Serumproben von NF1 Patienten wurden von der Abteilung Mund-Kiefer und Gesichtschirurgie, Universitätsklinik Eppendorf, Hamburg bezogen. Alle NF1 Patienten wurden klinisch nach den veröffentlichen Richtlinien und Kriterien diagnostiziert [31]. Serumproben von gesunden Probanden wurden von dem Institut für medizinische Immunologie, Charité-Universitätsmedizin Berlin bezogen. Alle Seren wurden mittels IRB-bestätigten Protokollen gesammelt. Detaillierte Informationen zu den Patienten Kohorten sind Tabelle 1 zusammengestellt. Venöses Blut (1 bis 10 ml) wurde gesammelt, innerhalb von 2 Stunden zentrifugiert und anschließend wurden die Serumproben aliquotiert und bei –80°C bis zur Verwendung gelagert. Das Auswählen von Kandidatenmarkern basierte auf einer manuellen Literatursuche in Veröffentlichungen und Datenbanken, welche in (i) Serum, Plasma oder Zellüberständen Proteinlevels von Patienten mit Nerventumoren oder Epitheltumoren oder Zelllinien beschreiben, und in (ii) differenzieller Genexpression zwischen den normalen peripheren Nervensystem, Neurofibromen und MPNST sowie (iii) immunmodulatorische Zytokine. Die so aufgestellte Kandidatenfaktorliste basiert auf der Genontologie (GO) und den Genkarteninformationen [32, 33] weiter reduziert indem solche Faktoren, die bereits eine bekannte Funktion in der Tumorgenese wie beispielsweise Wachstumsbeschleunigung, Migration und Metastasierung, Angiogenese und Immunmodulation aufweisen, ausgewählt wurden. Die abschließende Auswahl der Kandidatenfaktoren wurde anhand der Verfügbarkeit geeigneter Screening Plattformen ausgewählt. Individuell hergestellte humane Quantibodys® Arrays (Raybiotech, Inc., GA, USA) wurden verwendet um die Serumproteinkonzentration zu bestimmen. Die Analyse wurde gemäß den Herstellerangaben durchgeführt. Das Imaging wurde mittels Quantibody® Array Testing Software (Raybiotech, Inc., GA, USA) durchgeführt. Potenzielle Markerproteine wurden zuerst bei einem Screening von 30 Proteinkandidaten unter Verwendung von 60 NF1 Seren identifiziert. Eine weitere zweite Validierung wurde mit den 5 Proteinen, welche signifikante Unterschiede in der ersten Screening Runde aufzeigten, zusammen mit weiteren 26 Kandidatenproteinen durchgeführt. In der zweiten Runde wurden 104 NF1- und 41 Kontrollseren verwendet. Insgesamt wurden 56 Kandidatenproteine gescreent sowie 104 NF1- und 41 Kontrollsera verwendet. Serumlevel der Markerkandidaten in den NF1 und Kontrollobjekten wurden mittels der Medianebene und des Quartilabstands analysiert. Um alle Daten einer normalen Verteilung zu verifizieren, wurde der Kolmogorow-Smirnow Test verwendet. Ausdifferenzierte Patientengruppen wurden mittels des Mann-Whitney U Test für kontinuierliche nicht-parametrische Variablen verglichen. Um die Aussagefähigkeit der individuellen Marker zu prüfen wurden die Receiver Operating Characteristics (ROC) der Kurven und die Fläche unter dieser Kurve (AUC) berechnet. Es wurde eine Bonferroni Korrektur durchgeführt. Die P-Werte < 0,05 wurden als signifikant angenommen. Die Paraffinschnitte der Tumoren wurden entparaffiniert, rehydriert und gewaschen. Proben und Standards werden in die Wells pipettiert, sodass darin enthaltenes IGFBP1 fest an die Platte gebunden wird. Nichtgebundenes Material wird durch Waschen entfernt. Anschließend wird ein mit Biotin gekoppelter Antikörper gegen IGFBP1 zugegeben. Nach einem weiteren Waschschritt zur Entfernung des überschüssigen Antikörper-Enzym-Komplexes folgt die Zugabe von HRP-gebundenem Streptavidin. Nach einem letzten Waschschritt wird Substratlösung zugegeben. Das Substrat wird durch das gebundene Enzym umgesetzt und es kommt zu einem Farbumschlag der Lösung. PMA-Ionomycin simuliert die Stimulation des T-Zell-Rezeptor und Costimulation, wodurch sämtliche T-Zellen unabhängig von ihrer Rezeptorspezifität angeregt und zur Zytokinproduktion stimuliert werden. Zur intrazellulären Akkumulation der Zytokine wurde Brefeldin A (5 g/mL) zugesetzt. Die Zellen werden fixiert, anschließend mit BD Perm/WashT-Puffer permeabilisiert und dann intrazellulär mit den entsprechenden Antikörpern angefärbt. Die T-Zellen können mittels FACS Analyse durch das Anfärben der spezifischen Oberflächenrezeptoren CD3, CD4 und CD8 mit kommerziell erhältlichen Antikörpern identifiziert werden. Die intrazellulären Zytokine werden mittels anti-TNFalpha (Maus anti-human TNFalpha) sowie anti-IFNgamma (Maus anti-human IFNgamma) detektiert. The observation that IGF-1 and growth factor receptors in PNF and MPNST are expressed by NF1 patients and that IGF1 receptor levels correlate with an increased mitotic index of PNF indicates that these tumors are sensitive to IGFBP1-regulated factors [10, 44] , In conclusion, IGFBP1 may modulate IGF access to PNF and MPNST, but this mechanism should be further explored.

Schlussfolgerung: Die im Rahmen der vorliegenden Erfindung durchgeführten Experimente beinhalten die bisher größte Studie bezüglich der Anzahl von NF1-Patienten (n = 104), welche je für potenzielle Serummarker für diese seltene, genetisch bedingte, Tumorerkrankung vorgenommen wurde. Es konnten vier potenzielle Biomarker identifizieren werden, welche in der Diagnose von NF1 eingesetzt werden können und zwei weitere Marker (IGFBP1 und RANTES), die mit der Anwesenheit von MPNST korrelieren. Interessanterweise scheint IGFBP1 auch mit der internen Tumorlast zu korrelieren und deutet auf ein erhöhtes Risiko für eine maligne Transformation in NF1-Patienten hin. Weiterhin zeigen die vorliegenden erfindungsgemäßen Daten, dass NF1-Patienten systemische pro-inflammatorische Profile aufwiesen, welche wahrscheinlich durch eine NF1-Haploinsuffizienz hervorgerufen wird. Serumbiomarker, die bei der Früherkennung der malignen Umwandlung helfen sind extrem nützlich, da therapeutische Eingriffe vorher initiiert werden könnten bevor der Tumor sich weiter entwickelt und Metastasen entstehen.Conclusion: The experiments carried out within the scope of the present invention include the largest ever study of the number of NF1 patients (n = 104) ever for potential serum markers for this rare, genetic tumor disease. Four potential biomarkers could be identified that can be used in the diagnosis of NF1 and two additional markers (IGFBP1 and RANTES) that correlate with the presence of MPNST. Interestingly, IGFBP1 also appears to correlate with internal tumor burden, indicating an increased risk of malignant transformation in NF1 patients. Furthermore, the present data of the present invention shows that NF1 patients had systemic pro-inflammatory profiles, which is likely to be caused by NF1-haploinsufficiency.
