Description

The invention relates to a surgical instrument a mouth of two interacting jaw parts, whereby at least one jaw part releasably connected to an inner tube the and this inner tube slidable in an outer tube is arranged, and wherein the jaw part by one with the Outer tube connected axis pin rotates.

Such surgical instruments are diverse Form and design known and on the market. Example they become wise in endoscopy and minimal invasion active surgery. They serve the most varied surgical intervention in human or animal Body, which is why the jaw parts have a wide variety of shapes design according to their purpose. There are shearing, pinching, pinching, cutting, etc. mouth Parts known, can also be shaped accordingly The spoon-like jaw parts are also taken from tissue samples will. The present invention includes all conceivable and known configurations of the jaw parts.

In most cases, the known jaw parts have one articulated connection with the inner tube, whereby by this articulated connection opening and closing the Mouth parts can take place. This has a disadvantageous effect from that these hinge parts with the individual hinge are very difficult to clean. Even if that surgical instrument, as is common today, in his Individual parts can be disassembled, just make them These joint parts represent an area that can only be expenses properly cleaned and sterilized can be. With today's hygienic requirements This is a very significant disadvantage.

The present invention is based on the object to create surgical instrument which is easier to is clean, but at the same time also the opening and Closing movement of the jaw parts is improved. Further this instrument is said to be in a second part of the Erfin tion can also be supplied with electrical current, whereby simple and good insulation of the live parts takes place.

To solve the problem that the jaw part with a Driver engages in a slot in the inner tube and is loosely guided in this slot.

A first essential idea of the present invention is thus that known from the prior art Connection and movement joints and joint parts are eliminated len, so that there is no longer any possibility that for example, tissue fluid or blood on them Collect joint parts or between these joint parts can. Smooth parts and slots can be easiest Way clean.

The jaw part preferably consists of the just mentioned and a work area that corresponds to the  the jaw part with a cutting edge, a clamping area, can be shaped like a spoon or the like. Defini According to the working area through the hole be separated from the driver area by which the Axis pin is inserted around which the jaw part rotates.

In a preferred embodiment of the invention the instrument can be designed to be dismantled, d. that is Inner tube should be detachable from the jaw parts. Out for this reason, the drivers are designed and on ordered that when a certain opening is exceeded slide the mouth out of the slot so that the inner tube is released. But that doesn't mean that is done during normal surgery the inner tube is provided with a removable stop which this opening movement of the mouth is limited. Only after Removing the stop can open the mouth so far that the carriers slide out of the slots.

Such a movement of the carriers is usually only given when the driver opposite the Ar beitsbereich hook-shaped curved upward, so that the Driver in an opening movement of the jaw parts in the Interior of the inner tube or to the longitudinal axis of the chirur instrument is moved. Only through this Be the driver can move out of the slot and the inner tube are released.

If the inner tube is not detachable from the jaw parts desired, driver and work area could also be in Relative to the axis pin arranged or the driver of two jaws could be cross. In this case, a closing movement takes place Pull the inner tube instead, while in the other case the Closing movement takes place by pressure of the inner tube.

As mentioned above, however, the driver should preferably ha cone-shaped upwards or downwards on the same Be arranged with respect to the axis pin, such as the Workspace. In this case there are two possibilities the jaws. For one, by an end edge of the inner tube a pressure on a riser che of the work area are exercised, this Pressure leads to the closing of the jaw part. However, you can This pressure also comes from a rear inner edge of the Slot are taken over when moving the Inner tube forward on an inner sliding surface of the Driver pushes.

A front edge of the Slot that along a front sliding surface of the Driver slides and the driver to the inside of the Presses inner tube.

Should the rising surface of the work area and the front re sliding surface of the driver used for pressure and tension , they are preferably at an angle other, this angle usually between 70 and 110 ° degrees should be. They are also corresponding End edges or the front and rear sliding edges in the Slanted bevel so that they the riser or the Can slide sliding surfaces along. This results in a very good pressure transmission to the jaw parts.

Especially in the design of the just described Connection between inner tube and jaw parts and their Storage is recommended, the present surgical To design the instrument also electrically conductive, whereby however, protection is also required for the design becomes.

The embodiment of a monopo is relatively simple laren instrument. Here it is sufficient if the outer tube  and / or the inner tube is connected to a power source is, then the body of the living being to be treated is mostly used as a negative pole. Usually it did also proven to be favorable if Au outer tube and / or inner tubes with an insulating layer is / are so that the flow of electricity is not undesirable goes.

Above all, however, the present instrument offers itself as bipolar surgical instrument. In this case one jaw part over the outer tube with one pole and the other right jaw part over the inner tube with the other pole of the Power source connected. There is even the possibility to neglect the outer tube entirely as a conductor and a power line from the positive pole to the negative pole alone to accomplish the inner tube. In this case there is the inner tube consists of two metallic halves, which are separated by a Isolation layer are separated. The one metallic Half has the slot connection described above to that a mouth part, the other metallic half the top described slot connection to the other jaw part. Only the jaw parts now have to face each other be isolated so that there is no short circuit.

A connection to the power source is preferably made via a electrical connector, which with the handle is connected. There are many options here of the present invention are intended to be included.

According to FIG. 1, a surgical instrument R has a mouth 1 made of two jaw parts 2 and 3 . These jaw parts 2 and 3 are rotatably mounted together on an axle pin 4 and can perform an opening or closing movement. This is done by shifting an inner tube 5 indicated only by dashed lines, which slides along the double arrow 6 in an outer tube 7 .

The outer tube 7 is connected to a handle 8 , in which a part of the inner tube 5 is also inserted. The handle piece 8 goes into a leg 9 , at the end of which there is a grommet 10 .

The leg 9 forms near the transition to the handle 8 with a second leg 11 a pivot 12 , wel wel the leg 11 rotated relative to the leg 9 who can. The leg 11 engages beyond a further grip eyelet 13 with a nose 14 into a recess 15 in the inner tube 5 , so that this performs its movement in the direction of the double arrow 6 when the leg 11 is pivoted in the direction of the double arrow 16 .

A movement of the inner tube 5 to the right is limited by a stop 17 , which in the present exemplary embodiment forms a grub screw which is screwed into a bore 18 in the handle 8 .

According to the invention, at least one jaw part 3 according to FIG. 2 consists of a working area 20 and, after a bore 21 for receiving the axle pin 4, consists of a driver 22 . This driver 22 is curved in a hook shape relative to the working area 20 , so that the driver 22 with the working area 20 forms an angle w of approximately 90 °. The angle can, however, also be somewhat larger or slightly smaller than 90 °, depending on how the working area 20 and driver 22 are designed and are mounted in the outer tube 7 .

The angle w is formed by a rising surface 23 of the working area 20 and by a sliding surface 24 of the driver 22 .

The inner tube 5 has a slot 25 for receiving the driver 22 , the end edge 26 of which is preferably bevelled or rounded so that it can slide along the sliding surface 24 . Furthermore, an end edge 27 of the inner tube 5 is also slightly beveled so that it can slide along the rising surface 23 .

The mode of operation of the exemplary embodiment according to FIG. 2 is as follows:
If the inner tube 5 is moved to the left under the pressure of the nose 14 along the double arrow 6 , the end edge 27 presses on the rising surface 23 , so that the jaw part 3 rotates about the axle pin 4 and performs a closing movement against the jaw part 2 .

If, on the other hand, the legs 9 and 11 are spread apart, the inner tube 5 is shifted to the right, the end edge 26 of the slot 25 meeting the sliding surface 24 of the driver 22 and this sliding surface 24 sliding along ent. The jaw part 3 is in turn rotated about the axis pin 4 and performs an opening movement.

The stop 17 limits the movement of the inner tube 5 to the right. If the stop 17 is removed, for example the grub screw is rotated out of the bore 18 , the inner ear 5 can be moved to the right indefinitely. The driver 22 of the jaw part 3 slides completely out of the slot 25 and thus releases the inner tube 5 . The nose 14 slides after a certain rotation about the pivot joint 12 from the recess 15 and also releases the inner tube 5 . This can be removed and cleaned. Furthermore, easy cleaning of the jaw parts and the entire front area of the outer tube 7 is possible.

In Fig. 2 it can be seen that possibly also the outer tube 7 has a recess 28 into which the end of the driver 22 can slide. This depends on the geometry of the jaw part 3 . However, the driver 28 should not be able to emerge from this recess 28 so that it does not hinder the activity in the human body. It is also possible that the outer tube 7 must have a slot 29 in its end edge so that the jaw part 3 can slide into it, provided that the geometry of the jaw part 3 requires this.

In FIGS. 3 and 4 shows ge an embodiment in which both the jaw part 3 and a mouth part 2 are a movable construction. In this case, the jaw part 2 a is configured identically to the jaw part 3 , but in mirror image to this on the pin 4 angeord net. Accordingly, the jaw part 2 a with a participant 22 a, of the inner tube 5 a. to be led. For this purpose, the inner ear 5 a. another slot 25 a, which is designed in mirror image to the slot 25 . The same applies to an end edge 27 a of the inner tube 5 a. With a movement of the inner tube 5 a, as shown in Fig. 4, the jaw parts 3 and 2 a perform an opening movement, the respective end edges 26 and 26 a sliding along the sliding surfaces 24 and 24 a of the drivers 22 and 22 a.

When the inner tube 5 a moves to the left, the bevelled end edges 27 and 27 a meet the rising surfaces 23 and 23 a and close the jaw parts 3 and 2 a.

Another inventive concept of the present invention, which can be realized particularly well with the connection just described between the inner tube 5 and the jaw parts 2 and 3 , relates to a line of electric current to the jaw parts 2 and 3 . A distinction must be made between so-called monopolar and bipolar instruments. In the case of monopolar instruments, the instrument itself is connected to a positive pole and uses the human body as a negative pole. With the bipolar instrument, the current inside the instrument flows from a positive to a negative pole.

If the present instrument is used as a monopolar instrument, it is sufficient if the inner tube 5 and / or the outer tube 7 near the handle 8 is connected to a positive current source. The current is then transmitted via the inner tube 5 and / or the outer tube 7 to the jaw parts 2 and 3 , irrespective of whether these are fixed or are designed to be movable. The human body to be treated is used as a negative pole.

So that there is no misdirection of the current, at least the outer tube 7 , as shown in FIG. 5, should be surrounded by a shrink tube 30 . It is particularly important here that the driver 22 does not come into contact with the outer tube 7 in the recess 28 or damage the shrink tube. Furthermore, it is also contemplated to surround the inner tube 5 with a further shrink tube 31 so that undesired contact closures do not occur.

However, the present invention can also be used very well for a bipolar instrument, since the current can be transmitted separately to the two jaw parts 2 and 3 . For this purpose, the inner tube 5 is preferably used as a positive conductor and the outer tube 7 as a minus conductor. The jaw parts 2 and 3 are then isolated from each other, as described below for FIGS. 8 and 9.

In Fig. 5 it can be seen that the outer tube 7 is in the handle 8 and the heat shrink tubing 30 is ben. The very sump tube 30 merges into a plastic ring 32 in front of the handle 8 .

For connection to a negative pole, two metal rings 33 and 34 are provided, between which an insulation ring 35 is arranged. The metal rings 33 and 34 are in contact with the outer tube 7 , whereby the current is transmitted.

The inner tube 5 runs through, isolated by means of the heat-shrinkable tube 31, the outer tube 7, wherein the heat-shrinkable tube 31 terminates in the plane of the insulating ring 35th In this area, contact is made with a spring pin 36 of an electrical connector 37 , as will be described later.

Auf eine hintere Stirnfläche 41 des Innenrohres 5 drückt der Anschlag 17.A plastic center piece 39 adjoins a metallic contact area 38 of the inner tube 5 , followed by an end piece 40 . In this end piece 40 is also the recess 15 , in which the nose 14 of the leg 11 engages. The stop 17 presses on a rear end face 41 of the inner tube 5 .

The electrical connector 37 is designed so that it can be screwed into the handle 8 , for example, or, particularly with a rotatable instru ment, the handle 8 is rotatably mounted in the area of the metal rings 33 , 34 and the insulation ring 35 . Many possibilities are conceivable here and should be encompassed by the present invention. An external thread 42 is indicated for screwing in the electrical connecting piece 37 . This external thread 42 is incorporated into a metal sleeve 43 , which is used to connect to a negative pole. If it is desired, the metal sleeve 43 can be covered with an insulation layer on the outside.

Inside the metal sleeve 43 , the spring pin 36 leads GE, wherein it is supported against the metal sleeve 43 by insulation sleeves 44 , 45 and 46 . The insulation sleeve 45 has a larger diameter than the insulation sleeve 46 , so that here a chamber 47 is formed in which a spring ring 36 connected ner piston 48 can move against the pressure of a coil spring 49 . Thus, the spring pin 36 against the pressure of the coil spring 49 in the direction of arrow x be because.

The spring pin 36 protrudes with a connecting lug 50 from the metal sleeve 43 , a corresponding connecting socket can be pushed onto the connecting lug 50 .

In the position of use, this electrical connector 37 is screwed into the handle 8 , so that the metal sleeve 43 is connected to the two metal rings 33 and 34 , while the spring pin 36, the metallic contact area 38 through the insulation ring 35 or a corresponding recess in the Insulation area 35 pressurized through. The spring pin 36 allows movement of the inner tube 5 in the direction of the double arrow 6 without the contact being interrupted. Accordingly, the metallic region is relatively long before the middle piece 39 follows.

In the embodiment, an electrical contact shown in FIG. 5b is a spring pin 36 a is already integrated in the handle 8. This spring pin 36 a or its annular piston 48 a is supported by a coil spring 49 a against a metal ring 57 . If this metal ring 57 is supplied with electrical current, the current is also transmitted via the spring pin 36 a to the metallic contact area 38 of the inner tube 5 .

The metal ring 57 is separated on both sides with respect to the metal rings 33 and 34 via insulation washers 58 and 59 .

The exemplary embodiment is indicated in FIG. 7a, in which the current is led through the inner tube 5 to the jaw parts and through the outer tube 7 back to the negative pole, or vice versa. It is also conceivable that the inner tube 5 is ruled out both to the positive pole and to the negative pole. For this purpose, the inner tube, as shown in Fig. 7b ge, divided by an insulation layer 51 ausgebil det. An upper metallic half 52 is used for connection to one pole and a lower metallic half for connection to the other pole. The entire inner tube is then caught by the shrink tube 31 mentioned above.

In the latter case, it can be superfluous to isolate the outer tube 7 from the outside through a shrink tube. In practice, however, it is usually better to carry out this insulation measure.

Depending on how the current is conducted, the jaw parts 2 and 3 must also be insulated from one another and from the outer tube 7 . In Fig. 8, the insulation tion is indicated for the embodiment in which the inner tube 5 is used solely to conduct the current according to Fig. 7b. The jaw parts 2 and 3 do not sit directly on the axle pin 4 , but are isolated by corresponding insulation rings 54 and ring sleeves 55 and 56 in FIG. 9, insulated from the axle pin 4 and from one another. A power transmission takes place in each case by a metallic half 52 and the other me-metallic half 53 on the jaw parts 2 a and 3 in the area of cam 22, namely especially when the per weiligen end edges 27 and 27 a on the riser surfaces 23 and 23 a hit. In this case of a mouth part 2, the current flows to the mouth portion 3 through the tissue so that, it can be, if desired, the fabric is welded.

In the event that the outer tube 7 is accordingly used for power line Fig. 7a, one of the ring sleeves 55 or 56 is unnecessary, since the corresponding jaw part di can be directly in contact with the axle pin 4 or the outer tube 7 .

In Fig. 10 it is provided that a surgical instrument is also separable. Here, the outer tube 7 and the inner tube 5 are inserted into the handle 8 , the outer tube 7 having latching openings 60 . A locking pin 61 , which is not spring-mounted, can engage in these locking openings 60 .

To disassemble the instrument, the legs 9 and 11 are pressed against each other so that the nose 14 slides out of the recess 15 . Now the pin 61 is pulled down so that it slides out of the latch opening 60 . The outer tube 7 and inner tube 5 can then be pulled out of the handle 8 .
