Abstract
An instrument is described for minimally invasive and open surgical interventions, in particular for cutting and / or clamping and / or spreading tissue treatment as the main function. So that a safe and effective continuous monitoring of nerve function is possible in the treatment of tissues with sensitive nerve strands, at least one instrument component (12-1), which consists at least in sections of electrically non-conductive material, is provided with a conductive contact thread ( 32), which is installed on or embedded in this instrument component. This contact thread can be connected to a power source in a controlled manner and is guided to a stimulation area (34) lying on the surface of the instrument component. In this way, the surgeon can perform a functional monitoring of the nerve cells at any time during the tissue treatment by controlling the stimulation area with electric current without having to move the surgical instrument (10) out of the working channel.
Description
The invention relates to an instrument for minimally invasive and open surgical procedures, with the main function of a cutting and / or clamping and / or spreading tissue treatment is feasible.
Surgical instruments including those for minimally invasive surgical procedures are known in many embodiments. They are used for a variety of surgical procedures with a major function such. B. for cutting, staples, u. s. w. However, all such surgical instruments have in common that they require a working channel through which a controlled access to the site of the surgical procedure is created. In minimally invasive procedures, this channel is often provided by a so-called trocar, which at the same time can take over the task of guiding the surgical instrument.
In this known case, an endoscopic system is provided in which an instrument for minimally invasive surgery such. As a clamp, a needle, a loop, a radio-frequency source, a dispensing unit for a chemical or pharmaceutical substance or a photodynamic therapy device (PDT), equipped with a diagnostic device in the form of optical fibers. Via these optical fibers, electromagnetic excitation energy is transmitted to a tissue or an electromagnetic response from the tissue to the exit of the endoscope is reported. In this way, however, the surgical instrument is considerably more expensive.
It is also known to perform minimally invasive surgical procedures in such a way that in addition a so-called "neuro-monitoring" takes place. This monitoring is particularly important when the surgical procedure is performed in the vicinity of sensitive nerve cells. In addition to neuro-surgery, "neuro-monitoring" is performed, for example, during thyroid surgery. The position of the nerve responsible for the vocal cord function is constantly monitored. However, to preclude the nerve from being injured during surgery, it is necessary from time to time to check the nerve's response to stimulation during surgery. To do this, the instrument must be removed from the working channel and the nerve stimulation device inserted into the working channel instead. This not only increases the duration of the operation considerably, but it also increases the risk of unwanted tissue and / or nerve injuries, especially as in many endoscopic minimally invasive procedures, the working channel is constantly relocated.
The invention is therefore the object of developing an instrument for minimally invasive or open surgical procedures of the type described above such that the surgical procedures can be performed faster and thus cheaper and at the same time safer.
This object is solved by the features of claim 1.
According to the invention, the head of the surgical instrument is reconfigured in such a way that it can be used for the additional function of a directed, in particular precise, nerve stimulation. This is achieved in that at least one instrument component at least partially consists of electrically non-conductive material and that at this section a conductive contact thread is laid, which is connectable to a power source and guided to a lying on the surface of the instrument component stimulation area.
In this way, without changing the instrument design, in particular without changing the instrument design, the surgical instrument obtains an additional function for nerve stimulation, which can be provided without displacement of the surgical instrument. As a result, the surgical procedure can be performed much faster and at the same time considerably more economical, considering that the cost of an operating minute is about 50.00 EUR. At the same time the operation is safer by the inventive design of the instrument, since a multiple withdrawal and re-insertion of the example endoscopic instrument can be omitted. The entire surface of the instrument component can be used to form the stimulation area. In this way, depending on the respective instrument, an optimal position of the stimulation region can be selected, preferably in such a way that the least possible movement of the instrument component is required for the nerve stimulation.
The instrument component can be a variety of components of the relevant surgical instrument for minimally invasive or open surgery, but also to units for other tissue treatment, such. B. for ultrasound and / or laser and / or drug treatment of tissue.
A particularly economical production of the surgical instrument results when the instrument component as a whole consists of electrically non-conductive material, such as. Example of plastic or a ceramic material, preferably of aluminum oxide (Al 2 O 3 ). Such a ceramic material has the merit of good biomedical compatibility on the one hand and high mechanical strength on the other hand. Due to the high wear resistance, this material is also suitable for components that are mechanically highly stressed, such. B. stapling or scissors or knife components. In addition, this material has the advantage that there are reliable methods to embed in this material, other electrically conductive materials or to lay on this material. Thus, in such a material readily a gold thread permanently inserted or embedded, with a permanent solid connection with the ceramic material can be ensured. The stimulation region, for example, by vapor deposition of a metal layer, such. B. a gold layer can be provided at any point of the instrument surface. This has the advantage that the shape and position of the stimulation region can be varied within wide limits depending on the field of application.
The field of application of the instrument according to the invention is basically unlimited. Special advantages arise when used in neurosurgery and thyroid surgery.
Vorteilhafte Ausgestaltungen sind Gegenstand der übrigen Unteransprüche.Advantageous embodiments are the subject of the remaining dependent claims.

The particular advantage of the surgical instrument according to the invention is that the movements of the instrument component are minimal, so that valuable surgery time can be saved and thereby the operation can be carried out more cost-effectively. Due to the electrically insulating shielding of the electrical conductor in the configuration as a contact thread or as a contact coating, the stimulation of the tissue can be concentrated by means of electrical energy to the point where the surgeon wants the measurement result.

The illustrations are to be understood purely schematically. The actual design of the recesses for receiving the electrically conductive thread is substantially smaller in practice. They depend on the connection technology or on whether the conductor mechanically or by another connection technology, such. B. is fixed by a vapor deposition or sputtering technique on the carrier. It should also be emphasized that the invention is by no means limited to forming the conductor as a self-supporting structure or to guide it along the surface of the surgical instrument. It is also an embedding inside the instrument component possible.

Here, for example, a coating of conductive material, such. As a gold coating application, wherein the connection to the power source via a conductor as described above.
The contact thread or conductor routed to the stimulation region may also be completely within the surgical instrument component.
Furthermore, the instrument component can also be equipped with an electrically insulating coating.
Instead of a simple current-carrying conductor, it is also possible to lead several conductors with different functions to the stimulation area, for example a conductor with which the reaction of the nerve to the control unit is reported back.
The invention thus provides an instrument for minimally invasive and open surgical procedures, in particular for cutting and / or clamping and / or spreading tissue treatment as the main function. This contact thread can be connected to a power source in a controlled manner and guided to a stimulation area located on the surface of the instrument component. In this way, the surgeon can perform functional monitoring of the nerve cells at any time during tissue treatment by driving the stimulation region with electrical current, without having to move the surgical instrument out of the working channel.
