Abstract

背景技术
具有射频(RF)能量的电磁能量经常用于为了很多目的,例如,为了心脏切除、肿瘤切除等的目的,在人体和动物体的生物位点产生损害。RF能量还可以用于加热位点用于疼痛管理的处理。为将RF能量加到身体中的所需位点,将电极用作导体,其中电极尖端形成电路的第一端子并且在患者身体下的底板形成电路的地电极,使得当电极尖端和该位点接触时,形成闭合电路。这个布置的问题在于患者身体的阻抗很高,造成RF能量通过患者身体分散而不是集中在该位点处。
一般,在一位点使用单一的有源电极系统产生损害。将RF能量向着导管的端部加到小的电极尖端,导管通过患者的身体和地连接。
发明内容
根据本发明的第一方面,提供了一种用于加热在患者身体中的生物位点的系统,该系统包括:
变压器,其具有初级绕组和次级绕组,该次级绕组具有提供地基准的至少一个分接头和至少两个射频(RF)能量的源;
至少一个有源电极,其和每个源连接以将能量从它相关联的源加到该位点,由任意一个源的至少一个电极施加的能量和由任意其它源的至少一个电极施加的能量异相;以及
中性电极,其连接至分接头的地基准,并且地基准和中性电极被绑定在变压器的初级侧上的地基准。
该系统可以包括能量发生器,其用于产生RF能量,变压器的初级绕组和能量发生器的输出端连接。或者,可以使用阻抗匹配网络来方便现有设备的使用。
可以将基准电极或中性电极连接到至少一个分接头。
变压器在初级绕组和次级绕组之间可以具有1∶1的比率。优选的,分接头是中心接头以提供两个子绕组,其用作能量源,并且由源提供的能量关于彼此180°异相,但是幅度相同,使得加到位点的总能量等于由单一电极系统施加的能量。
至少一个有源电极可以连接每个子绕组的自由端,其和子绕组连接分接头的一端相对。

作为补充或者替代,次级绕组可以具有在地基准分接头和每个子绕组的自由端之间的至少一个中间分接头,以提供用作能量源的多于两个子绕组。至少一个有源电极可以连接每个中间分接头,选择中间分接头的位置以在位点的相邻电极之间保持足够的电势差,用于产生较长的损害。
设计系统特别用于加热位点到足以使得在心脏产生损害以处理心房纤维性颤动的程度,该系统还可以用于处理其它形式的心率不齐,例如,心室心动过速。因此,可以透壁地布置电极,也就是,通过心脏的心室壁,以在相关位点产生透壁损害从而处理心室心动过速。该系统还能够用于处理疼痛管理,其中加热该位点到消除由疼痛引入的不适但是不足以引起损害产生的温度。
为便于将电极安装在相关位点,具体的说,在处理心室心动过速时,至少一个有源电极可以是电极组件,其包括通常布置的电极对,该组件的电极关于彼此可替换地布置。至少一个电极可以具有可以被拧进位点的螺旋尖端。
作为这个布置的发展,组件的两个电极都成为螺旋尖端以拧进位点。螺旋尖端的电极可以具有不同螺距,使得在使用中电极延伸进位点的深度彼此不同。

具体实施方式
首先参考附图的图1,示出了根据本发明实施例的、用于加热在患者身体中的生物位点的系统,并且其通常由附图标记10指示。系统10包括用于产生电磁能量,更为具体的说,射频(RF)能量的发生器12。
变压器14和RF发生器12的输出端连接。至少两个有源电极16连接变压器14的输出端,如将在下面详细描述的。“有源”意味着除非在上下文中清楚地另外指示,该电极用于将能量加到该位点。
系统10使用患者的身体作为阻抗18,并且通过使用基准电极或中性电极20形成闭合电路。基准电极20和RF发生器12的地22绑定。
变压器14是中心分接的变压器,变压器14的次级绕组具有中心分接头24以形成两个分开的子绕组。基准电极20和中心分接头24连接。有源电极16之一连接变压器14的每个子绕组的相对的、或自由端,其由次级绕组的中心分接形成。变压器14可选地具有在中心分接头和每个子绕组的每个自由端之间形成的中间分接头(没有示出)。在这个情况中,至少一个有源电极可以连接相邻的中间分接头。选择中间分接头的位置以保持在该位点处相邻电极之间的足够的电势差以产生较长的损害。
变压器14在它的初级绕组和次级绕组之间使用1∶1的比率。考虑到如果次级绕组的绕组的匝数相对于初级绕组的增加,可以采用不同的比率,这时在每个次级绕组两端的电压增加,同时电流相应减小。
另外,选择用于变压器14的材料能够承受切除治疗中包含的能量级别和频率。变压器14和使用的材料最优地保证传输最大能量到有源电极16。
因此,用于变压器14的合适的材料包括用于变压器14的芯的镍-锌或锰-锌铁氧体,具体的说F8、F12、F14铁氧体。这些材料能够工作在所需频率并且具有所需的高的初始渗透性和高的饱和通量。应该认可选择芯的尺寸,绕组的匝数和用于绕组的直径使得变压器14具有低的插入损耗以保证传输足够能量。
变压器14的初级绕组匹配发生器的输出阻抗。用在试验中的发生器12具有在大约30和300欧姆之间的输出阻抗。可能需要一系列电阻和/或平行电容来影响阻抗匹配。
设计系统10特别用于在患者身体的位点产生损害以处理比如心房纤维性颤动、心室心动过速、肿瘤切除、疼痛管理等多种疾病。通常,用于处理这些疾病的系统使用单一电极和在患者身体之下的底板来形成返回连接。这造成大部分能量通过患者身体分散而不是用于在患者身体中的位点的切除目的。
通过在本发明的系统10中提供两个有源电极16,在有源电极16之间创建在26示意性示出的中间电极阻抗,使得相比通过患者身体,在电极16之间传送更大的能量。
这在附图的图2中示出,其中波形28是现有技术的系统的单一电极的电压波形。电压波形30和32是本发明的系统10的每个有源电极16的180°异相波形,并且波形34是波形30和32的幅度的绝对值的和。因此,应该注意在电极16之间的电压,如波形34表示的,和单一电极28的电压相同,但是该能量集中在有源电极16之间而不是在电极和在患者身体上的任意中性电极之间。
应该注意,总的来说,系统10的能量不高于现有技术的系统的能量,因为由变压器14加到每个有源电极16的能量是加到现有技术的系统的单一电极的能量的一半。
申请人相信,通过将能量集中在本发明的系统10的有源电极16之间,相比使用相同RF能量由单一电极产生的损害,可以在两个电极16之间形成更大和更深的损害。其原因在于中间电极阻抗26比患者身体的阻抗低很多,使得能量在电极16之间传送而不是通过患者的身体分散。
下面的表1示出了通过实验执行的多种测试。

再一次,比较项目24和28,关于透壁损害,使用本系统10的两个异相的电极16(测试24)相比使用两个同相电极(如测试28所示)产生显著更深的损害。
为创建透壁损害,具体的说为处理心房纤维性颤动,以胸腔镜的方式放置一个电极通过胸部,并且经导管将第二电极插入心脏内侧以实现通过心脏壁的损害。
预期用于本系统10的另一方法是使用元件30,其包括两个同轴布置的电极用于处理心室心动过速。在如附图的图3所示的实施例中,元件30包括内部电极32,其是经导管34插入的可收回电极。利用经导管34的内腔插入的螺钉驱动器探针(没有示出),内部电极32被拧进组织36中待处理的位点的位置,以将螺旋尖端电极32相对于外部第二电极38延伸到心脏壁的组织36。第二电极38可以是固定的或可收回的,放置其和心脏的心内膜接触以实现产生透壁损害40。
通过绝缘螺钉的一部分,可以最优化螺钉电极32的暴露金属的实际长度。例如,螺钉尖端可以是20mm长,但是仅最末端的5mm是暴露的金属。应该认可电极32拧进心脏壁的组织36的实际深度根据所需的处理是可变的。
应该认可两个电极32,38彼此绝缘。
这个布置的变型是使用彼此绝缘的两个螺钉尖端的电极。这个元件的实施例如附图的图4所示。通过参考附图的图3,除非特别说明,相似的附图标记表示相似的部分。
在这个实施例中,电极32和38都是螺钉尖端或螺旋尖端的。内部电极32的螺钉尖端具有比外部电极38更大的螺距。因此,当电极32、38延伸出导管34时,将电极32拧进心脏壁的组织36比电极38更大的深度。精确的螺距使得外部电极和心内膜接触。再一次,可以根据引起心率不齐的传导纤维的深度最优化螺钉深度。
申请人考虑到使用同心电极,具体的说,其可以用于处理心室心动过速,而且也可以用在其它应用中。
在本发明的另一实施例中,系统10的变压器14在它的次级绕组具有多个分接头。在附图的图5中示意性示出了本发明的这个实施例。通过参考附图的图1,除非特别说明,相似的附图标记表示相似的部分。
因此,变压器14具有绕在芯14.3周围的初级绕组14.1和次级绕组14.2。芯14.3由上面提到的材料之一制成。次级绕组14.3具有中心分接头24和连接在每个端分接头44和中心分接头24之间的中间分接头42。

系统10的最优化包括分接头24在变压器14的次级绕组上的定位以及电极16的形状和尺寸。为了减少在位点的炭化,可以以组布置,例如,成对布置连接至次级绕组的子绕组的电极。通过以组放置电极,每个电极可以将较低的能量加到该位点,由此减少炭化的可能。另外,使用多个电极能够用于将RF能量通过至少两个电极同时传输的疼痛管理。两个电极的定位比单一电极对于地电极的定位危险更少。例如,在将能量加到患者的脊柱的疼痛管理中,在脊柱的每一侧放置电极而不是直接放置一个在脊柱中危险更少。应该认可,对于疼痛管理,不发生切除并且因此电极工作在不足以造成组织切除的功率电平。
另外,对了切除肿瘤,通过将电极16放置在肿瘤的相对侧,相比使用单一电极能够切除更大质量的肿瘤。
因此,对于本发明提供系统10和方法是有益的,因为在有源电极16之间传递的能量产生更深的损害并且促进产生更精确的损害。另外,使用有源电极对减少了在处理多种疾病中产生损害时包含的危险。
使用至少两个有源电极在创建线性损害中具有显著的优点,比如用在“类迷宫(Maze-like)”过程以及产生对于处理心室心动过速有益的透壁损害。
系统10的另一主要优点是使用中心分接的变压器提供异相能量源。中心分接的变压器可观的减少了系统10的复杂性,并且避免了复杂和昂贵的控制电路的需要。变压器14以简单但是可靠的方式提供能量源。使用变压器14还避免了复杂的组织过程的需要。在效果上,变压器14仅需要连接到发生器12,可以容易的使用定位的电极16和系统10。使用系统10不需要复杂的校准或练习过程。

Claims (11)
1.一种用于加热在患者身体中的生物位点的系统,该系统包括:
变压器,其具有初级绕组和次级绕组,该次级绕组具有至少两个射频(RF)能量的源和用于提供地基准的至少一个分接头;
至少一个有源电极,其和每个源连接以将能量从它相关联的源加到该位点,由任意一个源的至少一个电极施加的能量与由任一其它源的至少一个电极施加的能量异相;以及
中性电极,其连接至所述分接头的地基准,并且所述地基准和所述中性电极被绑定在所述变压器的初级侧上的地基准。
