmente beein
flußt werden, empfiehlt es sich, die Anzahl der Segmente nicht
zu groß zu wählen. In einer besonders bevorzugten Ausführungs
form ist das erfindungsgemäße Bandscheibenimplantat aus vier
bis acht Segmenten zusammengesetzt.
Damit die Gesamtheit der Segmente zumindest angenähert die
Form und Abmessung der zu ersetzenden Bandscheibe oder eines
Teils der Bandscheibe annehmen kann, müssen die einzelnen Seg
mente eine entsprechend angepaßte Teilform aufweisen. Vorzugs
weise weist jedes der Segmente die Form eines bogen-, kreis-
oder ellipsenförmigen Ringsektors auf, da Segmente mit diesen
Formen am einfachsten und ohne Schwierigkeiten im Zwischenwir
belraum die gewünschte oder vorbestimmte Anordnung eingehen.
Aus Stabilitätsgründen müssen die Segmente massiv sein. Davon
abgesehen, kann die Konstruktion der Segmente sehr vielgestal
tig sein. Einfache Formen werden jedoch aus Wirtschaftlich
keitsgründen komplizierteren Formen vorgezogen, wobei Segmente
in blockförmiger Ausführung besonders bevorzugt sind. In einer
anderen, ebenfalls besonders bevorzugten Ausgestaltung weist
jedes einzelne Segment eine Grundplatte und eine Deckplatte
auf, die so stabil ausgeführt sind, daß sie nach dem Einbau in
den Zwischenwirbelraum den Zug- und Druckbelastungen mit Si
cherheit gewachsen sind. Zwischen der Grund- und Deckplatte
sind Zwischenelemente angeordnet, die vorzugsweise zumindest
teilweise aus elastischem Material bestehen.
In einer anderen bevorzugten Ausgestaltung der Erfindung sind
die Grund- und Deckoberflächen der Segmente bzw. die Oberflä
chen der Grund- und Deckplatten der Segmente mit Erhebungen
oder Vorsprüngen, beispielsweise mit dornartigen Fortsätzen,
versehen, durch die eine feste Verankerung des Implantats mit
den angrenzenden Wirbelkörpern bewirkt wird.
Damit das aus einer Anzahl Segmenten zusammengesetzte Band
scheibenimplantat vor dem Einführen in einen Zwischenwirbel
raum aufgeklappt oder gestreckt werden kann, sind die einzel
nen Segmente untereinander beweglich verbunden, und zwar sind
benachbarte Segmente vorzugsweise über Gelenke und/oder über
einen oder mehrere Zugdrähte miteinander verbunden.
Die Gelenke können in verschiedener, an sich bekannter Weise
ausgeführt sein, vorausgesetzt, daß sich die Segmente, die sie
miteinander beweglich verbinden, so weit aufklappen lassen,
daß der maximale Öffnungswinkel ausreicht, um die Gesamtheit
der aneinander gereihten Segmente in eine annähernd gestreckte
Lage zu bringen, so daß die Segmente z. B. durch ein gerades
Führungsrohr ohne weiteres hindurchgeführt werden können. In
einer bevorzugten Ausführungsform sind die Gelenke zwischen
den einzelnen Segmenten als mechanische Scharniere ausgebil
det, durch die die Segmente in annähernd gerader Reihe auf
klappbar und in geschlossener oder offener vorbestimmter Ge
stalt zuklappbar sind.
In einer weiteren, ebenfalls bevorzugten Ausführungsform sind
die Gelenke zwischen den einzelnen Segmenten als elastische
Werkstoffgelenke oder Bandgelenke mit elastischer Verformbar
keit ausgebildet, wobei der Öffnungswinkel wiederum so groß
gewählt wird, daß die Segmente in annähernd gerader Reihe auf
klappbar und in geschlossener oder offener vorbestimmter Ge
stalt zuklappbar sind.
Die Gestalt und Anzahl der Segmente wird vorteilhaft so ausge
wählt, daß die Gesamtheit der Segmente in zugeklappter Form
vorzugsweise ganz oder teilweise eine bogen-, kreis- oder el
lipsenförmige Gestalt aufweist, durch die die Gestalt der zu
ersetzenden Bandscheibe in ausreichender Annäherung nachgebil
det werden kann.
Die Ausführung der Gelenke zwischen den einzelnen Segmenten
als elastische Werkstoffgelenke oder Bandgelenke mit elasti
scher Verformbarkeit führt im Grenzfall zu einem einstückigen
Implantat, in dem die Segmente und Gelenke in einem einzigen
massiven Formkörper vereinigt sind. Diese, ebenfalls bevorzug
te Ausgestaltung der Erfindung zeichnet sich dadurch aus, daß
der in gleicher Weise bewegliche massive Formkörper aus einem
einzelnen bandförmigen, bogenförmig gekrümmten Segment be
steht, wobei seine Abmessungen auf die Abmessungen der zu er
setzenden Bandscheibe oder eines Teils dieser Bandscheibe ab
gestimmt sind.
In einer besonders bevorzugten Ausgestaltung ist dieser Form
körper sehr langgestreckt, spiralförmig aufrollbar und mit
einem versteifenden Profil versehen. Dabei kann das Profil des
Formkörpers beliebige Gestalt haben, wenn nur der Zweck einer
ausreichenden Versteifung des relativ langen Formkörpers er
reicht wird. Vorzugsweise ist das Profil des spiralförmig auf
gewickelten Formkörpers so ausgestaltet, daß zwei benachbarte
Teile dicht aneinander anliegen und/oder formschlüssig mitein
ander verbindbar sind, wodurch das Bandscheibenimplantat in
seiner endgültigen Form die nötige Festigkeit aufweist, um die
wichtigen Funktionen des Abstützens und Abstandhaltens in dem
Zwischenwirbelraum über sehr lange Zeiten sicher gewährleisten
zu können.
Besonders bevorzugt ist das Profil des spiralförmig aufgewic
kelten Formkörpers so ausgestaltet, daß die durch den Formkör
per gebildete Grundfläche und/oder Deckfläche Erhebungen oder
Vorsprünge, z. B. dornartige Fortsätze, aufweist, wodurch eine
erleichterte feste Verankerung des Implantats in den benach
barten Wirbelkörpern erreicht wird.
Sowohl der einstückige, bogenförmig gekrümmte Formkörper für
das bandförmig aufgebaute Bandscheibenimplantat als auch die
mehreren Segmente, aus denen das erfindungsgemäße Implantat
aufgebaut sein kann, bestehen vorzugsweise ganz oder teilweise
aus nichtrostendem Stahl, Titan oder Titanlegierungen, beson
ders bevorzugt zumindest teilweise aus Speziallegierungen mit
Formgedächtnis und/oder Pseudoelastizität, für die verschiede
ne Legierungen geeignet sind, von denen eine Speziallegierung
aus 54 bis 56 Gew.-% Nickel und 46 bis 44 Gew.-% Titan besonders
bevorzugt angewandt wird.
Eine zusätzliche Verbesserung in der praktischen Handhabung
des erfindungsgemäßen Bandscheibenimplantats beim Einführen
des Implantats in den Zwischenwirbelraum und bei der Herstel
lung und Fixierung der endgültigen Form des Implantats im Zwi
schenwirbelraum wird durch die Anwendung eines oder mehrerer
Zugdrähte erreicht, die sowohl bei dem einstückigen, aus einem
einzigen aufrollbaren Formkörper bestehenden Bandscheibenim
plantat als auch bei der mehrstückigen Ausführung durch eine
Anzahl beweglich miteinander verbundener Segmente vorgesehen
werden kann. Vorzugsweise sind der eine oder die mehreren Zug
drähte in dem aufrollbaren Formkörper seitlich untergebracht
und in den Segmenten in Kanälen, die insbesondere in den
Grund- und/oder Deckplatten untergebracht sind, oder in Aus
sparungen oder Rillen der Zwischenelemente der Segmente ange
ordnet.
Damit die beim Einbringen des Bandscheibenimplantats in den
Zwischenwirbelraum und bei der Bildung der endgültigen Form
des Implantats im Zwischenwirbelraum mit Hilfe der Zugdrähte
ausgeübten Zugkräfte auf den Formkörper bzw. die Segmente ein
wirken können, ist in einer bevorzugten Ausgestaltung der Er
findung vorgesehen, daß der eine oder die mehreren Zugdrähte
an der vorderen Stirnseite des ersten Segmentes befestigbar
sind. In einer anderen, ebenfalls bevorzugten Ausführungsform
sind die freien vorderen Enden des einen oder der mehreren
Zugdrähte nach Austritt aus dem vordersten (ersten) Segment zu
den freien hinteren Enden des einen oder der mehreren Zugdräh
te zurückführbar. Dadurch ergibt sich die Möglichkeit, das
Bandscheibenimplantat während des Einbringens in den Zwischen
wirbelraum und bei der Bildung der endgültigen zugeklappten
Form durch Ausüben von Zugkräften je nach Bedarf an einem Ende
oder an beiden Enden der Zugdrähte kontrolliert und regulie
rend zu führen.
Die Gelenke zwischen den Segmenten und die Zugdrähte können
aus jedem genügend festen, körperverträglichen Material beste
hen, z. B. aus nichtrostenden Stählen, Titan oder Titanlegie
rungen oder aus körperfreundlichen Kunststoffen wie Polyethy
len oder Polyurethan. Vorzugsweise bestehen die Gelenke
und/oder die Zugdrähte ganz oder teilweise aus einer Spezial
legierung mit Formgedächtnis und/oder Pseudoelastizität, be
sonders bevorzugt aus einer Speziallegierung aus 54 bis 56
Gew.-% Nickel und 46 bis 44 Gew.-% Titan.
Damit die mehreren Segmente eines Bandscheibenimplantats nach
Ausbildung der endgültigen Form im Zwischenwirbelraum mög
lichst exakt und fest in der vorgegebenen Anordnung verblei
ben, ist weiterhin bevorzugt vorgesehen, daß die Stirnseiten
je zweier benachbarter Segmente durch korrespondierende Vor
sprünge und Vertiefungen formschlüssig verbindbar sind.
Zum Einbringen eines aus einem oder mehreren Segmenten beste
henden Bandscheibenimplantats der vorstehend beschriebenen Art
in einen Zwischenwirbelraum, aus dem vorher die beschädigte
Bandscheibe ganz oder zum Teil entfernt wurde, kann das Band
scheibenimplantat in aufgeklappter oder gestreckter Form an
dem einen Ende einer Führungshülse (Trokar) eingeführt und
durch die Führungshülse hindurch über das andere, abgeschrägte
Ende der Führungshülse, das an dem Zwischenwirbelraum mög
lichst nahe positioniert ist, durch Zug- und/oder Druckkräfte
in den Zwischenwirbelraum befördert werden. Dort wird es in
eine vorbestimmte geschlossene oder offene Form gebracht und
füllt dann den Zwischenwirbelraum ganz oder teilweise aus. Da
bei wird vorzugsweise das aus mehreren zusammenhängenden be
weglichen Formkörpern in Gestalt von Segmenten bestehende
Bandscheibenimplantat in seiner Gesamtheit in dem Zwischenwir
belraum bogen-, kreis- oder ellipsenförmig und das aus einem
einzigen, aufrollbaren Formkörper bestehende Bandscheibenim
plantat in dem Zwischenwirbelraum bogen- bzw. spiralförmig an
geordnet.
Die Führungshülse zum Einführen des Bandscheibenimplantats in
den Zwischenwirbelraum kann die Implantatkonstruktion im auf
geklappten bzw. gestreckten Zustand aufnehmen. Damit das Ein
führen und Anordnen des Bandscheibenimplantats rasch und rei
bungslos erfolgen kann, ist in einer bevorzugten Ausgestaltung
vorgesehen, daß mit Hilfe von einem oder mehreren Zugdrähten,
die in dem Formkörper bzw. den mehreren Segmenten des Band
scheibenimplantats angeordnet sind, eine Zugkraft auf den
Formkörper oder die Segmente ausgeübt wird, durch die das
Bandscheibenimplantat in die gewünschte Gestalt oder Anordnung
gebracht wird.
Die Zugdrähte dienen zum kontrollierten Einführen des Band
scheibenimplantats in den Zwischenwirbelraum, aber auch zur
Sicherung der endgültigen Form des Implantates. Zu diesem
Zweck werden die Zugdrähte vorzugsweise nach Erreichen der ge
wünschten Gestalt oder Anordnung des Bandscheibenimplantats
und nach Entfernen der Führungshülse an ihren Enden dauerhaft
fixiert. In einer anderen bevorzugten Ausgestaltung der Erfin
dung können die freien vorderen Enden der Zugdrähte nach dem
Austreten aus dem vorderen Teil des Bandscheibenimplantats
durch die Führungshülse zurückgeführt werden, und es wird wäh
rend oder nach dem Einbringen des Bandscheibenimplantats in
den Zwischenwirbelraum auf die freien vorderen Enden und/oder
die freien hinteren Enden der Zugdrähte eine Zugkraft ausge
übt, durch die das Bandscheibenimplantat in die gewünschte Ge
stalt oder Anordnung gebracht wird.
Damit die im Zwischenwirbelraum gebildete Gestalt oder Anord
nung des Bandscheibenimplantats auch unter Belastung möglichst
lange Zeit erhalten bleibt, werden vorzugsweise die vorderen
und/oder hinteren freien Zugdrahtenden nach Erreichen der ge
wünschten Gestalt oder Anordnung des Bandscheibenimplantats
und nach Entfernen der Führungshülse dauerhaft fixiert.
In einer besonders bevorzugten Ausgestaltung der Erfindung ist
vorgesehen, daß eine pseudoelastische Formgedächtnislegierung,
der die gewünschte Gestalt oder Anordnung vorher eingeprägt
Wurde, als Implantatwerkstoff ganz oder teilweise für das
Bandscheibenimplantat eingesetzt wird. Hierfür eignet sich
insbesondere die oben erwähnte Speziallegierung aus 54 bis 56
Gew.-% Nickel und 46 bis 44 Gew.-% Titan.
Die Abmessungen des erfindungsgemäßen Bandscheibenimplantats
können ohne weiteres auf die für eine dorsale oder minimalin
vasive ventrale Implantation mögliche Öffnung des Zwischenwir
belraumes und auf den ursprünglichen Wirbelkörperabstand abge
stimmt werden. Ein weiterer Vorteil der Erfindung besteht dar
in, daß die mehreren Segmente oder der eine bogenförmig ge
krümmte Formkörper des Bandscheibenimplantats so ausgelegt
werden können, daß sie neben einer sicheren Abstützung weitge
hend auch die Dämpfungs- und Gelenkfunktionen einer intakten
Bandscheibe übernehmen.
Eine Verankerung des Bandscheibenimplantats mit den angrenzen
den Wirbelkörpern wird durch eine entsprechende Oberflächenge
staltung der Grund- und Deckoberflächen durch Erhebungen oder
Vorsprünge gewährleistet. Ein zusätzliches Repositionsinstru
mentarium kann erforderlich werden, um die betreffenden Wir
belkörper zum Zweck der Implantation auf den nötigen Abstand
einzustellen, der ein problemloses Einbringen des Implantates
ermöglicht. Ein weiterer Vorteil der Erfindung besteht darin,
daß im Regelfall unter günstigen Voraussetzungen auf einen
"fixateur intern" verzichtet werden kann.
Nachfolgend werden bevorzugte Ausführungsformen der Erfindung
anhand der beigefügten Zeichnungen näher erläutert. Es zeigen:
Fig. 1 eine perspektivische Gesamtansicht eines erfindungsge
mäß ausgestalteten, aus mehreren massiven Segmenten
bestehenden Bandscheibenimplantats zwischen zwei Wir
belkörpern;
Fig. 2 eine perspektivische Darstellung von zwei benachbarten
Segmenten in aufgeklappter Stellung;
Fig. 3 eine perspektivische Darstellung der beiden in Fig. 2
gezeigten Segmente in zugeklappter Stellung;

In Fig. 1 ist ein einen Zwischenwirbelraum ausfüllendes, aus
acht Segmenten 2, 3, 4.... einer speziellen Ausführungsform be
stehendes erfindungsgemäßes Bandscheibenimplantat 1 in seiner
endgültigen Position und Gestalt zwischen zwei den Zwischen
wirbelraum begrenzenden Wirbelkörpern 20 dargestellt. Das Im
plantat 1 ist so auf die Abmessungen der benachbarten Wirbel
körper 20 abgestimmt, daß es den Zwischenwirbelraum vollstän
dig ausfüllt und abstützt, wobei der äußere Rand der Segmente
2, 3, 4.... auf den kortikalen Rändern 21 der Wirbelkörper 20
aufliegt.
In Fig. 2 sind zwei Segmente 2 und 3 als Teil eines aus einer
Anzahl von untereinander beweglich verbundenen Segmenten be
stehenden Bandscheibenimplantats 1 in aufgeklappter Stellung
gezeigt. Jedes der Segmente 2 und 3 ist in Form eines kreis
förmigen Ringsektors ausgestaltet und besteht aus einer Grund
platte 22, einer Deckplatte 23 sowie zwischen der Grund- und
Deckplatte angeordneten Zwischenelementen 8, die zumindest
teilweise aus elastischem Material bestehen. Die Zwischenele
mente 8 haben in der Fig. 2 eine W-förmige Gestalt, durch die
sie den Segmenten 2, 3 bei Belastung eine gewisse Federelasti
zität verleihen.
In der in Fig. 2 gezeigten Ausführungsform sind die Segmente
2, 3 über Gelenke 5 und über Zugdrähte 13 miteinander beweg
lich verbunden. Die Gelenke 5 können, wie in Fig. 2 darge
stellt, so weit geöffnet werden, daß die Segmente 2, 3 in an
nähernd gerader Reihe aufklappbar sind. Wenn die Gelenke 5 ge
schlossen sind, bilden die Segmente 2, 3, 4... in ihrer Ge
samtheit ein kreisförmiges Bandscheibenimplantat 1, wie dies
in Fig. 6 schematisch gezeigt ist.
Die ein oder mehreren Zugdrähte 13, 13.1 können, wie der Fig.
2 entnehmbar ist, in Kanälen 12 durch die Segmente 2, 3, 4....
hindurchgeführt werden, wobei die Kanäle 12 zweckmäßig in den
Grund- und/oder Deckplatten 22, 23 untergebracht sind, oder in
Aussparungen oder Rillen 19, die durch W-, S-, M- oder U-
förmig ausgestaltete Zwischenelemente 8, 9 gebildet sind, an
geordnet sein. In Fig. 2 sind einfachheitshalber sowohl zwei
in Kanälen 12 geführte Zugdrähte 13 als auch ein in Aussparun
gen oder Rillen 19 liegender Zugdraht 13.1 dargestellt. In der
praktischen Durchführung kann man sich auf eine dieser Ausfüh
rungsformen mit einem oder mehreren Zugdrähten 13, 13.1 be
schränken, die für sich allein, gegebenenfalls im Zusammenwir
ken mit den Gelenken 5, bereits die erforderliche Beweglich
keit der einzelnen Segmente 2, 3, 4.... gewährleistet und beim
gewünschten Schließen oder Zuklappen der einzelnen Segmente 2,
3, 4.... im Zwischenwirbelraum eine ausreichende Zugkraft auf
diese Segmente ausübt, durch die diese in eine Lage gebracht
werden, die der endgültigen Gestalt des Bandscheibenimplantats
1 in dem Zwischenwirbelraum entspricht.
Die Zugdrähte sind vorzugsweise an der vorderen Stirnseite 24
des an der Spitze der mehreren Segmente 2, 3, 4.... befindli
chen ersten Segmentes 14 (siehe Fig. 6) durch z. B. plombenar
tige Verschlüsse befestigt.
Damit eine sichere Verankerung des Implantats 1 an den benach
barten Oberflächen der Wirbelkörper 20 gefördert wird, sind
die Oberflächen der Grund- und Deckplatten 22, 23 der Segmente
2, 3, 4.... mit Erhebungen oder Vorsprüngen 6 versehen, von
denen in Fig. 2 nur diejenigen auf den Deckplatten 23 zu sehen
sind.
Wie oben bereits erwähnt, können die Segmente 2, 3, 4.... ver
schieden ausgestaltet sein, wobei in Fig. 2 in einer bevorzug
ten Ausführungsform zwei Segmente 2, 3 mit federelastisch wir
kenden, im Querschnitt W-förmigen Zwischenelementen 8 gezeigt
sind. In Fig. 3 sind zwei solche Segmente 2, 3 im zugeklappten
Zustand dargestellt.
In Fig. 4 ist eine andere, M-förmige, federelastisch wirkende
Ausgestaltung der Zwischenelemente 9 an zwei Segmenten 2, 3
als Teil einer Anzahl von Segmenten 2, 3, 4.... gezeigt, die
ebenfalls im zugeklappten Zustand dargestellt sind.
Die einzelnen Segmente 2, 3, 4.... können auch in einer massi
ven blockförmigen Ausführung, wie in Fig. 5 gezeigt, vorlie
gen. Das in Fig. 5 abgebildete einzelne Segment 2 weist die
Form eines bogenförmigen Ringsektors auf, mit einer Grundober
fläche 22 (die in Fig. 5 verdeckt ist) und einer Deckoberflä
che 23, auf denen in Abständen Erhebungen oder Vorsprünge 6
angeordnet sind (von denen nur die auf der Deckoberfläche 23
sichtbar sind), die der besseren Verankerung des Implantats 1
mit den angrenzenden Wirbelkörpern 20 dienen. Die Zugdrähte 13
treten an der hinteren Stirnseite 25 in das Segment 2 ein,
werden über die Kanäle 12 durch das Segment 2 hindurchgeführt
und treten an der vorderen Stirnseite 24 aus dem Segment 2
aus. Mit Hilfe der Gelenke 5, von denen nur dasjenige an der
vorderen Stirnseite 24 in Fig. 5 sichtbar ist, ist das Segment
2 beweglich mit den benachbarten, hier nicht dargestellten
Segmenten verbunden, die in der gleichen Weise ausgestaltet
sind.
Das aus einer Anzahl von Segmenten 2, 3, 4... bestehende Band
scheibenimplantat 1 wird, wie in Fig. 6 schematisch gezeigt,
in aufgeklappter Form an dem einen Ende einer Führungshülse 10
eingeführt und durch die Führungshülse 10 hindurch über das
andere, durch Abschrägung an die Raumverhältnisse am Rand des
Zwischenwirbelraums angepaßte Ende 11 der Führungshülse 10 an
den Zwischenwirbelraum herangeführt. Durch Zug- und/oder
Druckkräfte, die mit Hilfe der am vordersten (ersten) Segment
14 mittels Befestigungen, z. B. Plomben, befestigten Zugdrähte
13 auf die Segmente übertragen werden, wird das Bandschei
benimplantat 1 über die Oberfläche des unter dem Zwischenwir
belraum befindlichen Wirbelkörpers 20 in den Zwischenwirbel
raum befördert, wo es in eine vorbestimmte geschlossene oder
offene Form, z. B. eine vollständige oder unvollständige
Kreisform, gebracht wird und dabei den Zwischenwirbelraum ganz
oder teilweise ausfüllt. In Fig. 6 ist das Stadium darge
stellt, bei dem das Einbringen der Segmente in den Zwischen
wirbelraum noch nicht ganz beendet ist. Während sich die er
sten Segmente bereits zu der vorbestimmten endgültigen Ring
form zusammenschließen, treten aus dem abgeschrägten Ende 11
der Führungshülse 10 noch die letzten Segmente in aufgeklapp
ter Form in den Zwischenwirbelraum ein. Die Segmente sind
hierbei durch die Gelenke 5 in horizontaler Ebene beweglich
miteinander verbunden und werden durch die Zugdrähte 13 in ih
re gewünschte Lage innerhalb des Zwischenwirbelraums gebracht.
Aus Gründen der besseren Handhabbarkeit werden die freien vor
deren Enden 16 der Zugdrähte 13, nachdem sie aus der vorderen
Stirnseite 24 des ersten Segmentes 14 herausgetreten sind,
durch die Führungshülse 10 nach außen zurückgeführt, so daß
sie, gegebenenfalls gemeinsam mit den hinteren Enden 17 der
Zugdrähte 13, zum Führen der Segmente 2, 3, 4.... und dem
Festlegen ihrer endgültig erreichten Gestalt oder Anordnung im
Zwischenwirbelraum benutzt werden können.
Nachdem sämtliche Segmente 2, 3, 4.... in den Zwischenwirbel
raum eingeführt worden sind und die endgültige Gestalt oder
Anordnung des Bandscheibenimplantats 1 erreicht ist, wird die
Führungshülse 10 entfernt und die endgültige Gestalt oder An
ordnung des Implantats 1 durch feste Fixierung der Zugdrähte
13 an ihren freien Enden 16, 17, z. B. durch Verknoten oder
Verdrehen dieser Enden, abgesichert.
Eine zusätzliche Stabilisierung und Sicherung des Bandschei
benimplantats 1 in seiner endgültigen Gestalt oder Anordnung
kann dadurch erfolgen, daß die Stirnseiten 24, 25 je zweier
benachbarter Segmente 2, 3, 4.... durch in den Figuren nicht
dargestellte korrespondierende Vorsprünge und Vertiefungen
formschlüssig miteinander verbindbar sind.
Eine andere, einstückige Ausgestaltung des erfindungsgemäßen
Bandscheibenimplantats 1 ist in den Fig. 7 und 8 darge
stellt. Das Implantat 1 besteht hier aus einem mit einem ver
steifenden Profil versehenen, bandförmigen, aufrollbaren Form
körper 30, wobei in den Fig. 7 und 8 die endgültige Gestalt
des Implantats 1 in Spiralform gezeigt ist, die das Implantat
1 nach dem Einbringen in den Zwischenwirbelraum aufweist. Das
Profil des Formkörpers 30 ist so ausgewählt, daß zwei benach
barte Teile des spiralförmig aufgewickelten Formkörpers 30
dicht aneinander anliegen und formschlüssig miteinander ver
bunden sind, wie insbesondere aus der Querschnittsansicht der
Fig. 8 zu ersehen ist. Ferner zeigen die Fig. 7 und 8 die
durch den spiralförmig aufgewickelten Formkörper 30 gebildete
Grundfläche 32 und Deckfläche 33, die zur Verbesserung der
Verzahnung mit den benachbarten Wirbelkörpern Erhebungen oder
Vorsprünge 31 aufweisen.
Die Einbringung des bandförmigen einstückigen Formkörpers 30
in den Zwischenwirbelraum erfolgt in der gleichen Weise wie
oben angegeben. Der Vorgang ist in Fig. 9 schematisch unter
Weglassung der Wirbelkörper gezeigt. Das aus dem Formkörper 30
bestehende Bandscheibenimplantat 1 wird in gestreckter Form in
die Führungshülse 10 eingeführt und durch die Führungshülse 10
hindurch über das abgeschrägte Ende 11 der Führungshülse 10 in
den Zwischenwirbelraum eingebracht, wo es sich in dem Maße, in
dem der Formkörper 30 durch die Führungshülse 10 in den Zwi
schenwirbelraum eindringt, spiralförmig aufrollt, wie dies in
Fig. 9 dargestellt ist. Hierbei können in ähnlicher Weise, wie
bei dem aus Segmenten bestehenden Bandscheibenimplantat be
schrieben, in den Fig. 7 bis 9 nicht gezeigte Zugdrähte 13
in dem Formkörper untergebracht sein, die während und nach dem
Einbringen des Implantats 1 in den Zwischenwirbelraum durch
Zug- und/oder Druckkräfte mitwirken, das Bandscheibenimplantat
in die gewünschte Gestalt und Position zu bringen. Eine weite
re Möglichkeit ergibt sich daraus, daß der Formkörper 30
und/oder die Zugdrähte 13 ganz oder teilweise aus einer Spezi
allegierung mit Formgedächtnis und/oder Pseudoelastizität be
stehen, die durch geringes Erwärmen das Bandscheibenimplantat
in die vorbestimmte spiralförmige Gestalt bringen.
Claims (38)
1. Bandscheibenimplantat, insbesondere für den Lendenwirbel-
und/oder Brustwirbelbereich, welches den Zwischenwirbelraum
zu einem wesentlichen Teil ausfüllt und abstützt, dadurch
gekennzeichnet, daß es einen streckbaren, auf- und zu
klappbaren oder ab- und aufrollbaren Formkörper bildet, der
massiv und so ausgestaltet ist, daß er im zugeklappten bzw.
aufgerollten Zustand die Form einer Bandscheibe oder zumin
dest eines Teils einer Bandscheibe nachbildet.
2. Bandscheibenimplantat nach Anspruch 1, dadurch gekennzeich
net, daß der Formkörper aus einer Anzahl einzelner, unter
einander beweglich verbundener Segmente (2, 3, 4...) besteht,
die so ausgestaltet sind, daß durch ihre Gesamtheit die
Form einer Bandscheibe oder zumindest eines Teils einer
Bandscheibe nachbildbar ist.
3. Bandscheibenimplantat nach Anspruch 2, dadurch gekennzeich
net, daß es aus vier bis acht Segmenten (2, 3, 4, ...) zusam
mengesetzt ist.
4. Bandscheibenimplantat nach Anspruch 2 oder 3, dadurch ge
kennzeichnet, daß jedes der Segmente (2, 3, 4...) die Form
eines bogen-, kreis- oder ellipsenförmigen Ringsektors auf
weist.
5. Bandscheibenimplantat nach einem der Ansprüche 2 bis 4, da
durch gekennzeichnet, daß die Segmente (2, 3, 4...) blockför
mig ausgeführt sind.
6. Bandscheibenimplantat nach einem der Ansprüche 2 bis 4, da
durch gekennzeichnet, daß jedes einzelne Segment (2, 3, 4...)
eine Grundplatte (22) und eine Deckplatte (23) aufweist mit
zwischen der Grund- und Deckplatte angeordneten Zwi
schenelementen (8, 9).
7. Bandscheibenimplantat nach Anspruch 6, dadurch gekennzeich
net, daß die Zwischenelemente (8, 9) zumindest teilweise aus
elastischem Material bestehen.
8. Bandscheibenimplantat nach einem der Ansprüche 2 bis 7, da
durch gekennzeichnet, daß die Grund- und Deckoberflächen
der Segmente (2, 3, 4...) oder die Oberflächen der Grund- und
Deckplatten (22, 23) der Segmente (2, 3, 4...) mit Erhebungen
oder Vorsprüngen (6) versehen sind.
9. Bandscheibenimplantat nach einem der Ansprüche 2 bis 8, da
durch gekennzeichnet, daß die Segmente (2, 3, 4...) ganz oder
teilweise aus nichtrostendem Stahl, Titan oder Titanlegie
rungen bestehen.
10. Bandscheibenimplantat nach einem der Ansprüche 2 bis 8, da
durch gekennzeichnet, daß die Segmente (2, 3, 4...) zumindest
teilweise aus Speziallegierungen mit Formgedächtnis
und/oder Pseudoelastizität bestehen.
11. Bandscheibenimplantat nach Anspruch 10, dadurch gekenn
zeichnet, daß die Speziallegierung aus 54 bis 56 Gew.-% Nic
kel und 46 bis 44 Gew.-% Titan zusammengesetzt ist.
12. Bandscheibenimplantat nach einem der Ansprüche 2 bis 11,
dadurch gekennzeichnet, daß benachbarte Segmente (2, 3) über
Gelenke (5) und/oder über einen oder mehrere Zugdrähte (13)
miteinander verbunden sind.
13. Bandscheibenimplantat nach Anspruch 12, dadurch gekenn
zeichnet, daß die Gelenke (5) und/oder die Zugdrähte (13)
ganz oder teilweise aus einer Speziallegierung mit Formge
dächtnis und/oder Pseudoelastizität bestehen.
14. Bandscheibenimplantat nach Anspruch 13, dadurch gekenn
zeichnet, daß die Speziallegierung aus 54 bis 56 Gew.-% Nic
kel und 46 bis 44 Gew.-% Titan zusammengesetzt ist.
15. Bandscheibenimplantat nach einem der Ansprüche 2 bis 14,
dadurch gekennzeichnet, daß die Gelenke (5) zwischen den
einzelnen Segmenten (2, 3, 4...) als mechanische Scharniere
ausgebildet sind, durch die die Segmente (2, 3, 4...) in an
nähernd gerader Reihe aufklappbar und in geschlossener oder
offener vorbestimmter Gestalt zuklappbar sind.
16. Bandscheibenimplantat nach einem der Ansprüche 2 bis 14,
dadurch gekennzeichnet, daß die Gelenke (5) zwischen den
einzelnen Segmenten (2, 3, 4...)als elastische Werkstoffge
lenke oder Bandgelenke mit elastischer Verformbarkeit aus
gebildet sind, durch die die Segmente (2, 3, 4...) in annä
hernd gerader Reihe aufklappbar und in geschlossener oder
offener vorbestimmter Gestalt zuklappbar sind.
17. Bandscheibenimplantat nach einem der Ansprüche 15 oder 16,
dadurch gekennzeichnet, daß die Gesamtheit der Segmente
(2, 3, 4...) in zugeklappter Form ganz oder teilweise eine
bogen-, kreis- oder ellipsenförmige Gestalt aufweist.
18. Bandscheibenimplantat nach einem der Ansprüche 2 bis 17,
dadurch gekennzeichnet, daß der eine oder die mehreren Zug
drähte (13) in Kanälen (12) in den Segmenten (2, 3, 4...),
insbesondere in den Grund- und/oder Deckplatten (22, 23)
oder in Aussparungen oder Rillen (19) der Zwischenelemente
(8, 9) der Segmente (2, 3, 4...), angeordnet sind.
19. Bandscheibenimplantat nach einem der Ansprüche 12 oder 18,
dadurch gekennzeichnet, daß der eine oder die mehreren Zug
drähte (13) an der vorderen Stirnseite (24) des ersten Seg
mentes (14) befestigbar sind.
20. Bandscheibenimplantat nach einem der Ansprüche 12 oder 18,
dadurch gekennzeichnet, daß die freien vorderen Enden (16)
des einen oder der mehreren Zugdrähte (13) nach Austritt
aus dem ersten Segment (14) zu den freien hinteren Enden
(17) des einen oder der mehreren Zugdrähte (13) zurückführ
bar sind.
21. Bandscheibenimplantat nach einem der Ansprüche 2 bis 20,
dadurch gekennzeichnet, daß die Stirnseiten (24, 25) je
zweier benachbarter Segmente (2, 3, 4...) durch korrespondie
rende Vorsprünge und Vertiefungen formschlüssig verbindbar
sind.
22. Bandscheibenimplantat nach Anspruch 1, dadurch gekennzeich
net, daß der in gleicher Weise bewegliche massive Formkör
per (30) aus einem einzelnen bandförmigen, bogenförmig ge
krümmten Segment besteht, wobei seine Abmessungen auf die
Abmessungen der zu ersetzenden Bandscheibe oder eines Teils
dieser Bandscheibe abgestimmt sind.
23. Bandscheibenimplantat nach Anspruch 22, dadurch gekenn
zeichnet, daß der Formkörper (30) spiralförmig ausgeführt
und/oder mit einem versteifenden Profil versehen ist.
24. Bandscheibenimplantat nach Anspruch 23, dadurch gekenn
zeichnet, daß das Profil des Formkörpers (30) so ausgestal
tet ist, daß zwei benachbarte Teile des spiralförmig aufge
wickelten Formkörpers (30) dicht aneinander anliegen
und/oder formschlüssig miteinander verbindbar sind.
25. Bandscheibenimplantat nach einem der Ansprüche 23 oder 24,
dadurch gekennzeichnet, daß das Profil des Formkörpers (30)
so ausgestaltet ist, daß die durch den spiralförmig aufge
wickelten Formkörper (30) gebildete Grundfläche (32)
und/oder Deckfläche (33) Erhebungen oder Vorsprünge (31)
aufweist.
26. Bandscheibenimplantat nach einem der Ansprüche 22 bis 25,
dadurch gekennzeichnet, daß der Formkörper (30) mit einem
oder mehreren, am vorderen Ende des Formkörpers (30) befe
stigbaren Zugdrähten (13) versehen ist.
27. Bandscheibenimplantat nach einem der Ansprüche 22 bis 26,
dadurch gekennzeichnet, daß die freien vorderen Enden (16)
des einen oder der mehreren Zugdrähte (13) nach Austritt
aus dem vorderen Ende des Formkörpers (30) zu den freien
hinteren Enden (17) des einen oder der mehreren Zugdrähte
(13) zurückführbar sind.
28. Bandscheibenimplantat nach einem der Ansprüche 22 bis 27,
dadurch gekennzeichnet, daß der Formkörper (30) ganz oder
teilweise aus nichtrostendem Stahl, Titan oder Titanlegie
rungen besteht.
29. Bandscheibenimplantat nach einem der Ansprüche 22 bis 28,
dadurch gekennzeichnet, daß der Formkörper (30) und/oder
die Zugdrähte (13) ganz oder teilweise aus einer Spezialle
gierung mit Formgedächtnis und/oder Pseudoelastizität be
stehen.
30. Bandscheibenimplantat nach Anspruch 29, dadurch gekenn
zeichnet, daß die Speziallegierung aus 54 bis 56 Gew.-% Nic
kel und 46 bis 44 Gew.-% Titan zusammengesetzt ist.
31. Bandscheibenimplantat nach einem oder mehreren der Ansprü
che 1 bis 30, dadurch gekennzeichnet, daß es in aufgeklapp
ter oder gestreckter Form an dem einen Ende einer Führungs
hülse (10) eingeführt und durch die Führungshülse (10) hin
durch über das andere, abgeschrägte Ende (11) der Führungs
hülse (10), das an dem Zwischenwirbelraum positioniert ist,
durch Zug- und/oder Druckkräfte in den Zwischenwirbelraum
befördert werden kann, wo es in eine vorbestimmte geschlos
sene oder offene Form gebracht werden kann und dabei den
Zwischenwirbelraum ganz oder teilweise ausfüllt.
32. Bandscheibenimplantat nach Anspruch 31, dadurch gekenn
zeichnet, daß das aus mehreren zusammenhängenden bewegli
chen Segmenten (2, 3, 4...) bestehende Bandscheibenimplantat
(1) in seiner Gesamtheit in dem Zwischenwirbelraum bogen-,
kreis- oder ellipsenförmig angeordnet werden kann.
33. Bandscheibenimplantat nach Anspruch 31, dadurch gekenn
zeichnet, daß das aus einem einzelnen Segment (30) beste
hende Bandscheibenimplantat (1) in dem Zwischenwirbelraum
bogen- oder spiralförmig angeordnet werden kann.
34. Bandscheibenimplantat nach einem der Ansprüche 31 bis 33,
dadurch gekennzeichnet, daß mit Hilfe von einem oder mehre
ren Zugdrähten (13), die in dem einen (30) oder mehreren
Segmenten (2, 3, 4...) des Bandscheibenimplantats (1) ange
ordnet sind, eine Zugkraft auf den Formkörper (30) oder die
Segmente (2, 3, 4...) ausgeübt werden kann, durch die das
Bandscheibenimplantat (1) in die gewünschte Gestalt oder
Anordnung gebracht wird.
35. Bandscheibenimplantat nach Anspruch 34, dadurch gekenn
zeichnet, daß die Zugdrähte (13) nach Erreichen der ge
wünschten Gestalt oder Anordnung des Bandscheibenimplantats
(1) und nach Entfernen der Führungshülse (10) an ihren En
den (16, 17) dauerhaft fixiert werden können.
36. Bandscheibenimplantat nach einem der Ansprüche 34 oder 35,
dadurch gekennzeichnet, daß die freien vorderen Enden (16)
der Zugdrähte (13) nach dem Austreten aus dem vorderen Teil
des Bandscheibenimplantats (1) durch die Führungshülse (10)
zurückgeführt werden können, und daß während oder nach dem
Einbringen des Bandscheibenimplantats (1) in den Zwischen
wirbelraum auf die freien vorderen Enden (16) und/oder die
freien hinteren Enden (17) der Zugdrähte (13) eine Zugkraft
ausgeübt werden kann, durch die das Bandscheibenimplantat
(1) in die gewünschte Gestalt oder Anordnung gebracht wird.
37. Bandscheibenimplantat nach Anspruch 36, dadurch gekenn
zeichnet, daß nach Erreichen der gewünschten Gestalt oder
Anordnung des Bandscheibenimplantats (1) im Zwischenwirbel
raum die Zugdrahtenden (16) und/oder (17) dauerhaft fixiert
werden können.
38. Bandscheibenimplantat nach einem der Ansprüche 31 bis 37,
dadurch gekennzeichnet, daß eine pseudoelastische Formge
dächtnislegierung, der die gewünschte Gestalt oder Anord
nung vorher eingeprägt wurde, als Implantatwerkstoff ganz
oder teilweise für das Bandscheibenimplantat (1) eingesetzt
werden kann.
