Abstract

The catheter has a large-area indifferent electrode that is provided for myocardial stimulation and integrated into the catheter through a coil or through vaporization of a conductor. The electrode is arranged in a distal end of the catheter and stays in contact with blood in an electrically conducting manner.

Description

Im Rahmen Katheder-inverntioneller Therapiemaßnahmen, insbesondere bei der Durchführung von perkutanen transluminalen Ballonangioplastien (PTCA) sowie der Rotablation ergeben sich häufig Situationen, bei denen schwerwiegende Bradykardien eine passagere Elektrostimulation des Herzens erfordern.in the Framework catheter-non-therapeutic therapies, in particular when performing percutaneous transluminal balloon angioplasty (PTCA) as well as the rotablation often arise situations, where severe bradycardia is a transient electrical stimulation of the heart.

In about 180,336 percutaneous coronary angioplates (PTCA) [ Bruckenberger et al., Herzkathederbericht 2000 ] in the Federal Republic of Germany in 2000, it can be assumed that approximately one third of these PCTA procedures were performed on the right coronary artery. There is a procedure-related risk of severe, circulatory bradycardia or even asystole. This risk is due, on the one hand, to the direct occlusion caused by balloon catheterization and thus to consequent ischemia of the coronary vessel and, on the other hand, to the possibility of embolic carryover of plaque material into those portions of the right coronary artery which are responsible for supplying blood to chronotropic centers, such as sinus and AV Knot, are responsible. As a result of bradycardia, it can lead to severe hypotension to shock symptoms, in the worst case to a complete cardiovascular arrest. So far, there are no reliable findings in which intervention a prophylactic transient implantation of a right ventricular stimulation probe is to be used. In addition, the risk of concomitant complications due to the use of pacemaker probes should be considered. Of importance is the over 6-fold risk of occurrence of ventricular arrhythmias in prophylactic placement of a temporary pacemaker electrode in the right ventricle in the context of diagnostic coronary angiography. An alternative to the costly transvenous stimulation is the transcoronary myocardial stimulation. This stimulation method has proven to be an effective method both in animal models and when used in conjunction with coronary interventions in humans. Here, the stimulation of the heart via the guide wire located in the coronary vessel against a unipolar Patch electrode, which is attached to the patient epicutaneously. The efficiency of this process is only 85% using the currently available coated corona wires.

Either the transvenous stimulation because of their existing risks as well as the transcoronary stimulation against an indifferent epicutaneous electrode can not be considered prophylactic because of unsatisfactory efficiency Stimulation method used.

It thus, the problem arose to develop a stimulation method, which bypasses the risks mentioned with a high efficiency.

According to the invention was solved the problem according to claim 1, characterized that a guide catheter with integrated or applied, large-area indifferent electrode used for myocardial stimulation which is made of a coil or a vapor-deposited on the catheter Ladder or the like exists. This indifferent electrode on the guide catheter is thus located in the lumen of the Aorta and has contact with the (electrically conductive) blood.

Of the Advantage of the guide catheter according to the invention consists in the immediate availability of an indifferent Electrode for transcoronary emergency stimulation in the frame of coronary interventions. The means of this PTCA guide catheter achievable stimulation efficiency is comparable to that of the transvenous Stimulation, since no transcutaneous resistance including subcutaneous Adipose tissue is overcome.
