Abstract

Description
技术领域
本发明涉及一种按权利要求1前序部分所述的椎间植入物,尤其是人造的椎间盘。
背景技术
目前在去除一个损伤的、天生的椎间盘或者一个损伤的椎间盘髓核后,将植入物或假体置入到两个相邻椎骨体之间的椎间腔内。植入这种植入物的目的是,重新形成尽可能天生的状态,也就是说尤其是原来的椎间盘高度并且从而恢复原来的在两个相邻椎骨体之间的距离。此外相邻椎骨体的运动应该可以相互相对尽可能没有妨碍地以其天生的方式进行。为此在椎骨体向前/向后倾斜时,也就是椎骨体的屈和伸时以及椎骨体在天生的极限范围内横向弯曲时均保持运动能力是重要的。天生的韧带和肌肉沿着脊柱基本上是完整无损的,这样它们就进一步稳定一个机械的椎间盘代替物的运动。
一种这样的椎间植入物由US 5,556,431BETTNER已知。这种已知的植入物包括一个下盖板和一个上盖板,其外表面可以贴靠到邻接椎骨体的盖板或者底板上,还包括一个设置在盖板之间的关节。所述关节主要由一个球形凸面的关节件和两个与盖板相连接的、互补的、作为第二关节件的关节壳构成,这样盖板可相互相对多轴地转动。这种已知的椎间植入物的缺点是,虽然两个盖板分别与一个关节件相连接,但是关节件不能互相固定。
发明内容
本发明克服弊病。本发明的目的是,提供一种椎间植入物,该椎间植入物具有一个可转动运动的关节,关节的部件在一个相互相对固定的状态下可预装配,并且可以在现地解除固定。
本发明用一种具有权利要求1特征的椎间植入物,尤其是一种人造的椎间盘来解决提出的目的。
本发明进一步有利的结构在从属权利要求中给出。
通过本发明实现的优点主要借助于按本发明的椎间植入物来说明:
-可实现低的制造成本;
-植入物在其植入前可预装配,这样外科医生就不必组装植入物,并且不会出现误操作;
-植入物在植入前和植入过程中不会散开,或者没有部件会丢失;并且
-关节件与盖板连接,这样关节件就不会移动和破坏周围的组织。
在一种优选的实施方式中,锁定装置构成为单件式的。由此可实现的简单的结构使在椎间植入物上装配和拆卸锁定装置时的操作简便。
在一种不同的实施方式中,锁定装置设计成使它允许所述两个件的自动固定。这例如可以通过其至少部分弹性的结构或通过楔作用在装配锁定装置时实现。借此可实现的优点是,可以不用外部的影响,例如手工协助,来固定这两个件。
在又一种不同的实施方式中,用于安装锁定装置的机构在上件上包括至少一个上槽以及在下件上包括至少一个下槽,其中各槽的纵轴线与相应的件的前侧的侧面相交。借此可实现的优点是,用于安装锁定装置的机构可以具有一种简单的结构。此外即使当分开器械还与椎间植入物相连接时,槽也允许去除锁定装置。
在一种另外的实施方式中,所述槽通到相应的件的前侧的侧面上。借此可实现,锁定装置在植入椎间植入物后可以容易地拔出。此外锁定装置的拔出在与植入椎间植入物时的同一条轴线上进行,这样在椎间腔内就不必进行附加的预先准备。
在又一种另外的实施方式中,椎间植入物在侧向的侧面之间的中心位置上具有一个包含中心轴线的中心平面,并且在中心平面的每一侧上包括一个上槽和一个下槽。因此可以实现的优点是,通过将锁定装置安置在中心平面的两侧可以实现所述两个件的稳定的固定。另外,所述固定相对于中心平面是对称的,因此明显简化外科器械在现地的操作。
在一种不同的实施方式中,槽的横向于中心平面的横截面朝着所述两个件的上面的或者下面的表面的方向逐渐变小,这样槽例如构成为燕尾形导槽,并且可以在两个平行于中心轴线的方向受力。
在又一种不同的实施方式中,槽是弯曲的,并且具有弧形的纵轴线,其中从纵轴线到中心平面之间的距离朝着前侧的侧面的方向增加。借此可实现,椎间植入物在植入前可以用夹钳形的锁定装置抱住和互相固定,这样各部件在植入的过程中不会相互相对运动,并且没有部件会丢失。
在一种另外的实施方式中,锁定装置包括一个平行于中心轴线设置的连接接片和位于其末端上的各一个侧边或者横接片。在第一种情况中,锁定装置例如可以设计成U形的带有弹性侧边和/或一个弹性连接接片的夹子。在第二种情况中例如可以是一个工字形的结构。这些结构的优点是,制造简单并且从而锁定装置的成本很低。此外操作锁定装置是简便的。锁定装置的复杂结构也易于折断和夹伤。
在一种不同的实施方式中,锁定装置设计成钳形的,并且包括两个可以绕着一个旋转铰链相互相对活动的、可导入到槽内的钳口。其中旋转铰链的旋转轴优选平行于中心轴线延伸。借此可实现简单的制造并且从而锁定装置的低成本。此外可以没有歪斜地打开和关闭锁定装置。
在一种另外的实施方式中,凸面的关节件包括一个球形的第一关节面,并且关节壳包括一个与其互补的第二关节面。通过关节面的球形结构可实现的优点是,所述两个件可相互相对多轴地转动。
关节面的尺寸根据使用情况是不同的,其中在不同的实施方式中,凸面的球形关节件的第一关节面的半径在3mm至25mm之间,优选在4mm至20mm之间。
在又一种另外的实施方式中,凸面的关节件和关节壳由一种金属-塑料的材料副制成。借此可实现的优点是,
-可使用已经提到过的关节代替材料的组合,例如一种高度交联的聚乙烯(X-UHMWPE)和一种钴-铬合金;
-在滑动面相对移动时可实现较小的摩擦力;并且
-可实现轴向冲击载荷的衰减。
在一种不同的实施方式中,关节面A、B用一碳化钛或无定形碳(ADLC)覆层,由此可显著减小摩擦系数。
在又一种不同的实施方式中,所述两个件在对合面上用钛覆层。
在一种另外的实施方式中,各对合面构成为凸形的。并且与邻接的椎骨体的天生的盖面或者底面相适配。
在又一种另外的实施方式中,所述两个件在其对合面上设有宏观结构,所述宏观结构优选构成为凸起。
所述凸起在不同的实施方式中可以包括:
-角锥状凸起;
-至少一个对称于中心平面的、楔形的筋条,该筋条在有关的对合面上沿着一条前后方向的直线设置,或
-对称于中心平面设置的、锯齿形的齿。
通过所述宏观结构的构造可实现的优点是,一方面绕着一个与对合面相交的旋转轴线的转矩可以更好地从椎骨体传递到椎间植入物上,并且另一方面可以提高骨头可粘附的面。齿的优点是,它们可以更好地进入到邻接的椎骨体的终板上。角锥状凸起的体积V优选在0.12mm3至1.4mm3之间。
在一种不同的实施方式中,凸起至少部分地用羟基磷灰石或用一种双相的羟基磷灰石-磷酸三钙混合物覆层,借此可实现的优点是,两个提到过的材料完全集成到骨头内或者甚至被新的、天生的骨组织代替。
锁定装置的最大体积在不同的实施方式中可以是12cm3,优选6cm3。
在又一种不同的实施方式中,椎间植入物包括一个无菌的包装,使得预装配的和借助于锁定装置固定的椎间植入物可以由外科医生不用预先操作就在现地植入。

具体实施方式
在图1至图3中描述的实施形式主要包括一个上件3、一个下件4和一个设置在这两个件3、4之间的关节9,所述上件具有一个与中心轴线相交的上对合面35,用于贴靠在邻接于其上方的椎骨体的底板上;所述下件具有一个与中心轴线2相交的下对合面45,用于贴靠在邻接于其下方的椎骨体的底板上;所述关节用于铰接这两个件3、4。关节9设计成两件式的,其中凸面的关节件12设计成球形、具有一个第一关节面10并且与下件4连接,与凸面的关节件12互补地构成的关节壳13具有一个第二关节面11并且与上件3连接。上件3和关节壳13在所示实施形式中是两件式的,因此上件3例如可以由钛或一种钴铬合金制成,而关节壳13例如可以由一种塑料制成。
这两个件3、4中的每一个都包括一个前侧的侧面31、41、一个背侧的侧面32、42以及两个横向于上述各侧面的侧向的侧面33、34、43、44。在侧向的侧面33、34、43、44中间有一个中心平面19,其中,中心轴线2在关节9不摆动时处于该中心平面19内。凸起18在此设计成角锥状凸起17。

两个锁定装置21中的每一个都设计成带有两个侧边27和一个将侧边27连接的连接接片28的夹子26,这样在中心平面19的一侧,一个夹子26的一个侧边27可导入到一个上槽22a内,并且同一个夹子26的另一个侧边27可导入到一个下槽23a内,以使夹子26平行于中心平面19设置。类似的是,一个第二夹子26的侧边27可导入到在中心平面19的另一侧上的槽22b、23b内。夹子26平行于中心轴线2的方向可弹性变形,以使侧边27在导入到槽22、23内后撑开,并且锁定装置21可以通过弹性的夹紧作用可松开地固定在所述两个件3、4上。
在图4至图7中示出的实施方式与在图1至图3中示出的实施方式的区别仅仅在于槽22、23和锁定装置21的不同结构。槽22、23朝着所述两个件3、4的上面的或者下面的表面36、46的方向是开口的。此外槽22、23朝着相应的上面的或者下面的表面36、46的方向逐渐变小,也就是说例如设计成燕尾形导槽。此外两个锁定装置21分别设计成工字形的,并且包括一个连接接片28以及位于其末端上的各一个横接片30。在此每一个锁定装置21可以分别在中心平面19的一侧导入到槽22、23内。槽22、23的横截面随着与前侧的侧面31、41的距离的增加而变小,以使锁定装置21在导入到槽22、23内时楔在槽内并且可松开地固定在所述两个件3、4上。
在图8至图10中示出的实施方式与在图1至图7中示出的实施方式的区别仅仅在于槽22、23和锁定装置21的不同结构。槽22、23朝着所述两个件3、4的上面的或者下面的表面36、46的方向是开口的并且具有弯曲的纵轴线24、25。槽22、23的曲线设计成使在两个设置在所述件3、4之一上的槽22、23之间的距离朝着前侧的侧面31、41方向增加。此外槽22、23朝着相应的上面的或者下面的表面36、46的方向逐渐变小。锁定装置21在这儿包括两个可导入到槽22、23内的带有手柄51的钳口50,各手柄通过一个旋转铰链52可以相互相对活动地连接。旋转铰链52的旋转轴线49平行于中心轴线2。钳口50具有一个U形横截面,并且各包括一个平行于中心轴线2的连接接片28和两个位于其末端上的横接片30,这样当锁定装置21导入到槽22、23内时,所述两个件3、4也平行于中心轴线2被固定。在手柄51的自由端53上可固定一个器械,由此椎间植入物1可导入到椎间腔内,并且在植入后锁定装置21可以容易地从槽22、23内拔出。
除了在图1至图10中示出的角锥状的凸起17外,在上和下对合面35、45上的凸起18可以如图11和图12所示的各包括一个楔形的、高出角锥状凸起17的筋条38,该筋条在对合面35、45上平行于一条在中心平面19内(图1)的直线。为了使椎间植入物1可以更好地导入到椎间腔内,筋条38的高度朝着背侧的侧面32的方向逐渐变小。
凸起18的另一种实施方式在图13和图14中示出。高出角锥状凸起17的齿39部分设计成锯齿形的,其中陡峭的齿面指向前侧的侧面31。齿39在这儿位于一条在中心平面19内(图1)的直线上,并且从前侧的侧面31一直延伸到背侧的侧面32上。
在图11至图14中各自只示意地示出上件3。带有角锥状凸起17、一个筋条38(图11和图12)或齿39(图13和图14)的凸起18的相同结构类似地可用在下件4上。
手术技术的说明
外科医生先从前侧通过一个经腹膜的或腹膜后的通道把椎间盘往前送。外科医生去除椎间盘(椎间盘切除术),例如它要求植入物的宽度。借助于一个合适的器械,外科医生可以分开椎间腔,以便用此后插入的椎间植入物产生一个缓解作用(Entlastung)(减压术)。借助于一个植入物样品(Probeimplant),手术医生能够确定植入物的大小。分开器始终保持使用。
外科医生现在可以借助于一个合适的器械和临时的锁定装置把椎间植入物插入到准备好的椎间盘腔。在此期间所有的器械仍在使用,植入物的最终位置必须通过敲击和移动借助于为此预备的器械来获得。为了使植入物获得功能性的活动能力,紧接着必须去除临时的锁定装置和所属的器械。最后外科医生可以拔出分开器并且缝合伤口。
Claims (28)
1.一种椎间植入物(1),尤其是人造的椎间盘,包括一条中心轴线(2)、一个上件(3)、一个下件(4)和一个沿轴向设置在这两个件(3、4)之间的关节(9),其中
A)上件(3)具有一个上对合面(35)、一个前侧的侧面(31)、一个背侧的侧面(32)和两个侧向的侧面(33、34),并且上对合面(35)适用于贴靠在一个位于其上方的椎骨体的底板上;
B)下件(4)具有一个下对合面(45)、一个前侧的侧面(41)、一个背侧的侧面(42)和两个侧向的侧面(43、44),并且下对合面(45)适用于贴靠在一个位于其下方的椎骨体的盖板上;并且
C)关节(9)包括一个凸面的带有一个第一关节面(10)的关节件(12)和一个与其互补的带有一个可滑动地支承在第一关节面(10)上的第二关节面(11)的关节壳(13);
其特征在于:
D)所述两个件(3、4)中的每一个都包括用于安装可临时固定的锁定装置(21)的机构(20),所述锁定装置用于相互相对地固定所述两个件(3、4);并且
E)椎间植入物(1)包括至少一个可导入到机构(20)内和可去除的锁定装置(21)。
2.根据权利要求1所述的椎间植入物(1),其特征在于:锁定装置(21)是单件式的。
3.根据权利要求1或2所述的椎间植入物(1),其特征在于:锁定装置(21)使得所述两个件(3、4)可以自动固定。
4.根据权利要求1至3之一所述的椎间植入物(1),其特征在于:机构(20)在上件(3)上包括至少一个上槽(22)以及在下件(4)上包括至少一个下槽(23),其中各槽(22、23)的纵轴线与相应的件(3、4)的前侧的侧面(31、41)相交。
5.根据权利要求4所述的椎间植入物(1),其特征在于:所述槽(22、23)通到相应的件(3、4)的前侧的侧面(31、41)上。
6.根据权利要求4或5所述的椎间植入物(1),其特征在于:椎间植入物在侧向的侧面(33、34、43、44)之间的中心位置上具有一个包含中心轴线(2)的中心平面(19),并且在中心平面(19)的每一侧上包括一个上槽(22a、22b)和一个下槽(23a、23b)。
