The present invention further provides a tubular brace support areas available, which is a tissue-like object made of biodegradable and bioabsorbable material (hereinafter referred to as "Embodiment II").
The invention relates to a tubular support element for staple seam areas. According to Embodiment I is part of the support member made of stretchable material to pass through Elasticity to fit the support element into an automatic stapler make it easier to prevent slipping by a secure attachment and thus cutting of the support element together with the attacked body area by the tension of the To facilitate and ensure support element.
According to embodiment II, the entire support element consists of biodegradable and bio absorbable material to remove non-biodegradable material after surgery make surgery unnecessary and prevent foreign material from remaining in the body, because they accidentally forgot to remove non-biological material from the body.
Eine schlauch artige Form schließt zylindrische, prismatische, flache Formen usw. ein."Tubular" means that both ends of the fabric-like article or each end of the fabric-like object and the stretchy fabric are connected. A hose like shape includes cylindrical, prismatic, flat shapes, etc.
The biodegradable and bioabsorbable material according to the invention, for the whole Stützele ment or for parts of the support element for staple seam areas is thereby characterized in that the material is only briefly stable as a foreign material in vivo.
The stretch fabric of embodiment I is made by knitting or weaving rubber threads, elastic polyurethane threads, crimped threads and bulk yarns, etc. into the fabric and has elasticity in the longitudinal and transverse directions. Although the fabric is not limited to this, is a stretch knitted fabric (power net), produced by weaving of polyurethane threads covered with nylon threads because of the easy connectivity by sewing etc. and their dimensional stability as a preferred example.
Examples of the tissue-like article made of biodegradable and bioabsorbable material are knitted fabrics, woven fabrics, non-woven fabrics, foils etc. made of polyglycol acid, polylactic acid, copolymers or mixtures of polylactic acid and polyglycolic acid re, para-dioxanone, poly-caprolactone, chitin and similar already in biomedical materia lien used substances are produced. One in terms of flexibility, breathability, moderate elasticity, thickness, blood absorption properties, permeability to staples and Degradability material is non-woven fabric, made by needle punching Knitted fabric or woven fabric made of polyglycolic acid, as in JP-A-5-18579 and JP-A-5-79586 described by the applicant. The non-woven fabric has little stretch.
When carrying out embodiment I, the joining of tissue-like counter and stretchy fabric achieved by stacking both materials (preferably with the same Size for easier processing), cutting to the desired size and together joining both ends (both selvedges) by sewing, gluing etc. An example of a a particularly preferred type of assembly is the white stitch with rough sewing thread Stitches to facilitate separation of the two layers after fixation by the clips. The ends of the seam are preferably allowed to protrude outwards in a suitable length in order to to facilitate the above separation. The sewing thread closes sewing threads, medical Suture, etc. The type of material is not particularly limited and includes nylon, Polyester, vinylon, cotton, silk and similar non-biodegradable materials and the above mentioned biodegradable and bioabsorbable materials. The sewing thread is preferred as from the same biodegradable and bioabsorbable material according to the invention, since the sewing thread is brought together with the support element and can remain in vivo. To achieve the object of the invention, it is also important that the inner through knife of the tubular support element, which was formed by the assembly, the is the same or less than the outer diameter of the stapler with which it is is used. Therefore, the size and shape of the fabric-like article and the Seam means for joining the preferably according to the respective situation customized.
When carrying out embodiment II, the manufacture of the tube is of the fabric type Item made from biodegradable and bioabsorbable material by both ends of the fabric-like object that are of a suitable size for use cut in the automatic stapling machine, connects, using any suitable Method can be used, such as sewing, gluing, etc. Temporary Sewing or gluing are particularly preferred for handling according to the below to facilitate the described sewing process. Tubular fabrics can be used as a tube like support elements for the staple area are used.
Fig. 1 shows an inventive support element for a stapling area. The Stützele element comprises non-woven fabric ( 3 ) made of biodegradable and bioabsorbable material, which is joined at both selvedges by seams ( 5 , 6 ) made of medical suture material as sewing thread in simple smooth stitches with stretchy warp-knitted fabric ( 4 ). Both ends of the seam ( 7 , 7 ) extend outwards.
Ein Beispiel einer Naht ist eine Polyglycolsäurenaht der Größe 4-0.The non-woven fabric ( 3 ) is obtained by stacking 4 layers of cylindrical knitted fabric with a weight of 45 g / m², which are slightly needled along the direction of the ribs, so that the rib direction of the one fabric and the direction the course of the adjacent fabric is aligned, followed by the needling of the stacked fabrics. The cylindrical knitted fabrics are obtained by knitting threads of 12 filaments and 33 deniers made of polyglycolic acid with a circular knitting machine ( 18 gauge). The stretchable warp-knit fabric ( 4 ) made of stretch warp-knit fabrics is produced by weaving 280-denier polyurethane threads and 24-filament and 70-denier nylon fibers with a 15:85 proportion of polyurethane to nylon with a warp-knitting machine. An example of a seam is a size 4-0 polyglycolic acid seam.
The non-woven fabric ( 3 ) and stretchable warp-knitted fabric ( 4 ) are cut to the same size of 80 × 25 mm, stacked and then sewn by simple smooth stitches by hand 2 mm inside the side edge with a stitch width of 10 mm to make that Invention-like tubular support element for the area of the staple seam ( 1 ). Both seam ends ( 7 ) extend outwards for about 8 cm. The same procedure is repeated, except that the fabrics are cut to 80 x 20 mm in size to give a different support member for the staple area ( 2 ).
Fig. 2 describes a commonly used automatic stapler of the double staple type ( 8 ), which is equipped with a built-in staple cartridge section ( 9 ) and a frame section ( 10 ) which has a slot for the staples. The cartridge section and the frame section are provided with the support elements for the clamp seam areas ( 1 , 2 ). In Fig. 2, ( 11 ) corresponds to the handle for compressing the clamp and ( 12 ) to the cutting tool. Fig. 3 is a section along the line AA of Fig. 2. In Fig. 3 stretchable warp knitted fabric ( 4 ) is drawn in the stretched state.
As shown in Fig. 3 and 4, an attacked part of the body tissue between non-woven fabric ( 3 , 3 ) of the support element, applied to cartridge section ( 9 ) and frame section ( 10 ), is pinched by pressing the handle during the operation. The attacked body tissue is connected to non-woven fabric by clips ( 15 ). The attacked part ( 13 ) and normal parts ( 14 ) of the body tissue are cut with the cutting tool ( 12 ). One pulls the end ( 7 ) of the suture to separate the area ( 14 ) remaining in vivo and the area to be removed. The cut off attacked part ( 13 ) and the stretchable fabric, ie stretchable warp-knitted fabric ( 4 ), are removed from the body.
Fig. 7 describes another inventive supporting element for stapling portions. The support element comprises non-woven fabrics ( 3 , 3 ') made of biodegradable and bioabsorbable material, which are joined together at both edges in the central region and in an end section by sewing together with sewing threads ( 5 , 6 ). In the other end section, the seams run diagonally from the edges to the central region of the end, so that the support element has a narrowed opening at one end. When the support element of FIG. 7 mounted to the example shown in Fig. 2 automatic stapling device (8) of the double clamp type, the narrowed end of which lies on the top of the staple cartridge area (9) and the frame portion (10), without that it shifts or slips, thereby preventing poor fitting.
The results described above were also achieved using polyester sewing thread achieved instead of medical sutures.
Fig. 5 zeigt das Stützelement für Klammernahtbereiche der Ausführungsform II. Fig. 5 shows the support element for clamp seam areas of embodiment II. The Stützele element ( 2 ') comprises the above non-woven fabric ( 3 ') made of polyglycolic acid. Both ends of the non-woven fabric are connected to each other with a suture made of medical suture material in simple smooth stitches ( 4 '). As shown in Fig. 6, the Stützel emente ( 2 ') non-woven fabric ( 17 ) made of bioabsorbable polymer in an edge region, which is fixed by clips. The non-woven fabric ( 17 ) is attached with adhesive ( 19 ) to inexpensive non-woven fabric ( 18 ) that is not made of biodegradable and bioabsorbable material.
The support element for clamp seam areas of embodiment II can in the same way as the support element of embodiment I shown in Example 1 can be used.
The support element according to the invention for staple seam areas is characterized in that it prevents air holes by cutting and tearing the soft tissue. The stretch Availability of the stretchable material attached to the fabric-like object facilitates this Attaching the support element to automatic staplers of various sizes or the positioning of the support element. The tensile stress of the stretchy material prevents after slipping and inaccurate attachment when attached to the surgical stapler tion in the seam area, and the cutting operation after fixation by the clamps facilitated. The support element is also economical because of its low proportion of expensive biodegradable and bioabsorbable material. The non-woven fabric made of biodegradable and bioabsorbable material can be combined with less stretchy fabric if stretchy sewing thread is used.
